Abstract:Will sensor-based exposure assessment be the future in workplace settings? Static instruments with embedded sensors are already applied to monitor levels of dangerous substances-in the context of acute health effects-at critical locations. However, with wearable, lightweight, miniaturized (low-cost) sensors developing quickly, much more is possible with sensors in relation to exposure assessment. Sensors can be applied in the work environment, on machines, or on employees and may include sensors that measure chemical exposures, but also sensors or other technologies that collect contextual information to support the exposure measurements. Like every technology it also has downsides. Sensors collect data on individuals that, depending on the purpose, need to be shared with others (e.g. health, safety and environment manager). One can imagine that people are afraid of misuse. To explore possible ethical and privacy issues that may come along with the introduction of sensors in the workplace, we organized a workshop with stakeholders (n = 32) to discuss three possible sensor-based scenarios in a structured way around five themes: purpose, efficacy, intrusiveness, proportionality, and fairness. The main conclusion of the discussions was that stakeholders currently see benefits in using sensors for applied targeted studies (short periods, clear reasons). In order to find acceptance for the implementation of sensors, all individuals affected by the sensors or its data need to be involved in the decisions on the purpose and application of sensors. Possible negative side effects need to be discussed and addressed. Continuous sensor-based monitoring of workers currently appears to be a bridge too far for the participants of this workshop.

New developments in exposure assessment: The impact on the practice of health risk assessment and epidemiological studies

Abstract:The field of exposure assessment has matured significantly over the past 10-15 years. Dozens of studies have measured the concentrations of numerous chemicals in many media to which humans are exposed. Others have catalogued the various exposure pathways and identified typical values which can be used in the exposure calculations for the general population such as amount of water or soil ingested per day or the percent of a chemical than can pass through the skin. In addition, studies of the duration of exposure for many tasks (e.g. showering, jogging, working in the office) have been conducted which allow for more general descriptions of the likely range of exposures. All of this information, as well as the development of new and better models (e.g. air dispersion or groundwater models), allow for better estimates of exposure. In addition to identifying better exposure factors, and better mathematical models for predicting the aerial distribution of chemicals, the conduct of simulation studies and dose-reconstruction studies can offer extraordinary opportunities for filling in data gaps regarding historical exposures which are critical to improving the power of epidemiology studies. The use of probabilistic techniques such as Monte Carlo analysis and Bayesian statistics have revolutionized the practice of exposure assessment and has greatly enhanced the quality of the risk characterization. Lastly, the field of epidemiology is about to undergo a sea change with respect to the exposure component because each year better environmental and exposure models, statistical techniques and new biological monitoring techniques are being introduced. This paper reviews these techniques and discusses where additional research is likely to pay a significant dividend. Exposure assessment techniques are now available which can significantly improve the quality of epidemiology and health risk assessment studies and vastly improve their usefulness. As more quantitative exposure components can now be incorporated into these studies, they can be better used to identify safe levels of exposure using customary risk assessment methodologies. Examples are drawn from both environmental and occupational studies illustrating how these techniques have been used to better understand exposure to specific chemicals. Some thoughts are also presented on what lessons have been learned about conducting exposure assessment for health risk assessments and epidemiological studies.

Abstract:Many epidemiologic studies of the health effects of exposure to ambient air pollution use measurements from central-site monitors as their exposure estimate. However, measurements from central-site monitors may lack the spatial and temporal resolution required to capture exposure variability in a study population, thus resulting in exposure error and biased estimates. Articles in this dedicated issue examine various approaches to predict or assign exposures to ambient pollutants. These methods include combining existing central-site pollution measurements with local- and/or regional-scale air quality models to create new or “hybrid” models for pollutant exposure estimates and using exposure models to account for factors such as infiltration of pollutants indoors and human activity patterns. Key findings from these articles are summarized to provide lessons learned and recommendations for additional research on improving exposure estimation approaches for future epidemiological studies. In summary, when compared with use of central-site monitoring data, the enhanced spatial resolution of air quality or exposure models can have an impact on resultant health effect estimates, especially for pollutants derived from local sources such as traffic (e.g., EC, CO, and NOx). In addition, the optimal exposure estimation approach also depends upon the epidemiological study design. We recommend that future research develops pollutant-specific infiltration data (including for PM species) and improves existing data on human time-activity patterns and exposure to local source (e.g., traffic), in order to enhance human exposure modeling estimates. We also recommend comparing how various approaches to exposure estimation characterize relationships between multiple pollutants in time and space and investigating the impact of improved exposure estimates in chronic health studies.

