Abstract:There is still a lack of large-scale studies focusing on acute fulminant myocarditis (AFM) and knowledge of this disease is unimpressive. To better understand the clinical features of AFM and improve the diagnosis and treatment of this disease, we analysed the data of consecutive Chinese AFM patients admitted to our department.This retrospective observational study analysed the data of 40 patients with a diagnosis of AFM admitted to our hospital between January 2003 and March 2010.The mean age of patients was 28.5 +/- 18.4 years and 70% of patients were females. 90.0% of patients had a short viral prodrome, 77.5% had cardiogenic shock and 35.0% had multiple organ failure, especially hepatic dysfunction. The most common manifestations in ECG were low voltage in the limb leads (87.5%) and sinus tachycardia (75%). Myocardial infarctions like ECG changes were not uncommon. Left ventricular ejection fraction (LVEF) was significantly reduced (25.3 +/- 7.5%), but the left cardiac sizes were normal. More than 90% of the patients were treated with glucocorticoids. 5.0% needed an intra-aortic balloon pump (IABP), and 35.0% were treated with bi-level positive airway pressure (BiPAP). In all, 39 (95.0%) patients were discharged alive and one man died from ventricular fibrillation. LVEF and left cardiac chambers at follow-up did not change as compared with discharge. No death, new onset heart failure or arrhythmias occurred during the follow-up.This study describes the clinical features of Chinese AFM patients. The short- and mid-term prognosis of AFM is good. AFM patients may benefit from a treatment with glucocorticoids.
Acute Effect of Astragalus Membranaceus on Blood Pressure in Spontaneously Hypertensive Rats

Abstract:Objective To observe the acute effect of Astragalus Membranaceus(AM) on blood pressure in spontaneously hypertensive rats(SHR).Methods Nineteen SHR were randomly divided into four groups. The AM groups (B、C、D group) were intraperitoneal administered AM parenteral solution 0.9ml,1.2ml and 1.8ml respectively and the control group(A group) was given 0.9ml Saline .Then observe the acute change of blood pressure by indirect manometr.After this experiment, SHR were intraperitoneal administered AM parenteral solution 0.9ml,1.2ml and 1.8ml in the AM groups respectively or 0.9ml Saline in group A for eight weeks. At last, the acute blood pressure effect was determined by direct manometr after SHR were injected different dosage AM or saline through vein.Results (1)Blood pressure in every group did not change after SHR were intraperitoneal administered AM or saline using direct manometr at each observing time point(5′、15′、30′、60′、120′). (2)Abdominal cavity injection of AM for long-term can control the blood pressure set-up. AM can descend SHR blood pressure apparently for short time by injection of vein using direct manometr.Conclusion AM do not change blood pressure in SHR by intraperitoneal administered, but abdominal cavity injection of AM for long-term can control the SHR blood pressure to rise. Injection of AM through vein can cause acute descent of blood pressure for short time in SHR.
Effect of Astragalus Membranaceus on Hypertenison and Mechanisms Investigation in Spontaneously Hypertensive Rats

Abstract:Objective To observe the effect of Astragalus Membranaceus(AM) on blood pressure and to explore the mechanisms of this effect.Methods Nineteen Spontaneously Hypertensive Rats were randomly divided into 1 control group(n=4) and 3 AM groups.The rats in AM groups were intraperitoneal administrated AM parenteral solution at the doseage of 1.8 g,2.4 g and 3.6 g respectively for 8 weeks.Then observe the change of blood pressure successively(The first week,The second week.The eighth week).At last,The left ventricle Hypertrophy index、calcium ions content in lymphocytes and plasma ET-1 level were also determined.Results Blood pressure in the control group became higher and higher from the fourth week to the 8th week(P0.01),but the other SHR administeredAM contained their blood pressure level.There is great significant difference of LVHI between middle dose group and high dose group in SHR(P0.01).The calcium ions content of lymphocytes in middle dose group and high dose group were lower than that in the control group in SHR(P0.05 orP0.01).There is great significant difference of the plasma ET-1 level in middle dose group and high dose group compared to that in low dose group in SHR(P0.05 or P0.01).Blood pressure,LVHI,calcium ions content of lymphocytes and plasma ET-1 level of Wky were no great significant difference after administrated AM for 8 weeks.Conclusion AM can prevent the blood pressure of SHR from higher level and relieve LVH after administrated for 8 weeks.These pharmacological actions about decreasing calcium ions content of lymphocytes and plasma ET-1 level in SHR may be one of the mechanisms for the effect of AM on blood pressure.
Astragalus membranaceus extract promotes neovascularisation by VEGF pathway in rat model of ischemic injury.

