Abstract:Purpose: To compare the safety and performance clinical outcomes of the 27-gauge (G) two-dimensional cutting vitrectomy probe versus a standard 25-G probe for retinal procedures. Methods: In this large randomized prospective study, all candidates for epiretinal membrane or macular hole surgery were randomized to the 27-G group or 25-G group. Outcome measures included surgery time, changes in best-corrected distance visual acuity (BCDVA), intraocular pressure (IOP), central macular thickness (CMT) between baseline and 1- and 3-months follow-up timepoints. Moreover, intraoperative and postoperative complications were evaluated as well as the rate of sutureless vitrectomy. Results: A total of 463 patients were included in this study, 227 patients in the 27-G group and 236 patients in the 25-G group. A similar total surgery time was found between both groups (p=0.0911). Similar BCDVA and CMT changes were observed between baseline and the 1- and 3-mont follow-up visits. No significant differences were reported in terms of intraoperative and postoperative complications rates. The rate of sutureless vitrectomy was 96.5% for 27-G group and 91.1% for 25-G group (p=0.0170). Conclusion: These results suggest that 27-G vitrectomy probe is similar to 25-G probe in terms of surgery time and complications, while decreasing the need for vitrectomy sutures.

Abstract:Introduction: The development of vitreoretinal surgery is inseparably linked to the gradual miniaturization and decreasing invasiveness of the procedure. The smallest available systems are 23G, 25G, and 27G, which are referred to as minimally invasive vitreoretinal surgery techniques. This paper presents the preoperative and postoperative results of three patients undergoing posterior vitrectomy using the 25G or 27G systems with the HIPERVIT probe (Alcon, USA). The effects were retrospectively analyzed in terms of changes in best corrected visual acuity, intraocular pressure, occurrence of complications, and changes in optical coherence tomography images of macular edema. Examinations were performed before and one day, two weeks, three months, and six months after vitrectomy. The potential benefits and drawbacks of using these systems in practice were analyzed. Case report: The analysis concerns the results of the performed posterior vitrectomy procedure in patients with full-thickness macular hole, tractional retinal detachment due to diabetic retinopathy, and vitreomacular traction syndrome. Conclusions: The performed procedures allowed for improvement in both anatomical and functional conditions of the macula. All analyzed patients showed an improvement in visual acuity. None of the analyzed patients required the use of scleral sutures. Serious complications were not observed during the surgery or during the 6-month observation period. 25G and 27G HYPERVIT vitrectomy can be successfully used in the discussed vitreoretinal diseases. In the discussed cases, it enabled the effective and safe performance of the procedure.

De novo malignancy after renal transplantation.

Abstract:We reviewed the incidence of de novo malignancy in renal transplant recipients who received their grafts between 1979 and 1989, and had at least two years of graft function. Multiple attempts were made to contact 236 patients who were then surviving, or, in whom the medical history could be validated prior to death. There were 94 successful contacts, among whom 19 patients (20.12%) had developed 30 malignant lesions. There were 24 instances of skin and 6 of non-skin malignancy. Squamous cell carcinoma of the skin was the most common new malignancy observed.
Acute Posttraumatic Stress Disorder in Victims of a Natural Disaster

Abstract:Five months after a tornado devastated a rural community in eastern North Carolina, the authors surveyed the mental health status of 116 disaster victims, using the Hopkins Symptom Checklist (HSCL) expanded to include most of DSM-III criteria for posttraumatic stress disorder (PTSD). A total of 69 (59%) victims met the criteria for acute PTSD, 19 of whom had a severe form. Although an inadequate degree of social support was more often noted in victims with severe PTSD, other demographic factors and degree of injury or property damage did not appear to be related to the presence of PTSD. Severity or presence of PTSD was supported by high scores on all HSCL subscale factors. These findings suggest a high incidence of acute PTSD in victims of natural disasters and the potential value of HSCL in screening for PTSD in large populations.
Geometry, penetration force, and cutting profile of different 23-gauge trocars systems for pars plana vitrectomy

