Abstract:ABSTRACT Classifying diffuse large B cell lymphomas, not otherwise specified (DLBCL, NOS), is based on their cell-of-origin (COO) which is included in the WHO classification (2016), is essential to characterize them better in context of prognostication. While gene expression profiling (GEP) considered the gold standard and more recently, the Nanostring-based approach, classify these tumors accurately, many laboratories with limited resources and instrumentation need an alternate approach that is reliable, inexpensive, and with a reasonable turnaround. The Reverse Transcriptase Multiplex Ligation Dependant Probe Amplification (RT-MLPA) to subtype DLBCL, NOS cases, as designed by CALYM group appears to provide a good alternative but needs to be validated in other centres. Therefore, this study evaluated DLBCL, NOS and compared the results of RT-MLPA to that obtained by immunohistochemistry using the Hans algorithm. Materials and Methods: Sixty-five DLBCL, NOS cases were included and the RT-MLPA was set up and standardized using probes that were designed by the CALYM study group. Briefly, RNA was extracted converted to cDNA and the 21-gene expression classifier that also included probes to detect MYD88 mutations and EBER mRNA was performed by MLPA. The results were analyzed by the open home grown software designed by the same group and compared to the results obtained by IHC. Results: Forty-four of the sixty-five cases provided concordant results (k = 0.35) and if the MYD88 results were to be used as a classifier the concordance would have improved from 67.7% to 82%. The 21 discordant cases were divided into five categories to provide a possible explanation for the discordance. Further 26% and 31% of the samples tested were positive for MYD88 mutations and EBER mRNA, respectively. The test had a turnaround of three days. Conclusion: The test provided moderate (67.7%) concordance when compared with IHC and perhaps would have provided higher concordance if compared with GEP. The test also has the advantage of providing information on the MYD88 and EBV infection status. It was found to be reliable, easy to perform and standardize, requiring only routine instruments available in most molecular laboratories. The RT-MLPA assay therefore provides an alternative for laboratories that would require subtyping of DLBCL, NOS cases in the absence of an access to GEP or other instrument intensive methods.

Abstract:7576 Background: Diffuse large B-cell Lymphoma (DLBCL) is a highly heterogenous disease. Gene microarrays were initially used to classify DLBCL into germinal center B-cell-like (GCB) or activated B-cell-like (ABC) Cell of Origin (COO) subtypes. ABC is associated with shorter overall survival. In newly diagnosed patients, COO classification by RNA profiling is a validated prognostic. A simpler immunohistochemical (IHC) staining of CD10, MUM1 and BCL6 is a proxy used in clinical practice in lieu of transcriptomics due to its expense, complexity and tissue requirements. Recent advances in the HTG EdgeSeq platform allow genome-scale profiling with minimal tissue. We successfully applied this novel technology to perform simultaneous COO classification, immune cell enrichment and tumor pathway analysis using a single FFPE slide. Methods: Accuracy of the HTG EdgeSeq panel (19,000 genes) was assessed in a head-to-head comparison with RNAseq using FFPE tumor samples (n = 8). DLBCL resections and core needle biopsies were commercially sourced and COO typed using Han’s algorithm into GCB or non-GCB (n = 65). Tumor locations included: lymphoid organs, gastrointestinal tract, testes, and the pleural cavity. EdgeSeq was performed on single slides with an average tissue area of 40mm 2 . Transcriptomic COO classification was performed using a linear combination of genes as described in Wright et al., PNAS 2003, substituting HTG platform-specific weights. Validated COO gene sets from literature and commercial diagnostic assays were tested. Immune cell gene signature enrichment analysis was performed using xCell (Aran et al., Genome Biol 2017); pathway analysis was performed with GSEA (Subramanian et al., PNAS 2005). Results: Gene expression levels estimated from whole transcriptome EdgeSeq on single slides were highly correlated to whole transcriptome RNAseq. Differential expression analysis of GCB vs non-GCB showed that key prognostic genes were detectable and enriched in the expected subtypes. Using these pre-established signatures, subtyping accuracy was ̃93% on the training set and 89% on the test set. Immune cell enrichment analysis identified class-switched memory B-cells as more prevalent in non-GCB subjects. This is consistent with emerging evidence that memory B-cells are the primary source of ABC DLBCL and not plasma cells (Venturutti & Melnick, Blood 2020). Pathway analysis identified genes regulated by the oncogene ic transcription factor MYC were enriched in non-GCB samples; MYC protein was found to be overexpressed in ABC in a large study (Hu et al., Blood 2013). Conclusions: Combined COO typing and whole transcriptome analysis from a single slide efficiently uses precious patient tissue. Longitudinal core needle sampling may yield insights into tumor evolution and therapeutic mechanisms of action across the DLBCL treatment landscape.

