Protective Factors for Early Childhood Caries in 3-Year-Old Children in Poland

Abstract:Background: Early childhood caries (ECC) remains highly prevalent in preschool children worldwide. Dental caries affects every second 3-year-olds in Poland. The aim of the study was to assess protective factors for ECC in 3-year-olds. Methods: A cross-sectional survey was conducted in 2017 among 3-year-olds attending kindergartens in all 16 provinces of Poland. The questionnaire included socioeconomic factors, and oral health behaviors. Decayed, missing due to caries and filled teeth and surfaces were assessed. Caries indices (dmft and dmfs), percentage of caries-free and severe ECC (S-ECC) were calculated. The Student's t-test, Spearman correlation, univariate and multivariate logistic regression (the odds ratios (OR) and adjusted odds ratios (AOR): confounding factors: socioeconomic conditions, oral health behaviors) were performed; p < 0.05. Results: A total of 1,638 children were included. ECC was confirmed in 41.1%; S-ECC in 23.4%. The dmft index was 1.85 ± 3.14, dmfs = 2.99 ± 6.56. Spearman correlation [socioeconomic factors, especially mother's education (r = -0.148, p < 0.001) and family's economic status (r = -0.071, p = 0.004)], despite were statistically significant, indicate very weak relationships with dmft index. After 12th month of life not giving any beverages before bedtime and at night, but water or unsweetened milk only, healthy snacking (sandwiches, fresh fruit), or water on a daily basis decreases the probability of caries development (for S-ECC: OR = 0.37, AOR = 0.47, AOR2 = 0.37; p < 0.01), even if inappropriate dietary behaviors or hygiene practices were simultaneously present. Brushing of a child's teeth twice a day lowered the odds of caries (for S-ECC: OR = 0.78, p = 0.031; AOR = 0.81, p = 0.081; AOR2 = 0.84, p = 0.131), but this effect was less intense when inappropriate dietary behaviors were accounted for. Children of parents with good self-assessment of their teeth were more often caries-free (61.4 vs. 42.9%; p = 0.006) and had lower dmft indices (1.67 vs. 2.93; p = 0.002). Conclusions: Preventing a child after 12th month of life from getting any beverages before bedtime, water or unsweetened milk only, sandwiches and fresh fruit as snacks, or water given to on a daily basis, tooth brushing twice a day decrease the odds of caries in 3-year-olds. Diet appears to have primary significance in the etiology of ECC, since tooth brushing can only partly attenuate the impact of inappropriate dietary behaviors on dental caries. Children are more often caries-free and have lower severity of caries if their parents' dentition is self-assessed as healthy.
[Analysis of the influence of some factors on the intensity of early childhood caries]

Abstract:Introduction: Early childhood caries is an important factor implicated in the loss of teeth in children. The chief causes of caries include improper oral hygiene by the parents and bad nutritional habits during the first years of life. The aim of this work was to analyze the influence of some factors on the intensity of early childhood caries. Materials and methods: The study was done in 105 children aged 3 years, pupils of kindergartens in Poznafi and Swarz dz near Poznaii. The clinical examination included determination of the caries intensity index (DMF) and the oral hygiene index (OHI). A questionnaire administered to parents addressed socio-economic factors (parent's education, age, and economic status), pregnancy and labor (course of pregnancy, diseases during pregnancy), and child history (antibiotic therapy, tooth eruption time, nutrition during infancy, consumption of sweets, oral hygiene, and fluoride prevention). The questionnaire was supplemented with a clinical examination. Results: DMF was 2.88 in the group; 3.02 in girls and 2.74 in boys. Nutritional habits of the child had an irppact on the intensity of caries. The greatest importance was attributed to the sweetening of foods and falling asleep during breast- or bottle-feeding. Poor oral hygiene represented by high OHI was a significant factor as well. Conclusion: Deficient knowledge of the parents concerning oral hygiene and proper nutrition in young children is the main factor determining the intensity of caries.
Hygienic and nutritional habits in dental caries prevention in 5-year-old children from Biala Podlaska

