Abstract:Background To measure the iodine nutritional status of island adults in Zhoushan, China.Methods A comparison study was carried out in eight villages. These were selected from iodized salt and non-iodized salt districts of Zhoushan island by random sampling. The Mann-Whitney test was used to compare the urinary iodine concentration and dietary iodine intake between the two districts.Results The median of urinary iodine concentration in the non-iodized salt group was 90 mug/l, which was lower than 194 mug/l in the iodized salt group (u = 14.673; p < 0.000), whereas the median of daily dietary iodine intake in the two groups was 128 and 147 mu g, respectively (u = 1.847; p = 0.065). There was no significant correlation between dietary iodine intake and urinary iodine concentration (p = 0.095).Conclusions Salt iodization is necessary. Special characteristics of the island diet should also be considered.
Serum Iodine as a Potential Individual Iodine Status Biomarker: A Cohort Study of Mild Iodine Deficient Pregnant Women in China

Abstract:Iodine deficiency during pregnancy is a widespread public health concern, but indicators and methods for assessing iodine nutritional status are lacking. Serum iodine concentration (SIC) is an important iodine metabolism biomarker and can, to some extent, predict the risk of thyroid diseases, making it a potential biomarker for assessing individual iodine nutrition levels. Our study aimed to analyze the relationship between SIC and thyroid function in a cohort of mild iodine deficient pregnant women in China in order to explore the potential of SIC as a biomarker of individual iodine status in pregnancy. A total of 1540 early pregnant women (gestation < 10 weeks) aged 18 to 45 years old were included in the final study from a Zhejiang multicenter population-based mother and child cohort. Repeated measures of SIC, urinary iodine concentration (UIC), and thyroid function were taken at approximately 10, 17, and 32 weeks of gestation. The SIC was statistically correlated with all thyroid function indexes in the first trimester, and a very strong positive correlation with FT4 over three trimesters (r = 0.449, 0.550, and 0.544, respectively). Pregnant women with an SIC < 72.4 mu g/L were at a higher risk of hypothyroxinemia (adjusted OR = 8.911, 95% CI = 5.141-15.447) and iodine deficiency (adjusted OR = 1.244, 95% CI = 1.031-1.502), while those with an SIC > 93.9 mu g/L were at a higher risk of thyrotoxicosis (adjusted OR = 11.064, 95% CI = 6.324-19.357) and excessive iodine (adjusted OR = 11.064, 95% CI = 6.324-19.357). In contrast, the UIC was not correlated with thyroid diseases (p > 0.05). These findings indicate that the SIC is a potential biomarker for assessing individual iodine nutrition and thyroid dysfunction in pregnant women.

Abstract:OBJECTIVE:To measure the iodine nutritional status on adult islanders and to evaluate the advantages and disadvantages of iodized salt prophylactic programs.METHODS:A comparative study was carried out in 8 rural townships selected from Dinghai (iodized salt district) and Daishan (non-iodized salt district) of Zhoushan islands by random sampling method. Mann-Whitney test was used to compare the urinary iodine concentration and dietary iodine intake of the two groups. Spearman correlation test was used to look for the correlation of urinary iodine concentration and dietary iodine intake in the two groups respectively.RESULTS:The amounts of daily iodine intake excluding the iodine intake from iodized salt in the two groups were 128 micro g and 147 micro g respectively but the difference was not statistically significant (u = 1.847, P = 0.065). The urinary iodine concentration of non-iodized salt group was 90 micro g/L, lower than 194 micro g/L in iodized salt group (u = 14.673, P = 0.000). There was no significant correlation between daily iodine intake and urinary iodine concentration (r(s) = 0.052, P = 0.095).CONCLUSIONS:In Zhoushan islands, the daily iodine intake did not meet the daily need (150 micro g/day) suggesting that iodized salt supplement was necessary. However, side effect due to overdose should be brought into attention.

