Abstract:Background Internationally, there is increased interest in reporting, treating and attempting to reduce complications of blood donation. Donor haemovigilance is the systematic monitoring of adverse reactions and incidents in the whole chain of blood donor care, with a view to improving quality and safety for blood donors. International Developments Several studies have described the risk factors for vasovagal reactions – female donors, young age, first‐time status, low body weight, etc. A water drink before donation, use of applied muscle tension and social support during donation have been found effective in reducing minor vasovagal reactions and/or increasing the likelihood of donors returning for subsequent donations. Haematomas and other venepuncture‐related complications are frequent and are the predominant cause of long‐term morbidity from complications of blood donation. International surveillance data on donor complications are captured in ‘ISTARE’ International Surveillance database for Transfusion‐associated Adverse Reactions and Events, which is being developed by the International Hemovigilance Network. Experience in The Netherlands: Better Monitoring of Donor Complications, Where Next? All donor complications and procedural problems are routinely recorded in the computer system eProgesa. Since 2009, the results have been monitored. An improved coding list was introduced in 2010 in order to record more details and cover new procedures such as Rhesus immunizations. The overall rate of donor complications increased from 0·64 in 2009 to 0·80 per 1000 whole blood collections (second half of 2010) and from 1·34 to 1·40 per 1000 plasmaphereses; there were no changes in donor demographics or procedures to explain this, so better recording is presumed. The recorded complications and procedural problems are regularly presented in team leaders’ and regional directors’ meetings. The data have been used to analyse the effect of a procedural modification: increasing the concentration of citrate solution used in plasmapheresis. The rate of terminating procedures because of citrate reactions was 0·14 per 1000 collections [95% confidence interval (CI) 0·018–0·27] before and 0·09 (95% CI 0·018–0·17) after the modification, signifying no relevant change. It is hoped that in 2012 a revised donor information brochure with instructions on avoiding vasovagal reactions will be piloted and subsequently tested. Nevertheless, only immediate and short‐term complications can be effectively addressed by the recording and analysis of complications. Longer term issues such as whole blood donor iron status are the subject of research in our country and abroad. Conclusion In our country as well as elsewhere, recording and monitoring the incidence of complications of blood donation has become the norm. Knowledge of risk factors and evidence‐based interventions will enable blood establishments worldwide to reduce donor complications. As yet, longer term complications are not well addressed.
The International Haemovigilance Network Database for the Surveillance of Adverse Reactions and Events in Donors and Recipients of Blood Components: technical issues and results

Abstract:BACKGROUND AND OBJECTIVES: The International Haemovigilance Network's ISTARE is an online database for surveillance of all adverse reactions (ARs) and adverse events (AEs) associated with donation of blood and transfusion of blood components, irrespective of severity or the harm caused. ISTARE aims to unify the collection and sharing of information with a view to harmonizing best practices for haemovigilance systems around the world.MATERIALS AND METHODS: Adverse reactionss and adverse events are recorded by blood component, type of reaction, severity and imputability to transfusion, using internationally agreed standard definitions.RESULTS: From 2006 to 2012, 125 national sets of annual aggregated data were received from 25 countries, covering 132.8 million blood components issued. The incidence of all ARs was 77.5 per 100 000 components issued, of which 25% were severe (19.1 per 100 000). Of 349 deaths (0.26 per 100 000), 58% were due to the three ARs related to the respiratory system: transfusion-associated circulatory overload (TACO, 27%), transfusion-associated acute lung injury (TRALI, 19%) and transfusion-associated dyspnoea (TAD, 12%). Cumulatively, 594 477 donor complications were reported (rate 660 per 100 000), of which 2.9% were severe.CONCLUSIONS: ISTARE is a well-established surveillance tool offering important contributions to international efforts to maximize transfusion safety.