Abstract:Background: Preschool children are particularly susceptible to dental caries development. Due to the fact that deciduous teeth are less mineralised, they are more likely to be affected by this condition. Therefore, preschool age is a significant period in shaping positive health-oriented attitudes regarding dental caries prevention. Objective: The aim of this study was to analyse hygienic and nutritional habits in children aged 5 from Biala Podlaska in the context of dental caries prevention. Material and method: The study included 454 preschool children (251 boys and 203 girls) from Biala Podlaska. Their hygienic and nutritional habits were examined with a diagnostic survey using the authors' own anonymous questionnaire. Mean consumption of nutrients (calcium, phosphorus and vitamin D) was estimated on the basis of average daily consumption calculated as a mean from three 24-hour interviews (conducted by the authors in a kindergarten or at children's houses). A nutritional value of a daily portion was calculated with the use of "Dieta 5.0" software (National Food and Nutrition Institute). Results: Research results revealed that in the case of only one in three children, parents began toothbrushing within the first year of a child's life. Approximately 90% of preschool children brushed their teeth at least twice a day. Half of the children under examination brushed their teeth for 1-2 minutes usually with a traditional toothbrush. Toothbrushing was supervised by 62.5% of the boys' parents and 38.9% of the girls' parents (p<0.05). Almost 66% of the parents took their children to a dentist for regular (once or twice a year) dental check-ups. Analyses of daily food rations revealed insufficient amounts of calcium (25%) and vitamin D (over 80%). Moreover, an excessive intake of phosphorus (twice as high as the norm) was noted. Conclusions: Developing appropriate hygienic and nutritional habits in children is an indispensable element in the prevention of lifestyle diseases, including dental caries.

Abstract:This cross-sectional study aimed to assess the occurrence of the consequences of dental caries and factors affecting dental service utilization in a population of 7-year-old children. The research included oral examination of 7-year-old schoolchildren and socio-medical study of their parents/legal caregivers. It was carried out in five primary schools of two provinces, i.e., Greater Poland and Lubusz. Dental examination was performed in accordance with World Health Organization (WHO) recommendations. The socio-medical study consisted of questionnaires with close-ended questions concerning socioeconomic characteristics of the family, reasons and time of the last visit at the dental office, consequences of child’s oral health problems, parents’ attitude towards dental visits, and parental opinion about their child’s teeth. Factors affecting utilization of dental services were statistically analyzed using univariate logistic regression assuming p < 0.05. The pufa index of examined children ranged from 0 to 7 (mean 0.80 ± 1.49), while the dmf index ranged from 0 to 14 (3.86 ± 3.32). Low financial burden of oral health expenditures and university education of at least one of the parents significantly increased the chance of visiting a dentist despite lack of pain (OR = 3.0 and 2.5, respectively). In spite of the availability of free dental care for children, socioeconomic factors still determine the utilization of dental services in Poland. Poor oral health status of examined population and negligence of regular dental check-ups emphasize a need to strengthen oral health literacy of parents and children, promoting proper attitudes towards dental care.

Biochemical changes of fresh and preserved freshwaterprawns (Macrobrachium rosenbergii) during storage

Abstract:Biochemical analysis was carried out for pH profiles, freshness in terms of K-values, amino acids profiles, total volatile bases (TVB), total volatile acids (TVA) and biogenic amines for fresh and preserved Macrobrachium rosenbergii. Results showed that pH profiles of Macrobrachium rosenbergii explain the inability of this parameter to be used to evaluate the quality of Macrobrachium rosenbergii. Thus changes in pH profiles of Macrobrachium rosenbergii should be combined with indicators such as total volatile acids and total volatile bases. Total volatile acids of the shrimps increased steadily in small amounts throughout the storage period. A rapid increase in TVB at 10 0 C was detected due to the increase in total aerobic bacteria at elevated temperatures. The microbial activities caused the decrease in the amino acids arginine, lysine and histidine which correlated well with the increase in the corresponding biogenic amines such as putrescine, cadaverine and histamine respectively. Preservatives used in this study controlled the production of these biogenic amines without significantly altering the pH of preserved shrimp.
AFFECTION ON CARIES AND ITS COMPLICATIONS OF TEMPORARY TEETH OF CHILDREN IN A REGION WITH EXCESS FLUORINE CONTENT IN DRINKING WATER

