Abstract:To develop a discrimination pipeline concerning both radiomics and spatial distribution features of brain lesions for discrimination of multiple sclerosis (MS), aquaporin-4-IgG-seropositive neuromyelitis optica spectrum disorder (NMOSD), and myelin-oligodendrocyte-glycoprotein-IgG-associated disorder (MOGAD). Hyperintensity T2 lesions were delineated in 212 brain MRI scans of MS (n = 63), NMOSD (n = 87), and MOGAD (n = 45) patients. To avoid the effect of fixed training/test dataset sampling when developing machine learning models, patients were allocated into 4 sub-groups for cross-validation. For each scan, 351 radiomics and 27 spatial distribution features were extracted. Three models, i.e., multi-lesion radiomics, spatial distribution, and joint models, were constructed using random forest and logistic regression algorithms for differentiating: MS from the others (MS models) and MOGAD from NMOSD (MOG-NMO models), respectively. Then, the joint models were combined with demographic characteristics (i.e., age and sex) to create MS and MOG-NMO discriminators, respectively, based on which a three-disease discrimination pipeline was generated and compared with radiologists. For classification of both MS-others and MOG-NMO, the joint models performed better than radiomics or spatial distribution model solely. The MS discriminator achieved AUC = 0.909 ± 0.027 and bias-corrected C-index = 0.909 ± 0.027, and the MOG-NMO discriminator achieved AUC = 0.880 ± 0.064 and bias-corrected C-index = 0.883 ± 0.068. Abstract:Background: Differential diagnosis of demyelinating diseases of the central nervous system is a challenging task that is prone to errors and inconsistent reading, requiring expertise and additional examination approaches. Advancements in deep-learning-based image interpretations allow for prompt and automated analyses of conventional magnetic resonance imaging (MRI), which can be utilized in classifying multi-sequence MRI, and thus may help in subsequent treatment referral. Methods: Imaging and clinical data from 290 patients diagnosed with demyelinating diseases from August 2013 to October 2021 were included for analysis, including 67 patients with multiple sclerosis (MS), 162 patients with aquaporin 4 antibody-positive (AQP4+) neuromyelitis optica spectrum disorder (NMOSD), and 61 patients with myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD). Considering the heterogeneous nature of lesion size and distribution in demyelinating diseases, multi-modal MRI of brain and/or spinal cord were utilized to build the deep-learning model. This novel transformer-based deep-learning model architecture was designed to be versatile in handling with multiple image sequences (coronal T2-weighted and sagittal T2-fluid attenuation inversion recovery) and scanning locations (brain and spinal cord) for differentiating among MS, NMOSD, and MOGAD. Model performances were evaluated using the area under the receiver operating curve (AUC) and the confusion matrices measurements. The classification accuracy between the fusion model and the neuroradiological raters was also compared. Results: The fusion model that was trained with combined brain and spinal cord MRI achieved an overall improved performance, with the AUC of 0.933 (95%CI: 0.848, 0.991), 0.942 (95%CI: 0.879, 0.987) and 0.803 (95%CI: 0.629, 0.949) for MS, AQP4+ NMOSD, and MOGAD, respectively. This exceeded the performance using the brain or spinal cord MRI alone for the identification of the AQP4+ NMOSD (AUC of 0.940, brain only and 0.689, spinal cord only) and MOGAD (0.782, brain only and 0.714, spinal cord only). In the multi-category classification, the fusion model had an accuracy of 81.4%, which was significantly higher compared to rater 1 (64.4%, p=0.04<0.05) and comparable to rater 2 (74.6%, p=0.388). Conclusion: The proposed novel transformer-based model showed desirable performance in the differentiation of MS, AQP4+ NMOSD, and MOGAD on brain and spinal cord MRI, which is comparable to that of neuroradiologists. Our model is thus applicable for interpretating conventional MRI in the differential diagnosis of demyelinating diseases with overlapping lesions.

