Abstract:Background: Gastrointestinal perforation in extremely low birth weight infants, characterized by its rapid onset, multiple complications, and critical condition, poses a significant risk of infant mortality. The aim of this study was to investigate the clinical characteristics of pneumoperitoneum in extremely low birth weight infants (ELBWI) and explore the risk factors associated with gastrointestinal perforation in very low birth weight preterm infants. Additionally, we shared our surgical experiences in managing gastrointestinal perforation among extremely low birth weight infants. Methods: The Department of Neonatology at Chengdu Women and Children's Central Hospital conducted a retrospective study on gastrointestinal perforation in extremely low birth weight infants (birth weight <1000 g) who were admitted between 2014 and 2021. After baseline analysis and comparing it with the control group, we identified the risk factors associated with gastrointestinal perforation in ELBWI by multiple logistic regression analysis. The Kaplan-Meier analysis was performed to assess the adverse effect of gastrointestinal perforation for survival in ELBW infants. Cox multivariate regression analysis was used to evaluate hazard level of different variables for ELBW infants survival. Results: Hemodynamically significant patent ductus arteriosus (hsPDA)(p = 0.043, OR = 2.779) and sepsis (p = 0.014, OR = 2.265) were significant risk factors for gastrointestinal perforation in extremely low birth weight infants. The Cox proportional hazard model revealed that intraventricular hemorrhage (HR = 2.854, p＜0.001) Sepsis (HR = 1.645, p = 0.015) and gastrointestinal perforation (HR = 1.876, p = 0.008) had detrimental effects on the survival of extremely low birth weight infants; conversely, ibuprofen (HR = 0.304, p＜0.001) and blood transfusion (HR = 0.372, p＜0.001) are beneficial factors for their survival. The preoperative indicators of infection in infants with spontaneous intestinal perforation (SIP) were significantly better than those in the necrotizing enterocolitis (NEC) group (p < 0.05). Conclusions: Gastrointestinal perforation poses a significant threat the survival of extremely low birth weight (ELBW) infants, with hsPDA and sepsis serving as predisposing factors for gastrointestinal perforation. The gastrointestinal perforation caused by various diseases exhibits distinct clinical characteristics, necessitating tailored surgical approaches based on operative conditions.

Abstract:A retrospective analysis was performed from May 2003 to October 2021 at our hospital. One hundred and eleven neonates diagnosed with gastric perforation were categorized into non-survivor and survivor groups based on prognosis. The mortality rate was 23.42%. Logistic regression was used to analyze the factors influencing the early neonatal death. The non-survivor groups showed more abdominal distension, vomiting, hypoxia, acidosis, hyperkalemia, and coagulation abnormalities. Serum potassium concentration (odds ratio [OR] 2.148, P = .043) was an independent risk factor; however, arterial partial pressure of oxygen (OR 0.977, P = .024), bicarbonate (OR 0.779, P = .023), and platelet count (OR 0.986, P = .014) had protective effects on neonatal death. Based on this, a visualized nomogram prediction model was constructed, patients who had a nomogram score of more than 163.787 was considered to have high risks of mortality, the area under the curve of this was up to 0.828 (95% confidence interval [0.733, 0.923]), the sensitivity was 80.0%, and the specificity was 76.9%.
Risk Factors of Gastrointestinal Perforation with a Poor Prognosis

