Abstract:Background: F-actin is involved in the progression of ischemic stroke and is associated with the disruption of the blood-brain barrier. In this article, we evaluated serum F-actin as a biomarker in stroke severity and early neurological deterioration (END) in acute ischemic stroke. Methods: In this study, serum F-actin was measured in consecutively collected 140 AIS patients and 144 healthy controls matched in gender and age by ELISA. Early neurological deterioration (END) was defined as the deterioration of neurological dysfunction within 72 hours of admission, with an increase of >= 4 points in the NIHSS score. Severe stroke was defined as a NIHSS score >8 at admission. Results: The serum F-actin level in AIS was significantly higher than healthy controls ( p = 0.041). In large-artery atherosclerosis stroke and cardioembolic stroke, serum F-actin were significantly higher than that in small artery occlusion stroke (p adjust = 0.019, p adjust < 0.001, respectively).F-actin level above the critical value ( >1.37 mu g/L) was significantly associated with severe stroke (OR, 3.015; 95 %CI, 1.014 -8.963; p = 0.047) . In addition, elevated level of F-actin was significantly associated with END (OR, 1.323; 95 % CI, 1.001-1.747, p = 0.049). When the level of F-actin was above the critical value ( >2.17 mu g/L), its association with END remained significant (OR, 6.303; 95 %CI, 2.160-18.394; p < 0.001) . Conclusion: F-actin is an important blood biomarker in the early stage of AIS, and high levels of F-actin are valuable in determining the severity of stroke and predicting early neurological deterioration.
Relationship between iron-catalysed lipid peroxidation potential and human sperm function.

Abstract:The relationship between lipid peroxidation and the functional competence of human spermatozoa has been investigated in a cohort of 31 infertility patients. Lipid peroxidation was assessed using a sensitive fluorometric assay for the generation of malondialdehyde in response to the presence of a ferrous ion promoter. Sperm function was evaluated by monitoring the movement characteristics of these cells and their capacity for sperm-oocyte fusion. Each sample was separated into high- and low-density sperm populations on discontinuous, two-step (40%:80%), Percoll gradients prior to analysis. The way in which individual ejaculates fractionated on these gradients was highly positively correlated (P < 0.001) with the lipoperoxidation status of the spermatozoa; the greater the potential for malondialdehyde generation, the higher the proportion of cells entering the low density region of the gradients. The lipoperoxidation potential of the freshly prepared spermatozoa was also highly predictive (P = 0.0001) of their capacity for movement at 3 and 24 h and their ability to exhibit sperm-oocyte fusion in response to the ionophore A23187. The potential for malondialdehyde generation in the 40% and 80% Percoll fractions was positively associated with midpiece abnormalities in the spermatozoa. These results emphasize the importance of lipid peroxidation in the pathophysiology of male infertility and suggest a mechanism by which such damage might arise.
Association between Brain-Derived Neurotrophic Factor and Lipid Profiles in Acute Ischemic Stroke Patients

Abstract:Ischemic stroke, the most prevalent form of stroke, leads to neurological impairment due to cerebral ischemia and affects 55–90% of the population. Brain-derived neurotrophic factor (BDNF) plays a crucial role in the central nervous system and regulates cardiometabolic risk factors, including lipids. This single-center study aimed to explore the relationship between lipid profiles and BDNF levels in 90 patients who had experienced AIS for the first time. The results show that the high BDNF group (≥3.227 ng/mL) had significantly higher HbA1C and TG levels; ratios of TC/HDL-C, LDL-C/HDL-C, and TG/HDL-C; and percentage of hyperlipidemia (60%) as well as lower levels of HDL-C, with an OR of 1.903 (95% CI: 1.187–3.051) for TG/HDL-C, 1.975 (95% CI: 1.188–3.284) for TC/HDL-C, and 2.032 (95% CI: 1.113–3.711) for LDL-C/HDL-C. Plasma BDNF levels were found to be significantly positively correlated with TG and negatively with HDL-C, with OR values of 1.017 (95% CI: 1.003–1.030) and 0.926 (95% CI: 0.876–0.978), respectively. TC/HDL-C, TG/HDL-C, and LDL-C/HDL-C ratios are associated with BDNF levels in AIS patients. The results also indicate that, in AIS patients, higher BDNF levels are associated with lower HDL and higher TG concentrations.

