Abstract:BACKGROUND Dentistry is a small component of the undergraduate medical curriculum, however many patients with oral disease seek advice from their doctor. Problems include dental pain, abscesses, avulsed teeth and fractured jaws. Some patients associate the concept of dental treatment with negative emotions (ie. fear), and doctors are generally seen as caring professionals who might provide reassurance and guidance. OBJECTIVE Solving dental problems often requires operative treatment which doctors cannot provide. Options may seem limited to antibiotics and analgesics, however neither treatment may be effective, and antibiotic treatment may be inappropriate. This article aims to provide practical advice to doctors regarding patients with dental problems. DISCUSSION Common dental problems such as acute pain, bleeding tooth sockets, and trauma are rarely life threatening but are time consuming for the medical practitioner and require immediate solutions. Suggestions for diagnosis and early management are offered.

Abstract:Family physicians commonly encounter patients with dental infections, such as dental caries and periodontal disease. Dental caries is caused by bacteria that destroy the enamel and dentin; it can be detected by an oral examination that shows stained pits or fissures on the tooth surface. Use of fluoride is the most effective prevention measure for dental caries. Untreated caries may progress to pulpitis and, eventually, to necrosis of the pulp. In irreversible pulpitis, the tooth dies and the patient may have a localized abscess that can spread to surrounding tissue. Periodontal infections are caused by bacteria in the subgingival dental plaque. In gingivitis, the inflamed gums bleed easily with brushing or flossing; the condition can be controlled with good oral hygiene. Periodontitis is characterized by a loss of supportive bone structure caused by chronic gingivitis; it is also associated with some systemic diseases. Localized periodontitis is treated with mechanical debridement and good oral hygiene, whereas generalized periodontitis requires adjunct antibiotic therapy. Pericoronitis results when food particles become trapped under the gum of an impacted tooth. This condition can be controlled by removal of food debris and good oral hygiene. For patients in whom dental infections are disseminated and have invaded the deeper oral spaces, antibiotic treatment should be initiated at the time of referral.
Simple and Flexible Privacy-Preserving Revocation Checking

Abstract:Digital certificates signed by trusted certification authorities (CAs) are used for multiple purposes, most commonly for secure binding of public keys to names and other attributes of their owners. Although a certificate usually includes an expiration time, it is not uncommon that a certificate needs to be revoked prematurely. For this reason, whenever a client (user or program) needs to assert the validity of another party’s certificate, it performs revocation checking. There are many revocation techniques varying in both the operational model and underlying data structures. One common feature is that a client typically contacts an on-line third party (trusted, untrusted or semi-trusted), identifies the certificate of interest and obtains some form of a proof of either revocation or validity (non-revocation) for the certificate in question. While useful, revocation checking can leak potentially sensitive information. In particular, third parties of dubious trustworthiness discover two things: (1) the identity of the party posing the query, as well as (2) the target of the query. The former can be easily remedied with techniques such as onion routing or anonymous web browsing. Whereas, hiding the target of the query is not as obvious. Arguably, a more important loss of privacy results from the third party’s ability to tie the source of the revocation check with the query’s target. (Since, most likely, the two are about to communicate.) This paper is concerned with the problem of privacy in revocation checking and its contribution is two-fold: it identifies and explores the loss of privacy inherent in current revocation checking, and, it constructs a simple, efficient and flexible privacy-preserving component for one well-known revocation method.

Abstract:Dental insurance and access to care need substantial improvement in the United States, given their current inadequacies that contribute to suboptimal oral and physical health outcomes and escalate national health care expenditures. The disparities in access to dental care exacerbate oral health disparities across socioeconomic lines. With their broad reach and comprehensive approach to health care, nurse practitioners are uniquely positioned to play a pivotal role in enhancing oral health outcomes. They can effectively integrate preventive measures and early intervention strategies for conditions such as odontogenic infections, thereby reducing emergency department visits and improving overall patient health. This clinical feature critically examines evidence-based practices for the presentation, diagnosis, and management of odontogenic infections within primary care settings. Disseminating this knowledge aims to empower nurse practitioners to deliver timely and effective care, ultimately fostering more significant equity and accessibility in dental health services nationwide.

