Abstract:Treatment implant-associated infections remains a severe challenge in the clinical practice. This work focuses on the fabrication of Cu-containing porous TiO 2 coatings on titanium (Ti) by a combination of magnetron sputtering and dealloying techniques. Additionally, photothermal therapy is employed to enhance the effect of Cu ions in preventing bacterial infection. After the dealloying, most of Cu element was removed from the magnetron sputtered Cu-containing films, and porous TiO 2 coatings were prepared on Ti. The formation of porous nanostructures significantly enhanced the photothermal conversion performance under NIR-II light irradiation. The combined effect of hyperthermia and Cu ions demonstrated enhanced antibacterial activity in both in vitro and in vivo experiments, and the antibacterial efficiency can reach 99% against Streptococcus mutans. Moreover, the porous TiO 2 coatings also exhibited excellent biocompatibility. This modification of the titanium surface structure through dealloying changes may offer a novel approach to enhance the antimicrobial properties of titanium implants.

Nanostructured Cu-doped TiO2 with photothermal effect for prevention of implant-associated infection

Abstract:Bacterial infection of titanium (Ti) dental implants is still a major clinical complication. In this study, a combination of copper (Cu) ions and photothermal therapy is used to combat implant-associated infection. Cu doped TiO2 (TiO2-Cu) films were prepared on Ti by magnetron sputtering and subsequently annealing. TiO2-Cu films had efficient photothermal conversion ability due to the generated nanostructure during the annealing process. Under the irradiation of 808 near infrared light, the combined actions of hyperthermia and Cu ions gave rise to excellent antibacterial activity against Streptococcus mutans on Ti as demonstrated by the experiments conducted in vitro and in vivo. The TiO2-Cu films also exhibited excellent biocompatibility. In addition, the surface hardness and corrosion resistance of TiO2-Cu films were greatly improved.
Rapid Photo-Sonotherapy for Clinical Treatment of Bacterial Infected Bone Implants by Creating Oxygen Deficiency Using Sulfur Doping.

Abstract:Periprosthetic infection is considered the main cause of implant failure, which is expected to be solved by fabricating an antibacterial coating on the surface of the implant. Nevertheless, systemic antibiotic treatment still represents the mainstream method for preventing infection, and few antibacterial coatings are applied clinically. This is because the externally introduced traditional antibacterial coatings suffer from the risk of invalidation and tissue toxicity induced by the consumption of antibacterial agents, degradation, and shedding. In this work, we proposed a rapid photo-sonotherapy by creating an oxygen deficiency on a titanium (Ti) implant through sulfur (S)-doping (Ti-S-TiO2-x), which endowed the implants with great sonodynamic and photothermal ability. Without introducing an external antibacterial coating, it reached a high antibacterial efficiency of 99.995% against Staphylococcus aureus under 15 min near-infrared light and ultrasound treatments. Furthermore, bone infection was successfully treated after combination treatments, and improved osseointegration was observed. Importantly, the S-doped Ti implant immersed in water for 6 months showed an unchanged structure and properties, suggesting that the Ti implant with intrinsic modification showed stable antibacterial performance under exogenous stimuli with a high antibacterial performance in vivo. This photo-sonotherapy based on sulfur doping is also promising for cancer therapy with biosafety.
Rapid Biofilm Elimination on Bone Implants Using Near-Infrared-Activated Inorganic Semiconductor Heterostructures.

Abstract:Bacterial infections often cause orthopedic surgery failures. It is hard for the immune system and antibiotics to clear bacteria adhered to implants after they form a mature biofilm, and a secondary surgery is required to remove the infected implants. To avoid this, a hybrid coating of Bi2 S3 @Ag3 PO4 /Ti is prepared to eliminate biofilm using near-infrared (NIR) light. Bi2 S3 nanorod (NR) arrays are prepared on titanium (Ti) implants through hydrothermal methods, and Ag3 PO4 nanoparticles (NPs) are loaded on Bi2 S3 NR arrays using a stepwise electrostatic adsorption strategy. The introduction of Ag3 PO4 NPs enhances the photocatalysis performances of Bi2 S3 , and the hybrid coating also exhibits good photothermal effects. After 808 nm light irradiation for 15 min, it shows superior bactericidal efficiency of 99.45% against Staphylococcus aureus, 99.74% against Escherichia coli in vitro, and 94.54% against S. aureus biofilm in vivo. Bi2 S3 @Ag3 PO4 /Ti also shows good cell viability compared to pure Ti. This NIR-activated-inorganic hybrid semiconductor heterojunction coating is biocompatible and could be employed to eliminate biofilm effectively, which makes it a very promising strategy for the surface modification of bone implant materials.
Surface modification of titanium substrate via combining photothermal therapy and quorum-sensing-inhibition strategy for improving osseointegration and treating biofilm-associated bacterial infection