Abstract:OBJECTIVE:To evaluate the iodine nutritional status and its relation to iodized salt intake in child islanders.METHODS:A comparing study was carried out in 4 townships selected by random sampling from Dinghai (iodized salt) and Daishan(non-iodized salt) of Zhoushan island and total 592 of children were included in the study. The Mann-Whitney test was used to compare the urinary iodine concentration and dietary iodine intake of two groups. The correlation of urinary iodine concentration and dietary iodine intake were examined by Spearman correlation test. Ordinal regression was used to analyse the dependent variables of urinary iodine concentration.RESULTS:The urinary iodine concentration of non-iodized salt district was lower than that in iodized salt district (87 microg/L compared with 150 microg/L, u=7.296, P=0.000) ,whereas the amount of daily iodine intake in the two groups was 34.5 microg/d and 62.3 microg/d (u=6.925, P=0.000). The urinary iodine concentration of 58.6 % children in non-iodized salt district was below 100 microg/L. Age and iodized-salt intake were significant factors in the final regression model (P<0.05) with the OR of 1.119 and 3.238, respectively.CONCLUSION:The daily dietary iodine intake for children in Zhoushan island is insufficient, the iodized salt prophylaxis is necessary.
The Effect of Iodine Status on the Risk of Thyroid Nodules: A Cross-Sectional Study in Zhejiang, China

Abstract:Objective. The aim of this study was to explore whether iodine nutrition is associated with the risk of thyroid nodules among adult population in Zhejiang Province, China. Methods. A cross-sectional study was conducted in the general population aged 18 years or older. A total of 2,710 subjects received physical examination, questionnaires, and thyroid ultrasonography. Urinary iodine concentration (UIC) and thyroid hormone levels were measured and documented for each subject. 4 multiple logistic regression models adjusted for other risk factors were applied to analyze the association between iodine nutrition and thyroid nodules. Results. The prevalence of thyroid nodules was 15.5% among all adults. As indicated by all 4 models, subjects with UIC varying from 200 μg l−1 to 399 μg l−1 had lower risk of thyroid nodules compared with those with relatively low UIC (<100 μg l−1), with approximately 37–57 percent reduction in risk. Moreover, subjects with UIC between 100 and 199 μg l−1 had a decreased risk of thyroid nodules in model 1 and 2 (OR = 0.75, 95% CI, 0.58–0.97; OR = 0.75, 95% CI, 0.58–0.97, respectively). However, there was no significant difference of risk in thyroid nodules between subjects with high UIC (≥400 μg l−1) and low UIC (<100 μg l−1). Furthermore, intake of iodized salt was inversely associated with risk of thyroid nodules, with approximately 69–77 percent reduction in risk. Conclusion. The relationship between UIC and the risk of thyroid nodules is U-shaped. Consumption of noniodized salt is an independent risk factor of thyroid nodules.
Salivary iodine concentrations can estimate iodine intake and diagnose abnormal thyroid function: a cross-sectional study in pregnant and lactating women in iodine-deficient areas

Abstract:Salivary iodine concentrations (SIC) and urinary iodine concentrations are correlated. This study aimed to verify the use of SIC as a biomarker for estimating iodine intake in pregnant and lactating women and to diagnose abnormal thyroid function. A cross-sectional study was conducted in northern Xinjiang, China. Participants provided venous blood, random urine, saliva and milk samples. A total of 607 pregnant and 171 lactating women volunteered to participate in the study. The average daily iodine intake was calculated for each participant. Pregnant women were divided according to trimester. The median daily iodine intake was 436·41 μg/d in the first trimester, 425·83 μg/d in the second trimester and 430·56 μg/d in the third trimester. The average daily iodine intake in lactating women was 416·16 μg/d. Different indicators were used to diagnose excessive iodine intake (> 500 μg/d). Among pregnant women, SIC had an AUC of 0·62 (P < 0·01), sensitivity of 51·75 % and specificity of 65 %. Among lactating women, SIC had an AUC of 0·63 (P = 0·03), sensitivity of 43·52 % and specificity of 85 %. SIC was an effective biomarker for diagnosing abnormal thyroid function (P = 0·03). In conclusion, this study demonstrated that SIC is a reliable biomarker for evaluating both iodine nutrition status and abnormal thyroid function in pregnant and lactating women.