Abstract:Objective: Aim: The aim of the study was to determine the indicators of caries and its complications in the temporary teeth of children who permanently live in a region with a high fluoride content in drinking water. Patients and methods: Materials and Methods: It was examined with the definition of caries and its complications 277 children in the age range from 2 to 13 years, who were born and permanently live in the urban-type settlement of Mashivka. The fluoride content in the drinking water of the settlement was 1.7-2.5 mg/l. Results: Results: During the analysis of data from the survey of children who were born and permanently lived in the urban-type settlement of Mashivka, it was determined that the prevalence of caries of temporary teeth probably increases with age. Half of the 3-5-year-old children had caries-affected teeth, and temporary tooth caries reached the highest rates in 10-year-old children. It should be noted that a fifth of children in the youngest age group (3-5 years old) suffer from pulpitis and periodontitis of temporary teeth. Conclusion: Conclusions: The conducted examination of children urban-type settlement of Mashivka confirms the opinion that the excessive content of fluorine in drinking water does not have a caries-protective effect, and the intensity of the process reaches the indicators characteristic of regions with its optimal content. Such a situation requires strengthening measures for both primary and secondary prevention of dental diseases.
[Evaluating the efficacy of selected programs for prophylaxis of caries and periodontal diseases in school children with special reference to guidelines for oral cavity hygiene]

Abstract:Endeavouring to improve the oral cavity soundness in 7-8 year-old children, an attempt was made to elaborate three prophylactic-educational programs with the possibility to apply them in school conditions. The aim of the work was to evaluate the efficacy of the said programs in the field of oral cavity hygiene, the status of periodontium and dental caries, as well as to observe the moulding of consciousness and correct hygiene habits. The clinical and questionnaire studies covered 255 pupils of two elementary schools in Szczecin. For the first 2-3 years of their lives the majority of children used tap water having been fluoridated to optimal level. The pupils of both schools were receiving planned stomatological treatment and contact fluoridation. The clinical evaluation was accomplished on the basis of 18-month-long observation of the increase in the number of teeth and dental surface with caries (DMFt and DMFs), status of periodontium (ind. GI) and state of hygiene (ind. OHI-S and QH). The tooth brushing method applied by children was additionally observed. The questionnaire studies were employed for estimating the changes in the health consciousness and hygienic habits. The three selected programs were implemented in three groups of studied children (Tab. 1). Program I was based on II individual, motivating instructions of tooth brushing. The main motivating factor was each time colouring of the bacterial plaque. Programs II and III were expanded by educational activity among children and parents. Program III was additionally enriched by professional cleaning of teeth, sealing and intensive conservative treatment. The above programs were carried out in cooperation of school stomatologist, teachers and V year stomatology students and girl pupils training for hygienists. The high frequency and intensity, hardly fair hygiene of the oral cavity and improper habits, having been disclosed in 7-year-old children during preliminary examinations, indicated that there was an urgent need to intensify the educational-prophylactic-therapeutic procedures. After 18-month-long studies it appeared that program I was effective with regard to improving hygiene (reduction of ind. QH by about 13%) (Fig. 1) and the status of gums (reduction of ind. GI by about 54%) (Fig. 2), however, it failed to exert any effective influence on the reduction of caries. Programs II and III were found to be effective with respect to the improvement of hygiene (reduction of ind. QH about 18 and 20%) (Fig. 1), status of gums (reduction of ind. GI about 67 and 67%) (Fig. 2) as well as reduction of caries (about 68 and 56%). Thus, for the exploitation under school conditions each of the three evaluated programs may be engaged, but the most recommendable would be program II or III. The actual studies have also shown the possibility of obtaining a distinct improvement in hygiene in a short time, however, a longer period of time was necessary to stabilize it. Moreover, the process of introducing a correct method of brushing the teeth appeared to be relatively difficult, but it was much easier to achieve that in the children who up to now have made use of partly correct method than in those who used an incorrect one. The implementation of the selected programs had stronger influence on the correction of health consciousness and weaker on the moulding of the habits of brushing the teeth with appropriate frequency. Reverse proportional dependence was also revealed to exist between caries intensity in children studied and the education of their parents.
Fluoride in the drinking water and dental caries experience by tooth surface susceptibility among adults

