J Strength Cond Res 32(6): 1692–1701, 2018—Recent prospective cohort studies have reported preseason functional performance test (FPT) measures and associations with future risk of injury; however, the findings associated with these studies have been equivocal. The purpose of this study was to determine the ability of a battery of FPTs as a preseason screening tool to identify female Division III (D III) collegiate athletes who may be at risk for a noncontact time-loss injury to the lower quadrant (LQ = low back and lower extremities). One hundred six female D III athletes were recruited for this study. Athletes performed 3 FPTs: standing long jump (SLJ), single-leg hop (SLH) for distance, and the lower extremity functional test (LEFT). Time-loss sport-related injuries were tracked during the season. Thirty-two (24 initial and 8 subsequent) time-loss LQ injuries were sustained during the study. Ten of the 24 initial injuries occurred at the thigh and knee. At-risk athletes with suboptimal FPT measures (SLJ ≤79% ht; (B) SLH ≤64% ht; LEFT ≥118 seconds) had significantly greater rates of initial (7.2 per 1,000 athletic exposures [AEs]) and total (7.6 per 1,000 AEs) time-loss thigh or knee injuries than the referent group (0.9 per 1,000 AEs; 1.0 per 1,000 AEs, respectively). At-risk athletes were 9 times more likely to experience a thigh or knee injury (odds ratio [OR] = 9.7, confidence interval [CI]: 2.3–39.9; p = 0.002) than athletes in the referent group. At-risk athletes with a history of LQ sports injury and lower off-season training habits had an 18-fold increased risk of a time-loss thigh or knee injury during the season (adjusted OR = 18.7, CI: 3.0–118.1; p = 0.002). This battery of FPTs appears useful as a tool for identifying female D III athletes at risk of an LQ injury, especially to the thigh or knee region.

Abstract:Interferon-alpha (IFN-alpha) treatment can suppress the hypersecretion syndrome associated with functional neuroendocrine tumors. Chromogranin A (CgA) is a matrix protein of neuroendocrine secretory vesicles and appears to be essential for an appropriate neuroendocrine secretory function. To test the hypothesis that IFN-alpha can directly interfere with CgA gene transcription, we performed transient transfection studies in pancreatic neuroendocrine tumor cells employing CgA-luciferase reporter gene constructs showing that IFN-alpha inhibited basal and protein kinase C-dependent CgA promoter activity. Using 5'-deletion constructs in combination with mutational analysis of the proximal CgA core promoter, a cyclic AMP response element (CRE) at -71 to -64 bp was identified as the IFN-alpha response element of the CgA gene. Furthermore, functional studies indicated that IFN-alpha exerts its effect on the CgA promoter via interference with CRE binding protein (CREB)/CREB binding protein (CBP)-dependent transactivation of the CgA-CRE.
Female National Collegiate Athletic Association Division-I Athlete Injury Prediction by Vertical Countermovement Jump Force-Time Metrics

J Strength Cond Res 38(4): 783–786, 2024—Vertical countermovement jump (CMJ) assessments on force plates have been purported to screen for musculoskeletal injury risk (MSKI) but with little scientific support. Thus, this study aimed to identify associations and noncontact lower-body injury predictability with CMJ force-time metrics in female athletes. The study entailed a retrospective analysis of routine injury and performance monitoring from 155 female National Collegiate Athletics Association Division I athletes. Noncontact lower-body injuries included in analysis were confirmed by medical staff, occurred during competition or training, resulted in time loss from training, and occurred within 3 months following CMJ testing (2 maximal effort, no arm swing, jumps on dual force plates). A total of 44 injuries occurred within 3 months following CMJ baseline testing and resulted in an average of 24.5 missed days from training. Those who sustained an injury were more likely to sustain another injury (15 of 44 injuries [33.1%]; odds ratio = 3.05 [95% CI = 1.31–6.99]). For every 1-unit increase from the mean in eccentric mean power and minimum eccentric force, there was a decrease in odds of sustaining a MSKI. Despite high overall model accuracy (85.6%), the receiving operating characteristic area under the curve (65.9%) was unacceptable and the true positive rate (recall) was 0.0%. Thus, no injuries in the testing data set were correctly classified by the logistic regression model with CMJ force-time metrics as predictors. Baseline CMJ assessment may not be useful for noncontact lower-body musculoskeletal injury screening or predictability in National Collegiate Athletics Association female athletes.

