Correlates of Insulin Selection as a First-Line Pharmacological Treatment for Gestational Diabetes

Abstract:Objective: The aim of this study was to investigate prenatal factors associated with insulin prescription as a first-line pharmacotherapy for gestational diabetes mellitus (GDM; compared with oral antidiabetic medication) after failed medical nutrition therapy. Study design: This is a retrospective cohort study of 437 women with a singleton pregnancy and diagnosis of A2GDM (GDM requiring pharmacotherapy), delivering in a university hospital between 2015 and 2019. Maternal sociodemographic and clinical characteristics, as well as GDM-related factors, including provider type that manages GDM, were compared between women who received insulin versus oral antidiabetic medication (metformin or glyburide) as the first-line pharmacotherapy using univariable and multivariable analyses. Results: In univariable analysis, maternal age, race and ethnicity, insurance, chronic hypertension, gestational age at GDM diagnosis, glucose level after 50-g glucose loading test, and provider type were associated with insulin prescription. In multivariable analysis, after adjusting for sociodemographic and clinical maternal factors, GDM characteristics and provider type, Hispanic ethnicity (0.26, 95% confidence interval [CI]: 0.09-0.73), and lack of insurance (0.34, 95% CI: 0.13-0.89) remained associated with lower odds of insulin prescription, whereas endocrinology management of GDM (compared with obstetrics and gynecology [OBGYN]) (8.07, 95% CI: 3.27-19.90) remained associated with higher odds of insulin prescription. Conclusion: Women of Hispanic ethnicity and women with no insurance were less likely to receive insulin and more likely to receive oral antidiabetic medication for GDM pharmacotherapy, while management by endocrinology was associated with higher odds of insulin prescription.This finding deserves more investigation to understand if differences are due to patient choice or a health disparity in the choice of pharmacologic agent for A2GDM. Abstract:Since the F response is the result of antidromic activation of limited number of alpha motor neurons, the motor neurons excitability level can be investigated and established by analyzing parameter values of F response. The excitability of alpha motor neurons is the result of balance between excitatory and inhibitory influences of mainly supraspinal origin. The aim of this study was to perform the analysis of basic parameters of the F response in the lesions of pyramidal, extrapyramidal and cerebellar system, and to determine the eventual predictive value of the F response in lesion outcome. The investigations were carried out in 65 patients and 30 neurologically healthy subjects in control group. The patient group consisted of 38 patients with pyramidal lesions due to ischemic stroke, 15 patients with Parkinson's disease and 12 patients with vascular cerebellar lesions. The F response investigations were repeated monthly during 12 months in 12 patients with ischemic stroke. The results showed significant increase in F amplitude and frequency. These changes were in positive correlation with the increase of spastic muscular tone. In patients with cerebellar lesions decreased amplitude and frequency of F response were in positive correlation with the reduction of muscle tone. It was concluded that the measurement of the F response parameters might be reliable marker of the alpha motor neurons excitability and might have predictive value in the early phase of ischemic stroke.
Pharmacological therapy in gestational diabetes - a comparison between insulin and oral therapy

Abstract:Background: The appropriate medical treatment for gestational diabetes mellitus (GDM) is controversial and recommendations vary between different organizations. Objective: To compare the safety and efficacy of glyburide and insulin as treatments for GDM. Methods: Retrospective analysis of all pregnant women diagnosed with GDM and treated with either glyburide or insulin. Demographic features, clinical characteristics, maternal and neonatal outcomes were compared according to type of pharmacological treatment. Results: Included in the study were 323 women, of whom 269 (83.28%) were treated with glyburide and 54 (16.72%) with insulin. There were no significant differences between the groups, apart from a higher one-hour oral glucose tolerance test (OGTT) value (191.80 mg/dl in the glyburide group, 204.33 in the insulin group, p = .01). Optimal glucose control was achieved in 130 women in the glyburide group (48.32%) and 15 in the insulin group (27.77%), p = .007. This difference remained significant after adjustment for age, BMI, and fasting glucose during OGTT (aOR = 2.22). Mean gestational weight gain was lower in the glyburide group vs. insulin group (10.01 vs. 11.99 kg, p = .048). Apart from higher maternal hypoglycemia rate (12.64% in glyburide group vs. 1.85% in insulin group, p = .016), there were no other differences in maternal and neonatal outcomes between the groups. Glyburide failure rate was 13.38%, and associated with higher fasting OGTT value (100.70 mg/dl in glyburide failure group vs. 94.67 mg/dl in the glyburide treatment until delivery group, p = .041). Conclusions: Glyburide is at least as safe and effective as insulin except for higher rates of maternal hypoglycemia. Considering its advantages compared to insulin (ease of use and storage, increased patient responsiveness, and lower cost), it may be considered as first line treatment in GDM, especially when fasting OGTT value is not high.