Abstract:Background: Despite medical progress, mortality from gastrointestinal perforation was relatively high. Our study's objective was to identify risk factors associated with a poor prognosis for gastrointestinal perforation.Methods: Patients diagnosed with gastrointestinal perforation at the Longchuan County People's Hospital between January 2019 and February 2022 were the subject of a retrospective analysis of their laboratory data. Patients were grouped based on length of hospital stay, septic shock, and mortality.Results: A total of 240 patients participated in our study. Using univariate and multivariate analysis, we identified several risk factors for gastrointestinal perforation associated with a dismal prognosis. Lower digestive tract perforation (OR=2.418, 95% CI 1.119-5.227, P=0.025), low total protein (OR=0.934, 95% CI 0.879-0.992, P=0.026) and low hemoglobin (OR=0.985, 95% CI 0.971-0.999, P=0.039) were linked to a longer length of stay, especially hemoglobin (OR=0.978, 95% CI 0.966-0.991, P=0.001) in upper digestive tract. High ratio of neutrophils to lymphocytes (NLR) (OR=1.043, 95% CI 1.012-1.076, P=0.007), high lymphocyte-to-monocyte ratio (LMR) (OR=2.158, 95% CI 1.495-3.115, P<0.001) and low prognostic nutrition index (PNI) (OR=0.814, 95% CI 0.751-0.833, P<0.001) predicted septic shock. In upper digestive tract, PLR (OR=1.001, 95% CI 1.000-1.002, P=0.067), LMR (OR=2.160, 95% CI 1.440-3.240, P<0.001) and PNI (OR=0.843, 95% CI 0.767-0.926, P<0.001) were risk factors for septic shock, and total protein (OR=0.796, 95% CI 0.686-0.923, P=0.003) was a risk factor for septic shock in lower digestive tract. High NLR (OR=1.056, 95% CI 1.019-1.093, P=0.003), high LMR (OR=1.760, 95% CI 1.177-2.632, P=0.006) and low PNI (OR=0.832, 95% CI 0.754-0.918, P<0.001) were the risk factors of mortality. In subgroup analysis of perforation site, albumin (OR=0.820, 95% CI 0.719-0.934, P=0.003) and LMR (OR=1.506, 95% CI 1.069-2.123, P=0.019) were risk factors for mortality in upper digestive tract and PNI (OR=0.636, 95% CI 0.445-0.908, P=0.013) was a risk factor for mortality in lower digestive tract.Conclusion: Our research found that the perforation site, total protein, albumin, hemoglobin, NLR, LMR, PLR and PNI were risk factors for gastrointestinal perforation with a poor prognosis.

Abstract:We experimentally demonstrate frequency translation of a nonclassical optical field via four-wave mixing (Bragg-scattering process) in a photonic crystal fiber (PCF). The high nonlinearity and the ability to control dispersion in PCF enable efficient translation between nearby photon channels within the visible to-near-infrared spectral range, useful in quantum networks. Heralded single photons at 683 nm were translated to 659 nm with an efficiency of 28.6±2.2 percent. Second-order correlation measurements on the 683- and 659-nm fields yielded g(683)(2) (0)=0.21±0.02 and g(659)(2) (0)=0.19±0.05, respectively, showing the nonclassical nature of both fields.
Risk factors for increased mortality rate of newborns with congenital diaphragmatic hernia in the single medical center

Abstract:Objective:To study the risk factors increasing mortality rate in newborns with congenital diaphragmatic hernia (CDH) after surgery.Method:From March 2009 to March 2018, infants diagnosed with CDH and received surgery in the Department of Neonatology of our hospital were enrolled for retrospective analysis. According to the clinical outcome, they were assigned into the survival group and the death group. The clinical conditions and treatments of the two groups were compared and the risk factors of mortality were analyzed.Result:A total of 58 infants with CDH were enrolled, including 34(58.6%) male cases and 24(41.4%) female cases. 48 cases(82.8%) survived and 10 cases(17.2%) died. Compared with the death group, the survival group had less proportion of infants with pH<7.15 on admission (4/48 vs. 9/10), fewer pulmonary vasodilators usage (24/48 vs. 9/10), fewer comorbidity of pulmonary hypertension (2/48 vs. 7/10) and bigger age receiving surgery [4(2,6)d vs. 1(0,4)d]( P<0.05). Multivariate Logistic regression analysis showed that pH<7.15 on admission ( OR=0.009, 95% CI 0.000~0.310) and pulmonary hypertension ( OR=0.041, 95% CI 0.020~0.880) were both independent risk factors for death in infants with CDH ( P<0.05). Conclusion:The survival rate of CDH after surgery is high. pH<7.15 on admission and pulmonary hypertension are the independent risk factors for death in infants with CDH.

