Abstract:Vestibular schwannomas in pregnancy have rarely been reported, and there is a lack of in-depth discussion on the experience of management of massive acoustic neuromas in pregnancy. Herein, we present a pregnant woman with a giant vestibular schwannoma and obstructive hydrocephalus who presented at 30 weeks of gestation. She was initially misdiagnosed as having a pregnancy-related reaction of headache, dizziness, and vomiting that had occurred 2 months earlier. After observation at home, her symptoms progressed at 30 weeks of gestation, and imaging findings revealed a brain tumor in the CPA region with secondary cerebella tonsil herniation and obstructive hydrocephalus, and she was transferred to our center for treatment. Consequently, we relieved her hydrocephalus with a ventriculoperitoneal shunt (V-P shunt) and used corticosteroids to simulate fetal maturation. After 10 days, her mental condition deteriorated, and her right limb muscle strength gradually decreased until grade 0 (MMT Grading). Finally, under a joint consultation with the Department of Neurosurgery, Obstetrics, and Anesthesiology, she underwent a cesarean section under general anesthesia and first-stage tumor removal at 31 weeks of gestation. Upon discharge, the previously observed neurological deficits, which were reversible and had manifested during her gestational period, had been successfully resolved, and the fetus had been conserved. The neuroimaging confirmed the complete tumor removal, while the neuropathologic examination revealed a vestibular schwannoma. Therefore, we recommend early diagnosis and treatment for these patients, especially people with headaches, vomiting, and sudden hearing loss during pregnancy. Herein, we concluded that our cases provide a valuable experience in the latest acceptable time frame for the operation to prevent irreversible neurological impairment and premature delivery in late pregnancy.
Large Malignant Ovarian Tumors During Pregnancy: Two Cases

Abstract:The present study reports two cases of large ovarian malignancy during pregnancy, which is very rare. The two patients were received between Nov 2012 and Feb 2013 at Gynecological and Obstetrical Department, Women's Hospital, School of Medicine, Zhejiang University (People's Republic of China). Both cases present tumor sized more than 20 cm, with one case of 40 cm. Both patients underwent timely cesarean section, with survival of the Child, and successful removal of the tumor. All patients showed good outcome in the follow-up period. Therefore the large ovarian malignancy during pregnancy could be well treated after careful clinical evaluation.
Taxanes in Combination with Platinum Derivatives for the Treatment of Ovarian Cancer During Pregnancy: A Literature Review

Abstract:Ovarian cancer is one of the most common types of solid carcinoma diagnosed during pregnancy. Taxane plus a platinum derivative is a combination therapy that is predominantly used in the treatment of ovarian cancer in non-pregnant women. Pregnancy adds various complexities to a course of treatment. In pregnant patients diagnosed with cancer during the first trimester, the risks of fetal malformations and fetal loss increase following the administration of cytotoxic drugs, and this is higher with multi-agent vs. single-agent chemotherapy (similar to 25 vs. 10%). Exposure during the second and third trimester has little influence on teratogenic effects but increases the risk of intrauterine growth retardation, prematurity, low birth weight, and bone marrow toxicity. The present study aimed to review the maternal and fetal safety of treatment with taxane plus platinum derivatives for ovarian cancer during pregnancy. Relevant literature was retrieved from the Embase and PubMed databases using the search terms "ovarian cancer", "pregnancy", "taxane", "paclitaxel", "docetaxel", "platinum", "cisplatin", and "carboplatin". All available data up until September 2016 was synthesized, with no language restrictions. A total of 11 articles (including 13 pregnancies and 14 newborns) were retrieved that reported on the use of standard-dose taxane and platinum chemotherapy, including 9 cases treated with paclitaxel and carboplatin, 3 cases treated with paclitaxel and cisplatin, and 1 case treated with docetaxel and cisplatin. In 13 of the 14 (92.9%) births included, a healthy neonate was born, with follow-up ranging from 2 to 160 months. The average weight of the neonates at the time of delivery was 2,442.1 g. In 7 of 9 the case reports that provided survival data, the mother was alive and disease-free at the end of follow-up (ranging from 2 to 40 months). In conclusion, combination therapy with taxanes and a platinum derivative may play a significant role in the management of pregnant patients with ovarian cancer during the second and third trimester. Exposure to this combination of agents during the second and third trimester does not appear to have a significant bearing on fetal mortality and abortion.

