Abstract:Outcome prediction is essential for the administration and treatment of critically ill patients. For those patients, clinical measurements are continuously monitored and the time-varying data contains rich information for assessing the patients' status. However, it is unclear how to capture the dynamic information effectively. In this work, multiple feature extraction methods, i.e. statistical feature classification methods and temporal modeling methods, such as recurrent neural network (RNN), were analyzed on a critical illness dataset with 18415 cases. The experimental results show when the dimension increases from 10 to 50, the RNN algorithm is gradually superior to the statistical feature classification methods with simple logic. The RNN model achieves the largest AUC value of 0.8463. Therefore, the temporal modeling methods are promising to capture temporal features which are predictive of the patients' outcome and can be extended in more clinical applications.
Improving the Prognostic Evaluation Precision of Hospital Outcomes for Heart Failure Using Admission Notes and Clinical Tabular Data: Multimodal Deep Learning Model

Abstract:Background: Clinical notes contain contextualized information beyond structured data related to patients' past and current health status. Objective: This study aimed to design a multimodal deep learning approach to improve the evaluation precision of hospital outcomes for heart failure (HF) using admission clinical notes and easily collected tabular data. Methods: Data for the development and validation of the multimodal model were retrospectively derived from 3 open-access US databases, including the Medical Information Mart for Intensive Care III v1.4 (MIMIC-III) and MIMIC-IV v1.0, collected from a teaching hospital from 2001 to 2019, and the eICU Collaborative Research Database v1.2, collected from 208 hospitals from 2014 to 2015. The study cohorts consisted of all patients with critical HF. The clinical notes, including chief complaint, history of present illness, physical examination, medical history, and admission medication, as well as clinical variables recorded in electronic health records, were analyzed. We developed a deep learning mortality prediction model for in-hospital patients, which underwent complete internal, prospective, and external evaluation. The Integrated Gradients and SHapley Additive exPlanations (SHAP) methods were used to analyze the importance of risk factors. Results: The study included 9989 (16.4%) patients in the development set, 2497 (14.1%) patients in the internal validation set, 1896 (18.3%) in the prospective validation set, and 7432 (15%) patients in the external validation set. The area under the receiver operating characteristic curve of the models was 0.838 (95% CI 0.827-0.851), 0.849 (95% CI 0.841-0.856), and 0.767 (95% CI 0.762-0.772), for the internal, prospective, and external validation sets, respectively. The area under the receiver operating characteristic curve of the multimodal model outperformed that of the unimodal models in all test sets, and tabular data contributed to higher discrimination. The medical history and physical examination were more useful than other factors in early assessments. Conclusions: The multimodal deep learning model for combining admission notes and clinical tabular data showed promising efficacy as a potentially novel method in evaluating the risk of mortality in patients with HF, providing more accurate and timely decision support.
FineEHR: Refine Clinical Note Representations to Improve Mortality Prediction

Abstract:Monitoring the health status of patients in the Intensive Care Unit (ICU) is a critical aspect of providing superior care and treatment. The availability of large-scale electronic health records (EHR) provides machine learning models with an abundance of clinical text and vital sign data, enabling them to make highly accurate predictions. Despite the emergence of advanced Natural Language Processing (NLP) algorithms for clinical note analysis, the complex textual structure and noise present in raw clinical data have posed significant challenges. Coarse embedding approaches without domain-specific refinement have limited the accuracy of these algorithms. To address this issue, we propose FINEEHR, a system that utilizes two representation learning techniques, namely metric learning and fine-tuning, to refine clinical note embeddings, while leveraging the intrinsic correlations among different health statuses and note categories. We evaluate the performance of FINEEHR using two metrics, namely Area Under the Curve (AUC) and AUC-PR, on a real-world MIMIC III dataset. Our experimental results demonstrate that both refinement approaches improve prediction accuracy, and their combination yields the best results. Moreover, our proposed method outperforms prior works, with an AUC improvement of over 10%, achieving an average AUC of 96.04% and an average AUC-PR of 96.48% across various classifiers.

