Abstract:BACKGROUND In controlled ovarian hyperstimulation protocols worldwide, depot gonadotropin-releasing hormone agonist (GnRH-a) pretreatment is generally used for pituitary desensitization. The delay between the GnRH-a administration and starting gonadotropin treatment varies greatly, from 25 to 60 days. However, the association between exposure days to GnRH-a before the onset of gonadotropin administration and the clinical outcomes remains unknown. MATERIAL AND METHODS This retrospective study included 7007 patients who underwent fresh embryo transfers between February 2016 and July 2019. The duration of pituitary downregulation was categorized into 3 groups: group 1, ≤30 days; group 2, 31-35 days; and group 3, ≥36 days. The rates of live birth were compared as the main outcome measure. Logistic regression analysis was also performed after controlling for a range of confounders. RESULTS The number of patients in groups 1, 2, and 3 was 2001, 2824, and 2182, respectively. Group 3 (≥36 days) had a noticeably higher live birth rate (48.1%) than the other 2 groups (42.6% and 43.9%, P=0.001). The rate of live birth was remarkably enhanced in group 3 (adjusted odds ratio: 1.264, 95% confidence interval: 1.098, 1.455, P=0.001) after controlling for confounders, while the difference was not found in group 2 (P=0.512) compared with group 1. CONCLUSIONS In the depot GnRH-a protocol, live birth rates are higher among patients needing a longer time to achieve the goal of pituitary downregulation.
[Gonadotropin-Releasing Hormone Antagonist Protocol in Patients with Risk of Poor Response to Ovarian Stimulation in IVF-ET].

Abstract:OBJECTIVE:To evaluate the application of gonadotrophin-releasing hormone antagonist (GnRH-ant) in patients with risk of poor response to controlled ovarian stimulation in IVF-ET.METHODS:Clinical data of 48 patients undergoing IVF with or without ICSI were retrospectively analyzed. Among them 24 patients were allocated to the GnRH-ant protocol and 24 to the long gonadotrophin-releasing hormone agonist (GnRH-a) protocol. The duration of down-regulation, duration of stimulation, amps of gonadotropin estradiol level on hCG day, number of oocytes retrieved, fertilization rate, total embryos obtained, high quality embryo obtained, embryos transferred, embryos frozen, implantation rate per transfer, clinical pregnancy rate per transfer, embryo survival rate, clinical pregnancy rate per frozen embryos transfer and per cycle were compared between two groups.RESULT:The duration of down-regulation, duration of stimulation, the amps of gonadotropin were significantly lower in the antagonist group than those in agonist group (P <0.001, <0.05, <0.05), the estradiol level on hCG day, the number of oocytes retrieved were significantly lower in the antagonist group than those in the agonist group (P<0.05, <0.05). No significant differences were noted in fertilization rate, total embryos obtained, high quality embryo obtained, embryos transferred, embryos frozen, implantation rate per transfer, clinical pregnancy rate per transfer, embryo survival rate, clinical pregnancy rate per frozen embryos transfer and per cycle.CONCLUSION:Compared with long GnRH-a protocol, the GnRH-ant protocol in patients with risk of poor response can reduce the dosage of gonadotropin and shorten the duration of stimulation, although the estradiol level on hCG day and the number of oocytes retrieved are lower, which does not affect the implantation rate and clinical pregnancy rate.

The live birth rate and clinical pregnancy rate were compared between the two groups. Other outcome measures included the implantation rate, miscarriage rate, and characteristics of stimulation procedures. Results: Live birth rates and clinical pregnancy rates were significantly higher in the GnRHa prolonged protocol group than in the other group (36.5% vs 20.8%, P=0.002; 43.9% vs 28.2%, P=0.006, respectively). The live birth rate was significantly increased in the prolonged protocol group (crude OR: 2.190, 95% CI: 1.311, 3.660; adjusted OR: 2.458, 95% CI: 1.430, 4.224) compared with that in the reference group. The implantation rate of the former group was also significantly higher than that of the latter group (35.4% vs 15.9%, P=0.000). There was no significant difference in miscarriage rates between the two protocols. In terms of stimulation procedures, the GnRH-a prolonged protocol group required significantly higher Gn time (10.9 vs 9.5 days, P=0.000) and Gn consumption (2625.0 vs 2047.5 IU, P=0.000) than the short GnRH-a long protocol group. Conclusion: The GnRH-a prolonged protocol in fresh embryo transfer cycles yielded better clinical outcomes of patients with thin endometrium than the short GnRH-a long protocol.

