Abstract:Chronic obstructive pulmonary disease (COPD) is the most common chronic airway disease affecting Chinese population and it ranks number four leading cause of death. The high mortality and morbidity, increased prevalence, significant loss of labor, and high medical expenditure warrants attention from the health organization and society to standardize the diagnosis and treatment of COPD. Overall COPD treatment efficacy has been significantly improved with application of long activating bronchial dilator (LABD), due to the imbalance and geographically different distribution of medical resource and education, standardized treatment under COPD guideline remain the major protocol in a long time period in the future. COPD is a disease with heterogeneity, such as different profile on clinical symptom, treatment response, and different prognosis. The mechanism of this nature of heterogeneity is due to the complexity of environment exposure and genetic background. It is because of the complicated airway antigen and toxicant exposure, the different inflammation response after exogenous stimulant, the complex metabolism and medical treatment response, COPD patients present far more versatile and heterogeneous phenotypes than other airway disease. The Global Initiative for Chronic Obstructive Lung Disease (GOLD) is the major guideline for diagnosis and treatment of COPD worldwide based on lung function measurement, life quality assessment, and risk factor evaluation for acute exacerbation.1 All patients could be classified into A, B, C, and D groups, then receive different regimen according to recommendation level. When reviewing several major global clinical trials, patients from same GOLD group showed very different treatment response and outcomes.2 It is therefore important to have more proper patient subclassification and select right population and medicine for more specific treatment. In this issue, Dr. Xie et al3 showed group classification based on pulmonary function correlated with hospitalization. The above correlation with modified British Medical Research Council grade=1 as the cut off threshold was better than that with the cutoff threshold at 2. These evidences suggested there was some gap between guideline and clinical practice in real world. It is useful to understand these differences in order to dissect the true meaning of heterogeneity. Those phenomena suggested the necessity of having a tailored process of COPD diagnosis and treatment under guideline. The study on COPD phenotype provided data and evidence for more specific and targeted therapy. COPD phenotype has been defined as “a single or combination of disease attributes that describe differences between individuals with COPD as they relate to clinically meaningful outcomes”.4 Phenotype based therapy should have a more specific target population with maximum treatment response or good correlation with prognosis. COPD phenotype includes but not limited to asthma-COPD overlap syndrome, bronchitis type, emphysemahyperinflation type and frequent exacerbation type. The guideline from Spanish Society of Pneumology and Thoracic Surgery and Czech Pneumological and Physiological Society provided specific treatment regimen based on phenotype.5,6 Those guidelines together with GOLD enriched the strategy of COPD diagnosis and treatment. Although the GOLD may be more authorative given high level of clinical evidence and global advisory board, the indication of COPD treatment was rather more general, while the guideline from Spanish and Czech was more specific and personalized. These guidelines were compensated to each other. The major difference of GOLD and phenotype-based therapy was the application of inhaled corticosteroid (ICS) and phosphodiesterase type 4 (PDE4) inhibitor, etc. For patient who has asthma-COPD overlap syndrome, ICS plus long-acting beta-agonist (LABA) are recommended no matter which group the patients belong to according to GOLD. It is known that overlap syndrome does exist in both A or B groups and adding tiotropium could further improve lung function besides ICS+LABA. Bronchitis phenotype indicates that for the patients who have recurrent cough with sputum, dyspnea after exertion, cyanosis on lips etc., the regimen include ICS+LABA, longacting muscarinic antagonist (LAMA) or triple therapy, PDE4 inhibitor and mucoactive medicine. Emphysemahyperinflation phenotype indicates dyspnea after exertion, with clear lung emphysema or bulle on CT scanning. Patients who were classified in this group usually have poor response to ICS and PDE4 inhibitor, while LABA, LAMA or LABA+LAMA, or surgical therapy is recommended. Those patients who have frequent acute exacerbation are suitable for adding ICS. Our unpublished data also suggested high level cytokines in blood suggest good response from adding ICS. A series of clinical parameters have been used to better define and evaluate COPD, such as gender, age, body mass index, bronchial dilator response, fraction of exhaled nitric oxide (FENO), airway esinophils/neutrophils cell count, life quantity score, CT scanning, 6-minute walk, lung function test, symptom, etc. The three parameters of forced expiratory volume in one second (FEV1), COPD assessment test /modified British Medical Research Council questionnaire /clinical COPD questionnaire, acute exacerbation have been used as major criteria for COPD evaluation by GOLD but other parameters stated above may help to define COPD phenotype and more precise and comprehensive treatment regimen, as well as prognosis. Besides above phenotypes, COPD combined with bronchioectasis might be a new COPD phenotype although the exact treatment strategy has not been universally accepted. Those patients may benefit from adding mucoactive medicine, macrolides, and proper antibiotic use besides LABD. The doubled mortality of COPD combining with bronchioectasis compared with COPD alone strongly suggests high demanding to this group of patients. In this issue, Dr. Wang et al7 found a high occurrence rate of COPD in type 2 diabetic patients, while disease progression was much faster in COPD patient once they had diabetes. All these data suggested possibly new COPD phenotypes and treatment regimen based on phenotypes classification. Of note, COPD treatment based on phenotypes is compliment to GOLD with more specific and tailored plan, it is essential to test the strategy in clinical practice and to improve COPD management with more data support.