Towards unstructured mortality prediction with free-text clinical notes

Abstract:Healthcare data continues to flourish yet a relatively small portion, mostly structured, is being utilized effectively for predicting clinical outcomes. The rich subjective information available in unstructured clinical notes can possibly facilitate higher discrimination but tends to be under-utilized in mortality prediction. This work attempts to assess the gain in performance when multiple notes that have been minimally preprocessed are used as an input for prediction. A hierarchical architecture consisting of both convolutional and recurrent layers is used to concurrently model the different notes compiled in an individual hospital stay. This approach is evaluated on predicting in-hospital mortality on the MIMIC-III dataset. On comparison to approaches utilizing structured data, it achieved higher metrics despite requiring less cleaning and preprocessing. This demonstrates the potential of unstructured data in enhancing mortality prediction and signifies the need to incorporate more raw unstructured data into current clinical prediction methods.

Abstract:Ensuring accurate predictions of inpatient length of stay (LoS) and mortality rates is essential for enhancing hospital service efficiency, particularly in light of the constraints posed by limited healthcare resources. Integrative analysis of heterogeneous clinic record data from different sources can hold great promise for improving the prognosis and diagnosis level of LoS and mortality. Currently, most existing studies solely focus on single data modality or tend to single-task learning, i.e., training LoS and mortality tasks separately. This limits the utilization of available multi-modal data and prevents the sharing of feature representations that could capture correlations between different tasks, ultimately hindering the model's performance. To address the challenge, this study proposes a novel Multi-Modal Multi-Task learning model, termed as M3T-LM, to integrate clinic records to predict inpatients' LoS and mortality simultaneously. The M3T-LM framework incorporates multiple data modalities by constructing sub-models tailored to each modality. Specifically, a novel attention-embedded one-dimensional (1D) convolutional neural network (CNN) is designed to handle numerical data. For clinical notes, they are converted into sequence data, and then two long short-term memory (LSTM) networks are exploited to model on textual sequence data. A two-dimensional (2D) CNN architecture, noted as CRXMDL, is designed to extract high-level features from chest X-ray (CXR) images. Subsequently, multiple sub-models are integrated to formulate the M3T-LM to capture the correlations between patient LoS and modality prediction tasks. The efficiency of the proposed method is validated on the MIMIC-IV dataset. The proposed method attained a test MAE of 5.54 for LoS prediction and a test F1 of 0.876 for mortality prediction. The experimental results demonstrate that our approach outperforms state-of-the-art (SOTA) methods in tackling mixed regression and classification tasks.
Enhancing In-Hospital Mortality Prediction Using Multi-Representational Learning with LLM-Generated Expert Summaries

Abstract:In-hospital mortality (IHM) prediction for ICU patients is critical for timely interventions and efficient resource allocation. While structured physiological data provides quantitative insights, clinical notes offer unstructured, context-rich narratives. This study integrates these modalities with Large Language Model (LLM)-generated expert summaries to improve IHM prediction accuracy. Using the MIMIC-III database, we analyzed time-series physiological data and clinical notes from the first 48 hours of ICU admission. Clinical notes were concatenated chronologically for each patient and transformed into expert summaries using Med42-v2 70B. A multi-representational learning framework was developed to integrate these data sources, leveraging LLMs to enhance textual data while mitigating direct reliance on LLM predictions, which can introduce challenges in uncertainty quantification and interpretability. The proposed model achieved an AUPRC of 0.6156 (+36.41%) and an AUROC of 0.8955 (+7.64%) compared to a time-series-only baseline. Expert summaries outperformed clinical notes or time-series data alone, demonstrating the value of LLM-generated knowledge. Performance gains were consistent across demographic groups, with notable improvements in underrepresented populations, underscoring the framework's equitable application potential. By integrating LLM-generated summaries with structured and unstructured data, the framework captures complementary patient information, significantly improving predictive performance. This approach showcases the potential of LLMs to augment critical care prediction models, emphasizing the need for domain-specific validation and advanced integration strategies for broader clinical adoption.

