Abstract:Background: Acrylamide-induced immunotoxicity and allergic dermatitis have been reported in animal experiments and clinical reports, respectively. However, epidemiological evidence from the general population is limited.Objectives: The purpose of the present study was to estimate the associations between acrylamide exposure and allergy-related outcomes in the general US population.Methods: A total of 6982 subjects were selected from the National Health and Nutrition Examination Survey 2005-2006 (NHANES). Internal exposure was measured by the hemoglobin adducts of acrylamide (HbAA) and its metabolite glycidamide (HbGA). Allergy-related outcomes including asthma, hay fever, allergy, itchy rash, sneeze, wheeze and eczema were obtained by self-administered questionnaires. Allergic sensitization was assessed by the total immunoglobulin E (IgE) levels. The associations of HbAA and HbGA quartiles with allergy-related outcomes were calculated using logistic regression models with multivariable adjustments. Analyses were additionally stratified according to age, gender and serum cotinine levels.Results: When setting quartile 1 of HbAA as reference, the odds ratios (ORs) [95% confidence intervals (CIs)] of quartile 2 to 4 for eczema were 1.18 (0.79-1.76), 1.14 (0.73-1.78) and 1.58 (1.14-2.18), respectively (P (trend) = 0.002). Individuals at the highest quartile of HbGA had significantly elevated likelihoods of itchy rash (OR = 1.37, 95% CI =1.02-1.83, p (trend) = 0.032) and eczema (OR = 1.45, 95% CI = 1.06-1.97, p (trend) = 0.044). The stratification analyses indicated various results in different subgroups.Conclusions: This study indicated significant associations between HbAA and HbGA levels and the likelihoods of allergy-related outcomes in the general US population, depending on age, gender and smoke exposure status. Abstract:BACKGROUND Assessment of IgE-mediated sensitization to flour allergens is widely used to investigate flour-induced occupational asthma. The diagnostic efficiency of detecting specific IgE antibodies (sIgE) against wheat and rye flour, however, has not been thoroughly compared to other diagnostic procedures. OBJECTIVE We sought to evaluate the diagnostic accuracy of sIgE against wheat and rye compared to specific inhalation challenge (SIC) with flour as the reference standard. METHODS This retrospective multicenter study included 264 subjects who completed a SIC with flour in eight tertiary centers, of whom 205 subjects showed a positive SIC result. RESULTS Compared with SIC , sIgE levels ≥0.35 kU A /L against wheat and rye provided similar sensitivities (84%-85%, respectively), specificities (71%-78%), positive predictive values (91%-93%), and negative predictive value (56%-61%). Increasing the threshold sIgE value to 5.10 kU A /L for wheat and to 6.20 kU A /L for rye provided a specificity ≥95% and further enhanced the positive predictive value to 98%. Among subjects with a positive SIC, those who failed to demonstrate sIgE against wheat and rye (n=26) had significantly lower total serum IgE level and blood and sputum eosinophil counts, and a lesser increase in post-challenge fractional exhaled nitric oxide as compared to the subjects with detectable sIgE. CONCLUSION High levels of sIgE against wheat and/or rye flour strongly support a diagnosis of flour-induced occupational asthma without the need for performing a SIC. The absence of detectable sIgE against wheat and rye in subjects with a positive SIC seems to be associated with lower levels of T H 2 biomarkers.

Abstract:Asthma in bakers and bakery workers is generally referred to as “baker's asthma.” Although a useful shorthand, this terminology belies the observation that asthma caused by the same agents occurs in workers in related occupations such as flour milling, other food production, and in industries that produce and package enzymes and other ingredients for the baking sector. Asthma among bakers and millers is arguably the most familiar and, notoriously, the oldest form of occupational asthma (OA). Most studies indicate that wheat (Triticum aestivum) flour proteins are allergens for 60%–70% of symptomatic bakers, although other cereals like rye (Secale cereale), barley (Hordeum vulgare), oats (Avena sativa), and corn, and noncereal sources, enzymes, and insects may be involved because bakeries are complex environments. Using a model similar for all chapters on specific agents causing OA, the following topics are covered in this chapter: Allergens, nature and source: In this area, considerable progress has been reached using recombinant allergens. The soybean allergens involved in baker's asthma are predominantly high-molecular-weight (HMW) proteins present both in soybean hull and flour. The bakery environment is complex and there are several case reports of baker's asthma caused by molds and insects, although these are of marginal importance to the burden of disease in bakers; nonetheless they should be kept in mind in clinical setting if no sensitization to common bakery allergens is found. Using simulation techniques based on cross-sectional data to examine effectiveness of several strategies to reduce burden of baker's asthma, the most successful approach was one of rigorous health surveillance identifying sensitized or symptomatic workers.
Glucoamylase: a current allergen in the baking industry