Abstract:Introduction: Targeted RNA sequencing using Next Generation Sequencing (NGS) has significant advantages over transcriptome sequencing. In addition to information on mutations, fusion and alternative splicing, RNA quantification using targeted RNA sequencing is sensitive, reproducible and provides better dynamic range. We used targeted RNA sequencing for RNA profiling of diffuse large B-cell lymphoma (DLBCL) and explored its utility in the sub-classification of DLBC to ABC and GCB. Method: RNA extracted from 441 FFPE lymphnode samples with DLBC lymphoma and sequenced targeting 1408 genes. These cases were previously subclassified as ABC vs GCB using expression profiling and immunohistochemistry. We first normalized RNA expression data to PAX5 expression, then we tried to narrow down important markers using univariate significance tests. Setting the cutoff for false discovery rate at 0.0001, 48 variables remained significant, including 46 RNA levels and two genes (MYD88 and EZH2) mutation status. Using 60% of samples as training set, we used multiple statistical approaches for classification. Deep learning emerged as the best approach. We used autoencoder with 5 hidden layers and developed a model for classification of ABC vs GCB. To further improve on classification, we divided patients in each subgroup based on survival using simple tree model. In this tree model, expression level of CD58 emerged as a powerful prognostic marker for the ABC group and RLTPR expression in the GCB group. Results: Using probability of scoring developed based on deep learning and logestic regression, approximately 30% of the cases had a score between 0.5 and 0.75. For the remaining 70% of patients, the ABC vs GCB classification showed sensitivity and specificity of 96% and 97% for the testing set. We also applied the same approach to 60 independent cases classified using NanoString (Lymph2Cx). Our model showed sensitivity and specificity of 96% and 97% in the NanoString independent cases. Using the tree model for further classification of the ABC and GCB classes, CD58 mRNA levels separated the ABC group into two subgroups (ABC1 and ABC2) and RLTPR mRNA separated the GCB into two subgroups (GCB1 and GCB2) with significant difference in overall survival (P=0.0010) and progression-free survival (PFS) (P=0.0027). Conclusion: Targeted RNA sequencing is very reliable and practical for the subclassification of DLBCL and can provide clinical-grade reproducible test for prognostically subclassification of DLBCL. Abstract:Several studies have shown that certain cytochrome-containing facultatively anaerobic bacteria do not form a competent terminal respiratory system when grown in the absence of oxygen. A similar phenomenon has been observed in the facultatively anaerobic yeast Saccharomyces cerevisiae. Anaerobically grown S. cerevisiae cells exhibit a much lower respiratory capacity and an altered cytochrome pattern when compared with cells grown aerobically. These physiological changes are accompanied by gross alterations in the appearance of the structures associated with the respiratory enzymes. Studies have indicated that typical yeast mitochondria are not present in cells grown anaerobically (2, 5). It seemed of interest to determine whether similar structural changes in membrane components would be observed in facultatively anaerobic bacteria when respiratory enzymes were diminished by anaerobic growth. Of special interest would be any changes in membranous organelles, such as the mesosomes, since these structures may be important sites of respiratory activity (4). In the present investigation, we have examined structural differences in aerobically and anaerobically grown cells of Staphylococcus epidermidis and Bacillus macerans by ultrathin sectioning and electron microscopy. Both organisms were capable of good growth under both aerobic and anaerobic conditions and exhibited a markedly decreased respiratory capacity and cytochrome content when grown anaerobically. Growth conditions for S. epidermidis strain AT 2 and measurement of cytochrome and respiratory activity have been previously described (1). B. macerans (ATCC 7048) was grown at 37 C in media containing 0.75% soluble starch, 0.5% tryptone, yeast extract, and 1.0% K2HPO4. Incubation and inoculation were done as previously described for S. epidermidis, except that anaerobic cultures were incubated in Erlenmeyer flasks filled to the neck and stoppered with cotton; vigorous production of CO2 and H2 during growth insured anaerobiosis. Cells were prepared for electron microscopy according to the procedures of Ryter and Kellenberger (3); specimens were embedded in Epon 812, sectioned with an LKB ultrotome, stained with uranyl nitrate and lead hydroxide, and examined with a Phillips EM-200 microscope. As previously described, anaerobically grown cells of S. epidermidis exhibit less than 15% of the cytochrome content and 10% of the respiratory rate of aerobically grown cells (1). Electron microscopy of thin sections of S. epidermidis (Fig. la, b) revealed mesosomes in both aerobically and anaerobically grown cells. Although the mesosome appeared to be less well defined in the anaerobically grown cell, this seemed to be a result of poor penetration of the fixative. The cell wall appeared to be thicker in anaerobically grown cells; this may account for the extreme difficulty we encountered in disrupting cells of anaerobically grown cells as compared to aerobically grown cells. When grown aerobically, cells of B. macerans exhibited cytochromes of the a2, a,, and b1 types (absorption maxima in reduced minus oxidized difference spectra at 628, 594, 560, 529 and 428 nm) and respired, with glucose as substrate, at the rate of 21 ,uliters of 02 per hr per mg of cells (dry weight). Abstract:Abstract Whole genome and exome sequencing studies have identified numerous genomic alterations in DLBCL, but these methods have limited applicability for the clinical care of lymphoma patients due to cost, specific tissue requirements, and laborious bioinformatic analysis. FoundationOne-Heme (FOH) is a novel next-generation sequencing platform designed to provide targeted assessment of the genomic landscape of hematologic malignancies, including identification of mutations within specific genes, copy number changes, and translocations. FOH can be performed on small quantities of formalin-fixed paraffin-embedded (FFPE) tissue, detect rare variants due to extensive depth of sequencing coverage, and rapidly provide results via streamlined bioinformatic interpretation. Here we report the first experience using this novel platform to evaluate the genetic landscape of DLBCL. Genomic DNA and total RNA were isolated from FFPE tissue on a cohort of 53 cases of DLBCL, including de novo (n=30), relapsed/refractory (n=12), and large cell transformation from low-grade lymphoma (n=11). The cohort included 25 cases with combined MYC and BCL2 overexpression by IHC (criteria for positivity: >40% MYC, >70% BCL2), of which only one had a known translocation involving MYC. Adaptor ligated sequencing libraries were captured by solution hybridization using two custom bait sets targeting 374 cancer-related genes and 24 genes frequently rearranged for DNA-seq, and 258 frequently-rearranged genes for RNA-seq. All captured libraries were sequenced to high depth (Illumina HiSeq), averaging >658x for DNA and >20,000,000 total pairs for RNA, to enable the sensitive and specific detection of genomic alterations. Significant non-synonomous variants were identified as mutations from the COSMIC database, amplifications of established oncogenes, or homozygous deletions and/or clear loss-of-function mutations of known tumor suppressors. The DNA sequencing component of FOH detected translocations in BCL2, BCL6, and MYC, while the RNA sequencing component detected fusion transcripts involving BLC6 and MYC, in agreement with independent cytogenetic analysis via karyotype and FISH where available. The assay detected copy number alterations of 44 different genes, most commonly amplification of REL (15%) or loss of CDKN2A/CDKN2B (17%). The most frequent alterations of known significance are detailed in Figure 1. The most commonly altered gene was CDKN2A, exhibiting either homozygous deletion or loss of function mutation in 28% of cases. Chromatin modifying factors (e.g. MLL2, CREBBP, EZH2) represented the most frequently altered biologic category with alterations occurring in >50% of cases. Recurrent alterations in components of the Notch pathway (NOTCH1/2/4, FBXW7, SPEN), each predicted to activate the pathway, were identified in 23% of cases. Cell-of-origin was determined as per the Hans model using IHC for CD10, BCL6, and IRF4/MUM1; CD79B mutations were detected exclusively in non-GCB and EZH2 mutations were found exclusively in GCB-phenotype cases. Furthermore, IHC MYC+/BCL2+ de novo DLBCL cases (n=11) exhibited more frequent hypermutation of PIM1 (46%) compared with the 19 cases of IHC MYC-/BCL2- de novo DLBCL (11%). When comparing the various clinical categories, we found that mutations in tumor suppressors were significantly more common in relapsed/refractory than de novo DLBCL (47% vs 75%, p=0.02). Alterations in TP53 were most frequently observed in transformed lymphoma (55%). Our results demonstrate the feasibility of using a targeted next-generation sequencing platform on FFPE clinical specimens from patients with DLBCL as a means of providing an integrated analysis of gene mutations, copy number alterations, and translocations. This streamlined approach combines multiple molecular and cytogenetic tests into a single platform and uses a small amount of tissue to perform a multifaceted assessment of genomic alterations with potential diagnostic, prognostic, and therapeutic implications. Future efforts will be directed at analyzing additional cases of DLBCL to better establish the biologic and clinical significance of the observed genetic alterations, and to prospectively incorporate this novel platform to select patients for mechanism-based targeted therapy. 1148P Identification and Validation of RET Fusions in Lung Adenocarcinoma Through DNA and RNA Sequencing