Abstract:Background: To investigate the geometry, penetration force, and cutting profile of 23-gauge trocar systems for pars plana vitrectomy based on their grinding methods in a standardized laboratory setting. Methods: In this experimental study, Eleven different commercially available 23-gauge sclerotomy trocar systems were divided into 4 groups according to their needle grinding and deburring: "back" bevel, "spear" bevel, "lancet" bevel, and "spatula" bevel. The normative geometrical data of the trocar systems were systematically analyzed according to nomenclature ISO 7864 and ISO 9626. Force to penetrate a 0.4-mm thick polyurethane foil was measured by a Penetrometer, when the trocar needle was piercing, cutting, and sliding through the foil at different defined loading phases and plotted as a load-displacement diagram. Magnified images of the consecutive cut were taken under a microscope after the entire penetration through the foil. Three physicians used all trocar systems in a masked fashion on human sclera to evaluate the manual penetration force in 30° and 90°. Results: The mean outer diameter of the trocar systems was 0.630 ± 0.009 mm, and the mean outer diameter of the trocars was 0.750 ± 0.013 mm. The mean point length was 3.11 ± 0.49 mm, and the mean length of the bevel was 1.46 ± 0.23 mm. The primary bevel angle was 10.75 ± 0.41°, and the secondary bevel angle was 65.9 ± 42.56°. The piercing forces of the back bevel and spear-pointed trocars/needles were at the same level (0.087 ± 0.028 N). The lancet-pointed needle had remarkable low piercing and cutting forces with 0.41 N (range, 0.35-0.47 N). The spatula bevel tip showed the highest penetration piercing force with 1.6 N (range, 1.59-1.73 N). The back bevel systems induced frequently triangular-shaped incisions, with two nearly rectangular cuts of short length. The spear bevels produced a regular characteristic linear cut. Especially, the lancet blade created straight cut with a linear wound apposition. Spatula trocar systems caused usually an arched accurate incision. The manual force to penetrate the human sclera in an angled and rectangular angle appeared in the surgeons hand lower with a back bevel, lancet, or spear tip, whereas higher with spatula bevel systems. Conclusion: Lancet and back bevel systems show less penetration force of inner needles than the spatula systems. The results of the penetration force experiments correlated well with the manual force on sclera.
Comparisons of the energy efficiency and intraocular safety of two torsional phacoemulsification tips

Abstract:Background During cataract phacoemulsification surgery, an Intrepid® balanced (IB) tip can achieve a larger amplitude, which may lead to higher energy efficiency than a Kelman (K) tip when paired with a torsional phaco platform. In this retrospective cohort study, we compared their energy efficiency and damage to the cornea under a new energy setting. Methods The medical records of 104 eyes of 79 patients were reviewed, with 47 eyes belonging to the IB group and 57 eyes belonging to the K group. All surgeries were performed on an Alcon Centurion® platform with gravity infiltration. Surgical parameters, visual outcome, central corneal thickness (CCT) changes, and endothelial cell density (ECD) loss rate were recorded and calculated. Results No significant differences in postoperative best corrected visual acuity (BCVA), intraocular pressure (IOP), total ultrasound time, estimated fluid aspirated, CCT changes, or ECD loss rate were observed between the two groups. We divided the included eyes into soft nucleus and hard nucleus subgroups and found lower cumulative dissipated energy (CDE, 8.15 ± 8.02 vs 14.82 ± 14.16, P = 0.023), cumulative torsional energy (CTE, 8.06 ± 7.87 vs 14.13 ± 13.02, P = 0.027), and cumulative longitudinal energy (CLE, 0.09 ± 0.17 vs 0.69 ± 1.37, P = 0.017) in the IB group than in the K group, implying less energy used and higher energy efficiency of the IB tip. Conclusion Lower CLE in the IB group indicates fewer phaco tip obstructions and a significantly higher capability to conquer hard nuclei with IB tips with statistical significance. With an ultra-perfusion cannula, the balanced tip does not cause more corneal damage.
Comparative Analysis of Three-Dimensional Heads-Up Vitrectomy and Traditional Microscopic Vitrectomy for Vitreoretinal Diseases.

