Abstract:SAGE Open, Volume 13, Issue 3, July-September 2023. Empathy is a fundamental quality that nursing staff should possess, but the empathy ability of nursing students in China is currently at an intermediate level and requires improvement. This study aimed to explore teaching methods for nursing humanistic education, that can improve the empathy ability of nursing students and cultivate high-quality nursing talents. This study employed narrative education and experiential teaching methods to develop empathic competence in teaching pediatric nursing courses. The empathic ability of nursing students was measured using the JSPE-NS scale before and after teaching and was analyzed to refine the teaching reflection theme words. After teaching, nursing students scored higher on the total empathy scale, perspective-taking dimension, and emotional care dimension than before teaching, with statistically significant differences (p < 0.05). Five themes were extracted from the teaching reflection, including emotional empathy, pediatric nursing skills, gratitude to parents, reverence for life, and course experience. The results show that the combination of narrative education and experiential teaching methods applied to pediatric nursing courses effectively improved nursing students' empathy, strengthened their identification with the nursing profession, and promoted the teaching reform of humanistic education in nursing courses. The combination of narrative education and experiential teaching methods can improve the empathic ability of nursing students. This approach is suitable for integrating humanistic education into nursing professional courses and provides a feasible idea for developing humanistic qualities in nurses.

Abstract:Objectives Empathy education is very important for medical students. There is little research on the influence of early clinical practice on the development of empathy and other aspects of professionalism in medical students. The aim of this study was to compare the self-reported empathy levels of first-year and second-year undergraduate medical students before and after their early clinical contact curriculum. Setting The study was conducted at the Shanghai University of Medicine & Health Sciences, Shanghai, China. Participants A total of 257 undergraduate medical students participated in the study. The 154 first-year students were studying in 10 community-based teaching hospitals, and the 103 second-year students were studying in 3 university-affiliated hospitals. Primary and secondary outcome measures Primary measures: the Jefferson Scale of Empathy—Student version (JSE-S) was compared between students of different sexes and in different academic years before their early clinical contact course. Secondary measures: comparisons were made after they finished the curriculum 3 weeks later. Results A total of 219 of 257 students responded (85.21% response rate), and 214 answers could be analysed (135 first-year and 79 second-year students; 120 female and 94 male individuals). No significant differences in the empathy scores before early clinical contact were observed between students of different sexes and in different academic years. After early clinical contact, the mean JSE-S score of the participants was significantly higher than the mean score at the beginning of the curriculum. Conclusions Empathy-focused training during early clinical contact can improve the empathetic capacity of undergraduate medical students. Fostering empathetic attitudes among undergraduate medical students is necessary for the early stage of their medical education. Further research is needed on the long-term effects of empathy-focused education in entry-level medical students.

Abstract:Background Empathy is a core skill essential to patient‐centred care. Yet a decline in empathy throughout undergraduate medical education is well recognised. The mainstay of existing teaching approaches to foster medical students' empathic ability has largely focused on the cognitive domain of empathy within the context of communication skills learning. However, there is growing evidence for educational initiatives that better evoke students' affective emotional responses. Approach A student‐led educational initiative was created to enhance medical students' understanding of empathy through undertaking different experiential approaches. Eight medical students were invited to participate in an empathy workshop that involved two experiential approaches: personal simulation, and patient narrative, selected given their expected focus on the affective domain. Evaluation A subsequent focus group discussion explored students' reactions, learning, and intended future change in practice. Discussions were coded and analysed for descriptive themes. Both initiatives were reported to generate students' empathic responses. Personal simulation evoked more affective domains whilst patient narratives additionally created cognitive empathic insight into the impact of the patient's condition. Students also reported intended changes to their future consulting practices. Conclusions Although this is a small‐scale exploration of medical students' experiences of empathy‐related teaching initiatives, it offers insight into how each initiative could target different aspects of empathy. Data support the use of even brief experiential teaching to improve medical students' empathy during their undergraduate training. We provide recommendations for clinical educators who are considering designing similar initiatives within their undergraduate medical curricula.

Abstract:Empathy means the ability of understanding, being sensitive to the feelings of another object. It's an important factor for the benign doctor-patient relationship and patient's "physical and mental healing" . Empathy cultivation among the medical students helps them to get high empathy ability and will benefit their following career much. This article analyzed the current situation of cultivation and study of empathy in Chinese medical school students and found the common empathy level was in a middle level among medical students. The empathy level was influenced by sex, age, specialties, etc. The systemic cultivation of empathy for students was far from the needs. It is necessary to expand the studies of empathy among more medical specialties, then establish the appropriate cultivation methods for medical students to build up high empathy level.

