Abstract:BACKGROUND Vision impairments (VI) and blindness are one of the core global public health issues. Visual acuity (VA) is one of the most crucial standard psychophysical test of visual function, and used widely in a broad range of healthcare domain, especially in many clinical settings. OBJECTIVE To assess the accuracy and application of using Mobile devices-based visual acuity measurement apps. METHODS We searched Pubmed, Embase, Cochrane Library, and Google Scholar for relevant articles published between January 1, 2008 and July 1 2020. Two reviewers independently selected studies that assessed the mobile-based VA measurement apps. We included all studies that assessed a tablet and/or smartphone VA measurement apps. RESULTS Most of the enrolled 22 studies considered as high quality studies, evaluating by QUADAS-2. In meta-analysis, six studies involving 24284 participants were included. In 3~5 years old group, the pooled sensitivity was 0.87 (95% CI 0.79, 0.93); the pooled specificity was 0.78 (95% CI 0.70, 0.85); In 6~22 years old group, the pooled sensitivity was 0.86 (95% CI 0.84, 0.87); the pooled specificity was 0.91 (95% CI 0.90, 0.91). In ≥55 years old group, the pooled sensitivity was 0.85 (95% CI 0.55, 0.98); the pooled specificity was 0.98 (95% CI 0.95, 0.99). CONCLUSIONS In this study, we conducted a comprehensive review of the state-of-the-art research to investigate the diagnostic value and limitations of existing mobile-based VA test applications. Evidence from this study shows that mobile-based app VA measurements may be useful tools for VI detection.

Abstract:Vision processing is one of the important function human brain performs utilizing its one third of resources. Any injury or damage in the skull housing brain results in various visual impairments that can vary from severe impairment to only cognitive visual problems. Individuals with visual impairments (VI) may have intact perception of their movements. The progression of such medical conditions of patients can be identified, defined and tracked using medical assessments tools. Any development of such a functioning and cost effective assessment tool assist the medical practitioner to access the history of patient resulting in successful diagnosis and treatment, continuous observation and experimentation. In this work a new tool is developed for the medical rehabilitation and treatment of the visually impaired patients–especially young children–and provide a safety measure by recommending preventive and corrective actions from further deteriorating the impairment. It provides a measure of a stable life for the individuals by assessing them for defining characteristics of visual and/or cognitive impairments. Seven modules are designed in this research, namely color detection, object size detection, static detection, linear detection, motion detection, vision detection for static objects and vision detection for mobile objects. Each module was tested on three groups of subjects. The results from this tool are verified by ophthalmologists and neurologists. The system can identify different states of multiple subjects. This assessment tool is easily available, reliable and very cheap.
[A case of malignant paraganglioma arising from the retroperitoneum].

Abstract:We report the case of 38-year-old male with a malignant and functional paraganglioma arising from the retroperitoneum. He was admitted to our hospital with a pathological fracture of the right femur, A diagnosis of paraganglioma was made after taking a biopsy specimen and discovering a hypervascular retroperitoneal tumor. The level of catecholamine in his urine was greatly increased, accompanied with hypertension, rapid pulse and sweating. He was treated with chemotherapy, radiotherapy and surgical therapy. However, he died from pulmonary metastasis.

Abstract:With the advances in smartphone and tablet screens, as well as their processing power and software, mobile apps have been developed reporting to assess visual function. This review assessed those mobile apps that have been evaluated in the scientific literature to measure visual acuity, reading metrics, contrast sensitivity, stereoacuity, colour vision and visual fields; these constitute just a small percentage of the total number of mobile apps reporting to measure these metrics available for tablets and smartphones. In general, research suggests that most of the mobile apps evaluated can accurately mimic most traditionally paper-based tests of visual function, benefitting from more even illumination from the backlit screen and aspects such as multiple tests and versions (to minimise memorisation) being available on the same equipment. Some also utilise the in-built device sensors to monitor aspects such as working distance and screen tilt. As the consequences of incorrectly recording visual function and using this to inform clinical management are serious, clinicians must check on the validity of a mobile app before adopting it as part of clinical practice.