Abstract:Gestational diabetes mellitus (GDM) is associated with an increased risk of adverse pregnancy outcomes in the setting of poor glycemic control. The initial management for GDM includes intensive lifestyle modification, which often requires behavioral and nutritional changes to optimize glycemic control. Pharmacotherapy for GDM is initiated when glycemic targets are not met. The rapid-acting bolus analogues aspart and lispro achieve postprandial targets with less hypoglycemia compared to regular insulin, with similar fetal outcomes. The long-acting insulin analogues glargine and detemir appear safe with similar maternal/fetal outcomes compared to NPH. While insulin has been the mainstay therapy for women with GDM to improve glycemic control when lifestyle modifications are insufficient, certain oral antihyperglycemic drugs (OADs) can be considered as alternative treatment options for GDM but continue to be controversial for use as first-line treatment options compared to insulin by many professional bodies. Metformin has good efficacy and short-term safety data but it freely crosses the placenta and long-term safety data are lacking. Glyburide has good efficacy and short-term data but it also crosses the placenta and may be associated with increased rates of large-for-gestational-age (LGA) infants and neonatal hypoglycaemia when compared with insulin. This review aims to give an overview of the pharmacological treatment for women with GDM including some of the known safety profiles of current therapeutic options.

Abstract:In rounding off this four-year series of studies of the Clouds of Magellan, we present in this communication some tabulations that will assist future students of these nearest of external galaxies in their work on the classical cepheids, especially in the use of our data on periods and magnitude. In the following pages we also report some new results on variable-star populations. Much remains to be done with the classical cepheids of the Clouds before a full knowledge is obtained of the four most outstanding characteristics of these variables, namely, (1) their high frequency among the giant and supergiant stars of the Clouds; (2) the great abundance in the Small Cloud, especially outside its central area, of periods less than two days in length, which are extremely infrequent in the Large Cloud and apparently also relatively rare in our galactic system; (3) the correlation, at least in the Small Cloud, of average period length with distance from the high-density central regions of the Clouds; and (4) the dependence of the shape of the light curve on period length. There will be, of course, extensive work in the future on the other supergiant variables of the Clouds, many of which we have found and studied; and even more time should be devoted to investigating the nonvariable stars whose spectra and colors await exploitation.
Use of metformin and insulin among pregnant women with gestation diabetes in the United Kingdom: A population-based cohort study

Abstract:Aims: The contemporary prescription patterns of antidiabetic drugs following guideline changes recommending metformin as first-line gestational diabetes (GDM) pharmacotherapy is underexplored. We aimed to examined use of metformin and insulin during pregnancy among women with GDM over 20 years in the United Kingdom. Methods: We conducted a population-based cohort study using linked data from the Clinical Practice Research Datalink, its pregnancy register and Hospital Episode Statistics from 1998 to 2017. We included pregnancies of women without prior diabetes history who received GDM diagnosis or initiated an antidiabetic drug after 20 weeks gestation. Patient-level and practice-level characteristics were compared between metformin initiators and insulin initiators. We described trends of initiating metformin as first-line treatment and described time to initiation of rescue insulin overall, and by body mass index among metformin initiators. Results: Our cohort included 5633 pregnancies from 5393 women with GDM, of whom 38.9% initiated pharmacotherapy (41% insulin, 59% metformin). Metformin prescriptions (as opposed to insulin) increased substantially, from <5% of pregnancies before 2007 to 42.5% in 2008. Over 85% of pregnancies that were prescribed pharmacotherapy were prescribed metformin as first-line treatment in 2015. Among metformin initiators, 16% initiated rescue insulin, typically occurring within 40 days of metformin initiation. Choice of GDM pharmacotherapy varied by characteristics, including smoking, obesity, race/ethnicity and general practice regions. Conclusions: Metformin was the most prescribed medication for GDM, with large increases over the past 2 decades. The increasing use of oral-antidiabetic drugs during pregnancy, consistent with other regions, highlights the need for future studies examining effectiveness and safety of antidiabetic drug use during pregnancy.
Sparse pallidal connections shape synchrony in a network model of the basal ganglia

