Abstract:Dental restorative resin composites are a pivotal class of organic/inorganic hybrid materials. The main reasons for their failure are secondary caries and dental composite fractures, which prevent their long‐term service in posterior restorations. A remedy to these problems is improving the interfacial adhesion between the resin matrix and the fillers. Surface‐modified inorganic fillers show strong potential as more than just a bond between these two phases and can therefore be considered a medium to compensate for the deficits of dental composites. This paper mainly discusses strategies to improve the interfacial adhesion of dental composites, including surface treatments of fillers with coupling agents, polymers, and tetraethyl orthosilicate (TEOS) coatings, as well as the selection of porous fillers capable of micromechanical resin matrix/filler interlocking. Additionally, the existing deficiencies of these strategies and the prospects for dental composites are discussed. Dental restorative composites are vulnerable to unsatisfactory mechanical properties, which are due in part to the degradation of the matrix/filler interfacial bonding during service. The main modification strategies to improve their interface adhesion are discussed in this work, including surface treatments of fillers with coupling agents, polymers, tetraethyl orthosilicate (TEOS) coating, and the selection of porous fillers.
A Multifunctional Dental Resin Composite with Sr-N-Doped TiO2 and n-HA Fillers for Antibacterial and Mineralization Effects

Abstract:Dental caries, particularly secondary caries, which is the main contributor to dental repair failure, has been the subject of extensive research due to its biofilm-mediated, sugar-driven, multifactorial, and dynamic characteristics. The clinical utility of restorations is improved by cleaning bacteria nearby and remineralizing marginal crevices. In this study, a novel multifunctional dental resin composite (DRC) composed of Sr-N-co-doped titanium dioxide (Sr-N-TiO2) nanoparticles and nano-hydroxyapatite (n-HA) reinforcing fillers with improved antibacterial and mineralization properties is proposed. The experimental results showed that the anatase-phase Sr-N-TiO2 nanoparticles were synthesized successfully. After this, the curing depth (CD) of the DRC was measured from 4.36 ± 0.18 mm to 5.10 ± 0.19 mm, which met the clinical treatment needs. The maximum antibacterial rate against Streptococcus mutans (S. mutans) was 98.96%, showing significant inhibition effects (p < 0.0001), which was experimentally verified to be derived from reactive oxygen species (ROS). Meanwhile, the resin exhibited excellent self-remineralization behavior in an SBF solution, and the molar ratio of Ca/P was close to that of HA. Moreover, the relative growth rate (RGR) of mouse fibroblast L929 indicated a high biocompatibility, with the cytotoxicity level being 0 or I. Therefore, our research provides a suitable approach for improving the antibacterial and mineralization properties of DRCs.