Abstract:Abstract Background Empathy is pivotal to effective clinical care. Yet, the art of nurturing and assessing empathy in medical schools is rarely consistent and poorly studied. To inform future design of programs aimed at nurturing empathy in medical students and doctors, a review is proposed. Methods This systematic scoping review (SSR) employs a novel approach called the Systematic Evidence Based Approach (SEBA) to enhance the reproducibility and transparency of the process. This 6-stage SSR in SEBA involved three teams of independent researchers who reviewed eight bibliographic and grey literature databases and performed concurrent thematic and content analysis to evaluate the data. Results In total, 24429 abstracts were identified, 1188 reviewed, and 136 included for analysis. Thematic and content analysis revealed five similar themes/categories. These comprised the 1) definition of empathy, 2) approaches to nurturing empathy, 3) methods to assessing empathy, 4) outcome measures, and 5) enablers/barriers to a successful curriculum. Conclusions Nurturing empathy in medicine occurs in stages, thus underlining the need for it to be integrated into a formal program built around a spiralled curriculum. We forward a framework built upon these stages and focus attention on effective assessments at each stage of the program. Tellingly, there is also a clear need to consider the link between nurturing empathy and one’s professional identity formation. This foregrounds the need for more effective tools to assess empathy and to better understand their role in longitudinal and portfolio based learning programs.

Abstract:Abstract Background The aims of this study were to examine the factor structure of the Chinese version of the Jefferson Scale of Empathy for medical students (JSE-S) and investigate differences in empathy scores among Chinese medical students according to gender, student cadre or not, future career preference, and parents’ education. Methods Medical students from three universities completed an online questionnaire containing the JSE-S. Exploratory factor analysis was conducted to determine the factor structure, and group comparisons of empathy scores were examined via t -tests and analysis of variance. Results Four factors emerged from the factor analysis: “perspective taking,” “compassionate care,” “standing in the patient’s shoes,” and an uninterpretable factor. The results indicated that students who were female, held positions as student cadres, preferred to become a doctor, and whose fathers had a high school education or below tended to have more empathy. Conclusions Overall, the findings provide information on the dimensions of empathy applicable to Chinese medical students and confirm the factors found in the original measure. The dimensions have implications for developing empathy among medical students throughout the world. Educators can use the information to design interventions to foster empathy among students in the context of medical education reform in many countries, including China.

Abstract:Despite the necessary focus on clinical skills and knowledge during the tertiary education of healthcare professionals, the literature highlights the importance of developing psycho-social competencies. Empathy, a cognitive-behavioral attribute linked to various benefits for patients and healthcare professionals, is one such competency. Pedagogical approaches to successfully develop empathy in tertiary healthcare students are available. However, these approaches are often integrated piecemeal throughout the tertiary education journey. Research on a more empathy-focused curriculum is scarce. This manuscript describes the design of a study that aims to examine the effects of a more empathy-focused curriculum on empathy in tertiary healthcare profession students in Singapore. Freshmen dentistry, medicine, nursing, and pharmacy students enrolled in a novel curriculum with a strong empathy focus will be recruited for the study and followed for the program's extent. Mixed-methods data collection at various time points will be conducted. Quantitative data will be collected on cognitive-behavioral empathy, intentions to provide empathic care, and engagement in courses of the curriculum. Qualitative data on perceptions of patient care and empathy in relation to relevant courses of the curriculum will be collected to provide context for quantitative findings. Developing Empathy Ability in Undergraduate Nursing Students

Abstract:To determine whether a training course will enhance empathic skills in undergraduate nursing students,86 students were selected by systematic sampling and randomly allocated to the intervention group(n=43) and the control group(n=43).Two groups took part in the regular study.The students in the intervention group received additional films and seminars in small groups to learn and develop communicative and empathic abilities.86 participants(80.2%women) completed the study.Data showed that the training was effective,especially for women: SPE_NS scores 107.42 at pre-intervention phase and 119.33 at the end of the academic course(P<0.001).Results for men were less clear(P>0.05).Men and women have different empathic traits,more women than men took advantage of the training course.Anyhow,our data show that specific training courses are effective for nursing students.