Frequency of rare, serious donor reactions: International perspective

Abstract:Background: Severe blood donor adverse events are rare, but due to their rarity studying them can be difficult. To get an accurate estimate of their frequency and rate in the donor population it may be necessary to combine donation data across countries. Study design and methods: International blood collection organizations (BCOs) provided data on rare/severe donor reactions as well as denominator information for their donor populations from 2015 to 2017. Donor reactions were classified using standardized definitions. Results: BCOs from six countries provided reaction data for more than 22 million donations. A total of 480 rare reactions were reported of which 76.7% were imputed as definite and 11% probable. Rates of rare reactions were higher in females and first-time donors. Systemic rare reactions were the most common reaction type, accounting for over three quarters of reactions reported. Of systemic reactions, vasovagal reactions with loss of consciousness and injury or off-site (n = 350) made up the majority and occurred 1.53 per 100,000 donations. For the 22.3% that were localized reactions, the majority of these were cellulitis (n = 71, 0.31 per 100,000 donations) followed by deep venous thrombosis (n = 21, 0.09 per 100,000 donations). Conclusion: Pulling together data from multiple BCOs across countries allows for a better understanding of rare reactions, such as vasovagal reaction with injury or cellulitis, and for generating a reliable incidence rate for air embolism or compartment syndrome. However, gaps remain due to missing elements such as unknown donor status or location of reaction.
A national surveillance system for continuous monitoring of blood transfusion safety: German haemovigilance data

Abstract:Background and Objectives Haemovigilance (HV) systems aim to improve transfusion outcomes in patients and donor safety. An important question for blood regulators is how to ensure an effective HV system. Materials and Methods We retrospectively analysed the HV reports submitted to Paul‐Ehrlich‐Institut over the last two decades. Results Between 2011 and 2020, 50.86 million units of blood components were used, and 8931 suspected serious donor and recipient adverse reactions (SARs), 874 serious adverse events (SAEs) and 12,073 donor look‐backs were reported. Following implementation of specific risk‐minimization measures (RMMs) between 2000 and 2010, SAR reporting rates decreased for transfusion‐transmitted viral infections (TTVIs), transfusion‐related acute lung injury (TRALI) and transfusion‐transmitted bacterial infections (TTBIs), while increasing for other serious adverse transfusion reactions. Within this decade, the overall blood component use decreased. Conclusion Long‐term data collection forms the basis to establish trends and changes in reporting and to evaluate the effect of RMM. Standardized criteria for reaction types, seriousness and imputability assessments and availability of a denominator are important elements. Central data collection and independent assessment allow for monitoring HV data in a nationwide context over time. Stakeholder involvement and transparent feedback on the benefit of RMM will help to achieve the objectives of HV.

Abstract:INTRODUCTION: Complications associated with blood donation significantly lower odds of subsequent donations. The aim of the study is to assess the prevalence of complications related to blood donation, identify the influencing factors, and come up with suggestions for minimizing discomfort to donors and making outdoor voluntary blood donation camps safer.MATERIALS AND METHODS: This study covered 181 blood donation camps organized by Sankalp India Foundation where 16 blood banks participated from 01-04-2011 to 01-08-2014 in Karnataka. Uniform protocols for donor selection, predonation preparation, counseling, postdonation care, and refreshments were used. The postdonation complications were recorded on a form immediately, after they were observed.RESULTS: We observed 995 (3.2%) complications in 30,928 whole blood donations. Of these 884 (2.86%) mild, 77 (0.25%) moderate, and 5 (0.02%) severe complications were observed. Local symptoms (blood outside vessels, pain, and allergy) contributed 1.0%, and generalized symptoms (vasovagal reaction) contributed 2.2% to all the complications.CONCLUSION: We observed 322 complications for every 10,000 donations. Since 27 out of every 10000 experience moderate and severe complication, the readiness to manage complications is crucial. Women donors, young donors, and donors with a lower weight are at a significantly greater risk of experiencing complications, highlighting the need for specific guidelines for the management of higher risk donor groups. Complications varied significantly between various blood banks. Predonation hydration was effective in limiting complications with generalized symptoms. We recommend a robust donor hemovigilance program for voluntary blood donation for monitoring complications and enable assessment of effectiveness and implementation of appropriate interventions.
A rare vascular injury in a blood donor after whole blood donation