Abstract:ABSTRACT Purpose: To investigate the visual outcomes and occurrences of perioperative complications after three-dimensional (3D) heads-up vitrectomy compared with traditional microscopic (TM) surgery for vitreoretinal diseases. Methods: A retrospective case series comparison of 3D and TM vitrectomy performed on 124 and 202 eyes with a minimum follow-up of 6 months, respectively, by the same surgeon. Changes in visual acuity, primary anatomical success, operation time, and perioperative complications were evaluated. Results: There were no significant differences in age, gender, axial length, ocular and surgical history, surgical indication, and visual acuity between the two groups at baseline. Both groups showed a significant improvement in visual acuity at the final visit (both P < .05) and experienced comparable anatomical recoveries regarding epiretinal membranes, vitreous hemorrhage, macular holes, retinal detachment, and pathological myopic foveoschisis. The mean overall operation time was not significantly longer in the 3D group (31.0 ± 17.6 min) than the TM (31.0 ± 15.9 min) group (P = .994). The incidence rates of postoperative complications during the follow-up were similar between the 3D (30.6%) and TM groups (30.2%) (P = .932). Conclusions: The 3D and TM vitrectomy surgeries yielded comparable visual and anatomical outcomes for treatment of vitreoretinal diseases without a significant difference in the rate of complications. The 3D heads-up vitrectomy may be considered the treatment of choice for patients with various vitreoretinal diseases.
Surgically Induced Refractive Changes in 25-Gauge Transconjunctival Sutureless Vitrectomy

Abstract:Objective To compare the changes in refractive status after 25-gauge sutureless vitrectomy and 20-gauge standard vitrectomy. Methods One hundred and twenty-one eyes of 121 patients who presented with vitreoretinal disease were included in this prospective study. Patients were randomly divided into two groups, the A group, 61 eyes that underwent 20-gauge standard vitrectomy, and the B group, 60 eyes that underwent 25-gauge vitrectomy. Outcome measures included visual acuity, refractive power, and changes in corneal topography after surgery. Data were analyzed with chi-square test, independent samples t test, one-way ANOVA and Mann-Whitney test. Results The visual acuity of the B group was much better than that of the A group at one week and one month after surgery (x2=8.81, P=0.012; x2=65, P=0.036). The postoperative refractive error in the A group developed into hyperopia, then gradually returned to baseline at three months after surgery. The spherical and astigmatism powers for each timepoint were significantly different in the A group (F=530.12, P＜0.05, F=392.06, P＜0.05), but no significant difference was found in the B group. In the A group, changes of spherical power at 1 week, 4 weeks, 12 weeks after surgery were (+1.93±0.07)D, (+1.06±0.04)D, (+0.15±0.03)D, and changes of astigmatism power were (1.62±0.07)D, (1.05±0.05)D, (0.41±0.03)D. In the B group, changes of spherical power at 1 week, 4 weeks, 12 weeks after surgery were (-0.06±0.02)D, (-0.04±0.03)D, (-0.03±0.02)D, and changes of astigmatism power were (0.05±0.01)D, (0.03±0.01)D, (0.03±0.02)D. Change of spherical power between the A and B groups at 1 week, 4 weeks, 12 weeks after surgery was significant different (t=200.6, 159.3, 26.0, P＜0.01 for all), as well as astigmatism power (t= 173.4, 156.3, 82.3, P＜0.01 for all). At one week after surgery, corneal topography analysis showed that corneal SimK1-K2, surface asymmetry index (SAI), and surface regularity index (SRI) were significantly different compared to baseline (F=4.21, 3.44, 3.28, P＜0.05 for all). However, the changes in comeal topography before and after surgery in the B group were still statistically insignificant. The changes of SimK1-K2, SAI, SRI in A group were significantly more than that in B group (U=51.5, 45.5, 47.0, P＜0.05 for all). Conclusion Compare to a 20-gauge vitrectomy system, a 25-gauge transconjunctival sutureless vitrectomy system provides a more stable refractive status and better improvement in visual function postoperatively in vitreoretinal surgeries.
In-Office Vitreous Biopsy With a Vitreous Cutter and Manual Actuation

Abstract:In-office vitreous biopsy is currently performed with a 25-gauge needle or less frequently with a specialized in-office surgical system. This article demonstrates in-office vitreous biopsy with a standard vitreous cutter, using syringes to actuate the cutter. A 79-year-old woman presented six days after intravitreal bevacizumab with endophthalmitis. After subconjunctival anesthesia, a valved 27-gauge trocar was inserted through the pars plana. Two syringes were connected to a pneumatic 27-gauge Alcon vitrectomy handpiece and manually actuated by an assistant while the physician aspirated with a third syringe to obtain the vitreous biopsy. Intravitreal vancomycin and ceftazidime were injected. A total of 0.5 cc of fluid was collected without complications. Manual actuated vitrectomy reliably collects sufficient vitreous samples for diagnostic evaluation and may be safer and more effective than needle biopsy. [Vitrectomy for Severe Proliferative Diabetic Retinopathy: a Comparison of Optical Fiber Free Intravitreal Surgery System with Traditional Vitrectomy].