Abstract:Astragalus membranaceus extract (AM E) is a widely used herbal product for the treatment of cardiovascular diseases in China. The present study aimed to evaluate the cardiac protective effects of AME, and to probe the underlying molecular mechanism related to angiogenesis. In this study, AME with 75 mu g/mL significantly increased proliferation, migration and tube formation on human umbilical vein endothelial cells (HUVECs). Moreover, in vivo experiments on rats with ligation of left anterior descending artery were performed to study the cardiac protective and angiogenic effect of AME (50 and 100 mg/kg i. g. for 3, 7, 14 days). The results showed that AME inhibited cardiac fibrosis, reduced infarct size, and increased capillary and arteriole densities. Meanwhile, western blot was used to determine protein levels of VEGF, p-AKT, p-GSK3 beta and p-mTOR. AME significantly elevated protein expression of VEGF and increased phosphorylation of AKT, GSK3 beta and mTOR. In conclusion, AME exerted cardiac protective and angiogenic effects in the ischemic injured heart. The activation of AKT/GSK3 beta and AKT/mTOR pathways and elevated expression of VEGF may contribute to the promoted neovascularisation by AME.
Effect of astragalus injection on left ventricular remodeling in HFmrEF: a systematic review and meta-analysis

Abstract:Objectives: The aim of this meta-analysis is to evaluate the effect of astragalus injection (AI) on left ventricular remodeling (LVR) in patients with heart failure with mildly reduced ejection fraction (HFmrEF). Methods: The randomized controlled trials (RCTs) of AI in treating HFmrEF were retrieved from 8 major English and Chinese electronic databases, up until November 30, 2023. To evaluate the methodological quality of the included studies, the Cochrane bias risk tool and the Modified Jadad Scale were employed. Stata 17.0 software was utilized for statistical analysis, sensitivity analysis, and assessment of publication bias. Results: Ten RCTs with 995 patients (562 males and 433 females) were identified. Meta-analysis indicated that compared to conventional treatment (CT), AI significantly improved LVR, specifically increasing left ventricular ejection fraction (LVEF, MD = 4.56, 95% CI: 3.68-5.44, p < 0.00001), decreasing left ventricular end-diastolic volume (LVEDV, MD = -7.89, 95% CI: -11.13 to -4.64, p < 0.00001), left ventricular end-diastolic diameter (LVEDD, MD = -4.18, 95% CI: -5.79 to -2.56, p < 0.00001), left ventricular end-systolic volume (LVESV, MD = -8.11, 95% CI: -11.79 to -4.43, p < 0.00001), and left ventricular end-systolic diameter (LVESD, MD = -3.42, 95% CI: -4.90 to -1.93, p < 0.00001). AI also improved clinical efficacy (RR = 4.62, 95% CI: 3.11-6.88, p < 0.00001), reduced N-terminal pro-brain natriuretic peptide (NT-pro BNP, MD = -27.94, 95% CI: -43.3 to -12.36) level, without increasing the incidence of adverse reactions (RR = 1.60, 95% CI: 0.59-4.29, p = 0.35). Sensitivity analysis confirmed the reliability of the merged results, and Begg's and Egger's tests showed no significant publication bias. Conclusion: The systematic review and meta-analysis revealed that combining AI with CT improves LVR without increasing adverse events in HFmrEF patients. However, caution is needed in interpreting the results due to limited evidence. Future high-quality RCTs are needed to support these conclusions. The Clinical Features of Children With Acute Fulminant Myocarditis and the Diagnostic and Follow-Up Value of Cardiovascular Magnetic Resonance