Abstract:Objective. The mechanisms driving multiple sclerosis (MS) are still largely unknown, calling for new methods allowing to detect and characterize tissue degeneration since the early stages of the disease. Our aim is to decrypt the microstructural signatures of the Primary Progressive versus the Relapsing-Remitting state of disease based on diffusion and structural magnetic resonance imaging data. Approach. A selection of microstructural descriptors, based on the 3D-Simple Harmonics Oscillator Based Reconstruction and Estimation and the set of new algebraically independent Rotation Invariant spherical harmonics Features, was considered and used to feed convolutional neural networks (CNNs) models. Classical measures derived from diffusion tensor imaging, that are fractional anisotropy and mean diffusivity, were used as benchmark for diffusion MRI (dMRI). Finally, T1-weighted images were also considered for the sake of comparison with the state-of-the-art. A CNN model was fit to each feature map and layerwise relevance propagation (LRP) heatmaps were generated for each model, target class and subject in the test set. Average heatmaps were calculated across correctly classified patients and size-corrected metrics were derived on a set of regions of interest to assess the LRP contrast between the two classes. Main results. Our results demonstrated that dMRI features extracted in grey matter tissues can help in disambiguating primary progressive multiple sclerosis from relapsing-remitting multiple sclerosis patients and, moreover, that LRP heatmaps highlight areas of high relevance which relate well with what is known from literature for MS disease. Significance. Within a patient stratification task, LRP allows detecting the input voxels that mostly contribute to the classification of the patients in either of the two classes for each feature, potentially bringing to light hidden data properties which might reveal peculiar disease-state factors.

Abstract:Multiple sclerosis (MS) and neuromyelitis optica spectrum disorder (NMOSD) are autoimmune inflammatory disorders of the central nervous system (CNS) with similar characteristics. The differential diagnosis between MS and NMOSD is critical for initiating early effective therapy. In this study, we developed a deep learning model to differentiate between multiple sclerosis (MS) and neuromyelitis optica spectrum disorder (NMOSD) using brain magnetic resonance imaging (MRI) data. The model was based on a modified ResNet18 convolution neural network trained with 5-channel images created by selecting five 2D slices of 3D FLAIR images. The accuracy of the model was 76.1%, with a sensitivity of 77.3% and a specificity of 74.8%. Positive and negative predictive values were 76.9% and 78.6%, respectively, with an area under the curve of 0.85. Application of Grad-CAM to the model revealed that white matter lesions were the major classifier. This compact model may aid in the differential diagnosis of MS and NMOSD in clinical practice.

Abstract:Background The diagnosis of multiple sclerosis (MS) relies heavily on neuroimaging with magnetic resonance imaging (MRI) and exclusion of mimics. This can be a challenging task due to radiological overlap in several disorders and may require ancillary testing or longitudinal follow up. One of the most common radiological MS mimickers is non-specific white matter disease (NSWMD). We aimed to develop and evaluate models leveraging machine learning algorithms to help distinguish MS and NSWMD. Methods All adult patients who underwent MRI brain using a demyelinating protocol with available electronic medical records between 2015 and 2019 at Cleveland Clinic affiliated facilities were included. Diagnosis of MS and NSWMD were assessed from clinical documentation. Those with a diagnosis of MS and NSWMD were matched using total T2 lesion volume (T2LV) and used to train models with logistic regression and convolutional neural networks (CNN). Performance metrices were reported for each model. Results A total of 250 NSWMD MRI scans were identified, and 250 unique MS MRI scans were matched on T2LV. Cross validated logistic regression model was able to use 20 variables (including spinal cord area, regional volumes, and fractions) to predict MS compared to NSWMD with 68.0% accuracy while the CNN model was able to classify MS compared to NSWMD in two independent validation and testing cohorts with 77% and 78% accuracy on average. Conclusion Automated methods can be used to differentiate MS compared to NSWMD. These methods can be used to supplement currently available diagnostic tools for patients being evaluated for MS.

