Abstract:AIM:To analyze the clinical risk factors for early variceal rebleeding after endoscopic variceal ligation (EVL). METHODS:342 cirrhotic patients with esophageal varices who received elective EVL to prevent bleeding or rebleeding at our endoscopy center between January 2005 and July 2010. were included in this study. The early rebleeding cases after EVL were confirmed by clinical signs or endoscopy. A case-control study was performed comparing the patients presenting with early rebleeding with those without this complication. RESULTS:The incidence of early rebleeding after EVL was 7.60%, and the morbidity of rebleeding was 26.9%. Stepwise multivariate logistic regression analysis showed that four variables were independent risk factors for early rebleeding: moderate to excessive ascites [odds ratio (OR) 62.83, 95% CI: 9.39-420.56, P < 0.001], the number of bands placed (OR 17.36, 95% CI: 4.00-75.34, P < 0.001), the extent of varices (OR 15.41, 95% CI: 2.84-83.52, P = 0.002) and prothrombin time (PT) > 18 s (OR 11.35, 95% CI: 1.93-66.70, P = 0.007). CONCLUSION:The early rebleeding rate after EVL is mainly affected by the volume of ascites, number of rubber bands used to ligate, severity of varices and prolonged PT. Effective measures for prevention and treatment should be adopted before and after EVL.

Abstract:Background and Aim Upper gastrointestinal bleeding is a rare and potentially life-threatening condition in children. Herein, clinical features and risk factors in children with upper gastrointestinal bleeding were analyzed, and a clinical scoring system was constructed to assess severity. Methods This retrospective cohort study involved 224 children hospitalized with upper gastrointestinal bleeding between January 2012 and April 2018. Demographic data, clinical information, and laboratory test results on admission were statistically examined. Results Out of 224 upper gastrointestinal bleeding cases, 76 were diagnosed as severe and 148 as mild cases according to the rate of blood loss and severity. Severe group was significantly different from mild group in 23 items including age, number of patients aged more than 7 years, and so forth (P P < 0.05). Positive detection rate of bleeding etiology was gradually decreased (P < 0.01) in relation to delay in timing of endoscopy. Analysis of logistic regression evinced five independent risk factors for severe cases to be associated with poor consciousness, hemoglobin < 80 g/L, hemoglobin drop of > 20 g/L, hematochezia, and anemic appearance (P P < 0.05). Using these five parameters, a number of scoring models were tested. The most predictive resulted in a scoring system constructed with a total of 16 and a cutoff for intervention of 8. Conclusions Amalgamation of risk factors with the scoring system plays an important role in assessing upper gastrointestinal bleeding severity in children.
Analysis of Risk Factors of Death in Patients with Esophagogastric Variceal Bleeding

Abstract:Background: Esophagogastric variceal bleeding(EVB) is a common complication of portal hypertension with a high rebleeding mortality.Aims: To define the risk factors of death in EVB patients during the first month of hospitalization.Methods: The clinical and laboratory data of 101 EVB patients from May 2009 to Jan.2010 were analyzed retrospectively.Multivariate Logistic regression analysis was conducted to screen the independent risk factors of death and Logistic regression model was constructed to predict the risk of death in EVB patients,and consistency check was conducted with the observed data.Results: The mortality of EVB patients during the first month of hospitalization was 38.6%.There were significant differences of hemoglobin,albumin,cholesterol,serum sodium,chlorine,potassium,white blood cell count,prothrombin time,bilirubin,ALT,urea nitrogen,creatinine,Child-Pugh score,amount of bleeding,ascites and hepatic encephalopathy between the death group and survival group(P<0.05).Multivariate Logistic regression analysis showed that serum sodium,white blood cell count and Child-Pugh score were independent risk factors of death in EVB patients.Sensitivity and specificity of the Logistic regression model for predicting patients' death were 69.2% and 93.5%,respectively,and the κ value was 0.653.Conclusions: Hyponatremia,white blood cell count and Child-Pugh score are risk factors of death in EVB patients during the first month of hospitalization,and the Logistic regression model constructed based on these parameters can well predict the risk of death of EVB patients.
Prognostic Factors of Patients with Acute Gastroesophageal Variceal Bleeding Caused by Liver Cirrhosis