Abstract:Background Limited data exists regarding the mortality of very low birth weight (VLBW) neonates with congenital diaphragmatic hernia (CDH). This study aims to quantify and determine predictors of mortality in VLBW neonates with CDH. Methods This analysis of 829 U.S. NICUs included VLBW [birth weight ≤1500g] neonates, born 2011-2021 with and without CDH. The primary outcome was in-hospital mortality. A generalized estimating equation regression model determined the adjusted risk ratio (ARR) of mortality. Results Of 426,140 VLBW neonates, 535 had CDH. In neonates with CDH, 48.4% had an additional congenital anomaly vs 5.5% without. In-hospital mortality for neonates with CDH was 70.4% vs 12.6% without. Of those with CDH, 73.3% died by day of life 3. Of VLBW neonates with CDH, 38% were repaired. A subgroup analysis was performed on 60% of VLBW neonates who underwent delivery room intubation or mechanical ventilation, as an indicator of active treatment. Mortality in this group was 62.7% for neonates with CDH vs 16.4% without. Higher Apgars at 1 minute and repair of CDH were associated with lower mortality (ARR 0.91; 95%CI 0.87,0.96 and ARR 0.28; 0.21,0.39). The presence of additional congenital anomalies was associated with higher mortality (ARR 1.14; 1.01,1.30). Conclusion These benchmark data reveal that VLBW neonates with CDH have an extremely high mortality. Almost half of the cohort have an additional congenital anomaly which significantly increases the risk of death. This study may be utilized by providers and families to better understand the guarded prognosis of VLBW neonates with CDH. Type of Study/Level of Evidence Level II.

Clinical Predictors of Spontaneous Intestinal Perforation vs Necrotizing Enterocolitis in Extremely and Very Low Birth Weight Neonates

Abstract:Purpose: Spontaneous intestinal perforation (SIP) and necrotizing enterocolitis (NEC) are distinct disease processes associated with significant morbidity and mortality. Initial treatment, laparotomy (LP) versus peritoneal drainage (PD), is disease specific however it can be difficult to distinguish these diagnoses preoperatively. We investigated clinical characteristics associated with each diagnosis and constructed a scoring algorithm for accurate preoperative diagnosis. Methods: A cohort of extreme and very low birth weight (<1500 g) neonates surgically treated for SIP or NEC between 07/2004-09/2022 were reviewed. Clinical characteristics included gestational age (GA), birth weight (BW), feeding history, physical exam, and laboratory/radiological findings. Intraoperative diagnosis was used to determine SIP vs NEC. Pre-drain diagnosis was used for patients treated with PD only. Results: 338 neonates were managed for SIP (n = 269, 79.6%) vs NEC (n = 69, 20.4%). PD was definitive treatment in 146 (43.2%) patients and 75 (22.2%) patients were treated with upfront LP. Characteristics associated with SIP included younger GA, younger age at initial laparotomy or drainage (ALD), and history of trophic or no feeds. Multivariate logistic regression determined pneumatosis, abdominal wall erythema, higher ALD and history of feeds to be highly predictive of NEC. A 0-8-point scale was designed based on these characteristics with the area under the receiver operating characteristic curve of 0.819 (95% CI 0.756-0.882) for the diagnosis of NEC. A threshold score of 1.5 had a 95.2% specificity for NEC. Conclusion: Utilizing clinical characteristics associated with SIP & NEC we developed a scoring system designed to assist surgeons accurately distinguish SIP vs NEC in neonates. Nomogram for assessment of risk factors for early mortality after laparotomy in neonatal gastric perforation from a tertiary care unit