Abstract:Background Blood donation is a safe process though reactions may still occur. We describe a rare vascular complication in a frequent donor, with improvements in the collection process aimed at avoiding future events. Methods A 63‐year‐old woman presented with local pain and an apparent collection in the left arm 8 days after donation. Duplex ultrasound identified a superficial liquid collection and signs of arteriovenous fistula (AVF) between the cubital vein and an arterial branch. A computed tomography (CT)‐angio performed 1 day after ultrasound did not identify signs of AVF, followed by a new duplex which confirmed CT‐angio findings. It was assumed that a traumatic AVF evolved with spontaneous thrombosis. In the early follow‐up (18 days), a progressive regression of hematoma was observed without any sequelae. Results Investigation showed a faster whole blood bag collection time (3 min; normal: 5–9 min), and the processed packed red blood cell had a brighter red color than usual. The donor reported local bleeding after needle withdrawal, not observed in previous donations and a bruise forming on the same day. No arterial puncture (AP) was noticed by the collection staff during the procedure. The staff was retrained and actions were taken focusing on more active surveillance of late reactions, highlighting the importance of post‐donation information by the donors, regardless of any adverse reaction observed, to detect late complications. Conclusion We described an uncommon AP in a donor that was not identified, leading to an AVF that spontaneously thrombosed.

Risk factors for vasovagal reactions in blood donors: A systematic review and meta‐analysis

Abstract:Background While blood donation is generally safe, some donors experience vasovagal reactions (VVRs) that may lead to injury and reduce likelihood of future donation. Several risk factors for VVRs have been identified, but the consistency, magnitude, and validity of their associations have not been systematically evaluated. Therefore, this systematic review and meta‐analysis synthesized evidence for VVR risk factors. Methods Database searches identified English‐language studies published before February 2024 describing VVR risk factors in voluntary whole blood donors. Study characteristics, VVR and risk factor assessment methods, and effect sizes were extracted. Random‐effects models pooled estimates across all studies and subgroups of geographical context, study quality, donation experience, and outcome severity. Inconsistently and infrequently reported risk factors were narratively synthesized. Results Totally 71 studies reporting a total of 19 million total donations were included. Female sex, new donor status, younger age, smaller blood volume, and lower blood pressure were positively associated with higher VVR risk. Donation‐related fear, anxiety, and disgust were associated with higher VVR risk in narrative syntheses. Substantial between‐study heterogeneity (I2 > 90%) was observed for the majority of risk factors, while there was no clear evidence of subgroup variability and small study effects. Conclusion This systematic review and meta‐analysis provides a comprehensive synthesis of risk factors for VVRs across wide‐ranging blood service contexts and symptom severities, reinforcing evidence for previously identified factors. The heterogeneous associations of several risk factors motivate large‐scale studies that enable comprehensive multivariable adjustment to evidence donor selection criteria and preventative intervention allocation.

Abstract:Background: The occurrence of adverse reactions to blood donation significantly hampers donor retention and negatively impacts on the universal availability of adequate numbers of blood donor units. Objective: To analyze the spectrum and prevalence of adverse reactions in blood donors in a tertiary hospital-based blood bank in Nigeria. Subjects and Methods: The details of 3520 blood donors who presented for donation over a 12 months period were retrieved from the departmental archives for analysis. These included sociodemographic information, type of donor, type and frequency of adverse reactions to blood donation. Data were analyzed using the Statistical Package for Social Sciences version 20.0 (SPSS Inc., Chicago, IL, USA) computer software. Descriptive and inferential statistics were employed to represent the distribution of donor characteristics (as percentages) and compare reaction rates by gender and severity, respectively. Results: The prevalence of adverse reactions to blood donation was (56/3520) 1.60%; this occurred more frequently in male and family replacement donors (55.35% and 100.0%, respectively). The spectrum of donor adverse reactions included anxiety 25 (44.64%), generalized body weakness 11 (19.64%), hematoma 10 (17.86%), fainting 5 (8.93%), and vomiting 5 (8.93%). Vasovagal reactions were the most frequent adverse reaction encountered among the donors (46/56; 82.14%). Conclusion: Vasovagal reactions are common adverse phenomena in our blood donor set; this has implications on transfusion safety and blood donor retention.
Haemolytic transfusion reactions caused by non‐ABO red cell antibodies reported to the Norwegian Haemovigilance System 2004–2020

Abstract:Background and Objectives The aim of this study was to analyse the reports received in the Norwegian Haemovigilance System from 2004 to 2020 on acute and delayed haemolytic transfusion reactions caused by non‐ABO red cell antibodies. Materials and Methods Antibody specificity, clinical symptoms and outcomes were included when available. Results After transfusion of 3.7 million red cell concentrates, reports on 78 cases of haemolytic transfusion reactions caused by non‐ABO red cell antibodies were received, corresponding to an incidence of 1 in 47,000 transfused red cell concentrates. There were 30 acute and 48 delayed haemolytic transfusion reactions. A total of 113 red cell antibodies were found: 82 alloantibodies, 6 autoantibodies and 25 cases where the antibody specificity could not be determined. Two fatalities occurred: one caused by anti‐Wra and one caused by an unidentified red cell antibody. The most frequently reported antibody specificities were those in the Rh and Kidd blood group systems, representing 24% and 14%, respectively, of all the antibodies identified. In six cases, errors occurred, leading to the issuing of blood units without the required phenotype match. Conclusions Despite the possible underreporting, the low number of serious haemolytic transfusion reactions reflects an adequate pre‐transfusion practice by the Norwegian blood banks.

