Abstract:Background A therapeutic revolution in the past decade is still a considerable unmet need in the treatment of rheumatoid arthritis (RA). On the other hand, dysfunction of regulatory T cells (Tregs) has been considered to be a pivotal cause of RA and correction of this dysfunction to be a potential RA therapy1. However, abnormalities of Tregs in patients with RA were reported controversially in previous studies2, in which only proportion was measured and Tregs were defined using different protein markers. Objectives In this study, we measured both absolute numbers and proportions of CD4+CD25+Foxp3+ Tregs in peripheral blood of RA patients and investigated the effects of low-dose recombinant human IL-2 (rhIL-2) on Tregs and CD4+ effector T cell subsets in patients with RA. Methods Both absolute numbers and proportions of Treg and Th17 cells in peripheral blood, defined as the CD4+CD25+FOXp3+T or CD4+IL-17 + T cell population, were examined by flow cytometry in 342 patients with RA with different 28-joint Disease Activities (DAS28), including 75 who had never received disease-modifying antirheumatic drugs (DMARD) and 151 who were receiving or had received DMARD. Among these patients, 112 consented at enrollment to receive rhIL-2 treatment. Before and after treatment, the Th17 and Treg cells in peripheral blood were analyzed by flow cytometry. Results The absolute count of Treg cells in patients with RA was significantly low compared with that of healthy controls (P<0.05), but the absolute count of Th17 cell was no different between RA and healthy controls. After the course of rhIL-2 treatment, there was a significant increase in the absolute count of Th17 and Treg cells in the CD4+ T cell population (P<0.01), but Th17/Treg was significantly low after the treatment. Conclusions The data suggest that, besides propotion, the decrease of Treg cell number, defined as the CD4+CD25+FOXp3+population, may contribute to the pathogenesis of RA. Over the treatment of rhIL-2, there was a more significant increase in the absolute count of Treg cells than that of Th17, and consequently restore the balance of Th17/Treg. Abstract:Failure of regulatory T (Treg) cells to properly control immune responses leads invariably to autoimmunity and organ damage. Decreased numbers or impaired function of Treg cells, especially in the context of inflammation, has been documented in many human autoimmune diseases. Restoration of Treg cell fitness and/or expansion of their numbers using low-dose natural IL-2, the main cytokine driving Treg cell survival and function, has demonstrated clinical efficacy in early clinical trials. Genetically modified IL-2 with an extended half-life and increased selectivity for Treg cells is now in clinical development. Administration of IL-2 combined with therapies targeting other pathways involved in the expression of autoimmune diseases should further enhance its therapeutic potential. Ongoing clinical efforts that capitalize on the early clinical success of IL-2 treatment should bring the use of this cytokine to the forefront of biological treatments for autoimmune diseases.

Abstract:Regulation of autoreactive CD4 T cells is essential to maintain self-tolerance and prevent autoimmune disease. Although CD8 T regulatory (Treg) cells that recognize self-peptides restricted by Qa-1 (HLA-E in humans) inhibit autoreactive CD4 cells and attenuate experimental autoimmune encephalomyelitis (EAE), the mechanism of this interaction is unclear. We generated Qa-1 mutant knock-in mice that impair Qa-1 binding to the T cell receptor (TCR) and CD94/NKG2A receptors. Analysis of these mice showed that TCR-dependent recognition of Qa-1-peptide complexes on target CD4 cells is essential for suppression by CD8 Treg cells. Further analysis revealed that genetic disruption of the Qa-1-CD94/NKG2A interaction unleashes robust CD8 Treg cell activity that completely abolishes development of EAE.