Abstract:Chronic obstructive pulmonary disease (COPD) is a leading cause of morbidity and mortality worldwide. While the main causes of COPD and clinical outcomes of concern have been well-described,[1] a deeper understanding of mechanisms of disease, innovational therapeutic paradigms, and the addressing the interface of COPD with other comorbidities are areas of innovation in modern COPD care and research. The guest editors of this dedicated issue on COPD aim to provide you with updated information on diagnosis and treatment of COPD and illustrate other important considerations in COPD care. We appreciate your readership and hope you learn as much from this issue as we did. Novel approaches to COPD treatment have been emerging rapidly. Among these are the identification of COPD endotypes, which may provide insight into the future of therapeutics and targeted medicine in this chronic disease.[2] Further, understanding how COPD is influenced by other comorbidities remains a necessary endeavor to ensure quality, all-encompassing care for patients.[3] In the office setting, the approach to COPD is changing rapidly and we are pleased to cover topics related to these changes in this issue. Beyond inhaled therapies, pulmonary rehabilitation, long-term oxygen therapy, and other medical therapies for COPD are minimally invasive procedures to treat the physiological effects of COPD such as hyperinflation and ventilation–perfusion mismatch due to emphysema in those patients who have persistent symptoms despite optimal pharmacological and physiological therapies. Bronchoscopic lung volume reduction (BLVR) with endobronchial valves is one such therapy and is recommended in the Global Initiative for Chronic Obstructive Lung Disease Report 2024 as it can improve exercise tolerance, health status, and lung function.[1] BLVR is not indicated for every patient with COPD and has somewhat narrow inclusion criteria based on pulmonary function testing, cardiac function, functional status, and others. There is early evidence that BLVR may lengthen survival; however, more research is needed to definitively assess the mortality benefit from this approach.[4] For those with advanced COPD, lung transplant may be an option. While improving the quality of life for patients who can undergo the surgery, lung transplant is a complicated process limited by donor organ availability, patient- and hospital-associated resources, and limitations based on patients' level of fitness to undergo the surgery. Other nonpharmacological interventions for COPD are on the horizon to help fill the gaps for patients who are not candidates for BLVR, lung transplant, or a combination of the two well-studied interventions. These include therapies targeting chronic bronchitis such as bronchial rheoplasty and cryospray to reduce COPD exacerbations such as targeted lung denervation. Other interventions are aimed to treat symptoms of dyspnea and include sealants, thermal ablation, and coils. These therapies are under investigation. Abstract:Microfinance institutions aim to reduce poverty. Some assess their impact through a cross-sectional impact methodology which compares veteran to new participants and then calls any difference between these two groups the "impact" of the program. Such studies have risen recently in popularity because they are cheap, easy to implement, and often encouraged by donors. USAID, through its AIMS project, encourages this methodology with its SEEP/AIMS practitioner-oriented tools. This paper intends to inform practitioners about the perils of using such a strategy, and suggests a couple of solutions to some of the larger problems with this approach.