Abstract:Insufficient osseointegration and biofilm-associated bacterial infection are important challenges for clinical application of titanium (Ti)-based implants. Here, we constructed mesoporous polydopamine (MPDA) nanoparticles (NPs) loaded with luteolin (LUT, a quorum sensing inhibitor), which were further coated with the shell of calcium phosphate (CaP) to construct MPDA-LUT@CaP nanosystem. Then, MPDA-LUT@CaP NPs were immobilized on the surface of Ti implants. Under acidic environment of bacterial biofilm-infection, the CaP shell of MPDA-LUT@CaP NPs was rapidly degraded and released LUT, Ca2+ and PO4 3- from the surface of Ti implant. LUT could effectively inhibit and disperse biofilm. Furthermore, under near-infrared irradiation (NIR), the thermotherapy induced by the photothermal conversion effect of MPDA destroyed the integrity of the bacterial membrane, and synergistically led to protein leakage and a decrease in ATP levels. Combined with photothermal therapy (PTT) and quorum-sensing-inhibition strategy, the surface-functionalized Ti substrate had an antibacterial rate of over 95.59% against Staphylococcus aureus and the elimination rate of the formed biofilm was as high as 90.3%, so as to achieve low temperature and efficient treatment of bacterial biofilm infection. More importantly, the modified Ti implant accelerated the growth of cell and the healing process of bone tissue due to the released Ca2+ and PO4 3-. In summary, this work combined PTT with quorum-sensing-inhibition strategy provides a new idea for surface functionalization of implant for achieving effective antibacterial and osseointegration capabilities.

Abstract:Titanium (Ti) and its alloys have good biocompatibility, mechanical properties and corrosion resistance, making them attractive for biomedical applications. However, their biological inertness and lack of antimicrobial properties may compromise the success of implants. In this review, the potential of micro-arc oxidation (MAO) technology to create bioactive coatings on Ti implants is discussed. The review covers the following aspects: 1) different factors, such as electrolyte, voltage and current, affect the properties of MAO coatings; 2) MAO coatings affect biocompatibility, including cytocompatibility, hemocompatibility, angiogenic activity, corrosion resistance, osteogenic activity and osseointegration; 3) antibacterial properties can be achieved by adding copper (Cu), silver (Ag), zinc (Zn) and other elements to achieve antimicrobial properties; and 4) MAO can be combined with other physical and chemical techniques to enhance the performance of MAO coatings. It is concluded that MAO coatings offer new opportunities for improving the use of Ti and its alloys in biomedical applications, and some suggestions for future research are provided.
Electrophoretic Deposited Stable Chitosan@MoS2 Coating with Rapid In Situ Bacteria-Killing Ability under Dual-Light Irradiation.

Abstract:Developing in situ disinfection methods in vivo to avoid drug-resistant bacteria and tissue toxicity is an urgent need. Here, the photodynamic and photothermal properties of the chitosan-assisted MoS2 (CS@MoS2 ) hybrid coating are simultaneously inspired to endow metallic Ti implants with excellent surface self-antibacterial capabilities. This coating, irradiated by only 660 nm visible light (VL) for 10 min, exhibits an antibacterial efficacy of 91.58% and 92.52% against Escherichia coli (E. coli) and Staphylococcus aureus (S. aureus), respectively. The corresponding value is 64.67% and 57.44%, respectively, after irradiation by a single 808 nm near infrared light for the same amount of time. However, the combined irradiation using both lights can significantly enhance the efficiency up to 99.84% and 99.65% against E. coli and S. aureus, respectively, which can be ascribed to the synergistic effects of photodynamic and photothermal actions. The former produces single oxygen species under 660 nm VL while the latter induces a rise in temperature of implants, which can inhibit the growth of both E. coli and S. aureus. The introduction of CS can also promote the biocompatibility of implants, which provides a facile, rapid, and safe in situ bacteria-killing method in vivo without needing a second surgery.
Highly Effective and Noninvasive Near‐Infrared Eradication of a Staphylococcus aureus Biofilm on Implants by a Photoresponsive Coating within 20 Min