Abstract:Abstract Purpose Summarize the clinical features of neonatal gastric perforation and establish a nomogram model to predict the risk factors of early mortality after laparotomy in neonatal gastric perforation (NGP) from a tertiary care unit. Methods Retrospective analysis was performed on NGP diagnosed in our hospital between May 2003 and October 2021. All patients underwent laparotomy, and according to the prognosis, they were divided into non-survival and survival groups. All clinical characteristics, preoperative laboratory features, intraoperative situation and outcomes were collected from electronic medical records. We conducted logistic regression analyses to identify the independent factors that contribute to early neonatal death after laparotomy and the nomogram prediction model was constructed. Results A total of 111 patients with NGP were included in our study, the mortality was 23.42% (26/111). Six independent mortality risk factors were identified: APTT(OR,1.015;95% CI,1.001,1.138;p = 0.039), PaO2 (OR,0.977;95%CI,0.957,0.997;p = 0.022), Hco3−(OR,0.777;95%CI,0.616,0.979;p = 0.032),PLT(OR,0.989;95%CI,0.979,0.998;p = 0.022), SPC(OR,2.693;95%CI,1.221,5.942;p = 0.014), Combined with NEC (OR,0.040; 95% CI,0.004,0.421 ; p = 0.007). A nomogram model was constructed based independent prognostic risk factors, and its AUC under ROC curve was 0.886(95%CI,0.806,0.965;P = 0.000), which had a good degree of discrimination. Patients who had a nomogram score of more than 178.548 was considered to take high risks of mortality, and the sensitivity and specificity of identifying postoperative NGP mortality were 91.8% and 76.9%. The correction curves evaluation results showed a high consistency of the model. Conclusion APTT, PaO2, Hco3−,PLT,SPC and combined with NEC change, are independently associated with an increased risk of early mortality in neonates with neonatal gastric perforation after laparotomy. The nomogram model constructed in this study can be used as a tool to predict the risk of mortality, predict postoperative survival, and help to develop an individualized treatment plan.
Identifying Preoperative Clinical Characteristics of Unexpected Gastrointestinal Perforation in Infants—A Retrospective Cohort Study

Abstract:Infants presenting with unexpected pneumoperitoneum upon abdominal X-ray, indicating a gastrointestinal perforation (GIP), have a surgical emergency with potential morbidity and mortality. Preoperative determination of the location of perforation is challenging but will aid the surgeon in optimizing the surgical strategy, as colon perforations are more challenging than small bowel perforations. Therefore, the aim of this study is to provide an overview of preoperative patient characteristics, determine the differences between the small bowel and colon, and determine underlying causes in a cohort of infants with unexpected GIP. Methods: All infants (age ≤ 6 months) who presented at our center with unexpected pneumoperitoneum (no signs of pneumatosis before) undergoing surgery between 1996 and 2024 were retrospectively included. The differences between the location of perforation were analyzed using chi-squared and t-tests. Bonferroni correction was used to adjust for multiple tests. Results: In total, 51 infants presented with unexpected pneumoperitoneum at our center, predominantly male (N = 36/51) and premature (N = 40/51). Among them, twenty-six had small bowel, twenty-two colon, and three stomach perforations. Prematurity (p = 0.001), birthweight < 1000 g (p = 0.001), respiratory support (p = 0.001), and lower median arterial pH levels (p = 0.001) were more present in patients with small bowel perforation compared with colon perforations. Pneumatosis intestinalis was more present in patients with colon perforation (p = 0.004). All patients with Hirschsprung disease and cystic fibrosis had colon perforation. The final diagnoses were mainly focal intestinal perforations (N = 27/51) and necrotizing enterocolitis (N = 9/51). Conclusion: Infants with unexpected GIP, birthweight <1000 g, and prematurity have more risk for small bowel perforation. In case of colon perforation, additional screening (for Hirschsprung and cystic fibrosis) should be considered.

