Abstract:OBJECTIVE:To explore the relationship between sleep quality and glucose level, diabetic complications in elderly type 2 diabetes mellitus.METHODS:A total of 130 hospitalized elderly type 2 diabetes in our hospital were included in the study. Questionnaires and other related clinical data were collected within one week after admission. Patients were divided into two groups: poor-sleeper group and good-sleeper group according to Pittsburgh Sleep Quality Index (PSQI).RESULTS:Sixty percent (78/130) of these patients were poor sleepers. The following parameters differed in the two groups: the duration of diabetes [(7.9 ± 1.8) years vs (7.2 ± 1.5) years, t = 2.318], systolic blood pressure [(148 ± 30) mm Hg (1 mm Hg = 0.133 kPa) vs (138 ± 23) mm Hg, t = 2.037], fasting plasma glucose (FPG) [(10.7 ± 2.2) mmol/L vs (9.8 ± 1.9) mmol/L, t = 2.410], hemoglobin A1c (HbA1c) [(8.6 ± 2.2)% vs (7.8 ± 2.1)%, t = 2.068], high-sensitive C-reactive protein (hs-CRP) [(5.27 ± 2.34) mg/L vs (4.44 ± 1.76) mg/L, t = 2.179], ratio of diabetic complications (61% vs 32%, χ(2) = 4.257), percentage of depression (20% vs 8%, χ(2) = 3.722), score of life quality [(98 ± 19) scores vs (89 ± 13) scores, t = 2.980], and proportion of patients treated with insulin (32% vs 12%, χ(2) = 4.489). All the above parameters were significantly higher in poor-sleeper group than the good-sleeper group (all P value < 0.05). Multiple correlation analysis showed that the factors affecting sleep quality were FPG, HbA1c, duration of diabetes, diabetic complications, depression, life quality and insulin application (r = 0.213, 0.257, 0.223, 0.335, 0.422, 0.3451, 0.231, respectively; all P value < 0.05). By multivariate logistic regression analysis, the followings were found: FPG (β = 1.29, P < 0.05) and PSQI (β = 1.07, P < 0.05) were found to be correlated with HbA1c. With increasing of PSQI, FPG, HbA1c, diabetic complications and life quality were changed significantly (all P value < 0.05). The independent risk factors of diabetic complications were duration of diabetes (OR = 1.32, 95%CI 1.01 - 2.01), HbA1c (OR = 2.01, 95%CI 1.63 - 2.67), hs-CRP (OR = 1.12, 95%CI 1.08 - 1.21) and PSQI (OR = 1.71, 95%CI 1.58 - 2.02).CONCLUSIONS:Elderly type 2 diabetes mellitus are usually poor sleepers. Sleep quality probably affects blood glucose regulation, and is closely correlated with the occurrence of complications. In addition, poor sleep quality results in poor life quality.
A Novel Risk Score for Type 2 Diabetes Containing Sleep Duration: A 7-Year Prospective Cohort Study among Chinese Participants.

Abstract:BACKGROUND:Sleep duration is associated with type 2 diabetes (T2D). However, few T2D risk scores include sleep duration. We aimed to develop T2D scores containing sleep duration and to estimate the additive value of sleep duration. METHODS:We used data from 43,404 adults without T2D in the Beijing Health Management Cohort study. The participants were surveyed approximately every 2 years from 2007/2008 to 2014/2015. Sleep duration was calculated from the self-reported usual time of going to bed and waking up at baseline. Logistic regression was employed to construct the risk scores. Integrated discrimination improvement (IDI) and net reclassification improvement (NRI) were used to estimate the additional value of sleep duration. RESULTS:After a median follow-up of 6.8 years, we recorded 2623 (6.04%) new cases of T2D. Shorter (both 6-8 h/night and <6 h/night) sleep durations were associated with an increased risk of T2D (odds ratio (OR) = 1.43, 95% confidence interval (CI) = 1.30-1.59; OR = 1.98, 95%CI = 1.63-2.41, respectively) compared with a sleep duration of >8 h/night in the adjusted model. Seven variables, including age, education, waist-hip ratio, body mass index, parental history of diabetes, fasting plasma glucose, and sleep duration, were selected to form the comprehensive score; the C-index was 0.74 (95% CI: 0.71-0.76) for the test set. The IDI and NRI values for sleep duration were 0.017 (95% CI: 0.012-0.022) and 0.619 (95% CI: 0.518-0.695), respectively, suggesting good improvement in the predictive ability of the comprehensive nomogram. The decision curves showed that women and individuals older than 50 had more net benefit. CONCLUSIONS:The performance of T2D risk scores developed in the study could be improved by containing the shorter estimated sleep duration, particularly in women and individuals older than 50.
Poor sleep quality is an overlooked risk for geriatric syndromes in older adults with type 2 diabetes mellitus