Abstract:Highlights • Fluorinated urchin-like hydroxyapatite (FUHA) was designed with novel structure. • FUHA filled dental resin composite (F5) showed well physicochemical performances. • F5 can remineralize dentin, protect pulp, and inhibit bacteria for caries repair. • F5 is promising in dentin caries repair, especially for deep lesions near the pulp. Caries is one of the most prevalent human diseases, resulting from demineralization of tooth hard tissue caused by acids produced from bacteria, and can progress to pulpal inflammation. Filling restoration with dental resin composites (DRCs) is currently the most common treatment for caries. However, existing DRCs suffer from low fracture strength and lack comprehensive anti-caries bioactivity including remineralization, pulp protection, and anti-cariogenic bacteria effects. In this study, inspired by plant roots' ability to stabilize and improve soil, fluorinated urchin-like hydroxyapatite (FUHA) with a three-dimensional whisker structure and bioactive components of calcium, phosphorus, and fluorine was designed and synthesized by a dynamic self-assembly method. Furthermore, versatile FUHA particles with different loading fractions were used as functional fillers to fabricate methacrylate-based DRCs, where the urchin-like hydroxyapatite (UHA) filled DRCs and commercial DRCs (Z350XT and BEAUTIFIL II) served as the control groups. The results demonstrated that FUHA with 50 wt% loading in resin matrix endowed DRC (F5) with excellent physicochemical properties, dentin remineralization property, cell viability, promotion of dental pulp stem cells mineralization, and antibacterial properties. Meanwhile, F5 also presented good clinical handling and aesthetic characteristics. Therefore, structure/functional-integrated FUHA filled DRCs have potential as a promising strategy for tooth restoration and anti-caries bioactivity. Abstract:Dental composites are comprised of a polymerizable matrix and reinforcing fillers that can be hardened into a solid restoration in the prepared tooth cavity. Composites are becoming increasingly popular due to their esthetics and improved mechanical and physical properties. However, dental composites still encounter several problems, mainly secondary (recurrent) caries, restoration fracture, excessive wear, marginal degradation, and tooth sensitivity. Therefore, extensive efforts are underway to improve the composite compositions and microstructure, and enhance their clinical performance and longevity. Relying on advances in materials science and technology, dental composites have been continuously improved and their clinical applications have been expanded. In this review article, the development of dental composites was summarized, including compositional changes, performance improvements in key areas, current research hot spots, and outlook for future direction. The intention is to provide a review of the history and development of dental composites with a discussion of strategies on addressing the current challenges facing dental composites. In addition, this review will provide a better understanding of dental composites and their properties for the practicing clinicians, to contribute to improving the quality of composite restorations. The review of literature indicates that while current composites are generally bio‐inert and can replace the missing tooth structure, future composites should be bioactive and therapeutic to inhibit caries, modulate biofilms, and protect the surrounding tooth structures, in order to increase the restoration longevity. © 2019 Wiley Periodicals, Inc. Abstract:It has been stated clearly that nanofillers could make an enhancement on the mechanical performances of dental composites. In order to address current shortage of traditional dental composites, fillers in forms of nanofibers or nanotubes are broadly regarded as ideal candidates to greatly increase mechanical performances of dental composites with low content of fillers. In this review, the efforts using nanofibers and nanotubes to reinforce mechanical performances of dental composites, including polymeric nanofibers, metallic nanofibers or nanotubes, and inorganic nanofibers or nanotubes, as well as their researches related, are demonstrated in sequence. The first purpose of current paper was to confirm the enhancement of nanofibers or nanotubes’ reinforcement on the mechanical performances of dental restorative composite. The second purpose was to make a general description about the reinforcement mechanism of nanofibers and nanotubes, especially, the impact of formation of interphase boundary interaction and nanofibers themselves on the advanced mechanical behaviors of the dental composites. By means of the formation of interface interaction and poststretching nanofibers, reinforced effect of dental composites by sorts of nanofibers/nanotubes has been successfully obtained.
Comparative Assessments of Dental Resin Composites: A Focus on Dense Microhybrid Materials

Abstract:The performance of dental resin composites is crucially influenced by the sizes and distributions of inorganic fillers. Despite the investigation of a variety of functional particles, glass fillers and nanoscale silica are still the predominant types in dental materials. However, achieving an overall improvement in the performance of resin composites through the optimization of their formulations remains a challenge. This work introduced a "dense" microhybrid filler system with 85 wt % filler loading, leading to the preparation of self-developed resin composites (SRCs). Comparative evaluations of these five SRCs against four commercial products were performed, including mechanical property, polymerization conversion, and shrinkage, along with water sorption and solubility and wear resistance. The results showed that among all SRC groups, SRC3 demonstrated superior mechanical performance, high polymerization conversion, reduced shrinkage, low water absorption and solubility, and acceptable wear resistance. In contrast to commercial products, this optimal SRC3 material was comparable to Z350 XT in flexural and diametral tensile strength and better in flexural modulus and surface hardness. The use of a "dense" microhybrid filler system in the development of resin composites provides a balance between physicochemical property and wear resistance, which may be a promising strategy for the development of composite products.