Abstract:Pneumatosis intestinalis and portal venous gas were previous harbingers of death, however our study reveals factors which predict mortality and surgical intervention. Backgrounds This study investigated the incidence of, and mortality and management outcomes following, pneumatosis intestinalis and/or portal venous gas on computed tomography. Methods A retrospective study of patients identified with pneumatosis intestinalis and/or portal venous gas on computed tomography at a quaternary centre (2013–2021) was performed. Data relating to clinical presentation (including quick sequential organ failure assessment score), co‐morbidities (Charlson Comorbidity Index), biochemical data (including peak lactate level), and radiological findings, were obtained. Factors associated with these were assessed by logistic regression. Results From 16 428 scans, 107 (0.65%) demonstrated pneumatosis intestinalis and/or portal venous gas (mean 65.2 years [SD 15.2]; 60 [56%] male). Overall, 37 patients (35%) had both findings present. Thirty‐three deaths (31%) were recorded. Fifty‐four patients (51%) underwent surgery. Death was associated with quick sequential organ failure assessment score (score 1: OR 5.71, 95% CI 1.31–24.87; score 2: OR 10.00, 95% CI 1.94–51.54), Charlson Comorbidity Index ≥5 (OR 2.86, 95% CI 1.19–6.84), peak lactate ≥2.6 mmol/L (OR 14.53, 95% CI 4.39–48.14), and concomitant pneumatosis intestinalis and portal venous gas (OR 8.25, 95% CI 3.04–22.38). The presence of free peritoneal fluid (OR 3.23, 95% CI 1.44–7.28) or perforated viscus (OR 5.10, 95% CI 1.05–24.85) were the only predictors for surgery. Conclusion Pneumatosis intestinalis and portal venous gas are rare findings. Despite traditionally portending a poor prognosis, mortality occurred in only one‐third of patients. There were clear indicators of mortality viz. sepsis severity, comorbidities, and concomitant pneumatosis intestinalis and portal venous gas. Factors predicting surgery warrant further investigation.

Abstract:【Objective】To explore the early clinical diagnosis of gastrointestinal perforation in preterm neonates.【Methods】Ten cases of premature gastrointestinal perforation were registered in five years.Based on their general conditions,the incidence time,early clinical manifestation,diagnostic methods and prognosis of gastrointestinal perforation and possible precipitating factor of perforation,the early clinical diagnosis of gastrointestinal perforation in preterm neonates were analyzed.【Results】1)Totally,there were ten cases(6 males) with the average gestational age of 32 weeks,average body weight of 1 555 g,which included five small gestational age(SGA) perterms and 7 babies were one of the twin infants(7/10).2)Perforation occurred during 9 hours to 70 hours after birth(average=26 hours).The first clinical symptoms of the perforation were vomiting and abdominal distension;typical clinical manifestation appearance time was from 18 hours to 96 hours after birth(average=48 hours).The typical clinical manifestations included abdominal distension,high tension,abdominal wall pallidness,bowel sounds disappearence.The abdominal orthostatic X-ray images showed a great quantity of free gas under the flat diaphragm,declined liver bound,and abdominal paracentesis in some cases.Two cases were with good post-surgical outcomes and 7 cases died(mortality=7/9);1 case was lost of following-up.3)Induce factors of the gastrointestinal perforation:infection followed by gastrointestinal perforation(2/10);spontaneous gastrointestinal perforation(8/10);gastrointestinal perforation followed by infection(6/8).Six cases began to feed before onset symptoms appeared.Five cases had noninvasive ventilator before gastrointestinal perforation(5/10).【Conclusions】Small gestational age and low birth weight are high-risk factors of the gastrointestinal perforation.One to two days after birth and during severe infection is the high-risk occurrence time.Vomiting and/or abdominal distension are the first clinical symptoms in premature infants with perforation.Congenital poor growth in digestive tract is the foundation of perforation in premature infants.Infection,feeding and noninvasive ventilation may be precipitating factors.Once above-mentioned conditions appear in premature infants,we should be precautious about the gastrointestinal perforation in clinic.