An Artificial-Intelligence- and Telemedicine-Based Screening Tool to Identify Glaucoma Suspects from Color Fundus Imaging

Abstract:Background and Objective. Glaucomatous vision loss may be preceded by an enlargement of the cup-to-disc ratio (CDR). We propose to develop and validate an artificial-intelligence-based CDR grading system that may aid in effective glaucoma-suspect screening. Design, Setting, and Participants. 1546 disc-centered fundus images were selected, including all 457 images from the Retinal Image Database for Optic Nerve Evaluation dataset, and images were randomly selected from the Age-Related Eye Disease Study and Singapore Malay Eye Study to develop the system. First, a proprietary semiautomated software was used by an expert grader to quantify vertical CDR. Then, using CDR below 0.5 (nonsuspect) and CDR above 0.5 (glaucoma suspect), deep-learning architectures were used to train and test a binary classifier system. Measurements. The binary classifier, with glaucoma suspect as positive, is measured using sensitivity, specificity, accuracy, and AUC. Results. The system achieved an accuracy of 89.67% (sensitivity, 83.33%; specificity, 93.89%; and AUC, 0.93). For external validation, the Retinal Fundus Image Database for Glaucoma Analysis dataset, which has 638 gradable quality images, was used. Here, the model achieved an accuracy of 83.54% (sensitivity, 80.11%; specificity, 84.96%; and AUC, 0.85). Conclusions. Having demonstrated an accurate and fully automated glaucoma-suspect screening system that can be deployed on telemedicine platforms, we plan prospective trials to determine the feasibility of the system in primary-care settings.

Using the C-Read As a Portable Device to Evaluate Reading Ability in Young Chinese Adults: an Observational Study

Abstract:We evaluated the reading characteristics of normal-sighted young adults using C-Read to provide baseline healthy population values. We also investigated the relationship between the National Eye Institute’s Visual Functioning Questionnaire (VFQ-25) score and reading ability, myopia, and hours of screen use, focusing on the extent to which these factors affect participants’ visual function and, ultimately, their vision-related quality of life (QoL). Overall, 207 young, healthy participants (414 eyes) aged 18–35 years were tested for reading speed using C-Read connected to a smartphone-based application between December 2022 and January 2023. Each participant received a VFQ-25 questionnaire to evaluate vision-related QoL. Data on daily e-screen usage hours were collected. Among the participants, 91 (44.0%) were women; their mean (SD) age was 22.45 (4.01) years. The mean (SD) reading acuity (RA) was 0.242 (0.124), 0.249 (0.120), and 0.193 (0.104) logarithmic minimum angle of resolution (logMAR) for the right, left, and both eyes, respectively. The mean (SD) maximum reading speed (MRS) was 171.65 (46.27), 168.59 (45.68), and 185.16 (44.93) words per minute (wpm) with the right, left, and both eyes, respectively. The mean (SD) critical print size (CPS) was 0.412 (0.647), 0.371 (0.229), and 0.419 (1.05) logMAR per the right, left, and both eyes, respectively. The RA and CPS were significantly different between sexes (p = 0.002 and p = 0.001). MRS was significantly different between the education level (p = 0.005) and myopia level groups (p = 0.010); however, it was not clear whether this difference was confounded by age. The myopic power in diopters significantly affected RA (coefficient, −0.012; 95% CI, −0.018 to −0.006; p = 0.001); screen time significantly affected MRS (coefficient, 0.019; 95% CI, 0.57 to 6.33; p = 0.019). RA (coefficient, −21.41; 95% CI, −33.74 to −9.08; p = 0.001) and duration of screen use (coefficient, -0.86; 95% CI, −1.29 to −0.43; p < 0.001) independently had a significantly negative correlation with VFQ-25 scores. Our findings provide a baseline value for C-Read in normal-sighted young adults. Refractive status significantly affected RA, while screen time significantly affected MRS. Interventions aimed at enhancing RA may have the potential to maximize vision-related QoL and enable older adults with impaired vision to achieve greater outcomes. Future, larger-scale, C-Read experiments will help provide newer, more optimal methods for the early diagnosis of visual impairment.