Abstract:Background The imbalance of T help 17 cells (Th17)/regulatory T cells (Tregs) is considered to be a pivotal cause of autoimmune diseases1, including systemic lupus erythematosus (SLE). However, previous reports1,2 describing the respective changes of Tregs and Th17 cells in SLE patients were controversial because a few samples or diverse markers were used to identify Tregs with little consensus. Objectives To clarify the status of Tregs and Th17 in SLE, we investigated the frequencies of Tregs and Th17 cells on a large scale and whether those defects can be corrected by the supplementation of low-dose human recombinant interleukin-2 (IL-2). Methods Two hundred and thirty-five SLE patients (219 women and 16 men), with mean age of 37.80±14.00 years, were enrolled. The disease activity using European League Against Rheumatism (EULAR) criteria was judged for SLE patients with erythrocyte sedimentation rate (ESR) and SLEDAI scores. The frequencies of CD3+CD4+FOXP3+Treg cells and Th17 cells in peripheral blood from these patients were measured by flow cytometry. And low-dose IL-2 was used among 127 patients at a dosage of fifty WIU every day for five days. Immunological and clinical assessments were performed again at the end of IL-2 treatment. Ninety healthy volunteers, matched for patients9 age and gender, were also included for the estimation of CD4+ T cell subsets. Results As compared to healthy controls (median of Treg cells: 33.09 cells/ul), the frequencies of circulating CD4+CD25+FOXP3+Treg cells were significantly decreased in SLE patients (median: 15.49 cells/ul, P<0.001). The median ratios of Th17/Tregs cells in patients were greatly higher than those of healthy volunteers [0.42 (0.19, 0.88) vs. 0.21 (0.15, 0.34), P<0.001]. There was not significantly different in circulating Th17 cell between two groups. Moreover, CD4+CD25+FOXP3+Treg cells were negatively correlated with ESR and SLEDAI score (r=-0.198, P=0.01; r=-0.25, P=0.002).While no obvious correlation was seen between Th17 cells and SLEDAI score. After IL-2 therapy in SLE, there was a four-fold increase in circulating CD4+CD25+FOXP3+Treg cells [43.73 (24.08,74.22) vs. 11.95 (7.51,20.34),P<0.001], whereas Th17 cells were increased slightly. The ratio of Th17/Tregs was decreased significantly in patients with IL-2 treatment [0.19 (0.09,0.41) vs. 0.52 (0.23,0.95),P<0.001], tended to balance and had no difference with healthy individual (P=0.275). Conclusions With the increase of disease activity,CD4+CD25+FOXP3+Treg cells were gradually reducing, while Th17 cells did not show a significant change, indicating that the reduction of Tregs but not the elevation of Th17 cells may be the major reason for imbalance of Th17/Tregs. It is speculated that SLE is an autoimmune disease triggered by the defect of immunotolerance. More importantly, low-dose IL-2 selectively modulated the abundance of Tregs, which effectively induced autoimmune tolerance and further improved clinical symptoms. Abstract:Regulatory T cells (Treg) are crucial for the maintenance of peripheral tolerance and for the control of ongoing inflammation and autoimmunity. The cytokine interleukin-2 (IL-2) is essentially required for the growth and survival of Treg in the peripheral lymphatic tissues and thus plays a vital role in the biology of Treg. Most autoimmune and rheumatic diseases exhibit disturbances in Treg biology either at a numerical or functional level resulting in an imbalance between protective and pathogenic immune cells. In addition, in some autoimmune diseases, a relative deficiency of IL-2 develops during disease pathogenesis leading to a disturbance of Treg homeostasis, which further amplifies the vicious cycle of tolerance breach and chronic inflammation. Low-dose IL-2 therapy aims either to compensate for this IL-2 deficiency to restore a physiological state or to strengthen the Treg population in order to be more effective in counter-regulating inflammation while avoiding global immunosuppression. Here we highlight key findings and summarize recent advances in the clinical translation of low-dose IL-2 therapy for the treatment of autoimmune and rheumatic diseases.