Portable Sensors for Dynamic Exposure Assessments in Urban Environments: State of the Science

Abstract:This study presents a fit-for-purpose lab and field evaluation of commercially available portable sensor systems for PM, NO2, and/or BC. The main aim of the study is to identify portable sensor systems that are capable of reliably quantifying dynamic exposure gradients in urban environments. After an initial literature and market study resulting in 39 sensor systems, 10 sensor systems were ultimately purchased and benchmarked under laboratory and real-word conditions. We evaluated the comparability to reference analyzers, sensor precision, and sensitivity towards environmental confounders (temperature, humidity, and O3). Moreover, we evaluated if the sensor accuracy can be improved by applying a lab or field calibration. Because the targeted application of the sensor systems under evaluation is mobile monitoring, we conducted a mobile field test in an urban environment to evaluate the GPS accuracy and potential impacts from vibrations on the resulting sensor signals. Results of the considered sensor systems indicate that out-of-the-box performance is relatively good for PM (R2 = 0.68–0.9, Uexp = 16–66%, BSU = 0.1–0.7 μg/m3) and BC (R2 = 0.82–0.83), but maturity of the tested NO2 sensors is still low (R2 = 0.38–0.55, Uexp = 111–614%) and additional efforts are needed in terms of signal noise and calibration, as proven by the performance after multilinear calibration (R2 = 0.75–0.83, Uexp = 37–44%)). The horizontal accuracy of the built-in GPS was generally good, achieving <10 m accuracy for all sensor systems. More accurate and dynamic exposure assessments in contemporary urban environments are crucial to study real-world exposure of individuals and the resulting impacts on potential health endpoints. A greater availability of mobile monitoring systems capable of quantifying urban pollutant gradients will further boost this line of research.

Abstract:Over time it became clear that exposure to chemical substances is complex in nature. We have come to understand that chemicals are distributed between the gas phase, airborne particles, settled dust and exposed surfaces, in a manner dependent on their respective physical properties. In terms of deposition and its effect, whether a molecule enters the lung via the gas phase or the particle phase makes a significant difference. The oral intake of substances from house dust is strongly dependent on the occupant’s age and habit. The importance of the dermal intake pathway via the room air is now known and is the subject of several studies. Furthermore, we have also learned that secondary products from chemical reactions in the gas phase and on surfaces also contribute to exposure. Today, biomonitoring methods are available for many of the substances, which can be found in indoors. The identification of priority chemicals and the development of methods for these, with help from expert groups, is undoubtedly also a purposeful and seminal approach. With this contribution, which bases on a recent review paper, the authors hope to demonstrate that research addressing the sources, redistribution and fate of chemicals found indoors and human biomonitoring not only complement one another but also belong together within the field of exposure analysis. With the appropriate experimental design, external and internal exposure as well as the intake and uptake of a substance can be determined. A further important aspect is the post hoc analysis of house dust and urine samples, which are stored in environmental specimen banks. The identification and temporal tracking of so-called emerging chemicals is thereby enabled. This, in turn, creates the possibility of taking a virtual look into the future and acting preemptively, instead of constantly just looking back into the past and watching the decay of substances which have not been utilized for a considerable length of time.

Abstract:The traditional model of nursing, in which patients are discharged with a follow-up medical appointment, is no longer sufficient in the current healthcare environment. Primary nurses working in a 58-bed rehabilitation unit located in a 1,700-bed tertiary care medical center implemented a follow-up telephone call program to support the patient's transition from acute rehabilitation nursing care to community living. Patients discharged to home within a five-state area were called 2 weeks and 6 weeks after discharge. Notes on the telephone conversations were entered on a data collection form and later analyzed using Orem's self-care deficit theory (Orem, 1991). A total of 144 follow-up calls were made. During the first call (n = 105), 157 problems (1.5 per call) were noted, whereas 79 (2.0 per call) were identified during the second call (n = 39). Medication, safety, and bladder problems were most often cited as concerns by patients and caregivers after discharge. The most frequently used helping interventions during both the first and the second telephone calls were guiding and supporting.