Abstract:Neural synchrony in the basal ganglia, especially in the beta frequency band (13–30 Hz), is a hallmark of Parkinson's disease and considered as antikinetic. In contrast, the healthy basal ganglia show low levels of synchrony. It is currently unknown where synchrony and oscillations arise in the parkinsonian brain and how they are transmitted through the basal ganglia, as well as what makes them dependent on dopamine. The external part of the globus pallidus has recently been identified as a hub nucleus in the basal ganglia, possessing intrinsic inhibitory connections and possibly also gap junctions. In this study, we show that in a conductance‐based network model of the basal ganglia, the combination of sparse, high‐conductance inhibitory synapses and sparse, low‐conductance gap junctions in the external part of the globus pallidus could effectively desynchronize the whole network. However, when gap junction coupling became strong enough, the effect was impeded and activity synchronized. In particular, sustained periods of beta coherence occurred between some neuron pairs. As gap junctions can change their conductance with the dopamine level, we suggest pallidal gap junction coupling as a mechanism contributing to the development of beta synchrony in the parkinsonian basal ganglia.

Abstract:Background: The rate of gestational diabetes (GDM) has increased over the past decade, now effecting up to 14% of pregnancies worldwide. Studies show that these woman benefit from diabetes education, nutrition education and a maternal-fetal medicine (MFM) consultation. However, accommodating this increasing number of women, especially in the middle of a global pandemic, can be challenging. Purpose: First, to develop a curriculum for women with GDM to empower them to optimally manage their diabetes in pregnancy. Second, to evaluate whether this curriculum could be used in a virtual shared medical appointment (SMA) to accommodate more patients while protecting their health during the COVID-19 pandemic. Results: A virtual SMA was created in the electronic medical record using a zoom platform. Up to ten women are seen during this 2-2.5 hour visit. From 1/1/2022 through 12/31/22, 159 women participated in the program. On average, women attended the SMA within 12.9 days of the consult request. All women received a copy of the curriculum via email prior to the visit. During the visit, patients received their MFM consult, diabetes education, and nutrition counseling in a group format. The visit included a one-on-one breakout room with a MFM provider to discuss personal history, glucose levels and use of medication if necessary. Women were billed through insurance for the MFM consultation as well as group nutrition counseling. Of the 159 participants, 139 have delivered. Eighty-nine (64%) women had a vaginal delivery and 50 (36%) had a cesarean delivery. Conclusion: A virtual SMA is an efficient and effective format for educating women with GDM during the COVID-19 pandemic and beyond. Access to these resources was faster for women in the SMA versus those who received individual MFM consultations, nutrition counseling and diabetes education. The rate of cesarean delivery was lower than the rate reported in the literature for women with GDM. Patient reported outcomes regarding this format have also been favorable. Abstract:Objective: To determine the impact of prior gestational diabetes mellitus (GDM) on perinatal outcomes in a subsequent GDM pregnancy. Methods: This retrospective cohort study included 544 multiparous patients with two consecutive pregnancies between 2012-2019, where the second (index) pregnancy was affected by GDM. The primary exposure was prior GDM diagnosis, categorized into medical and dietary management. The primary outcome was a composite including need for pharmacotherapy, large-for-gestational age, or neonatal hypoglycemia. Adjusted odds ratios (aOR) were calculated using multivariable logistic regression controlling for maternal age, pre-pregnancy body mass index, and gestational age at GDM diagnosis in the index pregnancy. Results: Of the 544 patients, 164 (30.1%) had prior GDM. Prior GDM significantly increased the likelihood of composite outcome compared to no prior GDM (74.4% vs. 57.4%; P < 0.001). After adjusting for confounders, prior GDM remained significantly associated with the composite outcome (aOR 2.03, 95% confidence interval [CI] 1.31-3.15). Stratifying by prior GDM treatment modality, a significant association was found for prior pharmacotherapy-controlled GDM (aOR 3.29, 95% CI 1.64-6.59), but not for prior diet-controlled GDM (aOR = 1.54, 95% CI 0.92-2.60). Conclusion: A history of pharmacotherapy-controlled GDM in a previous pregnancy increases odds of adverse perinatal outcomes in a subsequent GDM pregnancy.
GAP (gestational diabetes and pharmacotherapy) - study protocol for a randomized controlled, two-arm, single-site trial