Abstract:Background:Over a 10 year period a decline in the rate of sensitizations to α-amylase (Aspergillus oryzae) was observed in bakers investigated for allergic obstructive airway disease. At the same time, glucoamylase (Aspergillus niger) was identified as the currently the most relevant allergen in sensitizations to enzymes in the baking industry. The aim of the present study was to investigate whether, over a period of 10 years and in the case of new-onset disease, there had been any change in sensitization and exposure rates to enzymes used in the baking industry.Methods:Total immunoglobulin-E (IgE) levels and specific IgE to baking enzymes were determined in 433 bakers investigated in the Baker‘s Asthma prevention program (Bäckerasthma Präventionsprogramm, BAP) of the German Social Accident Insurance Institution for the foodstuffs and catering industry (Berufsgenossenschaft Nahrungsmittel und Gastgewerbe, BGN). At the same time personal dust exposure, including assessment of the level of α-amylase exposure in the area of exposure, was recorded.Results and conclusions:Serological investigations revealed a significant decline in the rate of sensitization to α-amylase from 26 % to 13 %. At 28 %, the rate of sensitization to the baking enzyme glucoamylase was significantly higher than to cellulase (16 %) and α-amylase among subjects in 2010. Multiple sensitizations to all three baking agents are common. In total, 30 % of affected bakers are currently sensitized to at least one of the baking enzymes investigated. Data from individual dust measurements revealed a decline in α-amylase exposure while overall dust exposure remained almost unchanged. Today, 11 % fewer bakers are exposed to α-amylase compared with ten years previously and, at the same time, enzyme concentrations in exposed bakers have dropped significantly. The high sensitization rate to glucoamylase in affected bakers gives cause to investigate exposure levels in bakeries and to assess sensitizations in the context of occupational disease proceedings.
CD08 ‘If it’s not the hair maybe it’s in the air?’ A case of allergic contact dermatitis from airborne exposure to acrylates

Abstract:Abstract A 38-year-old auditor was referred for patch testing for a suspected hair dye reaction following development of a florid eczematous reaction on her face after a hair dye appointment. This reaction predominantly affected her cheeks, with the left more so than the right, extending to just below her eye. She also had mild eye swelling. The description and photographs taken at the time were atypical for a hair dye reaction, and on further questioning she revealed she had been sat in close proximity to someone undergoing acrylic nail treatment during the aforementioned hair appointment. We undertook patch testing to our baseline, anti-infective, excipient, hairdressing, fragrance, acrylic nail, cosmetic, photopatch and perianal series. Positive results were observed for 14 acrylates found in acrylic nails, namely methyl methacrylate, 2-hydroxypropyl methacrylate, ethylene glycol dimethacrylate, triethylene glycol dimethacrylate, urethane dimethacrylate, bisphenol A dimethacrylate 2%, bisphenol A glycidyl dimethacrylate 2%, 1,6-hexanediol diacrylate, tetrahydrofurfuryl methacrylate, tetraethylene glycol dimethacrylate, butyl acrylate, ethyl acrylate, 2-hydroxyethyl acrylate, and triethylene glycol diacrylate. No reactions were observed to any hair dyes on patch testing. Skin prick testing for para-phenylenediamine (PPD) was also negative, excluding a type 1 allergy to PPD. She reported prior but infrequent use of acrylic nails, the last instance being over a year ago. It is therefore likely that the patient became sensitized through these exposures, leading to a pronounced reaction following subsequent airborne exposure to acrylates. An increasing incidence of allergic contact dermatitis (ACD) to acrylates has been noted, particularly among nail technicians and consumers. Although airborne contact dermatitis to acrylates is rare, it has been reported in occupational settings involving nail technicians and dental nurses, and in manufacturing. Facial dermatitis or dermatitis in exposed areas without direct allergen contact has been documented. Additionally, four cases of respiratory sensitization with acrylate-induced occupational asthma have been reported in conjunction with ACD. Respiratory problems were often alleviated with mask usage, but airborne contact dermatitis posed greater challenges, prompting job changes. Efforts to collect sensitizers using air pumps for vapours and filters for airborne aerosols have seen limited success. This unusual case underscores the diverse clinical presentations of acrylate sensitivity and emphasizes the importance of promptly detecting the allergen to prevent further exposure. Awareness of airborne allergic contact dermatitis to acrylates is useful in occupational disease reviews. Additionally, efforts should be made to minimize airborne methacrylate concentrations to reduce sensitization risk.