Abstract:Background Recent studies have shown that increased number of Th17 cells and decreased number of Treg cells in the peripheral blood contribute to ankylosing spondylitis (AS). However, current therapy for AS, including biological agents, immunosuppressant and glucocorticoid, can not correct the imbalance of Th17 and Regulatory T (Treg) cells in AS patients effectively. It has been reported that low dose IL-2 can selectively expand Treg cells and had a critical effect on homeostatic balance between the Th17 and Treg cells. Objectives The study is to explore the effect of low dose IL-2 therapy on the balance of Treg and Th17 cells in patients with AS and observe the efficiency and side effects of the therapy. Methods Seventeen patients, who met the 1984 modified New York criteria and had evidence of active inflammatory spondylitis (defined as Bath AS Disease Activity Index (BASDAI) score >4) despite of receiving standard therapy including glucocorticoid, immune-suppressants, biological agents or combination of them, were enrolled. These patients were not only given traditional treatment, but also injected subcutaneously low-dose IL-2 (50 WIU/day for 5 days). Clinical and laboratory indicators were compared before and after IL-2 treatment. The side effects were observed in the course of therapy. Results The number of Treg cells significantly increased after the treatment by 1 week (22.58±12.80 vs. 73.46±33.79, p<0.001). At the same time, there was a significantly decrease in the ratio of Th17/Treg cells (0.67±0.70 vs. 0.32±0.33, p=0.068). Besides, Th17 cells were also increased (12.83±9.24 vs. 19.26±13.24, p=0.054). Clinical manifestations were improved after the combination treatment of IL-2 and traditional drugs, especially BASDAI was decreased (4.57±0.61 vs. 1.98±0.83, P<0.001). No obvious adverse reactions were observed. Conclusions Low dose IL-2 therapy can restore and maintain the balance of Th17 and Treg cells in the active patients with AS. Manifestation improved after the combination therapy. The therapy is safe. Further research is needed to investigate long term benefits of low-dose IL-2 therapy. Abstract:Naturally occurring CD4+CD25(high)FOXP3+ regulatory T cells (Tregs) constitute a powerful mechanism of immune regulation and therefore, have important therapeutic potential for disorders such as autoimmune diseases, allograft rejection and graft-versus-host disease. Disruption of the IL-2R signalling pathway by genetic defects of the interleukin (IL)-2 gene or components of the IL-2 receptor (R) complex results in severe T cell-mediated autoimmunity rather than immunodeficiency, indicating a crucial role for IL-2R signalling for Treg development and function. Signalling downstream of the IL-2R can act through the phosphatidylinositol 3-kinase (PI3K)/Akt/mTOR pathway, the Janus kinase (JAK)/Signal transducers and Activators of Transcription (STAT) pathway and the mitogen-activated protein kinase (MAPK) pathway. In this report we focus on the relevance of these pathways as well as the impact of immunosuppressive drugs that may affect or enhance Treg function by targeting IL-2R signalling.

