Abstract:Although accounting for only 10% of all Parkinson’s disease (PD) cases, familial forms have contributed substantially to our understanding of its molecular pathophysiology. Among known autosomal-recessive forms of PD, several genes have been implicated in mitophagy, the process of degradation of dysfunctional mitochondria. The activation of the E3 ubiquitin ligase parkin (PARK2 [mendelian inheritance in man (MIM):602544]) is dependent on phosphatase and tensin homolog-induced kinase 1 (PINK1 [MIM: 608309]) and eventually leads to the mitophagy of reactive oxygen species (ROS)-producing mitochondria. Mutation of parkin and PINK1 in turn lead to the accumulation of ROS and dysfunctional mitochondria and ultimately cell damage and degeneration. DJ-1 (MIM:602533), among other roles, acts as a transcription factor influencing autophagy and mitochondrial dysfunction. Recently, Lesage and colleagues described homozygous and compound heterozygous mutations in vacuolar protein sorting 13C (VPS13C [MIM:608879]) as another monogenic, autosomal recessive cause of PD. The authors used a combination of whole-genome homozygosity linkage mapping in patients with confirmed consanguinity (inbreeding coefficient F 61⁄4 0) and 10-30x whole-exome sequencing and comparison to more than 500 healthy controls for gene identification. Clinically, the 3 identified subjects presented with an asymmetric bradykinetic-rigid syndrome with initially preserved Levodopa response but aggressive degenerative course and early cognitive decline similar to other recessive forms of PD. Postmortem analysis of 1 case revealed combined Lewy body and TAU pathology. Subsequent cell culture experiments on human HEK293T and nonhuman primate COS-7 cells verified the location of VSP13C on the mitochondrial surface and demonstrated its influence on mitochondrial activity and maintenance as well as multiple interdependent loops between parkin, PINK1, and VPS13C in the regulation of mitophagy. The involvement of vacuolar protein sorting family proteins in vesicular trafficking suggests that VSP13C might be involved in peroxisomal transport. Observational and experimental data in animal models and humans on the effect of MPTP, rotenone, and paraquat has long suggested a causal relationship between complex I inhibition and PD. Similarly, axonal dystrophy in PLA2G6 has recently been attributed to mitochondrial dysfunction. Strengthened by prior genetic work in familial PD, the discovery of VPS13C continues a long line of evidence that links 2 fundamental cell functions in the pathophysiology of PD: energy metabolism and degradation of dysfunctional cell organelles. At the same time, it further strengthens the role of PINK1/parkin-dependent regulation of mitophagy.