Mapping rebound effects from sustainable behaviours: key concepts and literature review

Abstract:2 The Sustainable Lifestyles Research Group (SLRG) is a multi-centre research network collaborating to further understanding of the opportunities and constraints on sustainable living in the UK. The SLRG is coordinated from the University of Surrey, and brings together project teams from Surrey's Centre for Environmental Strategy (CES) and Departments of Sociology and Psychology. It also includes teams at other internationally acclaimed research centres: the SLRG and SPRG aim to document and analyse the complex links between lifestyles, consumption, everyday practices, values and the transition to sustainable ways of life. While SPRG focuses on particular consumption practices and technologies, SLRG's projects examine the lifestyles of households and communities and model the implications of consumption choices and values. The working papers in this series reflect the outputs, findings and recommendations emerging from a truly inter-disciplinary research programme arranged around four thematic research strands: Moments of transition: in-depth quantitative and qualitative research with households to understand the values, attitudes and habits informing their lifestyles, and to assess the impact of moments of major change-moving house, the arrival of the first child, and retirement-on habits and aspirations. Community and lifestyle change: studies of sustainable lifestyle change and innovations at the level of communities, focusing on remote rural localities in Scotland and on civil society initiatives for food growing and local resilience in England. Economic modelling of lifestyle change: studies focusing on the size and nature of rebound effects in energy use that could reduce or cancel out the gains from energy efficiency advances; and on the price responsiveness of consumption in relation to energy and sustainably produced foodstuffs. Synthesis of research and implications for policy : workshops and papers seeking to integrate findings and concepts across the suite of SLRG and SPRG projects; and research into the relationship of research evidence on sustainable living to policy development. Abstract:PurposeThe aim of this work is to provide an evidence-based evaluation and overview of causative substances in order to improve disease management.MethodsWe conducted a database search with MEDLINE via PubMed, screened reference lists of relevant reviews and matched our findings with a list of agents denoted as “may cause sensitisation by inhalation” by the phrase H334 (till 2011 R42). After exclusion of inappropriate publications, quality of the selected studies was rated with the Scottish Intercollegiate Guideline Network (SIGN) grading system. The evidence level for each causative agent was graded using the modified Royal College of General Practitioners (RCGP) three-star system.ResultsA total of 865 relevant papers were identified, which covered 372 different causes of allergic work-related asthma. The highest level achieved using the SIGN grading system was 2++ indicating a high-quality study with a very low risk of confounding or bias and a high probability of a causal relationship. According to the modified RCGP three-star grading system, the strongest evidence of association with an individual agent, profession or worksite (“***”) was found to be the co-exposure to various laboratory animals. An association with moderate evidence level (“**”) was obtained for α-amylase from Aspergillus oryzae, various enzymes from Bacillus subtilis, papain, bakery (flour, amylase, storage mites), western red cedar, latex, psyllium, farming (animals, cereal, hay, straw and storage mites), storage mites, rat, carmine, egg proteins, atlantic salmon, fishmeal, norway lobster, prawn, snow crab, seafood, trout and turbot, reactive dyes, toluene diisocyanates and platinum salts.ConclusionThis work comprises the largest list of occupational agents and worksites causing allergic asthma. For the first time, these agents are assessed in an evidence-based manner. The identified respiratory allergic agents or worksites with at least moderate evidence for causing work-related asthma may help primary care physicians and occupational physicians in diagnostics and management of cases suffering from work-related asthma. Furthermore, this work may possibly provide a major contribution to prevention and may also initiate more detailed investigations for broadening and updating these evidence-based evaluations.

Abstract:Fungal alpha-amylase is an important occupational allergen in the bakery industry. Epidemiologic studies focusing on the relationship between alpha-amylase allergen exposure and work-related respiratory allergy, however, have not been reported yet. In this cross-sectional study, sensitization to occupational allergens and work-related symptoms were studied in 178 bakery workers and related to allergen exposure. Alpha-amylase allergen concentrations were measured in personal dust samples, using a sandwich enzyme immunoassay. All workers were categorized into groups on the basis of their job histories and the alpha-amylase exposure levels of their job titles. Of all workers 25% had one or more work-related symptoms. As much as 9% of the bakery workers showed a positive skin prick test reaction to fungal amylase, and in 8% amylase-specific IgE was demonstrated. Alpha-amylase exposure and atopy appeared to be the most important determinants of skin sensitization, with prevalence ratios for atopy of 20.8 (95% CI, 2.74 to 158) and for medium and high alpha-amylase exposure groups of 8.6 (95% CI, 1.01 to 74) and 15.9 (95% CI, 1.95 to 129), respectively. Furthermore, a positive association was found between positive skin prick tests to alpha-amylase and work-related respiratory symptoms. In conclusion, this study has shown that there is a strong and positive relationship between alpha-amylase allergen exposure levels in bakeries and specific sensitization in bakery workers.