Abstract:Abstract Context Medical education is committed to teaching patient centred communication and empathy. However, quantitative research suggests empathy scores tend to decline as students progress through medical school. In qualitative terms, there is a need to better understand how students and tutors view the practice and teaching of clinical empathy and the phenomenon of empathic erosion. Methods Working within a constructivist paradigm, researchers thematically analysed the individual interview data from a purposive sample of 13 senior students and 9 tutors. Results The four major themes were as follows: (1) ‘the nature of empathy’, including the concept of the innate empathy that students already possess at the beginning of medical school; (2) ‘beyond the formal curriculum’ and the central importance of role modelling; (3) ‘the formal curriculum and the tick-box influence of assessments’; and (4) the ‘durability of empathy’, including ethical erosion and resilience. A garden model of empathy development is proposed — beginning with the innate seeds of empathy that students bring to medical school, the flowering of empathy is a fragile process, subject to both enablers and barriers in the formal, informal, and hidden curricula. Conclusion This study provides insights into empathic erosion in medical school, including the problems of negative role modelling and the limitations of an assessment system that rewards ‘tick-box’ representations of empathy, rather than true acts of compassion. It also identifies factors that should enable the flowering of empathy, such as new pedagogical approaches to resilience and a role for the arts and humanities.
A comprehensive model for optimizing empathy in person-centered care

Abstract:Objective: This study examined perspectives regarding the use of empathy within medicine and developed a model to demonstrate the components of empathy in a medical setting. Methods: Grounded theory guided the methodology and final theory formation. Participants included 21 medical professionals representing multiple specialty areas and employed in a teaching hospital, private practice, or clinical setting in Eastern Virginia. Processes for transcription analysis and coding preserved participant perspectives and contributed to a final model. Results: Participant interviews revealed a seven-tier model that displays the facilitative conditions and potential barriers that may impact the full expression of empathy within the medical setting. Interviews also delineated between levels of empathy and described the benefits of providing empathic care, all of which are included in the final model. Conclusion: This new model of empathy describes a complex and dynamic process and conceptualizes ideal conditions for empathic treatment. The model presents concepts that may be useful in medical education, and creates new directions for empathy research. Practice implications: Physicians can assess themselves along each level of the model and can use it to identify barriers as well as ensure optimal conditions for empathic treatment. This new conceptualization of empathy also has implications for medical training and directions for future research.
Application and evaluation of empathic experience-based learning in the pain medicine

Abstract:Objective:To implement and evaluate empathic experience-based learning in the pain medicine.Methods:From January 2020 to August 2021, forty-eight resident trainees and visiting scholars (hereinafter referred to as trainees) were recruited from the Department of Pain Medicine of Peking University People's Hospital. Using the method of random control table, the trainees were divided into experimental group and control group with 24 in each. The experimental group was trained with empathy experience-based learning method, the control group adopted traditional teaching method. Evaluation of learning outcomes was carried out in the form of theoretical examination and Interpersonal Reactivity Index-C (IRI-C), and the teaching satisfaction evaluation was collected. The score results were statistically analyzed by t-test. Results:The theoretical test scores of the trainees in the experimental group were higher than those in the control group [(95.1±3.4) vs. (89.0±4.1), P<0.001]. The scores of all dimensions of IRI-C of the trainees in the experimental group were higher than those in the control group [such as viewpoint selection (16.2±2.2) vs. (14.3±2.8), imagination (20.5±2.3) vs. (17.8±2.9), all P<0.05]. The teaching satisfaction evaluation scores of the trainees in the experimental group were higher than those in the control group [such as stimulating learning interest (94.8±4.1) vs. (87.0±5.0), clinical thinking capacity (93.3±3.6) vs. (83.1±5.3), all P<0.001]. Conclusions:The effect of empathic experience-based learning in the pain medicine is better than traditional teaching method, which improves trainees' mastery of theoretical knowledge and teaching satisfaction, and so supports the building of trainees' empathic capacity.

Perceptions of Empathy in the Classroom Among Educators in Higher Education and Healthcare Disciplines: A Qualitative Study

Abstract:Empathy is an important human response and contributes to effective interpersonal relationships. Empathy found in the classroom, specifically demonstrated by faculty in higher education, may impact student performance and outcomes. This qualitative study investigated how instructors (defined in this study as any faculty rank) in healthcare disciplines and higher education define and demonstrate empathy in the classroom. In addition, this study explored instructors' self-perceived empathy and factors that impact their empathy development. A purposive sample of instructors across four universities in undergraduate and graduate academic programs and from various healthcare professions, including nutrition and dietetics, nursing, physical therapy, and physician assistant programs, were invited to participate. Twenty-nine semi-structured virtual interviews were conducted, each lasting about 1 hour. Thematic analysis was completed using the interview data and NVivo v20 software. Three major themes were identified: 1) empathy is multidimensional, 2) empathy can be demonstrated in multiple ways, and 3) self-perceived levels of empathy evolve and are influenced by personal experiences. Subthemes were identified regarding how empathy was defined and demonstrated. Future research should include the development of a self-administered tool to measure self-perceived empathy in higher education and investigate empathy and student outcomes.
Supervised Topic Models for Video Activity Recognition