Abstract:Background Refractory system lupus erythematosus (SLE) still relies on aggressive treatment with high-dose glucocorticoids and immunosuppressive agents,but a significant proportion of patients persist with activity or relapse and show serious side-effects by the treatment.However, the imbalance of Th17 cells and regulatory T (Treg) cells in peripheral blood of patients with SLE may be an important factor in the pathogenesis of SLE.Because low-dose IL-2 can selectively enhance Treg function while avoiding the activation of effector T cells and rapamycin can promote the proliferation of Treg cells1–2,the combination of the low-dose IL-2 and rapamycin has been considered to treat refractory SLE for the purpose of remission. Objectives To observe the effect of low-dose IL-2 combined with rapamycin on the balance of Th17/Treg cells in patients with refractory SLE. Methods Eighty-two refractory SLE patients (80 women and 2 men),with a mean duration of 72.41±37.52 months and mean age of 36.22±12.48 years, were enrolled.They are in line with the standard of ACR in 1997,who are treated with glucocorticoid and immunosuppressant for more than one year, but the subjects continues to rise to a peak.After the eligible patients are given IL-2 and rapamycin in combination therapy with conventional therapy at 0, 6, 12, 24 week respectively after medication.The clinical symptoms, blood routine, urine routine, ESR, Th17 cells, Treg cells, Th17/Treg cells and the dosage of corticosteroids and immunosuppressant are registered one by one. Results At 24 week after treatment,28.9% patients with refractory SLE were relieved.Low-dose IL-2 combined with rapamycin leaded to an increase in the absolute counts of Treg cells in refractory SLE patients,from a median of 12.98 cells/ul (at week 0) to 22.1 cells/ul (at week 24) (P=0.002).The ratio of Th17/Treg cells shows a reduction from a median of 0.44 at week 0 to 0.29 at week 24 (P=0.029).No significant difference was observed in the absolute counts of Th17 after combined treatment.At week 24, the mean dosage of prednisone which refractory SLE patients were receiving decreased from 17.20 mg/d to 8.87 mg/d.And the categories of DMARDs use were also reduced (P<0.05). Conclusions Our results suggest that refractory SLE is major associated with the decreased number of Treg but not that of Th17.The Th17 and Treg cells in the peripheral blood of patients with refractory SLE tends to balance due to the significantly increase the number of Treg cells after low-dose IL-2 combined with rapamycin treatment.Low-dose IL-2 combined with rapamycin treatment can reduce the dosage of glucocorticoid and DMARDs. Abstract:Regulatory T cells (Tregs) are a small yet critical subset of CD4+ T cells, which have the role of maintaining immune homeostasis by, for example, regulating self-tolerance, tumor immunity, anti-microbial resistance, allergy and transplantation rejection. The suppressive mechanisms by which Tregs function are varied and pleiotropic. The ability of Tregs to maintain self-tolerance means they are critical for the control and prevention of autoimmune diseases. Irregularities in Treg function and number can result in loss of tolerance and autoimmune disease. Restoring immune homeostasis and tolerance through the promotion, activation or delivery of Tregs has emerged as a focus for therapies aimed at curing or controlling autoimmune diseases. Such therapies have focused on the Treg cell subset by using drugs to suppress T effector cells and promote Tregs. Other approaches have trialed inducing tolerance by administering the autoantigen via direct administration, by transient expression using a DNA vector, or by antigen-specific nanoparticles. More recently, cell-based therapies have been developed as an approach to directly or indirectly enhance Treg cell specificity, function and number. This can be achieved indirectly by transfer of tolerogenic dendritic cells, which have the potential to expand antigen-specific Treg cells. Treg cells can be directly administered to treat autoimmune disease by way of polyclonal Tregs or Tregs transduced with a receptor with high affinity for the target autoantigen, such as a high affinity T cell receptor (TCR) or a chimeric antigen receptor (CAR). This review will discuss the strategies being developed to redirect autoimmune responses to a state of immune tolerance, with the aim of the prevention or amelioration of autoimmune disease.

Abstract:Abstract Regulatory T cells (Tregs) are key immune cells for enforcing peripheral tolerance against self. Supporting this, in humans and in mice, a defect in Treg number or function results in severe autoimmune disease. Tregs limit autoimmunity by restraining the activation and cytokine production of conventional T cells (among other mechanisms). Our data reveal how two E3 ubiquitin ligase-activating proteins, Ndfip1 and Ndfip2, regulate Treg fate and function. Ndfip (Nedd4 family interacting proteins) 1 and 2 are known to activate several members of the Nedd4-family of E3 ligases, such as Itch, that have known roles in T cells. We generated mice that constitutively lack Ndfip2 and conditionally lack Ndfip1 in Foxp3+ cells (Treg cDKO mice). These Treg cDKO mice, display severe spleen and lung pathology marked by pulmonary arterial fibrosis, and ectopic lymphoid follicles. Additionally, Treg cDKO mice have higher circulating antibody levels. Since Foxp3 is an X-linked gene, we expect female DKO Treg mice to have both normal and Ndfip-deficient Tregs. Surprisingly, we find that even these female mice are characterized by splenomegaly, pulmonary arterial fibrosis, and these mice show circulating autoantibodies by 16 weeks of age. We will present data explaining why both male and female Treg cDKO mice get sick and discuss the roles of Ndfip-proteins in Treg activation, Treg cytokine production, PD-1 expression, and FoxP3 stability.