Abstract:Objective: To investigate the clinical features and the diagnostic and follow-up value of acute fulminant myocarditis (AFM) in children.Methods: A total of 20 children diagnosed with AFM admitted to our department were reviewed, and the clinical manifestations; pathogenic examination results; myocardial injury biomarkers; and electrocardiography, echocardiogram, and cardiovascular magnetic resonance (CMR) results were analyzed.Results: Twenty children with AFM, including 12 males and 8 females, aged 3–16 years, were analyzed. The initial symptoms were abdominal pain, vomiting, fatigue, syncope, and convulsions. All children had significantly increased hs-cTnT and NT-pro BNP. In addition to nonspecific ST-T changes, there were 10 cases of complete atrioventricular block, 2 cases of advanced atrioventricular block, and 1 case of ventricular tachycardia. Echocardiography showed an increase in the cardiac chamber sizes in 15 patients and a decrease in left ventricular ejection fraction (LVEF) in 17 patients. There were 16 patients with abnormal CMR findings, including 13 cases of high T2-weighted image (T2WI) signal and 14 cases of late gadolinium enhancement (LGE). In the patients who underwent CMR within 14 days of onset, the sensitivity of T2WI and LGE and the positive diagnosis rate were higher than in those who underwent CMR after 14 days, but the difference was not statistically significant. CMR was followed up in 10 patients: 7 patients returned to normal, 2 patients still had mild LGE, and 1 patient developed inflammatory dilated cardiomyopathy. All patients were treated with high-dose immunoglobulin, 11 of whom received high-dose immunoglobulin combined with glucocorticoids. Eight patients received temporary pacemakers, and 1 patient received ECMO. None of the patients died. The peak of hs-cTnT was significantly higher in the glucocorticoid group than in the unused glucocorticoid group (2853.4 ± 2217.2 and 1124.7 ± 527.3 pg/ml, respectively).Conclusion: Children with AFM have unique clinical features. Early identification and effective treatment can reduce the mortality rate and improve the prognosis. CMR is highly sensitive in the diagnosis of ARM, especially within 14 days of onset, and is a useful noninvasive imaging technique for the early identification of AFM in children. The dynamic observation and follow-up of children with AFM through CMR can guide clinical decision-making and prognosis assessment.

Abstract:BACKGROUND: In recent years, with the enormous advances in the field of cardiac intervention technology, the survival rate of patients with acute myocardial infarction (AMI) has been improved significantly. However, the risk of arrhythmias and heart failure remains very high in AMI patients for long-term prognosis. Chinese herbal medicine (CHM) is more and more used in the treatment of AMI because of its good curative effect and less side effects. The target of this research is to analyze the efficacy and safety of Astragalus (Huangqi) preparation in the treatment of AMI by meta-analysis and also to provide a better evidence for clinical practice.METHODS: Seven databases will be searched in this study: The Cochrane Library, PubMed, Web of Science, the Chinese National Knowledge Infrastructure (CNKI), the Chinese Scientific Journal Database (CSJD), the Chinese Biomedical Literature Database (CBM), and Wanfang DATA. The following search terms will be used: (Huangqi OR Huang Qi OR Astragalus OR radix astragali) AND (acute myocardial infaction OR myocardial infaction OR AMI) AND (randomized controlled trial OR RCT OR randomized). No language limitations and the searches will be conducted up to March, 2019.INCLUSION CRITERIA: randomized controlled trial (RCT) of Astragalus (Huangqi) preparation in patients with AMI. Main outcome measures will be left ventricular end systolic volume (LVESV), left ventricular end diastolic volume (LVEDV), left ventricular ejection fraction (LVEF), left ventricular mass index (LVMI), recanalization rate, mortality rate, incidence of reperfusion arrhythmias, postinfarction angina pectoris, and re-infarction rate. Secondary outcome indicators were the incidence of adverse reactions and the effective rate of traditional Chinese medicine (TCM) treatment. Two independent reviewers will filter the literature and extract data which based to the Cochrane manual. The relevant data, including bias risk assessment, data synthesis, subgroup analysis, meta-analysis, and final meta-analysis, will be analyzed with RevMan 5.3 software. The funnel diagram will be used to evaluate the reported deviation, and the Egger test will be used to evaluate the symmetry of the funnel graph.RESULTS: This systematic review study will provide a clear basis for evaluating the efficacy and safety of Astragalus (Huangqi) preparation with the treatment of AMI.CONCLUSION: This study will provide an up-to-date evidence for evaluating the efficacy and safety of Astragalus (Huangqi) preparation.PROSPERO REGISTRATION NUMBER: CRD42019124843.