Ovarian tumor in pregnancy: A review of 60 cases

Abstract:Objective To study the clinical characters, diagnosis, and treatment of ovarian tumor in pregnancy. Methods 60 cases with ovarian tumor in pregnancy were reviewed. Results The incidence of pregnancy with Ovarian tumor was 5. 30 (60/11 396). Tumor-like lesions were the most common cases (31/60, 51.7%), followed by benign tumors (27/60, 45.0%)and malignant tumors (2/60, 3.3%). Mature cystic teratoma accounted for 30.0%, endometrioma cyst 20. 0%, adnexal torsion 11. 7%, the rupture of the tumor 3. 3%, and the arrest of descent of the presentation 3. 3%, Of the 60 cases, 53 cases were presented as term delivery, 4 premature birth, 2 spontaneous abortion, and 1 induced abortion. No spontaneous abortion occurred in the 5 patients who underwent laparotomy during 16 to 22 weeks of gestation. Conclusion To avoid adverse pregnancy outcome, excision of ovarian tumor during 16 to 22 weeks of gestation is recommended.
Clinicopathological Characteristics and Treatment Outcomes of Pregnancy Complicated by Malignant Ovarian Germ Cell Tumors.

Abstract:PURPOSE:This study aimed to analyze the clinicopathological features, treatment, and feto-maternal outcomes of pregnancy complicated by malignant ovarian germ cell tumors (MOGCTs), to increase the awareness on this condition.PATIENTS AND METHODS:We retrospectively reviewed the medical records of patients diagnosed with MOGCTs during pregnancy, who were treated and followed-up at Peking Union Medical College Hospital from January 2000 to December 2017. The demographic characteristics, pathological features, treatment and prognosis were analyzed.RESULTS:The histological subtypes varied in 14 patients (dysgerminoma, n=1; immature teratoma, n=4; yolk sac tumor, n=6; and mixed germ cell tumors, n=3). Ten (71.4%) patients, including three who opted for conservative therapy until childbirth, one who only received salvage chemotherapy during pregnancy, and six who underwent cystectomy or unilateral salpingo-oophorectomy during pregnancy, desired fetal preservation. After undergoing surgery, four patients chose surveillance instead of timely adjuvant chemotherapy. Eight patients delivered their babies, and the preterm delivery rate was 50.0%. One newborn died of premature birth. The median follow-up period was 44 (range: 13 to 86) months. During the current study period, 12 patients had survived and did not report any diseases. However, two died due to disease progression.CONCLUSION:Pregnant women with MOGCTs had favorable outcomes. However, when a malignant tumor is suspected, surgery cannot be avoided. Thus, instead of timely postoperative adjuvant chemotherapy, close surveillance may be an acceptable alternative for pregnant women with low-risk MOGCTs.

Abstract:Ovarian cancer is extremely rare in pregnancy, but should be always considered when complex cysts are present and persist, especially when ascites and extra-ovarian nodularity are present. While epithelial cancer of the ovary is most common, germ cell and stromal cancers may also arise. In the absence of a true emergency, such as torsion, surgery should be delayed until the second trimester. If a cancer is discovered, a full staging should be carried out unless, in the opinion of the surgeon, certain procedures would be too dangerous at that time. The additional surgery can always be done if a cesarean section is indicated, or electively after a vaginal delivery and following the conclusion of chemotherapy. If chemotherapy is indicated, it should be started during the pregnancy, using the same regimen and doses that would be given in the nonpregnant state. Carboplatin and paclitaxel are the drugs most often used, and complications of these drugs are not more common in patients who are pregnant. Although there are case reports of the harmful effects of chemotherapy on the fetus, most of the time the drugs are well tolerated by the baby, who is usually born without any side effects secondary to chemotherapy.It is not uncommon for a woman who develops ovarian cancer during her pregnancy to suffer from a form of post-traumatic stress disorder. This condition should be anticipated by the health-care team, and appropriate counseling must be provided. Pregnancy does not augur for a poorer outcome; indeed, the common use of ultrasound early in pregnancy may allow for earlier detection and a more favorable outcome.
Primary ovarian choriocarcinoma mimicking ectopic pregnancy managed with laparoscopy - Case report