Abstract:Background: Aging and type 2 diabetes mellitus (DM) are two different conditions that impair sleep quality. The primary objective of this study was to investigate the impact of sleep quality on geriatric syndromes in older diabetic adults. Methods: Comprehensive geriatric assessment was performed on 236 patients. The assessment included the Geriatric Depression Scale (GDS), Beck Anxiety Inventory (BAI), European Quality of Life Questionnaire (EQ-5D), Mini Mental Statement Examination (MMSE), Pittsburgh Sleep Quality Index (PSQI), Mini Nutritional Assessment, and measurement of handgrip strength (HGS). Results: Of the participants, 55% had DM, and 61.4% had poor sleep quality. Poor sleep prevalence was higher in diabetic patients (68.5%) than in non-diabetics (52.8%). Diabetic patients with poor sleep quality had higher frequencies of chronic pain, recurrent urinary tract infections, confirmed sarcopenia, and frailty (p = 0.015, p < 0.001, p = 0.027, and p < 0.001, respectively), a higher number of comorbidities and higher GDS and BAI scores (p = 0.046, p < 0.001 and p < 0.001, respectively), and lower scores of activities of daily living, HGS, and EQ-5D index (p = 0.023, p = 0.004, and p < 0.001, respectively) compared to diabetic patients with good sleep quality. According to the correlation analysis results, PSQI score had a positive correlation with GDS score (r = 0.461, p < 0.001) and a negative correlation with Mini Nutritional Assessment score (r = -0.317, p < 0.001), skeletal muscle mass index (r = -0.283, p = 0.002) and HGS (r = -0.307, p < 0.001) scores in diabetic older adults. Conclusion: Poor sleep quality in older diabetic patients has a negative impact on several geriatric syndromes. Our findings suggest that poor sleep quality is an independent risk factor for depression, malnutrition, and lower muscle mass in older diabetic adults. Improving sleep quality may play a crucial role in preventing and managing geriatric syndromes in this population.

Abstract:Systemic lupus erythematosus (SLE) is still a disease with significant mortality. Although 5 yr after diagnosis 92% of patients are alive, the prognosis falls to 82% survival at 10 yr, 76% at 15 yr and only 68% at 20 yr in Toronto w1x. There has been improvement in survival, with the standardized mortality ratio in patients recruited to the Toronto cohort in 1970–1977 being 10.1 (95% CI 6.5–15.0), compared with 3.3 (95% CI 1.8–5.7) for those recruited between 1986 and 1994 w2x. Data from other centres in the USA and Europe has been similar. Studies published around 1980 found that about 80% of patients survived 5 yr and about 60% of patients survived 10 yr. More recent studies have shown that 5-yr survival is now nearer 90–95% and that 70–85% of patients survive 10 yr w3x. In most studies, patients with renal involvement have had a poorer prognosis than those without renal disease. Nevertheless, survival has shown improvement in those with renal disease presenting to a UK centre between 1976 and 1986 (81% 10-yr survival), compared with those presenting between 1963 and 1975 (56% 10-yr survival) w4x. The commonest cause of death has been infection, both in early and late deaths w1, 3x. Active SLE contributes to about a third of early deaths but less commonly to late deaths. However, deaths related to acute and chronic vascular disease including sudden death are more common in those dying more than 5 yr after diagnosis. However, there is more to prognosis than just death. There is considerable morbidity associated with more prolonged survival after the diagnosis of SLE. Most physicians caring for lupus patients will be familiar with patients in whom active disease has resolved but the patients have suffered from symptoms related to the accumulation of chronic damage w5x. Both active disease and damage can be associated with impaired quality of life and reduced functional ability, although other factors such as the psycho-social background of the patient will affect a patient’s perception of their disease as well w6x. Having improved therapy for active lupus disease, the challenge is now to understand and prevent the long-term complications of this disease, whether they are due to effects of the disease itself, the therapies used, or co-morbid disease (perhaps with associated underlying disease mechanisms or linked genetic predisposition).