Abstract:In the modern "omics" era, measurement of the human exposome is a critical missing link between genetic drivers and disease outcomes. High-resolution mass spectrometry (HRMS), routinely used in proteomics and metabolomics, has emerged as a leading technology to broadly profile chemical exposure agents and related biomolecules for accurate mass measurement, high sensitivity, rapid data acquisition, and increased resolution of chemical space. Non-targeted approaches are increasingly accessible, supporting a shift from conventional hypothesis-driven, quantitation-centric targeted analyses toward data-driven, hypothesis-generating chemical exposome-wide profiling. However, HRMS-based exposomics encounters unique challenges. New analytical and computational infrastructures are needed to expand the analysis coverage through streamlined, scalable, and harmonized workflows and data pipelines that permit longitudinal chemical exposome tracking, retrospective validation, and multi-omics integration for meaningful health-oriented inferences. In this article, we survey the literature on state-of-the-art HRMS-based technologies, review current analytical workflows and informatic pipelines, and provide an up-to-date reference on exposomic approaches for chemists, toxicologists, epidemiologists, care providers, and stakeholders in health sciences and medicine. We propose efforts to benchmark fit-for-purpose platforms for expanding coverage of chemical space, including gas/liquid chromatography-HRMS (GC-HRMS and LC-HRMS), and discuss opportunities, challenges, and strategies to advance the burgeoning field of the exposome.
Exposure information in environmental health research: Current opportunities and future directions for particulate matter, ozone, and toxic air pollutants

Abstract:Understanding and quantifying outdoor and indoor sources of human exposure are essential but often not adequately addressed in health effect studies for air pollution. Air pollution epidemiology, risk assessment, health tracking, and accountability assessments are examples of health effect studies that require but often lack adequate exposure information. Recent advances in exposure modeling along with better information on time–activity and exposure factor data provide us with unique opportunities to improve the assignment of exposures for both future and ongoing studies linking air pollution to health impacts. In September 2006, scientists from the US Environmental Protection Agency and the Centers for Disease Control and Prevention along with scientists from the academic community and state health departments convened a symposium on air pollution exposure and health to identify, evaluate, and improve current approaches for linking air pollution exposures to disease. This manuscript presents the key issues, challenges, and recommendations identified by the exposure working group, who used case studies of particulate matter, ozone, and toxic air-pollutant exposure to evaluate health effects for air pollution. One of the overarching lessons of this workshop is that obtaining better exposure information for these different health effect studies requires both goal setting for what is needed and mapping out the transition pathway from current capabilities for meeting these goals. Meeting our long-term goals requires definition of incremental steps that provide useful information for the interim and move us toward our long-term goals. Another overarching theme among the three different pollutants and the different health study approaches is the need for integration among alternate exposure-assessment approaches. For example, different groups may advocate exposure indicators, biomonitoring, mapping methods (GIS), modeling, environmental media monitoring, and/or personal exposure modeling. However, emerging research reveals that the greatest progress comes from integration among two or more of these efforts.

Abstract:This paper describes a Bayesian model for the assessment of inhalation exposures in an occupational setting; the methodology underpins a freely available web-based application for exposure assessment, the Advanced REACH Tool (ART). The ART is a higher tier exposure tool that combines disparate sources of information within a Bayesian statistical framework. The information is obtained from expert knowledge expressed in a calibrated mechanistic model of exposure assessment, data on inter- and intra-individual variability in exposures from the literature, and context-specific exposure measurements. The ART provides central estimates and credible intervals for different percentiles of the exposure distribution, for full-shift and long-term average exposures. The ART can produce exposure estimates in the absence of measurements, but the precision of the estimates improves as more data become available. The methodology presented in this paper is able to utilize partially analogous data, a novel approach designed to make efficient use of a sparsely populated measurement database although some additional research is still required before practical implementation. The methodology is demonstrated using two worked examples: an exposure to copper pyrithione in the spraying of antifouling paints and an exposure to ethyl acetate in shoe repair.