Abstract:Introduction: The acute fulminant myocarditis (AFM) is a special type of myocarditis, characterized by rapid onset, progression with abnormal hemodynamics causing high mortality. The combination of extracorporeal membrane oxygenation (ECMO) and intra-aortic balloon counterpulsation (IABP) in these cases have improved the acute phase attacks resulting in increased survival of the effected individuals. To observe the effect of vein-artery (V-A) mode assisted ECMO combined with IABP on the disease outcomes and troponin level in patients with acute fulminant myocarditis (AFM). Methods: A total of 45 patients with AFM were admitted to our hospital from January 2016 to December 2021, they were divided into group A (21 patients, receiving traditional treatment) and group B (24 patients, receiving V-A mode assisted ECMO and IABP based on traditional treatment) according to the treatment plan. The hypersensitive troponin (hs-cTnI), B-type natriuretic peptide (BNP), arterial blood gas analysis index, multiple organ dysfunction score (MODS), simplified acute physiology score II (SAPS II), cardiac ultrasound index, and disease outcomes were compared between the two groups. Results: After treatment, the hs-cTnI, BNP, blood lactic acid, MODS score and SAPS score in both groups were lower than before treatment (p < 0.05), the arterial oxygen saturation (SaO2), left ventricular ejection fraction (LVEF) and cardiac index (CI) was higher than before treatment (p < 0.05), the hs-cTnI, BNP, blood lactic acid, MODS score and SAPS II score in group B were lower than group A (p < 0.05), SaO2, LVEF and CI was higher than group A (p < 0.05). The in-hospital mortality of group B (9.52%, 2/21) was lower than group A (45.83%, 11/24) (χ2 = 7.188, p = 0.007). Among the recovered discharged patients, the length of stay in group B (19 patients, 14.42 ± 4.59 days) was shorter than group A (13 patients, 18.34 ± 5.72 days) (t = 2.147, p = 0.040). Conclusions: V-A mode assisted ECMO combined with IABP can improve the condition and arterial blood gas analysis index and cardiac function of AFM patients, alleviate myocardial injury, reduce mortality in the hospital, and shorten the hospitalization time of the recovered patients.

[Astragalus injection mediates autophagy and myocardial remodeling in rats with ischemic cardiomyopathy through calumenin]

Abstract:Objective: To study the effects of astragalus injection on myocardial remodeling, calumenin and autophagy in rats with ischemic cardiomyopathy. Methods: Thirty-six male SD rats were divided into normal control group, ischemic cardiomyopathy group and astragalus injection group, 12 in each group. Electrocardiogram (ECG) and echocardiography were performed before operation in three groups. Rats in ischemic cardiomyopathy group and astragalus injection group underwent thoracotomy and ligation of coronary artery for 20 minutes, then thoracic cavity was closed after reperfusion. In the astragalus injection group,10 g/kg body weight of Astragalus injection was injected once a week, four times in total. Four weeks after operation, rats in three groups were executed by echocardiography and their hearts were collected for Hematoxylin-Eosin (HE) staining and Van Gieson (VG) staining to observe myocardial pathological changes. Calumenin, LC3-I, LC3-II expressions and LC3-I/LC3-II ratio were detected by Western blot. Results: Compared with ischemic cardiomyopathy group, the echocardiography and myocardial pathology of rats in astragalus injection group changed obviously, and the expressions of calumenin, LC3-I, LC3-II and LC3-I/LC3-II ratio changed significantly (P＜0.01). Conclusion: Astragalus injection has apparent inhibitory effect on ventricular remodeling and autophagy of myocardial cells in rats with ischemic cardiomyopathy, which may be mediated by calumenin.
Effect of Astragalus membranaceus on left ventricular remodeling in HFrEF: a systematic review and meta-analysis

Abstract:Background: Left ventricular remodeling (LVR) is a key factor leading to the onset and progression of heart failure with reduced ejection fraction (HFrEF). Improving LVR can delay the progression of HFrEF and improve quality of life. Objective: To evaluate the improvement effect of Astragalus membranaceus (A. membranaceus) on LVR in patients with HFrEF. Method: We retrieved randomized controlled trials (RCTs) of A. membranaceus in treating HFrEF from eight Chinese and English databases, up until 31 October 2023. To assess the quality of the literature, we utilized the bias risk tool from the Cochrane Handbook. For meta-analysis, we employed Review Manager 5.4.1 software. Additionally, we performed sensitivity analysis and publication bias assessment using Stata 17.0 software. Result: Totally 1,565 patients were included in 19 RCTs. Compared to conventional treatment (CT), the combination therapy of A. membranaceus with CT demonstrated significant improvements in LVR, specifically increasing left ventricular ejection fraction (LVEF, MD = 5.82, 95% CI: 4.61 to 7.03, p < 0.00001), decreasing left ventricular end-diastolic diameter (LVEDD, MD = −4.05, 95% CI: −6.09 to −2.01, p = 0.0001), and left ventricular end-systolic diameter (LVESD, MD = −12.24, 95% CI: −15.24 to −9.24, p < 0.00001). The combination therapy of A. membranaceus with CT also improved clinical efficacy (RR = 4.81, 95% CI: 3.31 to 7.00, p < 0.00001), reduced brain natriuretic peptide (BNP, MD = −113.57, 95% CI: −146.91 to −81.22, p < 0.00001) level, and increased 6-min walking distance (6-MWD, MD = 67.62, 95% CI: 41.63 to 93.60, p < 0.00001). In addition, the combination therapy of A. membranaceus with CT mitigated inflammatory responses by reducing tumor necrosis factor-alpha (TNF-α, MD = −16.83, 95% CI: −22.96 to −10.71, p < 0.00001), interleukin-6 (IL-6, MD = −29.19, 95% CI: −36.08 to −22.30, p < 0.00001), and high-sensitivity C-reactive protein (hs-CRP, MD = −0.98, 95% CI: −1.43 to −0.52, p < 0.0001). Notably, the combination therapy of A. membranaceus with CT did not increase the incidence of adverse reactions (RR = 0.86, 95% CI: 0.25 to 2.96, p = 0.81). Conclusion: This systematic review and meta-analysis revealed that the combination therapy of A. membranaceus with CT has more advantages than CT alone in improving LVR and clinical efficacy in HFrEF patients, without increasing the incidence of adverse reactions. However, due to the limited quality of included studies, more high-quality investigations are required to provide reliable evidence for clinical use. Effect of astragaloside on myocardial fibrosis in viral myocarditic mice