Abstract:Background Transfusion‐related errors are largely preventable but may lead to blood product wastage and adverse reactions, resulting in patient harm. In the United States, the incidence of transfusion‐related errors is poorly understood nationally. We used data from the National Healthcare Safety Network (NHSN) Hemovigilance Module to describe and quantify transfusion‐related errors, as well as associated transfusion‐related adverse reactions and blood product wastage. Methods During 2014–2022, data from the NHSN Hemovigilance Module were used to analyze errors, including near misses (errors with no transfusion), incidents (errors with transfusion), and associated serious adverse reactions (severe, life‐threatening, or death). Results During 2014–2022, 80 acute care facilities (75 adult; 5 pediatric) reported 63,900 errors. Most errors occurred during patient blood sample collection (21,761, 34.1%) and blood sample handling (16,277, 25.5%). Less than one‐fifth of reported errors (9822, 15.4%) had a completed incident form. Of those, 8780 (89.3%) were near misses and 1042 (10.7%) incidents. More than a third of near misses (3363, 38.3%) were associated with a discarded blood product, resulting in 4862 discarded components. Overall, 87 adverse reactions were associated with errors; six (7%) were serious. Conclusions Over half of the transfusion‐related errors reported to the Hemovigilance Module occurred during blood sample collection or sample handling. Some serious adverse reactions identified were associated with errors, suggesting that additional safety interventions may be beneficial. Increased participation in the Hemovigilance Module could enhance generalizability and further inform policy development regarding error prevention.

Abstract:Background Many blood banks use upper age limits for donors out of concern for a higher donor complication rate in older donors. Experienced donors are known to have lower donor complication rates, and older donors are often more experienced, confounding the effect of age on donor complication rate. Study Design and Methods We studied donor complication rates in whole blood, plasma, and plateletpheresis donors from 2012 to 2022. Donor complication rates were compared between age groups in inexperienced (<20th donation) and experienced (≥20th donation) donors. In addition to this direct comparison, we made use of logistic regression with finer‐grained experience groups, to further quantify the effects of age, experience and other factors on donor complication rate. Results While overall rate of vasovagal reaction was lower, rate of moderate/severe vasovagal syncope was highest in 70–79 year donors, however, only reached significance for plasma donors. Furthermore, rates of failed stab were highest in this age group. Hematoma rate showed a U‐shaped pattern with regard to age, where the rate was not higher in the 70–79 year age group than in the 18–23 year age group. Pain decreased with age, however, rates were higher in the 70–79 year age group than in the 65–69 year age group. Discussion When properly accounting for donor experience, donor complication rate profiles clearly change with age. The increased risk for moderate/severe vasovagal syncope in older donors should be clearly communicated. Extra caution is needed if these donors are accepted for first‐time donations.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> INTRODUCTION</h3><p>This report provides supplemental results from the 2017 National Blood Collection and Utilization Survey on characteristics of the donor population, autologous and directed donations and transfusions, platelets, plasma and granulocyte transfusions, pediatric transfusions, severe donor‐related adverse events, cost of blood units, hospitals policies and practices, and inventory, dosing, and supply.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> METHODS</h3><p>Weighting and imputation were used to generate national estimates including number of donors, donations, donor deferrals, autologous and directed donations and transfusions, severe donor‐related adverse events, platelet and plasma collections and transfusions, number of cross‐match procedures, irradiation and leukoreduction, and pediatric transfusions.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> RESULTS</h3><p>Between 2015 and 2017, successful donations decreased slightly by 2.1% with a 10.3% decrease in donations by persons aged 16‐18 years and a 14.4% increase in donations by donors aged >65 years. The median price paid for blood components by hospitals decreased from $211 to $207 for leukoreduced red blood cell units, from $523 to $517 for leukoreduced apheresis platelet units, and from $54 to $51 for fresh frozen plasma units. Plasma transfusions decreased 13.6%, but group AB plasma units transfused increased 24.7%.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> CONCLUSION</h3><p>Between 2015 and 2017, blood donations declined slightly because of decreases in donations from younger donors, but the number of donations from older donors increased. The price hospitals pay for blood has continued to decrease. Plasma transfusions have decreased, but the proportion of plasma transfusions involving group AB plasma have increased.</p></section>