Epidemiology and outcomes of traumatic dental injuries: a review of the literature

Abstract:Dental trauma is a significant public health problem because of its frequency, impact on economic productivity and quality of life. It is not a disease and no individual is ever at zero risk of sustaining these potentially life-changing injuries. The aim of this article was to review the literature on the prevalence, incidence, aetiology, prognosis and outcomes of dental trauma. The importance of standardized reporting, oral health policy, adjunctive research methods, prevention and education will also be discussed. A search for relevant articles appearing in databases such as Medline, Cochrane and SSCI formed the basis of this review. Epidemiological studies indicate the annual incidence of dental trauma globally is at about 4.5%. Approximately one-third of children and toddlers (primary teeth) and one-fifth of adolescents and adults (permanent teeth) sustained a traumatic dental injury. The majority involved the maxillary central incisors, mainly from falls in toddlers at home and contact sport in adolescents. Despite these trends, there is considerable variation between studies within and across jurisdictions. There is a need to standardize research with a consistent approach to reporting, classification and methodology. This will improve research and form a greater basis for predicting prognosis. This research basis will assist in consent and clinical management.

Abstract:A 6-year-old female presented with right hemichorea, initially thought to be post-streptococcal, which subsequently progressed to a right dystonic hemiplegia. At 7 1/2 years she developed right focal and secondary generalized tonic-clonic seizures. These became intractable. A brain biopsy was consistent with Rasmussen's encephalitis (RE). At 9 1/2 years the child underwent a left hemispherectomy making a good recovery with resolution of seizures. At 12 years she presented with recurrent episodes of abdominal pain, fever, and malar rash with serological evidence of systemic lupus erythematosus (SLE). A possible link between RE and SLE is discussed.

Abstract:The Action in Diabetes and vascular disease: preterAx and diamicoN-MR Controlled Evaluation (ADVANCE) trial was designed to assess the effects of blood pressure lowering, irrespective of initial blood pressure levels or the use of other blood pressure lowering drugs, on vascular disease in patient with type 2 diabetes [1]. In this randomized trial, 11,140 patients with type 2 diabetes, older than 55 years with other cardiovascular risk factors were randomized to a fixed dose of the ACE inhibitor (perindopril) with a thiazide-like diuretic (indapamide) and compared to placebo. All other concurrent medications except for the randomized drugs classes were allowed to continue. The primary endpoint of this trial was the composite of major macro and microvascular events. After a mean follow-up of 4.3 years, patients on the fixed dose combination of perindopril and indapamide had a mean reduction in blood pressure of 5.6/2.2 mmHg compared to placebo. The relative risk of major micro and macro vascular event was reduced by an additional 9% in this group. The separate reductions in microvascular and macrovascular events were not significant. The relative risk of death from any cardiovascular cause was reduced by 18% and all cause mortality by 14%. Thus, for every 79 patients treated one death could be avoided. Based on these data the authors conclude that a fixed dose combination of perindopril and indapamide should be recommended for all patients with type 2 diabetes, irrespective of their existing blood pressure. The question is did ADVANCE advance our knowledge? The cardiovascular benefits of ACE inhibitors in patients with type 2 diabetes are well established by multiple clinical trials. The HOPE trial showed that ramipril reduced rates of death, cardiovascular events in patients with type 2 diabetes and supported the use of ramipril in every patient with diabetes regardless of blood pressure status [2]. Additionally, the PROGRESS trial that studied the same agents as in ADVANCE, demonstrated that when a diuretic was added to an ACE inhibitor stroke incidence was reduced, but this too was related to a lower blood pressure [3]. So, should all patients with type 2 diabetes take a fixed dose combination of perindopril+indapamide or should we simply aim at lowering blood pressure to recommended goals using the best tolerated most cost effective agents? Other large clinical trials have shown that tight control of blood pressure in patients with diabetes resulted in better outcomes. Cardiovascular events were reduced by 51 % in the diabetic subgroup of the Hypertension Optimal Treatment (HOT) trial [4], a trial that randomized a dihydropyridine calcium antagonist compared to conventional treatment. In HOT the group with the lower events had a 4 mmHg lower diastolic blood pressure. In the United Kingdom Prospective Diabetes Study (UKPDS) patients who had tight control of blood pressure i.e. 10/5 mmHg lower than control, had a 24% risk reduction for any diabetes related endpoint. In UKPDS, however, no differences were observed between ACE inhibitor and β-blocker randomized patients [5]. The conclusion of these studies is that blood pressure lowering regardless of agent was similarly effective in reducing incidence of diabetic complications. To further bolster this argument in the ADVANCE trial, because of the study design, 55% of patients in placebo group were receiving perindopril at the end of the follow up however, none were allowed to receive a thazide diuretic. Thus, one could argue that like PROGRESS, where the addition of the diuretic resulted in further blood pressure lowering and a lower stroke risk, this is what we see in ADVANCE. One may come to the conclusion that the beneficial effects of the perindopril+ indapamide combination are not exclusive to this combination. Moreover, it is clear that a diuretic is needed to further lower blood pressure when an ACE inhibitor is used in such patients. Effects of cytokine application on glucocorticoid secretion in an animal model for systemic scleroderma.