Abstract:Objective To evaluate the prognostic factors of patients with liver cirrhosis,who were hospitalized for acute esophageal variceal bleeding(GEVB). Methods Retrospectively analyzed 175 patients(121 male and 54 female) with liver cirrhosis who were admitted to our hospital for acute GEVB from January 2000 to December 2006.The vital signs,laboratory results,regimen,Child-Pugh grade and MELD(model for end stage liver disease) score were recorded.In the patients,11 died at the hospital.Results Single factor Cox regression analysis showed that Child-Pugh grade,MELD score,bleeding frequency,WBC count,prothrombin time and international normalized ratio(INR) were risk factors for short-term prognosis(HR=8.956,1.020,3.449,1.112,1.134 and 2.359,respectively,P<0.05).Whereas,serum concentration of Na+ and albumin,and prothrombin time activity percentage were protective factors(HR=0.856,0.857 and 0.919,respectively,P<0.05).Conclusion Child-Pugh grade,MELD score,WBC count and serum concentration of Na+ are prognostic factors for hospital mortality of patients with liver cirrhosis-induced acute GEVB.

Abstract:BACKGROUND: Discrepancies exist in reported mortality rates of nonvariceal upper gastrointestinal bleeding (NVUGIB). OBJECTIVE: To perform a systematic review assessing possible reasons for these disparate findings and to more reliably compare them. METHODS: The MEDLINE, EMBASE and ISI Web of Knowledge databases were searched for studies reporting mortality rates in NVUGIB involving adults and published in English. To ensure robust and contemporary estimates, studies spanning 1996 to January 2011 that included more than 1000 patients were selected. RESULTS: Eighteen of 3077 studies were selected. Ten studies used administrative databases and the remaining eight used registries. The mortality rates reported in these studies ranged from 1.1% in Japan to 11% in Denmark. There were variations in reported mortality rates among countries and also within countries. Reasons for these disparities included a spectrum of quality in reporting as well as heterogeneous definitions of case ascertainment, differing patient populations with regard to severity of presentation and associated comorbidities, varying durations of follow-up and different health care system-related practices. CONCLUSIONS: Wide differences in reported NVUGIB mortality rates are attributable to differences in adopted methodologies and populations studied. More uniform standards in reporting are needed; only then can true observed variations enable a better understanding of causes of death and pave the way to improved patient outcomes.
Assessing the Predictive Factors for Bleeding in Esophageal Variceal Disease: A Systematic Review

Abstract:Esophageal varices, dilated submucosal distal esophageal veins, are a common source of upper gastrointestinal bleeding in patients with portal hypertension. This review aims to comprehensively assess predictive factors for both the first occurrence and subsequent risk of esophageal variceal bleeding. A systematic search was conducted in PubMed/MEDLINE (Medical Literature Analysis and Retrieval System Online) and Cochrane databases. A total of 33 studies were selected using rigorous inclusion and exclusion criteria. The risk of bias was assessed using the Newcastle-Ottawa Scale. Several predictive factors were identified for esophageal variceal bleeding, including the Child-Pugh score, Fibrosis Index, specific endoscopic findings, ultrasound parameters, portal vein diameter, presence and size of collaterals, CT scan findings, ascites, platelet counts, coagulation parameters, albumin levels, Von Willebrand Factor, bilirubin levels, diabetes mellitus, and the use of b-blocking agents in primary prophylaxis. The findings of this systematic review shed light on multiple potential predictive factors for esophageal variceal bleeding. Endoscopic findings were found to be reliable predictors. Additionally, ultrasound parameters showed associations with bleeding risk. This systematic review identifies multiple potential predictive factors for esophageal variceal bleeding in patients with portal hypertension. While certain factors exhibit strong predictive capabilities, further research is needed to refine and validate these findings, considering potential limitations and biases. This study serves as a critical resource for bridging knowledge gaps in this field.