Abstract:Background This study aimed to develop a prediction model for intestinal perforation from Meconium ileus (MI) based on findings from plain X-ray images. Methods Very low birth weight (VLBW) infants with MI hospitalized in two tertiary centers between 2011 and 2022 were included in this study. We retrospectively reviewed clinical parameters and assessed plain X-ray images from 0 to 5 days of age. The standardized transverse diameter of intestinal gas (STDI) was calculated by dividing the largest diameter of the intestinal gas by the distance from the upper edge of L1 to the lower edge of L4. We then compared the STDI of patients with and without intestinal perforation. Results Among 81 VLBW infants with MI, intestinal perforation occurred in 6 (7%). Among known risk factors, significant differences were observed between the two groups regarding pregnancy-induced hypertension (PIH) (p=0.03), weeks of gestation (p<0.01), birthweight (p=0.02), and indomethacin administration (p<0.01). The mortality rate was higher in the perforation group (33%) than in the non-perforation group (3%) (p=0.021). There were significant differences between the perforated and non-perforated groups regarding STDI except at 0 days of age. The positive and negative predictive cut-off values of STDI were respectively 0.08 and 0.93 on day 0, 0.30 and 1.00 on day 1, 0.33 and 0.97 on day 2, 0.33 and 1.00 on day 3, 0.29 and 1.00 on day 4, and 0.33 and 0.98 on day 5, respectively. Conclusions Our novel prediction model, using STDI, predicted intestinal perforation in VLBW infants with MI. Levels of Evidence Level III

Abstract:Necrotizing enterocolitis (NEC) and spontaneous intestinal perforation (SIP) are severe gastrointestinal complications of prematurity. The clinical presentation and treatment of NEC and SIP (peritoneal drain vs laparotomy) can overlap; however, the pathogenesis is distinct. Therefore, a patient initially treated for SIP can subsequently develop NEC. This phenomenon has only been described in case reports, and no risk factor evaluation exists. We evaluate clinical characteristics, risk factors, and outcomes of patients treated for a distinct episode of NEC after SIP.

Clinical Characteristics and Prognoses of Neonatal Alimentary Tract Perforation

Abstract:Objective To explore the clinical characteristics and prognoses of neonatal alimentary tract perforation.Methods Retrospective review was conducted for 65 neonatal cases with alimentary tract perforation confirmed intraoperatively from January 2015 to December 2016.There were 43 boys and 22 girls.They were premature (n =45) and full-term (n =20).The perforations were gastric (n=7,group A),intestinal (n=34,group B),colonic (n=22,group C) and biliary tract (n=1,group D).The major clinical manifestations were abdominal distention and pneumoperitoneum.Results Gastric perforation tended to occur at the age of ＜5 days.Premature alimentary tract perforation was located in small intestine (n =29,64.4％).And the causes were neonatal necrotizing enterocolitis (NEC,n=26,57.8％),H irschsprung's disease (HSCR,n =5,11.1％) and idiopathic intestinal perforation (n =4,8.9 ％).For term infants,alimentary tract perforation was predominantly located in colon (n =11,55.0％).And the causes were Hirschsprung's disease (n =4,20.0％),congenital defects of gastric musculature (n=4,20.0％) and neonatal necrotizing enterocolitis (n =3,15.0％) respectively.No significant difference existed in distribution among groups A,B and C according to birth weight (P＞0.05).Preoperative pH value of group B (7.31 ± 0.10) was significantly lower than that of group C (7.38 ± 0.12) and preoperative hemoglobin of group C (117.6 ± 31.2) g/L was significantly lower than that of group A (161.1 ± 32.9) g/L and B (137.8 ± 34.5) g/L.No significant difference existed in preoperative levels of lactic acid or albumin (P＞0.05) among groups A,B and C.And 4/65(6.2％) died and no significant difference existed in mortality rate with regards to gestational age,birth weight or perforation site.Eight (13.1％) cases had incision infections.Conclusions Neonatal alimentary tract perforation occurs frequently in premature boys.Premature alimentary tract perforation is located predominantly in small intestine and the etiology is NEC.For term infants,alimentary tract perforation is found predominantly in colon and the etiologies include HSCR and congenital defects of gastric musculature.A better prognosis of alimentary tract perforation may be achieved after active,timely and standardized treatments.