Abstract:Objective Growing evidence suggests multiple pathophysiological mechanisms linking red blood cells (RBC) transfusions to thrombosis. This study examined blood donor, component, and recipient factors which may be associated with thromboembolic outcomes following RBC transfusion. Methods We utilized the Recipient Epidemiology Donor Evaluation Study‐III (REDS‐III) database on patients transfused in 12 hospitals between 2013–2016. Stratified Cox proportional hazards regression models with time‐dependent exposures were used to examine associations of donor and component modification characteristics on venous thromboembolism (VTE) in patients transfused RBC units. Results 59,603 patients were transfused 229,500 RBC units during 79,298 hospitalizations with post‐transfusion VTE occurring in 1869 (2.4%) of patients. In adjusted regression analyses, a per RBC‐unit risk of VTE was present for gamma irradiation (HR = 1.03; 95% CI: 1.02–1.03), female donor sex (HR = 1.01; 95% CI: 1.00–1.01), storage duration greater than 5 weeks (HR = 1.01; 95% CI: 1.01–1.02), AS‐1 storage solution (HR = 1.01; 95% CI: 1.00–1.01), and apheresis‐derived collections (HR = 1.01; 95% CI: 1.01–1.02). Among recipient factors, male sex (HR = 1.03; 95% CI: 1.02–1.04), pre‐transfusion hemoglobin level (HR = 0.94; 95% CI: 0.94–0.94), body mass index strata (HR = 1.11; 95% CI: 1.08–1.14), and principal diagnoses including malignancy (HR = 1.13; 95% CI: 1.10–1.16), cardiac arrest (HR = 1.38; 95% CI:1.07–1.77) and hip fracture (HR = 1.59; 95% CI:1.53–1.66) were associated with VTE in adjusted analyses. Discussion We identified several donor, component, and recipient‐specific factors associated with VTE in transfused hospitalized adult patients. In adjusted models, the dose‐dependent associations of donor and component‐specific factors with VTE were modest in contrast to those of recipient‐specific risk factors. Additional mechanistic and clinical studies linking blood donor and component factors with thrombotic outcomes are needed.

However, surveillance of these patients in a specialized paediatric surgery unit is essential because of the possibility of rare, but unavoidable complications. The authors review the 20-year experience of conservative management of hepatic trauma reported in the literature.

Abstract:Background Systems for quality and safety assurance in organ donation and transplantation are vital, especially those that seek to minimize donor disease transmission. Australia has developed a national vigilance and surveillance system to identify, review, and analyze actual and potential donor‐derived infections and other disease transmissions. Methods The system involves notification of incidents to the Australian Organ and Tissue Authority for review by a Vigilance and Surveillance Expert Advisory Committee (VSEAC). The VSEAC grades incidents, O makes recommendations, and issues communications both publicly and to the clinical donation and transplant sector. Results Annual notifications have increased since the inception of the system in 2012 until 2022. The vast majority relate to procedural aspects including donor assessment, information/data issues, and the recovery, offer, allocation, preservation and transportation of organs. Possible donor‐derived disease accounted for 19% of all notifications, and those related to possible donor‐derived infection only 12%. The VSEAC, as a result of reviewing these incidents, has made recommendations resulting in revisions to donor screening, organ allocation, packaging and transportation. The review of incidents has led to changes in clinical guidance for increased viral risk donor assessment, testing, and ensuing organ utilization and recipient surveillance. Guidance has also been reviewed for other infectious risks including strongyloides, human T‐lymphotropic virus, and HEV. Conclusion The Australian vigilance and surveillance system has enabled national retrospective reporting and evaluation of serious adverse events or reactions to identify trends and inform processes and guidelines, therefore improving the safety of donation and transplantation.