Abstract:Anterior cruciate ligament (ACL) injuries in female adolescent athletes occur at disproportionately high levels compared to their male counterparts. However, limited prospective data exist on the validity of low-cost screening tools that can proactively identify ACL injury risk, specifically for female athletes. The purpose of this study was to assess the concurrent validity of a three-task injury risk factor assessment by comparing visually derived outcome scores from two-dimensional (2D) video data with dichotomized three-dimensional (3D) biomechanical variables collected using motion capture technology. A total of 41 female club volleyball athletes (14.7 ± 1.4 years) were tested and asked to perform three tasks: double-leg vertical jump (DLVJ), single-leg squat (SLS), and single-leg drop landing (SLDL). One rater was trained on the scoring criteria for the 2D data and independently scored one forward-facing and one side-facing video for each task. Risk factors identified included poor knee position, lateral trunk lean, and excessive trunk flexion/extension. In addition, 3D joint angles were calculated for the trunk and knee in the sagittal and frontal planes and converted to dichotomous variables based on biomechanical thresholds of injury risk. For comparison of 2D and 3D outcomes, percent agreement and Cohen's kappa were calculated for each risk factor individually. Overall, 2D scores were found to exhibit moderate to excellent percent agreement with 3D outcomes for trunk position (69.1–97.1%). Specifically, ipsilateral trunk lean during single-leg tasks exhibited the highest agreement (85.3–88.2%) with moderate reliability (κ = 0.452–0.465). In addition, moderate to substantial reliability was found for trunk flexion during double-leg tasks (κ = 0.521–0.653); however, an evaluation of single-leg tasks resulted in only fair reliability (κ = 0.354). Furthermore, 2D scores were not successful in identifying poor knee position as percent agreement fell below 50% for both the single-leg tasks and averaged 60% agreement across both the phases of the DLVJ. Kappa coefficients further emphasized these trends indicating no to slight concurrent validity (κ = −0.047–0.167) across tasks. Overall, these findings emphasize the potential for valid, low-cost screening tools that can identify high-risk movement patterns. Further study is needed to develop improved assessment guidelines that may be employed through visual assessment in sports environments.

Abstract:Seven cases of patients with multiple myeloma in which eyes were obtained at autopsy are reviewed. Four demonstrated characteristic cysts of the ciliary body and pars plana. The four patients with cysts had received urethane during their course. They had an average period of survival of 4.6 years and showed elevated serum protein levels. The three without cysts had not received urethane, survived an average of one year, and had normal serum protein levels. Urethane, the duration of the disease, and associated blood changes may be etiological factors in the formation of these characteristic cysts.
Reactive postural responses predict risk for acute musculoskeletal injury in collegiate athletes

Abstract:Identifying risk factors for musculoskeletal injury is critical to maintain the health and safety of athletes. While current tests consider isolated assessments of function or subjective ratings, objective tests of reactive postural responses, especially when in cognitively demanding scenarios, may better identify risk of musculoskeletal injury than traditional tests alone. Objectives: Examine if objective assessments of reactive postural responses, quantified using wearable inertial measurement units, are associated with the risk for acute lower extremity musculoskeletal injuries in collegiate athletes. Design: Prospective survival analysis. Methods: 191 Division I National Collegiate Athletic Association athletes completed an instrumented version of a modified Push and Release (I-mP&R) test at the beginning of their competitive season. The I-mP&R was performed with eyes closed under single- and dual-task (concurrent cognitive task) conditions. Inertial measurement units recorded acceleration and angular velocity data that was used to calculate time-to-stability. Acute lower extremity musculoskeletal injuries were tracked from first team activity for six months. Cox proportional hazard models were used to determine if longer times to stability were associated with faster time to injury. Results: Longer time-to-stability was associated with increased risk of injury; every 250 ms increase in dual-task median time-to-stability was associated with a 36% increased risk of acute, lower-extremity musculoskeletal injury. Conclusions: Tests of reactive balance, particularly under dual-task conditions, may be able to identify athletes most at risk of acute lower extremity musculoskeletal injury. Clinically-feasible, instrumented tests of reactive should be considered in assessments for prediction and mitigation of musculoskeletal injury in collegiate athletes.