Abstract:Topic models successfully capture latent structure useful for unsupervised analysis of bag-of-words data. Applying these models to domains such as video activity recognition requires two critical extensions: (1) incorporating supervised information (activity labels) to recover topic structure with greater discriminative power and (2) moving beyond the bag-of-words assumption to model temporal dynamics. We propose two parallel investigations to accomplish these tasks. First, we will study generic supervision techniques for topic models, exposing shortcomings in previously published generative approaches and exploring new discriminative models based on Mixtures of Experts. Second, we will apply these supervised models to video activity classification on the challenging Hollywood2 and Olympic Sports datasets, and explore extensions that capture chronological structure inherent in real-world activities.

Abstract:Background Empathy is a fundamental component of all relationships between health care workers and patients, but very little is known about how best to teach empathy to neonatal intensive care unit (NICU) nurses—particularly with respect to their ability to recognize and respond empathically to parents. This study describes the development and implementation of simulation-based empathic communication training and the evaluation of training efficacy. Method A quasi-experimental, single-group design was used which was evaluated with a pretest and posttest. Thirty-two NICU nurses from a Chinese hospital participated in simulation-based training for empathic communication skills. Nurses completed an observer-reported assessment at both before and after training, as well as a survey on their self-reported attitude, confidence, and understanding of empathy. Results The majority of nurses (>90%) were satisfied with the training. Their self-reported attitude and confidence concerning their empathy skills as well as their understanding of empathic communication reflected significant improvement. The behaviors of nurses toward communicating empathetically improved significantly after undergoing the simulation training (p < .001). Conclusions The training was effective for improving the ability of NICU nurses to communicate empathically with parents.

Abstract:Empathy and compassion are vital components of health care quality; however, physicians frequently miss opportunities for empathy and compassion in patient care. Despite evidence that empathy and compassion training can be effective, the specific behaviors that should be taught remain unclear. We synthesized the biomedical literature on empathy and compassion training in medical education to find the specific curricula components (skills and behaviors) demonstrated to be effective.We searched CENTRAL, MEDLINE, EMBASE, and CINAHL using a previously published comprehensive search strategy. We screened reference lists of the articles meeting inclusion criteria to identify additional studies for potential inclusion. Study inclusion criteria were: (1) intervention arm in which subjects underwent an educational curriculum aimed at enhancing empathy and/or compassion; (2) clearly defined control arm in which subjects did not receive the curriculum; (3) curriculum was tested on physicians (or physicians-in-training); and (4) outcome measure assessing the effect of the curriculum on physician empathy and/or compassion. We performed a qualitative analysis to collate and tabulate effects of tested curricula according to recommended methodology from the Cochrane Handbook. We used the Cochrane Collaboration's tool for assessing risk of bias.Fifty-two studies (total n = 5,316) met inclusion criteria. Most (75%) studies found that the tested curricula improved physician empathy and/or compassion on at least one outcome measure. We identified the following key behaviors to be effective: (1) sitting (versus standing) during the interview; (2) detecting patients' non-verbal cues of emotion; (3) recognizing and responding to opportunities for compassion; (4) non-verbal communication of caring (e.g. eye contact); and (5) verbal statements of acknowledgement, validation, and support. These behaviors were found to improve patient perception of physician empathy and/or compassion.Evidence suggests that training can enhance physician empathy and compassion. Training curricula should incorporate the specific behaviors identified in this report.

Abstract:The "hidden curriculum" in medical school includes a stressful work environment, un‐empathic role models, and prioritisation of biomedical knowledge. It can provoke anxiety and cause medical students to adapt by becoming cynical, distanced and less empathic. Lower empathy, in turn, has been shown to harm patients as well as practitioners. Fortunately, evidence‐based interventions can counteract the empathy dampening effects of the hidden curriculum. These include early exposure to real patients, providing students with real‐world experiences, training role models, assessing empathy training, increasing the focus on the biopsychosocial model of disease, and enhanced wellbeing education. Here, we provide an overview of these interventions. Taken together, they can bring about an "empathic hidden curriculum" which can reverse the decline in medical student empathy.