Abstract:Biofilms have been related to the persistence of infections on medical implants, and these cannot be eradicated because of the resistance of biofilm structures. Therefore, a biocompatible phototherapeutic system is developed composed of MoS2, IR780 photosensitizer, and arginine–glycine–aspartic acid–cysteine (RGDC) to safely eradicate biofilms on titanium implants within 20 min. The magnetron‐sputtered MoS2 film possesses excellent photothermal properties, and IR780 can produce reactive oxygen species (ROS) with the irradiation of near‐infrared (NIR, λ = 700–1100 nm) light. Consequently, the combination of photothermal therapy (PTT) and photodynamic therapy (PDT), assisted by glutathione oxidation accelerated by NIR light, can provide synergistic and rapid killing of bacteria, i.e., 98.99 ± 0.42% eradication ratio against a Staphylococcus aureus biofilm in vivo within 20 min, which is much greater than that of PTT or PDT alone. With the assistance of ROS, the permeability of damaged bacterial membranes increases, and the damaged bacterial membranes become more sensitive to heat, thus accelerating the leakage of proteins from the bacteria. In addition, RGDC can provide excellent biosafety and osteoconductivity, which is confirmed by in vivo animal experiments.

Abstract:Over 90 different mutations in the gene encoding copper/zinc superoxide dismutase (SOD1) cause ∼2% of amyotrophic lateral sclerosis (ALS) cases by an unknown mechanism. We engineered 14 different human ALS-related SOD1 mutants and obtained high yields of biologically metallated proteins from an Sf21 insect cell expression system. Both the wild type and mutant “as isolated” SOD1 variants were deficient in copper and were heterogeneous by native gel electrophoresis. By contrast, although three mutant SOD1s with substitutions near the metal binding sites (H46R, G85R, and D124V) were severely deficient in both copper and zinc ions, zinc deficiency was not a consistent feature shared by the as isolated mutants. Eight mutants (A4V, L38V, G41S, G72S, D76Y, D90A, G93A, and E133Δ) exhibited normal SOD activity over pH 5.5–10.5, per equivalent of copper, consistent with the presumption that bound copper was in the proper metal-binding site and was fully active. The H48Q variant contained a high copper content yet was 100-fold less active than the wild type enzyme and exhibited a blue shift in the visible absorbance peak of bound Cu(II), indicating rearrangement of the Cu(II) coordination geometry. Further characterization of these as-isolated SOD1 proteins may provide new insights regarding mutant SOD1 enzyme toxicity in ALS.
Modulating antibacterial properties of titanium surface coatings: The role of low intensity pulsed ultrasound and light stimuli

Abstract:Medical titanium and titanium alloys were susceptible to infection following implantation due to their limited antibacterial properties. One effective approach to mitigate this risk was the application of antibacterial coatings on the medical titanium materials surface. Additionally, there was a pressing need in clinical practice to develop a titanium surface coating that can modulate antibacterial properties in response to external conditions. In this study, a zinc oxide-loaded silver nanorod arrays coating was fabricated on the titanium surface using intensity pulsed ultrasound and light as stimuli, aiming to achieve the desired regulation of antibacterial properties. The coating morphology and the silver distribution within the coating were investigated. The surface roughness and wetting angle were tested. The effect of low intensity pulsed ultrasound and light on the cumulative release of silver ions was studied, and its mechanism was analyzed. The bacteriostasis rate of the polarized ZnO-loaded silver nanorods array coating reached 99.96%. The synergistic bactericidal mechanism of piezoelectric effect, catalysis, silver ion bactericidal effect and nanorod puncture effect on bacteria was expounded. Finally, the ZnO-loaded silver nanorod array coating exhibited non-cytotoxicity and supported full osteoblast adherence morphology. The polarized coating was observed to be coated with abundant granular hydroxyapatite after immersion in simulated body fluids for 7 days. The results provide a possibility for modulating the antibacterial properties of titanium surface coatings in vitro.