Effects of Gonadotropin-Releasing Hormone Antagonists on the Expression of Vascular Endothelial Growth Factor and Its Receptors in a Rat Model of Ovarian Hyperstimulation Syndrome.

Abstract:BACKGROUND:Ovarian hyperstimulation syndrome (OHSS) is one of the most life-threatening complications of assisted reproduction treatments. Gonadotropin-releasing hormone antagonists (GnRHanta) are thought to be effective in preventing this complication, and some clinical trials have found lower incidences of OHSS in patients treated with GnRHanta. Our aim was to investigate the effects of GnRHanta on vascular permeability and the expression of vascular endothelial growth factor (VEGF) and its receptors in a rat model of OHSS.METHODS:An immature early OHSS rat model was established. Three ovarian stimulation protocols were used: pregnant mare serum gonadotropin/human chorionic gonadotropin (hCG) alone, with a GnRHanta, or with a gonadotropin-releasing hormone agonists (GnRHa). Blood and tissue samples were collected at 48 hours after hCG administration. Vascular permeability was evaluated by measuring the Evans-Blue content of extravasated peritoneal fluids. The expression of VEGF and its receptors, including flt-1 and KDR, were detected by reverse transcriptase-polymerase chain reaction and Western blotting.RESULTS:Treatment with both a GnRHanta and a GnRHa resulted in significant reductions in serum estradiol and peritoneal vascular permeability, as well as decreased ovarian expression of VEGF and its two receptors. However, GnRHanta treatment caused a greater reduction in serum estradiol concentrations, and in VEGF receptor mRNA expression than GnRHa. There were no significant reductions in the expression of VEGF or its receptors in extra-ovarian tissues, including the liver, lungs and peritoneum.CONCLUSION:Our results reveal that GnRHanta are more potent than GnRHa in preventing early OHSS through down-regulation of the expression of VEGF and its receptors in hyperstimulated ovaries.

Abstract:【Objective】 To compare the pregnant rate and safety of gonadotropin releasing hormone agonist(GnRHa) and human chorionic gonadotropin(HCG) in ovulation induction cycles.【Methods】 100 patients who received ovulation induction and sexing intervention / Intrauterine insemination / artificial insemination by donor were retrospectively analyzed.According to different drugs for trigger used in dominant follicle≥18 mm,100 patients were divided into 2 groups: Group A injected GnRHa 0.1 mg with a total of 46 cycles;Group B injected HCG 6000 IU with a total of 54 cycles.serum LH,E2,and P in the trigger day,ovulation rate,pregnant rate,mutiplets rate,abortion rate,ovarian hyperstimulation syndrome(OHSS) rate,etc.were compared.【Results】 Group A had different higher E2 level,more follicles and more ovulation number in the trigger day than Group B.However,there were similar pregnancy rate,mutiplets rate,abortion rate and OHSS rate between two groups.【Conclusions】 For the patients with 3～5 dominant follicles,it was possible to use GnRHa rather than HCG in the trigger day to ensure the effectiveness and security throughout the therapy.
A Half-Zippered SNARE Complex Represents a Functional Intermediate in Membrane Fusion