Abstract:Objective: The prevalence of poor sleep quality has been greatly escalating over the past years, along with the surging of type 2 Diabetes Mellitus (DM). The aging population is most concerned as sleep quality is notably impaired and influences the diabetic condition. This study aimed to observe the sleep quality and factors related to poor sleep quality in elderly patients with type-2 DM. Materials and Methods: A cross-sectional study of patients with type 2 DM, aged 60 years and above was conducted. The questionnaires included demographic data, the Thai version of the Pittsburgh Sleep Quality Index (T-PSQI), the Thai version of the Diabetic-39, and the Thai Geriatric Depression Scale-15. Factors associated with sleep quality were analyzed using the logistic regression model. Results: Among 385 participants with a mean age of 67 years old was 63.90% female. The prevalence of poor sleep quality was 9.88%. No association was found between sleep quality and glycemic levels. The related factors for poor sleep quality were sex (aOR = 2.57, p = 0.035, 95%CI = 1.06-5.93), body mass index (aOR =1.09, p = 0.028, 95% CI = 1.00-1.20), and diabetic complications. Diabetic retinopathy showed highest odd ratio (aOR = 6.28, p = 0.021, 95% CI = 1.32-29.94). Conclusion: The prevalence of poor sleep quality was low in the current study. We found a strong association between diabetic complications and poor sleep quality. Evaluation of sleep quality may help to enhance overall health and care for diabetic geriatric patients.

Abstract:Abstract Background Diabetes type 2 is a chronic metabolic disease with major physical and health problems that is on the rise. The aim of this study was to evaluate the factors affecting sleep quality in patients with type 2 diabetes in Iran. This cross-sectional study was conducted on 160 patients with type 2 diabetes referred to the diabetes clinic of Kosar Hospital of Semnan city (Iran) in 2018. The sampling was convenient. The data collection tools were four questionnaires including demographic-clinical variables, Pittsburgh Sleep Quality Index (PSQI) questionnaire, Epworth Sleepiness Scale (ESS), and Beck depression inventory II questionnaire. The data was analyzed by STATA 14 and the crude and adjusted coefficient regression ( β ) with a 95% confidence interval (CI) were reported. Results The mean (± S.D) of age was 58.70 (± 12.16) years and 60% were female. The multivariate linear regression model showed depression ( β = 0.11; 95% CI 0.07–0.16) and sex ( β = 1.24; 95% CI 0.17–2.30) are the most important predictors of sleep quality in diabetic patients. However, only underlying disease ( β = 1.73; 95% CI 0.33–3.12) was the most important predictors of daily sleep disorder in diabetic patients. Conclusions Depression and sex were the most important predictors of sleep quality in diabetic patients; therefore, it seems necessary to pay attention to these factors in the design of sleep disorder prevention and control programs.
Subjective sleep disturbances and glycemic control in adults with long-standing type 1 diabetes: The Pittsburgh's Epidemiology of Diabetes Complications study