Abstract:FROM an academic viewpoint, the outpatient department of a typical teaching hospital has not been "where the action is". It has functioned under a formidable list of handicaps. Its patients have generally been medically in¬ digent, without reasonable alternative sources of medical care, and have presented themselves in large numbers. A teaching hospital cannot ignore service to patients. Indeed, it must be prepared to give exemplary service, because the reason for its existence is to teach undergradu¬ ate and graduate students of medicine to provide such service. But if a teaching hospital, or a part of a teaching hospital, is service-oriented to the extent that it cannot control the number of patients within the limits its teachers consider optimal for educational purposes, the institution is doomed from the start. There have also been other difficulties. The outpatient department was apt to have insufficient money to provide optimal care for its patients, and in making the best possible compromises to provide maximal service to the most patients for the least money, it almost inevitably created an unpleasant and crowded environment. This repelled the patient and discouraged and frustrated even the most dedicated clinical teacher and his students. Cer¬ tainly the clinical teacher could scarcely make the outpatient clinic his base of operations, for he was unlikely to bring his private patients there. All in all, it is fair to say that an outpatient teaching appointment has not been regarded as an academic plum; and the embryo academic physician or surgeon might be forgiven if he looked upon such an appointment as a bit of white water he had to shoot before he could reach the quiet, deep and academically satisfying waters of an in-patient service of his own. Abstract:As an autoimmune-mediated inflammatory demyelinating disease of the central nervous system, multiple sclerosis (MS) is often confused with cerebral small vessel disease (cSVD), which is a regional pathological change in brain tissue with unknown pathogenesis. This is due to their similar clinical presentations and imaging manifestations. That misdiagnosis can significantly increase the occurrence of adverse events. Delayed or incorrect treatment is one of the most important causes of MS progression. Therefore, the development of a practical diagnostic imaging aid could significantly reduce the risk of misdiagnosis and improve patient prognosis. We propose an interpretable deep learning (DL) model that differentiates MS and cSVD using T2-weighted fluid-attenuated inversion recovery (FLAIR) images. Transfer learning (TL) was utilized to extract features from the ImageNet dataset. This pioneering model marks the first of its kind in neuroimaging, showing great potential in enhancing differential diagnostic capabilities within the field of neurological disorders. Our model extracts the texture features of the images and achieves more robust feature learning through two attention modules. The attention maps provided by the attention modules provide model interpretation to validate model learning and reveal more information to physicians. Finally, the proposed model is trained end-to-end using focal loss to reduce the influence of class imbalance. The model was validated using clinically diagnosed MS () and cSVD () patients from the Beijing Tiantan Hospital. The performance of the proposed model was better than that of two commonly used DL approaches, with a mean balanced accuracy of 86.06% and a mean area under the receiver operating characteristic curve of 98.78%. Moreover, the generated attention heat maps showed that the proposed model could focus on the lesion signatures in the image. The proposed model provides a practical diagnostic imaging aid for the use of routinely available imaging techniques such as magnetic resonance imaging to classify MS and cSVD by linking DL to human brain disease. We anticipate a substantial improvement in accurately distinguishing between various neurological conditions through this novel model.

Abstract:The detection of contrast-enhancing lesions (CELs) is fundamental for the diagnosis and monitoring of patients with multiple sclerosis (MS). This task is time-consuming and suffers from high intra- and inter-rater variability in clinical practice. However, only a few studies proposed automatic approaches for CEL detection. This study aimed to develop a deep learning model that automatically detects and segments CELs in clinical Magnetic Resonance Imaging (MRI) scans. A 3D UNet-based network was trained with clinical MRI from the Swiss Multiple Sclerosis Cohort. The dataset comprised 372 scans from 280 MS patients: 162 showed at least one CEL, while 118 showed no CELs. The input dataset consisted of T1-weighted before and after gadolinium injection, and FLuid Attenuated Inversion Recovery images. The sampling strategy was based on a white matter lesion mask to confirm the existence of real contrast-enhancing lesions. To overcome the dataset imbalance, a weighted loss function was implemented. The Dice Score Coefficient and True Positive and False Positive Rates were 0.76, 0.93, and 0.02, respectively. Based on these results, the model developed in this study might well be considered for clinical decision support.