Abstract:RET fusion has emerged as a targetable oncogenic driver in non-small cell lung cancer. The optimal methods to identify actionable RET fusions during clinical practice remain to be evaluated. Herein, we performed a comparative analysis on RET fusions using targeted next-generation sequencing at DNA (DNA-seq) and RNA (RNA-seq) levels. A total of 53 lung adenocarcinoma patients harboring non-canonical RET fusions identified by DNA-seq were selected to receive RNA-seq of their remaining tumor samples using a 115-cancer gene panel (OncoRNA). Of them, 43 passed quality control of RNA-seq. The breakpoints and partners of RET-fusion identified by DNA- and RNA-Seq were described and compared. Of the 43 patients, DNA-seq identified 25 patients with concurrent 5’ RET fusions and 3’ canonical RET fusions (KIF5B-RET (K15:R12) or CCDC6-RET (C1:R12), subgroup A), 2 patients with 5’ RET fusions alone (subgroup B), 8 patients with frameshift or out-of-frame fusions (subgroup C) and 8 patients with 3’ non-canonical RET fusions alone (subgroup D). In total, 40 of 43 patients (93%) were positive for RET-fusions at RNA level. In subgroup A, RNA-seq detected all canonical RET-fusions with identical breakpoints to DNA-seq, while no transcripts of the 5’ RET fusions were detected. Patients in subgroups B and C did not have in-frame actionable fusions detected by DNA-seq. However, in subgroup B, RNA-seq identified 1 KIF5B-RET transcript with rare breakpoint (K24:R10). In subgroup C, only 1 patient was negative while 7 patients were positive at RNA level including 6 with canonical RET fusions of which the RET breakpoints were identical to DNA-seq. The discordance might be explained by complex DNA structural rearrangements and RNA splicing. In subgroup D, RNA-seq identified 7 of 8 patients with RET fusions concordant with DNA-seq, and 1 patient was negative. Most RET fusions in this subgroup involved rare partners including KIAA1468, LMNA, TRIM33, EDC4, ANKRD26, and AKAP13. DNA-seq and RNA-seq showed high consistency in detecting RET fusions. RNA-seq could be valuable in determining the actionability of 5’ or frameshift/out-of-frame DNA fusions.
High Accuracy, Low-Cost Transcriptional Diagnostic to Transform Lymphoma Care in Low- and Middle-Income Countries