Monitoring ICU Mortality Risk with A Long Short-Term Memory Recurrent Neural Network

Abstract:In intensive care units (ICU), mortality prediction is a critical factor not only for effective medical intervention but also for allocation of clinical resources. Structured electronic health records (EHR) contain valuable information for assessing mortality risk in ICU patients, but current mortality prediction models usually require laborious human-engineered features. Furthermore, substantial missing data in EHR is a common problem for both the construction and implementation of a prediction model.Inspired by language-related models, we design a new framework for dynamic monitoring of patients' mortality risk. Our framework uses the bag-of-words representation for all relevant medical events based on most recent history as inputs. By design, it is robust to missing data in EHR and can be easily implemented as an instant scoring system to monitor the medical development of all ICU patients. Specifically, our model uses latent semantic analysis (LSA) to encode the patients' states into low-dimensional embeddings, which are further fed to long short-term memory networks for mortality risk prediction. Our results show that the deep learning based framework performs better than the existing severity scoring system, SAPS-II. We observe that bidirectional long short-term memory demonstrates superior performance, probably due to the successful capture of both forward and backward temporal dependencies.
Integrating Physiological Time Series and Clinical Notes with Deep Learning for Improved ICU Mortality Prediction

Abstract:Intensive Care Unit Electronic Health Records (ICU EHRs) store multimodal data about patients including clinical notes, sparse and irregularly sampled physiological time series, lab results, and more. To date, most methods designed to learn predictive models from ICU EHR data have focused on a single modality. In this paper, we leverage the recently proposed interpolation-prediction deep learning architecture(Shukla and Marlin 2019) as a basis for exploring how physiological time series data and clinical notes can be integrated into a unified mortality prediction model. We study both early and late fusion approaches and demonstrate how the relative predictive value of clinical text and physiological data change over time. Our results show that a late fusion approach can provide a statistically significant improvement in mortality prediction performance over using individual modalities in isolation.

Early Prediction of Mortality in Critical Care Setting in Sepsis Patients Using Structured Features and Unstructured Clinical Notes

Abstract:Sepsis is an important cause of mortality, especially in intensive care unit(ICU) patients. Developing novel methods to identify early mortality is critical for improving survival outcomes in sepsis patients. Using the MIMIC-III database, we integrated demographic data, physiological measurements and clinical notes. We built and applied several machine learning models to predict the risk of hospital mortality and 30-day mortality in sepsis patients. From the clinical notes, we generated clinically meaningful word representations and embeddings. Supervised learning classifiers and a deep learning architecture were used to construct prediction models. The configurations that utilized both structured and unstructured clinical features yielded competitive F-measure of 0.512. Our results showed that the approaches integrating both structured and unstructured clinical features can be effectively applied to assist clinicians in identifying the risk of mortality in sepsis patients upon admission to the ICU.

Abstract:We present an interpretable neural network for predicting an important clinical outcome (1-year mortality) from multi-modal Electronic Health Record (EHR) data. Our approach builds on prior multi-modal machine learning models by now enabling visualization of how individual factors contribute to the overall outcome risk, assuming other factors remain constant, which was previously impossible. We demonstrate the value of this approach using a large multi-modal clinical dataset including both EHR data and 31,278 echocardiographic videos of the heart from 26,793 patients. We generated separate models for (i) clinical data only (CD) (e.g. age, sex, diagnoses and laboratory values), (ii) numeric variables derived from the videos, which we call echocardiography-derived measures (EDM), and (iii) CD+EDM+raw videos (pixel data). The interpretable multi-modal model maintained performance compared to non-interpretable models (Random Forest, XGBoost), and also performed significantly better than a model using a single modality (average AUC=0.82). Clinically relevant insights and multi-modal variable importance rankings were also facilitated by the new model, which have previously been impossible.

How to Leverage Multimodal EHR Data for Better Medical Predictions?