Abstract:Reactive strength is the ability to effectively use a fast stretch shortening cycle (SSC) with the eccentric into a concentric contraction (Rebelo et al., 2022, Healthcare, 10, 593). A fast SSC is determined to be <200 ms due to a preliminary study by (Schmidtbleicher & Komi, 1992, Encyclopedia Sports Med. Vol. 3 Strength and Power in Sport) reviewing muscular contractions. Reactive strength is determined through two different metrics used within the literature which are ground contact time (GCT) and jump height (JH) allowing for understanding on how an athlete can utilise the myotatic reflex to react with the floor. Reactive Strength Index (RSI) is typically measured via drop jump (DJ) and 10/5 reactive pogo jumps. A novel way to measure RSI is via a rebound countermovement jump (RCMJ) which utilises RSI-Modified and includes time to take off instead of ground contact time. RSI is an important metric to measure because it has implications within sport-specific metrics like sprinting and jumping (Jarvis et al., 2022, Sports Med, 52, 301-330). However, some of the limitations of RSI are it is a ratio metric, and therefore RSI can mask underpinning metrics. As a result, additional metrics such as such a force, velocity and power can help better understand athlete development and the underpinning key performance indicators. However, many metrics within reactive jumps remain unsearched. Data will be collected from around 16 university athletes aged 18-25 currently competing in sport with at least one year’s experience in resistance training. Data will be collected using Hawkins dynamic force plates, sampling at 1000 Hz, and has recently been reported to be valid and reliable (Badby et al., 2023, Sports, 23, 4820) Participants will be asked to complete 3 DJ, RCMJ and 10/5 jump trials on two separate occasions separated by a minimum of 48 hours. Athletes will be instructed to perform a quiet standing period for 1 second before and after each jump to calculate body weight by the 5 × standard deviation method. Therefore, the study aims to compare the disparities between all three jumps aswell, sexes and the potential implications on performance. Previous studies displayed significant disparities between male and female participants. Therefore, the null hypothesis for this study is to display a non-significant difference across sexes aswell as between jumps. A limitation to this study is controlling the standardisation of variables such as squat depth during all tests.
The association between reactive strength index and reactive strength index modified with approach jump performance

Abstract:Jumping performance is one of the key components of volleyball game, thus evaluating jumping ability through different biomechanical variables offers opportunity for performance optimization. The aim of this study was to assess the associations between reactive strength index (RSI), reactive strength index modified (RSI mod ) and approach jump performance in male volleyball players. Forty volleyball players performed drop jump (DJ) form 40 cm high box, bilateral and unilateral countermovement jumps (CMJ) and approach jump. RSI in DJ was calculated as the ratio between jump height and ground contact time, while the RSI in CMJ tasks (RSI mod ) was calculated as ratio between jump height and jump time. Our results indicate that the relationships among different RSI variants and approach jump in volleyball players are moderate to strong (r = 0.42–0.73), with the highest correlations being observed for RSI mod from bilateral CMJ (r = 0.676–0.727). Those observations are in line with the principle of movement specificity, which suggests that the best performance indicator should be the task that best resembles the demands of the sport-specific movements. Further research is needed to reveal more about the potential of implementing these findings for training optimization through monitoring RSI and RSI mod values.