Abstract:Objective: To describe the characteristics and management of asthma in clinical practice in the Czech Republic in the context of international guidelines and clinical realities.Methods: Data were collected over four seasons from summer 2016 to spring 2017 and are mostly presented using descriptive statistics.Results: We obtained valid data for 4557 adult patients with asthma, including detailed phenotyping (71% eosinophilic allergic, 10% eosinophilic non-allergic, 19% non-eosinophilic non-allergic asthma) from 58 allergologists and 56 pulmonologists. The average time to diagnosis was 3 years. In more than half of the subjects, bronchodilator testing (BDT) results were available at primary diagnosis. More than 10% of physicians did not test for mold allergy. Occupational asthma was diagnosed in 0.7% of subjects. According to the attending physician, 68% of patients had well-controlled and 10% had uncontrolled asthma. Ninety-four percent of patients were on preventive treatment, with 91% using an inhaled corticosteroids (ICS) at an average dose of 705 µg/day budesonide equivalent. Approximately 75% of patients were on an ICS/LABA, with 91% using fixed combinations. Among patients using ICS/formoterol, a maintenance and reliever therapy regime was prescribed in 67%.Conclusions: The quality of asthma management in the Czech Republic is comparable to that of other developed countries and better in some respects (frequent BDT, phenotyping, and use of preventive treatment). Nevertheless, there is unnecessary delay in diagnosis and lack of research on possible environmental causes (workplace, molds). Pharmacotherapy shows good adherence to guidelines. Although 10% of patients show poor control, there is concurrently a trend for overtreatment.

Abstract:Chronic obstructive pulmonary disease (COPD) ranks as the third leading cause of global illness and mortality. It is commonly triggered by exposure to respiratory irritants like cigarette smoke or biofuel pollutants. This multifaceted condition manifests through an array of symptoms and lung irregularities, characterized by chronic inflammation and reduced lung function. Present therapies primarily rely on maintenance medications to alleviate symptoms, but fall short in impeding disease advancement. COPD's diverse nature, influenced by various phenotypes, complicates diagnosis, necessitating precise molecular characterization. Omics-driven methodologies, including biomarker identification and therapeutic target exploration, offer a promising avenue for addressing COPD's complexity. This analysis underscores the critical necessity of improving molecular profiling to deepen our comprehension of COPD and identify potential therapeutic targets. Moreover, it advocates for tailoring treatment strategies to individual phenotypes. Through comprehensive exploration-based molecular characterization and the adoption of personalized methodologies, innovative treatments may emerge that are capable of altering the trajectory of COPD, instilling optimism for efficacious disease-modifying interventions.

Abstract:Chronic obstructive pulmonary disease (COPD) is a heterogeneous disease, and not all patients respond to all currently available drugs. The importance of personalized treatment of COPD is increasingly recognized. The new GOLD guidelines have moved the principles of treatment of stable COPD forward by including concepts of symptoms and risks into the decision for therapy. COPD phenotypes are the basis of personalized treatment in clinical practice. Consensus has been reached concerning several phenotypes: phenotypes according to the evaluation of image; the frequent exacerbator and infrequent exacerbator; asthma and COPD overlap syndrome; and the persistent systemic inflammation phenotype. These phenotypes can help clinicians identify patients that respond to specific pharmacological interventions. Comorbidities and other factors, such as social and economic status, must also be considered. Future research needs to validate potential phenotypes in longitudinal studies, and examine the responses of different phenotypes to existing and future therapies.

Abstract:Chronic obstructive pulmonary disease (COPD), a complex disease triggered mostly by exposure to cigarette smoke, is a leading cause of morbidity and mortality worldwide, leading not only to pulmonary damage but also to systemic impairment. There is growing awareness of systemic inflammation and cardiovascular, neurologic, psychiatric, and endocrine comorbidities associated with COPD. The diagnosis of CODP is based upon the clinical presentation, measurement of the pulmonary function, investigation of comorbidities and exclusion of differential diagnoses. COPD is a heterogeneous disease including various phenotypes. A number of drugs reduce or alleviate symptoms, increase exercise capacity, or reduce the number and severity of exacerbations. Non-pharmacologic measures such as smoking cessation, nutritional support, long term oxygen therapy, physiotherapy, rehabilitation, lung volume reduction and lung transplantation may be available for appropriate patients and can improve health status.