Abstract:Healthcare is becoming a more and more important research topic recently. With the growing data in the healthcare domain, it offers a great opportunity for deep learning to improve the quality of medical service. However, the complexity of electronic health records (EHR) data is a challenge for the application of deep learning. Specifically, the data produced in the hospital admissions are monitored by the EHR system, which includes structured data like daily body temperature, and unstructured data like free text and laboratory measurements. Although there are some preprocessing frameworks proposed for specific EHR data, the clinical notes that contain significant clinical value are beyond the realm of their consideration. Besides, whether these different data from various views are all beneficial to the medical tasks and how to best utilize these data remain unclear. Therefore, in this paper, we first extract the accompanying clinical notes from EHR and propose a method to integrate these data, we also comprehensively study the different models and the data leverage methods for better medical task prediction. The results on two medical prediction tasks show that our fused model with different data outperforms the state-of-the-art method that without clinical notes, which illustrates the importance of our fusion method and the value of clinical note features. Abstract:Accurate mortality prediction allows Intensive Care Units (ICUs) to adequately benchmark clinical practice and identify patients with unexpected outcomes. Traditionally, simple statistical models have been used to assess patient death risk, many times with sub-optimal performance. On the other hand deep learning holds promise to positively impact clinical practice by leveraging medical data to assist diagnosis and prediction, including mortality prediction. However, as the question of whether powerful Deep Learning models attend correlations backed by sound medical knowledge when generating predictions remains open, additional interpretability tools are needed to foster trust and encourage the use of AI by clinicians. In this work we show a Deep Learning model trained on MIMIC-III to predict mortality using raw nursing notes, together with visual explanations for word importance. Our model reaches a ROC of 0.8629 (+/-0.0058), outperforming the traditional SAPS-II score and providing enhanced interpretability when compared with similar Deep Learning approaches.

Abstract:Perpetually improving mortality prediction in intensive care units (ICUs) via the implementation of eHealth evaluation approaches has become a major research hotspot in the field of medical data mining for the purpose of saving lives. Recently, researchers have attempted to achieve improved prediction accuracy by using only deep learning-based techniques. However, some problems remain. (1) Most of the existing methods utilize independent clinical features to predict mortality by eliminating the correlations between the latent features, but this technique may fail to comprehensively capture and evaluate the statuses of patients. (2) Several clinical features are needed to ensure strong prediction accuracy, but most methods only use static features that are not extendable. (3) An effective practical framework that unifies traditional ICU scoring systems and state-of-the-art deep learning methods to predict mortality is also lacking. (4) Moreover, the interpretability of existing deep learning-based methods needs to be further improved. Therefore, we propose a novel d ual-core m utual l earning f ramework (DMLF) between ICU scoring systems and clinical features for mortality prediction. In particular, we mutually utilize s equential o rgan f ailure a ssessment (SOFA) scores and clinical measurement features to learn a unified model for enhancing the accuracy and interpretability of our DMLF. Experiments conducted on five real-world disease datasets show that the DMLF achieves significantly better prediction accuracy and area under the receiver operating characteristic curve (AUROC) values than six baselines and four state-of-the-art methods. Moreover, clinicians utilize a familiarized SOFA system to conduct mortality prediction and achieve increased interpretability, which benefits the adoption of the proposed framework in real clinical scenarios.

Knowledge-Aware Deep Dual Networks for Text-Based Mortality Prediction

Abstract:Mortality prediction is one of the essential tasks in medical data mining and is significant for inferring clinical outcomes. With a large number of medical notes collected from hospitals, there is an urgent need for developing effective models for predicting mortality based on them. In contrast to structured electronic health records, medical notes are unstructured texts written by experienced caregivers and contain more complicated information about patients, posing more challenges for modeling. Most previous studies rely on tedious hand-crafted features or generating indirect features based on some statistical models such as topic modeling, which might incur information loss for later model training. Recently, some deep models have been proposed to unify the stages of feature construction and model training. However, domain concept knowledge has been neglected, which is important to gain a better understanding of medical notes. To address the above issues, we propose novel Knowledge aware Deep Dual Networks (K-DDN) for the text-based mortality prediction task. Specifically, a simple deep dual network is first proposed to fuse the representations of medical knowledge and raw text for prediction. Afterward, we incorporate a co-attention mechanism into the basic model, guiding the knowledge and text representation learning with the help of each other. Experimental results on two publicly real-world datasets show the proposed deep dual networks outperform state-of-the-art methods and the co-attention mechanism can further improve the performance.