Abstract:(1) Background: The arch structure and mobility of the foot are considered injury risk factors in volleyball. However, there are limited studies presenting differences in injury prevalence and the risk of lower limb injuries in relation to the competitive level in male volleyball. Therefore, the main aim of the current study was to evaluate foot mobility (through navicular drop test) as an injury risk factor in volleyball players from different competitive levels. (2) Methods: The reliability and usefulness of navicular drop testing were initially assessed in test–retest procedures (based on a sample of eight participants and 16 feet measurements), with primary analyses conducted using foot measurements of the twelve top-level volleyball players (24 feet) and eighteen academic-level volleyball players (36 feet). The modified navicular drop test was conducted, and the feet were classified based on arch height, and injury prevalence was retrospectively assessed with a previously validated questionnaire. Chi-squared tests, receiver operating curves, and logistic regression were used as statistical methods. The navicular drop test was verified as a reliable tool by intraclass correlation coefficient (ICC) (3.1) analysis. (3) Results: There were no significant differences in injury prevalence between academic- and top-level volleyball players, though there was a significant relationship between pronated foot and injury risk independent of competitive level. Generally, for both groups, thresholds above 10 mm of the navicular drop were predictors of lower limb injuries. The risk of injury if the foot was pronated ranged from 70% (academic level) to over 90% (top-level players). However, no statistically significant effect of competitive level on the chance of injury was observed. (4) Conclusions: Our study found a high prevalence of foot injuries independently of competitive level. There was a relationship between pronation of the foot and the risk of injury. However, the risk of lower limb injury was higher in pronated top-level players. Also, a navicular drop greater than 10 mm was an excellent predictor of injuries at both competitive levels.

Abstract:Background: Functional tests (FT) are assessment tools that attempt to evaluate balance, flexibility, strength, power, speed, or agility through performance of gross motor skills. FT are frequently administered by coaches or sports medicine professionals to evaluate athletic ability, to predict performance, to identify athletes at risk for injury, or to evaluate an athlete's ability to return to sport after injury. Functional tests which can provide accurate or predictive information regarding athletic ability would be advantageous to coaching staffs or medical professionals. Purpose: The primary purpose of this study was to identify correlations between preseason FT scores and in-season game statistics in a cohort of female collegiate level volleyball (VB) players. A secondary purpose was to present FT descriptive data for this cohort based on level of competition, player position, and starter status. Study design: prospective cohort; correlational. Methods: One hundred and thirty-one female collegiate VB players representing three levels of competition completed four FT [standing long jump (SLJ), single-leg hop (SLH), lower extremity functional test (LEFT), and the Y-Balance Test - Lower Quarter (YBT-LQ)] at the start of the preseason. Player statistics were collected from team records at the completion of the season. Results: Starters performed significantly better on all tests. There were moderate negative correlations between LEFT scores and game statistics for liberos, defensive specialists, and outside hitters. There were moderate positive correlations between YBT-LQ composite scores and game statistics for liberos, defensive specialists, hitters, and middle blockers. There were also low to moderate level positive correlations between SLJ and SLH scores and game performance for outside hitters. There were low to moderate level positive correlations between SLH scores and game performance for middle blockers and opposite side hitters. Conclusions: The results of this study indicate that there are low to moderate correlations between some preseason functional test scores and some game statistics. The SLJ, SLH, LEFT, and YBT-LQ tests may help coaches with talent identification and/or may influence training strategies. Level of evidence: 3.
Prediction of ACL Injuries from Vertical Jump Kinetics in Division 1 Collegiate Athletes

Abstract:BACKGROUND: The incidence of ACL injuries continues to rise secondary to an increase in sport participation. Evidence supports the use of force plate testing to quantify kinetics during rehabilitation after injury and recovery; however, there is limited current research regarding if jump kinetics can identify athletes who are at higher risk for injury. Altered kinetics could potentially lead to abnormal force dissipation and resultant injury.PURPOSE: The purpose of this investigation was to identify whether the force-time variables from vertical jumps could predict ACL injuries in collegiate athletes.STUDY DESIGN: Retrospective cohort.METHODS: Vertical jump testing is performed by all healthy varsity collegiate athletes at several intervals throughout the athletic year at a Division I institution using a commercially available force plate system with dedicated software. Athletes who sustained an ACL injury between 1/1/15 and 6/1/19 were identified (n=16) and compared to healthy athletes who participated in the same sports (n = 262). ACL injuries were considered for this study if they occurred no more than 10 weeks after a jump test. The outcome variables were load, explode, and drive, operationally defined as the average eccentric rate of force development, average relative concentric force, and concentric relative impulse, respectively, which the system normalized to T scores. Mann-Whitney U tests were used to assess group differences for load, explode, drive, and the ratio between the variables. Logistic regression was used to determine if the battery of variables could predict whether or not an athlete would sustain an ACL injury. The p-value was set to 0.10 for the Mann-Whitney U tests, and 0.05 for the logistic regression.RESULTS: Significant differences between the ACL and healthy groups were seen for explode (p=0.08), drive (p=0.06), load:explode ratio (p=0.06), and explode:drive ratio (p=0.03). Explode and drive, when entered into the regression equation, showed the ability to predict injury, χ 2 = 6.8, df = 2, p=0.03.CONCLUSIONS: The vertical jump force plate variables were able to identify athletes who sustained an ACL injury within 66 days of testing. Athletes who sustained an ACL injury demonstrated altered kinetics and less ability to transmit forces during the vertical jump.LEVEL OF EVIDENCE: 3.
Reactive Strength Index Modified Is a Valid Measure of Explosiveness in Collegiate Female Volleyball Players