Abstract:Chronic obstructive pulmonary disease (COPD) is a common, preventable, and treatable chronic respiratory disease, which is characterized by persistent airflow limitation and respiratory symptoms. Pathological changes are mainly airway and/or alveolar structural abnormalities. Numerous factors, such as exposure to harmful particles or gases, genetic susceptibility, abnormal inflammatory responses, and abnormal lung development, are involved in the pathogenesis of COPD, those which determine the heterogeneity of COPD. Individuals show different pathophysiological changes, different disease evolution rules, and different clinical manifestations due to different etiologies, different susceptibility genes, and different chronic processes of “injury-inflammation-repair.” Therefore, disease managers need to conduct a multifaceted assessment of the whole body and the local area from the individual characteristics of COPD. With the sustained advancement of new technologies, from multiple perspectives, including genomics, exposomes, transcriptomics, mechanisms related to inflammation and immune regulation, microbiota, metabolomics, imaging features and radiomics, and the interaction of lungs and systemic organs to further explore the law of the occurrence and development of COPD, and finally, form an optimized prevention and treatment strategy. On the basis of thorough exploration, a COPD evaluation system that can meet clinical needs will be finally formed, so as to formulate scientific and effective individualized management strategies.

Abstract:Chronic obstructive pulmonary disease (COPD) is a heterogeneous disease. Historically, two COPD phenotypes have been described: chronic bronchitis and emphysema. Although these phenotypes may provide additional characterization of the pathophysiology of the disease, they are not extensive enough to reflect the heterogeneity of COPD and do not provide granular categorization that indicates specific treatment, perhaps with the exception of adding inhaled glucocorticoids (ICS) in patients with chronic bronchitis. In this review, we describe COPD phenotypes that provide prognostication and/or indicate specific treatment. We also describe COPD-like phenotypes that do not necessarily meet the current diagnostic criteria for COPD but provide additional prognostication and may be the targets for future clinical trials.

Novel triple formulations of ICS, LABAs and LAMAs are becoming the mainstay of treatment, however there is still a lack of clinical evidence for personalized therapy. Patients and Methods: RATIONALE was a non-interventional, prospective, 52 week study, assessing the effectiveness of beclometasone/formoterol/glycopyrronium-bromide (BDP/FF/G), in symptomatic COPD patients, with moderate airflow obstruction. The study included 4 visits, where data on demographic parameters, exacerbations, symptoms, quality of life (based on the EQ-5D-3L questionnaire) and lung function were collected. Data on adherence to treatment, based on prescriptions filled was collected from the database of the National Health Insurance Fund, with the patients' consent. The primary objective was the change of adherence to treatment during the study, compared to baseline. Results: Altogether 613 patients had been enrolled. Their average age was 64.56 years and 50.5% were female. The average CAT score was 20.86, and most patients had suffered minimum one exacerbation (82.2%). Average FEV1 was 59.6%. Most patients had some limitation in one or more dimensions of EQ-5D-3L, with an average visual analogue scale score (VAS) of 60.31. After 12 months of treatment, adherence improved significantly – proportion of patients in the highest adherence group increased from 29.8% to 69.7% (p< 0.001). The average CAT score improved by 7.02 points (95% CI 5.82– 8.21, p< 0.001). There was a significant improvement in all dimensions of EQ-5D-3L, with an average increase of 17.91 (95% CI 16.51– 19.31, p< 0.001) points in the VAS score. Exacerbation frequency also decreased significantly. Conclusion: Although limitations of observational studies are present, we observed that early introduction of fixed triple combination results in a marked improvement in adherence to treatment, symptom scores, exacerbation frequency and quality of life. The optimal choice of treatment is crucial for reaching the highest possible adherence. The disease progresses over time, leading to worsening respiratory function and exacerbation of symptoms despite appropriate treatment. Periodic acute exacerbations, triggered by viral or bacterial infections, air pollution, or other environmental factors, also characterize COPD. The goal of maintenance therapy for COPD is to reduce the risk of exacerbations, prevent their occurrence, alleviate daily symptoms, improve quality of life, and preserve respiratory function. 3 The Global Initiative for Chronic Obstructive Lung Disease (GOLD) provides guidelines for the diagnosis and management of COPD. The choice of maintenance therapy depends on exacerbation frequency and symptoms, particularly dyspnea. 4 Options include long-acting muscarinic antagonists (LAMA), beta-adrenergic agonists (LABA), dual bronchodilator therapy (LABA/LAMA), and combination therapy with inhaled corticosteroids (ICS/LABA). The latest treatment option includes fixed triple combination therapies containing three different active ingredients (ICS/LABA/LAMA), recommended when patients present with exacerbation and elevated blood eosinophil counts or when exacerbations occur despite established dual bronchodilator therapy. 4 The initiation of therapy with fixed triple combinations was newly introduced in the 2023 GOLD guidelines due to recognition of their high efficacy. Which Pulmonary Function Tests Best Differentiate Between COPD Phenotypes?