Abstract:Background and Objectives Implementation of national haemovigilance programmes has significantly improved donor and recipient safety. Recently, India completed a decade of successful implementation of its national haemovigilance programmes. The national programme is still enrolling more blood centres. This study aimed to highlight the strengths and weaknesses of Haemovigilance Programme of India (HvPI), thereby providing valuable insights for future initiatives. Materials and Methods The National Coordinating Centre (NCC) conducted a multi‐centre, cross‐sectional questionnaire‐based survey among the reporting blood centres (January to April 2022). The survey consisted of three sections with a total of 27 questions focusing on the demographics of the participant blood centre as well as the impact on the recipient and donor haemovigilance. The survey was sent to 733 blood centres regularly reporting to the donor and recipient HvPI through Donor and Hemovigil Software. Results Total 296 responses were received (response rate of 40.4%) with maximum participation of private non‐teaching hospital‐based blood centres (33.8%). After their involvement in recipient HvPI, 85.7% of the respondents reported changes in their blood centre's work procedures, with the maximum improvement seen in the documentation of transfusion reactions (92.7%). Out of the 278 respondents who participated in donor HvPI, 89.9% (250) found that their blood centre's policies or work process changed as a result of their involvement in the programme. Conclusion In conclusion, our haemovigilance programme facilitates national collaboration for learning and sharing experiences, leading to improved policies and practices in reducing adverse reactions for both recipients and donors.

Transfusion-related adverse events at the tertiary care center in North India: An institutional hemovigilance effort

Abstract:AIM: This study was designed to analyze the incidence and spectrum of adverse effects of blood transfusion so as to initiate measures to minimize risks and improve overall transfusion safety in the institute.MATERIALS AND METHODS: During the period from July 2002 to July 2003 all the adverse events related to transfusion of blood and blood components in various clinical specialties were recorded. They were analyzed and classified on the basis of their clinical features and laboratory tests. Attempt was also made to study the predisposing risk factors.RESULTS: During the study period 56,503 blood and blood components were issued to 29,720 patients. A total of 105 adverse reactions due to transfusion were observed during the study period. A majority of the adverse reactions was observed in hemato-oncology patients 43% (n = 45) and in presensitized patient groups 63% (n = 66). FNHTR 41% (n = 43) and allergic reactions 34% (n = 36) were the most common of all types of adverse transfusion reactions, followed by AcHTR 8.56% (n = 9). Majority of these AcHTR were due to unmonitored storage of blood in the refrigerator of wards resulting in hemolysis due to thermal injury. Less frequently observed reactions were anaphylactoid reactions (n = 4), bacterial sepsis (n = 4), hypervolemia (n = 2), hypocalcemia (n = 2), TRALI (n = 1), DHTR (n = 1), and TAGvHD (n = 1).CONCLUSION: Analysis of transfusion-related adverse outcomes is essential for improving safety. Factors such as improvement of blood storage conditions outside the blood bank, improvement in cross-matching techniques, careful donor screening, adherence to good manufacturing practices while component preparation, bedside monitoring of transfusion, and documentation of adverse events will help in reducing transfusion-related morbidity and mortality.

Abstract:Introduction Sanquin donor medicine department is informed when donations or their components are rejected. This can occur isolated or frequently. It is undesirable because the donations cannot be used and there may be an underlying medical cause. Based on regional approaches, a uniform procedure was developed. Methods Information about whole blood, plasma‐ plateletpheresis donations from which one or more components were rejected for filtration time (>2 h), hemolysis or clots were extracted from blood bank information system. After rejection of two successive components or donations or total ≥3 the donor is contacted. Depending on the medical history and investigation by the family doctor, the donor carrier is re‐evaluated. We looked for the causes of the discarded products and performed a survey among blood services regarding polices with discarded products. Results One or more components from 1742 of about 2.2 million successful donations (0.08%) were rejected. The highest percentage of rejection was seen in plateletpheresis (1.5%), all for clots. No underlying medical causes were found. 24 whole blood donors were found to have sickle cell trait (SCT) and were permanently deferred. The policies for follow‐up after discarded products or acceptance of SCT donors vary between the 16 blood banks. Six organizations do not follow‐up donors and seven accept SCT for blood or plasma donation. Conclusion Informing donors with repeated discarded products avoids the non‐use of donations. Causes of repeated discarded products can be found by follow‐up of donors. The results of the survey indicate a large discrepancy in policies applied worldwide.