Abstract:Aims: To date, studies on sleep disturbances in type 1 diabetes (T1D) have been limited to youth and/or small samples. We therefore assessed the prevalence of subjective sleep disturbances and their associations with glycemia and estimated insulin sensitivity in individuals with long-standing T1D. Methods: We conducted a cross-sectional study including 222 participants of the Epidemiology of Diabetes Complications study of childhood-onset T1D attending the 25-year examination (mean age=52years, diabetes duration=43years). The Berlin Questionnaire (risk of obstructive sleep apnea, OSA), the Epworth Sleepiness Scale (daytime sleepiness), and the Pittsburgh Sleep Quality Index (sleep quality, bad dreams presence, and sleep duration) were completed. Associations between sleep disturbances and poor glycemic control (HbA1c⩾7.5%/58mmol/mol), log-transformed HbA1c, and estimated insulin sensitivity (estimated glucose disposal rate, eGDR, squared) were assessed in multivariable regression. Results: The prevalences of high OSA risk, excessive daytime sleepiness, poor sleep quality, and bad dreams were 23%, 13%, 41%, and 26%, respectively, with more women (51%) reporting poor sleep quality than men (30%, p=0.004). Participants under poor glycemic control were twice as likely to report bad dreams (p=0.03), but not independently (p=0.07) of depressive symptomatology. Sleep duration was directly associated with HbA1c among individuals with poor glycemic control, but inversely in their counterparts (interaction p=0.002), and inversely associated with eGDR (p=0.002). Conclusions: These findings suggest important interrelationships between sleep, gender, depressive symptomatology, and glycemic control, which may have important clinical implications. Further research is warranted to examine the mechanism of the interaction between sleep duration and glycemic control.

Computerized tomography is a valuable means of assessing the pre-operative extent of disease and the efficacy of subsequent major surgery. The tissue diagnosis is facilitated by electron microscopy, demonstrating the presence of dence core vesicles and cytoplasmic filaments. Radiotherapy followed by craniofacial resection is the treatment of choice for olfactory neuroblastomas that are locally extensive at presentation.
Sleep Quality and Its Determinants Among Type 2 Diabetes Patients with Comorbid Metabolic Syndrome

Abstract:Purpose: The prevalence of poor sleep quality in patients with diabetes was higher than the general population. This study aimed to explore risk factors for not only poor sleep quality, but also long sleep latency, short sleep duration and low sleep efficiency, in type 2 diabetes patients (T2DM) with comorbid metabolic syndrome (MS). Patients and methods: A total of 281 patients aged 18-75 years were enrolled from Ningbo First Hospital during October 2021 to March 2022. Sleep quality was evaluated by the Pittsburgh Sleep Quality Index (PSQI). Sleep latency, sleep duration and sleep efficiency were obtained by a response to the questionnaire. Descriptive, independent two-sample t-test, Chi-square test and multiple logistic regression were conducted using SPSS Version 28. Results: The prevalence of poor sleep quality in T2DM with comorbid MS patients was 59.10%. The factors significantly associated with poor sleep quality were depression symptoms (OR = 3.10, 95% CI: 1.38 to 6.96, P = 0.006), poor quality of life (OR = 2.49, 95% CI: 1.24 to 4.99, P = 0.010), and age (OR = 1.07, 95% CI: 1.04 to 1.10, P < 0.001). The factor significantly associated with long sleep latency was depression symptoms (OR = 2.19, 95% CI: 1.15 to 4.16, P = 0.017). The factors significantly related to short sleep duration were depression symptoms (OR = 2.56, 95% CI: 1.31 to 5.00, P = 0.006) and age (OR = 1.05, 95% CI: 1.02 to 1.08, P = 0.002). The factor significantly related to short sleep efficiency was age (OR = 1.03, 95% CI: 1.01 to 1.06, P = 0.019). Conclusion: This study found that depression symptoms, together with poor quality of life, and increasing age were associated with poor sleep quality. Symptoms of depression were related to long sleep latency and short sleep duration. The increasing age was associated with short sleep duration and low sleep efficiency.