Elevated Serum and Cerebrospinal Fluid Free Fatty Acid Levels Are Associated with Unfavorable Functional Outcome in Subjects with Acute Ischemic Stroke

Abstract:The aim of this study was to evaluate the prognostic value of serum and cerebrospinal fluid (CSF) free fatty acid (FFA) levels in a cohort of patients with an acute ischemic stroke (AIS). In a prospective study, FFA levels were measured using an enzyme cycling method on admission in serum and CSF of 252 consecutive patients with AIS. The prognostic value of FFA to predict the functional outcome and mortality within 90-day was compared with the National Institutes of Health Stroke Scale score and with other known outcome predictors. Serum and CSF levels of FFA increased with increasing severity of stroke as defined by the NIHSS score (all P < 0.001). Patients with an unfavorable outcomes and non-survivors had significantly increased FFA serum and CSF levels on admission (all P < 0.0001). Multivariate logistic regression analysis adjusted for common risk factors showed that serum FFA ≥0.71 mmol/L (third quarters) was an independent predictor of functional outcome (odds ratios (OR) = 4.86; 95 % confidence interval (CI) 2.26-10.48) and mortality (OR = 7.72; 95 % CI 3.01-21.48). The area under the receiver operating characteristic curve of serum FFA was 0.79 (95 % CI, 0.72-0.86) for functional outcome and 0.86 (95 % CI, 0.78-0.94) for mortality. Similarly, CSF FFA level also was an indicator for predicting of functional outcome and mortality. FFA levels in serum and CSF may serve as independent biomarkers in addition of the traditional methods for assessing the functional outcome and mortality of AIS.

Abstract:Cellular Fn-EDA (fibronectin containing extra domain A) is expressed in activated endothelial cells and elevated in circulation in patients with cardiovascular diseases. Although global deficiency of Fn-EDA in mice improves stroke outcome, the specific contribution of plasma versus endothelium Fn-EDA in stroke outcome is currently unknown. We investigated the role of plasma versus endothelial Fn-EDA in stroke exacerbation in the comorbid condition of hyperlipidemia. We generated novel plasma Fn-EDA −/− ( Fn-EDA fl/fl Alb Cre ) and endothelial Fn-EDA −/− ( Fn-EDA fl/fl Tie2 Cre ) strains on hyperlipidemic apolipoprotein E-deficient ( ApoE −/− ) background. By following the Stroke Therapy Academic Industry Roundtable guidelines, we evaluated stroke outcome in male and female mice. Susceptibility to ischemia/reperfusion injury was evaluated in 2 different models of stroke: intraluminal monofilament and embolic model on days 1, 3, and 7. Quantitative assessment of stroke outcome was evaluated by measuring infarct volume (by magnetic resonance imaging), cerebral blood flow (by laser speckle imaging), neurological and sensory-motor outcome, and postischemic thrombo-inflammation (platelet thrombi, fibrin, neutrophil, phospho-NFκB [nuclear factor κB], TNFα [tumor necrosis factor α], and IL1β [interleukin 1β]). Stroke outcome was comparable in ApoE −/− Fn-EDA fl/fl Tie2 Cre and control ApoE −/− Fn-EDA fl/fl mice suggesting endothelial Fn-EDA does not contribute to stroke. ApoE −/− Fn-EDA fl/fl Alb Cre mice exhibited significantly smaller infarcts and improved neurological and sensory-motor outcome at days 1, 3, and 7 in monofilament and embolic models of stroke. Improved stroke outcome was concomitant with enhanced survival, and decreased postischemic thrombo-inflammatory response ( P <0.05 versus ApoE −/− Fn-EDA fl/fl ). No sex-based differences were observed. Laser speckle imaging revealed significantly improved regional cerebral blood flow at 1 hour in ApoE −/− Fn-EDA fl/fl Alb Cre mice suggesting plasma Fn-EDA promotes postischemic secondary thrombosis. Coinfusion of anti–Fn-EDA antibody with r-tPA (recombinant tissue-type plasminogen activator) in ApoE −/− mice, 1 hour after embolization, improved stroke outcome with enhanced survival, and improved neurological outcome ( P <0.05 versus r-tPA). Genetic evidence suggests that plasma Fn-EDA exacerbates stroke outcome by promoting postischemic thrombo-inflammation. Abstract:This study was aimed to investigate whether brain-derived neurotrophic factor (BDNF) can modulate local cerebral inflammation in ischemic stroke. Rats were subjected to ischemia by occluding the right middle cerebral artery (MCAO) for 2 hours. Rats were randomized as control, BDNF, and antibody groups. The local inflammation was evaluated on cellular, cytokine, and transcription factor levels with immunofluorescence, enzyme-linked immunosorbent assay, real-time qPCR, and electrophoretic mobility shift assay, respectively. Exogenous BDNF significantly improved motor-sensory, sensorimotor function, and vestibulomotor function, while BDNF did not decrease the infarct volume. Exogenous BDNF increased the number of both activated and phagocytotic microglia in brain. BDNF upregulated interleukin10 and its mRNA expression, while downregulated tumor necrosis factor α and its mRNA expression. BDNF also increased DNA-binding activity of nuclear factor-kappa B. BDNF antibody, which blocked the activity of endogenous BDNF, showed the opposite effect of exogenous BDNF. Our data indicated that BDNF may modulate local inflammation in ischemic brain tissues on the cellular, cytokine, and transcription factor levels.
Serum Free Fatty Acids Independently Predict Adverse Outcomes in Acute Heart Failure Patients