Abstract:Abstract Background Dental caries is the most prevalent non-communicable health condition globally. The surface-based susceptibility hierarchy indicates that surfaces in the same group have similar susceptibility to caries, where the most susceptible group consists of occlusal surfaces of first molars and buccal surfaces of lower first molars, and the least susceptible surfaces are smooth and proximal surfaces of first premolars, canines and incisors. Therefore, fluoride in the drinking water could impact one group more than the other group. The present study examined the association between fluoride levels in the drinking water and dental caries experience in adults in the context of varying tooth surface susceptibility. Methods Data from the cross-sectional National Lithuanian Oral Health Survey conducted in 2017–2019 included a stratified random sample of 1398 35–74-year-olds (52% response rate). Dental caries experience in dentine was measured at a surface level. The surfaces were grouped according to their caries susceptibility (group 1 being the most and group 4 the least susceptible), and dental caries experience was calculated separately for each susceptibility group, creating four outcomes. Information about explanatory variable, fluoride levels in the drinking water, was provided by the water suppliers. The questionnaire inquired about potential determinants: sociodemographic characteristics and oral health-related behaviors. Chi-square, Mann–Whitney U and Kruskal Wallis tests were used for descriptive statistics, and linear regression analyses to examine the association between fluoride levels and four outcomes. Results The proportions of median decayed, missing, filled surfaces decreased following the surface-based susceptibility hierarchy (group 1–33%, group 2–28%, group 3–24%, group 4–15%). When adjusted for potential determinants, higher-level fluoride (≥ 0.7 ppm vs < 0.7 ppm) in the drinking water associated with lower dental caries experience in all surface-based susceptibility hierarchy groups; Group 1: β = − 0.23 (95 %CI − 0.44; − 0.001), Group 2: β = − 0.44 (95 %CI − 0.82; − 0.07), Group 3: β = − 1.14 (95 %CI − 1.88; − 0.41) and Group 4: β = − 6.28 (95 %CI − 9.29; − 3.30). Conclusions The higher-level fluoride in the drinking water associated with lower dental caries experience in adults and this was observed in all surface-based susceptibility groups. However, there is a need to validate the surface-based susceptibility hierarchy in longitudinal adult studies.

Assessment of the drinking water quality for planning endogenic prevention of dental caries

Abstract:Relevance. Dental caries is a leading dental disease in children. Aim – to study the quality of drinking water in Smolensk and its region to provide evidence for implementing dental caries preventive measures in children. Materials and methods. We took tap water samples from ten water intake points in seven districts of Smolensk and six Smolensk regions. An accredited testing laboratory of the Center for Hygiene and Epidemiology in the Smolensk Region carried out water chemical analysis. The study analyzed standard parameters of drinking water quality: pH 6.5-8.5, fluoride content 0.60-1.2 mg/l; water hardness 7.0-9.0 (Sanitary Regulations and Standards 2.1.4.1116-02). The parameter was considered normal if its 95% confidence interval was within the reference range. Results. In Smolensk, the hydrogen ion concentration in centralized drinking water supply systems is within normal limits and amounts to 7.39 (95% CI: 7.32-7.46; р < 0,05) pH units. The mean fluoride concentration in the Smolensk water is 0.19 (95% CI: 0.14-0.23; р < 0,05) mg/l, which is below the normal range. In most Smolensk districts, water hardness is within normal limits, 8.21 mmol/l (95% CI: 7.03-9.39; р < 0,05). However, the upper limit of the confidence interval of 9.39 mg/l and the maximum of 12.0 mg/l exceed the normal range. In the Smolensk region cities, the hydrogen ion concentration is 7.2 (95% CI: 7.02-7.38; р < 0,05) pH units in the centralized drinking water supply. The fluoride concentration in the Smolensk region water is 0.45 mg/l (95% CI: 0.23-0.68; р < 0,05), which demonstrates the fluoride deficiency in water. In the Smolensk region cities, mean water hardness is 6.66 mmol/l (95% CI: 6.00-7.03; р < 0,05), which is below the normal values. However, the CI upper limit of 7.03 mmol/l and the maximum of 7.05 mmol/l are within normal limits. Conclusions. The water of the centralized drinking water supply system in Smolensk and the Smolensk region is low in fluorides. Urgent community and individual preventive measures should be taken to expose children to fluoride.
[An attempt to assess the effectiveness of a program for planned dental care during the developmental period in light of clinical examination and questioning of dentistry students from the Pomeranian Medical Academy]