Abstract:OBJECTIVE To compare the optical fiber free intravitreal surgery system with traditional vitrectomy in eyes with severe proliferative diabetic retinopathy (PDR). METHODS 22 eyes of 18 patients with extensive fibrovascular proliferation associated with PDR were undergone vitrectomy using optical fiber free intravitreal surgery system (OFFISS) with bimanual technique, peeling of epiretinal membrane and/or internal limiting membrane, intraocular laser coagulation, air-fluid exchange, tamponading of air and expansive gases or silicon oil were performed according to the condition of diseases. 22 eyes with similar severity of fibrovascular proliferation treated with traditional vitrectomy were served as control. RESULTS The operations were finished successfully in both different vitrectomy systems, but fundus cannot be visualized during fluid-gas exchange in phakic eyes, and intraocular illumination had to been used again in OFFISS group. Postoperatively, retina was fully attached in all eyes, best-corrected visual acuity of 19 eyes had improved two lines and more in OFFISS group, and 17 eyes in control group. No significant difference in surgical complications was found between OFFISS and traditional vitrectomy group. Mean duration of peeling membrane was less in OFFISS group than control group. CONCLUSIONS The efficacy of vitrectomy using different system for severe PDR both are satisfactory and comparable. We consider vitrectomy using OFFISS more safe and effective during complicated surgical manipulations than using traditional vitrectomy system.
Comparative evaluation of the safety of the lens and the ora serrata during vitrectomy with scleral incisions at 5.0 mm or 4.0 mm posterior to the limbus

Abstract:Objective To investigate the lens and ora serrata safety during 23G vitrectomy with sclera incisions at 5.0 mm or 4.0 mm posterior to the limbus.Methods A prospective case-controlled study was adopted.From April 2016 to January 2018,290 consecutive primary 23G vitrectomy patients (300 eyes) with vitreoretinal disease in Department of Ophthalmology of Subei People's Hospital Affiliated to Yangzhou University were enrolled in this study.Among them,146 patients (150 eyes) received 23G pars plana vitrectomy (PPV) with seleral incisions at 5.0 mm posterior to the limbus (5.0 mm group),and 144 patients (150 eyes) at 4.0 mm (4.0 mm group).No statistically significant difference was found in age,axial length(t=-1.324,0.867;P=0.186,0.387) and in gender,right/left eyes,proportion of indications (x2=1.366,2.615,10.195;P=0.242,0.106,0.070) between the two groups.The incidence rate of complications between the two groups were comparatively observed,such as lens injury,retinal tears close to the scleral incision,retinal hemorrhage,supra-choroidal expulsive hemorrhage and iatrogenic retinal detachment.Independent sample t test and x2 test were performed for comparison between the two groups.Results Lens injury was observed in 4 eyes (2.67％) and 14 eyes (9.33％) respectively in the 5.0 mm and 4.0 mm group during surgery (x2=5.910,P=0.015).Retinal tears close to the scleral incision sites were observed in 5 eyes (3.33％) and 6 eyes (4.00％) respectively in the 5.0 mm and 4.0 mm group during surgery (2x2=0.094,P=0.759).The mean time of removing the vitreous base was 6.17 ± 2.76 min and 10.03 ± 5.56 min respectively in the 5.0 mm and 4.0 mm group (t=7.599,P＜0.01).No other surgical complications occurred in any group,such as retinal hemorrhage,supra-choroidal expulsive hemorrhage and iatrogenic retinal detachment,etc.Conclusion In primary 23G PPV,the safety ofora serrata with incisions at 5.0 mm posterior to limbus is similar to that at 4.0 mm,but the safety of lens and the efficiency of vitreous resection is higher with incisions at 5.0 rnm.
Comparison of Two Techniques for Toric Intraocular Lens Implantation: Hydroimplantation Versus Ophthalmic Viscosurgical Devices