Abstract:Preventing the occurrence of secondary caries serves as one of the significant issues in dental clinic, thus make it indispensable to improving the properties of conventional composite resin (CR) by developing a novel CR. In present study, two groups of experimental CRs loaded with different contents of fluoride-doped nano-zirconia fillers (25 wt% and 50 wt%) were fabricated. The surface topography, mechanical performance, fluoride release, antibacterial effect, aging property and cytotoxicity of the experimental CRs were evaluated subsequently. A uniform distribution of the F-zirconia fillers over the whole surface of resin matrix could be observed. The experimental CRs showed continuous fluoride release within 28 days, which was positively correlated with the content of F-zirconia fillers. Moreover, the amount of fluoride release increased in the acidic buffer. Addition of F-zirconia fillers could improve the color stability, wear resistance and microhardness of the experimental CRs, without reducing the flexure strength. Furtherly, the fluoride ions released continuously from the experimental CRs resulted in effective contact and antibacterial properties, while they showed no cytotoxicity. As a consequence, considerations can be made to employ this new kind of composite resin loaded with fluoride-doped nano-zirconia fillers to meet clinical requirements when the antimicrobial benefits are desired.

Abstract:Dental caries (tooth decay) is the most frequent oral disease in humans. Filling cavities with a dental restorative material is the most common treatment, and glass ionomer cements are the main fluoride ion release restorative materials. The goal of this study was to develop a restorative compound with superior fluoride ion release and recharge abilities. Previously developed fluorinated bentolite and hydrophobized 3YSZ were used as two different inorganic fillers mixed in a bisphenol A-glycidyl methacrylate (Bis-GMA) matrix. XRD, FTIR, and TGA were used to determine the hydrophobic modification of these two inorganic fillers. In mechanical tests, including diameter tensile strength, flexural strength, and wear resistance, the developed composite resin was significantly superior to the commercial control. A WST-1 assay was used to confirm that the material displayed good biocompatibility. Furthermore, the simulation of the oral environment confirmed that the composite resin had good fluoride ion release and reloading abilities. Thus, the composite resin developed in this study may reduce secondary caries and provide a new choice for future clinical treatments.

Abstract:OBJECTIVES: To present an overview of fiber-reinforced composites (FRCs) that are a group of non-metallic dental biomaterials used in several fields of dentistry.METHODS: A range of relevant publications from the past half century are surveyed, with emphasis upon recent publications.RESULTS: FRCs vary according to the type of fiber fillers and orientation of fibers, the latter being responsible for several properties which can vary from isotropic to anisotropic. The length of the fibers, i.e. the aspect ratio of the filler, is another factor or variable that contributes to the properties and the development of new types of composite resins for restorative and prosthetic dentistry, as well as to reconstructive medicine.SIGNIFICANCE: Understanding the anisotropic nature of FRCs from the perspective of dental applications has increased in recent years. This review describes some fiber orientation related anisotropic properties of FRCs which contribute to the increased use of FRCs in clinical dentistry.

Abstract:This research work aims to represent the renovations in the physical and mechanical properties of nano-hydroxyapatites (nHAPs) filled dental restorative composite (DRCs). The concept of adhesive bonding has revolutionized adhesive dentistry. New or upgraded products are constantly being evolved with enhanced properties over their forebears and being introduced in the market. To achieve better interfacial strength and evaluate the effect of coupling agents, nano-hydroxyapatites particles were surface functionalizes with two different coupling agents (namely (3-methacryloxypropyl) trimethoxy silane (MPTS) and (3-aminopropyl) triethoxysilane (APTES). These nanoparticles were used in the formulation of high strength, photo-curable DRCs with varying the wt. % (0, 4, 8 and 12) of functionalized nHAPs. The matrix of the DRCs consists of bisphenol A-glycidyl methacrylate (Bis-GMA) as a base monomer and Tri-ethylene glycol dimethacrylate (TEGDMA) and 2-hydroxyethyl-methacrylate (HEMA) were used as a diluent monomer. The experimental investigations indicate that void contents and depth of cure of the composite shows the increasing trend with an increase in the functionalized filler particles. Hardness and compressive strength of the DRCs were improved by 39.52% for 12 wt.% MPTS treated (12 M) and 29.33% for 8 wt.% APTES treated (8 A) fillers respectively. However, the Flexural strength of the composite decreases with increases in the filler amount from 173.76 MPa to 122.84 MPa and further up to 83.63 MPa, but still this value is higher than that of an acceptable limit of flexural strength for dentistry (i.e. 74.2 MPa). Since it can be concluded that functionalized nHAPs filled dental composite is a promising material and has the potential to become a successful substitute for restorative dentistry.