Abstract:Abstract A 52-year-old woman presented to dermatology with severe widespread dermatitis, characterized by spongiosis on a recent skin biopsy. Despite thorough investigation, a definitive cause remained elusive. The patient underwent patch testing with the baseline, photopatch, anti-infective, excipient, cosmetic, textile, fragrance and acrylate series. Patch testing revealed multiple positive 3+ blistering reactions to various acrylates: 2-hydroxyethyl methacrylate, 2-hydroxypropyl methacrylate, ethylene glycol dimethacrylate, 1,4-butanediol dimethacrylate, 1,6-hexanediol diacrylate, tetrahydrofurfuryl methacrylate, tetraethylene glycol dimethacrylate, butyl acrylate, ethyl acrylate, 2-hydroxyethyl acrylate and triethylene glycol diacrylate. The patient lacked any direct exposure to acrylates through common sources such as gel, shellac or acrylic nails; dental materials; dressings or bone cement. Further inquiry revealed that the husband’s profession was in windscreen repairs, where he routinely handled resins containing acrylates, which he would wipe on to his work overall. It emerged that the transfer of resin onto his clothing, subsequently laundered alongside the patient’s own clothes, likely facilitated sensitization. Analysis of the husband’s work-related resin products confirmed the presence of several acrylates implicated in the patient’s patch test reactions. Given the severity of her symptoms, including blistering reactions, a consort allergic contact dermatitis (CACD) to occupational acrylates was strongly suspected. Following counselling on allergen avoidance strategies, the patient consciously avoided close contact with her partner’s work clothing and began laundering her garments separately. Remarkably, this intervention led to complete resolution of her symptoms. CACD, also referred to as connubial contact dermatitis, is a recognized phenomenon wherein one develops allergic reactions to allergens originating from another individual. A recent systematic review reported a female predominance (62.8%) in cases of CACD, with partners or spouses being the most common consorts (50.0%) (Lee J, Guo S, Dinalo J et al. Consort allergic contact dermatitis: a systematic review. Dermatitis 2022; 33: 181–6). Exposure from a partner’s occupation was noted in a subset of cases (14.6%), underscoring the relevance of occupational history in such presentations. A caregiving relationship was also frequently involved. This case represents the one of the first documented instances of CACD attributed to occupational acrylates. It highlights the need for comprehensive social and occupational history taking in dermatology settings. It underscores the importance of considering potential sources of allergen exposure from partners or dependants, particularly in cases with unusual clinical presentations.

Abstract:Wheat allergy dependent on augmentation factors (WALDA) is the most common gluten allergy in adults. IgE-mediated sensitizations are directed towards ω5-gliadin but also to other wheat allergens. The value of the different in vitro cellular tests, namely the basophil activation test (BAT) and the active (aBHRA) and passive basophil histamine-release assays (pBHRA), in the detection of sensitization profiles beyond ω5-gliadin has not been compared. Therefore, 13 patients with challenge-confirmed, ω5-gliadin-positive WALDA and 11 healthy controls were enrolled. Specific IgE (sIgE), skin prick tests, BATs, aBHRA, and pBHRA were performed with allergen test solutions derived from wheat and other cereals, and results were analyzed and compared. This study reveals a distinct and highly individual reactivity of ω5-gliadin-positive WALDA patients to a range of wheat allergens beyond ω5-gliadin in cellular in vitro tests and SPT. In the BAT, for all tested allergens (gluten, high-molecular-weight glutenin subunits, α-amylase/trypsin inhibitors (ATIs), alcohol-free wheat beer, hydrolyzed wheat proteins (HWPs), rye gluten and secalins), basophil activation in patients was significantly higher than in controls (p = 0.004–p < 0.001). Similarly, significant histamine release was detected in the aBHRA for all test substances, exceeding the cut-off of 10 ng/mL in all tested allergens in 50% of patients. The dependency of tests on sIgE levels against ω5-gliadin differed; in the pBHRA, histamine release to any test substances could only be detected in patients with sIgE against ω5-gliadin ≥ 7.7 kU/L, whereas aBHRA also showed high reactivity in less sensitized patients. In most patients, reactivity to HWPs, ATIs, and rye allergens was observed. Additionally, alcohol-free wheat beer was first described as a promising test substance in ω5-gliadin-positive WALDA. Thus, BAT and aBHRA are valuable tools for the identification of sensitization profiles in WALDA.