Abstract:Objective: To ascertain glycemic control and sleep quality in patients with Type 2 Diabetes Mellitus and compare them with age-matched non-diabetic control subjects.Methodology: In this cross-sectional study, plasma glycated hemoglobin (HbA1c) levels and sleep patterns in diabetic subjects were compared with non-diabetic control subjects. Ethical approval was granted by the institutional ethical review board of Central Park Medical College vide number CPMC/IRB/1728. After taking written informed consent, a total of fifty participants between the ages of 40-60 years were recruited by convenience sampling from the medical outpatient clinic at Central Park Teaching Hospital in 2018. Based on the fasting blood glucose levels, participants were categorized as Group I (diabetic subjects) and Group II (non-diabetic control subjects). Sleep quality was assessed using two validated scales, namely Insomnia Severity Index (ISI) and Epworth Sleepiness Scale (ESS). HbA1c levels were evaluated using standardized blood assays. Independent samples t-test was employed to determine mean differences between the two groups. A p-value of ? 0.05 was considered significant.Results: Mean age of the study participants (n=50) was 49.28±2.34 years. The diabetic subjects had significantly higher HbA1c levels (10.15±2.50 vs. 6.26±1.04; p-value 0.001) and ISI scores (12.64±7.91 vs. 4.68±4.88; p-value 0.000) as compared to non-diabetic control subjects, indicating poorer glycemic control and greater degree of insomnia.Conclusion: Poorly controlled diabetes mellitus as reflected by higher HbA1c levels is associated with increased insomnia.

Abstract:Objectives Type 2 diabetes mellitus (T2DM) is a serious public health issue. Compared with the general population, patients with T2DM have a higher risk of poor sleep quality, which could ultimately result in poor prognosis. Therefore, this study aimed to evaluate sleep quality and its associated factors among patients with T2DM in Hunan, China. Design This was a cross-sectional study. Setting A tertiary hospital in Hunan, China. Participants Patients with T2DM hospitalised at the Endocrinology Department were consecutively enrolled between March 2021 and December 2022. Sociodemographic characteristics, lifestyle factors and T2DM-related information were collected retrospectively. Primary and secondary outcome measures Sleep quality was evaluated using the Pittsburgh Sleep Quality Index, with a cut-off value of >7 suggesting poor sleep quality. Multivariate logistic regression analysis was used to determine factors associated with poor sleep quality. Results Of the 1039 participants included, 1001 provided complete data. The mean age of the study sample was 60.24±10.09 years, and 40.5% (95% CI 37.5% to 43.5%) of patients had poor sleep quality. Multivariate logistic regression analysis showed that female sex (adjusted OR (aOR) 1.70, 95% CI 1.25 to 2.29), unmarried status (aOR 1.72, 95% CI 1.05 to 2.83), diabetic retinopathy (aOR 1.38, 95% CI 1.04 to 1.83), diabetic foot (aOR 1.80, 95% CI 1.11 to 2.93) and a per capita monthly household income of >5000 RMB (aOR 0.66, 95% CI 0.47 to 0.93) were associated with poor sleep quality. Conclusions Nearly two-fifths of patients with T2DM reported poor sleep quality in Hunan, China. Sex, marital status, diabetic retinopathy, diabetic foot and household income were independently associated with sleep quality among patients with T2DM in Hunan, China.