Abstract:The aim of the study was to assess the effectiveness of planned dental care in educational institutions. The investigation was carried out among 339 3rd, 4th and 5th year students of the Pomeranian Medical University in Szczecin, aged 22-26, who attended nurseries and schools in the years 1969-1989 in different communities (large towns, medium-sized towns, small towns, villages). The first part of the study consisted of questionary answers concerning dental care in general in nurseries, primary and high schools. On the basis of these answers two groups were distinguished: a group of students who had been subjected to planned dental care (positive surveys) and a group of students who had not been subjected to planned care (negative surveys) (Tab. 1). At the second stage the clinical examination of the surveyed students was carried out and the obtained data were registered on 1986 WHO record cards. The examination was performed among students who were subjected to dental care in primary and high school together with those students who were not subjected to this care. All examinations were done at a dental surgery with the use of a mirror, probe and a calibrated WHO-621 probe. Results of the surveys showed that systematically performed examinations of the oral cavity and systematic dental treatment were carried out for over 60% of surveyed students in primary schools and about 40% of surveyed students in high schools. In primary schools the organization of health education was more effective, its activity was confirmed by about 80% of respondents, while in high schools by only 40%. In response to question concerning topical fluoride preventive measures 40% of positive answers were obtained from the surveyed students concerning primary schools and 10% concerning high schools. Survey questions in the part referring to nurseries were often answered "I don't remember" therefore it is impossible to interpret the results. Fluoride tablets were taken by about 40% of surveyed students. Over 65% of respondents with diagnosed malocclusion were treated orthodontically. Among students with dental calculus over a half had it removed earlier. Much better results of clinical examinations were obtained in the group subjected to planned dental care at developmental age. In this group the average DMFT number was 11.31, the average number of teeth with untreated caries (D) was 0.82, average number of removed teeth because of caries (M) was 0.55, whereas in the negative group (not subjected to dental care) the average DMFT number was 14.61, average D number was 1.41, average M number was 2.54. The average number of filled teeth (F) showed no significant difference in both groups and was about 10 (Tab. 2). Malocclusion was diagnosed in about 30% of surveyed students in the group subjected to treatment and in over 45% of surveyed students not subjected to dental care in school (Tab. 2). In the group of positive surveys the proportion of respondents with healthy periodontium was over 11% whereas in the group of negative surveys it was about three times lower (Tab. 4). The results of the study show that planned program of dental care is most effectively carried out in primary schools. The results have confirmed that systematic dental care at development age has a positive effect on the stomatognathic system at mature age.
Caries indicators in relation to healthcare access and hygienic‐dietary habits in Spanish preschoolers