Abstract:We previously reported on an altered immune-endocrine feedback loop via the hypothalamo-pituitary-adrenal (HPA) axis in Obese strain (OS) chickens afflicted with spontaneous autoimmune thyroiditis. These animals are deficient in plasma corticosterone increase after antigenic challenge or application of cytokine-containing conditioned medium of mitogen-stimulated spleen cells (CM). To investigate whether the impaired ability to respond to cytokines with glucocorticoid-increasing factor (GIF) activity, e.g. interleukin 1 (IL 1), is restricted to OS chickens as a model for an organ-specific autoimmune disease, we extended our experiments to another autoimmune-prone animal strain, the chickens of the University of California at Davis line 200 (UCD-200). These animals develop an inherited inflammatory fibrotic disease that closely resembles human progressive systemic sclerosis (scleroderma). Application of GIF-containing CM to UCD-200 chickens leads to a transient increase in glucocorticoid serum levels within 1-2 hours comparable to that of controls. But, while corticosterone levels in the latter returned to normal baseline levels after 4 hours, they were still elevated in autoimmune chickens. Although the peak of the glucocorticoid hormone serum concentrations was equal to that of controls, UCD-200 had to secrete twice as much adrenocorticotropic hormone to achieve this corticosterone serum level due to an apparent hyporesponsiveness of the adrenal gland to this secretagogue. The altered cytokine-induced glucocorticoid secretion is found in early as well as in chronic, sclerotic stages of the disease. Cellular alterations in the peripheral blood of UCD-200 chickens during the prolonged elevated corticosterone section, i.e. between 2-4 hours after CM application, are characterized by a significant decrease in the percentage of CD4+ and CD8+ cells. Furthermore, a significant increase in B cells up to 24 hours with a maximum after 1 hour was found. The proliferative response to the mitogen concanavalin A of peripheral mononuclear cells was inversely correlated to the serum corticosterone level, showing a permanent decrease of 80-90% after 1-4 hours in autoimmune animals. This functional alteration in UCD-200 was accompanied by an 80% decrease in serum interleukin 2 (sIL 2) activity 4 hours after CM application. Twenty-four hours later an eight-fold increase in sIL 2 rebound activity was found, indicating that the inhibitory effect of corticosterone in UCD-200 chickens is not long-lasting.
Paramedic Exposure and Response to Non-Traumatic Dental Conditions

Abstract:Introduction As the global demand for emergency health services continues to expand, the role of paramedics persists and grows alongside the expansion of ambulance services. The need to explore alternative pathways for enhancing service efficiency and alleviating hospital demands is increasingly evident, particularly for low-acuity cases. Non-traumatic dental conditions (NTDC) can be ongoing and have far-reaching health implications if not treated correctly. This scoping review aims to explore what is known about the incidence of NTDC cases attended by paramedics internationally. Methods Three electronic databases (Medline, Scopus, CINAHL) were searched between 1 Jan 2000 and 30 April 2023, using the primary search terms “paramedics,” “prehospital,” and “dental emergency.” In addition, grey literature was examined using Google Scholar. Studies were included if they investigated NTDC as the primary treatment condition in the prehospital setting (excluding clinical settings) and were written in English. Results Of 1755 articles identified only four studies met the inclusion criteria. Three were quantitative descriptive surveys, and one study included interview data. Studies originated in Poland, the United States, Turkey, and India. Paramedics who reported attending an NTDC case varied between 28% and 72%. Two papers reported on the demographics of patients most affected by NTDC, including young children aged 7-15 and older individuals between the ages of 45-64. Despite limitations and bias across all studies, results indicated insufficient knowledge and education of paramedics regarding dental anatomy or proper management of NTDC. Conclusion This scoping review identified a paucity of robust research and publications investigating the paramedic role in initial treatment and referral pathways of dental injuries. There was limited information on the paramedic’s knowledge, attitude, and training in dental injuries. Further research is required to determine paramedics’ preparedness to manage and improve patient outcomes to reduce the growing burden on emergency departments.