Abstract:Background: Perturbation of energy metabolism exacerbates cardiac dysfunction, serving as a potential therapeutic target in congestive heart failure. Although circulating free fatty acids (FFAs) are linked to insulin resistance and risk of coronary heart disease, it still remains unclear whether circulating FFAs are associated with the prognosis of patients with acute heart failure (AHF).Methods: This single-center, observational cohort study enrolled 183 AHF patients (de novo heart failure or decompensated chronic heart failure) in the Second Affiliated Hospital, Zhejiang University School of Medicine. All-cause mortality and heart failure (HF) rehospitalization within 1 year after discharge were investigated. Serum FFAs were modeled as quartiles as well as a continuous variable (per SD of FFAs). The restricted cubic splines and cox proportional hazards models were applied to evaluate the association between the serum FFAs level and all-cause mortality or HF rehospitalization.Results: During a 1-year follow-up, a total of 71 (38.8%) patients had all-cause mortality or HF rehospitalization. The levels of serum FFAs positively contributed to the risk of death or HF rehospitalization, which was not associated with the status of insulin resistance. When modeled with restricted cubic splines, the serum FFAs increased linearly for the incidence of death or HF rehospitalization. In a multivariable analysis adjusting for sex, age, body-mass index, coronary artery disease, diabetes mellitus, hypertension, left ventricular ejection fraction and N-terminal pro-brain natriuretic peptid, each SD (303.07 mu mol/L) higher FFAs were associated with 26% higher risk of death or HF rehospitalization (95% confidence interval, 2-55%). Each increasing quartile of FFAs was associated with differentially elevated hazard ratios for death or HF rehospitalization of 1 (reference), 1.71 (95% confidence interval, [0.81, 3.62]), 1.41 (95% confidence interval, [0.64, 3.09]), and 3.18 (95% confidence interval, [1.53, 6.63]), respectively.Conclusion: Serum FFA levels at admission among patients with AHF were associated with an increased risk of adverse outcomes. Additional studies are needed to determine the causal-effect relationship between FFAs and acute cardiac dysfunction and whether FFAs could be a potential target for AHF management.
Low Plasma Proportion of Omega 3-Polyunsaturated Fatty Acids Predicts Poor Outcome in Acute Non-Cardiogenic Ischemic Stroke Patients

Abstract:BACKGROUND AND PURPOSE: Alterations in blood fatty acid (FA) composition are associated with cardiovascular diseases. We investigated whether plasma FA composition was related to stroke severity and functional outcome in acute ischemic stroke patients.METHODS: We prospectively enrolled 156 patients with first-episode cerebral infarction, within 7 days of symptom onset. The proportion of FAs was analyzed using gas chromatography, and the summation of the omega-3 polyunsaturated fatty acids (ω3-PUFA), 18:3 ω3 α-linolenic acid, 20:3 ω3 eicosatrienoic acid, 20:5 ω3 eicosapentaenoic acid (EPA), and 22:6 ω3 docosahexaenoic acid (DHA) was reported as Σω3-PUFAs. Stroke severity was assessed using the National Institutes of Health Stroke Scale (NIHSS) score on admission. Poor functional outcome was defined by modified Rankin scale (mRS) ≥3 at three months after the index stroke.RESULTS: Lower proportions of EPA (β=-0.751), DHA (β=-0.610), and Σω3-PUFAs (β=-0.462) were independently associated with higher NIHSS score, after adjusting for stroke subtype, hemoglobin, high density lipoprotein, high sensitivity C-reactive protein, fasting glucose, 16:0 palmitic acid, and Σsaturated fatty acids. Moreover, a lower proportion of DHA (odds ratio [OR]: 0.20, 95% confidence interval [CI]: 0.04-0.88), and Σω3-PUFAs (OR: 0.22, 95% CI: 0.05-0.84) showed an independent relationship with poor functional outcome after adjusting for age, sex, smoking status, NIHSS score, stroke subtype, and 16:0 palmitic acid.CONCLUSIONS: Our results demonstrate that ω3-PUFAs correlated with stroke severity on admission and functional outcomes at 3 months. ω3-PUFAs are potential blood biomarkers for prognosis of acute non-cardiogenic ischemic stroke patients.