Abstract:The current multiple sclerosis (MS) diagnostic criteria lack specificity, and this may lead to misdiagnosis, which remains an issue in present-day clinical practice. In addition, conventional biomarkers only moderately correlate with MS disease progression. Recently, advanced MS lesional imaging biomarkers such as cortical lesions (CL), the central vein sign (CVS), and paramagnetic rim lesions (PRL), visible in specialized magnetic resonance imaging (MRI) sequences, have shown higher specificity in differential diagnosis. Moreover, studies have shown that CL and PRL are potential prognostic biomarkers, the former correlating with cognitive impairments and the latter with early disability progression. As machine learning-based methods have achieved extraordinary performance in the assessment of conventional imaging biomarkers, such as white matter lesion segmentation, several automated or semi-automated methods have been proposed for CL, CVS, and PRL as well. In the present review, we first introduce these advanced MS imaging biomarkers and their imaging methods. Subsequently, we describe the corresponding machine learning-based methods that were used to tackle these clinical questions, putting them into context with respect to the challenges they are still facing, including non-standardized MRI protocols, limited datasets, and moderate inter-rater variability. We conclude by presenting the current limitations that prevent their broader deployment and suggesting future research directions.

3D Compressed Convolutional Neural Network Differentiates Neuromyelitis Optical Spectrum Disorders From Multiple Sclerosis Using Automated White Matter Hyperintensities Segmentations

Abstract:Background Magnetic resonance imaging (MRI) has a wide range of applications in medical imaging. Recently, studies based on deep learning algorithms have demonstrated powerful processing capabilities for medical imaging data. Previous studies have mostly focused on common diseases that usually have large scales of datasets and centralized the lesions in the brain. In this paper, we used deep learning models to process MRI images to differentiate the rare neuromyelitis optical spectrum disorder (NMOSD) from multiple sclerosis (MS) automatically, which are characterized by scattered and overlapping lesions. Methods We proposed a novel model structure to capture 3D MRI images’ essential information and converted them into lower dimensions. To empirically prove the efficiency of our model, firstly, we used a conventional 3-dimensional (3D) model to classify the T2-weighted fluid-attenuated inversion recovery (T2-FLAIR) images and proved that the traditional 3D convolutional neural network (CNN) models lack the learning capacity to distinguish between NMOSD and MS. Then, we compressed the 3D T2-FLAIR images by a two-view compression block to apply two different depths (18 and 34 layers) of 2D models for disease diagnosis and also applied transfer learning by pre-training our model on ImageNet dataset. Results We found that our models possess superior performance when our models were pre-trained on ImageNet dataset, in which the models’ average accuracies of 34 layers model and 18 layers model were 0.75 and 0.725, sensitivities were 0.707 and 0.708, and specificities were 0.759 and 0.719, respectively. Meanwhile, the traditional 3D CNN models lacked the learning capacity to distinguish between NMOSD and MS. Conclusion The novel CNN model we proposed could automatically differentiate the rare NMOSD from MS, especially, our model showed better performance than traditional3D CNN models. It indicated that our 3D compressed CNN models are applicable in handling diseases with small-scale datasets and possess overlapping and scattered lesions.

Deep Learning for Accurate Diagnosis of Liver Tumor Based on Magnetic Resonance Imaging and Clinical Data

Abstract:Background:Early-stage diagnosis and treatment can improve survival rates of liver cancer patients. Dynamic contrast-enhanced MRI provides the most comprehensive information for differential diagnosis of liver tumors. However, MRI diagnosis is affected by subjective experience, so deep learning may supply a new diagnostic strategy. We used convolutional neural networks (CNNs) to develop a deep learning system (DLS) to classify liver tumors based on enhanced MR images, unenhanced MR images, and clinical data including text and laboratory test results. Methods:Using data from 1,210 patients with liver tumors (N= 31,608 images), we trained CNNs to get seven-way classifiers, binary classifiers, and three-way malignancy-classifiers (Model A-Model G). Models were validated in an external independent extended cohort of 201 patients (N= 6,816 images). The area under receiver operating characteristic (ROC) curve (AUC) were compared across different models. We also compared the sensitivity and specificity of models with the performance of three experienced radiologists. Results:Deep learning achieves a performance on par with three experienced radiologists on classifying liver tumors in seven categories. Using only unenhanced images, CNN performs well in distinguishing malignant from benign liver tumors (AUC, 0.946; 95% CI 0.914-0.979 vs. 0.951; 0.919-0.982,P= 0.664). New CNN combining unenhanced images with clinical data greatly improved the performance of classifying malignancies as hepatocellular carcinoma (AUC, 0.985; 95% CI 0.960-1.000), metastatic tumors (0.998; 0.989-1.000), and other primary malignancies (0.963; 0.896-1.000), and the agreement with pathology was 91.9%.These models mined diagnostic information in unenhanced images and clinical data by deep-neural-network, which were different to previous methods that utilized enhanced images. The sensitivity and specificity of almost every category in these models reached the same high level compared to three experienced radiologists. Conclusion:Trained with data in various acquisition conditions, DLS that integrated these models could be used as an accurate and time-saving assisted-diagnostic strategy for liver tumors in clinical settings, even in the absence of contrast agents. DLS therefore has the potential to avoid contrast-related side effects and reduce economic costs associated with current standard MRI inspection practices for liver tumor patients.
Global and Regional Deep Learning Models for Multiple Sclerosis Stratification From MRI