Abstract:AIM: To investigate the effect of astragaloside, one of the active components of Astragalus membranaceus, on myocardial fibrosis in coxsackievirus B3 (CVB3) induced murine myocarditis model. METHODS: One hundred BALB/C mice were randomly divided into 6 groups. Group A (n=12,treated with normal saline 0.1 mL, ig, for 1 wk) was served as normal control and group B (n=16, treated with 9 % astragaloside 0.1 mL, ig, qd for 1 wk) as treatment control. The seventy-two infected animals treated with normal saline (NS) and 1 %,3 %,9 % astragaloside 0.1 mL, ig, qd for 1 wk (n=18 in each group), were served as group C,D,E,F, respectively. The mice were killed after 14 d. Serum concentrations of procollagen peptides (PIP and PIIINP) were measured by ELISA. Collagen volume fraction (CVF) was determined on picrosirius red-stained sections with an automated image analysis system. Apoptosis index of cardiac myocytes was measured by flow cytometry. RESULTS: The mortality was significantly reduced in 9 % astragaloside treated infected animals that was 22 %(4/18) vs 44 %(8/18) in group C, P<0.05. In uninfected animals, 9 % astragaloside did not cause any death; CVF and serum procollagen peptides concentrations were significantly lower in group F than that in group C ((10±s 3 ) %vs (21±8) %, (1.4±0.6) mg*L-1 vs (3.0±1.9) mg*L-1, (22±4) mg*L-1 vs (29±5) mg*L-1, respectively, P<0.01). The survival, CVF and serum peptides were not affected by 1 % and 3 % astragaloside. Apoptosis index of cardiac myocytes was lower in 9 % astragaloside treated infected murines compared with that in group C ((13 ±6 ) %vs (36±11) %,P<0.01)).There was a direct correlation between apoptosis index and CVF( r = 0.717, P<0.01), the concentrations of PIP( r = 0.385, P<0.01) and PIIINP( r = 0.391, P<0.01), respectively . CONCLUSION: Results show that 9 % astragaloside can improve survival rate, reduce collagen synthesis and cardiac myocytes apoptosis effectively and safely in this model. The antiapoptotic effect of astragaloside can play an important role for alleviating and reversing myocardial fibrosis.
Study on Astragalus injection in patients with chronic heart failure on brachial endothelial function

Abstract:Objective To study the endothelial function of Astragalus injection and the effects of Astragalus on chronic heart failure(CHF)in patients with cardiac function in the mechanism.Methods 68 patients with CHF were randomly divided into treatment group and the control group,each of 34 cases.The control group used of ACEI,β-blockers,digitalis and diuretics such as conventional Western medicine treatment,the treatment group on the base of the control group used Astragalus injection.Two group used of echocardiography parameters such as changes in brachial artery diameter and were observed endothelium-dependent relaxation of Astragalus on the role and function of the CHF patients.Results A month later,the patients of left ventricular function decentralized ≥Ⅲ level were visibly reduced than before treating patients(P<0.05).After treatment,the endothelium function FMD in two groups had improved significantly,and treatment groups were better than control group(P<0.01 or P<0.05).After treatment,NMD,Vmean,PI and RI of two groups had without change,and than before treating,the difference had no statistical significance(P>0.05).Conclusion Astragalus injection has a good curative effect on CHF patients by significantly improving the patients'brachial endothelial function.
Astragalus mongholicus (Fisch.) Bge Improves Peripheral Treg Cell Immunity Imbalance in the Children With Viral Myocarditis by Reducing the Levels of miR-146b and miR-155