Abstract:Stroke, Ahead of Print. Background:BDNF (brain-derived neurotrophic factor) is widely implicated in the pathophysiological process of stroke, but the effect of BDNF on poststroke cognitive impairment (PSCI) remains unclear. We aimed to investigate the association between baseline serum BDNF and the risk of PSCI at 3 months in a multicenter study based on a preplanned ancillary study of the CATIS trial (China Antihypertensive Trial in Acute Ischemic Stroke).Methods:We examined serum BDNF levels at baseline and used the Mini-Mental State Examination and Montreal Cognitive Assessment to evaluate cognitive function at 3-month follow-up after ischemic stroke. PSCI was defined as Mini-Mental State Examination score <27 or Montreal Cognitive Assessment score <25. Logistic regression analyses were performed to evaluate the association between serum BDNF and the risk of 3-month PSCI.Results:In this ancillary study, a total of 660 patients with ischemic stroke with hypertension were included, and 593 patients (mean age, 59.90±10.44 years; 410 males and 183 females) were finally included in this analysis. According to mini-mental state examination score, after adjustment for age, sex, education, baseline National Institutes of Health Stroke Scale score,APOE ɛ4carriers, and other potential confounders, the odds ratio of PSCI for the highest tertile of BDNF was 0.60 ([95% CI, 0.39–0.94];P=0.024) compared with the lowest tertile. Multiple-adjusted spline regression model showed a linear association of serum BDNF levels with PSCI at 3 months (Pvalue for linearity=0.010). Adding serum BDNF to conventional prognostic factors slightly improved the risk reclassification of PSCI (net reclassification improvement: 27.46%,P=0.001; integrated discrimination index: 1.02%,P=0.015). Similar significant findings were observed when PSCI was defined by the Montreal Cognitive Assessment score.Conclusions:Elevated serum BDNF levels were associated with a decreased risk of PSCI at 3 months, suggesting that serum BDNF might be a potential predictive biomarker for PSCI among patients with ischemic stroke with hypertension.

Dynamics of serum BDNF After Aerobic Exercise in Subacute Stroke Patients: A Sub-Analysis of the randomized controlled Phys-Stroke trial

Abstract:During the very early phase of stroke recovery, the brain is hypothesized to be in a neuroplastic state that is responsive to external stimuli. Here, brain-derived neurotrophic factor (BDNF) is thought to play an important role. BDNF, a member of the neurotrophin family, is implicated in processes such as synaptogenesis and long-term potentiation. Post-stroke, reduced BDNF levels are observed compared to the healthy population. An intervention that has been shown to elevate BDNF is Aerobic exercise. Yet, the impact of aerobic exercise in stroke patients remains unclear. This sub-analysis of the multicenter, randomized, controlled 'Physical Fitness Training in Patients with Subacute Stroke' trial aimed to investigate serum BDNF dynamics and the effects of a four-week aerobic exercise intervention on long-term BDNF levels. Data from 200 patients, with missing data imputed, revealed a modest increase in serum BDNF levels up to three months post-stroke (22.6 ng/ml, 95% CI 19.2 to 26) which plateaued until six months (24.3 ng/ml, 95% CI 20.6 to 27.9). Despite higher baseline BDNF levels in the exercise group, no treatment effect was observed until six months (training 24.6 ng/ml, 95% CI 20.2 to 29 vs relaxation: 24.3 ng/ml, 95% CI 19.8 to 28.8, p= 0.95). A sex-related interaction was identified in the relaxation group, with female patients exhibiting higher BDNF increases until end of follow up (20.9 ng/ml, 95% CI 15.7 to 26.2 (males) vs 31.0 ng/ml, 95% CI 22.9 to 39.0 (females), p = 0.039). Dose-response analyses and associations with recurrent events yielded no substantial differences. While serum BDNF levels increased early after stroke in the first three months, aerobic exercise did not demonstrate a significant impact on BDNF levels within this dataset. Further investigations with more measurements during the early subacute phase are warranted to elucidate the interplay between exercise, BDNF, and recovery.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Objective</h3><p>To investigate the correlation between brain-derived neurotrophic factor (BDNF) and risk factors as well as functional outcome in post-stroke depression (PSD) or post-stroke anxiety (PSA).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Design</h3><p>A cohort study.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Setting</h3><p>Stroke patients admitted to an urban rehabilitation hospital.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Participants</h3><p>This study included 162 stroke patients without any previous history of depression and anxiety.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Interventions</h3><p>Not applicable.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Main Outcome Measures</h3><p>Sociodemographic information and comorbidities were recorded during hospital admission. Functional outcomes were assessed using functional independent measure (FIM) scores at time of admission and discharge. The influence of various factors such as BDNF and patient characteristics on functional outcome was investigated. Single-factor effect was examined using simple logistic regression, as was multi-factor effect using multiple logistic regression. The goodness-of-fit of those regression models was evaluated by the integrated area under ROC curve (iAUC).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>PSD was diagnosed in 61 (37.7%) patients, and PSA was diagnosed in 40 (24.7%). Multiple logistic analysis showed that BDNF as well as divorce/separation and history of smoking were significantly associated with the occurrence of PSD but not with the occurrence of PSA. The model combining low BDNF level and divorce/separation improved the prediction for PSD. Among the variables analyzed for prediction of functional outcome, serum BDNF had a minimum correlation with motor FIM scores in PSD but no significant correlation with motor FIM scores in PSA.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>BDNF is a valuable prediction for the occurrence of PSD but not for PSA. More strikingly, ischemic stroke patients who are divorced or separated with low serum BDNF have a much higher risk for PSD. BDNF has a minimum correlation with motor function outcome in PSD but no significant correlation with motor outcome in PSA.</p>