Evaluation of median urinary iodine concentration cut-off for defining iodine deficiency in pregnant women after a long term USI in China

Abstract:Abstract Background The WHO/UNICEF/ICCIDD define iodine deficiency during pregnancy as median urinary iodine concentration (MUIC) ≤ 150 μg/L. China implemented universal salt iodization (USI) in 1995, and recent surveillance showed nationwide elimination of iodine deficiency disorders (IDD). Data from 2014 showed that the MUIC in 19,500 pregnant women was 154.6 μg/L and 145 μg/L in 9000 pregnant women in 2015. However, symptoms of iodine deficiency were absent. Our study sought to evaluate whether MUIC below 150 μg/L affects thyroid function of Chinese pregnant women and their newborns in Chinese context. Methods We screened 103 women with normal thyroid function and MUIC lower than 150 μg/L during week 6 of pregnancy at Peking Union Medical College Hospital. Patient demographics and dietary salt intake were recorded. Subjects were followed at 12, 24, and 32 gestational weeks. At each visit, a 3-day dietary record, drinking water samples, and edible salt samples were collected and analyzed for total dietary iodine intake. Additionally, 24-h urine iodine and creatinine were measured. Blood tests assessed thyroid function in both mothers and newborns. Results Of 103 pregnant women enrolled, 79 completed all follow-up visits. Most subjects maintained normal thyroid function throughout pregnancy. However, 19 had thyroid dysfunction based on thyroid stimulating hormone and free thyroxine levels. The median serum iodine was 71 μg/L (95% CI: 44, 109). The median thyroglobulin was < 13 μg/L. values above this level indicate iodine deficiency in pregnant women. The median dietary iodine intake during pregnancy, derived from the 3-day record and measures of water and salt, was 231.17 μg/d. Assuming 90% urinary iodine excretion (UIE), 200.11 μg/d UIE means the 222.34 μg iodine loss per day, suggesting that subjects had a positive iodine balance throughout pregnancy. All neonatal blood samples showed TSH levels lower than 10 mIU/L, indicating normal thyroid function. No significant difference was found among gestational weeks for urinary iodine, and the MUIC in subjects who completed 3 follow-up visits was 107.41 μg/L. Conclusion Twenty years after implementing USI, expectant Chinese mothers with MUIC of 107.4 μg/L, less than the WHO’s 150 μg/L benchmark, maintained thyroid function in both themselves and their newborn babies.

Abstract:Background. Associations between iodine intake and thyroid nodules (TNs) were not consistent. We aimed to illustrate the relationship between urinary iodine concentration (UIC) and TNs. Methods. A total of 12,698 participants were enrolled in analysis. All of the participants filled out questionnaires and underwent physical examinations, laboratory tests, and thyroid ultrasonography. UIC, serum thyrotropin (TSH), thyroid peroxidase antibodies (TPOAb), and thyroglobulin antibodies (TgAb) were measured in the central laboratory. Results. The prevalence of TNs was 16.00%, and the median UIC was 206.1 μg/L. TNs and UIC were negatively related when UIC was less than 527 μg/L (adjusted OR=0.87; 95% CI, 0.80, 0.94), and the relationship between UIC and TNs was not statistically significant when UIC was greater than 527 μg/L (adjusted OR=1.25; 95% CI, 0.98, 1.60). In women, UIC was negatively associated with risk for TNs (adjusted OR 0.95; 95% CI, 0.91, 0.99). Conclusion. The relationship between TNs and UIC differed between men and women. The risk of TNs decreased with the elevation of UIC in men when UIC was lower than 527 μg/L, while UIC and the presence of TNs were negatively correlated in women. In the future, cohort studies or other studies that can explain causality must be conducted to explore the relationship between iodine status and TNs.

Abstract:Breast milk iodine concentration (BMIC) can be different when median urinary iodine concentration (UIC) is similar. The BMIC, UIC/creatinine (Cr), estimated 24-h urinary iodine excretion (24-h UIE) of lactating women in Taiwan is unknown. This study enrolled lactating women from Taipei Veterans General Hospital (August 2021–February 2023). Each participant provided a random spot urine sample, two breast milk samples, a blood sample, and completed a food frequency questionnaire on the same day. Iodine measurement was performed by inductively coupled plasma mass spectrometry. The median UIC of the enrolled 71 women was 91.1 μg/L, indicating insufficient iodine status; however, the median BMIC was 166.6 μg/L and this suggested that the amount of iodine delivered through breast milk was adequate for the breastfed infants. BMIC was correlated with UIC/Cr and 24-h UIE (both rs = 0.49) but not with UIC (rs = 0.18) or thyroid stimulating hormone (rs = 0.07). Women who did not consume dairy products (adjusted odds ratio: 24.41, 95% confidence interval: 1.26–471.2) and multivitamins (adjusted odds ratio: 8.26, 95% confidence interval: 1.76–38.79) were at increased odds for having lower BMIC. The results suggest that measuring maternal UIC alone may not be sufficient, as BMIC, UIC/Cr, and 24-h UIE are all important biomarkers. Ingestion of dairy products and multivitamins were independently associated with BMIC.