Abstract:Background Regulatory T cells (Tregs) play a major role in the maintenance of immune homeostasis and in the suppression of autoimmune responses. Breakdown of immune tolerance, a hallmark of autoimmune diseases, has been linked to both altered generation and impaired function of Tregs. However, the mechanism that leads to this defect remains elusive. Mitochondria are indispensible for proper function of the cell and clearance of damaged mitochondria through operation of mitophagy is essential for the survival of the cell. However whether this mechanism is functional in Tregs during an autoimmune response has not been demonstrated. Thus our hypothesis is that impaired mitophagy in Tregs could affect their suppressive capacity and lead to development of autoimmune disorders. Materials and methods For the purposes of this study, we utilised Foxp3gfp.KI transgenic mice that express the gfp protein under the promoter of the foxp3 gene and Atg5ΔCd4 mice, which lack the autophagy pathway in CD4+ T cells. Experimental Autoimmune Encephalomyelitis (EAE) was induced by immunisation with the myelin peptide (Mog35-55) emulsified in complete Freund adjuvant (CFA). Results Herein we show that Tregs isolated from draining lymph nodes of Mog35-55/CFA immunised mice exerted increased autophagosome formation and lysosomal function compared to naïve Tregs, whereas their mitophagy was impaired. In addition Tregs from immunised animals exhibited decreased mitochondrial membrane potential, enhanced production of mitochondrial ROS and increased metabolic requirements compared to naïve mice. Finally, Tregs deficient in authophagy demonstrated elevated numbers of mitochondria, increased production of mitochondrial ROS and lysosomal function. Conclusions Overall, our results indicate an impaired mitophagy in Tregs during an autoimmune response that results in accumulation of dysfunctional mitochondria that might contribute to loss of Treg suppressive function. Future experiments will be focused on understanding the molecular mechanism underlying the deregulated mitophagy in Tregs in autoimmune disorders that will orientate us in new potential therapeutic targets.

Abstract:Regulatory T cell deficiency is evident in patients with lupus, but the casual relationship and underlying mechanism leading to Treg deficiency are unclear. We analyzed the Treg profile, induction and functions of Treg in a lupus mouse model. A characteristic age‐dependent biphasic change of Treg frequency was observed in the MRL/lpr mice, which developed a spontaneous lupus‐like disease. After an early increase, Treg frequency in the peripheral lymphoid organs rapidly declined with age. Functionally, Treg from both young and old MRL/lpr mice were fully competent in suppressing the wild‐type MRL/+ T effector cell (Teff) responses. Adoptive transfer of MRL/+ Treg markedly suppressed clinical disease in the MRL/lpr mice. We demonstrated that the reduced Treg frequency was a result of insufficient peripheral Treg expansion due to defective MRL/lpr Teff in IL‐2 production, and the associated defects in dendritic cells, which could be fully restored by exogenous IL‐2. In the absence of IL‐2, MRL/lpr Teff but not MRL/lpr Treg were highly responsive to IL‐15 and could expand rapidly due to enhanced IL‐15R expression and IL‐15 synthesis. These findings thus provide a clear causal relationship and immunological mechanism underlying Treg deficiency and systemic autoimmunity.