Abstract:SIGNIFICANCE A new device attached to a smartphone was created for objective vision screening of young children including infants and newborns. The device is compact, lightweight, portable, cost-effective, and easy to operate. Therefore, it is suitable for screening large numbers of children in clinical settings, schools, and communities. PURPOSE This article introduces a new device attached to a smartphone for objective vision screening. It can detect and categorize significant refractive errors, anisometropia, strabismus, cloudy ocular media, and ptosis that may cause amblyopia. METHODS The new device applies the same principles as conventional streak retinoscopy but examines both eyes simultaneously and records the results electronically. The device comprises optical elements that produce a precise streak light beam and move it across a child's both eyes. The smartphone's video camera catches and records the motion of retinal reflex inside the child's pupils. By observing the direction of motion of the retinal reflex relative to the light beam motion, as well as its speed, width, and brightness, the examiner is able to assess the individual and comparative refractive status, ocular alignment, and other conditions. RESULTS Vision screening with this device does not require any subjective response from children. The examination can be performed and analyzed by nonprofessionals after a short learning period of time. Because the examination results are electronically recorded by the smartphone, they can be stored in the child's files and sent out for professional consultations. CONCLUSIONS The new device will provide the same functions as conventional streak retinoscopy but examines a child's both eyes simultaneously, so that, in addition to categorizing refractive errors and assessing clarity of refractive media of the eyes, it can also detect anisometropia, strabismus, and anisocoria. In addition to showing the examination results on the smartphone's screen, the device can also store the results electronically.
Development and validation of an artificial intelligence based screening tool for detection of retinopathy of prematurity in a South Indian population

Abstract:Purpose: The primary objective of this study was to develop and validate an AI algorithm as a screening tool for the detection of retinopathy of prematurity (ROP). Participants: Images were collected from infants enrolled in the KIDROP tele-ROP screening program. Methods: We developed a deep learning (DL) algorithm with 227,326 wide-field images from multiple camera systems obtained from the KIDROP tele-ROP screening program in India over an 11-year period. 37,477 temporal retina images were utilized with the dataset split into train ( n = 25,982, 69.33%), validation ( n = 4,006, 10.69%), and an independent test set ( n = 7,489, 19.98%). The algorithm consists of a binary classifier that distinguishes between the presence of ROP (Stages 1–3) and the absence of ROP. The image labels were retrieved from the daily registers of the tele-ROP program. They consist of per-eye diagnoses provided by trained ROP graders based on all images captured during the screening session. Infants requiring treatment and a proportion of those not requiring urgent referral had an additional confirmatory diagnosis from an ROP specialist. Results: Of the 7,489 temporal images analyzed in the test set, 2,249 (30.0%) images showed the presence of ROP. The sensitivity and specificity to detect ROP was 91.46% (95% CI: 90.23%–92.59%) and 91.22% (95% CI: 90.42%–91.97%), respectively, while the positive predictive value (PPV) was 81.72% (95% CI: 80.37%–83.00%), negative predictive value (NPV) was 96.14% (95% CI: 95.60%–96.61%) and the AUROC was 0.970. Conclusion: The novel ROP screening algorithm demonstrated high sensitivity and specificity in detecting the presence of ROP. A prospective clinical validation in a real-world tele-ROP platform is under consideration. It has the potential to lower the number of screening sessions required to be conducted by a specialist for a high-risk preterm infant thus significantly improving workflow efficiency.

Abstract:Background: Inflammatory bowel diseases (IBD) are characterized by periods of remission with recurrent episodes of symptom exacerbation because of acute intestinal inflammation, which is correctly evaluated by endoscopy with biopsy sampling. However, many surrogate markers of intestinal inflammation, including fecal calprotectin (FC), are detected as potential predictors of mucosal inflammation in IBD patients. The aim of our study was to retrospectively assess the clinical efficacy of the calprotectin assay in determining histological relapses of pediatric IBD patients. Methods: We retrospectively reviewed the histological examinations, clinical records, and FC values of patients who had undergone colonoscopy at our hospital over an 8‐year period, from December 31, 1998, to December 31, 2006. Only patients with a first histological examination showing a quiescent IBD who submitted to a second histological examination during the next 3 years were selected. Results: Seventy‐three IBD patients, all with a first biopsy showing a quiescent IBD, were studied; at the second histological examination, 32 presented with relapse and 41 presented with remission. Relapsed patients showed significantly increased FC levels compared with nonrelapsed patients. A FC value of 275 &mgr;g/g achieved sensitivity and negative predictive value of 97% and specificity and positive predictive value of 85% in predicting histological relapse. Conclusions: FC seems to be a direct measure of intestinal inflammation and therefore a good marker of the risk of histological relapse in pediatric IBD patients. The application of this test in clinical practice may enable the avoidance of invasive tests as well as targeting treatment.