Abstract:Approximately 50% of children under the age of 15 are victims of various kinds of injuries in the orofacial region. Post-traumatic complications may occur, including crown discolouration, cervical root fracture, ankylosis, root resorption and tooth loss. The most severe complication after dental injury in primary dentition can affect the developing permanent tooth germ, and various consequences may be seen several years later when the permanent tooth erupts. In the permanent dentition, the most severe dental injury affects the surrounding alveolar bone structure and will lead to loss of the tooth. Current literature emphasises that awareness of appropriate triage procedures following dental trauma is unsatisfactory and that delay in treatment is the single most influential factor affecting prognosis. What should a paediatrician know, and more importantly, how should he/she advise parents and caretakers? In an emergency situation such as tooth avulsion, reimplantation within 30 min is the best treatment option at the site of the accident. If reimplantation of the tooth is impossible, milk, saline or even saliva are the preferred transport media. The prognosis for an avulsed tooth depends upon prompt care, which is a determinant factor for successful treatment of the traumatised tooth. In all other dental trauma cases, it is important to refer the child to a paediatric dentist, to follow up the healing process and reduce late post-traumatic complications. With timely interventions and appropriate treatment, the prognosis for healing following most dental injuries is good. In conclusion, it is important that paediatricians are able to inform parents and caretakers about all possible and long-lasting consequences of different dental injuries.

Abstract:Introduction Oral haemorrhages (OH) represent a relatively rare reason for an emergency consultation, among dental pain, infectious processes and traumas. Various haemorrhagic risk factors are described, particularly those related to a general medical context or local factors. It is common to associate OH with haemorrhagic risk patients. Current studies mainly focus on patients considered at risk, but there is limited data on the characteristics of OH in patients without known haemorrhagic risk. Method We conducted a 2‐year retrospective study in a dental emergency department to identify OH occurring in patients without known haemorrhagic risk and to study their characteristics. Results OH accounted for approximately 2% of the reasons for consultation among all consultations carried out for dental emergencies. Their frequency was similar in the population of patients without hemorrhagic risk and those with a confirmed risk. They mainly occurred in young patients, without medical history. They also led to the diagnosis of an underlying pathology in about 1% of cases. Conclusion This study underscores the importance of accurately documenting patients' medical history during preoperative consultations, as well as identifying associated risk factors. It also highlights that OH can be an early sign of a systemic disorder.

Clinical Analysis of Dental Trauma Patients in the Emergency Room

Abstract:OBJECTIVE:To Analyze the etiological and clinical characteristics of dental trauma in dental emergency.METHODS:A comprehensive review of 975 emergencies of dental trauma seen at the Emergency Room of Peking University School and Hospital of Stomatology during a two-year period was performed. General characteristics, such as age and gender, time of visit, causes of dental trauma, places where the dental trauma happened, diagnoses, and related treatments were analyzed.RESULTS:The two age peaks of dental trauma patients under 18 years were 2 to 4 years and 7 to 12 years. There were more male patients than females in the groups above 7-year-old, but there were no difference in gender distribution in infants and toddlers. Spring had the lowest frequency of dental traumatic emergencies, but winter was the busiest season. Dental emergencies with traumatic causes followed a pattern during the week. Most patient visits presented during weekends. 24.1% of the patients presented to the dental clinic within 2 hours after injury, only 4.7% of the patients visited the clinic after 24 hours. 64.4% of the children younger than 5 years were injured at home, but 82.3% of children aged 5 and older and 93.1% adults were injured in the outdoor environment. The number of injured teeth in left and right side had no significant difference (P>0.05). Maxillary incisors were easily to be affected, especially the maxillary central incisor. Crown fracture was the most common traumatic type. The combination of dental trauma and oral and maxillofacial soft tissue injury was experienced by 692 patients (71.0%).CONCLUSION:Dental trauma was one of the common diseases in oral emergencies. Knowledge of the etiological and clinical characteristics of emergency dental trauma could help us to treat them and make specific preventive measures. Clinical examination should be comprehensive and meticulous to avoid misdiagnosis, for emergency dental trauma often involving multiple teeth, and associated with different degrees of maxillofacial soft tissue injury.