Abstract:Introduction: The majority of people worldwide lack access to high accuracy diagnostics to guide lymphoma therapy. As a consequence, many patients receive incorrect or no treatment. We hypothesized that a low-cost, parsimonious gene expression assay using FFPE biopsies from low-income settings could distinguish multiple lymphoma subtypes. Accurate diagnoses would make it possible to extend high therapeutic index agents currently available within high-income countries to underserved patients around the world. Methods: We reviewed 900 patient cases from INCAN, the public cancer hospital in Guatemala City, for which a biopsy was obtained between 2006-2018 due to the clinician's suspicion for lymphoma. Whole-slide sections were assessed by H&E and then involved areas were embedded into tissue microarrays and analyzed at Stanford University according to the 2016 WHO classification (>35,000 individual IHC and FISH assessments). Consensus diagnosis for each case was made by two expert hematopathologists (YN and OS). Diagnoses were then binned into: high grade B-cell lymphoma (BCL), diffuse large B-cell lymphoma (DLBCL), follicular lymphoma (FL), Hodgkin's lymphoma (HL), mantle cell lymphoma (MCL), marginal zone lymphoma (MZL), NK/T-cell lymphoma (NKTCL), T-cell lymphoma (TCL) or non-lymphoma (NL). The latter included lymphoid hyperplasia, granulomatous inflammation, chronic inflammation, reactive follicular hyperplasia, angiomatous hamartoma, chronic gastritis, dermatitis, normal skin and tonsil. DLBCL cell-of-origin (COO) was classified based on Hans algorithm. We selected 37 genes based on studies that defined subtype-specific expression, DLBCL COO, or therapeutic relevance. We established a chemical ligation-based probe amplification (CLPA, DxTerity Diagnostics) assay that quantifies expression of the 37 genes (plus 2 normalizer genes) by routine capillary electrophoresis at a cost of <$10/sample. Candidate models were trained on data from the diagnostic samples using 10-fold cross validation with 5 repeats using the Classification And REgression Training (caret) package in R v.3.5.1. The data were split 70/30 into training and validation sets. A 2-staged classification approach was used to determine the sample's class label. Fourteen models were used as "base learners" and the class probabilities from each model were then used as predictors in a random forest ("super learner") to assign the class label with the highest probability value. An additional two-class model was developed using analogous methodology to classify samples called DLBCL in the first stage as non-GCB or GCB. Results: The 900 patient cases included >50 different malignant and non-malignant disorders. We selected 648 cases for gene expression analysis to ensure adequate statistical representation of major lymphoma types for model building and validation. FFPE scrolls were utilized for CLPA, of which 59 (9.1%) failed quality control and 38 were from patients with relapsed disease. Assay turnaround time was <7 days. 551 diagnostic samples were divided into 70% (n=391) training and 30% (n=160) validation cohorts (Table). Overall accuracy for the validation cohort was 88.8% [95% CI; 82.8, 93.2], with >90% accuracy for DLBCL, HL, MCL, NKTCL and NL (Table). Among cases diagnosed as BCL by standard IHC/FISH but DLBCL by CLPA, 3 of 4 had Ki67<50%, suggesting biology more similar to DLBCL. 6 of 7 misclassified MZL and FL cases were classified by CLPA as DLBCL, raising the possibility that small areas of transformed disease may have been present within the biopsies. Accuracy for DLBCL COO classification from the validation cohort compared to Hans algorithm staining (n=59) was 89.8% [95% CI; 79.2-96.1]. Accuracy for relapse samples as a test cohort (n=38) was >90% for DLBCL, FL, HL, MCL and NKTCL. Summary: Classification of biopsies into biologically- and therapeutically-relevant bins is feasible based on parsimonious, gene expression-based, statistical-learning algorithms. Importantly, this approach has high accuracy both for lymphoma subtypes and for non-lymphoma diagnoses. The assay is highly cost-effective, rapid, uses basic clinical laboratory equipment, and is now being performed on site at INCAN. In summary, a CLPA-based transcriptional assay could have broad utility across the globe for diagnosis, subtyping, COO classification and assessment of therapeutic targets within FFPE biopsies. Mutational profiling in acute lymphoblastic leukemia by RNA sequencing and chromosomal genomic array testing

Abstract:Background: Comprehensive molecular and cytogenetic profiling of acute lymphoblastic leukemia (ALL) is important and critical to the current standard of care for patients with B-acute lymphoblastic leukemia (B-ALL). Here we propose a rapid process for detecting gene fusions whereby FusionPlex RNA next-generation sequencing (NGS) and DNA chromosome genomic array testing (CGAT) are combined for a more efficient approach in the management of patients with B-ALL. Methods: We performed RNA NGS and CGAT on 28 B-ALL samples and, in four patients, compared fixed cell pellets to paired cryo-preserved samples as a starting material to further assess the utility of cytogenetic fixed pellets for gene expression analysis. Results: Among the fixed specimens, when using alternative techniques as references, including karyotype, fluorescence in situ hybridization, CGAT, and RT-qPCR, fusions were detected by RNA NGS with 100% sensitivity and specificity. In the four paired fixed versus fresh cryopreserved samples, fusions were also 100% concordant. Four of the 28 patients showed mutations that were detected by RNA sequencing and three of four of these mutations had well-known drug resistance implications. Conclusions: We conclude that FusionPlex is a robust and reliable anchored multiplex RNA sequencing platform for use in the detection of fusions in both fresh cryopreserved and cytogenetic fixed pellets. Gene expression data could only be obtained from fresh samples and although limited variant data are available, critical hotspot variants can be determined in conjunction with the fusions.