Abstract:The main aim of this article is to highlight the clinical features indicating gastric perforation in neonates so that prompt surgery can provide a good outcome for an otherwise fatal condition. Data was collected retrospectively from all neonates who presented to our tertiary care institute with subsequent diagnosis of gastric perforation from January 2020 to December 2023 (three years). Simple statistical analysis involving sums, means, averages, and percentages was used. Five neonates were operated over a period of three years with a diagnosis of gastric perforation. Two of them were spontaneous. Of the remaining three, each one was associated with malrotation, prematurity, and COVID-19. All five cases could be diagnosed with the finding of free gas in the peritoneum on the abdominal radiograph. Overall mortality was 60% (three of five neonates). Neonatal gastric perforation typically occurs in the first week of life, specifically within the second to seventh day. Symptom onset is usually sudden, with abdominal distension as the first sign, with acidic contents causing severe peritonitis and rapid progression to sepsis and shock. Early diagnosis with subsequent timely resuscitation and surgical repair is crucial to good outcomes. Massive pneumoperitoneum on abdominal radiographs with typical signs in a neonate should raise suspicion of gastric perforation, especially in the first week of life.
Surgical Outcomes in Adults with Acute Small Bowel Perforation and Risk Factors for Mortality: A Single-Center Retrospective Cohort Study.

Abstract:Abstract Background Small bowel perforation is a life-threatening surgical emergency even after immediate surgical intervention, while the surgical outcomes are rarely discussed in the literature due to relatively low incidence. This study aimed to investigate the surgical outcomes of small bowel perforation and risk factors for mortality.Methods Consecutive patients with small bowel perforation confirmed by emergency surgery at Zhongshan Hospital, Fudan University from February 2011 to May 2020 were analyzed retrospectively. Clinical features, laboratory indicators, surgical findings and pathology were reviewed based on the medical records. Results 199 patients were included in this study, of which 50 patients underwent perforation repair, 117 underwent primary anastomosis, and the other 32 underwent small bowel ostomy. 52.3% (104/199) patients transferred to ICU just after surgery and malignant tumor was the leading cause responsible for perforation in these patients (40.4%, 42/104), although foreign bodies ingestion (27.1%, 54/199) were most common for all cases in this study. The overall postoperative morbidity and mortality rate were 54.3% (108/199) and 10.6% (21/199) respectively, which were higher in the ICU group (74.0% (77/104) and 19.2% (20/104), respectively). Malignant tumor related perforation (OR, 3.567; 95%CI, 1.175-10.823; P=0.025) and high postoperative arterial blood lactate (LAC) level (OR,1.583; 95%CI, 1.127-2.225; P=0.008) were independent risk factors for post-operative mortality for patients transferred to ICU. Conclusion Small bowel perforation is associated with significant morbidity and mortality rates after emergency surgery, especially for patients transferred to ICU. Malignant tumor related perforation, as well as higher postoperative blood lactate had increased risk of postoperative mortality.

Abstract:Background: Convincing evidence on the risk factors for perioperative death of small bowel perforation is lacking. This study aimed to investigate the etiologies of small bowel perforation and risk factors for perioperative mortality.Methods: Consecutive patients with small bowel perforation confirmed by emergency surgery at Zhongshan Hospital, Fudan University from January 2015 to December 2019 were analyzed retrospectively.Results: Of the 85 patients included in this study, ingestion of foreign bodies (19, 22.4%), lymphoma (17, 20.0%) and metastatic cancer (13, 15.3%) were the most common causes for small bowel perforation. Of them, 23 patients underwent perforation repair, 46 underwent segmental resection with primary anastomosis, and the other 16 underwent small-bowel ostomy. The operation time was the shortest for the perforation repair procedure ( P <0.001), and the hospital stay was the longest for patients undergoing ostomy ( P =0.010). Altogether 11 patients died postoperatively with a perioperative mortality rate of 12.9%. Serum procalcitonin ( P =0.037), blood lactate ( P <0.001) and APACHE II score ( P =0.0032) were higher in the dead than those in survivors.Conclusions: Foreign body ingestion, lymphoma and metastatic neoplasm were the most common causes for small bowel perforation. Serum precalcitonin, blood lactate and APACHE II score seem to be risk factors of perioperative mortality.†These authors contributed equally to this work