Abstract:BACKGROUND: Adequate breast milk iodine concentration (BMIC) is essential for the growth and cognitive development of exclusively breastfed infants; however, data on variations in BMIC over 24 h are limited.OBJECTIVE: We aimed to explore in lactating women the variation in 24-h BMIC.METHODS: Thirty pairs of mothers and breastfed infants aged 0-6 mo were recruited from the cities of Tianjin and Luoyang, China. A 3-d 24-h dietary record, including salt intake, was performed to assess the dietary iodine intake of lactating women. Breast milk samples before and after each feeding for 24 h and 24-h urine samples were collected from the women for 3 d to estimate iodine excretion. A multivariate linear regression model was used to analyze the factors influencing BMIC. A total of 2658 breast milk samples and 90 24-h urine samples were collected.RESULTS: The median BMIC and 24-h urine iodine concentration (UIC) of lactating women for a mean of 3.6 ± 1.48 mo were 158 μg/L and 137 μg/L, respectively. The interindividual variability of BMIC (35.1%) was higher than that observed within individuals (11.8%). The variation in BMIC showed a "V" shaped curve over 24 h. The median BMIC at 08:00-12:00 (137 μg/L) was significantly lower than that at 20:00-24:00 (163 μg/L) and 00:00-04:00 (164 μg/L). A progressively increasing curve was obtained for BMIC until it peaked at 20:00 and plateaued at a higher concentration from 20:00 to 04:00 than at 08:00-12:00 (all P < 0.05). BMIC was associated with dietary iodine intake (β: 0.366; 95% CI: 0.004, 0.018) and infant age (β: -0.432; 95% CI: -1.07, -0.322).CONCLUSIONS: Our study shows that the BMIC presents a "V" shaped curve over 24 h. We recommend that breast milk samples be collected between 08:00 and 12:00 for evaluation of the iodine status of lactating women.

Abstract:The iodine status of children has improved and stabilized since China implemented salt iodization measures 25 years ago, but routine monitoring of iodine cannot reflect regional factors that influence the iodine level in children. Therefore, we conducted a regional spatial–temporal analysis of children's median urinary iodine concentration (MUIC) and searched for possible factors that might affect children's iodine levels by mining monitoring data. We analyzed data from Xinjiang collected as part of the "Iodine Deficiency Disease National Monitoring Program" from 2017 to 2020. The study population consisted of 76,268 children who participated in the study. We used global autocorrelation analysis to determine whether the MUIC of children was spatially clustered, local autocorrelation analysis to identify specific clustering areas, local cold and hot spot analysis to verify the reliability of the local autocorrelation results, and a spatial lag model to identify factors affecting the children's MUIC. The MUIC values were 217.70, 227.00, 230.67, and 230.67 μg/L in 2017, 2018, 2019, and 2020, respectively. Global autocorrelation analysis showed that the MUIC of all children in the study was significantly related to region (Z scores all > 1.96, P values all < 0.05) from 2017 to 2020. Partial auto-correlation analysis showed that counties with clusters of high values were mostly concentrated in the southwestern region of Xinjiang, whereas counties with clusters of low values were located in the northern part of Xinjiang. Partial cold spot and hot spot analysis showed the same trend, and there are more overlapping districts and counties in 4 years. Three-dimensional trend analysis indicated that children from the western part of Xinjiang had high levels of urinary iodine. According to spatial lag model, urine iodine concentration level is positively correlated with thyroid volume, average salary, and urbanization rate classification. The MUIC of 8–10-year-old children in Xinjiang was spatially clustered and related to geographic region. Our results show that spatial analysis of survey data combined with geographic technology and public health data can accurately identify areas with abnormal iodine concentrations in children. Additionally, understanding the factors that influence iodine levels in the human population is conducive to improving monitoring methods.