Abstract:Dietary fibre intake is negatively associated with stroke and overall mortality. A plausible hypothesis is that this happens via the gut microbiota, producing metabolites known as short-chain fatty acids (SCFAs) during fibre fermentation. Indeed, our unpublished data showed that low-fibre (LF) intake worsens stroke outcomes while high-fibre (HF) intake ameliorates it. Thus, we aimed to investigate whether switching from a LF diet pre-stroke to either a HF diet or supplementation with SCFAs post-stroke can improve functional and histological outcomes. We also aimed to determine if this effect is dependent on SCFAs-sensing G-protein coupled receptors GPR41, GPR43, and GPR109A. Male C57BL/6J mice (8–9 weeks, n = 8/group) were randomized and treated with a LF diet for 4-weeks prior to photothrombotic stroke surgery. Post-stroke, the mice were either fed with LF, HF or SCFAs for 7-days. Using MRI, we observed no difference in the infarct volume at day 1 (D1) post-stroke. However, a HF diet and SCFAs supplementation reduced infarct volume at day 7 (D7) (mean brain infarct volume ± SEM; LF:13.52 ± 2.43 mm3 vs HF:7.71 ± 1.12 mm3 vs SCFAs:5.25 ± 1.18 mm3, P = 0.007), and improved brain recovery rate (LF:64.93 ± 2.94% vs HF:78.06 ± 1.98% vs SCFAs:85.05 ± 2.45%, P < 0.001). High-fibre and SCFAs supplementation improved wire-hanging grip strength and coordination at D7, which measures motor function (mean % improvement ± SEM; LF:12.59 ± 2.66% vs HF:69.77 ± 8.44% vs SCFAs:97.42 ± 1.54%, P < 0.001). Immunofluorescence was performed to access gut barrier integrity, immune cell infiltration and neurogenesis in the brain at D7. HF and SCFAs improved gut barrier integrity (ZO-1, P < 0.001). Moreover, SCFAs-treated mice had fewer neutrophils (MPO+, P = 0.017) and more neuroblasts (DCX+/Tuj1+, P < 0.001) in the infarct core, with a positive correlation between DCX+/Tuj1+ and recovery rate post-stroke (R2 = 0.50, P < 0.001). To identify the receptors by which SCFAs improve stroke recovery, wild-type (WT) and GPR41/43/109a-/- mice (8–9 weeks, n = 8/group) were fed a HF diet pre- and post-stroke. GPR41/43/109a-/- mice showed worse stroke outcomes with larger brain infarct volume at D1 (P = 0.048) and D7 (P = 0.008), and worse recovery rate (P = 0.002). Consistent with this, wire-hanging grip strength was reduced (P < 0.001) at D7. In addition, compared to WT mice, GPR41/43/109a-/- mice had a leaky gut (P < 0.001), more neutrophils (P = 0.007) and very few neuroblasts (P = 0.011) in the infarct core. A high-fibre diet, resulting in the production of SCFAs, targets GPR41/43/109a signalling to reduce stroke-induced brain injury and improve functional recovery post-stroke. This improvement is likely to occur via neurogenesis and the suppression of the inflammatory response in the ischaemic brain.