Abstract:Abstract Background: Diffuse large B cell lymphoma (DLBCL) is an aggressive malignancy of mature B cells. The disease has traditionally been subdivided into cell-of-origin (COO) subtypes - germinal centre B cell-like (GCB) or activated B cell-like (ABC) - as determined by expression profiling or immunohistochemistry of the tumor cells. However the role of the immune microenvironment, and how the tumor and immune system interact to influence patient outcomes, remains to be fully investigated. Methods: In this project, we used mass cytometry (CyTOF) to deeply profile both tumor cell phenotypes and the immune microenvironments, alongside ABC/GCB classification and mutation profiling, in a discovery cohort of 54 DLBCL cases. As well, a validation cohort of 129 DLBCL patients were immunologically profiled by high-dimensional conventional flow cytometry, and their immune profiles alongside ABC/GCB classification, mutation profiling, and RNAseq data, were correlated with patient outcomes as measured by progression-free survival (PFS). Results: Analysis of the CyTOF/discovery cohort demonstrated that DLBCL tumor cells are phenotypically unique to each patient, with a small number of samples displaying distinct sub-clonal structure, often distinguished by differential expression of immune-related proteins like MHC-II. ABC/GCB classifications could be recapitulated based on tumor cell phenotypes, demonstrating that while COO was a robust feature, a great deal of heterogeneity exists within these established subtypes. Immunological profiling of the CyTOF/discovery cohort revealed that DLBCL samples could be divided into three distinct groups which roughly correlated with abundances of naïve, activated, or terminally differentiated T cells, respectively. This profiling schema was extended to the validation cohort of 129 patients which in turn led to identification of a subset of patients with a very high risk of disease progression (5-year PFS; 30% high risk vs. 80% low risk, p<0.0001). This final classifier was based on a combination of ABC-DLBCL designation, combined with the presence of an immune microenvironment dominated by terminally differentiated (CD57+) T cells. We performed a limited series of functional studies using primary DLBCL biopsy samples to characterize further these CD57+ T cells as clonally restricted and incapable of responding to antigenic challenge. Interestingly, traditional immune markers of T cell exhaustion such as PD-1, TIM3, LAG3 and TIGIT were not correlated with patient outcomes. Conclusions: Overall, this study demonstrates the utility of immune profiling in risk stratification based on initial diagnostic biopsy material and highlights a subset of DLBCL patients who may benefit from immune-based therapies to rejuvenate the anti-tumor T cell response. We conclude that T cell senescence, rather than exhaustion, is the more relevant feature in DLBCL disease biology and highlights an alternate target for immunomodulatory therapy. Figure 1 Figure 1. Abstract:Diffuse large B-cell lymphoma (DLBCL) is the most common aggressive B-cell lymphoma with high clinical and biological heterogeneity. Despite current effective immunochemotherapy, up to 40% of patients do not respond or develop refractory disease. DLBCL is characterized by two major cell-of-origin (COO) subtypes, germinal center B cell-like (GCB) and activated B cell-like (ABC), with recent research uncovering additional molecular subgroups based on genomic alterations or tumor microenvironment (TME) features. However, current classifiers use bulk or deconvolution profiling which blurs intra-tumor heterogeneity, hindering the discovery of true functional cell states underlying clinical diversity. Here, we constructed a multimodal single-cell atlas of 62 DLBCL cases, integrating single-cell transcriptomes, immune repertoire and genetic features, to decrypt recurrent B cell states and TME ecosystems while identifying clinically relevant prognostic biomarkers (Fig.1). We performed simultaneous 5'-end single-cell RNA, B-cell receptor (BCR) and T-cell receptor sequencing on 63 DLBCL lymph node biopsies, recovering 109,294 cells after stringent quality controls. These biopsies were obtained from the real-world, clinically-annotated, multicentric CeVi collection and included 43 patient samples at diagnosis (ABC = 17, GCB = 11, Unclassified = 6, Not Evaluated = 9; 21 of which did not achieve event-free survival at 24 months (EFS24) after R-chemotherapy regimens), 11 relapsed/refractory DLBCL and 8 DLBCL transformed from indolent lymphoma. We distinguished malignant B-cells from TME using canonical markers and BCR sequence, exploring heterogeneity in both components. We developed an original bioinformatic approach to define malignant B cell transcriptional archetypes shared across patients. To assess whether subclonal genetics are associated with transcriptional states, we inferred copy number variations (CNV) and studied BCR intraclonal variations of malignant cells. We also integrated TME data (39,674 cells) to quantify subpopulations and clonally expanded T-cell subsets, and performed robust statistical analyses to identify clinically relevant features linked to patient outcomes. To circumvent interpatient heterogeneity in malignant B cells, we defined transcriptional cellular archetypes conserved across patients by combining 3 unsupervised methods: rank-biased overlap meta-clustering, variational autoencoder and canonical correlation analysis integration. We identified 5 cellular archetypes (Arch.1-5): Arch.1 was characterized by plasma cell identity, Arch.2 by MYC and NFKB pathways, Arch.3 by MYC pathway only, Arch.4 by quiescence markers, and Arch.5 by GC markers. Several archetypes co-existed in each tumor sample, and archetype identity was superimposed to the transcriptional profile conferred by COO molecular class as an additional layer of transcriptional heterogeneity. Cell state-specific CNV and BCR subclones were very rare, suggesting that subclonal genetic changes were not the main drivers of malignant B cell plasticity and archetypes. Kaplan-Meier analysis showed a significant negative association between proportion of Arch.4 cells and continuous EFS in our diagnosis cohort. Unsupervised analysis of archetype composition further distinguished ABC patients who achieved EFS24 (enriched in Arch.3 cells) from those with a particularly poor prognosis (enriched in Arch.4 cells). Integrated analysis of the TME revealed three major ecosystems, including one enriched for effector and cytotoxic T cells. In diagnosis samples, pseudo-temporal trajectory analysis revealed the enrichment of clonally-expanded hyperactivated CD8+ T cells expressing canonical exhaustion markers in patients achieving EFS24, suggesting a positive link between the extent of CD8+ T cell differentiation and response to R-CHOP based regimens. Shedding new light on DLBCL biological and clinical heterogeneity, our comprehensive single-cell atlas of DLBCL uncovered recurrent tumor cell states and TME features crucial for stratifying non-responsive ABC patients and guiding treatment decisions. Ongoing single-cell spatial transcriptomics analyses will be used to further validate our findings and elucidate archetype-TME interactions that may drive malignant B cell plasticity, with the potential to uncover novel therapeutic targets.

Comparison of Immunohistochemistry Assay Results with Gene Expression Profiling Methods for Diffuse Large B-Cell Lymphoma Subtype Identification in Matched Patient Samples