Abstract:Introduction Dental caries continues to be a public healthcare problem due to its high prevalence and morbidity. It was proposed to evaluate caries indicators in relation to healthcare access and hygienic‐dietary habits in Spanish preschoolers. Materials and Methods A cross‐sectional study was carried out on a random sample consisting of 343 patients ranging from 3 to 5 years of age in the primary care setting, the main measurements were: dental care access, hygienic‐dietary habits, and caries registration according to WHO criteria. Results The prevalence of caries was 35.3% and mean ± standard deviation (SD) dft was 1.51 ± 2.93. Preschoolers who had ever visited the dentist had a higher dft occurrence than those who had never attended the dentist. The lower prevalence of caries was significantly associated with the frequency of two daily brushings versus non‐daily brushing (p = 0.007; 25.3% vs. 47.1%) and was 10.7% lower in patients who had the habit of night brushing (p = 0.041). The consumption of sugary liquids between meals implied a higher dft index. Conclusions The high prevalence of early childhood caries and the dental care access associated with poor oral health suggest the need to implement a multidisciplinary oral healthcare program that involves both healthcare and dental healthcare professionals in Primary Health Care Units without forgetting the participation of the community and local administrations.

Long-term effect of intensive prevention on dental health of primary school children by socioeconomic status

Abstract:Objectives: Children in a German region took part in regular toothbrushing with fluoride gel during their time in primary school after having received a preventive program in kindergarten. The study aimed at determining the dental health of the students as a function of prevention in kindergarten and at school while taking into account their socioeconomic status and other confounders. Materials and methods: The subjects were in six groups: groups 1 and 2, intensive prevention in kindergarten with and without fluoride gel at school; groups 3 and 4, basic prevention in kindergarten with and without fluoride gel at school; groups 5 and 6, no organized prevention in kindergarten with and without fluoride gel at school. Two dental examinations were performed for assessing caries experience and calculating caries increment from second grade (7-year-olds) to fourth grade (9-year-olds). A standardized questionnaire was used to record independent variables. To compare caries scores and preventive measures of various subgroups, non-parametric tests and a binary logistic regression analysis were performed. Results: A significant difference was found in the mean decayed, missing, and filled tooth/teeth (DMFT) depending on socioeconomic status (no prevention in kindergarten, fluoride gel at school in children with low SES: DMFT = 0.47 vs. DMFT = 0.18 in children with high SES; p = 0.023). Class-specific differences were no longer visible among children who had taken part in an intensive preventive program combining daily supervised toothbrushing in kindergarten and application of fluoride gel in school. Conclusions: Early prevention, focusing on professionally supported training of toothbrushing in kindergarten and at school, has a positive effect on dental health and is able to reduce class-specific differences in caries distribution. Clinical relevance: Early training of toothbrushing and fissure sealing of first permanent molars are the most important factors for the dental health of primary school children.

Abstract:Background: Dental caries among preschool children were prevalent worldwide and had a significant impact on children and their families. Understanding its prevalence and risk factors helps to optimize the delivery of oral health care to the target population and promote their oral health ultimately. This cross-sectional study aimed to examine the prevalence of dental caries and its associated factors among 3- to 5-year-old children in Huizhou, Guangdong Province, China. Method: We recruited children from 21 kindergartens adopting multistage sampling method. Two examiners performed oral examination. They assessed children's dental caries experience following the World Health Organization criteria. Children's dental caries activity, malocclusion, tonsil size and pH value of saliva were evaluated. Parental questionnaires collected child's sociodemographic background and oral-health-related behaviors. Data were analyzed by univariate analysis and logistic regression using SPSS. Results: This study invited 1,485 children and recruited 1,348 (53.2% boys) (response rate: 90.8%). Dental caries prevalence rate was 58.2% for 3-, 70.7% for 4-, 80.5% for 5-year-old and 72.9% for all recruited children. The mean dmft score (±SD) was 3.38 (±4.26) for 3-, 4.75 (±4.96) for 4-, 5.81 (±5.71) for 5-year-old and 4.99 (±5.02) for all children. Age, family status (singleton or not), monthly family income, mother and father's education level, tonsil grading score, spacing in dentition, Cariostat score (reflecting the caries activity), dental plaque index, duration of breastfeeding, dental visit experience, tooth brushing habits and sugary snacking before sleeping were statistically related to the prevalence of dental caries (p < 0.050) in univariate analysis. These factors were further analyzed in the regression model. The results of the final model indicated dental caries were associated with age (p < 0.001), Cariostat score (p < 0.001), spacing (p < 0.001), tonsil grading score (p = 0.013), singleton or not (p = 0.002), sugary snacking habit before bed (p < 0.001) and breast-feeding duration (p = 0.050). Conclusion: Dental caries was prevalent among 3-to 5-year-old preschool children in Huizhou, China. Children's age, caries activity, tonsil size, malocclusion, family background, sugary snacking habit and breast-feeding habit were related to the prevalence of dental caries. More emphasis should be placed on prevention targeting the risk factors from early life.