Abstract:Viral myocarditis (VMC) is a common cardiac disease, however, there still lacks an effective therapeutic strategy for VMC. Astragalus mongholicus (Fisch.) Bge (AB), a Chinese herb with some functional metabolites, may have some pharmacological effects on VMC. AB ingredients were measured by a full-scan LCQ mass spectrum. We aimed to explore the effects of AB on the VMC children by investigating peripheral Treg cell homeostasis. A total of 68 VMC children were random and evenly assigned into an AG group (received 10-mL AB oral liquid daily), and a CG group (received placebo daily). Peripheral blood mononuclear cells (PBMC) were obtained from peripheral blood and Treg cells were isolated. The levels of miR-146b, miR-155, Treg immunity activity and myocarditis biomarkers were measured in Treg cells. There were four main components (sucrose, calycosin, Astragaloside IV and calycosin-7-glucoside) in AB. The cases sinus tachycardia, frequent premature ventricular contractions, and supraventricular tachycardia were significantly reduced in the AG group (P < 0.05). Meanwhile, the myocardial enzymes and cardiac function indexes were improved in the AG group when compared with the CG group (P < 0.05). The time of electrocardiogram recovery, symptom duration and hospital stay was shorter in the AG group than in the CG group (P < 0.05). The levels of miR-146b and miR-155 were higher in the CG group than in the AG group (P < 0.05). The levels of ROR-γt (retinoic acid receptor-related orphan nuclear receptor gamma), FoxP3 (forkhead transcription factor), IL-10 (interleukin-11) and TGF-β (transforming growth factor beta) were lower in the CG group than in the AG group (P < 0.05). In contrast, the levels of IL-17, IL-21, CK-MB (creatine kinase-MB), cTnI (cardiac troponin I), GrB (granzyme B), sFasL (soluble fas ligand) and caspase-3 were higher in the CG group than in the AG group (P < 0.05). Furthermore, the levels of ROR-γt, FoxP3, IL-10, and TGF-β were positively, whereas the levels of IL-17, IL-21, CK-MB, cTnI, GrB, sFasL and caspase-3 were negatively, associated with the levels of miR-146b and miR-155 (P < 0.05). AB treatment improved cardiac functions, peripheral Treg cell immunity imbalance in the children with VMC by reducing the levels of miR-146b and miR-155.