Abstract:Canonical interleukin-2 (IL-2) signaling via the high-affinity CD25-containing IL-2 receptor-Janus kinase (JAK) 1,3-signal transducer and activator of transcription 5 (STAT5) pathway is essential for development and main-tenance of CD4+CD25HiFoxp3+ regulatory T cells (Tregs) that support immune homeostasis. Here, we report that IL-2 signaling via an alternative CD25-chemokine receptor pathway promotes the suppressive function of Tregs. Using an antibody against CD25 that biases IL-2 signaling toward this alternative pathway, we establish that this pathway increases the suppressive activity of Tregs and ameliorates murine experimental autoimmune encephalomyelitis (EAE). Furthermore, heparan sulfate, an IL-2-binding element of cell surfaces and extracellular matrix, or an engineered IL-2 immunocytokine can also direct IL-2 signaling toward this alternative pathway. Overall, these data reveal a non-canonical mechanism for IL-2 signaling that promotes suppressive functions of Tregs, further elucidates how IL-2 supports immune homeostasis, and suggests approaches to promote or suppress Treg functions.

Abstract:The expansion of T follicular helper (Tfh) cells correlates with disease progression in human and murine systemic lupus erythematosus (SLE). Unfortunately, there are no therapies to deplete Tfh cells. Importantly, low-dose rIL-2-based immunotherapy shows potent immunosuppressive effects in SLE patients and lupus-prone mice, primarily attributed to the expansion of regulatory T cells (Tregs). However, IL-2 can also inhibit Tfh cell differentiation. In this study, we investigate the potential of low-dose rIL-2 to deplete Tfh cells and prevent autoantibody responses in SLE. Our data demonstrate that low-dose rIL-2 efficiently depletes autoreactive Tfh cells and prevents autoantibody responses in lupus-prone mice. Importantly, this immunosuppressive effect was independent of the presence of Tregs. The therapeutic potential of eliminating Tfh cells was confirmed by selectively deleting Tfh cells in lupus-prone mice. Our findings demonstrate the critical role of Tfh cells in promoting autoantibody responses and unveil, (to our knowledge), a novel Treg-independent immunosuppressive function of IL-2 in SLE.
Block of Both TGF-β and IL-2 Signaling Impedes Neurophilin-1+ Regulatory T Cell and Follicular Regulatory T Cell Development.

Abstract:Understanding the mechanisms that lead to autoimmunity is critical for defining potential therapeutic pathways. In this regard there have been considerable efforts in investigating the interacting roles of TGF-β and IL-2 on the function regulatory T cells. We have taken advantage of dnTGF-βRII Il2ra-/- (abbreviated as Il2ra-/-Tg) mouse model, which allows a direct mechanistic approach to define the relative roles of TGF-β and IL-2 on Treg development. Il2ra-/-Tg mice spontaneously developed multi-organ autoimmune diseases with expansion of pathogenic T cells and enhanced germinal center response at 3-4 weeks of age. Importantly, peripheral Treg cells from Il2ra-/-Tg mice demonstrated an activated Th1-like stable phenotype and normal in vitro suppressive function, while thymus Treg increased but manifested decreased suppressive function. Interestingly, neither thymus nor peripheral Treg cells of Il2ra-/-Tg mice contained Neuropilin-1+ or PD-1hi phenotype, resulting in defective follicular regulatory T (Tfr) cell development. Such defective Tfr development led to elevated follicular T helper cells, enhanced germinal center responses and increased plasma cell infiltration. These data demonstrate an important synergetic role of TGF-β and IL-2 in the generation, activation and stability of Treg cells, as well as their subsequent development into Tfr cells.