Abstract:Background: Gestational diabetes (GDM) complicates 10% of pregnancies in the US. First-line treatment is medical nutrition therapy (MNT) and exercise. Second line is pharmacotherapy. The definition of what constitutes an unsuccessful trial of MNT and exercise has not been established. Tight glycemic control has been demonstrated to reduce GDM-related neonatal and maternal clinical complications. However, it could also increase rates of small-for-gestational age and carry negative effects on patient-reported outcomes such as anxiety and stress. We will study the effect of earlier and stricter pharmacotherapy in GDM on clinical and patient-reported outcomes. Methods: GDM and pharmacotherapy (GAP) study is a two-arm parallel, pragmatic randomized controlled trial, where 416 participants with GDM are randomized 1:1 to: 1) Intervention group - insulin initiation at 20% elevated glucose values on a weekly glucose log following MNT and exercise trial and insulin titration to keep elevated glucose values <20%; or 2) Active control group - insulin initiation at 40% elevated glucose values on a weekly log following MNT and exercise and insulin titration to keep elevated glucose values <40%. The primary outcome is a composite neonatal outcome of large-for-gestational-age, macrosomia, birth trauma, preterm birth, hypoglycemia, and hyperbilirubinemia. Secondary outcomes include preeclampsia, cesarean birth, small-for-gestational-age, maternal hypoglycemia, and patient-reported outcomes of anxiety, depression, perceived stress, and diabetes self-efficacy. Conclusions: The GAP study will investigate the optimal glycemic threshold for pharmacotherapy addition to MNT and exercise in GDM. The GAP study will promote standardization in GDM management and will have direct relevance for clinical practice.

Abstract:OBJECTIVE In most gestational diabetes mellitus (GDM) studies, cohorts have included women combined into study populations without regard to whether hyperglycemia was present earlier in pregnancy. In this study we sought to compare perinatal outcomes between groups: women with early GDM (EGDM group: diagnosis before 20 weeks but no treatment until 24–28 weeks if GDM still present), with late GDM (LGDM group: present only at 24–28 weeks), and with normoglycemia at 24–28 weeks (control subjects). RESEARCH DESIGN AND METHODS This is a secondary analysis of a randomized controlled treatment trial where we studied, among women with risk factors, early (<20 weeks’ gestation) GDM defined according to World Health Organization 2013 criteria. Those receiving early treatment for GDM treatment were excluded. GDM was treated if present at 24–28 weeks. The primary outcome was a composite of birth before 37 weeks’ gestation, birth weight ≥4,500 g, birth trauma, neonatal respiratory distress, phototherapy, stillbirth/neonatal death, and shoulder dystocia. Comparisons included adjustment for age, ethnicity, BMI, site, smoking, primigravity, and education. RESULTS Women with EGDM (n = 254) and LGDM (n = 467) had shorter pregnancy duration than control subjects (n = 2,339). BMI was lowest with LGDM. The composite was increased with EGDM (odds ratio [OR] 1.59, 95% CI 1.18–2.12)) but not LGDM (OR 1.19, 95% CI 0.94–1.50). Induction of labor was higher in both GDM groups. In comparisons with control subjects there were higher birth centile, higher preterm birth rate, and higher rate of neonatal jaundice for the EGDM group (but not the LGDM group). The greatest need for insulin and/or metformin was with EGDM. CONCLUSIONS Adverse perinatal outcomes were increased with EGDM despite treatment from 24–28 weeks’ gestation, suggesting the need to initiate treatment early, and more aggressively, to reduce the effects of exposure to the more severe maternal hyperglycemia from early pregnancy.