In recent years, there has been increasing research on mobile app-based VA assessment designed to allow people to test their VA at any time and any location. Objective: The goal of the review was to assess the accuracy and reliability of using mobile VA measurement apps. Methods: We searched PubMed, Embase, Cochrane Library, and Google Scholar for relevant articles on mobile apps for VA assessment published between January 1, 2008, and July 1, 2020. Two researchers independently inspected and selected relevant studies. Eventually, we included 22 studies that assessed tablet or smartphone apps for VA measurement. We then analyzed sensitivity, specificity, and accuracy in the 6 papers we found through a meta-analysis. Results: Most of the 22 selected studies can be considered of high quality based on the Quality Assessment of Diagnostic Accuracy Studies-2. In a meta-analysis of 6 studies involving 24,284 participants, we categorized the studies based on the age groups of the study participants (ie, aged 3-5 years, aged 6-22 years, and aged 55 years and older), examiner (ie, professional and nonprofessional examiners), and the type of mobile devices (ie, smartphone, iPad). In the group aged 3 to 5 years, the pooled sensitivity for VA app tests versus clinical VA tests was 0.87 (95% CI 0.79-0.93; P=.39), and the pooled specificity was 0.78 (95% CI 0.70-0.85; P=.37). In the group aged 6 to 22 years, the pooled sensitivity for VA app tests versus clinical VA tests was 0.86 (95% CI 0.84-0.87; P<.001), and the pooled specificity for VA app tests versus clinical VA tests was 0.91 (95% CI 0.90-0.91; P=.27). In the group aged 55 years and older, the pooled sensitivity for VA app tests versus clinical VA tests was 0.85 (95% CI 0.55-0.98), and the pooled specificity for VA app tests versus clinical VA tests was 0.98 (95% CI 0.95-0.99). We found that the nonprofessional examiner group (AUC 0.93) had higher accuracy than the professional examiner group (AUC 0.87). In the iPad-based group, the pooled sensitivity for VA app tests versus clinical VA tests was 0.86, and the pooled specificity was 0.79. In the smartphone-based group, the pooled sensitivity for VA app tests versus clinical VA tests was 0.86 (P<.001), and the pooled specificity for VA app tests versus clinical VA tests was 0.91 (P<.001). Conclusions: In this study, we conducted a comprehensive review of the research on existing mobile apps for VA tests to investigate their diagnostic value and limitations. Evidence gained from this study suggests that mobile app-based VA tests can be useful for on-demand VI detection.
Accuracy of a commonly used mobile ophthalmology application's vision assessment tools in measuring five vision assessment parameters

Abstract:Background/objectives: The use of mobile ophthalmology applications (MOA) is increasing, but many of these tools have not been validated. This study was performed to assess the accuracy of a popular MOA, Eye Handbook, in measuring five commonly-tested vision assessment parameters (distance visual acuity (DVA), near visual acuity (NVA), colour vision testing (CVT), contrast sensitivity (CS), and pupillary distance (PD)) was compared with traditional vision assessment methods (TVAM) [i.e. Snellen chart, Rosenbaum near card, Ishihara, Pelli Robson test, etc.] performed in the eye clinic setting. Subjects/methods: Prospective crossover clinical trial of 129 patients meeting inclusion criteria. Results: Participants averaged significantly better DVA (p = 0.0008), NVA (p < 0.0001), and CVT (p = 0.0105) in the MOA than the TVAM, but all three MOA assessments were predictive of the TVAM values. CS was significantly better with the MOA (p < 0.0001). Linear regression and Spearman correlation tests were applied to assess the effect of CS on NVA, which showed no clear relationship between the difference in NVA and the difference in CS. PD using the two methods was in agreement with no significant difference (p = 0.2889). Conclusion: The studied MOA offers an effective means of measuring four common vision parameters: DVA, NVA, CVT, and PD. The MOA can potentially be used by eye care providers, health care providers, and patients, both as a screening tool with correction factor and to monitor ocular pathologies. Atypical MOA measurements should prompt testing in the clinic with formal TVAMs.
Validity and Feasibility of a Self-Administered Home Vision Examination in Yueqing, China: a Cross-Sectional Study