Abstract:Background To compare the results between hydroimplantation of a single-piece, acrylic foldable toric intraocular lens (IOLs) and conventional implantation using an ophthalmic viscosurgical device (OVD). Methods In this study, 60 eyes with cataract and preexisting regular corneal astigmatism of 1.0 to 3.0 diopters (D) underwent the implantation of the AcrySof toric IOLs (Alcon Laboratories, Inc.). The patients were randomly assigned to a conventional implantation technique with an OVD or a hydroimplantation technique. Comparison of preoperative and postoperative parameters was performed using paired Student t tests, and independent Student t test was used to compare between the two groups. Results Three months postoperatively, the mean subjective astigmatism was 0.45 D ± 0.24 (SD) in the OVD group and 0.49 ± 0.29 D in the hydroimplantation group ( P = 0.492). The mean endothelial cell density (ECD) loss was 7.54% ± 0.82% and 7.32% ± 0.59%, respectively ( P = 0.117). The mean absolute IOL rotation was 4.77 ± 2.32 degrees and 4.70 ± 1.95 degrees, respectively ( P = 0.334). The mean time for IOL implantation was 71.50 ± 8.10 s and 37.60 ± 3.90 s, respectively ( P < 0.001). Two hours, 1 day, 1 week, 1 month, and 3 months postoperatively, there was no significant difference in IOP between the two groups ( P > 0.05), although IOP two hours postoperatively seemed to be a little higher in the OVD group. Conclusions Compared with the use of OVDs for toric IOLs implantation, the hydroimplantation technique provided advantages of increased efficiency, reduced surgical time and cost, and no concerns of OVD-induced elevated IOP. Abstract:Purpose: The objective of this study is to demonstrate, based on objective clinical indicators, the advantages of depth of field provided by the 3D surgical video system compared to the traditional microscope during vitrectomy for treating epiretinal membranes(ERM) or macular holes(MH). Methods: A total of 38 patients were included in this study and randomly assigned to either the 3D surgical video group or the conventional microscope group. Surgical parameters such as the Focal plane adjustment frequency (FPAF), membrane peeling time, and number of attempts to peel the membrane were recorded for each patient. Additionally, patients were followed up for 3 months postoperatively. Results: No significant differences in age, gender, operated eyes, or follow-up rates between the groups. The 3D group had significantly lower Focal Plane Adjustment Frequency (FPAF) in macular hole surgery and epiretinal membrane surgery . No significant differences in peeling maneuvers, time, or total surgical time. Postoperative follow-up data showed no significant differences. Conclusion: In conclusion,the 3D surgical video system exhibits potential advantages in depth of field.The 3D surgical video system is a safe and effective technology in vitrectomy for macular diseases.

Abstract:European Journal of Ophthalmology, Ahead of Print. PurposeTo compare the complication rates and surgical duration of cataract surgery using two 3D visualization systems and a traditional binocular microscope among experienced and inexperienced surgeons.MethodsThis retrospective case series included 571 eyes that received cataract surgery using either heads up cataract surgery, via a 3D head mounted system (N = 148–Group 1) or a 3D display screen (N = 338 eyes–Group 2), or traditional binocular microscope (N = 85 eyes–Group 3). The surgical records of consecutive patients who underwent cataract surgery by two groups of surgeons (experienced and inexperienced) were reviewed. Patients in all groups received either femtosecond laser assisted cataract surgery (FLACS) or traditional phacoemulsification. Complication rate, as well as duration of cataract surgery were evaluated between all three visualization approaches, between experienced and inexperienced surgeons.ResultsThere was no statistically significant difference in duration of surgery between all 3 visualization approaches for both experienced and inexperienced surgeons (p < 0.05). Furthermore, the type of surgical technique (manual or FLACS) did not affect the surgical duration for both experienced and inexperienced surgeons (p < 0.05). No intraoperative complications were demonstrated in the current cohort.ConclusionsThe implementation of heads up–3D visualization either through a screen or a head mounted platform for cataract surgery seems to offer similar safety and efficiency as the traditional binocular microscope, and both experienced and inexperienced surgeons demonstrate the same outcomes in terms of safety and efficiency.