Abstract:Nongestational ovarian choriocarcinomas are extremely rare and pose diagnostic challenges in reproductive-aged patients because of elevated human chorionic gonadotrophin (hCG). A 23-year-old nulliparous Chinese woman with nongestational ovarian choriocarcinoma escaped diagnostic testing and was initially treated for an ectopic pregnancy. Three months after her first visit, a diagnostic laparoscopy demonstrated a nongestational ovarian choriocarcinoma. Comprehensive surgical staging was performed by laparoscopy. The tumor was confined to the left ovary. The patient was categorized as FIGO Stage IA. She was given four courses of combined chemotherapy after laparoscopic surgery and has been disease-free for 36 months.

Abstract:BACKGROUND:Intrahepatic cholestasis of pregnancy (ICP) is a common gestational complication characterized by pruritus and elevated bile acids, usually occurring in the third trimester when the serum estrogen and progesterone levels are highest. Hyperandrogenism during pregnancy is a pathological state that is mostly induced by polycystic ovary syndrome (PCOS) but rarely by concomitant androgen-secreting ovarian tumours. To date, no correlation has been drawn between ICP and hyperandrogenism. CASE PRESENTATION:Here, we present a rare case of early-onset severe ICP in a PCOS patient conceived via in vitro fertilization-embryo transfer, with worsening hirsutism and acne due to high levels of testosterone and dehydroepiandrosterone sulphate, both of which were produced by a fast-growing ovarian Sertoli-Leydig cell tumour. Her serum estradiol was also very high, which was speculated to be converted from the circulating androgens by the placenta. She had preterm premature rupture of membranes and delivered at 30 weeks, followed by a rapid remission of ICP as her serum estradiol dropped. However, the excessive androgens did not retreat until the large ovarian tumour was surgically removed. CONCLUSION:This unusual case highlights the concurrence of original hyperandrogenism and subsequent hyperestrogenism during pregnancy and the resultant confounding manifestations. Obstetricians should be aware of the potential association between androgen excess and ICP via placental aromatization.
Primary ependymoma of the abdominal and pelvic cavity: a case report and literature review

Abstract:Purpose To describe the clinicopathologic,immunohistochemical characteristics and differential diagnosis of extra-axial ependymoma.Methods The clinicopathologic,immunohistochemical characteristics of a recurrent ependymoma of the abdominal and pelvic cavity was summarized and the literature database from the PubMed and the China Academic Journal were also reviewed.Results Except for the sacrococcygeal region,the most common extra-axial ependymomas originated from the ovary(24 cases),and the other sites including mediastinum(9 cases),uterine ligaments(6 cases),omentum(1 case),lung(1 case),bowel(1 case),liver(1 case) and douglas pouch(1 case).All of the reported cases in China were found in the ovary.The current case we described which was found in the abdominal and pelvic cavity with multiple lesions not involving the ovaries,had never been reported domestic and foreign.All 44 cases were women,ranging from ages 13 to 71 years,with an average of 36 years,part of them underwent multiple surgeries.The glial fibrillary acidic protein(GFAP),estrogen receptor(ER) and progesterone receptor(PR) were often positive strongly.The present case was a 36-year-old female who underwent three surgeries.Histological characteristics of the three specimens were not completely consistent,perivascular cell-free zone and papillary structures could be found easily in the third slides.GFAP,ER and PR were positive in three tumors.Conclusions Extra-axial ependymoma is a slowly growing and low-grade malignant tumor which has a high recurrence rate.Simultaneous expression of GFAP,ER and PR may help to confirm the diagnosis.
Clinical analysis of 236 pregnant patients with ovarian tumor