Abstract:Objective: Ischemic stroke triggers oxidative and biochemical responses that disrupt cell homeostasis. We previously demonstrated that Elovanoids (ELVs), a novel class of homeostatic lipid mediators, reduced infarct volumes and improved neurologic function when administered IV following middle cerebral artery occlusion (MCAo). The present study defines neuroprotection by very long-chain polyunsaturated fatty acid (VLC-PUFA) ELV precursors (C-32:6 and C-34:6) delivered intranasally following experimental ischemic stroke. Methods: Male SD rats underwent 2 hours of MCAo. Treatments with C-32:6, C-34:6 (1μg/μl, 20 μg per rat), or vehicle were administered at 1, 24, and 48h after onset of MCAo. Behavior testing was performed on days 1, 2, 3, and 7. Ischemic core and penumbra were computed from T2WI on day 7 followed by immunohistochemistry. RT-qPCR gene expression was investigated in a separate cohort of rats treated as above at 72 h. Results: C-32:6 and C-34:6 improved behavioral function on days three by (41% and 20%) and seven by (45% and 24%); T2WI total lesion volumes were reduced by (87% and 61%), respectively, compared to the vehicle. In the ischemic penumbra, C-32:6 and C-34:6 reduced TMEM119 + microglia by (38% and 29%), increased NeuN + neurons by (87% and 110%), reduced IgG staining by (54% and 25%), and increased the number of SMI-71 + vessels by (50% and 26%). In the core, C-32:6 and C-34:6 reduced TMEM119 + microglia by (12% and 15%), increased NeuN + neurons by (251% and 89%) and increased the number of SMI-71 + vessels by (31% and 70%). Anti-inflammatory marker genes for protective microglia and astrocyte phenotypes Tm4sf1, Thbs1, Ptx3, Gpc4, Fizz1, Cd14, Msr1, Manf , and Osmr were upregulated with C-32:6 and genes Fcrls, S1pr3, and Vim were downregulated with C-34:6. Conclusion: These data suggest that IN administered C-32:6 C-34:6 improved behavior, protected the ischemic penumbra, increased neurogenesis and angiogenesis, and reduced the number of microglia cells. The overall gene expression changes observed from free fatty acid C-32:6 indicate that its modulatory effects promote the biological activity of astrocytes, microglia, and cerebrovascular cells involved in plasticity.

Abstract:Background Brain-derived neurotrophic factor (BDNF) is crucial for neuronal survival and may be implicated in the pathophysiological process of depression. This study aimed to prospectively investigate the association between serum BDNF and post-stroke depression (PSD) at 3 months in a multicenter cohort study. Methods A total of 611 ischemic stroke patients with serum BDNF measurements from the China Antihypertensive Trial in Acute Ischemic Stroke were included in this analysis. We used the 24-item Hamilton Depression Rating Scale to assess depression status at 3 months after ischemic stroke, and PSD was defined as a score of ≥8. Results Baseline serum BDNF was inversely associated with the risk of depression after ischemic stroke. The multivariable-adjusted odds ratio of PSD for the highest tertile of BDNF was 0.53 (95 % confidence interval, 0.34–0.82; P for trend = 0.004) compared with the lowest tertile. Multivariable-adjusted spline regression model also showed a linear does-response association between serum BDNF levels and PSD at 3 months ( P for linearity = 0.006). In addition, adding serum BDNF to conventional risk factors significantly improved the risk reclassification of PSD (net reclassification improvement: 16.98 %, P = 0.039; integrated discrimination index: 0.93 %, P = 0.026). Limitations All patients in this study were Chinese, so our findings should be applied to other populations cautiously. Conclusions Higher serum BDNF levels at baseline were significantly associated with a decreased risk of PSD at 3 months, suggesting that BDNF might be a valuable predictive biomarker and potential therapeutic target for PSD among ischemic stroke patients.

The association between circulating fatty acids and stroke in hypertensive patients

Abstract:Background: Stroke is prevalent in hypertensive population. It has been suggested that unsaturated fatty acids (USFA) have protective effect on stroke. The effect of saturated fatty acids (SFAs) on stroke is still unclear. Therefore, we studied the relationship between circulating fatty acids and acute ischemic stroke (AIS) in hypertensive patients. Methods: Eighty-nine pairs including 100 men and 78 women matched by sex and age were recruited. Each pair included a hypertensive patient within 48h of AIS onset and a hypertensive patient without stroke. Six circulating fatty acids were methylated before concentration determination which was repeated twice with percent recovery estimated. Results: There were differences in educational level (P = 0.002) and occupation (P < 0.001) between stroke and non-stroke participants. All the 6 fatty acid levels were higher in non-stroke participants (P = 0.017 for palmitoleic acid, 0.001 for palmitic acid, <0.001 for linoleic acid, <0.001 for behenic acid, <0.001 for nervonic acid and 0.002 for lignoceric acid). In logistic regression analysis, AIS was inversely associated with fatty acid levels except for lignoceric acid. After adjustment for education and occupation, the palmitoleic acid and palmitic acid levels were no longer inversely associated with AIS. After further adjustment for systolic blood pressure, smoking, drinking, total cholesterol and triglyceride, the inverse associations of linoleic acid (OR = 0.965, 95%CI = 0.942-0.990, P = 0.005), behenic acid (OR = 0.778, 95%CI = 0.664-0.939, P = 0.009), nervonic acid (OR = 0.323, 95%CI = 0.121-0.860, P = 0.024) with AIS remained significant. Conclusions: Circulating fatty acids except lignoceric acid were inversely associated with AIS. Both USFAs and SFAs may have beneficial effect on stroke prevention in hypertensive population.