Abstract:We are still at the early phase of finding useful phenotypes in COPD that can guide therapy. However, in a simple sense, "sick patients die." Many phenotypic measurements of severity correlate with mortality in COPD: FEV(1), the ratio of inspiratory capacity to total lung capacity (IC/TLC), diffusing capacity of the lung for carbon monoxide (D(LCO)), 6-min walk distance, and maximum oxygen (O(2)) consumption or maximum watts on exercise testing. However, composite parameters, such as the BODE index (body mass index, air flow obstruction, dyspnea, exercise capacity), perform better, likely because they capture different aspects of severity that affect functional impairment and risk of death. Bronchodilator responsiveness is just a relative feature that aids in distinction of asthma and COPD but is not diagnostic in this use. A normal D(LCO) helps to rule out exercise-induced O(2) desaturation, but those with a low D(LCO) and COPD need exercise measurements to confirm desaturation. Currently, pulmonary function tests (PFTs) alone do not define subsets who respond to particular therapies. The presence of air flow obstruction and its severity increase the risk of lung cancer in COPD patients. Inflammatory biomarkers (exhaled nitric oxide and eosinophilia in sputum or bronchoalveolar lavage fluid) help distinguish asthma from COPD. Genetics is a promising area to elucidate pathophysiology and treatment for asthma and COPD, but currently alpha-1 antitrypsin deficiency is the only genetically-determined phenotype that has relevance for COPD management. The best promise for the future seems to be in composite phenotypes or scores, both for distinguishing asthma from COPD, and for guiding therapeutic options. It may be better to throw out the old, limiting diagnostic concepts. If, instead, we start from outcomes of interest, perhaps we can work back to predictors of these outcomes, and organize new diagnostic entities that have predictive relevance for treatment choices, functional outcomes, and mortality.