Abstract:Objective The aim of this study was to describe the general characteristics of the ovarian tumor in pregnant women at Obstetrics department of Peking University Shenzhen Hospital. Methods A retrospective analysis focusing on clinical characters, diagnosis & treatment and pregnancy outcomes was performed among patients treated at Obstetrics department of Peking University Shenzhen Hospital between 2012-2017. Results A total of 236 cases of ovarian tumors were confirmed by surgery and pathology within 6 years, of which ovarian benign tumors accounted for 52. 54％ (123/236), followed by tumor-like lesions (42. 80％, 101/236) and borderline and malignant tumors (4. 66％, 11/236). The most common pathological types were mature cystic teratomas, accounting for 24. 15％ (57/236), followed by ovarian corpus luteum cysts and ovarian endometriotic cysts, which were 14. 41％ (34/236). Ovarian tumor pedicle torsion was the most common complication, accounting for 5. 51％ (13/236), of which 4 were mature cystic teratomas, 1. 27％ (3/236) of ovarian tumor ruptures, and 1 case of ovarian purulent inflammation with abscess formation, accounting for 0. 42％ (1/236), 2 cases of hospitalization due to compression symptoms in secondary or the third trimester pregnancy, accounting for 0. 85％ (2/236), which were both borderline ovarian and malignant tumors. Of the 236 cases, 203 cases were presented as term delivery, 30 premature births, and 2 induced abortions. Of nineteen patients who underwent laparotomy for lower abdominal pain or abdominal distension, 8 patients underwent cesarean section for termination of pregnancy. Six of them had premature delivery and there was no neonatal death. Ten patients underwent adnexectomy or cystectomy and continued pregnancy, and all of them were with a full-term delivery and a maternal-fetal prognosis. Conclusions Ovarian tumors of pregnant women are mainly benign. Ovarian malignant tumors are difficult to find during pregnancy and should be excluded before pregnancy.

Abstract:ABSTRACT Patients suffering from COPD not controlled by a single bronchodilator should be given two bronchodilators with different mechanisms of action. Addition of a LABA to a LAMA induces a larger bronchodilation than that obtained with the LAMA as monotherapy and also improves many patient-reported outcomes. Since the clinical-functional sign regarding simultaneous use of tiotropium, which is still the dominant LAMA, and olodaterol was very strong, the combination of these two bronchodilators has been developed as FDC delivered via a single inhaler (Respimat) with the aim of simplifying the treatment regimen and improving patient adherence to treatment. The large TOviTO program of Phase III clinical trials that is involving more than 15,000 patients with different levels of COPD severity worldwide and includes several clinical studies is documenting the efficacy and safety of tiotropium/olodaterol FDC as maintenance therapy in patients with moderate to very severe COPD.
Rapid growth of an ovarian clear cell carcinoma expressing LH/hCG receptor arising from endometriosis during early pregnancy

Abstract:The complete clinical course of a case of ovarian clear cell carcinoma expressing luteinizing hormone (LH)/human chorionic gonadotropin (hCG) receptor arising from endometriosis in a pregnant woman is presented. A 31-year-old woman visited a private clinic in May 1993 for screening tests for infertility. Transvaginal ultrasonography revealed no abnormal findings in the uterus or ovaries. Her menstrual cycle was regular; however, a slight luteal insufficiency was noted. She had been treated with clomiphene, and soon became pregnant. She was diagnosed to be at 5 weeks gestation in June, and at the same time, an ovarian tumor with the diameter of 5 cm was identified. Since the tumor had grown rapidly and was 9 cm in diameter 1 week later, she was referred to our hospital. When she was admitted to our hospital at 9 weeks gestation, the tumor diameter was 14 cm and we found the solid portion within the ovarian tumor. The levels of the tumor markers CA125 and CA19-9 were 106 and 51 U/ml, respectively. The crown-rump length of the fetus (24 mm) was compatible with the gestational age, and fetal heartbeat was confirmed. Under the diagnosis of ovarian carcinoma, right salpingo-oophorectomy was performed at 10 weeks of gestation. Postoperative histological examination revealed a clear cell carcinoma and endometriosis of the right ovary. Immunohistochemically, the clear cell carcinoma stained positively for LH/hCG receptors and estrogen receptors, but not progesterone receptor. No malignant cells were detected by ascitic cytology. Exploratory specimens obtained at the time of operation from the left ovary and pelvic lymph nodes exhibited no malignant cells. Based on these findings, the pregnancy was allowed to proceed, and she delivered a 3010-g male baby at 39 weeks of gestation. She had no signs of recurrence for 2 years after the operation.