Abstract:Background: The cell of origin (COO) in diffuse large B-cell lymphoma (DLBCL) has prognostic importance. While the COO was originally classified into germinal center B-cell (GCB) or activated B-cell (ABC) subtypes by microarray analysis, routine use was not practical. Immunohistochemistry (IHC)–based methods are widely used with varying results due to lack of standardization. Several classification methods have been developed recently; understanding the concordance between these and existing methods is essential to their practical application. Therefore, we evaluated concordance between 3 commercial assays: a standardized Hans-based IHC method and 2 gene expression profiling (GEP) methods and compared these to the accepted microarray classification.Methods: 137 DLBCL-confirmed tumor samples were evaluated using a standardized Hans-based IHC method for GCB or non-GCB subtype, by a published microarray-based assay, a digital gene expression-based Lymphoma Subtyping Test (LST) and a next-generation sequencing-based assay (EdgeSeq COO). Subtype calls from the 3 GEP methods were harmonized to “GCB” or “non-GCB” and assessed for concordance.Results: Concordance between the Hans-based IHC assay and the microarray-based assay, LST assay and EdgeSeq assay was 79.6% (N=137), 80.0% (N=125) and 78.2% (N=64), respectively; the positive percent agreement (PPA) in non-GCB was 88.9%, 87.0% and 78.1%, respectively. Concordance for the Hans-based IHC assay versus GEP methods was especially high for direct GCB calls (91.0%, 88.3% and 78.1% for microarray, LST and EdgeSeq COO methods, respectively). The newly developed GEP assays performed well against the microarray GEP method, against which they were calibrated (concordance 93.7% and 87.5% and PPA 94.3% and 92.9%, respectively, for LST and EdgeSeq COO).Conclusion: These results demonstrated good consistency between various platforms for stratification of DLBCL into COO subtype classifications. Application of a standardized Hans-based IHC assay offers a robust, rapid and easily accessible platform to classify DLBCL into prognostically important subtypes.

Abstract:Abstract Background Diffuse large B-cell lymphoma (DLBCL) is a heterogenous blood cancer, but can be broadly classified into two main subtypes, germinal center B-cell-like (GCB) and activated B-cell-like (ABC). GCB and ABC subtypes have very different clinical courses, with ABC having a much worse survival prognosis. It has been observed that patients with different subtypes also respond differently to therapeutic intervention, in fact, some have argued that ABC and GCB can be thought of as separate diseases altogether. Due to this variability in response to therapy, having an assay to determine DLBCL subtypes has important implications in guiding the clinical approach to the use of existing therapies, as well as in the development of new drugs. The current gold standard assay for subtyping DLBCL uses gene expression profiling on formalin fixed, paraffin embedded (FFPE) tissue to determine the “cell of origin” and thus disease subtype. However, this approach has some significant clinical limitations in that it 1) requires a biopsy 2) requires a complex, expensive and time-consuming analytical approach and 3) does not classify all DLBCL patients. Methods Here, we took an epigenomic approach and developed a blood-based chromosome conformation signature (CCS) for identifying DLBCL subtypes. An iterative approach using clinical samples from 118 DLBCL patients was taken to define a panel of six markers (DLBCL-CCS) to subtype the disease. The performance of the DLBCL-CCS was then compared to conventional gene expression profiling (GEX) from FFPE tissue. Results The DLBCL-CCS was accurate in classifying ABC and GCB in samples of known status, providing an identical call in 100% (60/60) samples in the discovery cohort used to develop the classifier. Also, in the assessment cohort the DLBCL-CCS was able to make a DLBCL subtype call in 100% (58/58) of samples with intermediate subtypes (Type III) as defined by GEX analysis. Most importantly, when these patients were followed longitudinally throughout the course of their disease, the EpiSwitchTM associated calls tracked better with the known patterns of survival rates for ABC and GCB subtypes. Conclusion This proof-of-concept study provides an initial indication that a simple, accurate, cost-effective and clinically adoptable blood-based diagnostic for identifying DLBCL subtypes is possible.
A refined cell-of-origin classifier with targeted NGS and artificial intelligence shows robust predictive value in DLBCL

Abstract:Abstract Diffuse large B-cell lymphoma (DLBCL) is a heterogeneous entity of B-cell lymphoma. Cell-of-origin (COO) classification of DLBCL is required in routine practice by the World Health Organization classification for biological and therapeutic insights. Genetic subtypes uncovered recently are based on distinct genetic alterations in DLBCL, which are different from the COO subtypes defined by gene expression signatures of normal B cells retained in DLBCL. We hypothesize that classifiers incorporating both genome-wide gene-expression and pathogenetic variables can improve the therapeutic significance of DLBCL classification. To develop such refined classifiers, we performed targeted RNA sequencing (RNA-Seq) with a commercially available next-generation sequencing (NGS) platform in a large cohort of 418 DLBCLs. Genetic and transcriptional data obtained by RNA-Seq in a single run were explored by state-of-the-art artificial intelligence (AI) to develop a NGS-COO classifier for COO assignment and NGS survival models for clinical outcome prediction. The NGS-COO model built through applying AI in the training set was robust, showing high concordance with COO classification by either Affymetrix GeneChip microarray or the NanoString Lymph2Cx assay in 2 validation sets. Although the NGS-COO model was not trained for clinical outcome, the activated B-cell–like compared with the germinal-center B-cell–like subtype had significantly poorer survival. The NGS survival models stratified 30% high-risk patients in the validation set with poor survival as in the training set. These results demonstrate that targeted RNA-Seq coupled with AI deep learning techniques provides reproducible, efficient, and affordable assays for clinical application. The clinical grade assays and NGS models integrating both genetic and transcriptional factors developed in this study may eventually support precision medicine in DLBCL.