Abstract:The predictions of environmental pollution and its changes, as well as the formulation of pollution control strategies involve many variables with nonlinear, time-varying, multi-source and multi-objective characteristics, such as real-time monitoring data, population activity data, pollutant exposure data, meteorological data, etc. (Liu et al., 2022). In addition, complex relationships exist between many variables (Zhu et al., 2023). They make it extremely difficult to achieve expected prediction performance. Improved computational methodologies, algorithms and models, such as density functional theory, microkinetic modeling, classical molecular dynamics, continuum modeling, and machine learning (ML), provide a good solution to solve this complexity (Wong et al., 2024). For example, artificial intelligence (AI) technologies can automate analyses by making inferences from data and identifying patterns without explicit programming by humans, thereby providing new opportunities to integrate and analyze large volumes of data (Heacock et al., 2022). ML algorithms, as a subfield of AI technology, can help analyze and model large amounts of complicated data more robustly and efficiently than traditional statistical models (Taoufik et al., 2022).These advanced computational and data analytical approaches have been employed to tackle complexities of uncertain from interactive and dynamic environmental problems, playing a leading role in modeling, optimization, prediction and control (Ye et al., 2020). However, the current application is mainly based on the direct use of existing functions or commands. Very few models have been developed specifically for environmental issues, which ignore the complexity and specificity of environmental problems. We hope that researchers try to construct environment-specific models including ready-to-use tools or source code to make predictions, which truly integrate the computational and data science methods into traditional environmental modeling to reveal hidden patterns or correlations, thereby promoting environmental management and pollution control. In addition, the complexity of environmental problems leads to an added challenge regarding the interpretation of the modeling results due to the complicated or black-box relationships between input and output variables.Therefore, this Research Topic aims to feature Original Research articles and Reviews on the developments and applications of modeling and computing technologies in scientific studies on the sources, environmental behavior, fate, transformation, toxicity, risk and removal of pollutants. A series of articles have been published under this research topic, involving the important applications of modeling and computing technology in addressing environmental pollution and change, as follows.Islam et al. (2023) investigated the exposure levels of trace elements (Cr, Cu, Ni, As, Zn, Cd, Mn, Fe and Pb) in a long-term banana cultivation field's soil and different tissues of the banana plant (Musa spp.). Then the pollution status and potential ecological and health risks of the trace elements were evaluated using various established models and chemometric approaches.Based on the proposed possible degradation routes of a next-generation propellant HFO-1234ze(E) for pressurized metered-dose inhalers (pMDIs) devices in the atmosphere, Tewari et al. (2023) quantified the yield of trifluoroacetic acid (TFA) from HFO-1234ze(E) in the range of 0% to 4% using a box-modeling approach under Europe-relevant different atmospheric conditions. Then they estimated an annual rainwater concentration of TFA to be about 0.025 μg/L in France, which could be a negligible level. This work further supports its suitability as a non-persistent, non-bioaccumulative, and non-toxic future propellant for pMDI devices to safeguard access for patients to these essential medicines.Halama et al. (2024) characterized the spatial and temporal patterns of daily and interannual tire road wear particle (TRWP) deposition and associated fate and transport of 6PPD-Quinone in an urbanized watershed, using a process-based ecohydrological model Visualizing Ecosystem Land Management Assessments (VELMA). The modeled daily average 6PPD-Quinone concentration was generally consistent with the observed grab sample concentrations. The results demonstrated the capabilities of VELMA for prioritizing the locations, amounts, and types of green infrastructure that can most effectively reduce 6PPD-Quinone stream concentrations to levels protective of coho salmon and other aquatic species.Neira-Alboronoz et al. Abstract:Biomonitoring has been widely used in assessing exposures in both occupational and public health complementing chemical risk assessments because it measures the concentrations of chemical substances in human body fluids (e.g., urine and blood). Biomonitoring considers all routes and sources of exposure. An occupational biomonitoring guidance document has been elaborated ( https://www.oecd.org/chemicalsafety/risk-assessment/occupational-biomonitoring-guidance-document.pdf ) within the OECD framework and specifically, the Working Parties on Exposure and Hazard Assessment by scientific experts from 40 institutes and organizations representing 15 countries. The guidance provides practical information for assessing chemical exposures in occupational settings including the three common routes of exposure: inhalation, skin absorption and ingestion due to hand to mouth contact. The elaborated stepwise approach for conducting biomonitoring is tailored for occupational health professionals, scientists, risk assessors, and regulators. It includes methods for selecting appropriate biomarkers, devising sampling strategies, and assessing laboratories for validated analytical methods for the biomarker of interest, and ensuring timely feedback of results. Furthermore, it describes procedures for setting up efficient biomonitoring programs based on the Similar Exposure Group (SEG) approaches. Derived health-based human exposure biomarker assessment values called Occupational Biomonitoring Levels (OBLs) are proposed for use in occupational exposure and risk assessment. It also helps with the interpretation of biomonitoring results routinely collected and procedures for communicating biomonitoring results at individual, collective, and workplace levels. Ethical considerations associated with biomonitoring are also discussed. The ultimate goal of this biomonitoring approach is to promote harmonized application and interpretation of biomarkers as well as evidence-based occupational risk management measures.