Abstract:Objective A decline in sleep quality and regularity has been reported in patients with type 1 diabetes mellitus (T1D) in many studies. However, research on medical-based sleep disorders in patients with T1D is limited. Diagnosing sleep disorders is crucial, as it negatively impacts academic performance, cardiovascular health, and cognitive functions among children as well as essential skills for effective diabetes management. Our objective was to assess sleep disturbances in patients diagnosed with T1D and explore whether these patients experience significantly more sleep disturbances compared to their healthy peers. Methods This study, designed as a cross-sectional case-control investigation, involved a cohort of 250 participants (144 T1D, 106 control cases) aged 6–15 years. The Sleep Disturbance Scale for Children (SDCS) scores of the T1D group were compared with those of the control group. Furthermore, the study explored the correlation between clinical/biochemical parameters and SDCS scores within the T1D group. Results The mean age of individuals in the T1D group was 10.27 ± 3.25 years, while the control group had a comparable mean age of 10.48 ± 3.5 years (P = 0.303). Within the T1D group, the median duration of diabetes was 5 (1-15) years, and the median glycosylated hemoglobin A1 c (HbA1 c ) level for the past one year was 8.4 %. Although there was no significant difference in total SDSC scores between the T1D and control groups, both groups exhibited average scores that remained close to the threshold indicative of sleep disturbances (>39). Notably, individuals with total SDSC scores surpassing 39 were identified at rates of 48.6 % in the T1D group and 47.6 % in the control group, respectively. Furthermore, disorders of arousal nightmares (DA) were more prevalent in T1D patients compared to their healthy peers (P = 0.049). Additionally, HbA1 c showed a positive correlation with scores for disorders of excessive somnolence (DOES) and total scores (P < 0.001, R = 0.368; P = 0.003, R = 0.243). Conclusion Our study found that the prevalence of sleep disturbances among children and adolescents with T1D was not significantly higher than that observed in their healthy peers. Nevertheless, it is crucial to note that a notable portion, 48.6 % of T1D cases and 47.6 % of healthy cases, displayed sleep disturbances based on SDSC scores. To optimize diabetes management and proactively address potential challenges, incorporating routine screening for sleep disturbances in the monitoring of T1D patients can yield valuable benefits.

Abstract:Introduction: Patients with type 2 diabetes (T2D) may have inadequate (insufficient or excessive) sleep which could accelerate CVD onset, but existing evidence from prospective studies is limited. Hypothesis: We hypothesized that inadequate sleep duration would be associated with higher risks of CVD incidence and mortality among patients with T2D. Methods: This prospective study included 19,345 participants with T2D who were free of CVD and cancer at baseline from the UK Biobank. Sleep duration was obtained at baseline using questionnaires. Cox proportional hazards regression models were used to examine the association of sleep duration with CVD events. Results: During an average follow-up of 11.0-12.0 years, we documented 2,219 incident CVD cases (1,555 CADs and 673 strokes) and 652 CVD deaths. Compared with those sleeping for 7-8 h/day, the multivariable-adjusted hazard ratios (95% confidence intervals) of sleeping for ≤5 and ≥10 h/day were 1.39 (1.19-1.61) and 1.38 (1.13-1.68) for incident CVD, 1.26 (1.04-1.51) and 1.23 (0.95-1.57) for CAD, 1.68 (1.30-2.16) and 1.75 (1.27- 2.42) for stroke, and 1.36 (1.02-1.80) and 1.94 (1.42-2.63) for CVD mortality. These results largely remained similar in sensitivity analyses that aimed to address potential reverse causation and in the joint analysis of sleep duration and metabolic control or diabetes severity status. Conclusions: Our findings showed inadequate sleep duration was independently associated with increased risks of CVD onset and death among patients with T2D.
Prevalence of Sleep Disorders Among Patients With Type 2 Diabetes Mellitus in Makkah City: A Cross-Sectional Study

Abstract:BACKGROUND: Due to rising rates of morbidity and mortality associated with type 2 diabetes mellitus (T2DM), Saudi Arabia is ranked second in the Middle East and seventh overall among nations with the greatest incidence of diabetes mellitus (DM). Significant sleep abnormalities have been linked to difficulties in managing blood sugar levels, suggesting a link between sleep disorders and diabetes. This study aimed to find out how common sleep disturbances were among patients with T2DM in Makkah, Saudi Arabia.METHODS: This descriptive cross-sectional study was conducted between June and August 2022 in Makkah City, Saudi Arabia. Patients with T2DM who visited primary healthcare facilities in Makkah during the study's duration were included in the study. To evaluate sleep quality, the Pittsburgh Sleep Quality Index in Arabic was employed. Patients who met the inclusion criteria were given an interview questionnaire to fill out.RESULTS: In total, 355 patients with T2DM were enrolled in this study. The patients' median age was 49.24 years. Other than DM, a majority of them (58.9%) had chronic illnesses, with hypertension (64.5%) and cardiovascular disease (65.5%) as the most prevalent comorbidities. Only 22% of the patients had controlled diabetes. Of the individuals who had sleep disorders, 63.7% stated having poor sleep quality.CONCLUSION: Sleep problems are a common occurrence in patients with T2DM. Additionally, compared to people with other chronic disorders, people with endocrine diseases had poorer sleep quality. Hence, the duration of diabetes has an impact on sleep quality.