Abstract:Regulatory T cells (Tregs) play a central role in the induction and maintenance of immune homeostasis and self-tolerance. Tregs constantly express the high-affinity receptor to IL-2. IL-2 is a pleiotropic cytokine and a key survival factor for Tregs. It maintains Tregs' suppressive function by promoting Foxp3 expression and subsequent production of immunoregulatory cytokines. Administration of low-dose IL-2 is shown to be a promising approach to prevent allograft rejection and to treat autoimmune and inflammatory conditions in experimental models. The combination of IL-2 with its mAb (JES6-1) has also been shown to increase the <i>t</i><sub>1/2</sub> of IL-2 and further enhance Treg frequencies and function. Low-dose IL-2 therapy has been used in several clinical trials to treat conditions such as hepatitis C vasculitis, graft-versus-host disease, type 1 diabetes, and systemic lupus erythematosus. In this paper, we summarize our findings on low-dose IL-2 treatment in corneal allografting and review recent studies focusing on the use of low-dose IL-2 in transplantation, autoimmunity, and other inflammatory conditions. We also discuss potential areas of further investigation with the aim to optimize current low-dose IL-2 regimens.

Abstract:The common γ chain (γc; IL-2RG) is a subunit of the interleukin (IL) receptors for the γc cytokines IL-2, IL-4, IL-7, IL-9, IL-15, and IL-21. The lack of appropriate neutralizing antibodies recognizing IL-2RG has made it difficult to thoroughly interrogate the role of γc cytokines in inflammatory and autoimmune disease settings. Here, we generated a γc cytokine receptor antibody, REGN7257, to determine whether γc cytokines might be targeted for T cell–mediated disease prevention and treatment. Biochemical, structural, and in vitro analysis showed that REGN7257 binds with high affinity to IL-2RG and potently blocks signaling of all γc cytokines. In nonhuman primates, REGN7257 efficiently suppressed T cells without affecting granulocytes, platelets, or red blood cells. Using REGN7257, we showed that γc cytokines drive T cell–mediated disease in mouse models of graft-versus-host disease (GVHD) and multiple sclerosis by affecting multiple aspects of the pathogenic response. We found that our xenogeneic GVHD mouse model recapitulates hallmarks of acute and chronic GVHD, with T cell expansion/infiltration into tissues and liver fibrosis, as well as hallmarks of immune aplastic anemia, with bone marrow aplasia and peripheral cytopenia. Our findings indicate that γc cytokines contribute to GVHD and aplastic anemia pathology by promoting these characteristic features. By demonstrating that broad inhibition of γc cytokine signaling with REGN7257 protects from immune-mediated disorders, our data provide evidence of γc cytokines as key drivers of pathogenic T cell responses, offering a potential strategy for the management of T cell–mediated diseases.

Abstract:Background A Tregs insufficiency is central to autoimmune and inflammatory diseases pathophysiology and low dose interleukin-2 (IL-2 LD ) can specifically activate Tregs. Objective To assess IL-2 LD therapeutic potential and select diseases for further clinical development, we performed an open-label, phase 2a, disease-finding, "basket trial" involving patients with one of 13 different autoimmune diseases. Methods 81 patients treated with IL-2 LD (1 million IU/day) for 5 days, followed by fortnightly injections. The first 48 patients received diluted Proleukin®, while the subsequent 33 received ready-to-use ILT-101®. The primary endpoint was the change in Tregs at day-8 compared to baseline. Key secondary endpoints included clinical efficacy assessments using the Clinical Global Impression (CGI) scale, disease-specific scores, and EuroQL-5D-5L. Results Our study unveiled a universal and significant expansion and activation of Tregs, without concomitant Teffs activation, across all 13 autoimmune diseases. Both Proleukin® and ready-to-use ILT-101® demonstrated identical effects on Tregs. CGI scores reflecting activity, severity, and efficacy were significantly reduced in the overall patient population. Disease-specific clinical scores improved in five of the six disease cohorts with at least six patients, namely ankylosing spondylitis, systemic lupus erythematosus, Behçet's disease, Sjögren's syndrome, and systemic sclerosis. Urticaria was the only severe adverse event related to treatment. Conclusion IL-2 LD was well-tolerated, exhibiting specific Treg activation and clinical improvements across the 13 autoimmune diseases. Clinical implication Tregs stimulation by IL-2 LD is a promising therapeutic strategy and IL-2 LD holds considerable promise for integration into combinatorial therapeutic approaches.