Abstract:Background Although small-bowel perforation is a life-threatening emergency even after immediate surgical intervention, studies have rarely investigated surgical outcomes due to its relatively low incidence. This study aimed to investigate the outcomes of emergency surgery for patients with small-bowel perforation transferred to the intensive care unit (ICU) and the risk factors for mortality. Methods Consecutive patients with small-bowel perforation who were confirmed via emergency surgery and transferred to the ICU in Zhongshan Hospital, Fudan University (Shanghai, China) between February 2011 and May 2020 were retrospectively analysed. Medical records were reviewed to determine clinical features, laboratory indicators, surgical findings, and pathology. Results A total of 104 patients were included in this study, among whom 18 (17.3%), 59 (56.7%), and 27 (26.0%) underwent perforation repair, segmental resection with primary anastomosis, and small-bowel ostomy, respectively. Malignant tumours were the leading cause of perforation in these patients (40.4%, 42/104). The overall post-operative complication rate and mortality rates were 74.0% (77/104) and 19.2% (20/104), respectively. Malignant tumour-related perforation (odds ratio [OR], 4.659; 95% confidence interval [CI], 1.269-17.105; P = 0.020) and high post-operative arterial blood-lactate level (OR, 1.479; 95% CI, 1.027-2.131; P = 0.036) were identified as independent risk factors for post-operative mortality in patients with small-bowel perforation transferred to the ICU. Conclusions Patients with small-bowel perforation who are transferred to the ICU after emergency surgery face a high risk of post-operative complications and mortality. Moreover, those patients with malignant tumour-related perforation and higher post-operative blood-lactate levels have poor prognosis.

Abstract:Background: Meconium peritonitis occurs when meconium leaks into the peritoneal cavity as a result of intrauterine gastrointestinal perforation. In this study, we aimed to evaluate the results of newborn patients who were followed and treated due to intrauterine gastrointestinal perforation in the pediatric surgery clinic. Methods: All newborn patients who were followed up and treated for intrauterine gastrointestinal perforation in our clinic between December 2009-2021 were analyzed retrospectively. Newborns who had no congenital gastrointestinal perforation were not included in our study. The data were analyzed using NCSS (Number Cruncher Statistical System) 2020 Statistical Software. Results: Within twelve years, intrauterine gastrointestinal perforation was detected in 41 newborns, including 26 (63.4%) males, and 15 (36.6%) patients who were operated on in our pediatric surgery clinic. Surgical findings of 41 patients diagnosed with intrauterine gastrointestinal perforation revealed the presence of volvulus (n=21), meconium pseudocyst (n=18), jejunoileal atresia (n=17), malrotation-malfixation anomaly (n=6), volvulus due to internal hernia (n=6), Meckel`s diverticulum (n=2), gastroschisis (n=2), perforated appendicitis (n=1), anal atresia (n=1), and gastric perforation (n=1). Eleven patients (26.8%) died. Total intubation time was significantly higher in deceased cases. Postoperatively, deceased cases passed their first stool significantly earlier than surviving newborns. Besides, ileal perforation was seen significantly more frequently in deceased cases. However, the frequency of jejunoileal atresia was significantly lower in the deceased patients. Conclusions: Although sepsis has been held primarily responsible for the deaths in these infants from past to present, insufficiency in lung capacity necessitating intubation negatively affects their survival. Early passage of stool is not always an indicator of good prognosis after the operation, and patients may die due to malnutrition and dehydration, even after they are discharged after feeding, defecating and having weight gain.