Abstract:Objective: Epithelioid trophoblastic tumor (ETT) is very rare and few cases have been published in the English literature. Hysterectomy is the recommended treatment, due to the high rate of recurrence and mortality. The objective of this article is to present a rare case of ETT with fertility-preserving treatment and review published similar cases. Case report: We report the case of ETT in a 19-year-old Chinese woman, who had a strong desire of fertility preservation. She presented with vaginal spotting and hysteroscopy showed an isolated solid mass (2.0 x 1.5 x 1.5 cm) at the right corner of the uterine cavity. Serum 13-human chorionic gonadotropin (13-HCG) persisted at low level elevation about 100 IU/L. We treated her with a lesionectomy and 3 cycles EP-EMA (etoposide, cisplatin/etoposide, methotrexate and actinomycin) chemotherapy regimen. The patient is now in stable condition, without any signs of recurrence during 20 months of follow-up. Conclusion: Fertility-preserving surgery would probably be a feasible and safe strategy for the patients whose lesions can be completely removed. Abstract:Bovine leukemia virus (BLV) induces a persistent infection in the B-cells causing polyclonal expansion of B-cells in one-third of infected cattle and lymphosarcoma in less than 5% of infected cattle. While BLV is difficult to detect in vivo, it is readily produced by cultured lymphocytes and is diminished when supplemented by bovine plasma. This phenomenon is attributed to a poorly characterized plasma blocking factor (PBF). We assessed the effects of bovine plasma on cell viability and BLV p24 expression, and the effects of purified PBF on protein synthesis and gene expression of short-term cultures of bovine lymphocytes. The addition of 25% plasma or semi-purified PBF to cultures had no significant effect on cell viability but caused significant decreases in BLV p24 production and significantly increased de novo protein synthesis. Utilizing a human microarray, the RNA messages of 83 genes involved in cell division, cell metabolism, and gene regulation were up-regulated.
The pregnancy and oncology outcome of fertility-sparing management for synchronous primary neoplasm of endometrium and ovary

Abstract:Purpose: To investigate the efficacy of fertility-preserving treatment for young women with synchronous primary neoplasm of endometrium and ovary. Methods: We retrospectively reviewed eight patients with concurrent primary grade 1 presumed stage IA endometrioid endometrial adenocarcinoma (EEA) or endometrial atypical hyperplasia (EAH) and primary stage I ovarian tumors who underwent fertility-sparing treatment in the Obstetrics and Gynecology Hospital of Fudan University between April 2016 and December 2022. Results: Synchronous endometrial and ovarian cancers (SEOC) accounted for 50% of these eight patients. The median age of patients was 30.5 years (range, 28-34 years). None of them received chemotherapy. The median treatment time was 4 months (range, 3-8 months). 87.5% (7/8) cases achieved complete response (CR), and the median time to CR was 3.8 months (range, 1.5-7.7 months). Among patients who got CR, none of them showed any signs of recurrence. Pregnancies and successful deliveries were achieved in 4 of 5 patients. Till September 2023, the median follow-up period was 50.5 months (range:15.2-85.2 months). Conclusion: Fertility-sparing treatment is feasible for highly selected patients with synchronous neoplasm of the endometrium and ovary, but strict screening and monitoring are mandatory. Though the results of our limited cases are encouraging, long follow-up and more clinical data are required. Enrolled patients must be fully informed of the risks during conservative treatment.
Ovarian yolk sac tumor in postmenopausal females: A case series and a literature review.