Abstract:Aim of the study: Endoscopic variceal ligation (EVL) is important for emergency as well as prophylactic management of esophageal varices. Early bleeding after EVL is associated with significant morbidity and mortality. Assessing the likelihood of early post-EVL bleeding and its determinants can help deciding therapeutic strategies for high-risk patients. The aim of the present meta-analysis was to identify predictors of early bleeding after EVL.Material and methods: A comprehensive search of the literature was conducted from 2000 to November 2021 for studies evaluating the incidence, predictors and outcome of post-EVL bleeding. Pooled odds ratios (OR), mean difference (MD) and their 95% confidence intervals (CI) were calculated for prognostic variables.Results: A total of 16 studies with data on 13,378 patients were included in the meta-analysis. Among 34 parameters, 14 parameters were assessed for association with early bleeding after EVL. Lower hemoglobin at admission (MD = 1.11, 95% CI: -1.91 to -0.31), higher MELD score (MD = 2.00, 95% CI: 0.51-3.50), associated gastric varices (OR = 5.99, 95% CI: 1.06-33.90), higher number of bands (MD = 0.49, 95% CI: 0.02-0.97), and peptic esophagitis (OR = 11.38, 95% CI: 1.21-106.81) were significantly associated with increased risk of bleeding. However, there was significant heterogeneity among the studies with respect to all the analyzed parameters.Conclusions: Major predictors for early post-EVL bleeding in cirrhosis are admission hemoglobin level and MELD score, associated gastric varices, number of bands deployed during EVL, and peptic esophagitis on follow-up endoscopy. These risk factors may be useful for risk stratification after EVL in cirrhotics.

Abstract:Data on emergency endoscopic treatment following endotracheal intubation in patients with esophagogastric variceal bleeding (EGVB) remain limited. This retrospective study aimed to explore the efficacy and risk factors of bedside emergency endoscopic treatment following endotracheal intubation in severe EGVB patients admitted in Intensive Care Unit. A total of 165 EGVB patients were enrolled and allocated to training and validation sets in a randomly stratified manner. Univariate and multivariate logistic regression analyses were used to identify independent risk factors to construct nomograms for predicting the prognosis related to endoscopic hemostasis failure rate and 6-week mortality. In result, white blood cell counts ( p = 0.03), Child-Turcotte-Pugh (CTP) score ( p = 0.001) and comorbid shock ( p = 0.005) were selected as independent clinical predictors of endoscopic hemostasis failure. High CTP score ( p = 0.003) and the presence of gastric varices ( p = 0.009) were related to early rebleeding after emergency endoscopic treatment. Furthermore, the 6-week mortality was significantly associated with MELD scores ( p = 0.002), the presence of hepatic encephalopathy ( p = 0.045) and postoperative rebleeding ( p < 0.001). Finally, we developed practical nomograms to discern the risk of the emergency endoscopic hemostasis failure and 6-week mortality for EGVB patients. In conclusion, our study may help identify severe EGVB patients with higher hemostasis failure rate or 6-week mortality for earlier implementation of salvage treatments.

Abstract:We investigated, by microdialysis in various brain areas, the possibility that dopamine could be captured by the norepinephrine transporter when the dopamine transporter is pharmacologically blocked. Administration of reboxetine, a selective blocker of the norepinephrine transporter, 20 min after the administration of GBR 12909, a selective blocker of the dopamine transporter, produced an increase of dopamine output in the nucleus accumbes shell (+408% above basal) greater than that obtained by GBR 12909 alone (+308% above basal). On the contrary, reboxetine did not increase further the dopamine output produced by GBR 12909 in the nucleus accumbens core or in the dorsal caudate, areas lacking a consistent noradrenergic innervations. A cumulative effect of dopamine and norepinephrine transporter blockade on the output of dopamine in dialysates was also observed in the bed nucleus of stria terminalis and in the prefrontal cortex. This study shows that dopamine extracellular concentration can be elevated by norepinephrine transporter blockade, even in areas where the dopamine transporter is predominant, when the latter is pharmacologically blocked. This phenomenon may have relevance in psychostimulant dependence as well as in antidepressant pharmacology.

Abstract:Background Esophageal cancer is a highly malignant neoplasm with poor prognosis. Of its patients, upper gastrointestinal bleeding (UGIB) is one of the most challenging and threatening conditions in the emergency department (ED). However, no previous studies have analyzed the etiologies and clinical outcomes in this specific population. This study aimed to identify the clinical characteristics and risk factors for 30-day mortality in esophageal cancer patients with UGIB. Methods This retrospective cohort study enrolled 249 adult patients with esophageal cancer presenting with UGIB in the ED. Patients was divided into the survivor and non-survivor groups, and their demographic information, medical history, comorbidities, laboratory parameters, and clinical findings were recorded. The factors associated with 30-day mortality were identified using Cox’s proportional hazard model. Results Among the 249 patients in this study, 30-day mortality occurred in 47 patients (18.9%). The most common causes of UGIB were tumor ulcer (53.8%), followed by gastric/duodenal ulcer (14.5%), and arterial-esophageal fistula (AEF) (12.0%). Multivariate analyses indicated that underweight (HR = 2.02, p = 0.044), history of chronic kidney disease (HR = 6.39, p < 0.001), active bleeding (HR = 2.24, p = 0.039), AEF (HR = 2.23, p = 0.046), and metastatic lymph nodes (HR = 2.99, p = 0.021) were independent risk factors for 30-day mortality. Conclusions The most common cause of UGIB in esophageal cancer patients was tumor ulcer. AEF, accounting for 12% of UGIB in our study, is not an uncommon cause. Underweight, underlying chronic kidney disease, active bleeding, AEF, and tumor N stage > 0 were independent risk factors for 30-day mortality.