Intrapartum and Early Postpartum Glycemic Profiles in Women with Gestational Diabetes Mellitus: an Observational Study

Abstract:Abstract Background: Data on the glycemic profile of pregnant women with gestational diabetes mellitus (GDM) during the perinatal period are sparse. This study described the intrapartum and early postpartum glucose profiles among pregnant women with GDM, and analyzed factors potentially affecting glycemic parameters during the period. Methods: This was a prospective observational study conducted from March 2020 to November 2021. Pregnant women with GDM receiving lifestyle interventions alone during pregnancy and matched women with non-diabetic pregnancies (NDPs) were enrolled from among patients admitted to the obstetrics department for childbirth. Glucose monitoring was performed via a flash glucose monitoring (FGM) system on admission, and glucose readings during labor and early postpartum were analyzed. The clinical characteristics and FGM-based parameters of participants in the two groups were compared. Results: A total of 124 participants (mean age: 29.5 ± 3.5 years, 92 [74.2%] primipara) were included in the final analysis. A total of 17,571 glucose readings were retrieved. There were no significant differences in clinical characteristics between the GDM (n = 60) and NDP (n = 64) groups. The average glucose level was 92.2 mg/dL, and the level was higher in the GDM group (95.5 ± 12.1 mg/dL vs. 89.1 ± 13.4 mg/dL, P = 0.008) during the intrapartum and early postpartum periods. The data were split into the intrapartum period (from the start of labor to delivery of the placenta) and the early postpartum period (within 24 h after placental delivery) for analysis. During intrapartum, women with GDM exhibited glycemic profiles and fluctuations similar to those in the NDP group. However, women with GDM had higher postpartum glucose levels (97.7 ± 13.4 mg/dL vs. 90.8 ± 15.3 mg/dL, P = 0.009), a longer time spent >140 mg/dL (8.7 ± 9.3% vs. 5.9 ± 10.3%, P = 0.011), and greater glycemic fluctuations than those with NDP. Postpartum hyperglycemia in GDM might be associated with high parity and postprandial glucose abnormalities in GDM screening tests. Conclusion: Compared to those with normoglycemia, pregnant women with GDM receiving lifestyle interventions alone had similar intrapartum glucose profiles but higher early postpartum glucose levels and greater glucose variability, providing evidence for modification of the current perinatal glucose monitoring strategy for GDM. Abstract:Gestational diabetes (GDM) is defined as any degree of glucose intolerance with onset during pregnancy and is associated with increased feto-maternal morbidity as well as long-term complications in mothers and the offspring. Women detected to have diabetes early in pregnancy receive the diagnosis of overt, non-gestational, diabetes (glucose: fasting ≥ 126 mg/dl, spontaneous ≥ 200 mg/dl or HbA1c ≥ 6.5% before 20 weeks of gestation). GDM is diagnosed by an oral glucose tolerance test (oGTT) or increased fasting glucose (≥ 92 mg/dl). Screening for undiagnosed type 2 diabetes at the first prenatal visit is recommended in women at increased risk (history of GDM/pre-diabetes; malformation, stillbirth, successive abortions or birth weight > 4500 g previously; obesity, metabolic syndrome, age > 35 years, vascular disease; clinical symptoms of diabetes (e.g. glucosuria) or ethnic origin with increased risk for GDM/T2DM (Arab, South- and Southeast Asian, Latin American)) using standard diagnostic criteria. Performance of the oGTT (120 min; 75 g glucose) may already be indicated in the first trimester in high-risk women but is mandatory between gestational week 24-28 in all pregnant women with previous non-pathological glucose metabolism. Following WHO recommendations, which are based on the Hyperglycemia and Adverse Pregnancy Outcome (HAPO) study, GDM is defined, if fasting venous plasma glucose is ≥ 92 mg/dl or 1 h ≥ 180 mg/dl or 2 h ≥ 153 mg/dl after glucose loading (international consensus criteria). In case of one pathological value a strict metabolic control is mandatory. After bariatric surgery we do not recommend to perform an oGTT due to risk of postprandial hypoglycemia. All women with GDM should receive nutritional counseling, be instructed in blood glucose self-monitoring and motivated to increase physical activity to moderate intensity levels-if not contraindicated (Evidence level A). If blood glucose levels cannot be maintained in the therapeutic range (fasting < 95 mg/dl and 1 h after meals < 140 mg/dl, Evidence level B) insulin therapy should be initiated as first choice (Evidence level A). Maternal and fetal monitoring is required in order to minimize maternal and fetal/neonatal morbidity and perinatal mortality. Regular obstetric examinations including ultrasound examinations are recommended (Evidence level A). Neonatal care of GDM offspring at high risk for hypoglycaemia includes blood glucose measurements after birth and if necessary appropriate intervention. Monitoring the development of the children and recommendation of healthy lifestyle are important issues to be tackled for the whole family. After delivery all women with GDM have to be reevaluated as to their glucose tolerance by a 75 g oGTT (WHO criteria) 4-12 weeks postpartum. Assessment of glucose parameters (fasting glucose, random glucose, HbA1c or optimally oGTT) are recommended every 2-3 years in case of normal glucose tolerance. All women have to be instructed about their increased risk of type 2 diabetes and cardiovascular disease at follow-up. Possible preventive meassures, in particular lifestyle changes as weight management and maintenance/increase of physical activity should be discussed (evidence level A).
Pharmacokinetics, efficacy and safety of glyburide for treatment of gestational diabetes mellitus