Abstract:BACKGROUND: Both insufficient and excess iodine may produce thyroid disease. After salt iodization in China, the median urine iodine concentration (UIC) of children aged 8-10 years appeared adequate. However, it is unknown whether dietary changes due to rapid economic development in Shanghai have affected whole population iodine nutrition.OBJECTIVE: To assess dietary iodine intake, UIC and the prevalence of thyroid disease in the general population of Shanghai.DESIGN: A cross-sectional survey was conducted with general participants aged 5-69 years (n = 7,904) plus pregnant and lactating women (n = 380 each) selected by stratified multistage sampling. The iodine concentrations in their salt, drinking water and urine were measured. Daily iodine intake was estimated using the total diet study approach. Serum thyroid hormone concentrations and thyroid-related antibodies were measured and thyroid ultrasonography was performed.RESULTS: The median iodine concentration in salt was 29.5 mg/kg, and 12.8 µg/L in drinking water. Iodized salt, used by 95.3% of participants, contributed 63.5% of total dietary iodine. Estimated daily iodine intake was 225.96 µg. The median UIC of general participants was 146.7 µg/L; UIC <100 µg/L (iodine insufficiency) was seen in 28.6%; UIC >300 µg/L (iodine excess) in 10.1%. Pregnant women had a median UIC of 135.9 µg/L, with UIC <150 µg/L in 55.4%. Thyroid nodules and subclinical hypothyroidism were found in 27.44% and 9.17%, respectively.CONCLUSIONS: According to published criteria, the current dietary iodine intake in Shanghai was generally sufficient and safe, but insufficient in pregnant women. Thyroid nodules and subclinical hypothyroidism were the commonest thyroid diseases identified.

Abstract:Purpose: Iodine deficiency disorder (IDD) is an ongoing worldwide recognized problem with over two billion individuals having insufficient iodine intake. School-aged children and pregnant women are often target groups for epidemiological studies, but there is a lack of knowledge on the general adult population. The aim of this study was to assess the iodine status among a Portuguese public university staff as a proxy for the adult working population. Methods: The population study covered 103 adults within the iMC Salt randomized clinical trial, aged 24-69 years. Urinary iodine concentration was measured spectrophotometrically using the Sandell-Kolthoff reaction. Iodine food intake was assessed using a 24-h dietary recall. The contribution of discretionary salt to the iodine daily intake was assessed through 24-h urinary sodium excretion (UIE) and potentiometric iodine determination of household salt. Results: The mean urine volume in 24 h was 1.5 L. The median daily iodine intake estimated from 24-h UIE was 113 µg/day, being lower among women (p < 0.05). Only 22% of participants showed iodine intake above the WHO-recommended cutoff (150 µg/day). The median daily iodine intake estimated from the 24-h dietary recall was 58 µg/day (51 and 68 µg/day in women and men, respectively). Dairy, including yoghurt and milk products, were the primary dietary iodine source (55%). Iodine intake estimated from 24-h UIE and 24-h dietary recall was moderately correlated (Spearman rank correlation coefficient r = 0.34, p < 0.05). The average iodine concentration in household salt was 14 mg I/kg, with 45% of the samples below the minimum threshold preconized by WHO (15 mg I/kg). The contribution of discretionary salt to the daily iodine intake was around 38%. Conclusion: This study contributes new knowledge about iodine status in Portuguese working adults. The results revealed moderate iodine deficiency, particularly in women. Public health strategies and monitoring programs are needed to ensure iodine adequacy in all population groups.