Abstract:Osteomyelitis is a severe bone infection with a high rate of relapse. Given the fact that occurrence/relapse of osteomyelitis is associated with the presence of foreign bodies, it is highly desired to develop orthopedic implant with long-term anti-infection and anti-inflammation properties. In current work, zinc oxide-selenium (ZnO-Se) composite coatings were fabricated on titanium surface via micro-arc oxidation (MAO) processing purposed to generate integrated functional surface for osteomyelitis control. On one hand, the combination of ZnO and Se could inhibit the adherence of S. aureus, which accounts for majority of osteomyelitis occurrence. On the other hand, the anti-oxidation significance of Se was purposed to reduce inflammation severity. The subsequent material characterizations, DPPH radicals scavenge test, cell culture and anti-bacterial test confirmed the potential of ZnO-Se MAO coating in osteomyelitis control, thereby providing a facile option for titanium implant surface modification.
Rapid and Highly Effective Noninvasive Disinfection by Hybrid Ag/CS@MnO 2 Nanosheets Using Near-Infrared Light.

Abstract:The bacterial infection on the surface of medical apparatus and instruments as well as artificial implants is threatening the human health greatly. Antibiotics and traditional bacterial-killing agents even silver nanoparticles can induce bacterial resistance during long-term interaction with bacteria. Hence, rapid surface sterilization and long-term prevention of bacterial infection are urgent for biomedical devices especially for artificial implant materials. Herein, a hybrid coating composed of chitosan (CS), silver nanoparticles (AgNPs) and MnO2 nanosheets was constructed on the surface of titanium plates, which can endow the metallic implants with rapid and highly effective antibacterial efficacy of 99.00% and 99.25% against Staphylococcus aureus (S. aureus) and Escherichia coli (E. coli), respectively, within 20 min of 808 nm near-infrared light (NIR) irradiation. The exogenous NIR irradiation can trigger the MnO2 nanosheets to produce enough hyperthermia within 10 minutes, which can combine with low concentration of pre-released Ag ions from the hybrid coating to achieve superior antimicrobial efficacy through synergistic effects. As a contrast, either pre-released Ag ions or photothermal effect alone can achieve much lower antibacterial efficiency under the same concentration, i.e., 24.00% and 30.01% for the former while 30.00% and 42.54% for the later against S. aureus and E. coli, respectively. The possible cytotoxicity of coatings could be eliminated owing to the low concentration of AgNPs and chitosan encapsulation. Thus, the novel bifunctional coating Ag/CS@MnO2 can exhibit great potential in deep site disinfection of Ti implants through the synergy of pre-released Ag ions and photothermal effect within a short time.
Photoelectrons Sequentially Regulate Antibacterial Activity and Osseointegration of Titanium Implants

Abstract:Abstract Titanium implants are widely used ; however, implantation occasionally fails due to infections during the surgery or poor osseointegration after the surgery. To solve the problem, an intelligent functional surface on titanium implant that can sequentially eradicate bacteria biofilm at the initial period and promote osseointegration at the late period of post‐surgery time is designed. Such surfaces can be excited by near infrared light (NIR), with rare earth nanoparticles to upconvert the NIR light to visible range and adsorb by Au nanoparticles, supported by titanium oxide porous film on titanium implants. Under NIR irradiation, the implant converts the energy of phonon to hot electrons and lattice vibrations, while the former flows directly to the contact substance or partially reacts with the surrounding to generate reactive oxygen species, and the latter leads to the local temperature increase. The biofilm or microbes on the implant surface can be eradicated by NIR treatment in vitro and in vivo. Additionally, the surface exhibits superior biocompatibility for cell survival, adhesion, proliferation, and osteogenic differentiation, which provides the foundation for osseointegration. In vivo implantation experiments demonstrate osseointegration is also promoted. This work thus demonstrates NIR‐generated electrons can sequentially eradicate biofilms and regulate the osteogenic process, providing new solutions to fabricate efficient implant surfaces.