Abstract:The purpose of this study was to investigate the validity of the reactive strength index modified (RSImod) as a measure of lower body explosiveness. Fifteen female, National Collegiate Athletic Association Division I volleyball players performed vertical countermovement jumps (CMJs) while standing on a force plate. Each player performed 3 CMJs. The vertical ground reaction forces collected during each jump were used to calculate jump height, time to take-off, time to peak force, peak force, peak rate of force development, and peak power; the latter 3 variables were all normalized to body mass. Reactive strength index modified was calculated as the ratio between jump height and time to take-off. All variables, except for jump height, were then entered a factor analysis, which reduced the input data into 2 factors: a force factor and a speed factor. Although RSImod loaded more strongly onto the force factor, further analysis showed that RSImod loaded positively onto both force and speed factors. Visual analysis of the Cartesian coordinates also showed that RSImod loaded into the quadrant of greater force and speed abilities. These results indicate that the construct of RSImod, as derived from CMJ force-time data, captures a combination of speed-force factors that can be interpreted as lower body explosiveness during the CMJ. Reactive strength index modified therefore seems to be a valid measure to study lower body explosiveness.

Using an Affordable Motion Capture System to Evaluate the Prognostic Value of Drop Vertical Jump Parameters for Noncontact ACL Injury

Abstract:The American Journal of Sports Medicine, Ahead of Print. Background:Knee kinematic parameters during a drop vertical jump (DVJ) have been demonstrated to be associated with increased risk of noncontact anterior cruciate ligament (ACL) injury. However, standard motion analysis systems are not practical for routine screening. Affordable and practical motion sensor alternatives exist but require further validation in the context of ACL injury risk assessment.Purpose/Hypothesis:To prospectively study DVJ parameters as predictors of noncontact ACL injury in collegiate athletes using an affordable motion capture system (Kinect; Microsoft). We hypothesized that athletes who sustained noncontact ACL injury would have larger initial and peak contact coronal abduction angles and smaller peak flexion angles at the knee during a DVJ.Study Design:Case-control study; Level of evidence, 3.Methods:102 participants were prospectively recruited from a collegiate varsity sports program. A total of 101 of the 102 athletes (99%) were followed for an entire season for noncontact ACL injury. Each athlete performed 3 DVJs, and the data were recorded using the motion capture system. Initial coronal, peak coronal, and peak sagittal angles of the knee were identified by our software.Results:Five of the 101 athletes sustained a noncontact ACL injury. Peak coronal angles were significantly greater and peak sagittal flexion angles were significantly smaller in ACL-injured athletes (P = .049, P = .049, respectively). Receiver operating characteristic (ROC) analysis demonstrated an area under the curve of 0.88, 0.92, and 0.90 for initial coronal, peak coronal, and peak sagittal angle, respectively. An initial coronal angle cutoff of 2.96° demonstrated 80% sensitivity and 72% specificity, a peak coronal angle cutoff of 6.16° demonstrated 80% sensitivity and 72% specificity, and a peak sagittal flexion cutoff of 93.82° demonstrated 80% sensitivity and 74% specificity on the study cohort.Conclusion:Increased peak coronal angle and decreased peak sagittal angle during a DVJ were significantly associated with increased risk for noncontact ACL injury. Based on ROC analysis, initial coronal angle showed good prognostic ability, whereas peak coronal angle and peak sagittal flexion provided excellent prognostic ability. Affordable motion capture systems show promise as cost-effective and practical options for large-scale ACL injury risk screening.
Lower Back Injuries in NCAA Female Volleyball Athletes: A 5-Year Epidemiologic Characterization