Teresa Pique, Mallorca, Spain, Email Purpose: Asthma and chronic obstructive pulmonary disease (COPD) are associated with substantial morbidity and mortality. This analysis assessed patient, caregiver, and healthcare provider (HCP) opinions regarding asthma and COPD diagnosis and treatment, including inhaler device preferences. Patients and Methods: Insights were from: (1) face-to-face/telephone patient/HCP communications with GSK Global Medical Teams; (2) social media listening; (3) a self-completed online patient/caregiver survey. Data were anonymized and informed consent was provided. Qualitative outputs were extracted, analyzed inductively, and coded per key themes, such as treatment preferences (including perceptions of single inhaler triple therapy [SITT] versus multiple inhaler triple therapy [MITT]) and long-term treatment goals. Data could be assigned to ≥ 1 theme. Results: Overall, 2966 patient and HCP insights (patients, 1150; HCPs, 1816), 988 social media posts, and 44 survey responses (patients, 33; caregivers, 11) were included. Within this analysis, some patients delayed seeking medical advice due to lack of disease understanding and used alternative information sources to better understand treatment options (eg, social media). Patients preferred SITT over MITT in terms of convenience, ease of use, and perceived efficacy; some believed that MITT provided greater coverage due to frequent dosing. HCPs recognized the tendency of patient preferences to drive treatment decisions, and highlighted a reluctance to change therapy among some patients, particularly those well controlled in current therapy. Patients preferred therapies with a shorter onset of action. Patients tended to measure treatment success by symptom improvement and quality of life, whereas HCPs measured treatment success through clinical tests. HCPs reported a lack of patient awareness of the mortality risk associated with COPD. Conclusion: There is discordance between patient and HCP perceptions of treatment success and goals in asthma and COPD. Improved patient education and HCP-patient communication are needed to facilitate meaningful shared decision-making, optimize care plans, and provide early treatment options. Symptoms can impact day- and night-time functioning, have an emotional and social impact, and can affect work productivity and absenteeism. 4,5 These QoL impacts become more pronounced with increasing disease severity. 6,7 Inhaled medications are recommended as maintenance and reliever therapy for both asthma and COPD. 8,9 Though these guideline-recommended therapies have established efficacy in terms of exacerbation control and an acceptable safety profile, 8,9 suboptimal prescribing and inappropriate use can limit their utility. Different proposals have been put forward which combine all the available treatments and the different clinical presentations in an effort to select the best therapeutic options for each clinical context. As COPD is a chronic progressive disease, the escalation of therapy has traditionally been considered the most natural way to tackle it. However, the notion of COPD as a constantly progressing disease has recently been challenged and, in specific areas, this points to the possibility of a de-escalation in treatment. In this context, the clinician requires simple, specific recommendations to guide these changes in treatment in their daily clinical practice. To accomplish this, the first step must be a correct evaluation and an accurate initial preliminary diagnosis of the patient's condition. Thereafter, the first escalation in therapy must be introduced with caution as the disease progresses, since clinical trials are not designed with clinical decision-making in mind. During this escalation, three possibilities are open to change the current treatment for a different one within the same family, to increase non-pharmacological interventions or to increase the pharmacological therapies. Beyond that point, a patient with persistent symptoms represents a complex clinical scenario which requires a specialized approach, including the evaluation of different respiratory and non-respiratory comorbidities. Unfortunately, there are few de-escalation studies available, and these are mainly observational in nature. The debate on de-escalation in pharmacological treatment, therefore, involves two main discussion points: the withdrawal of bronchodilators and the withdrawal of inhaled steroids. Altogether, the scheme for modifying treatment must be more personalized than just adding molecules, and the therapeutic response and its conditioning factors should be evaluated at each step before proceeding further. Abstract:This study aims to investigate the characteristics of the phenotype and management of chronic obstructive pulmonary disease (COPD) patients in the general population in China. We analyzed spirometry-confirmed COPD patients who were identified from a population-based, nationally representative sample in China. All participants were measured with airflow limitation severity based on post-bronchodilator FEV1 percent predicted, bronchodilator responsiveness, exacerbation history, and respiratory symptoms. Among a total of 9134 COPD patients, 90.3% were non-exacerbators, 2.9% were frequent exacerbators without chronic bronchitis, 2.0% were frequent exacerbators with chronic bronchitis, and 4.8% were asthma-COPD overlap. Less than 5% of non-exacerbators ever had pulmonary function testing performed. The utilization rate of inhaled medication in non-exacerbators, exacerbators without chronic bronchitis, exacerbators with chronic bronchitis, and asthma-COPD overlap was 1.4, 23.5, 29.5, and 19.4%, respectively. A comprehensive strategy for the management of COPD patients based on phenotype in primary care is urgently needed.

Abstract:Chronic obstructive pulmonary disease (COPD) is a major health problem with high prevalence, rising incidence, and substantial morbidity and mortality. Its course is punctuated by acute episodes of increased respiratory symptoms, termed exacerbations of COPD (ECOPD). ECOPD are important events in the natural history of the disease as they are associated with lung function decline and prolonged negative effects on quality of life. The present-day therapy of ECOPD with short courses of antibiotics, steroids, and escalation of bronchodilators has resulted in only modest improvements in outcomes. Recent data indicate that ECOPD are heterogeneous, raising the need to identify distinct etio-endo-phenotypes, incorporating traits of the acute event and of patients who experience recurrent events, to develop novel and targeted therapies. These characterizations can provide a complete clinical picture, the severity of which will dictate acute pharmacological treatment, and may also indicate whether a change in maintenance therapy is needed to reduce the risk of future exacerbations. This review discusses the latest knowledge of ECOPD types based on clinical presentation, etiology, natural history, frequency, severity, and biomarkers in an attempt to characterize these events.