Abstract:Rationale:Ovarian yolk sac tumors (YSTs) are the second most common histologic type of ovarian germ cell tumors. Most patients are adolescent and young women, while cases in postmenopausal women were rarely reported. Due to its rarity, we know little about the treatment and prognosis of postmenopausal patients with ovarian YSTs. We reported 3 cases of mixed ovarian YST in postmenopausal females reviewed the related current English literature.Patient concerns:The ages of the three patients were 61, 58 and 77 respectively. The three patients came to the hospital because of the abdominal discomfort or tenderness, and the third patient also has vaginal bleeding.Diagnoses:Imaging examination revealed pelvic mass with cystic and solid components. The elevated serum AFP level and pathologcial examination confirmed mixed ovarian YST.Interventions:All patients received surgery and chemotherapy. Two patients received PEB (cisplatin, etoposide, and bleomycin) chemotherapy initially and one patient received TC (paclitaxel carboplatin) chemotherapy.Outcomes:One patient relapsed 8 months after diagnosis and underwent re-cytoreductive surgery. The three patients all survived at last follow-up.Lessons:The diagnosis of postmenopausal ovarian YST is relatively difficult and it can coexist with other germ cell or epithelial tumors. Postmenopausal ovarian YSTs are aggressive, and may have a worse prognosis compared with those in young patients. More aggressive treatment is needed. When YST mixed with epithelial cancer components, adjuvant chemotherapy regimen should include platinum-based chemotherapy aiming at both epithelial ovarian cancer and germ cell tumors.
Ruptured primary ovarian pregnancy: A case report with a literature review

Abstract:Rationale: Ovarian pregnancy is a rare form of ectopic pregnancy, accounting for 0.5% to 3% of the total number of ectopic pregnancies. Its diagnostic rate is very low and it can be easily misdiagnosed before laparoscopy, due to the clinical presentation being very similar to tubal pregnancy. The ovarian blood supply is abundant, and in case of rupture of ovarian pregnancy, intra-abdominal hemorrhage or even hemorrhagic shock may occur, endangering the patient's life. We report a case of ruptured primary ovarian pregnancy through natural conception. Patient concerns: This patient had a history of menopause with lower abdominal pain and tenderness. Ultrasound showed a thick-walled cystic echo in the left adnexal region, a dark area of fluid in the pelvis, and an irregular, slightly strong echo posterior to the uterus. Unclotted blood was punctured from the posterior fornix, and her hemoglobin was decreasing with a serum β-human chorionic gonadotropin of 1800.00 mIU/mL. Diagnoses: Through early recognition of clinical manifestations, ultrasonography, laparoscopic exploration, and the final histopathologic examination, this patient was diagnosed with an ovarian pregnancy. Interventions: Then, removal of the left ovarian pregnancy lesion was performed, which was visible as villi. And Methotrexate 50 mg was administered locally. Outcomes: Through conservative surgical treatment, she recovered well and was discharged with a satisfactory follow-up. Lessons: Gynecologists should be alert to patients with menopausal lower abdominal pain with or without vaginal bleeding and consider ectopic pregnancy in rare sites, such as ovarian pregnancy. Surgery is the mainstay of treatment, and early laparoscopic exploration may be beneficial in clarifying the diagnosis and performing the concurrent surgical treatment.
Paraneoplastic Internal Jugular Vein Thrombosis Leading to Diagnosis of Bilateral Ovarian Ependymoma

Abstract:Ovarian ependymomas are extremely rare tumors of the ovary. We present a case of a 67-year-old lady presented to us with swelling in the right side of neck for 2 months followed by pelvic pain, lower abdominal distention, and weight loss for 1 month. Her coagulation profile, blood chemistry, lipid profile, and tumor markers were within normal limits. Neck Doppler ultrasonography revealed thrombus in the right internal jugular vein and CT scan of the abdomen showed bilateral ovarian masses. Patient was subjected to debulking surgery for suspected ovarian cancer and microscopy revealed a highly cellular tumor composed of small cells with hyperchromatic, round-to-oval nuclei with scanty cytoplasm, and perivascular pseudorosettes. Diagnosis was confirmed by immunophenotype showing strong positivity to glial fibrillary acidic protein, estrogen receptors, and progesterone receptors. Patient was successfully managed with anticoagulants, adjuvant chemotherapy with BEP regimen, and letrozole. After ruling out other common conditions for thrombosis in this age group, this seems to be a paraneoplastic presentation of ovarian malignancy that preceded the diagnosis of ependymoma by 2 months. To the best of our knowledge this is the first case report in the world literature as “paraneoplastic internal jugular vein thrombosis leading to diagnosis of bilateral ovarian ependymoma.”