Abstract:Introduction: Gestational diabetes mellitus (GDM) complicates 10% of all pregnancies and is defined as hyperglycemia first noted during pregnancy. Rates of GDM are rising and untreated GDM results in complications for both mother and fetus. GDM is often managed by diet and exercise but 30-40% of women will require pharmacological intervention. Insulin has traditionally been the treatment of choice but since 2007, glyburide, a second generation sulfonylurea has become the most prescribed medication for GDM. Areas covered: This review will cover the pharmacokinetics, efficacy, and safety of glyburide for the management of GDM. Expert opinion: Management of GDM is challenging secondary to the stringent glycemic goals that mimic the lower glucose levels in pregnancy. Glyburide is generally effective in treating hyperglycemia. However, several studies have raised safety concerns showing higher neonatal intensive care unit (NICU) admissions, higher rates of macrosomia, large for gestational age and pre-eclampsia in the mother. For this reason, insulin should be first-line therapy for GDM. In areas of limited resources where the self-monitoring needed for accurate insulin dosing is not possible, where access to refrigeration for insulin storage is not universal, or severe needle phobia then the benefits of glyburide (controlling hyperglycemia) outweighs the harm of NICU admissions and macrosomia.
Image Fragile Watermarking through Quaternion Linear Transform in Secret Space

Abstract:In this paper, we apply the quaternion framework for color images to a fragile watermarking algorithm with the objective of multimedia integrity protection (Quaternion Karhunen-Loeve Transform Fragile Watermarking (QKLT-FW)). The use of quaternions to represent pixels allows to consider the color information in a holistic and integrated fashion. We stress out that, by taking advantage of the host image quaternion representation, we extract complex features that are able to improve the embedding and verification of fragile watermarks. The algorithm, based on the Quaternion Karhunen-Loeve Transform (QKLT), embeds a binary watermark into some QKLT coefficients representing a host image in a secret frequency space: the QKLT basis images are computed from a secret color image used as a symmetric key. A computational intelligence technique (i.e., genetic algorithm) is employed to modify the host image pixels in such a way that the watermark is contained in the protected image. The sensitivity to image modifications is then tested, showing very good performance.