Abstract:PURPOSETo evaluate the effect of fluoride varnish Duraphat on preventing caries of primary teeth caries, and provide scientific basis for the establishment of oral health care.METHODS1140 preschool children aged 3~4 years old were chosen from 25 public kindergartens in Xuhui district of Shanghai by cluster sampling method, they were divided into 2 groups on a voluntary basis with the informed consent of the child's parents. 762 preschool children in Fluoride Varnish group were taken oral examination and used fluoride varnishes every 6 months, 378 preschool children in the control group were without treatment. Oral health instruction was given to all the children. The data of caries prevalence, mean dmft, caries incidence and mean increments of dmft (decayed, missing and filled teeth) of 2 groups were compared. The results were analyzed using SAS 9.4 software package.RESULTSAfter 2 years, caries prevalence, mean dmft, caries incidence and mean increments of dmft of fluoride varnish groups were lower than those of control group and the difference was statistically significant (P＜0.05).CONCLUSIONSFluoride varnish Duraphat is effective in decreasing caries incidence and mean dmft increments of primary teeth in preschool children and is worthy to be used widely in Xuhui district.
Early Childhood Caries in a Preschool-based Sample in Northeast Italy: Socioeconomic Status and Behavioral Risk Factors

Abstract:Aim: To assess the relationship among caries and age, gender, immigrant condition, socioeconomic status (SES), and a behavioral risk factor (toothbrushing) in a sample of 3-5-year-old children.Materials and methods: We performed a random cross-sectional survey from January to December 2017, carrying out clinical examinations to estimate the decayed, missing, and filled teeth (dmft) score. Parents filled in a questionnaire reporting their education level (SES) and the daily frequency of children's toothbrushing. The multivariate analysis assessed the association between caries occurrence and the independent variables. The dmft score was assessed by zero-inflated negative binomial regression (ZINBR).Results: Out of 1,441 children in the sample, 357 (26.0%) presented at least one caries-affected tooth. Caries risk significantly increased with age and irregular toothbrushing, and it was significantly higher in children with lower SES levels. We modeled caries risk by means of ZINBR. The degree of caries experience increased in children from lower SES positions, immigrant status, and of older age; regular toothbrushing (twice a day) is a predicting factor to belong to the "zero caries" group.Conclusion: Dental caries represent a significant burden in preschool children and can be regarded as an early marker of social disadvantage.Clinical significance: The confirmation of the earliest preventive approach as the only chance to grant a "caries free" dentition in all ages and the first target for a pediatric dentist.How to cite this article: Ferro R, Besostri A, Olivieri A, <i>et al.</i> Early Childhood Caries in a Preschool-based Sample in Northeast Italy: Socioeconomic Status and Behavioral Risk Factors. Abstract:Introduction. Dental caries is a significant public health issue due to its high incidence among the population. While modern scientific research highlights effective treatments for this condition, prevention remains crucial, directly impacting health and quality of life, especially when early preventive habits are instilled. This study aims to explore how parents' educational levels and other factors influence the acquisition, use, and proper application of oral hygiene knowledge among primary school children. Material and methods. The study surveyed 678 parents of primary school children across 9 administrative regions, including both urban and rural areas of the Republic of Moldova. It employed a randomized approach, involving respondents from all social groups. Results. This study found that parental education level is a crucial factor influencing the proper development of oral hygiene skills, which can prevent dental conditions. Higher parental education was positively associated with urban living environment and family income. Additionally, well-educated parents tend to visit dentists more regularly, which also reflects in better oral hygiene practices at home for their children. Conclusions. Children's oral hygiene habits are strongly shaped by socioeconomic factors and the care parents show. Increasing parental education levels, whether in urban or rural settings, plays a pivotal role in fostering correct dental hygiene practices early in life, thereby ensuring long-term oral health habits into adulthood.
Pediatric Dental Fluorosis and Its Correlation with Dental Caries and Oral-Health-Related Quality of Life: A Descriptive Cross-Sectional Study among Preschool Children Living in Belagavi