Abstract:Objective: Sleep disturbances are common in Type 2 diabetes (T2DM) patients and this exacerbates the severity of disease and results in poor quality of life. Brain-derived neurotrophic factor (BDNF) has been reported to mediate the association between T2DM and poor sleep health. We investigated the burden of self-reported poor sleep quality and duration in T2DM and their association with serum BDNF levels. Methods: In a case-control design, the Pittsburgh Sleep Quality Instrument (PSQI) was used to assess self-reported sleep quality and duration in 100 T2DM patients and 80 nondiabetic controls. Sociodemographic data and medical history were collected from case notes and/or using a structured questionnaire. 5 ml of fasting venous blood samples were collected to measure plasma lipid profile and serum BDNF levels. Results: T2DM patients had low levels of BDNF, poor sleep quality (61.9% vs 27.5%, p<0.001), and shorter sleep duration (6.1±2.2 vs 6.9±1.1 hours, p=0.003). T2DM status was associated with doubling the odds of poor sleep quality [OR (95% CI) = 2.06 (1.07 – 6.43), p=0.039] and 1.6 times the odds of short sleep duration [1.63 (1.03 – 3.79), p=0.028]. There was no association between serum BDNF levels and sleep status in multivariable logistic regression analysis. However, there was a negative biological interaction between T2DM and BDNF levels on poor sleep quality, resulting in 0.28 relative excess risk due to the interaction and a 12% attributable proportion due to the interaction. Conclusion: T2DM patients in our study population had a high burden of self-reported poor quality of sleep and shorter sleep duration compared to the nondiabetic controls. T2DM interacts negatively with serum BDNF levels to affect sleep quality.

Abstract:Objective: We explored the relationship between the Sleep Hygiene Practices Scale (SHPS) and sleep quality and sleep-related impairment in Black and Latinx adults with type 2 diabetes (T2DM). Methods: Forty Black and Latinx adults with T2DM participated. Self-reported measures include the Pittsburg Sleep Quality Index (PSQI), Patient Reported Outcomes Measurement Information System (PROMIS) Sleep Disturbance (SD) and Sleep-Related Impairment (SRI) measures, and SHPS (domains include sleep schedule and timing, arousal-related behaviors, poor eating/drinking habits prior to sleep, and poor sleep environment). Results: SHPS Cronbach's alpha coefficients were 0.58 (schedule), 0.78 (arousal), 0.29 (eating), 0.81 (environment) and 0.88 (overall for four domains). SHPS scores correlated with PSQI (Pearson correlation r = 0.67, 95% CI [0.44, 0.81], PROMIS-SD (r = 0.61 [0.36-0.77]), and PROMIS-SRI (r = 0.43, [0.13-0.65]). There remained a significant relationship between sleep hygiene and both sleep quality and sleep-related impairment adjusting for hemoglobin A1c, age, and body mass index in regression models. Conclusions: We observed moderate correlations between sleep quality and sleep-related impairment with sleep hygiene using the SHPS in Black and Latinx adults with T2DM.
Associations between sleep characteristics and glycemic variability in youth with type 1 diabetes