Abstract:SNARE (soluble N-ethylmaleimide-sensitive factor attachment protein receptor) proteins mediate fusion by pulling biological membranes together via a zippering mechanism. Recent biophysical studies have shown that t- and v-SNAREs can assemble in multiple stages from the N-termini toward the C-termini. Here we show that functionally, membrane fusion requires a sequential, two-step folding pathway and assign specific and distinct functions for each step. First, the N-terminal domain (NTD) of the v-SNARE docks to the t-SNARE, which leads to a conformational rearrangement into an activated half-zippered SNARE complex. This partially assembled SNARE complex locks the C-terminal (CTD) portion of the t-SNARE into the same structure as in the postfusion 4-helix bundle, thereby creating the binding site for the CTD of the v-SNARE and enabling fusion. Then zippering of the remaining CTD, the membrane-proximal linker (LD), and transmembrane (TMD) domains is required and sufficient to trigger fusion. This intrinsic property of the SNAREs fits well with the action of physiologically vital regulators such as complexin. We also report that NTD assembly is the rate-limiting step. Our findings provide a refined framework for delineating the molecular mechanism of SNARE-mediated membrane fusion and action of regulatory proteins.

Novel method is based on calculation of nucleotide occurrence probability in fixed size moving window and determination of high order central moments. Description of nucleotide occurrence probability can provide high accuracy point mutation detection, and the insight into local sequence.
Effect of thymic humoral factor on a local graft-versus-host reaction of lymphocytes in patients with lipoid nephrosis.

Abstract:Sir, The etiology of lipoid nephrosis (LN) is poorly understood. We have recently shown that patients with LN have a systemic disorder of cell-mediated immunity, characterized by decreased T cell numbers, depressed delayed hypersensitivity reactions and impaired local graft-versus-host reaction (GVHR) [3, 4]. Our previous study [5] also illustrated an increase in concanavalin-A-induced suppressor cell activity in the majority of LN patients in relapse. Our present study is designed to compare the effect of thymic humoral factor (THF) on local GVHR in 16 patients with renal disease and 17 normal individuals. In all patients, the diagnosis was made by renal biopsy. All LN patients with nephrotic syndrome (NS) were receiving no medication at the time of the study. Donor peripheral blood lymphocytes (PBL) were isolated by separation on Ficoll-Hypaque gradients. The THF was prepared from young calf thymus according to the method described by Kook et al. [2]. The effect of THF on the patients’ PBL was studied by preincubating 10 × 106 PBL with 100 µg of THF for 1 h at 37 °C. Control experiments as above using normal PBL and THF treatment were also performed. A suspension of 10 × 106 PBL with or without THF treatment in a volume of 0.1 ml RPMI 1640 medium was injected intradermally into the closely shaven abdominal skin of Lewis rats immunosuppressed previously with 100 mg/kg cytoxan according to the method of Shohat and Joshua
Short-term endocrine response to gonadotropin-releasing hormone agonist initiated in the early follicular, midluteal, or late luteal phase in normally cycling women

Abstract:Objective: To determine short-term pituitary and ovarian hormonal responses to GnRH agonist (GnRH-a) administered during various phases of the menstrual cycle, in the absence of controlled ovarian hyperstimulation (COH), to determine its independent effect on hormonal parameters previously demonstrated to influence assisted reproductive technology cycle outcome. Design: Prospective, randomized, controlled crossover study of five regularly cycling women. The GnRH-a, leuprolide acetate (LA), was administered 1 mg SC daily for 5 days beginning on cycle day 3 (early follicular); 8 days post-LH surge (midluteal); or 13 days post-LH surge (late-luteal). Setting: Clinical research unit at a tertiary care medical center. Main outcome measures: Serum gonadotropins (LH and FSH) and gonadal steroids (E2, estrone [E1], P, A, and T) measured daily during GnRH-a administration begun in the early follicular, midluteal, or late luteal phase of the menstrual cycle. Gonadotropin pulse amplitude and frequency were determined after frequent serum sampling on the 2nd day of GnRH-a administration in each treatment cycle. Results: Serum LH elevations, 4- to 10-fold greater than observed for FSH, did not differ by cycle day of GnRH-a initiation. Initial increases in FSH did not differ by cycle day, however, early follicular initiation resulted in a more pronounced suppression of FSH. Mean LH pulse amplitude and frequency increased to a similar extent in all three groups, however, FSH pulse amplitude and frequency varied significantly by cycle day of GnRH-a initiation. Gonadotropin-releasing hormone agonist initiated in the early follicular phase resulted in significant increases in E2, E1, and P levels compared with both midluteal or late luteal. Increases in serum androgens were significantly greater after early follicular and late luteal initiation as compared with midluteal GnRH-a initiation. Conclusions: Relative FSH suppression and marked androgen elevations in both late luteal and early follicular groups, which may have potential detrimental effects on oocytes of the developing cohort, suggest little advantage of late luteal or early follicular over midluteal initiation of GnRH-a for COH.
The Effectiveness of Gonadotrophin Releasing Hormone Agonist on Infertile Patients with Ovulation Disorder and Poor Response