Source apportionment of ambient PM10 collected at three sites in an urban-industrial area with multi-time resolution factor analyses

Abstract:Chemical speciation data for PM10, collected for annual trend analyses of health-relevant species, at three receptor sites in a highly industrialized area (IJmond) in the Netherlands were used in a multi-time resolution receptor model (ME-2) to identify the PM10 sources in this area. Despite the available data not being optimized for receptor modelling, five-factor solutions were obtained for all sites based on independent PMF analysis on PM10 data from the three sites (IJM, WAZ and BEV). Four factors were common to all three sites: nitrate-sulphate (average percentage contributions to PM10: IJM: 35.3 %, WAZ: 37.7 %, and BEV: 36.3 %); sea salt (20.2 %, 23.7 %, 15.2 %); industrial (8.1 %, 11.0 %, 18.1 %) and brake wear/traffic (31.4 %, 21.2 %, 20.6 %). At WAZ, a local/site-specific factor containing most of the PAH measurements was found (6.4 %) while a crustal matter factor was resolved at IJM (7.6 %) and BEV (9.8 %). Additionally, sludge-drying was a potential source of the marker species in the industrial factor at WAZ. Bootstrapping (BS) and factor displacement (DISP) were applied to the factor profiles in this work for error estimation. In general, the factor profiles at all three sites had very small intervals from both BS and DISP methods. To our knowledge, this is the first time DISP was applied in a complex model such as the multi-time resolution model. Most of the measured metal and PAH concentrations found in the IJmond area during the 2017-2019 period had local sources, with significant contributions from several processes related to the steel industry. This study shows that available detailed PM10 chemical speciation data, although primarily collected for annual trend analyses of health-relevant species, could also be used in receptor modelling by applying a multi-time framework. We propose general recommendations for the optimization of the measurement strategy for source apportionment of PM in areas with similar urban-industrial land use.
Assessing Human Exposure To Svocs In Materials, Products, And Articles: A Modular Mechanistic Framework

Abstract:A critical review of the current state of knowledge of chemical emissions from indoor sources, partitioning among indoor compartments, and the ensuing indoor exposure leads to a proposal for a modular mechanistic framework for predicting human exposure to semivolatile organic compounds (SVOCs). Mechanistically consistent source emission categories include solid, soft, frequent contact, applied, sprayed, and high temperature sources. Environmental compartments are the gas phase, airborne particles, settled dust, indoor surfaces, and clothing. Identified research needs are the development of dynamic emission models for several of the source emission categories and of estimation strategies for critical model parameters. The modular structure of the framework facilitates subsequent inclusion of new knowledge, other chemical classes of indoor pollutants, and additional mechanistic processes relevant to human exposure indoors. The framework may serve as the foundation for developing an open-source community model to better support collaborative research and improve access for application by stakeholders. Combining exposure estimates derived using this framework with toxicity data for different end points and toxicokinetic mechanisms will accelerate chemical risk prioritization, advance effective chemical management decisions, and protect public health.