Abstract:Ethnopharmacological relevance Myocardial fibrosis (MF) is a common pathological manifestation of many cardiovascular diseases at a certain stage, with excessive accumulation of collagen fibers, excessive increase in collagen content, and a significant increase in collagen volume as the main pathological changes. There are currently no effective drugs for the treatment of myocardial fibrosis. Traditional Chinese medicine (TCM), the main component of the medical practice used for more than 5000 years, especially in China, often exerts a wider action spectrum than previously attempted options in treating human diseases. In recent times, the great potential of TCM in the treatment of MF has received much attention. Especially many experimental studies on the treatment of MF by Astragalus mongholicus Bunge have been conducted, and the effect is remarkable, which may provide more comprehensive database and theoretical support for the application of Astragalus mongholicus Bunge in the treatment of MF and could be considered a promising candidate drug for preventing MF. Aim of the review This review summarizes the chemical components of Astragalus mongholicus Bunge, Astragalus mongholicus Bunge extrac t, Astragalus mongholicus Bunge single prescription, and Astragalus mongholicus Bunge compound preparation in the treatment of MF, and provides comprehensive information and a reliable basis for the exploration of new treatment strategies of botanical drugs in the therapy of MF. Methods The literature information was obtained from the scientific databases on ethnobotany and ethnomedicines (up to August 2022), mainly from the PubMed, Web of Science, and CNKI databases. The experimental studies on the anti-myocardial fibrosis role of the effective active components of Astragalus mongholicus Bunge and the utility of its compound preparation and the involved mechanisms were identified. The search keywords for such work included: "myocardial fibrosis" or "Cardiac fibrosis ", and " Astragalus mongholicus Bunge", "extract," or "herb". Results Several studies have shown that the effective active components of Astragalus mongholicus Bunge and its formulas, particularly Astragaloside IV, Astragalus polysaccharide, total saponins of Astragalus mongholicus Bunge, triterpenoid saponins of Astragalus mongholicus Bunge, and cycloastragenol, exhibit potential benefits against MF, the mechanisms of which appear to involve the regulation of inflammation, oxidant stress, and pro-fibrotic signaling pathways, etc. Conclusion : These research works have shown the therapeutic benefits of Astragalus mongholicus Bunge in the treatment of MF. However, further research should be undertaken to clarify the unconfirmed chemical composition and regulatory mechanisms, conduct standard clinical trials, and evaluate the possible side effects. The insights in the present review provided rich ideas for developing new anti-MF drugs. Abstract:Abstract Background Coronary heart disease (CHD) is a clinical syndrome caused by coronary atherosclerosis (AS) or functional changes in coronary arteries. We have previously reported that astragaloside IV (in astragalus) and tanshinone IIA (in Salvia miltiorrhiza ) improve myocardial ischemic injury. Methods This study will employ the randomized, blinded, prospective, single-center experiments approach. Briefly, a total of 160 eligible patients will be equally randomized into three treatment groups and placebo control groups. Patients will receive appropriate treatments every 24 h for seven days after enrollment and followed up for 28 days. The main evaluation indicators will be cell count, serum high-sensitivity C-reactive protein (hs-CRP) level, monocyte chemoattractant protein 1 (MCP-1), interleukin-6 (IL-6), interleukin-1β (IL-1β), interleukin-8 (IL-8), interleukin-18 (IL-18), interleukin-10 (IL-10), tumor necrosis factor (TNF)-α, oxidized low density lipoprotein (OX-LDL), angina grade, and Traditional Chinese Medicine (TCM) symptom changes scale. Secondary indicators to be evaluated will include B-type natriuretic peptide (BNP) levels, troponin (cTn), muscle enzyme isoenzyme (CK-MB), heart-type fatty acid binding protein (H-FABP), liver and renal functions, as well as blood coagulation. Close monitoring of adverse events during the trial will also be conducted. Discussion This randomized controlled trial of Chinese herbal extracts for the treatment of coronary heart disease will generate results that are expected to provide valuable clinical evidence to inform future development of therapies towards management of CHD. Clinical observation of Radix Astragali injection on severe septic patients

Abstract:Objective To observe the effects of Radix Astragali injection on severe septic patients. Method This was a prospective study. Fifty severe septic patients admitted in ICU from 2006 to 2008 were col-lected, malignent disease like carcinoma, pregnant ones were excluded. They were randomly divided into two groups: Radix Astragali injection treatment group (n = 30) and control one (n = 20). The data of the two groups were comparable. With the same basic therapy, the experimental group treated with additional Radix Astragali in-jection. Then, APACHE Ⅱ scores were calculated on day 0(before therapy), day 7 and day 14 after treatment with Radix Astragali injection, and the 28th day mortality was also calculated for both groups. Blood CD4, CD8, CD4/CD8, NK-cell expression level was measured by direct immunofluorescence, IL-1β, IL-6, TNF-α level was tested by ELASA. SPSS 11.5 software, was used for statistic. Results The APACHE Ⅱ scores in Radix Astragali treatment group were 20.73±5.06, while in control groups were 35.00±3.70 after treatment for 14 days, and mortality in the 28th day was 25.3% and 41.6% respectively, there were significant deferences between the two groups (P < 0.05). At 14 days after treatment, the level of CD4, CD4/CD8 and NK-cell in the Radix Astragali treatment group was significantly increased, with (34.11±14.09) %, (2.05±0.98) %, (28.83±8.08) %, re-spectively, and compared with control group, there were significant differences (P < 0.01). Though, the level of CD8, IL-6 and TNF-α was showed (20.84±9.84) %,(710.87±313.52) pg/mL,(14.63±30.38) pg/mL respectively, significantly decreased after treatment with Radix Astragali injection, there were also significant dif-ferences between the two groups (P < 0.01). Conclusions Radix Astragali injection can improve the immunity disorder of severe septic patients, it will be a hopeful therapy for severe sepsis.