Abstract:Bidirectional communication of the microbiota–gut–brain axis is crucial in stroke. Recanalization therapy, namely intravenous thrombolysis (IVT) and endovascular thrombectomy (EVT), are recommended for eligible patients with acute ischemic stroke (AIS). It remains unclear whether gut microbiota metabolites, namely trimethylamine N-oxide (TMAO) and short-chain fatty acids (SCFAs), can predict the prognosis after recanalization therapy. This prospective study recruited patients with AIS receiving IVT, EVT, or both. The National Institutes of Health Stroke Scale (NIHSS) and modified Rankin scale (mRS) scores were used to assess the severity and functional outcomes of AIS, respectively. A functional outcome of mild-to-moderate disability was defined as a mRS score of 0–3 at discharge. Plasma TMAO and SCFA levels were measured through liquid chromatography with triple-quadrupole mass spectrometry. Fifty-six adults undergoing recanalization therapy for AIS were enrolled. Results showed that TMAO levels were not associated with stroke severity and functional outcomes, while isovalerate levels (one of the SCFAs) were negatively correlated with NIHSS scores at admission and discharge. In addition, high isovalerate levels were independently associated with a decreased likelihood of severe disability. The study concluded that an elevated plasma isovalerate level was correlated with mild stroke severity and disability after recanalization therapy for AIS.

Abstract:Summary Background & aims Some ω3 polyunsaturated fatty acids (PUFAs) are said to demonstrate a dose-related risk of atrial fibrillation (AF), conversely, some ω6 PUFAs might have AF protective potential. However, few investigated the relation among ischemic strokes. Primarily, we aimed to examine a relation between ω3 and ω6 PUFAs and the presence of AF in ischemic strokes. Further, since, some PUFAs are said to affect the cardiac load, we secondarily aimed to investigate the association between ω3 and ω6 PUFAs and brain natriuretic peptide (BNP) and the occurrence of cerebral large vessel occlusion (LVO) in ischemic strokes with AF. Methods Consecutive patients with ischemic stroke admitted between 2012 and 2022 were retrospectively screened. Plasma levels of PUFAs, including eicosapentaenoic acid (EPA), docosahexaenoic acid, dihomo-γ-linolenic acid (DGLA) and arachidonic acid (AA), were assayed. Data were analyzed using a Poisson regression analysis with a robust variance estimator and a multiple linear regression analysis. Results We screened 2112 consecutive ischemic strokes, including 1574 (1119 [71%] males, median age 69 years). Lower DGLA (prevalence ratio (PR) 0.885, 95% CI 0.811–0.966, p = 0.006), lower AA (PR 0.797, 95% CI 0.649–0.978, p = 0.030), and higher EPA/AA ratio (PR 1.353, 95% CI 1.036–1.767, p = 0.026) were associated with AF. Checking the linearity between AF and PUFAs, negative linear trends were observed between DGLA quartiles (Q1: PR 1.901, Q2: PR 1.550, Q3: PR 1.423, Q4: 1.000, p < 0.001 for trend) and AA quartiles (Q1: PR 1.499, Q2: PR 1.204, Q3: PR 1.125, Q4: 1.000, p = 0.004 for trend), with positive linear trends between EPA/AA ratio quartiles (Q1: 1.000, Q2: PR 1.555, Q3: PR 1.612, Q4: PR 1.797, p = 0.001 for trend). Among patients with AF, a negative association between AA and BNP (unstandardized coefficient −1.316, 95% CI -2.290∼-0.342, p = 0.008) was observed, and lower AA was associated with LVO (PR 0.707, 95% CI 0.527–0.950, p = 0.021). Conclusion Lower DGLA and AA and a higher EPA/AA ratio might be related to the development of AF in ischemic strokes. Further, AA might have a cardio-cerebrovascular protective role in ischemic strokes with AF.

Background & Aims:Some poly unsaturated fatty acids (PUFA) are said to be associated with the development of atrial fibrillation (AF). Our aim is to elucidate the relation between PUFA and AF in acute ischemic stroke.Methods:Consecutive patients with ischemic stroke admitted between October 2012 and February 2022 were retrospectively screened. The inclusion criteria were as follows: 1) onset to door time within 7 days; and 2) availability of PUFA measurements from the day of the hospital visit or the next day. Plasma levels of PUFA, including eicosapentaenoic acid (ω3: EPA) docosahexaenoic acid (ω3: DHA), dihomo-γ-linolenic acid (ω6: DGLA), and arachidonic acids (ω6: AA), were assayed by gas chromatography. We investigated the association between each of PUFA levels and AF among the acute ischemic stroke patients.Results:We screened 2,122 consecutive ischemic stroke patients. Of them, 1,574 were identified (1,119 [71%] males, median age 69 years). In Poisson regression analysis with a robust variance estimator, factors associated with AF were lower DGLA level (prevalence ration (PR) 0.885, 95% CI 0.811 to 0.966,p= 0.006), lower AA level (PR 0.797, 95% CI 0.649 to 0.978,p= 0.029), and higher EPA/AA ratio (PR 1.353, 95% CI 1.036 to 1.766,p= 0.026). In Poisson regression analysis with a robust variance estimator for AF using PUFA quartile as indicator variables, negative linear trends were observed between the quartile of DGLA (Quartile 1: PR 1.884, Quartile 2: PR 1.530, Quartile 3: PR 1.429, Quartile 4: reference) and AA (Quartile 1: PR 1.499, Quartile 2: PR 1.199, Quartile 3: PR 1.137, Quartile 4: reference), and a positive linear trend between the quartile of EPA/AA ratio (Quartile 1: reference, Quartile 2: PR 1.544, Quartile 3: PR 1.629, Quartile 4: PR 1.781, Figure).Conclusions:In acute ischemic stroke, lower ω6 level and higher ω3/ω6 ratio were associated with the presence of AF. ω3/ω6 PUFA may have contrasting effects on the incidence of AF in acute ischemic stroke.