Abstract:Roadside concentrations of harmful pollutants such as NOx are highly variable in both space and time. This is rarely considered when assessing pedestrian and cyclist exposures. We aim to fully describe the spatio-temporal variability of exposures of pedestrians and cyclists travelling along a road at high resolution. We evaluate the value added of high spatio-temporal resolution compared to high spatial resolution only. We also compare high resolution vehicle emissions modelling to using a constant volume source. We highlight conditions of peak exposures, and discuss implications for health impact assessments. Using the large eddy simulation code Fluidity we simulate NOx concentrations at a resolution of 2 m and 1 s along a 350 m road segment in a complex real-world street geometry including an intersection and bus stops. We then simulate pedestrian and cyclist journeys for different routes and departure times. For the high spatio-temporal method, the standard deviation in 1 s concentration experienced by pedestrians (50.9 μg.m-3) is nearly three times greater than that predicted by the high-spatial only (17.5 μg.m-3) or constant volume source (17.6 μg.m-3) methods. This exposure is characterised by low concentrations punctuated by short duration, peak exposures which elevate the mean exposure and are not captured by the other two methods. We also find that the mean exposure of cyclists on the road (31.8 μg.m-3) is significantly greater than that of cyclists on a roadside path (25.6 μg.m-3) and that of pedestrians on a sidewalk (17.6 μg.m-3). We conclude that ignoring high resolution temporal air pollution variability experienced at the breathing time scale can lead to a mischaracterization of pedestrian and cyclist exposures, and therefore also potentially the harm caused. High resolution methods reveal that peaks, and hence mean exposures, can be meaningfully reduced by avoiding hyper-local hotspots such as bus stops and junctions.

Abstract:Although external concentrations are more readily quantified and often used as the metric for regulating and mitigating exposures to environmental chemicals, the toxicological response to an environmental chemical is more directly related to its internal concentrations than the external concentration. The processes of absorption, distribution, metabolism, and excretion (ADME) determine the quantitative relationship between the external and internal concentrations, and these processes are often susceptible to saturation at high concentrations, which can lead to nonlinear changes in internal concentrations that deviate from proportionality. Using generic physiologically-based pharmacokinetic (PBPK) models, we explored how saturable absorption or clearance influence the shape of the internal to external concentration (IEC) relationship. We used the models for hypothetical chemicals to show how differences in kinetic parameters can impact the shape of an IEC relationship; and models for styrene and caffeine to explore how exposure route, frequency, and duration impact the IEC relationships in rat and human exposures. We also analyzed available plasma concentration data for 2,4-dichlorophenoxyacetic acid to demonstrate how a PBPK modeling approach can be an alternative to common statistical methods for analyzing dose proportionality. A PBPK modeling approach can be a valuable tool used in the early stages of a chemical safety assessment program to optimize the design of longer-term animal toxicity studies or to interpret study results.

Abstract:The chemical burden on the environment and human population is increasing. Consequently, regulatory risk assessment must keep pace to manage, reduce, and prevent adverse impacts on human and environmental health associated with hazardous chemicals. Surveillance of chemicals of known, emerging, or potential future concern, entering the environment-food-human continuum is needed to document the reality of risks posed by chemicals on ecosystem and human health from a one health perspective, feed into early warning systems and support public policies for exposure mitigation provisions and safe and sustainable by design strategies. The use of less-conventional sampling strategies and integration of full-scan, high-resolution mass spectrometry and effect-directed analysis in environmental and human monitoring programmes have the potential to enhance the screening and identification of a wider range of chemicals of known, emerging or potential future concern. Here, we outline the key needs and recommendations identified within the European Partnership for Assessment of Risks from Chemicals (PARC) project for leveraging these innovative methodologies to support the development of next-generation chemical risk assessment.

Abstract:As the use of engineered nanomaterials becomes more prevalent, the likelihood of unintended exposure to these materials also increases. Given the current scarcity of experimental data regarding fate, transport, and bioavailability, determining potential environmental exposure to these materials requires an in depth analysis of modeling techniques that can be used in both the near- and long-term. Here, we provide a critical review of traditional and emerging exposure modeling approaches to highlight the challenges that scientists and decision-makers face when developing environmental exposure and risk assessments for nanomaterials. We find that accounting for nanospecific properties, overcoming data gaps, realizing model limitations, and handling uncertainty are key to developing informative and reliable environmental exposure and risk assessments for engineered nanomaterials. We find methods suited to recognizing and addressing significant uncertainty to be most appropriate for near-term environmental exposure modeling, given the current state of information and the current insufficiency of established deterministic models to address environmental exposure to engineered nanomaterials.