Abstract:Objective To investigate the validity and feasibility of a self-administered home vision examination programme in China. Design Cross-sectional study. Setting Yueqing, China. Participants A two-stage convenience sampling procedure was used to randomly select 600 households from 30 communities participating in the Yueqing Eye Study (YES). The aim of YES is to encourage home-based vision screening, reporting of visual acuity (VA) annually through social media and encouraging people to attend follow-up clinic appointments as a way to improve eye care access for adults with VA ≤+0.5 log of the minimum angle of resolution (logMAR). Interventions Household screeners (one per household) who tested other family members’ VA completed a questionnaire on family structure, demographic information and knowledge about screening procedures. Other family members then underwent confirmatory VA testing by researchers. Outcome measures The completion rate of home-based VA screening, its sensitivity and specificity were used to evaluate validity. Factors that determined whether families participated in the self-VA screening were used to evaluate feasibility. Results 345 (66%) of the 523 (87.2%) households with valid data form their home-based vision examinations also were retested by researchers. There was no statistically significant difference in scores on the family-administerd or researcher-administerd VA test (VA≤+0.5 logMAR, p=0.607; VA >+0.5 logMAR, p=0.612). The sensitivity and specificity of home-based vision screening were 80.5% (95% CI 70.2% to 86.9%) and 95.1% (95% CI 92.6% to 96.8%), respectively. 14.7% (77/523) of tested respondents had VA ≤+0.5 logMAR. Predictors of performing home screening for VA remaining in regression models included higher economic status (‘fair and above’ vs ‘poor’: OR 1.74; 95% CI 1.08 to 2.76; p=0.022), age (<45 years vs ≥45 years: OR 0.46; 95% CI 0.25 to 0.85; p=0.014) and living in a nuclear (OR 5.17; 95% CI 2.86 to 9.36; p<0.001) or extended family (OR 8.37; 95% CI 4.93 to 14.20; p<0.001). Conclusion Self-administered home vision screening is reliable and highly accepted by Chinese adults.
Effectiveness of a smartphone application for testing near visual acuity

Abstract:PurposeThe purpose of this study was to evaluate the discrepancy between the near visual acuity (VA) measurements using the EyeHandBook smartphone application and the conventional method of using the near vision card.MethodsUsing similar environmental/examinational conditions, near VA measurements were obtained and compared using the near vision card and the EyeHandBook app for IPhone 5 from 100 subjects. The obtained data were changed to LogMAR format and the relationship between the two techniques was analyzed by paired sample t-test and scatterplot.ResultsWith a P-value of<0.0001, our results indicate that the EyeHandBook application running on IPhone 5 overestimates the near VA compared with the conventional near vision card by an average of 0.11 LogMAR unless the measurement done by the near vision card was 20/20.ConclusionsOwing to vast utilization of portable high-definition screens in VA measurements, eye-care providers have to be mindful of the potential disparity in VA measurement between different platforms, which in our study was likely secondary to the high contrast and brightness levels of the smartphone’s high-definition screen when compared with the near vision card.

Abstract:Background To develop and assess the usability of a smartphone-based visual acuity (VA) test with an automatic distance calibration (ADC) function, the iOS version of WHOeyes. Methods The WHOeyes was an upgraded version with a distinct feature of ADC of an existing validated VA testing app called V@home. Three groups of Chinese participants with different ages (≤20, 20–40, >40 years) were recruited for distance and near VA testing using both an Early Treatment Diabetic Retinopathy Study (ETDRS) chart and the WHOeyes. The ADC function would determine the testing distance. Infrared rangefinder was used to determine the testing distance for the ETDRS, and actual testing distance for the WHOeyes. A questionnaire-based interview was administered to assess the satisfaction. Results The actual testing distance determined by the WHOeyes ADC showed an overall good agreement with the desired testing distance in all three age groups (p>0.50). Regarding the distance and near VA testing, the accuracy of WHOeyes was equivalent to ETDRS. The mean difference between the WHOeyes and ETDRS ranged from −0.084 to 0.012 logMAR, and the quadratic weighted kappa (QWK) values were >0.75 across all groups. The test–retest reliability of WHOeyes was high for both near and distance VA, with a mean difference ranging from −0.040 to 0.004 logMAR and QWK all >0.85. The questionnaire revealed an excellent user experience and acceptance of WHOeyes. Conclusions WHOeyes could provide accurate measurement of the testing distance as well as the distance and near VA when compared to the gold standard ETDRS chart.