Abstract:Objective To compare surgical outcomes between 27 and 25-gauge vitrectomy in proliferative diabetic retinopathy (PDR) with tractional retinal detachment (TRD) Methods This retrospective study was conducted to compare the intraoperative status, operation time, use of instruments, endotamponade substance, wound suture number, and iatrogenic break, between 27 and 25-gauge vitrectomy in 43 eyes afflicted by PDR with TRD. The post-surgical results, best-corrected visual acuity, intraocular pressure, recurrent vitreous haemorrhage, and re-operation rate were regularly followed up for 6 months. Results Patients in the 25 and the 27-gauge groups did not differ significantly in terms of pre-surgical conditions, such as age, gender, pre-existing glaucoma, best-corrected visual acuity (BCVA) and the severity of their TRD. The mean operation time was 56.7 minutes in the 27-gauge group and 63.7 minutes in the 25-gauge group (p = 0.94). There is significantly less use of micro forceps in the 27-gauge group (p = 0.004). No difference between micro scissors and chandelier usage were noted; neither was their difference in iatrogenic retinal breaks. Significantly fewer wound sutures were noted in the 27-gauge group (p < 0.001). The post-operative results revealed no significant difference in ocular hypertension, hypotony, BCVA improvement, recurrent vitreous haemorrhage and re-operation rate. Conclusions The 27-gauge vitrectomy system offers comparable surgical outcomes in PDR with TRD. The 27-gauge vitrectomy system is suitable for complicated retinal surgery.

Comparison of Surgical Times Between Manual and Robot-Assisted Epiretinal Membrane Peeling

Abstract:Purpose: Epiretinal membranes (ERM) pose a common challenge in vitreoretinal pathology, often causing vision impairment in older adults. The Preceyes Surgical System (PSS) supports the surgical removal of ERM through robot-assisted membrane peeling (RA-MP). This study compares surgical times and iatrogenic hemorrhages between manual membrane peeling (MMP) and RA-MP using PSS. Methods: Nine patients underwent RA-MP with PSS, whereas 16 patients (18 eyes) underwent MMP for comparative analysis. Surgical durations were categorized into RA-MP, manual forceps utilization in PSS surgeries (mRA-MP), and traditional MMP. Cumulative manual manipulation duration (cMMP), instrument grasps, and intraoperative hemorrhages were statistically analyzed using the Mann-Whitney U test. Results: RA-MP showed significantly longer peeling times compared to MMP (P < 0.001). Flap initiation grasps were similar between methods (P = 0.86), RA-MP demonstrated a significant reduction in peeling grasps (P = 0.01) and mean grasps per minute (P < 0.001). Although RA-MP resulted in fewer hemorrhages, the difference did not reach statistical significance relative to MMP (P = 0.08). Discussion: Although RA-MP tended to extend surgical time, it offered advantages in reducing tissue trauma and intraoperative hemorrhages. Further research is needed to explore the learning curve for novice surgeons and evaluate the safety profile of RA-MP. Translational relevance: RA-MP may offer potential advantages over manual surgery, particularly in terms of reduced tissue trauma and intraoperative hemorrhages. Despite its longer duration compared with manual techniques, RA-MP may lead to fewer grasping maneuvers and lower rates of hemorrhages, thereby enhancing the safety and precision of vitreoretinal surgeries.

Abstract:Purpose: To compare peak surge and surge duration time after occlusion break, incision leakage compensation, and passive vacuum for 2 phacoemulsification systems. Setting: Carl Zeiss Meditec AG, Oberkochen, Germany. Design: Laboratory study. Methods: A spring-eye model was used to test Alcon Centurion Vision and Zeiss Quatera 700 systems. Peak surge and duration was measured after an occlusion break. Quatera tested in flow and vacuum priority modes. Vacuum limits ranged from 300 to 700 mm Hg with intraocular pressure (IOP) set at 30 mm Hg, 55 mm Hg, and 80 mm Hg. IOP vs incision leakage rates of 0 to 15 cc/min and passive vacuum were measured. Results: At 30 mm Hg IOP set point and vacuum limits ranging 300 to 700 mm Hg, the surge duration time after occlusion break ranged 419 to 1740 milliseconds (ms) for Centurion, 284 to 408 ms for Quatera in the flow mode, and 282 to 354 ms for Quatera in the vacuum mode. At 55 mm Hg, values ranged 268 to 1590 ms for Centurion, 258 to 471 ms for Quatera in the flow mode, and 239 to 284 ms for Quatera in the vacuum mode. At 80 mm Hg, values were 243 to 1520 ms for Centurion, 238 to 314 ms for Quatera in the flow mode, and 221 to 279 ms in the vacuum mode. Centurion exhibited slightly less peak surge than the Quatera. At 55 mm Hg: incision leakage rates 0 to 15 cc/min, Quatera held the IOP within ±2 mm Hg of target; Centurion was unable to hold IOP target allowing a 11.7 mm Hg decrease with 32% higher passive vacuum. Conclusions: Quatera demonstrated slightly higher surge peak values and notably shorter surge duration times after occlusion break than Centurion. Quatera demonstrated better incision leakage compensation and lower passive vacuum than Centurion.