Abstract:BACKGROUND:Elevated levels of asymmetric dimethylarginine (ADMA) and symmetric dimethylarginine (SDMA) are accompanied by endothelial dysfunction and predict adverse outcome after ischemic stroke. Via induction of oxidative stress, dimethylarginines are possibly linked to the inflammatory cascade after stroke that is known to considerably contribute to secondary progression of brain injury. We sought to investigate the association between dimethylarginines and inflammatory mediators in patients with acute ischemic stroke.METHODS:Plasma levels of ADMA and SDMA were measured in prospectively collected blood samples of 58 patients with acute ischemic stroke. Blood samples were taken at 6 hours, 12 hours, 24 hours, 3 days and 7 days after onset of symptoms. Analyses of ADMA and SDMA were done by high-performance liquid chromatography-tandem mass spectrometry. Monocyte chemotactic protein-1 (MCP-1), matrix metalloproteinase-9 (MMP-9), tissue inhibitor of matrix metalloproteinase-1 (TIMP-1), interleukin-6 (IL-6), C-reactive protein (CRP) and S100B as markers of inflammation and brain damage were determined by commercially available immunometric assays. Patient data were compared with control data from 32 age-adjusted healthy volunteers. Baseline stroke severity was evaluated by the National Institutes of Health Stroke Scale (NIHSS) (NIHSS 0 to 1: mild stroke; NIHSS 2 to 8: moderate stroke; NIHSS ≥9: severe stroke).RESULTS:Plasma ADMA and SDMA levels significantly correlated with blood levels of inflammatory mediators up to day 7 after stroke. On multiple stepwise linear regression analysis ADMA correlated with TIMP-1 at 6 hours, 24 hours, 3 days and 7 days, MMP-9 at 12 hours and IL-6 at 7 days (P <0.05) while SDMA correlated with MCP-1 at 6 hours, 24 hours, 3 days and 7 days as well as IL-6 at 3 days and 7 days (P <0.05).CONCLUSIONS:The levels of the vasoactive compound ADMA as well as levels of its structural isomer SDMA are associated with levels of inflammatory mediators after acute ischemic stroke. Further studies need to elucidate the cause and effect relationship of these crucial players.
Dietary Supplementation with Omega-3 Polyunsaturated Fatty Acids Robustly Promotes Neurovascular Restorative Dynamics and Improves Neurological Functions after Stroke.

Abstract:Stroke is a devastating neurological disease with no satisfactory therapies to preserve long-term neurological function, perhaps due to the sole emphasis on neuronal survival in most preclinical studies. Recent studies have revealed the importance of protecting multiple cell types in the injured brain, such as oligodendrocytes and components of the neurovascular unit, before long-lasting recovery of function can be achieved. For example, revascularization in the ischemic penumbra is critical to provide various neurotrophic factors that enhance the survival and activity of neurons and other progenitor cells, such as oligodendrocyte precursor cells. In the present study, we hypothesized that chronic dietary supplementation with fish oil promotes post-stroke angiogenesis, neurogenesis, and oligodendrogenesis, thereby leading to long-term functional improvements. Mice received dietary supplementation with n-3 PUFA-enriched fish oil for three months before and up to one month after stroke. As expected, dietary n-3 PUFAs significantly increased levels of n-3 PUFAs in the brain and improved long-term behavioral outcomes after cerebral ischemia. n-3 PUFAs also robustly improved revascularization and angiogenesis and boosted the survival of NeuN/BrdU labeled newborn neurons up to 35days after stroke injury. Furthermore, these pro-neurogenic effects were accompanied by robust oligodendrogenesis. Thus, this is the first study to demonstrate that chronic dietary intake of n-3 PUFAs is an effective prophylactic measure not only to protect against ischemic injury for the long term but also to actively promote neurovascular restorative dynamics and brain repair.