Abstract:Background and objective: Wheat ingestion can lead to disorders such as IgE-mediated food allergy and wheat-dependent exercise-induced anaphylaxis (WDEIA), both of which are associated with impaired quality of life and significant morbidity. Allergy to wheat is relatively benign in children, although its natural history in adults is still unknown. Objective: We used placebo-controlled challenge to evaluate the natural history of wheat hypersensitivity in atopic patients with adultonset wheat allergy. Methods: We enrolled 13 patients from an initial cohort of adult patients with IgE-mediated wheat allergy (mean age, 40 years). After diagnosis, the patients observed a wheat-free diet and were followed as outpatients for 5 years to evaluate wheat exposure. Wheat-IgEtiters were determined at the end of follow-up, and a second wheat-challenge was performed. Results: Ten out of 13 patients took part in the study. The mean period of wheat avoidance was 4.2 years. Three patients had spontaneously reintroduced wheat before the second evaluation, after a mean (IQR) of 28 (18-36) months, with only mild gastrointestinal discomfort at reintroduction. At the end of follow-up, 9 of the 10 patients were wheat-tolerant. Two patients had a history of WDEIA. We observed a reduction in IgE levels, with median (IQR) IgE falling from 2.77 (0.35-100) kU/L at diagnosis to 0.88 (0.1-20.8) kU/L. The association between IgE and a negative challenge result was not statistically significant. Conclusion: IgE-mediated wheat allergy in adults is benign and represents a temporary break in gastrointestinal tolerance. Future studies may improve our knowledge of wheat allergens, routes of and factors leading to sensitization, and prognostic biomarkers.

Abstract:<p>Occupational exposure to foods is responsible for up to 25% of cases of occupational asthma and rhinitis. Animal and vegetable high‐molecular weight proteins present in aerosolized foods during food processing, additives, preservatives, antioxidants and food contaminants are the main inhalant allergen sources. Most agents typically cause IgE‐mediated allergic reactions, causing a distinct form of food allergy (<i>Class 3 food allergy</i>). The allergenicity of a food protein, allergen exposure levels and atopy are important risk factors. Diagnosis relies on a thorough medical and occupational history, functional assessment, assessment of sensitization, including component resolved diagnostics where appropriate, and in selected cases specific inhalation tests. Exposure assessment, including allergen determination, is a cornerstone for establishing preventive measures. Management includes allergen exposure avoidance or reduction (second best option), pharmacological treatment, assessment of impairment and worker's compensation. Further studies are needed to identify and characterise major food allergens and define occupational exposure limits, evaluate the relative contribution of respiratory versus cutaneous sensitization to food antigens, evaluate the role of raw versus cooked food in influencing risk, and define the absolute or relative contraindication of patients with ingestion‐related food allergy, pollinosis or oral allergy syndrome continuing to work with exposure to aerosolised food allergens.</p><p>This article is protected by copyright. Occupational allergy to phytase: case series of eight production workers exposed to animal feed additives

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Background</h3><p>In agricultural meat production, adding enzymes such as phytase to animal feed is widespread, but there is little awareness of the allergenic potential and health risks of these fungal enzymes.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Patients and Methods</h3><p>We report on eight patients working in a plant producing phytase granulates. All patients complained about work‐related rhinitis occurring within six months of the onset of exposure to phytase dust. Asthmatic symptoms and contact urticaria also occurred. To detect sensitizations to phytase, skin prick‐, patch‐, and basophil activation test were carried out with the factory product. Levels of IgE and IgG against phytase were also measured.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>There was a positive reaction to phytase with skin prick testing in seven of the eight patients. IgE specific to phytase was detectable in four of the eight patients, and IgG specific to phytase was detectable in six of the eight patients. The basophil activation test was positive in four out of seven patients tested, but the patch test was negative in all patients tested. Transfer to a different workplace with no exposure to phytase completely eliminated the symptoms.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>Mold enzymes such as phytase are highly potent occupational allergens. Occupational safety measures must be strictly implemented in order to protect the health of workers.</p></section>