Abstract:The implant materials with both osteogenic and anti-bacterial properties are promising for orthopedic and dental applications. Moreover, the inflammatory response induced by biomaterials has been recently recognized as one of the critical factors in determining implantation fate. A new generation of implant materials should have modulatory effects on the local inflammatory environment such that it favors osteogenesis and osteointegration instead of being bio-inert. In this study, the micro-arc oxidation (MAO) technique was employed to fabricate Cucontaining ceramic coatings on titanium substrates. The macrophages cultured on Cu-containing MAO-fabricated surfaces were polarized to M1 phenotype, evidenced by the high expression levels of inducible nitric oxide synthase (iNOS), low expression levels of arginasel (Arg1), enhanced pro-inflammatory cytokine interleukin-6 (IL-6) release and inhibited IL-4 and IL-10 (anti-inflammatory cytokines) release. The MAO-treated surface incorporated with larger amounts of Cu (referred as Cu(h)-MAO) could modulate a favorable inflammatory microenvironment for osteoblast-like cell differentiation. Moreover, the macrophages cultured on Cu(h)-MAO surface exhibited enhanced bacteria uptake and killing rate, indicating that the Cu(h)-MAO surface promoted the bactericidal capacity of macrophages. Together, Cu could be used as a promising modulatory agent for macrophage functions. The integration of Cu in biomaterials could lead to enhanced macrophage-mediated osteogenesis and bactericidal capacity.
Functional Titanium Substrates Synergetic Photothermal Therapy for Enhanced Antibacterial and Osteogenic Performance via Immunity Regulation

Abstract:Implant-associated infections (IAIs) significantly impair the integration between titanium (Ti) implants and bone tissues. Bacteria colonized on the surface of the implant can induce innate immune suppression of the host to resist clearance. Herein, an interfacial functionalization strategy is employed to introduce FeIII TA nanoparticles (NPs) and acetyl Bletilla striata polysaccharide (acBSP) on the Ti substrate to obtain the Ti-TF-acBSP system. Under near-infrared (NIR) irradiation, the hyperthermal effect induced by FeIII TA NPs directly killed bacteria. Meanwhile, macrophages are induced by acBSP to polarize into pro-inflammatory M1 phenotype, which enhanced the phagocytosis ability of macrophages and activated host innate immunity. Moreover, the asBSP instructed macrophages to secrete pro-osteogenic cytokine, which promoted osteogenic differentiation of MSCs. The results of the animal experiment in vivo confirmed that the Ti-TF-acBSP implant effectively eliminated bacterial infection under NIR irradiation, enhanced the expression of pro-inflammatory cytokine, and induced the production of bone-forming related factors. In a word, the functionalized Ti implant not only have a direct bactericidal effect but also regulate macrophage polarization as well as macrophage-mediated bactericidal and osteogenic effect. The strategy of combining photothermal therapy with immunoregulation will present a potential candidate for the development of novel antibacterial orthopedic devices.
A Self-Activating Electron Transfer Antibacterial Strategy: Co3O4/TiO2 P-N Heterojunctions Combined with Photothermal Therapy.

Abstract:Implant-associated infections are significant impediments to successful surgical outcomes, often resulting from persistent bacterial contamination. It has been hypothesized that bacteria can transfer electrons to semiconductors with comparable potential to the biological redox potential (BRP). Building on this concept, we developed an antibiotic-free bactericidal system, Co3O4/TiO2-Ti, capable of achieving real-time and sustainable bactericidal effects. Our study demonstrated that Co3O4/TiO2-Ti, possessing an appropriately set valence band, initiated charge transfer, reactive oxygen species (ROS) production, and membrane damage in adherent Staphylococcus aureus (S. aureus). Notably, in vivo experiments illustrated the remarkable antibacterial activity of Co3O4/TiO2-Ti, while promoting soft-tissue reconstruction and demonstrating excellent cytocompatibility. Transcriptomic analysis further revealed a down-regulation of aerobic respiration-associated genes and an up-regulation of ROS-associated genes in S. aureus in the presence of Co3O4/TiO2-Ti compared to Ti. Kyoto Encyclopedia of Genes and Genomes (KEGG) analysis and gene set enrichment analysis (GSEA) identified alterations in respiratory metabolism, oxidative phosphorylation, and the synthesis of amino acid in S. aureus cultured on Co3O4/TiO2-Ti. Furthermore, when combined with near-infrared (NIR) irradiation and photothermal therapy (PTT), Co3O4/TiO2-Ti eliminated 95.71% of floating and adherent S. aureus in vitro. The findings suggest that this antibiotic-free strategy holds substantial promise in enhancing implant sterilization capabilities, thereby contributing to the prevention and treatment of bacterial infections through bandgap engineering of implants and NIR irradiation.
Immunomodulation and osteointegration of infected implants by ion-riched and hierarchical porous TiO2 matrix