Abstract:<p>Diffuse large B‐cell lymphoma (DLBCL) is the most common histologic subtype of non‐Hodgkin lymphoma and is notorious for its clinical heterogeneity. Patient outcomes can be predicted by cell‐of‐origin (COO) classification, demonstrating that the underlying transcriptional signature of malignant B‐cells informs biological behavior in the context of standard combination chemotherapy regimens. In the current study, we used mass cytometry (CyTOF) to examine tumor phenotypes at the protein level with single cell resolution in a collection of 27 diagnostic DLBCL biopsy specimens from treatment naïve patients. We found that malignant B‐cells from each patient occupied unique regions in 37‐dimensional phenotypic space with no apparent clustering of samples into discrete subtypes. Interestingly, variable MHC class II expression was found to be the greatest contributor to phenotypic diversity. Within individual tumors, a subset of cases showed multiple phenotypic subpopulations, and in one case, we were able to demonstrate direct correspondence between protein‐level phenotypic subsets and DNA mutation‐defined subclones. In summary, CyTOF analysis can resolve both intertumoral and intratumoral heterogeneity among primary samples and reveals that each case of DLBCL is unique and may be comprised of multiple, genetically distinct subclones. © 2019 International Society for Advancement of Cytometry</p>

Abstract:3074 Background: While NGS applications of DNAseq and RNAseq have proven to be powerful tools for genomic discovery, NGS remains underutilized in the clinic. We hypothesized that the clinical translation of NGS would be greatly improved by addressing two critical issues: first, having a single assay for both DNA and RNA sequencing, and second, including all the bioinformatics to enable rapid analysis and reporting of clinically relevant findings within 2 days rather than 2-3 weeks which remains the widespread standard. We developed Duoseq to address these issues. Methods: In most cancer biopsies, RNA constitutes nearly 80% of the total nucleic acid and DNA only 20%. Duoseq incorporates interfering factors that make the ligation of sequencing barcodes to RNA much less efficient than that to DNA to, in effect, invert the proportion of DNA and RNA from the sample. This enables the efficient generation of high quality DNA and RNA libraries. The assay targets over 450 recurrently altered genes in hematologic cancers. We developed secure bioinformatics software that connects to an Illumina sequencer to render four major classes of clinical measurements: mutations, gene expression (cell of origin in DLBCL), Epstein Barr virus (EBV) status and and common translocations (BCL2, BCL6, MYC, IRF4, CCND1). Results: Technicians at three different CLIA labs were trained to perform Duoseq at the respective sites using a local Illumina sequencer. The assay was performed on 111 FFPE lymphoma cases including diffuse large B cell lymphoma, and other B and T cell lymphomas that were characterized using clinical standard assays. The standards included Sanger sequencing or Foundation One testing (mutations), in situ hybridization (EBV), immunohistochemistry or Nanostring (expression) and fluorescent in situ hybridization (translocation). The included Duoseq software provided the bioinformatics results as well as annotations. With a minimum input of 50ng of nucleic acid/sample, Duoseq generated a mean depth of coverage of 438X. We found that, compared to clinical standards, Duoseq performed accurately for mutations (95.9%), EBV status (96.5%), translocations (94.1%) and expression (100%). In addition, Duoseq provided information on clonality, specific translocation partners and identified novel fusions that were not available from standard assays. Conclusions: Duoseq accurately recapitulates the clinical workup of lymphomas replacing the need for single analyte tests such as FISH, while providing a wealth of additional data. The simplicity of the assay and the included bioinformatics software enables its performance at any clinical lab without the need for specialized personnel or computing infrastructure. We anticipate the application of Duoseq as a diagnostic assay, as well as in clinical trial selection and companion diagnostic applications owing to its high accuracy and short turnaround time.

Lymphoma Diagnosis and Classification Using Next Generation Sequencing of 30 CD Markers and Machine Learning As an Alternative to Immunohistochemistry

Abstract:Introduction: Lymphoma diagnosis and classification requires pathologist interpretation of morphology and large numbers of immunohistochemistry (IHC) stains of various CD markers. This process is subjective and requires a significant amount of tissue. In contrast, RNA quantification of the same CD markers used in IHC using next generation sequencing (NGS) requires little tissue and is less influenced by the antigen retrieval process used in IHC. However, IHC staining and microscopic examination allows evaluation of the expression in various subpopulations and makes diagnosis possible. In contrast, when total RNA is evaluated by NGS, distinguishing between subpopulations is lost. Machine learning algorithms are capable of multi-marker normalizing and compensate for the loss of subpopulation analysis. To confirm this, we explored the capability of using RNA quantification of 30 CD markers by NGS from FFPE tissue along with machine learning in the clinical diagnosis and classification of various types of lymphoma. Methods: Formalin-fixed paraffin-embedded (FFPE) tissue from 130 diffuse large B-cell lymphoma (DLBCL), 70 mantle cell lymphoma, 92 T-cell lymphoma, 48 follicular lymphoma, 36 Hodgkin lymphoma, and 52 marginal zone lymphoma samples were used for extracting mRNA. The studied samples were consecutive without selection and included mainly lymph node excisional biopsies or core biopsies. RNA sequencing was performed using a targeted hybrid capture panel that included CD1A, CD2, CD3D, CD3E, CD3G, CD4, CD5, CD7, CD8A, CD8B, CD10, CD14, CD19, CD20, CD22, CD33, CD34, CD38, CD40, CD44, CD47, CD68, CD70, CD74, CD79A, CD79B, CD81, CD138, CD200, and CD274 genes. Salmon v1.4.0 software was used for expression quantification (TPM). Random forest machine learning algorithm was used for predicting diagnosis. Randomly selected two thirds of samples were used for training and one third was used for testing. Results: In some cases, diagnosis can be made by simply inspecting the RNA levels of various CD markers. However, machine learning shows remarkably high sensitivity and specificity in the diagnosis of most lymphoma subclasses. Area under the curve (AUC) was at 1.00 (95% CI: 1.000-1.00) for DLBCL vs. T-cell lymphoma, Hodgkin vs. T-cell, Hodgkin vs. DLBCL, mantle vs. DLBCL, and Follicular lymphoma vs. marginal lymphoma with 100% sensitivity and specificity in the testing set. AUC was at 0.974 (95% CI: 0.920-1.000) for marginal lymphoma vs. mantle cell lymphoma with sensitivity of 88% and specificity of 100%. The AUC was at 0.887 (95% CI: 0.776-0.999) for follicular lymphoma vs. DLBCL with sensitivity of 81.3% and specificity of 83.7%. Conclusions: This data demonstrates that NGS quantification of RNA from 30 CD markers when combined with machine learning is adequate for reliable classification of various types of lymphoma. This approach can provide valuable information to distinguish between difficult diagnoses, and if trained adequately has the potential to expand to more borderline cases. More importantly, this technology can be automated and less susceptible to human errors. RNA quantification using NGS has the potential to replace the need for IHC and can be applied when samples are limited such as in needle aspiration or core biopsies.