Abstract:Background: Although lower back injuries (LBIs) are common among National Collegiate Athletic Association (NCAA) female volleyball athletes, their incidence and etiology has not been well-defined. Purpose: To describe the epidemiology of LBIs in collegiate female volleyball athletes over a 5-year period from the academic years 2009 to 2010 and 2013 to 2014. Study design: Descriptive epidemiology study. Methods: The incidence and characteristics of spine injuries were identified utilizing the NCAA Injury Surveillance Program database. Rates of injury were calculated as the number of injuries by the total number of athlete-exposures (AEs). AEs were defined as any student participation in any single NCAA-sanctioned practice or competition. The injury rate was computed as the number of injuries per the total number of AEs and reported as a ration of injuries per 10,000 exposures. The ratio was then reported as overall number as well as stratified for event, time of season, and athletic NCAA division. Incidence rate ratios were then calculated to compare rates between event type. Results with 95% CIs that did not include 1.0 were considered statistically significant. Results: An estimated 3384 LBIs occurred in NCAA female volleyball players during this 5-year time frame. These LBIs occurred at a rate of 4.89 injuries per 10,000 AEs. LBIs were 2.76 times more likely in preseason when compared with regular season. More injuries occurred in practice (85%) when compared with competition (15%). The outside hitter and middle blocker were the most commonly position to sustain an LBI. Almost 70% of injuries were new injuries, and another 29% were recurrent injuries. The most common mechanism of injury was equally split between contact (50.4%) and overuse (45.5%) injuries, whereas the remaining mechanisms of injury were secondary for unknown reasons (4.14%). Most players returned to play within 24 hours (72.3%) followed by 1 to 6 days (16.4%), and finally 7 to 12 days (11.3%). No patient required surgical intervention. Conclusion: The rate of LBIs was high (4.89/10,000 AEs) and injuries commonly recurred (29.2%). Most injuries were new, with most athletes returning to play with 24 hours.
COMPARISON of LOWER QUARTER Y-BALANCE TEST SCORES for FEMALE COLLEGIATE VOLLEYBALL PLAYERS BASED on COMPETITION LEVEL, POSITION, and STARTER STATUS

Abstract:Background: The Lower Quarter Y-Balance Test (YBT-LQ) is used by sports medicine professionals to measure an athlete's dynamic balance. The YBT-LQ is used by clinicians to track recovery during clinical rehabilitation, assess an athlete's readiness to return to sport after injury, and to identify athletes potentially at-risk for a time-loss injury. Normative data for the YBT-LQ are lacking for female collegiate volleyball (VB) players. The purpose of this study was to examine preseason YBT-LQ scores and their relationships to level of competition, starter status, player position, and prior lower quadrant (i.e., low back and lower extremities) injury history. Methods: One-hundred thirty-four female collegiate VB players (mean age = 19.3 ± 1.1 years) representing athletes from three levels of competition (D II = 32, D III = 77, NAIA = 25) participated in this study. Athletes reported their prior injury history and performed the YBT-LQ testing protocol. Results: NAIA and D III athletes demonstrated significantly greater reach measures on the YBT-LQ than D II athletes in several directions. Starters demonstrated significantly greater reach measures in five out of eight reach directions. Liberos/defensive specialists/setters demonstrated significantly greater posterolateral and composite reach measures bilaterally. There was no difference in reach measures based on prior history of lower quadrant (low back and lower extremities) injury. Conclusion: This study provides normative data for YBT-LQ in female collegiate volleyball players. The data presented in this report may be used by coaches and rehabilitation professionals when evaluating dynamic balance in healthy volleyball players and by clinicians to compare an injured athlete's recovery to norms. Abstract:Background: Risk for non-contact anterior cruciate ligament (ACL) injury can be assessed based on drop vertical jump (DVJ). However, biomechanics of DVJ may differ with various sporting activities. The purpose of the present study was to clarify whether biomechanical features of DVJ are different among various sporting activities in female athletes. Methods: A total of 42 female athletes, including 25 basketball, 8 soccer and 9 volleyball players, participated in the current investigation. DVJ was done for each female athlete using a three-dimensional motion analysis system which consisted of six cameras, two force plates and 46 retro-reflective markers. Kinematic and kinetic data were recorded for both limbs in each athlete. Simultaneously, frontal and sagittal plane views of the DVJ were recorded using two different high-resolution video cameras to evaluate Landing Error Scoring System (LESS) score. Three-dimensional biomechanical parameters at the knee joint and LESS were compared among three different sporting activities using ANOVA or Kruskal-Wallis test after confirming normality assumption. Thereafter post hoc Tukey or Steel-Dwass was utilized for multiple comparison. Results: Soccer players had better LESS score, and peak knee flexion angle was significantly larger in soccer players compared to the other sports. In addition, knee abduction angle at initial contact (IC), peak knee abduction angle, knee internal rotation angle, and knee abduction moment within 40 ms from IC were significantly smaller in soccer players, compared to basketball players. In terms of volleyball players, knee abduction angle at IC and knee internal rotation angle at IC were significantly larger than soccer players, whereas no significant biomechanical differences were found between basketball and volleyball players. Conclusions: From the present study, female basketball and volleyball players have worse LESS score, smaller peak knee flexion angle, greater knee abduction angle at IC and greater knee internal rotation angle at IC, compared to female soccer players. Thus, female basketball and volleyball players may have an increased risk of non-contact ACL injury during the jump-landing task, compared to soccer players. Biomechanics of DVJ depends on characteristics of the athlete's primary sport.
Reactive Strength Index and its Associations with Measures of Physical and Sports Performance: A Systematic Review with Meta-Analysis