Abstract:Background: The combination of anatomical MRI and deep learning-based methods such as convolutional neural networks (CNNs) is a promising strategy to build predictive models of multiple sclerosis (MS) prognosis. However, studies assessing the effect of different input strategies on model's performance are lacking. Purpose: To compare whole-brain input sampling strategies and regional/specific-tissue strategies, which focus on a priori known relevant areas for disability accrual, to stratify MS patients based on their disability level. Study type: Retrospective. Subjects: Three hundred nineteen MS patients (382 brain MRI scans) with clinical assessment of disability level performed within the following 6 months (~70% training/~15% validation/~15% inference in-house dataset) and 440 MS patients from multiple centers (independent external validation cohort). Field strength/sequence: Single vendor 1.5 T or 3.0 T. Magnetization-Prepared Rapid Gradient-Echo and Fluid-Attenuated Inversion Recovery sequences. Assessment: A 7-fold patient cross validation strategy was used to train a 3D-CNN to classify patients into two groups, Expanded Disability Status Scale score (EDSS) ≥ 3.0 or EDSS < 3.0. Two strategies were investigated: 1) a global approach, taking the whole brain volume as input and 2) regional approaches using five different regions-of-interest: white matter, gray matter, subcortical gray matter, ventricles, and brainstem structures. The performance of the models was assessed in the in-house and the independent external cohorts. Statistical tests: Balanced accuracy, sensitivity, specificity, area under receiver operating characteristic (ROC) curve (AUC). Results: With the in-house dataset, the gray matter regional model showed the highest stratification accuracy (81%), followed by the global approach (79%). In the external dataset, without any further retraining, an accuracy of 72% was achieved for the white matter model and 71% for the global approach. Data conclusion: The global approach offered the best trade-off between internal performance and external validation to stratify MS patients based on accumulated disability. Abstract:Multiple Sclerosis (MS) is a degenerative neurological disease caused by damage to the central nervous system's axons and myelin sheaths. MS lesions alter shape, position, and size over time in patients, therefore radiologists must be vigilant in detecting and evaluating MS lesions appropriately. Magnetic resonance imaging (MRI) has gotten a lot of attention from doctors for diagnosing MS, but there are other ways as well. The information provided by MRI modalities to doctors about the brain's anatomy and function is critical for making an MS diagnosis quickly. For automatic extraction of MS lesions from three-dimensional (3D) MR images, this work introduces a new feature selection approach. The approach described here can be used to treat a variety of MS lesions. MS MRI diagnosis takes a long time, is difficult, and is prone to human mistake. The design of a Computer-Aided Diagnosis System (CADS) based on Artificial Intelligence (AI) to diagnose MS incorporates standard machine learning and deep learning approaches. Traditional machine learning uses trial and error to extract, select, and classify features. Deep learning, on the other hand, uses deep layers with values that are automatically learned. In this research work, a Priority based Apposite Feature Extraction Model with Image Segmentation (PAFEM-IS) is proposed for segmentation and feature extraction. With proposed method, a large number of image attributes can be learned with little effort and bias on the part of the user. Apart from that, by using unlabelled data for feature learning, the classifier training can benefit from the significantly larger amount of generally available unlabelled data. The proposed model is compared with the traditional models and the proposed model exhibits better performance levels.