Abstract:Abstract Introduction: B or T cell receptor (BCR/TCR) clonal rearrangements have served as important diagnostic markers and minimal residual disease (MRD) tracking markers to guide treatment decisions in lymphoid malignancies. Next generation sequencing (NGS)-based BCR/TCR clonality assessment allows easier sample preparation, higher sensitivity and simpler standardization than flow cytometry and polymerase chain reaction (PCR)-based assays. Here, we developed NGS-based BCR/TCR clonality assays to identify and track disease-associated clonotypes of IGH, IGK, IGL, TCRB and TCRG rearrangements and BCL1/2-IGH translocations in lymphoid malignant cells. Our studies validated the analytical performance of the assays using genomic DNA (gDNA) from cell lines and patient samples diagnosed with acute lymphoblastic leukemia (ALL), multiple myeloma (MM), chronic lymphocytic leukemia (CLL), lymphoma and lymphoid blast phase chronic myeloid leukemia (BP-CML), as well as peripheral blood gDNA and circulating tumor DNA (ctDNA) from lymphoma patients. Methods: Our BCR/TCR clonality assays were based on two rounds of multiplex PCR followed by NGS. In the first-round PCR, the sequences of complementary determining region 3 (CDR3) in rearranged immune receptor genes were amplified by multiplex primers, and sample-specific index and NGS adapters were then added in the second-round PCR. Sequencing was performed on NovaSeq 6000 System and processed with customized bioinformatics pipelines. BCR clonality assays could identify IGH (V H-D H-J H or D H-J H), IGK (V κ-J κ, V κ-Kde and intronRSS-Kde) and IGL (V λ-J λ) rearrangements, as well as BCL1-IGH and BCL2-IGH translocations. TCR clonality assays could detect TCRB (V β-D β-J β) and TCRG (V γ-J γ) rearrangements. To evaluate the performance of our assays, we detected BCR or TCR clonality in gDNA from 4 cell lines and 40 clinical samples with ALL, MM, CLL and lymphoma, as well as paired chronic- and blast-phase samples of CML. Limit-of-detection (LOD) was estimated by clinical samples and cell lines with a background of peripheral blood gDNA from healthy donors. Linearity of detection was established with gDNA of cell lines spiked into normal gDNA to generate across orders of magnitude of clonal frequencies. To ensure consistent performance of the assays, we tested separate reactions and a single mixed reaction for IGH VDJ, IGH DJ, IGK and IGL. In addition, peripheral blood gDNA matching aforementioned lymphoma cases and ctDNA samples were also tested by our assays. Results: Both BCR clonality of B-cell lymphoid malignancies (B-ALL, MM, CLL and lymphoma) and TCR clonality of T-cell lymphoid malignancies (T-ALL and lymphoma) showed above 90% positive detection rate and mostly positive in more than one receptor gene. Chronic and blast phase samples from the same CML patient showed an identical dominant clonotype. The assays had high sensitivity, with LoD defined between 1 to 2 malignant cells in both BCR and TCR clonality assessment. Linearity was observed with clonal frequencies from 1 to 10 -6, which indicated consistence between observed and expected frequencies. The sequencing results with a single adjusted mixed reaction for IGH VDJ, IGH DJ, IGK and IGL were comparable to that with separate reactions, suitable for both diagnosed samples and MRD samples, suggesting the robustness of the assays. Our testing results also showed that peripheral blood gDNA of lymphoma patients carried identical clonotypes found in malignant tissues and ctDNA. Conclusion: We characterized and validated the performance of our NGS -based BCR/TCR clonality assays. We also demonstrated its potential application as a highly sensitive tool for diagnosis and MRD tracking for lymphoid malignancies, including ALL, MM, CLL, lymphoma and even CML at risk of lymphoid blast transformation. Comparison of protein-based cell-of-origin classification to the Lymph2Cx RNA assay in a cohort of diffuse large B-cell lymphomas in Malaysia

Abstract:Aims The cell of origin (COO) based molecular characterisation into germinal centre B-cell-like (GCB) and activated B-cell-like (ABC) subtypes are central to the pathogenesis and clinical course in diffuse large B-cell lymphoma (DLBCL). Globally, clinical laboratories employ pragmatic but less than ideal immunohistochemical (IHC) assay for COO classification. Novel RNA-based platforms using routine pathology samples are emerging as new gold standard and offer unique opportunities for assay standardisation for laboratories across the world. We evaluated our IHC protocols against RNA-based technologies to determine concordance; additionally, we gauged the impact of preanalytical variation on the performance of Lymph2Cx assay. Methods Diagnostic biopsies (n=104) were examined for COO classification, employing automated RNA digital quantification assay (Lymph2Cx). Results were equated against IHC-based COO categorisation. Assay performance was assessed through its impact on overall survival (OS). Results 96 (92%) informative samples were labelled as GCB (38/96; 40%) and non-GCB (58/96; 60%) by IHC evaluation. Lymph2Cx catalogued 36/96 (37%) samples as GCB, 45/96 (47%) as ABC and 15/96 (16%) as unclassified. Lymph2Cx being reference, IHC protocol revealed sensitivity of 81% for ABC and 75% for GCB categorisation and positive predictive value of 81% versus 82%, respectively. Lymph2Cx-based COO classification performed superior to Hans algorithm in predicting OS (log rank test, p=0.017 vs p=0.212). Conclusions Our report show that current IHC-based protocols for COO classification of DLBCL at UKM Malaysia are in line with previously reported results and marked variation in preanalytical factors do not critically impact Lymph2Cx assay quality.