Spray-drying-assisted fabrication of CaF2/SiO2 nanoclusters for dental restorative composites

Abstract:Objective: The objective of this study is to develop novel CaF2/SiO2 nanoclusters (NCs) fillers, which can endow the dental resin composites (DRCs) with excellent mechanical properties, stable and sustained fluoride ion release, and good antibacterial activity. Methods: The CaF2/SiO2 NCs were efficiently fabricated by assembling CaF2/SiO2 nanoparticles (NPs) as building blocks with a spray-drying technology. CaF2/SiO2 NCs with different SiO2 coating amounts (20 wt%, 50 wt% and 80 wt%) were incorporated into the DRCs at the filler content of 55 wt% for the measurement of mechanical properties including flexural strength, flexural modulus, compressive strength, and hardness. The effect of the filling amount of CaF2/50SiO2 NCs (50 represents 50 wt% SiO2 coating amount) in the DRCs was investigated, while CaF2/50SiO2 NPs were adopted as comparison group. The fluoride ion release and antibacterial activity of the DRCs with the optimal mechanical performances were evaluated. Furthermore, the statistical analyses were performed for mechanical properties. Results: Spherical CaF2/50SiO2 NCs with an average size of 2.4 µm were obtained at the feed rate of 7.4 mL/min and the CaF2/50SiO2 NPs solid content of 2 wt% in the suspension. The optimum comprehensive performances of the DRCs can be achieved by filling 55 wt% CaF2/50SiO2 NCs. Compared with CaF2/50SiO2 NPs, the filling amount of CaF2/50SiO2 NCs was increased by 5 wt% (50-55 wt%), and under the same filling amount of 50 wt%, the flexural strength, flexural modulus, compressive strength, and hardness of the DRCs containing CaF2/50SiO2 NCs were improved by 9.8%, 17.7%, 7.5% and 69.8%, respectively. Furthermore, the DRCs filled with 50 wt% CaF2/50SiO2 NCs exhibited more cumulative F-release by 126% and more stable F-release rate than the counterpart filled with 50 wt% CaF2/50SiO2 NPs after immersed for 1800 h. And 55 wt% CaF2/50SiO2 NCs filled DRCs could inhibit the growth of S. mutans, reaching an antibacterial ratio of 93%. Significance: The spray-dried CaF2/50SiO2 NCs are promising fillers for the development of high-performance multifunctional DRCs.

Abstract:Restorative composites have evolved significantly since they were first introduced in the early 1960s, with most of the development concentrating on the filler technology. This has led to improved mechanical properties, notably wear resistance, and has expanded the use of composites to larger posterior restorations. On the organic matrix side, concerns over the polymerization stress and the potential damage to the bonded interface have dominated research in the past 20 y, with many "low-shrinkage" composites being launched commercially. The lack of clinical correlation between the use of these materials and improved restoration outcomes has shifted the focus more recently to improving materials' resistance to degradation in the oral environment, caused by aqueous solvents and salivary enzymes, as well as biofilm development. Antimicrobial and ester-free monomers have been developed in the recent past, and evidence is mounting for their potential benefit. This article reviews literature on the newest materials currently on the market and provides an outlook for the future developments needed to improve restoration longevity past the average 10 y.