Abstract:BackgroundReactive strength index (RSI) is used frequently in the testing and monitoring of athletes. Associations with sports performance measures may vary dependent on the task but a literature synthesis has not been performed.ObjectivesThe aim of this meta-analysis was to examine associations between RSI measured during rebound jumping tasks and measures of strength, linear and change of direction speed, and endurance performance.MethodsA systematic literature search with meta-analysis was conducted using databases PubMed, SPORTDiscus, Web of Science, and Ovid. Inclusion criteria required studies to (1) examine the relationship between RSI and an independent measure of physical or sporting performance for at least one variable; and (2) provide rebound test instructions to minimise ground contact time and maximise displacement of the jump. Methodological quality was assessed using a modified version of the Downs and Black Quality Index tool. Heterogeneity was examined via the Q statistic and I2. Pooled effect sizes were calculated using a random-effects model, with Egger's regression test used to assess small study bias (inclusive of publication bias).ResultsOf the 1320 citations reviewed, a total of 32 studies were included in this meta-analysis. RSI was significantly and moderately associated with strength (isometric: r = 0.356 [95% CI 0.209–0.504]; isotonic: r = 0.365 [0.075–0.654]; pooled strength measures: r = 0.339 [0.209–0.469]) and endurance performance (r = 0.401 [0.173–0.629]). Significant moderate and negative associations were indicated for acceleration (r = − 0.426 [− 0.562 to − 0.290]), top speed (r = − 0.326 [− 0.502 to − 0.151]), and significant large negative associations were noted for change of direction speed (r = − 0.565 [− 0.726 to − 0.404]). Heterogeneity was trivial to moderate across all measures (I2 = 0–66%), and significant for isotonic strength and change of direction speed (p < 0.1). Evidence of small study bias was apparent for both acceleration and change of direction speed (p < 0.05).ConclusionsWe identified primarily moderate associations between RSI and independent measures of physical and sporting performance, and the strength of these relationships varied based on the task and physical quality assessed. The findings from this meta-analysis can help practitioners to develop more targeted testing and monitoring processes. Future research may wish to examine if associations are stronger in tasks that display greater specificity.