Abstract:Objective: This study aimed to determine sleep characteristics and their associations with glycemic variability in youth with type 1 diabetes (T1D). Material and methods: This cross-sectional study conducted at two pediatric diabetes centers in Istanbul, Turkey, included 84 children with T1D (mean age 10.5 years). Sleep characteristics and glycemic variability were determined by actigraphy, DSM-5 Level 2-Sleep Disturbance Scale Short Form and continuous glucose monitoring. Circadian preference was evaluated by the Children's Chronotype Questionnaire. Sleep disturbances were assessed by the. The sleep quality was determined by actigraphy-derived sleep measures. Results: Eighty-eight percent of participants had insufficient age-appropriate total sleep time (TST) (<9 h for 6-13-year-olds and <8 h for 14-17-year-olds). Chronotype was classified as intermediate in 50%, evening in 45.2%, and morning in 4.8%. A higher chronotype score indicating a stronger eveningness preference was associated with more time spent in hypoglycemia (β = 0.433, p = 0.002). On nights when participants had lower sleep efficiency and longer sleep onset latency, they had significantly higher overnight glycemic variability (β = -0.343, p = 0.016, β = 0.129, p = 0.017, respectively). Prolonged nocturnal wake duration was significantly associated with more time spent in daytime hypoglycemia (β = 0.037, p = 0.046) and higher overnight glycemic variability (J index, β = 0.300, p = 0.015). The associations between TST and glycemic variability indices were not significant. Conclusions: Sleep quality rather than TST was significantly associated with glycemic variability in children with T1D. Eveningness preference might contribute to an increased risk of hypoglycemia. Addressing sleep patterns and chronotypes can be crucial in management plans for youth with T1D.

Abstract:The aim of this study was to assess the rates of depression, anxiety, and stress and quality of sleeping among COVID-19 patients with and without type 2 diabetes mellitus (T2DM). A case and control design has been employed, involving patients affected by COVID-19 infection (884 with T2DM vs. 884 controls without T2DM) and hospitalized in Istanbul (Turkey) from January to December 2021. A multivariate stepwise regression approach was used to test the associations between sociodemographic, metabolic, serum markers, mental health scores, and T2DM/COVID-19 patients' clinical presentation. A statistically significant difference between T2DM and non-T2DM was found with respect to age, gender, BMI (body mass index), smoking, physical exercise, and physical comorbidities as well as levels of depression, anxiety, stress, and sleeping disorders (0.0003 ≤ all p = 0.025). With regard to serum biomarkers, vitamin D and ferritin were identified as useful parameters of reduction of glycated hemoglobin as well as COVID-19 infection among T2DM patients. This study detected that 25% of patients with COVID-19 and T2DM experienced mental distress, with sleeping disturbances and lifestyle changes markedly impacting their clinical outcome alongside metabolic and serum parameters.

Abstract:Introduction: In diabetes mellitus (DM) patients, obtaining a good night's sleep is crucial for maintaining body caloric intake, controlling insulin levels, and reducing the likelihood of engaging in unhealthy behavior. Patients with poor sleep quality may experience impaired glycemia, playing a significant role in the development of chronic complications. This study aims to explore the effects of DM complications on sleep quality among Saudi Arabian patients. Patient and methods: This is a cross-sectional study conducted among patients with diabetes. A self-administered, validated questionnaire translated into Arabic was distributed among diabetic patients using an online survey. The questionnaire includes sociodemographic characteristics (i.e. age, gender, marital status, etc.), the medical history of the patients, and a questionnaire about sleep quality. Results: Out of 4171 patients involved, 52.7% were females and 27.7% were aged between 40 and 60 years old. The prevalence of patients with perceived poor sleep quality was 24.1%. Significant predictors of poor sleep quality were the gender female, having social stressors, comorbid sleep disorders, associated comorbidities, increasing HbA1c levels, being overweight/obese, and diabetes complications. Furthermore, sleep disturbance, taking sleep medications, daytime sleepiness, and having bad dreams during sleep were also identified as prognostic factors for poor sleep quality. Conclusion: The subjective poor sleep quality of patients with diabetes was 24.1%. Poor sleep quality was significantly demonstrated by females who were having social stressors, comorbid sleep disorders, comorbidities, uncontrolled HbA1c levels, elevated BMI levels, and complications of diabetes. However, regular physical activity and adequate sleep were estimated to be the protective factors against poor sleep quality. Further research is needed to establish the effect of sleep quality among patients with DM.