Abstract:Objective: To compare management based on maternal glycemic criteria with management based on relaxed glycemic criteria and fetal abdominal circumference (AC) measurements in order to select patients for insulin treatment of gestational diabetes mellitus (GDM) with fasting hyperglycemia. Research design and methods: In a pilot study, 98 women with fasting plasma glucose (FPG) concentrations of 105-120 mg/dl were randomized. The standard group received insulin treatment. The experimental group received insulin if the AC, measured monthly, was > or =70th percentile and/or if any venous FPG measurement was >120 mg/dl. Power was projected to detect a 250-g difference in birth weights. Results: Gestational ages, maternal glycemia, and AC percentiles were similar at randomization. After initiation of protocol, venous FPG (P = 0.003) and capillary blood glucose levels (P = 0.049) were significantly lower in the standard group. Birth weights (3,271 +/- 458 vs. 3,369 +/- 461 g), frequencies of birth weights >90th percentile (6.3 vs 8.3%), and neonatal morbidity (25 vs. 25%) did not differ significantly between the standard and experimental groups, respectively. The cesarean delivery rate was significantly lower (14.6 vs. 33.3%, P = 0.03) in the standard group; this difference was not explained by birth weights. In the experimental group, infants of women who did not receive insulin had lower birth weights than infants of mothers treated with insulin (3,180 +/- 425 vs. 3,482 +/- 451 g, P = 0.03). Conclusions: In women with GDM and fasting hyperglycemia, glucose plus fetal AC measurements identified pregnancies at low risk for macrosomia and resulted in the avoidance of insulin therapy in 38% of patients without increasing rates of neonatal morbidity.

Abstract:Introduction Women with a history of gestational diabetes mellitus (GDM) are at high risk of developing type 2 diabetes, while the exact mechanisms underlying its pathophysiology are still unclear. We investigated the association of glucagon-like peptide-1 (GLP-1) response to oral glucose with parameters of glycemic control in women with previous GDM in the prospective PPSDiab (Prediction, Prevention, and Subclassification of Type 2 Diabetes) study. Research design and methods Glucose metabolism parameters and GLP-1 secretion were analyzed during oral glucose tolerance test (OGTT) in women with previous GDM (n=129) and women with a history of normal glucose tolerance (n=67) during pregnancy (controls). First- and second-phase insulin and GLP-1 secretion in relation to plasma glucose (PG) levels were assessed, and development of pre-diabetes was analyzed after 5-year follow-up among women with previous GDM and a normal glycemic state at baseline (n=58). Results The area under the curve (AUC during the OGTT 0–120 min) of PG and insulin but not GLP-1 differed significantly between post-GDM women and controls. However, women with previous GDM had a significantly decreased GLP-1 response in relation to PG and plasma insulin during the second phase of the OGTT. After a follow-up of 5 years, 19.0% post-GDM women with a normal glycemic state at the baseline visit developed abnormal glucose metabolism. The total, first- and second-phase AUC GLP-1/PG and GLP-1/insulin ratios were not associated with development of abnormal glucose tolerance. Conclusions Women with previous GDM showed a reduced GLP-1 response in relation to PG and insulin concentrations indicating early abnormalities in glucose metabolism. However, the altered GLP-1 response to oral glucose did not predict progression to pre-diabetes and type 2 diabetes in the first 5 years after GDM.

Abstract:Background: Very few studies have assessed the role of a clinical pharmacist in women with gestational diabetes mellitus (GDM). To improve pharmaceutical care, we explored a method to improve the control of blood glucose and perinatal outcomes in females with GDM through the application of a diabetes management system. Methods: A randomized controlled trial was conducted from October 2017 to October 2018 for 200 outpatients with GDM. In the study, a diabetes management system with pharmaceutical care was used for the intervention group. The clinical outcomes of all patients were recorded at the end of delivery. Results: From one sample of 200 patients, 169 finished the research. Compared with the control group, patients in the intervention group manifested greater reductions in fasting plasma glucose (5.22 +/- 0.37 vs. 6.05 +/- 1.06 mmol/L; P = 0.018), 2 h postprandial plasma glucose (6.66 +/- 0.57 vs. 9.69 +/- 1.58 mmol/L; P = 0.00), and glycated hemoglobin A1c corrected values (1.02 +/- 0.12 vs. 1.16 +/- 0.22; P = 0.023). Moreover, the rate of polyhydramnios was significantly lower in the intervention group than in the control group (0% vs. 10.59%; P = 0.003) as well as significantly fewer macrosomia in the intervention group (7.14% vs. 18.82%; P = 0.038). Conclusions: Using a diabetes management system, clinical pharmacists can improve the control of blood glucose and perinatal results in GDM females. With a diabetes management system, the comprehensive management of GDM is a new model for pharmaceutical care in the future.