Abstract:PURPOSE: The aim of this study was to compare the visual acuity (VA) by smartphone-based applications – EyeChart and the Peek Acuity to the standard Snellen chart to explore the possibility of using them as an alternative in tele-ophthalmology in the current COVID-19 pandemic. MATERIALS AND METHODS: An analytical type of observational study was done on 360 eyes of 184 patients above 18 years of age. Patients with VA <6/60 and gross ocular pathology were excluded from the study. VA measured by these three methods was converted to logMAR scale for ease of statistical analysis. One-way analysis of variance with post Tukey HSD was used to compare the VA measured by these three methods. RESULTS: There was no statistically significant difference between VA measured using the smartphone-based apps (EyeChart and Peek Acuity) and the Snellen chart ( F = 2.5411, P = 0.7925) in 360 eyes assessed. VA measured by Peek Acuity ( P = 0.5225) was more comparable to Snellen chart than EyeChart ( P = 0.4730). Intraclass correlation coefficient (ICC) demonstrated a strong positive correlation for EyeChart (ICC: 0.982, P < 0.001) and Peek Acuity (ICC: 0.980, P < 0.001) with Snellen chart. A Bland–Altman difference plot showed good limits of agreement for both EyeChart and Peek Acuity with Snellen chart. In subgroup analysis, VA measured by Peek Acuity was not statistically different from Snellen in any subgroups, but in EyeChart, it was statistically different in emmetropes. CONCLUSION: VA measured by smartphone apps (EyeChart and Peek Acuity) was comparable with traditional Snellen chart and can be used as an effective, reliable, and feasible alternative to assess VA in tele-ophthalmology.

Abstract:BackgroundTo investigate the agreement between a mobile applet-based visual acuity (VA) self-test program and the conventional VA tests. MethodsThis consecutive case series study included 121 children and adults (242 eyes). Patients were classified into three groups according to age (children, adolescents, and adults). They underwent uncorrected distance visual acuity (UDVA) testing, distance visual acuity with available spectacle correction (DVA with ASC) testing at 2.5-m distance, uncorrected near visual acuity (UNVA) testing, and near visual acuity with available spectacle correction (NVA with ASC) testing at a 0.4-m distance using a mobile applet-based VA self-test program and conventional VA tests in two eyes. ResultsCorrelations among UDVA, DVA with ASC, UNVA, and NVA with ASC between the two methods were significant in all subjects (all p < 0.001). The intraclass correlation coefficient were 0.960, 0.845, 0.960, and 0.669, respectively (all p < 0.001). The proportions of bias outside the 95% confidence interval limit of agreements were 6.20%, 4.82%, 7.08%, and 6.10%, respectively. There were significant differences in NVA with ASC between the two methods (p < 0.05) in the adolescent group, but no differences in measured parameters were found among children and adult groups. ConclusionsThere is good agreement between the mobile applet-based VA self-test program and the conventional VA tests. The VA self-test has good practical value especially in the current pandemic, allowing self-screening of visual acuity, myopia control, and remote management of visual impairment in ocular morbidity.
Validation of Home Visual Acuity Tests for Telehealth in the COVID-19 Era

Abstract:ImportanceVisual acuity (VA) is one of the most important clinical data points in ophthalmology. However, few options for validated at-home VA assessments are currently available.ObjectiveTo validate 3 at-home visual acuity tests in comparison with in-office visual acuity.Design, Setting, and ParticipantsBetween July 2020 and April 2021, eligible participants with VA of 20/200 or better were recruited from 4 university-based ophthalmology clinics (comprehensive, cornea, glaucoma, and retina clinics). Participants were prospectively randomized to self-administer 2 of 3 at-home VA tests (printed chart, mobile phone app, and website) within 3 days before their standard-of-care clinic visit. Participants completed a survey assessing usability of the at-home tests. At the clinic visit, best-corrected Snellen distance acuity was measured as the reference standard.Main Outcomes and MeasuresThe at-home VA test results were compared with the in-office VA test results using paired and unpairedttests, Pearson correlation coefficients, analysis of variance, χ2tests, and Cohen κ agreement. The sensitivity, specificity, positive predictive value, and negative predictive value of each at-home test were calculated to detect significant VA changes (≥0.2 logMAR) from the in-office baseline.ResultsA total of 121 participants with a mean (SD) age of 63.8 (13.0) years completed the study. The mean in-office VA was 0.11 logMAR (Snellen equivalent 20/25) with similar numbers of participants from the 4 clinics. Mean difference (logMAR) between the at-home test and in-office acuity was −0.07 (95% CI, −0.10 to −0.04) for the printed chart, −0.12 (95% CI, −0.15 to −0.09) for the mobile phone app, and −0.13 (95% CI, −0.16 to −0.10) for the website test. The Pearson correlation coefficient for the printed chart was 0.72 (95% CI, 0.62-0.79), mobile phone app was 0.58 (95% CI, 0.46-0.69), and website test was 0.64 (95% CI, 0.53-0.73).Conclusions and RelevanceThe 3 at-home VA test results (printed chart, mobile phone app, and website) appeared comparable within 1 line to in-office VA measurements. Older participants were more likely to have limited access to digital tools. Further development and validation of at-home VA testing modalities is needed with the expansion of teleophthalmology care.