Abstract:Repairing tooth defects with dental resin composites is currently the most commonly used method due to their tooth-colored esthetics and photocuring properties. However, the higher than desirable failure rate and moderate service life are the biggest challenges the composites currently face. Secondary caries is one of the most common reasons leading to repair failure. Therefore, many attempts have been carried out on the development of a new generation of antimicrobial and therapeutic dental polymer composite materials to inhibit dental caries and prolong the lifespan of restorations. These new antimicrobial materials can inhibit the formation of biofilms, reduce acid production from bacteria and the occurrence of secondary caries. These results are encouraging and open the doors to future clinical studies on the therapeutic value of antimicrobial dental resin-based restoratives. However, antimicrobial resins still face challenges such as biocompatibility, drug resistance and uncontrolled release of antimicrobial agents. In the future, we should focus on the development of more efficient, durable and smart antimicrobial dental resins. This article focuses on the most recent 5 years of research, reviews the current antimicrobial strategies of composite resins, and introduces representative antimicrobial agents and their antimicrobial mechanisms.

Abstract:Direct composite resin restoration is the primary treatment for dental caries. However, the polymerization contraction of resin usually results in microgap formation at the resin/dentin interface, which inevitably accommodates cariogenic bacteria and their biofilm, thus eventually leading to secondary caries, one of the most important causes accounting for the failure of dental restoration. Inhibiting cariogenic bacteria adhesion and promoting dentin remineralization are common approaches to prevent secondary caries. However, the former is prone to cause unexpected side-effects and the latter can only temporarily slow down, but not completely prevent the development of secondary caries. Therefore, strategies to avoid these microgaps, thus preventing secondary caries are highly demanding. Here, we developed a growable dental composite resin, which is characterized by its fast formation of large quantity of hydroxyapatite upon reacting with physiological medium, for example, saliva or body fluid, empowered by bioactive glass fillers. The newly formed hydroxyapatite provides a perfect sealing of resin/dentin microgaps, successfully achieving a seamless dental caries filling. The moderate hydrophobic modification on the surface of bioactive glass enables the composite resin to possess excellent mechanical properties, thereby overcoming the contradiction between their mineralization capability and interfacial compatibility. This work provides an innovative and facile approach to tackle the challenge of secondary dental caries, thus may have immediate clinical significance.
Mechanical and Antimicrobial Properties of Boron Nitride/Methacrylic Acid Quaternary Ammonium Composites Reinforced Dental Flowable Resins

Abstract:Dental resin composites (DRCs) are commonly used to restore teeth affected by dental caries or defects. These materials must possess excellent properties to withstand the complex oral environment. The objective of this study was to prepare and characterize Boron nitride nanosheets (BNN)/ dimethyl amino hexadecyl methacrylate (DMAHDM) composites (BNN/DMA), and to evaluate them as functional fillers to enhance the mechanical and antimicrobial properties of dental resins. The BNN/DMA composites were successfully prepared under the theoretical guidance of molecular dynamics (MD), and then the physicochemical and morphological characterization of the BNN/DMA composites were carried out by using various test methods, such as FT-IR, XRD, UV-vis spectroscopy, SEM, TEM, and AFM. It was doped into the dental flowable resin in a certain proportion, and the results showed that the flexural strength (FS), elastic modulus (EM), compressive strength (CS), and microhardness (MH) of the modified resin composites were increased by 53.29, 47.8, 97.59, and 37.1%, respectively, with the addition of 0.8 wt % of BNN/DMA composite fillers. It has a good inhibition effect on Streptococcus mutans, with an inhibition rate as high as 90.43%. Furthermore, this effect persists even after one month of aging. In conclusion, the modification of flowable resins with low-concentration BNN/DMA composites favorably integrates the mechanical properties and long-term antimicrobial activity of dental resins. At the same time, they have good biocompatibility and do not affect the aesthetics. The BNN/DMA composite modified flowable resin has the potential to become a new type of antimicrobial dental restorative material.