Abstract:Objective To study the effectiveness of goriadotrophin releasing hormone agonist (CnRH - a) on patients with ovulation disorder and poor response to clomiphene citrate (CC) or human menopausal gonadotropin (HMG) . Methods 13 cases with ovulation disorder and poor response to CC or HMG were enrolled, including 5 cases of polycystic ovary syndrome (PCOS) and 8 cases of small follicle ovulation. All cases were treated with GnRH - a + HMG. Transvaginal ultrasonography was taken to monitor the growth of follicle on the day 8th of the cycle. When the average diameter of dominant follicle arrived 15 mm urine LH assay was tested every day. When the average diameter arrived 18 mm or urine LH ( + ), HCG was given to induce ovulation. Results Dominant follicles developed in all 19 treatment cycles of GnRH - a + HMG. Average diameter of the dominant follicle arrived 18mm and urine LH were still negative in 16 cycles (84.2% ), premature luteinization happened in other 3 cycles so HCG was stopped because of small follicle. Monofollicle growth happened in 36.8% cycles, less than 3 dominant follicles in 75% cycles, 4 to 10 in 8.3% cycles, more than 10 in 18.8% cycles. Pregnancy rates was 58.3% , while pregnancy rate per cycle was 41.2%. 4 cases were singletons, 2 cases twins and 1 case quadruplets. Miscarriage rate was 14.3% . Conclusions GnRH - a could enhance the ovarian response to HMG in patients with PCOS and prevent premature luteinizan'on, facilitate follicu-lar development and give higher pregnancy rate and satisfactory pregnancy outcome. GnRH - a may be an efficient therapy in patients with PCOS and small follicular ovulation poor response to CC or HMG. After low doses HMG less than 3 follicles will be found in 75% cycles.
Diagnosis and Assisted Reproductive Treatment of 19 Cases of Hypothalamic Amenorrhea

Abstract:Objective: To explore clinical features and assisted reproductive treatment methods of hypothalamic amenorrhea (HA). Methods: A retrospective study for clinical features of HA was conducted on 19 women wth HA(HA group). The clinical parameters(serum FSH, LH, E2, PRL, T, uterus volume, ovarian volume and overall basic antral follicles) was compared between them and 19 normal controls with regular menstruation (the control). All patients were treated with hormone replacement. Ovulation induction with hMG was conducted on 11 HA patients who desired fertility. Results: Clinical characteristics of HA included primary or secondary amenorrhea, no or partial or incomplete sexual development, no hot flashes, normal karyotype. Serum FSH, LH and PRL levels in HA patients were significantly lower than those of the control (P<0.01). Serum LH and FSH levels were lownormal. There were no differences in serum E2 and T levels between the two groups. Mean uterine and ovarian volume and the number of overall basic antral follicles were significantly smaller or less in HA group as compared with the control (P<0.01). Uterus became larger after hormone replace treatment. Ovulation happened when ovulation induction with hMG in 11 HA patients and 7 patients got clinical pregnant. Conclusion: Hormone replacement and ovulation induction with hMG are effective treatments for HA patients who desire fertility. The principles are low initial dose, step up and step down gradually, which is the key to prevent ovarian hyperstimulation syndrome and multiple pregnant. If there are too many developed follicles, the rescue methods are to puncture the surplus follicles or switch to IVF, which can result in high success rate.