Abstract:Background: Swept-source anterior segment optical coherence tomography (SS-AS-OCT) is a suitable examination for the vitreolenticular interface. Methods: In a prospective study using Anterion (Heidelberg Engineering, Heidelberg, Germany), 102 eyes of 102 patients were examined in pupil dilation, preoperatively and 6 times over 1-year follow-up. Preoperatively anterior hyaloid membrane (AHM) visibility was determined with Imaging App with high reliability. Postoperatively capsular bag–AHM distance was measured on six points by using Metrics App. Results: The AHM was visible in 18.6% preoperatively and postoperatively as well (Group 1), 49% of the preoperatively adherent AHMs became visible (Group 2A), 32.4% remained attached (Group 2B). Group 1: the average deepest point on the first day was 782.5 ± 324.1 microns, and it significantly differed from the later follow-up values. Group 2A: the average deepest value was 184.1 ± 220.1 microns, and there was no statistically significant difference between the postoperative visit values. The difference between the groups was statistically significant at every location and at each time point. Conclusions: AS-SS-OCT can be used to check BS both preoperatively (with limitations) and postoperatively.

Abstract:Zinc is now recognized to be one of the physiological elements, since it is essential for the growth of rats, and forms part of the carbonic anhydrase system [Shohl, 1939; Keilin & Mann, 1940]. Many foodstuffs contain comparable amounts of Fe and Zn, and the two metals tend to vary together. Thus white flour, egg-white and fish are poor sources of both, whereas brown flour, egg-yolk and meat are the reverse. There are, however, exceptions to this generalization. Milk contains three to five times as much Zn as Fe and the inside leaves of cabbage more Zn than the outer leaves [Sylvester & Hughes, 1936], whereas the reverse is true for Fe, and incidentally for Ca. Yeast, oysters and one or two exotic foods are very rich sources of Zn, but not necessarily of Fe. The metabolism of Zn has not been fully worked out, although the subject has attracted attention from time to time, mainly owing to the extensive use of Zn in industry. Some excellent pioneering work was done by R6st & Weitzel [1918-19], Bachelor, Fehnel, Thomson & Drinker [1926], Drinker, Fehnel & Marsh [1927], and an informative review of the subject was published by Lutz [1926]. It had been established by that date that the normal human intake of Zn was of the order of 10-15 mg./day and that most of this was excreted in the faeces. Drinker et al. [1927] studied the Zn metabolism of two subjects for a week, in the middle of which a meal very rich in Zn was consumed. The urinary excretions did not rise, but within the next 24 hr. much additional Zn was passed in the faeces. Within the past 10 years the treatment of diabetics with Zn protamine insulin has been introduced and gained considerable popularity. The quantities of Zn administered are small, but they are given by subcutaneous injection andwould amount to about 0.1 mg. of Zn per day in a patient who was receiving 50 units of Zn protamine insulin. The excretion of Zn after injection has been very little studied, although this seems the only way of finding out whether Zn is, or is not, freely excreted into the alimentary canal. In 1938, metabolism experiments on six normal people were undertaken to study the fate of injected Ca, Mg, Fe and Zn. The way in which these experiments were done, and the results for Ca, Mg and Fe, have already been published [McCance & Widdowson, 1938; 1939]. In 1940-41 metabolism experiments were carried out to investigate the effects of brown and white bread on Ca absorption [McCance & Widdowson, 1942 a]. These presented opportunities of making further experiments with Zn, and work on patients has also been possible from time to time. Some 45 Zn 'balances', each lasting for about a fortnight, have been carried out on twelve persons, and Zn has been injected intravenously into ten of them. Particulars about the subjects and the technical aspects of the experiments may be found in the papers to which reference has already been made. Zinc determinations were made on the ash extracts of foods, urines and faeces, the method used being that described by Keilin & Mann [1940].