Exploration of Neuroprotective Therapy

Abstract:Aim: Up to date many successful attempts to identify various types of lesions with machine learning (ML) were made, however, the recognition of Alzheimer’s disease (AD) from brain images and interpretation of the models is still a topic for the research. Here, using AD Imaging Initiative (ADNI) structural magnetic resonance imaging (MRI) brain images, the scope of this work was to find an optimal artificial neural network architecture for multiclass classification in AD, circumventing the dozens of images pre-processing steps and avoiding to increase the computational complexity. Methods: For this analysis, two supervised deep neural network (DNN) models were used, a three-dimensional 16-layer visual geometry-group (3D-VGG-16) standard convolutional network (CNN) and a three-dimensional residual network (ResNet3D) on the T1-weighted, 1.5 T ADNI MRI brain images that were divided into three groups: cognitively normal (CN), mild cognitive impairment (MCI), and AD. The minimal pre-processing procedure of the images was applied before training the two networks. Results: Results achieved suggest, that the network ResNet3D has a better performance in class prediction, which is higher than 90% in training set accuracy and arrives to 85% in validation set accuracy. ResNet3D also showed requiring less computational power than the 3D-VGG-16 network. The emphasis is also given to the fact that this result was achieved from raw images, applying minimal image preparation for the network. Conclusions: In this work, it has been shown that ResNet3D might have superiority over the other CNN models in the ability to classify high-complexity images. The prospective stands in doing a step further in creating an expert system based on residual DNNs for better brain image classification performance in AD detection.
Automated Classification of Intramedullary Spinal Cord Tumors and Inflammatory Demyelinating Lesions Using Deep Learning

Abstract:Accurate differentiation of intramedullary spinal cord tumors and inflammatory demyelinating lesions and their subtypes are warranted because of their overlapping characteristics at MRI but with different treatments and prognosis. The authors aimed to develop a pipeline for spinal cord lesion segmentation and classification using two-dimensional MultiResUNet and DenseNet121 networks based on T2-weighted images. A retrospective cohort of 490 patients (118 patients with astrocytoma, 130 with ependymoma, 101 with multiple sclerosis [MS], and 141 with neuromyelitis optica spectrum disorders [NMOSD]) was used for model development, and a prospective cohort of 157 patients (34 patients with astrocytoma, 45 with ependymoma, 33 with MS, and 45 with NMOSD) was used for model testing. In the test cohort, the model achieved Dice scores of 0.77, 0.80, 0.50, and 0.58 for segmentation of astrocytoma, ependymoma, MS, and NMOSD, respectively, against manual labeling. Accuracies of 96% (area under the receiver operating characteristic curve [AUC], 0.99), 82% (AUC, 0.90), and 79% (AUC, 0.85) were achieved for the classifications of tumor versus demyelinating lesion, astrocytoma versus ependymoma, and MS versus NMOSD, respectively. In a subset of radiologically difficult cases, the classifier showed an accuracy of 79%-95% (AUC, 0.78-0.97). The established deep learning pipeline for segmentation and classification of spinal cord lesions can support an accurate radiologic diagnosis. Abstract:This study aims to automatically detect multiple sclerosis (MS) in terms of the changes in retinal vessels using Scanning laser ophthalmoscopy (SLO) images. Although much research has been done to diagnose MS patients, these diagnostic techniques have always been based on using Magnetic resonance imaging (MRI) images which cannot be a complete technique in diagnosing this disease. Using SLO images and examining the condition of its vessels using computer technology, biomarkers in the vessel can be identified to help diagnose MS patients. However, in the first step, the color images are converted to gray and after that are improved using a combination of algorithm Tylor Coye and DWT, then, the images are segmented and retinal vessels are extracted. Besides, two different techniques are used in classification stage. In the first technique, classic Machine learning different features are extracted from the resulting regions and entered into several multiple classifiers, the results of which give us an accuracy of 72%, moreover in the second technique segmented images enter the transfer learning model and ultimately lead us to 98% accuracy in the distinction between MS patients and Healthy Controls (HCs).