Abstract:Objective To investigate the effectiveness and safety of low dose human chorionic gonadotropin (hCG) dual trigger, by comparing thein vitrofertilization/intracytoplasmic sperm injection (IVF/ICSI) outcomes of high responders using low dose hCG dual trigger or standard hCG trigger. Methods This was a retrospective cohort study. The patients accepted IVF/ICSI treatment in Reproductive Medical Center of Peking University, between Jan. 1st 2014 and Dec. 31st 2015, with gonadotropin-releasing hormone antagonist (GnRH-A) flexible protocol. The research group was high responders using low dose hCG dual trigger (group A,n=431), paired by age and number of oocytes retrieved (1∶2), over the same period (group B,n=862). The general characteristics and treatment outcomes were compared between the two groups. Results There were no significant differences between the two groups refer to general characteristics, and gonadotropin (Gn) used dosage (P>0.05). The serum E2level on hCG trigger day in group A [(16 261±6 561pmol/L] was significantly higher than that in group B [(12 795±6 133)pmol/L, P=0.000]. The fertilization rate of ICSI and MIIoocyte rate were comparable between two groups (P>0.05). The rate of two pronucleus (2PN) oocytes was significantly higher in group A (56.0%) than that in group B (54.7%) (P=0.001). The clinical pregnancy rate, the implantation rate, the miscarriage rate and live birth rate were comparable between two groups (P>0.05). There were 5 patients in group A who suffered severe ovarian hyperstimulation syndrome (OHSS). Conclusion For high responders, besides of GnRH antagonist protocol, using low dose hCG dual trigger, will get satisfied clinical outcomes and do not increase the incidence of OHSS, and get satisfied clinical outcomes in fresh cycle. Abstract:OBJECTIVE:To compare the outcomes achieved by GnRH-antagonist and GnRH-agonist in IVF-ET for patients with poor ovarian responses, and to find out a better protocol for ovulation stimulation. METHODS:(1) Patients with poor ovarian responses were assigned to an experimental (n = 63) and a control group (n = 58), treated respectively with GnRH-Ant and oral contraceptive plus micro-dose GnRH-a (OC + GnRH-a), and comparisons were made of the medication doses, laboratory results and pregnancy outcomes between the two groups. (2) Twenty of the patients were treated first with GnRH-Ant and then with OC + GnRH-a, and the same comparisons were made between the two protocols. RESULTS:Between the experimental and the control groups, there were no significant differences in the dose of Gn and number of retrieved oocytes and transplanted embryos (P > 0.05), nor in the pregnancy rate of transplantation cycles (37.29% vs 35.29%). The cycle cancellation rate was lower in the experimental than in the control group (6.35% vs 12.07%), with no statistical difference (P > 0.05). The cycle duration was significantly different between the two groups ([9.65 +/- 1.60] d vs [19.05 +/- 3.94] d) (P < 0.05). As for the comparison of GnRH-Ant with GnRH-a, no significant differences were observed in the dose of Gn and the numbers of retrieved oocytes and transplanted embryos (P > 0.05). GnRH-Ant achieved a higher pregnancy rate of transplantation cycles but a significantly lower cancellation rate than GnRH-a (38.09% vs 17.64% and 0% vs 15%) (P > 0.05 and P < 0.01), respectively. The cycle duration of the former was statistically shorter than that of the latter ([9.91 +/- 2.49] d vs [27.74 +/- 25.39] d) (P < 0.05). CONCLUSION:Compared with micro-dose GnRH-a, GnRH-Ant can shorten the cycle duration and reduce the cancellation rate in IVF-ET for patients with poor response. And for those who have failed to respond to GnRH-a, GnRH-Ant may be tried in another attempt at IVF-ET.
Effect of GnRH agonist alone or combined with different low-dose hCG on cumulative live birth rate for high responders in GnRH antagonist cycles: a retrospective study