Abstract:Aim: This study aims to assess the prevalence of dental fluorosis and its association with dental caries, oral health behaviors, oral-health-related quality of life and parents’ perceptions among preschool children in the 3–5-year-old preschool children living in the Belagavi district of Karnataka, a non-endemic fluorosis region. Methods: A descriptive cross-sectional questionnaire-based study was conducted among 1200 individuals of the preschool population from 48 government-sponsored child-care development centers in Belagavi, Karnataka, over a three-month period. They were examined following the Dean’s fluorosis index (1942) and dmft (decayed, missed, and filled) scores of the participants were also recorded. Parents’ perception of oral health was assessed using the self-administered Early Childhood Oral Health Impact Scale (ECOHIS). SPSS software (version 20) was used for statistical analysis. Chi-square test computed categorical data. One-way ANOVA test was used for multiple group comparisons. p < 0.05 was considered statistically significant. Results: Of the 1200 children examined, 10 (0.83%) children exhibited dental fluorosis. Of the 10 children having fluorosis, six had fluorosis present on two or more of their primary teeth, and four had fluorosis on four or more teeth. The mean dmft score was 3.01 ± 1.38 to 3.60 ± 1.72 in 3–5-year-old children, respectively, with statistical significance difference (p < 0.001). The mean score of oral health-related quality of life was 10.74 ± 2.06, which was significantly correlated to the child’s age and parents’ education level (p < 0.05). Conclusion: The study shows a negligible amount of prevalence of dental fluorosis in the non-endemic fluorosis residential district. It also elucidates that children from lower and lower-middle socioeconomic status are more prone to suffer from dental fluorosis as compared to other groups. The mean score of ECOHIS increased proportionally with the caries experience, indicating a significant relationship between the dmft and ECOHIS score. Deciduous dentition fluorosis is often neglected, especially in areas that are not considered endemic for fluorosis and with only optimum fluoride levels in their groundwater, which supports the disease’s nature as multi-factorial and shows that a broader perspective is of prime importance to assess, diagnose, and prevent dental fluorosis among the preschoolers, thereby appraising their overall health and hygiene status.

Abstract:Abstract Background Early childhood caries (ECC) is one of the most common chronic diseases among children. In Poland 86.9% of six-year-olds have ECC. One of the factors determining adherence to ECC prophylaxis and oral hygiene is mothers’ knowledge. The aim of this study was to assess the level of knowledge demonstrated by pregnant women about ECC prevention and oral hygiene, and to analyse the determinants of this knowledge. Methods A quantitative survey was conducted using Computer Assisted Telephone Interview technique on a randomly selected representative sample of 1,000 women over the age of 18 in their second and third trimesters of pregnancy. The significance level was established at 0.05 and p -values were presented as: p < 0.05, p < 0.01 and p < 0.001. Results The highest percentage of wrong or “I don’t know” answers were related to questions about: the number of free dental check-ups for children (76.8%), the date of the child’s first visit to the dentist (66.5%), the age when the child has mixed dentition (72.2%). Women with higher education had better knowledge than women with lower or secondary education. Women with good and very good financial situation showed a higher level of knowledge compared to women with average, bad and very bad financial situation. Conclusions When developing prevention strategies and educational programs as part of prenatal care for women to reduce the incidence of ECC, it is important to take into account the identified areas that need support and specific target groups (mothers with lower socioeconomic status).