Abstract:Objective To study the prognostci factors of patients bleeding from large esophageal varices.Methods 41 patients bleeding from large esophageal varices were classified as getting better group(15 cases) and dead group(26 cases).Following data were collected and analysed:child-pugh score,laboratory parameters[prothrombin time(PT),coagutation factors,blood platelet count(BPC)],spleen sizes,age and disease course,frequency of hematemesis and interval time of bleeding.Results Significant distinction were found in PT,BPC,child-pugh score,age,interval time of bleeding between both groups.The P value was0.05,0.05,0.02,0.02,0.01 respeetively.Conclusion PT,child-pugh score,age were signifiantly positively associated with prognostic in bleeding from esophageal varices,BPC,interval time of bleeding were signifiantly negatively positively associated with prognostic in bleeding from esophageal varices.
Endoscopic Treatment of Gastroesophageal Varices: Efficacy and Safety in Children

Abstract:Purpose: Our purpose was to evaluate the efficacy and safety of endoscopic variceal band ligation (EVL) and Histoacryl injection for the treatment of gastroesophageal varices in children. Risk factors of rebleeding were analysed preliminarily. Methods: 15 children with esophageal varices and/or gastric varices who received EVL and/or Histoacryl injection were enrolled at our endoscopy center between October 2009 and March 2012. Demographic data, clinical characteristics and endoscopic data were registered and analysed as a Single-center prospective study. Results: The mean age was 9.5 +/- 3.3 years (2.5-14.3 years). The mean of clinical follow-up was 13.0 +/- 10.8 months (range 1-30 months). EVL and/or Histoacryl injection was successfully performed in all children. Immediate control of bleeding was achieved in all cases. The number of rubber bands placed in all patients ranged from 2 to 6. Early slippage of the rubber bands were not been found in the post-EVL rebleeding. Varices relapse with rebleeding was observed in 4 of 15 (26.7%) patients after a mean time of 14.8 +/- 6.5 weeks (5-24 weeks) follow-up. Patients with variceal rebleeding were retreated. Aggravated coagulation state, decreased platelets and ascites may be meaningful in predicting rebleeding. During follow-up, no fatal complication was observed in the patients. Conclusion: In children, the use of EVL and Histoacryl injection is safe and effective for the treatment of gastroesophageal varices. Effective measures for prevention and treatment of rebleeding should be adopted before and after operation.

Abstract:Abstract Background There has always been a debate on the optimal timing of endoscopy in patients with acute variceal bleeding (AVB). Objective This study aimed to examine the relation between the timing of endoscopy and the short-term outcomes of patients with AVB. Methods Patients with AVB who underwent endoscopy within 24 h after admission at our tertiary care center from 2014 to 2022 were evaluated retrospectively. The primary outcomes were the 6-week mortality and re-bleeding. The secondary outcomes included the total number of blood units transfused, the length of hospital stay, and the need for salvage therapy. We used Cox proportional hazards model to analyze the predictors of 6-week mortality in all patients as well as in those who were at high risk of further bleeding or death. Results A total of 312 patients were enrolled. Among them, 170 patients (54.49%) underwent urgent endoscopy (< 6 h), and 142 patients (45.51%) underwent early endoscopy (6–24 h). There were no significant differences between the urgent-endoscopy group and the early-endoscopy group, regarding the 6-week mortality (16.47% vs. 10.56%; P value = 0.132) and 6-week re-bleeding rate (11.2% vs. 16.2%; P value = 0.196). In multivariate analysis, time to endoscopy was independent of 6-week mortality (P value = 0.170), but the time between the beginning of bleeding and endoscopy (within 12 h) was significantly associated with low 6-week mortality (OR: 0.16; 95% CI: 0.06–0.46; P value = 0.001). Time to endoscopy was still not associated with 6-week mortality in patients at high risk for further bleeding or death (Glasgow-Blatchford score ≥ 12, n = 138, P value = 0.902). Conclusions Endoscopy performed within 6 h of admission, rather than within 6 to 24 h, did not improve six-week clinical outcomes in patients in stable condition with AVB and even those who were at high risk of further bleeding and death.
The Prognosis of Liver Cirrhotic Patients with Acute Esophageal Variceal Bleeding: a Cox Regression Analysis