Abstract:Patients with macular pathology, including that caused by age-related macular degeneration and diabetic macular oedema, must attend frequent in-clinic monitoring appointments to detect onset of disease activity requiring treatment and to monitor progression of existing disease. In-person clinical monitoring places a significant burden on patients, caregivers and healthcare systems and is limited in that it only provides clinicians with a snapshot of the patient's disease status. The advent of remote monitoring technologies offers the potential for patients to test their own retinal health at home in collaboration with clinicians, reducing the need for in-clinic appointments. In this review we discuss visual function tests, both existing and novel, that have the potential for remote use and consider their suitability for discriminating the presence of disease and progression of disease. We then review the clinical evidence supporting the use of mobile applications for monitoring of visual function from clinical development through to validation studies and real-world implementation. This review identified seven app-based visual function tests: four that have already received some form of regulatory clearance and three under development. The evidence included in this review shows that remote monitoring offers great potential for patients with macular pathology to monitor their condition from home, reducing the need for burdensome clinic visits and expanding clinicians' understanding of patients' retinal health beyond traditional clinical monitoring. In order to instil confidence in the use of remote monitoring in both patients and clinicians further longitudinal real-world studies are now warranted.

Abstract:AIM: To develop and evaluate the validity and reliability of a knowledge, attitude, and practice questionnaire related to vision screening (KAP-VST) among preschool teachers in Malaysia. METHODS: The questionnaire was developed through a literature review and discussions with experts. Content and face validation were conducted by a panel of experts (n=10) and preschool teachers (n=10), respectively. A pilot study was conducted for construct validation (n=161) and test-retest reliability (n=60) of the newly developed questionnaire. RESULTS: Based on the content and face validation, 71 items were generated, and 68 items were selected after exploratory factor analysis. The content validity index for items (I-CVI) score ranged from 0.8-1.0, and the content validity index for scale (S-CVI)/Ave was 0.99. Internal consistency was KR20=0.93 for knowledge, Cronbach’s alpha=0.758 for attitude, and Cronbach’s alpha=0.856 for practice. CONCLUSION: The KAP-VST is a valid and reliable instrument for assessing knowledge, attitude, and practice in relation to vision screening among preschool teachers in Malaysia.

Abstract:Rapid diagnostic tests are crucial for timely disease detection and management, yet accurate interpretation of test results remains challenging. In this study, we propose a novel approach to enhance the accuracy and reliability of rapid diagnostic test result interpretation by integrating artificial intelligence (AI) algorithms, including convolutional neural networks (CNN), within a smartphone-based application. The app enables users to take pictures of their test kits, which YOLOv8 then processes to precisely crop and extract the membrane region, even if the test kit is not centered in the frame or is positioned at the very edge of the image. This capability offers greater accessibility, allowing even visually impaired individuals to capture test images without needing perfect alignment, thus promoting user independence and inclusivity. The extracted image is analyzed by an additional CNN classifier that determines if the results are positive, negative, or invalid, providing users with the results and a confidence level. Through validation experiments with commonly used rapid test kits across various diagnostic applications, our results demonstrate that the synergistic integration of AI significantly improves sensitivity and specificity in test result interpretation. This improvement can be attributed to the extraction of the membrane zones from the test kit images using the state-of-the-art YOLO algorithm. Additionally, we performed SHapley Additive exPlanations (SHAP) analysis to investigate the factors influencing the model's decisions, identifying reasons behind both correct and incorrect classifications. By facilitating the differentiation of genuine test lines from background noise and providing valuable insights into test line intensity and uniformity, our approach offers a robust solution to challenges in rapid test interpretation.