Abstract:Abstract <sec> Objectives Iodine is an essential micronutrient that plays a critical role in child growth and development. This study aimed to examine median urinary iodine concentrations (mUIC) by country and key health and sociodemographic factors. </sec> <sec> Methods Nationally representative surveys from Bangladesh (n = 1282), Ecuador (n = 6063), Malawi (n = 758), and the U.S. (n = 3548) were analyzed to estimate mUIC among school-age children (6–12 years old) from the Biomarkers Reflecting Inflammation and Nutritional Determinants of Anemia (BRINDA) project. WHO classifies mUICs into four categories: insufficient iodine intake, defined as mUIC < 100 μg/L; adequate iodine nutrition categorized as mUIC from 100 to 199 μg/L; above the required intake as mUIC from 200 to 299 μg/L; and excessive intake as mUIC >300 μg/L. Inflammation was defined by Alpha 1-acid glycoprotein (AGP) of >1 g/L or C-reactive protein (CRP) of >5 mg/L. Underweight was defined by Body Mass Index (BMI) for age z-score < −2 and overweight was defined as BMI z-score >2. Complex survey designs were applied to calculate the mUIC for each country. Design-based median tests were used to examine whether mUICs differed by age, sex, weight status, and inflammation status by country. </sec> <sec> Results mUIC was adequate in Bangladesh (138.5 μg/L) and above the required intake in Ecuador (251.0 μg/L), Malawi (265.5 μg/L), and the U.S. (230.0 μg/L). Boys had a significantly higher mUIC than girls in Ecuador (257.0 vs. 237.0 μg/L, P = 0.03), Malawi (302.0 vs. 226.8 μg/L, P = 0.01), and the U.S. (264.0 vs. 210.3 μg/L, P = 0.04). mUIC was not significantly different by age groups (6–9 vs. 10–12 years), weight status, and inflammation status, except in Bangladesh where the group with inflammation had a higher mUIC than the group without (153.0 vs. 130.0 μg/L, P = 0.04). </sec> <sec> Conclusions Although heterogeneity existed in mUICs across different countries, iodine intake was adequate or above requirements in all four countries. Abstract:Iodine is an essential trace element for the synthesis of thyroid hormones (THs). THs play an important role in growth and development, metabolism and body organ function. There is no iodine-specific food frequency questionnaire (I-FFQ) for pregnant women in China. This study aimed to validate and optimize the I-FFQ. A total of 1802 pregnant women were included in this study. The iodine nutrition survey was performed using I-FFQ and three-day estimated food diary (3DEFD). Seventy-one women of reproductive age repeated I-FFQ twice to assess for FFQ reproducibility. The I-FFQ shortens survey time by streamlining items based on iodine contribution. Correlation analyses and Bland-Altman analyses were used to verify the consistency of I-FFQ with 3DEFD, as well as the stability and feasibility of I-FFQ optimization. I-FFQ and 3DEFD had a strong correlation (R=0.76, p <0.001) and agreement (Kappa=0.731, p <0.001). Bland-Altman plot showed that 5.1% of participants exceeded the limit of agreement (LOA). Non-pregnant women of reproductive age completed the I-FFQ twice, and the results had a strong correlation (R=0.72, p <0.001). Bland-Altman analysis showed that 5.6% of individuals were located outside the LOA. The consistency of I-FFQ before and after optimization is good (Kappa=0.982, p <0.001). Therefore, I-FFQ could be used as a valid tool to estimate iodine intake among Chinese pregnant women. The optimized I-FFQ could shorten the survey time without affecting its accuracy.

Abstract:Iodine is a trace element required to produce the thyroid hormones, which are critical for development, growth and metabolism. To ensure appropriate population iodine nutrition, convenient and accurate methods of monitoring are necessary. Current methods for determining iodine status either involve a significant participant burden or are subject to considerable intra-individual variation. The continuous secretion of iodide in saliva potentially permits its use as a convenient, non-invasive assessment of status in populations. To assess its likely effectiveness, we reviewed studies analysing the association between salivary iodide concentration (SIC) and dietary iodine intake, urinary iodide concentration (UIC) and/or 24-h urinary iodide excretion (UIE). Eight studies conducted in different countries met the inclusion criteria, including data for 921 subjects: 702 healthy participants and 219 with health conditions. SIC correlated positively with UIC and/or UIE in four studies, with the strength of relationship ranging from r=0.19 to r=0.90 depending on sampling protocol, age, and if salivary values were corrected for protein concentration. Additionally, SIC positively correlated with dietary intake, being strongest when saliva was collected after dinner. SIC varied with external factors, including thyroid function, use of some medications, smoking, and overall health status. Evidence provided here supports the use of SIC as a viable, low-burden method for determining iodine status in populations. However small sample sizes and high variability indicates the need for more extensive analyses across age groups, ethnicities, disease states and dietary groups to clarify the relative accuracy and reliability in each case and standardise procedure.