Abstract:Background: There is insufficient evidence regarding the impact of dual trigger on oocyte maturity and reproductive outcomes in high responders. Thus, we aimed to explore the effect of gonadotropin-releasing hormone agonist (GnRHa) trigger alone or combined with different low-dose human chorionic gonadotropin (hCG) regimens on rates of oocyte maturation and cumulative live birth in high responders who underwent a freeze-all strategy in GnRH antagonist cycles. Methods: A total of 1343 cycles were divided into three groups according to different trigger protocols: group A received GnRHa 0.2 mg (n = 577), group B received GnRHa 0.2 mg and hCG 1000 IU (n = 403), and group C received GnRHa 0.2 mg and hCG 2000 IU (n = 363). Results: There were no significant differences in age, body mass index, and rates of oocyte maturation, fertilization, available embryo, and top-quality embryo among the groups. However, the incidence of moderate to severe ovarian hyperstimulation syndrome (OHSS) was significantly different among the three groups (0% in group A, 1.49% in group B, and 1.38% in group C). For the first frozen embryo transfer (FET) cycle, there were no significant differences in the number of transferred embryos and rates of implantation, clinical pregnancy, live birth, and early miscarriage among the three groups. Additionally, the cumulative ongoing pregnancy rate and cumulative live birth rate were not significantly different among the three groups. Similarly, there were no significant differences in gestational age, birth weight, birth height, and the proportion of low birth weight among subgroups stratified by singleton or twin. Conclusions: GnRHa trigger combined with low-dose hCG (1000 IU or 2000 IU) did not improve oocyte maturity and embryo quality and was still associated with an increased risk of moderate to severe OHSS. Therefore, for high responders treated with the freeze-all strategy, the single GnRHa trigger is recommended for final oocyte maturation, which can prevent the occurrence of moderate to severe OHSS and obtain satisfactory pregnancy and neonatal outcomes in subsequent FET cycles.

Abstract:Background and Aims: Gonadotropin releasing hormone agonist (GnRH-a) trigger has been shown to be associated with significantly lower risk of moderate or severe ovarian hyperstimulation syndrome (OHSS) when compared among women at high risk of OHSS. However, trigger failure has been shown to occur in 2% of cycles using GnRH-a as trigger. This study aims to review the use of GnRH-a as ovulation trigger. Method: Retrospective review of records in antagonist cycles from an ART unit in a tertiary hospital from 2017-2021 was performed. Follicles were considered mature when they reach [Formula: see text] 15 mm. Ovulation triggered with urinary human chorionic gonadotropin (uhCG) (5000-10000 IU), recombinant hCG (rhCG) 250 mcg, or triptorelin 0.2 mg when there were [Formula: see text] 3 mature follicles. All embryos obtained from cycles using triptorelin were cryopreserved. Outcome including the number of oocytes retrieved, the follicle to oocyte index, ovarian hyperstimulation syndrome were reviewed. Results: 1378 cycles were included. 1089 cycles had uhCG or rhCG as trigger, while 284 cycles had triptorelin as trigger. Women in the triptorelin group were significantly younger with a lower basal follicle stimulating hormone level and a higher antral follicle count (Table 1). In the triptorelin group, the total number of mature sized follicles, estrogen level at trigger, oocytes retrieved, number of blastocysts were significantly higher compared to the hCG group (Table 2). Follicle to oocyte index was significantly higher in the triptorelin group but the proportion of mature oocytes were similar in both groups. There were no cases of trigger failure and OHSS in the triptorelin group, but 30 cases of OHSS, with18 cases early onset, 12 cases late onset, amongst them 10 cases (1%) were severe OHSS) in the hCG group. Conclusion: GnRH-a trigger is a good candidate as ovulatory trigger in women at risk of OHSS without hampering the laboratory outcome.

Abstract:This paper presents an approach to the detection of changed buildings using multitemporal Very High Resolution (VHR) Synthetic Aperture Radar (SAR) images. The proposed approach is based on two concepts i) the extraction of information on changes associated with increase and decrease of backscattering at the optimal building scale; and ii) the exploitation of the expected backscattering proprieties of buildings to detect new and fully destroyed buildings with their grade of reliability. Experimental results obtained on a dataset made up of two COSMO-SkyMed (CSK©) spotlight images acquired in 2009 over the city of L'Aquila (Italy) before and after an earthquake demonstrated that the proposed approach allows an effective identification of destroyed buildings.