Abstract:Natural Language Processing (NLP) is a study of automated processing of text data. Application of NLP in the clinical domain is important due to the rich unstructured information implanted in clinical documents, which often remains inaccessible in structured data. Empowered by the recent advance of language models (LMs), there is a growing interest in their application within the clinical domain. When applying NLP methods to a certain domain, the role of benchmark datasets are crucial as benchmark datasets not only guide the selection of best-performing models but also enable assessing of the reliability of the generated outputs. Despite the recent availability of LMs capable of longer context, benchmark datasets targeting long clinical document classification tasks are absent. To address this issue, we propose LCD benchmark, a benchmark for the task of predicting 30-day out-of-hospital mortality using discharge notes of MIMIC-IV and statewide death data. Our notes have a median word count of 1687 and an interquartile range of 1308 to 2169. We evaluated this benchmark dataset using baseline models, from bag-of-words and CNN to Hierarchical Transformer and an open-source instruction-tuned large language model. Additionally, we provide a comprehensive analysis of the model outputs, including manual review and visualization of model weights, to offer insights into their predictive capabilities and limitations. We expect LCD benchmarks to become a resource for the development of advanced supervised models, prompting methods, or the foundation models themselves, tailored for clinical text. XAI for In-hospital Mortality Prediction via Multimodal ICU Data

Abstract:Predicting in-hospital mortality for intensive care unit (ICU) patients is key to final clinical outcomes. AI has shown advantaged accuracy but suffers from the lack of explainability. To address this issue, this paper proposes an eXplainable Multimodal Mortality Predictor (X-MMP) approaching an efficient, explainable AI solution for predicting in-hospital mortality via multimodal ICU data. We employ multimodal learning in our framework, which can receive heterogeneous inputs from clinical data and make decisions. Furthermore, we introduce an explainable method, namely Layer-Wise Propagation to Transformer, as a proper extension of the LRP method to Transformers, producing explanations over multimodal inputs and revealing the salient features attributed to prediction. Moreover, the contribution of each modality to clinical outcomes can be visualized, assisting clinicians in understanding the reasoning behind decision-making. We construct a multimodal dataset based on MIMIC-III and MIMIC-III Waveform Database Matched Subset. Comprehensive experiments on benchmark datasets demonstrate that our proposed framework can achieve reasonable interpretation with competitive prediction accuracy. In particular, our framework can be easily transferred to other clinical tasks, which facilitates the discovery of crucial factors in healthcare research.

Abstract:In recent years, data-driven remote medical management has received much attention, especially in application of survival time forecasting. By monitoring the physical characteristics indexes of patients, intelligent algorithms can be deployed to implement efficient healthcare management. However, such pure medical data-driven scenes generally lack multimedia information, which brings challenge to analysis tasks. To deal with this issue, this paper introduces the idea of ensemble deep learning to enhance feature representation ability, thus enhancing knowledge discovery in remote healthcare management. Therefore, a multiview deep learning-based efficient medical data management framework for survival time forecasting is proposed in this paper, which is named as "MDL-MDM" for short. Firstly, basic monitoring data for body indexes of patients is encoded, which serves as the data foundation for forecasting tasks. Then, three different neural network models, convolution neural network, graph attention network, and graph convolution network, are selected to build a hybrid computing framework. Their combination can bring a multiview feature learning framework to realize an efficient medical data management framework. In addition, experiments are conducted on a realistic medical dataset about cancer patients in the US. Results show that the proposal can predict survival time with 1% to 2% reduction in prediction error.
MedCT-BERT: Multimodal Mortality Prediction using Medical ConvTransformer-BERT Model

Abstract:In the Intensive Care Unit (ICU), mortality prediction tasks primarily rely on clinical records consisting of patients’ clinical time series data and physicians’ diagnostic opinions. However, due to the irregularity of time series data and clinical records, existing medical multimodal models focus on generating complete and regular data to address this issue, neglecting the impact of generated modality data on multimodal feature fusion. Inaccurate generation of modality data may lead to noise. To overcome this problem, we propose a novel medical multimodal model named MedCT-BERT in this paper. Specifically, the existing models can address the irregularities in time series data but are limited to handling fixed time series data. We optimize their hyperparameters for better performance. By doing so, our model can process and generate complete regular imputed values in parallel for time series data with varying missing values and sequence lengths. To iteratively refine the generated imputed values, we introduce a time series feature correlation information to reduce noise in multimodal data fusion.