Abstract:Objectives: The ability of the gliadin fraction of wheat gluten to exacerbate coeliac disease is well documented. We investigated the possible toxicity of high molecular weight glutenin subunits (HMW-GS) in coeliac disease in vitro using gluten-sensitive T cells, and in vivo with challenge studies in patient volunteers. Methods: A mixture of four HMW-GS was chemically separated from wheat flour and checked for purity by HPLC, SDS-PAGE and ELISA. T-cell lines, grown up from small intestinal biopsies from coeliac patients (n=17), were tested for their reactivity to HMW-GS. Adults with coeliac disease and who were on a gluten-free diet (n=3) underwent in-vivo challenges with HMW-GS. Duodenal biopsies, taken prior to the challenge and at intervals up to 6 h afterwards, were assessed for morphology, intra-epithelial lymphocyte count, and interleukin 15 (IL-15) expression, by immunohistochemistry. Results: T-cell lines from 11 of 17 patients were stimulated by HMW-GS. There was a significant change in small intestinal morphology 4 h after commencing infusions with HMW-GS in all three subjects. For example villus height to crypt depth ratios were reduced in the three patients from 3.0+/-0.5 to 1.29+/-0.2, 2.53+/-0.7 to 0.81+/-0.6 and 3.0+/-0.7 to 1.85+/-0.3, P<0.0001 in all cases. There was increased expression of IL-15 in the small intestine from 2 h after the HMW-GS challenges. Conclusion: Mixed HMW-GS stimulate T-cell lines from some coeliac patients and exacerbate coeliac disease in vivo, inducing expression, within 2 h, of IL-15. This suggests an innate immune response to these proteins.
Wheat-dependent exercise-induced anaphylaxis in Chinese people: a clinical research on 33 cases with antigenic analysis of wheat proteins

Abstract:Background Wheat-dependent exercise-induced anaphylaxis (WDEIA) is a severe allergic condition in which wheat ingestion together followed by physical exercise induces anaphylaxis. For patients with WDEIA, omega-5 gliadin is considered to be one of the major allergens. Aim To analyse the clinical features and allergen spectrum of WDEIA and to investigate the relationship between WDEIA and serum levels of platelet-activating factor (PAF), interleukin (IL)-9 and IL-33. Methods Medical histories and conditions of WDEIA cases were collected and summarized, with allergen tests of wheat proteins measured at the same visit. Of the 33 patients enrolled, 13 also had serum levels of PAF, IL-9 and IL-33 measured. The healthy control (HC) group consisted of 13 healthy individuals, who also underwent both the wheat-protein allergen tests and the inflammatory-mediator tests. Results All patients experienced severe allergic reaction during exercise after wheat ingestion. Manifestations of WDEIA included facial oedema, generalized urticaria and respiratory symptoms. Unconsciousness was also observed in 21 cases. In the patient group, 57.6% were confirmed as hypersensitive to glyceraldehyde-3-phosphate dehydrogenase (GAPDH), while 54.5% were allergic to omega-5 gliadin. PAF concentration was significantly higher in patients with WDEIA compared with HCs, whereas there was no significant difference in IL-9 or IL-33 between the two groups. Conclusions WDEIA is a rare type of anaphylaxis. GAPDH and omega-5 gliadin may be the most common allergy-causing wheat proteins for Chinese people. PAF may be associated with the onset and development of WDEIA.

Abstract:Human Langerhans cells (LC) express CD45, but clear data about the isoform(s) and their function(s) are lacking. In the present study, double labeling experiments reveal that freshly isolated LC from normal skin are CD45RO+/RA−/RB−. However, after isolation and short‐time culture where LC undergo an in vitro maturation resembling that to lymphoid dendritic cells, CD45RB emerges whereas CD45RO expression decreases. This evolution results from dynamic alternative RNA splicing. Addition of granulocyte‐macrophage colony‐stimulating factor or tumor necrosis factor‐α to short‐time cultures has no significant effect on CD45RB, but both cytokines accelerate the loss of CD45RO. LC isolated from lesional skin of atopic eczema highly express CD45RO and CD45RB. Cross‐linking of CD45 on LC isolated from atopic individuals inhibits the calcium mobilization in response to activation via Fcε receptor type I(FcεRI). Hence, the protein tyrosine phosphatase CD45 from human LC is subjected to a splicing phenomenon related to the differentiation and activation stage of these cells and regulates their FcεRI‐mediated activation.