Abstract:Objective To evaluate the risk factors for prognosis in liver cirrhotic patients with acute esophageal variceal bleeding (EVB). Methods Retrospective analysis was carried out in patients with liver cirrhosis,who were admitted to the hospital for EVB between January 2000 and December 2006 . All patients were follow-up to August 31st, 2009. The death of the patients was identified as the end of the study. The vital signs and results of laboratory tests were recorded when patients were admitted to the hospital. Child-Pugh score/grade, model for end stage liver disease (MELD) score and Rockall score were calculated. Results Ninty-five cases (66 males and 29 females) were enrolled in the study. Among them, 60 cases died and 35 cases survived for (42. 61±33.21) months (ranged from 2 days to 114 months). Univariate Cox regression analysis showed that age, male, Child-Pugh score/grade, MELD score, Rockall score, white blood cell counts and active hepatitis B were risk factors for death in liver cirrhotic patients with EVB (P＜0.05), while high level of hemoglobin and high hematocrit, endoscopic variceal sclerosis therapy or surgical treatment were protective factors (P＜0.05). Multivariate Cox regression analysis revealed that Child-Pugh grade (RR= 4. 997,95%CI:2. 787～8. 960, P＜0.01),Rockall score (RR= 1. 284,95%CI:1. 062～1. 553,P=0. 010), high counts of white blood cells (RR= 1. 072,95%CI: 1. 001 ～ 1. 148, P= 0. 046) were risk factors for prognosis of liver cirrhotic patients with EVB. Conclusion It is demonstrated that Child-Pugh grade,Rockall score and high counts of white blood cells are risk factors for prognosis of liver cirrhotic patients with EVB.

Abstract:Background: Acute variceal hemorrhage (AVH) is a frequent cause of upper gastrointestinal bleeding (UGIB) in liver cirrhosis. Most cases require urgent endoscopic intervention due to potentially life‐threatening courses. Different endoscopic hemostasis techniques can be used, in particular endoscopic variceal ligation (EVL) and endoscopic sclerotherapy (EST), depending on the bleeding side (esophageal, fundal, and gastric) as well as radiological interventions (e.g., embolization and transjugular intrahepatic portosystemic shunt [TIPS]). This study aimed to investigate trends in incidence, treatment modalities, and outcome parameters, such as in‐hospital mortality and adverse events in Germany. Methods: We evaluated the current epidemiological trends, therapeutic strategies, and in‐hospital mortality of AVH in Germany based on the standardized hospital discharge data provided by the German Federal Statistical Office from 2010 to 2019. Results: A total of 65,357 AVH cases, predominately males (68.3%), were included in the analysis. The annual incidence rate (hospitalization cases per 100,000 persons) was 8.9. The in‐hospital mortality was 18.6%. The most common underlying disease was alcohol‐related liver cirrhosis (60.6%). The most common clinical complication was bleeding anemia (60.1%), whereas hypovolemic shock (12.8%) was the less frequent. In esophageal variceal hemorrhage (EVH), EVL was the most frequently performed endoscopic therapy, while in gastric variceal hemorrhage (GVH), EST and fibrin glue injection were the most commonly performed therapies. EVL showed the lowest in‐hospital mortality (12.3%) in EVH, while EST showed favorable results (14% in‐hospital mortality) in GVH. Combination therapies overall showed a higher in‐hospital mortality and were more frequent in GVH. The presence of hypovolemic shock, AKI, sepsis, artificial ventilation, ARDS, bleeding anemia, hepatic encephalopathy, and male sex was associated with a significantly worse outcome. Conclusion: Our study provides detailed insight into the incidence, patient‐related risk factors, endoscopic treatment, and in‐hospital mortality in a sizeable AVH collective in Germany. These data might help improve risk stratification and treatment strategies for AVH patients in the future.