Abstract:Dental problems are the most frequent causes of orofacial pain. Without dental pathology, dentine sensitivity causes pain and in relation to disease, pain can result from dental caries (decay), dental erosion (soft drinks), and cracked or fractured teeth. Consequently, a thorough dental examination is warranted in every patient presenting with orofacial pain. Management primarily consists of surgery by the dentist, while adjunctive pharmacological management of the pain, using combined paracetamol and NSAIDs, is indicated in the acute pre- or postoperative phase.Pain associated with temporomandibular disorders (TMD) has the second highest prevalence in the orofacial region after dental pain and can clinically be expressed as masticatory muscle pain (temporalis, masseter, and pterygoid muscles) and/or TMJ-arthralgia. Joint function and loading of the masticatory system usually aggravates the pain, for example in prolonged mouth opening during dental treatment, or chewing gum excessively. Often, the pain is also expressed as a (tension-type) headache. Patients will often present with limitation of jaw movement, interference during movement, or locking of the TMJ with or without pain. In line with the biopsychosocial model, recent findings confirm a multifactorial etiology grouped around the interaction between psychosocial distress and pain amplification and influenced by genetic regulation and environmental background. In view of the self-limiting and benign character of TMD, management aims at providing optimal circumstances for the body to adapt and heal. Most treatment approaches are reversible and fit into the biopsychosocial approach: correct information, counseling to avoid overload of the masticatory system (tooth clenching), local physical therapy, intraoral appliances, and medication (NSAIDs). A surgical approach is seldom needed and should be restricted to specific cases including repeated persistent open or closed locking, pathology, and trauma (arthrocentesis or joint surgery). In the limited number of patients that develop chronic pain, intensified CBT and antidepressants are indicated.

Abstract:OBJECTIVES: Vulnerable populations in underserved communities are disproportionately at high risk for multiple medical, dental, and behavioral health conditions. This study aims to: a) examine the occurrence of acute dental needs and b) investigate the association of acute dental needs and self-rated general and mental health status among the adult dentate health center population.METHODS: This cross-sectional study analyzed data on adult patients (n = 5,035) from the 2014 Health Center Patient Survey, a nationally representative survey of health center patients. Multivariate logistic regression was used to assess the association of acute dental needs and a) self-rated general health status and b) mental health status.RESULTS: Approximately, two thirds of adult dentate heath center patients reported having an acute dental need. After adjusting for confounding factors, not having or having had health insurance that pays for dental care, general health status of fair or poor, and ever having a mental illness were associated with higher odds of having an acute dental need.CONCLUSIONS: The results highlight the role of health centers in addressing oral health disparities among vulnerable populations and the importance of a team-based multidisciplinary approach to ensuring the integration and coordination of oral health services within a comprehensive primary care delivery system.

Abstract:Dental care is the most common unmet health care need of children. Those at increased risk for problems with oral health and access to care are from poor or minority families, lack health insurance, or have special health care needs. These factors place more than 52 percent of children at risk for untreated oral disease. Measures of access and parental report indicate unmet oral health needs, but do not provide guidance as to the nature of children's oral health needs. Children's oral health needs can be predicted from their developmental changes and position in the life span. their dependency and environmental context, and current demographic changes. Specific gaps in education include training of general dentists to care for infants and young children and those with special health care needs, as well as training of pediatric providers and other professionals caring for children in oral health promotion and disease prevention. Educational focus on the technical aspects of dentistry leaves little time for important interdisciplinary health and/or social issues. It will not be possible to address these training gaps without further integration of dentistry with medicine and other health professions. Children's oral health care is the shared moral responsibility of dental and other professionals working with children, parents, and society. Academic dental centers hold in trust the training of oral health professionals for society and have a special responsibility to train future professionals to meet children's needs. Leadership in this area is urgently needed.

Abstract:This chapter discusses the perioperative anesthetic considerations and management of the most common conditions requiring surgical intervention in the head and neck region. Many children scheduled to undergo head and neck surgery present with nasal discharge that may represent either allergic rhinitis or an upper respiratory tract infection (URI). Decreased functional residual and diffusion capacity and increased closing volumes and airway resistance have been noted during a URI. The chapter also discusses otolaryngological surgery, including ear surgery and nasal and pharyngeal surgery and reviews endoscopy of the larynx, trachea and bronchial tree. Pediatric dental procedures are routinely performed in a dentist's office with local anesthesia, with or without sedation. General anesthesia allows the dentist to complete dental restoration in a timely fashion without administering excessive amounts of local anesthetic or approaching the maximum limits of sedation with which they are comfortable in their dental office.