Abstract:Infection is a main cause of implant failure. The microenvironment cell experiencing after antimicrobial therapy of infected implants is very crucial for immunomodulation and subsequent osteointegration. Based on the superiority of photothermal therapy in anti-infection, a matrix of nanograined TiO2 with (Si/P)-dual ions and hierarchical nano/microporous topography is developed on Ti implant by micro-arc oxidation. It shows good photothermal properties due to the lattice distortion induced by ion doping and can kill bacteria with near-infrared (NIR) light irradiation efficiently. Due to the combined action of porous topography and release of Si/P ions, the TiO2 matrix achieves M1-to-M2 phenotype switch of macrophages timely at different simulating infection stage and accelerates recruitment, proliferation and osteogenic differentiation of bone marrow mesenchymal stem cells (BMSCs) in vitro. In vivo, the TiO2 matrix also shows desired antibacterial performance after photothermal therapy and exhibits outstanding immunomodulatory ability and new bone formation efficiency. This study provides a promising TiO2 matrix applied in clinic for settling implant infection by NIR light irradiation and promoting bone regeneration through osteoimmunomodulation.

Abstract:Lacking osseointegration and peri-implantitis induced by bacterial infiltration are the pivotal issues for the long-term clinical success of implants. In order to improve the bioactivity and antibacterial properties of implant materials, volcano-shaped microporous TiO2 coatings doped with Zinc (Zn) were fabricated via a micro-arc oxidation (MAO) method on pure titanium (Ti). The microstructure, morphology, and chemical composition of the Zn-doped coatings were systematically studied. In cell culture tests, the formed coatings promoted the adhesion and proliferation of bone mesenchymal stem cells (BMSCs), exhibiting good biocompatibility. The antibacterial experiments revealed that Zn-TiO2 coatings possess excellent antibacterial properties against Staphylococcus aureus (S. aureus) and Porphyromonas gingivalis (P. gingivalis).

Abstract:Titanium (Ti) and its alloys are widely recognized as preferred materials for bone implants due to their superior mechanical properties. However, their natural surface bio-inertness can hinder effective tissue integration. To address this challenge, micro-arc oxidation (MAO) has emerged as an innovative electrochemical surface modification technique. Its benefits range from operational simplicity and cost-effectiveness to environmental compatibility and scalability. Furthermore, the distinctive MAO process yields a porous topography that bestows versatile functionalities for biological applications, encompassing osteogenesis, antibacterial, and anti-inflammatory properties. In this review, we undertake an examination of the underlying mechanism governing the MAO process, scrutinize the multifaceted influence of various factors on coating performance, conduct an extensive analysis of the development of diverse biological functionalities conferred by MAO coatings, and discuss the practical application of MAO in implants. Finally, we provide insights into the limitations and potential pathways for further development of this technology in the field of bone implantation.

Abstract:The bacterial colonization and poor osseointegration of Ti implants significantly compromise their applications in load-bearing bone repair and replacement. To endorse the Ti with both excellent bioactivity and antibacterial ability, we developed a microarc oxidation coating that was modified uniformly by hydroxyapatite (HA) nanodots arrays and loaded regionally with chitosan hydrogel containing ciprofloxacin. The bonding between the HA nanodots covered coating and the chitosan hydrogel is further enhanced via silanization and chemical grafting of glutaraldehyde. Benefiting from the regionally loaded structure of the chitosan hydrogel, the chitosan hydrogel unloaded area can promote the cell adhesion and proliferation with excellent bioactivity, though relatively low OD value of cck8 has been observed at the beginning of the cell culturing. Whereas, the OD value of cck8 rises with the prolongation of the cell culturing time due to the degradation of the regionally loaded chitosan hydrogel. With the help of the laden ciprofloxacin in chitosan hydrogels, the sample effectively sterilizes the bacterial with a bacteriostatic ring. Therefore, regional loading of chitosan hydrogel containing ciprofloxacin on the modified microarc oxidation coating is a good approach to endorse Ti with both excellent bioactivity and antibacterial ability.