The Relationship between Landing Error Scoring System Performance and Injury in Female Collegiate Athletes

Abstract:BACKGROUND: The Landing Error Scoring System (LESS) is a standardized tool used to identify aberrant biomechanical movement patterns during a jump-landing task. Prior authors have examined the value of the LESS in identifying ACL injury risk in athletic populations. Yet, no study has evaluated the association between LESS performance and incidence of any type of lower extremity injury in female collegiate athletes across multiple sports.PURPOSE: The purpose of this study was to examine the association between LESS performance as measured with a markerless motion-capture system and lower extremity injury in female collegiate athletes.STUDY DESIGN: Prospective cohort study.METHODS: One hundred and ten DI female collegiate athletes (basketball, n=12; field hockey, n=17; gymnastics, n=14; lacrosse, n=27; softball, n=23; volleyball, n=17) completed a jump-landing test prior to the start of their sport seasons. The LESS was automatically scored using a Microsoft Kinect sensor and Athletic Movement Assessment software (PhysiMax®). Participants were tracked throughout one competitive season for incidence of time-loss lower extremity injury. A Receiver Operating Characteristic curve determined the optimal cutpoint for the total LESS score for predicting injury. Pearson's Chi squared statistics examined the association between injury and LESS total scores >5. The Fisher exact test evaluated group differences for the frequency of receiving an error on individual LESS test items.RESULTS: Female collegiate athletes with LESS scores >5 were not more likely to be injured than those with scores ≤5 (χ2=2.53, p=0.111). The relative risk of injury to this group was 1.78 (95% CI=0.86, 3.68) while the odds ratio was 2.10 (95% CI=0.83, 5.27). The uninjured group was more likely to receive an error on lateral trunk flexion at initial contact than the injured group (p=0.023).CONCLUSION: The LESS total score was not associated with an increased odds of lower extremity injury in this cohort of female collegiate athletes. Future studies to examine the association between individual LESS item scores and injury are warranted.LEVEL OF EVIDENCE: 1b.
LOWER QUARTER- AND UPPER QUARTER Y BALANCE TESTS AS PREDICTORS OF RUNNING-RELATED INJURIES IN HIGH SCHOOL CROSS-COUNTRY RUNNERS

Abstract:Background: While cross-country running is a popular interscholastic sport, it also has a high incidence of running-related injuries (RRIs). Recent literature suggests that functional tests may identify athletes at increased risk of injury. The Y-Balance Test (YBT) is an objective measure used to assess functional muscle strength, balance, and expose asymmetries between tested limbs. Purpose/hypothesis: The purpose of this study was to determine if the YBT could predict RRI in high school cross-country runners. It was hypothesized that an asymmetric right (R)/left (L) YBT reach distance for the lower or upper extremities would be associated with an increased risk of RRI. Study design: Prospective observational cohort. Methods: One hundred forty-eight athletes (80 girls, 68 boys) who competed in interscholastic cross-country in Southern California during the 2015 season participated in the study. Prior to the cross-country season, the runners completed Lower-Quarter YBT (LQ-YBT) and Upper-Quarter YBT (UQ-YBT) testing to assess lower and upper extremity asymmetry, respectively. The runners were prospectively monitored for RRI occurrence throughout the season using the Daily Injury Report form. Results: Forty-nine runners (33.1%) incurred a RRI during the 2015 season, with the lower leg (shin/calf) and knee the most common RRI sites. Girls had a higher RRI occurrence (38.8%) than boys (26.5%) (p = 0.12). Boys had greater raw scores for LQ-YBT R and L anterior (ANT), posteromedial (PM), posterolateral (PM) and composite reach distances than girls (p<0.05). With the exception of normalized superolateral reach distance, boys had significantly greater scores for raw and normalized R and L UQ-YBT reach distances and raw composite scores than girls (p<0.05). After adjusting for prior RRI, while boy runners with a LQ-YBT PM reach difference ≥4.0 cm were five times more likely to incur a RRI (Adjusted odds ratio [AOR] = 5.05, 95% CI: 1.3-19.8; p = 0.02), girl runners with a UQ-YBT inferolateral (IL) reach difference ≥ 4.0 cm were 75% less likely to incur a RRI (AOR = 0.25, 95% CI: 0.1-0.7; p = 0.005). By lower extremity body region, boy runners with a UQ-YBT superolateral (SL) reach difference ≥ 4.0 cm were seven times more likely to incur a hip/thigh/knee RRI [AOR] = 7.20, 95% CI: 1.1-45.6; p = 0.002). Conclusion: Greater lower extremity (PM) or upper extremity (SL) reach distance asymmetry, as measured by the LQ-YBT or UQ-YBT, respectively, were associated with RRI in boy high school cross-country runners.