Tuberculosis-associated COPD is increasingly recognized as a distinct phenotype of COPD that potentially exhibits unique clinical features. A thorough understanding of the precise definition, clinical manifestations, prognosis, and most effective pharmacological strategies for tuberculosis-associated COPD warrants further investigation. Methods: This prospective, observational cohort study aims to enroll over 135 patients with tuberculosis-associated COPD and 405 patients with non-tuberculosis-associated COPD, across seven tertiary hospitals in mainland China. The diagnosis of tuberculosis-associated COPD will be established based on the following criteria: (1) history of pulmonary tuberculosis with standard antituberculosis treatment; (2) suspected pulmonary tuberculosis with radiological evidence indicative of tuberculosis sequelae; (3) no definitive history of pulmonary tuberculosis but with positive interferon-gamma release assay results and radiological signs suggestive of tuberculosis. At baseline, demographic information, medical history, respiratory questionnaires, complete blood count, interferon-gamma release assays, medications, spirometry, and chest computed tomography (CT) scans will be recorded. Participants will be followed for one year, with evaluations at six-month intervals to track the longitudinal changes in symptoms, treatment, lung function, and frequencies of COPD exacerbations and hospitalizations. At the final outpatient visit, additional assessments will include chest CT scans and total medical costs incurred. Discussion: The findings of this study are expected to delineate the specific characteristics of tuberculosis-associated COPD and may propose potential treatment options for this particular phenotype, potentially leading to improved clinical management and patient outcomes. Abstract:COPD is a heterogeneous condition, the onset and trajectory of which is influenced not only by tobacco exposure but also an individual's genetics and the exposures they accumulate over their life course. In such a complex chronic disease, phenotyping individuals based on similar clinical or molecular characteristics can aid in guiding appropriate therapeutic management. Treatable traits, characteristics for which evidence exists for a specific favorable treatment response, are increasingly incorporated into COPD clinical guidelines. But the COPD phenotyping literature is evolving. Innovations in lung imaging and physiologic metrics, as well as omics technologies and biomarker science, are contributing to a better understanding of COPD heterogeneity. This review summarizes the evolution of COPD phenotyping, the current use of phenotyping to direct clinical care, and how innovations in clinical and molecular approaches to unraveling disease heterogeneity are refining our understanding of COPD phenotypes.

Abstract:Background: In patients with chronic diseases, including those with chronic obstructive pulmonary disease (COPD), knowledge on the disease and its self-management is considered as relevant for improving disease control and long-term outcome. We studied to which extent components of knowledge depended on potential predictors, such as participation in educational programs and disease severity. For example, the perception of exacerbations or GOLD grade might modulate the content and reliability of COPD understanding. Methods: Data from the German COSYCONET 2 COPD cohort was used, and 13 questions addressing knowledge and self-management were analyzed. Results: Overall, 310 patients with the diagnosis of COPD of GOLD grades 1/2/3/4 as well as the former grade 0 were included (39.7% female, median age 66.0 years). The answers to 3 questions (knowledge of term exacerbation, pursed lip breathing technique, criteria for contacting a doctor) were improved (p < 0.05 each) when patients had had moderate-to-severe exacerbations, and to one question (breathing exercise) when having COPD grade 3/4 versus 0/1/2. The other 9 responses did not depend on disease severity, but most of the knowledge was improved when having participated in an educational COPD program. This was particularly true for knowledge that also depended on exacerbations, or if the treating physician was a pulmonary specialist. In some responses, the proportion of correct answers was significantly reduced in males compared to females. The dependence on education level, existence of a treatment plan, self-reported level of risk aversion and low depression score was weak and heterogeneous. Conclusion: These findings suggest that part of the disease-related knowledge in patients with COPD was reinforced by the experience of exacerbations, especially knowledge regarding criteria on contacting a physician. These observations might help in focusing education on those parts of knowledge that are considered as important by patients based on their own experience.