Abstract:Purpose: There have been no reports on the application of salivary iodine concentration (SIC) in evaluating iodine nutrition in pregnant women. This study aimed to clarify the relationship between SIC and indicators of iodine nutritional status and thyroid function during pregnancy, to investigate whether salivary iodine can be applied to the evaluation of iodine nutritional status in pregnant women, and to provide a reference basis for establishing a normal range of salivary iodine values during pregnancy. Methods: Pregnant women were enrolled in the Department of Obstetrics, the people's hospital of Yuncheng Country, Shandong Province, from July 2021 to December 2022, using random cluster sampling. Saliva, urine, and blood samples were collected from pregnant women to assess iodine nutritional status, and venous blood was collected to determine thyroid function. Results: A total of 609 pregnant women were included in this study. The median spot urinary iodine concentration (SUIC) was 261 μg/L. The median SIC was 297 μg/L. SIC was positively correlated with SUIC (r = 0.46, P < 0.0001), 24-h UIC (r = 0.30, P < 0.0001), 24-h urinary iodine excretion (24-h UIE) (r = 0.41, P < 0.0001), and estimated iodine intake (EII) (r = 0.52, P < 0.0001). After adjusting for confounders, there was a weak correlation between SIC and serum total iodine and serum non-protein-bound iodine (P = 0.02, P = 0.04, respectively). Pregnant women with a SIC < 176 μg/L had a higher risk of insufficient iodine status (OR = 2.07, 95% CI 1.35-3.19) and thyroid dysfunction (OR = 2.71, 95% CI 1.18-6.21) compared to those with higher SIC. Those having SIC > 529 μg/L were more likely to have excessive iodine status (OR = 2.82, 95% CI 1.81-4.38) and thyroid dysfunction (OR = 3.04, 95% CI 1.36-6.78) than those with lower SIC values. Conclusion: SIC is associated with urinary iodine concentration and thyroid function in pregnant women. SIC < 176 μg/L was associated with an increased risk for iodine deficiency and hypothyroxinemia, while SIC > 529 μg/L was related to excess and thyrotoxicosis. SIC can be used as a reference indicator for evaluating the iodine nutrition status of pregnant women, but it needs further investigation and verification. Abstract:Abstract Background/Aims: Iodine deficiency is a public health problem over the world. China has achieved the iodine deficiency disorders (IDD) elimination goal through a mandatory universal salt iodization (USI) program. The aim of this study is to assess the current iodine nutritional status and thyroid volume of school-aged children in Wuhan, central of China. Methods The cross-sectional study was conducted among 8–10 years old children from 5 districts in Wuhan. A total of 1000 students were investigated. Salt iodine concentrations, urine iodine concentrations were determined using the standardized methods recommended by WHO/ICCIDD. An ultrasonography of thyroid and anthropometric measurements were performed in all subjects. Results The median urinary iodine concentration(MUI) of students was 247.50 μg/L with interquartile range of 147.00 to 384.00 μg/L. Two districts were classified as regions with adequate iodine nutrition, two districts as regions above the requirement, and one district as a region excessive according to the MUI. The prevalence of goiter was 3.0% and 7.8% according to Chinese national and WHO reference values, respectively, and the difference was not observed among different iodine nutrition groups(P༞0.05). The median thyroid volume(Tvol) of all students was 2.56 ml and the P97 of thyroid volume of both male and female in our study was generally higher than that reported by WHO/ICCIDD in 2007 as a function of age and BSA. The Tvol was in significant association with age, height, weight and BSA by Spearman’s correlation analysis(P < 0.001),and the result of multivariate linear regression analysis showed that only BSA were found to have a significant effect on thyroid volume after log conversion (t = 13.437, P < 0.001). Conclusion The present study indicated that the iodine nutrition among school-aged children in Wuhan was generally adequate with a relatively low goiter rate, while some of them have excess iodine nutrition. Thyroid volume in our study was higher than the reference of WHO and showed a significant correlation with BSA. So further researches were needed to evaluate the thyroid function in children with iodine excessive and the applicability of WHO reference in goiter diagnose. It was also highly recommended to establish the local reference values for thyroid volume that might be applicable to precisely define goiter prevalence in Wuhan.