The Effect of Astragalus Injection on the Expression of Caspase-3 after Hypoxia/hypoglycemia and Reoxygenation in Hippocampal Neurons of Rats

Abstract:Objective To investigate the effect of astragalus injection on the expression of caspase-3 interrelated by apoptosis after hypoxia/ hypoglycemia and reoxygenation in hippocampal neurons of rats.Methods The hippocampal neurons cultured for eight days were divided into normal control(normal),hypoxia/ hypoglycemia and reoxygenation(model),astragalus injection solution(solution),astragalus injection groups(astragalus).All groups were treated with hypoglycemia and reoxygenation after deprived of oxygen and glucose for 30 minutes except normal group.Western blot was used to measure the expression of caspase3 protein,RT-PCR was used respectively to measure the expression of caspase-3 mRNA after hypoxia/ hypoglycemia and reoxygenation 0,0.5,2,6,24,48,72,120 h.Results Compared with that of normal group,the expression of caspase-3 protein in hippocampal neurons of rats every time points was increased obviously in hypoxia/hypoglycemia and reoxygenation group except 0 and 0.5 h(P0.05), and reached peak at 24 h.Compared with that of hypoxia/hypoglycemia and reoxygenation group,the expression of caspase-3 protein every time points in astragalus injection solution group had no obvious change,however the expression of caspase-3 protein in hippocampal neurons of rats every time points was decreased obviously in the astragalus injection group except 0 and 0.5 h(P0.05).Compared with that of normal control group,the mean optic density(MOD) of expression of caspase3 mRNA in hippocampal neurons of rats every time points was increased obviously in hypoxia/hypoglycemia and reoxygenation group except 0 and 0.5 h(P0.05),and reached peak at 24 h.Compared with that of hypoxia/hypoglycemia and reoxygenation group,the MOD of expression of caspase-3 mRNA every time points in astragalus injection solution group had no obvious change,however the MOD of expression of caspase-3 mRNA in hippocampal neurons of rats every time points was decreased obviously in the astragalus injection group except 0 and 0.5 h(P0.05).Conclusions Astragalus injection could inhibit the expression of caspase-3 after hypoxia/hypoglycemia and reoxygenation,accordingly inhibit hippocampal neuronal apoptosis.

Abstract:Objective: To summarize clinical experience of using extracorporeal membrane oxygenation (ECMO) in rescuing children with acute fulminant myocarditis (AFM). Method: Data of 12 children with acute fulminate myocarditis (6 boys and 6 girls, median age 8.3 (0.6, 13.0) years, median weight 33.1 (6, 61) kg) who were rescued with ECMO in Children's Hospital, Zhejiang University from September 2009 to August 2015 were analyzed retrospectively. The analysis focused on the intervene timing of ECMO for the cardiogenic shock and hypoperfusion caused by heart failure and(or) lethal arrhythmia and the essentials of ECMO cardiopulmonary resuscitation(ECPR) for cardiac arrest in pediatric AFM were summarized. Result: The median ECMO duration was 110(22, 240) h. Ten cases survived and 2 were dead of the total of 12 patients. Six ECPR patients survived and 2 were dead in the total of 8 ECPR patients. The complication of 10 survivors were cannula site bleeding (3 cases), hypernatremia and intracranial hemorrhage (1 case), limping (1 case), hoarse voice (1 case), and cerebral injury (1 case). Conclusion: The key points of improving ECMO rescuing outcome for the AFM children are grasping the ECMO intervene timing and training skilled ECMO team. For ECPR patients, keeping effective chest compressions resuscitation is the key to achieve survival and improve the quality of life.

Abstract:Disagreement exists regarding the long-term prognosis and recovery of left ventricular (LV) function in patients with fulminant myocarditis (FM). This study reported the outcome and LV ejection fraction (EF) changes in FM treated with Chinese protocol, and assessed whether global longitudinal strain (GLS) by two-dimensional speckle tracking echocardiography (2-D STE) could provide additional information. This retrospective study included 46 FM adult patients who applied timely circulatory support and immunomodulatory therapy with adequate doses of both glucocorticoids and immunoglobulins as core approaches and survived after acute phase. They all presented with acute onset of cardiac symptoms < 2 weeks. LV end-diastolic dimensions, LVEF and GLS at discharge and 2-year were obtained and compared. We then performed linear regression and ROC analysis to determine independent factors to predict normalization of GLS at 2-year. At 2 years, the survival was 100% in our cohort. And the GLS improved modestly (15.40 ± 3.89% vs 17.24 ± 2.89%, P = 0.002). At two years, a proportion of patients whose LV function remained abnormal, being 22% evaluated by EF (< 55%) and higher to 37% by GLS (< 17%). Moreover, GLS at discharge but not at presentation correlated with GLS at 2-year (r = 0.402, P = 0.007). The FM adult treated with Chinese protocol have good survival and modest improvement of LV function during 2-year.