However, exposed collagen fibers are found in the hybrid layer of the resin-dentin bonding interface due to incomplete resin penetration. As a result, the hybrid layer is susceptible to attack by internal and external factors such as hydrolysis and enzymatic degradation, and the durability of dentin bonding remains limited. Therefore, efforts have been made to improve the stability of the resin-dentin interface and achieve long-term clinical success. New ion-releasing adhesive resin materials are synthesized by introducing remineralizing ions such as calcium and phosphorus, which continuously release mineral ions into the bonding interface in resin-bonded restorations to achieve dentin biomimetic remineralization and improve bond durability. As an adhesive resin material capable of biomimetic mineralization, maintaining excellent bond strength and restoring the mechanical properties of demineralized dentin is the key to its function. This paper reviews whether ion-releasing dental adhesive materials can maintain the mechanical properties of the resin-dentin bonding interface by supplementing the various active ingredients required for dentin remineralization from three aspects: phosphate, silicate, and bioactive glass.

Abstract:Resin-based materials have been prevalent for dental restorations over the past few decades and have been widely used for a variety of direct and indirect procedures. Typically, resin-based dental materials are required to be flowable or moldable before setting and can provide adequate mechanical strength after setting. The setting method may include, but is not limited to, light-curing, self-curing or heating. In this review, based on different indications of resin-based dental materials (e.g., dental filling composite, dental bonding agent, resin luting cement), their rheological and mechanical properties were reviewed. Viscous and flexible properties were focused on for materials before setting, while elastic properties and mechanical strength were focused on for materials after setting. At the same time, the factors that may affect their rheological and mechanical properties were discussed. It is anticipated that the insightful information and prospections of this study will be useful to the future development and fabrication of resin-based dental restorative materials.

The Effect of Inorganic Filler Content on the Properties of BPA-Free Bulk-Fill Dental Resin Composites

Abstract:This study aimed to enhance the performance of dental resin composites (DRCs) by increasing the content of inorganic fillers while addressing potential health risks associated with Bisphenol A (BPA). To achieve this, the BPA-based resin monomer Bis-GMA was replaced with BPA-free Bis-EFMA. The study then explored the impact of varying inorganic filler contents on the physiochemical properties of Bis-EFMA-based bulk-fill dental resin composites (BF-DRCs). Four distinct Bis-EFMA-based BF-DRCs were formulated, each with different inorganic filler contents ranging from 70 wt% to 76 wt%. The study tested the depth of cure (DOC), double-bond conversion (DC), water sorption (WS), solubility (SL), and cytotoxicity of the system. It notably investigated the effects of increasing filler content on mechanical properties through flexural strength (FS), flexural modulus (FM), Vickers microhardness (VHN), and wear resistance, as well as the impact on polymerization shrinkage, including volumetric shrinkage (VS) and shrinkage stress (SS). To assess the commercial application potential of Bis-EFMA-based BF-DRC, the research used the commercially available BF-DRC Filtek Bulk-Fill Posterior (FBF) as a control. The results indicated that a higher filler content did not affect the DOC of Bis-EFMA-based BF-DRCs. Inorganic fillers at higher concentrations significantly enhanced overall mechanical properties while significantly reducing volumetric shrinkage (VS; p < 0.05). When the concentration of inorganic fillers in the resin system reached 76 wt%, most of the performance of the Bis-EFMA-based BF-DRC surpassed that of the commercial control FBF, except for FS, FM, and SS. These findings highlight the potential of Bis-EFMA-based BF-DRC as a long-term restorative material for dental applications.

Abstract:Adhesive failure due to resin contraction is one of the major reasons for dental restoration failure, which leads to the exposure of dentinal tubules, and remineralization in saliva would provide a great solution for the above problem. In this study, bioactive amphiphilic raspberry-like composite nanoparticles were used as fillers for resin composites, which have good compatibility with the resin matrix and dispersed well in the matrix. Thus, the resin composites showed improved mechanical property and resistance to water sorption and solubility. Furthermore, the incorporation of bioactive nanoparticles endued the resin composites with bioactivity, forming apatite on resin composites upon reacting with artificial saliva within seven days, inducing denser mineral precipitation on the dentin surface and stimulating human dental pulp cell attachment and proliferation. Therefore, this bioactive nanoparticle filled composite resin may offer great benefits for dental restoration.