Abstract:This study aims to develop an AI-enhanced methodology for the expedited and accurate diagnosis of Multiple Sclerosis (MS), a chronic disease affecting the central nervous system leading to progressive impairment. Traditional diagnostic methods are slow and require substantial expertise, underscoring the need for innovative solutions. Our approach involves two phases: initially, extracting features from brain MRI images using first-order histograms, the gray level co-occurrence matrix, and local binary patterns. A unique feature selection technique combining the Sine Cosine Algorithm with the Sea-horse Optimizer is then employed to identify the most significant features. Utilizing the eHealth lab dataset, which includes images from 38 MS patients (mean age 34.1 ± 10.5 years; 17 males, 21 females) and matched healthy controls, our model achieved a remarkable 97.97% detection accuracy using the k-nearest neighbors classifier. Further validation on a larger dataset containing 262 MS cases (199 females, 63 males; mean age 31.26 ± 10.34 years) and 163 healthy individuals (109 females, 54 males; mean age 32.35 ± 10.30 years) demonstrated a 92.94% accuracy for FLAIR images and 91.25% for T2-weighted images with the Random Forest classifier, outperforming existing MS detection methods. These results highlight the potential of the proposed technique as a clinical decision-making tool for the early identification and management of MS.

Shallow vs deep learning architectures for white matter lesion segmentation in the early stages of multiple sclerosis

Abstract:In this work, we present a comparison of a shallow and a deep learning architecture for the automated segmentation of white matter lesions in MR images of multiple sclerosis patients. In particular, we train and test both methods on early stage disease patients, to verify their performance in challenging conditions, more similar to a clinical setting than what is typically provided in multiple sclerosis segmentation challenges. Furthermore, we evaluate a prototype naive combination of the two methods, which refines the final segmentation. All methods were trained on 32 patients, and the evaluation was performed on a pure test set of 73 cases. Results show low lesion-wise false positives (30%) for the deep learning architecture, whereas the shallow architecture yields the best Dice coefficient (63%) and volume difference (19%). Combining both shallow and deep architectures further improves the lesion-wise metrics (69% and 26% lesion-wise true and false positive rate, respectively).

Abstract:Background: Identifying active lesions in magnetic resonance imaging (MRI) is crucial for the diagnosis and treatment planning of multiple sclerosis (MS). Active lesions on MRI are identified following the administration of Gadolinium-based contrast agents (GBCAs). However, recent studies have reported that repeated administration of GBCA results in the accumulation of Gd in tissues. In addition, GBCA administration increases health care costs. Thus, reducing or eliminating GBCA administration for active lesion detection is important for improved patient safety and reduced healthcare costs. Current state-of-the-art methods for identifying active lesions in brain MRI without GBCA administration utilize data-intensive deep learning methods. Objective: To implement nonlinear dimensionality reduction (NLDR) methods, locally linear embedding (LLE) and isometric feature mapping (Isomap), which are less data-intensive, for automatically identifying active lesions on brain MRI in MS patients, without the administration of contrast agents. Materials and Methods: Fluid-attenuated inversion recovery (FLAIR), T2-weighted, proton density-weighted, and pre- and post-contrast T1-weighted images were included in the multiparametric MRI dataset used in this study. Subtracted pre- and post-contrast T1-weighted images were labeled by experts as active lesions (ground truth). Unsupervised methods, LLE and Isomap, were used to reconstruct multiparametric brain MR images into a single embedded image. Active lesions were identified on the embedded images and compared with ground truth lesions. The performance of NLDR methods was evaluated by calculating the Dice similarity (DS) index between the observed and identified active lesions in embedded images. Results: LLE and Isomap, were applied to 40 MS patients, achieving median DS scores of 0.74 ± 0.1 and 0.78 ± 0.09, respectively, outperforming current state-of-the-art methods. Conclusions: NLDR methods, Isomap and LLE, are viable options for the identification of active MS lesions on non-contrast images, and potentially could be used as a clinical decision tool.

Revolutionizing early Alzheimer's disease and mild cognitive impairment diagnosis: a deep learning MRI meta-analysis

Abstract:Background: The early diagnosis of Alzheimer's disease (AD) and mild cognitive impairment (MCI) remains a significant challenge in neurology, with conventional methods often limited by subjectivity and variability in interpretation. Integrating deep learning with artificial intelligence (AI) in magnetic resonance imaging (MRI) analysis emerges as a transformative approach, offering the potential for unbiased, highly accurate diagnostic insights. Objective: A meta-analysis was designed to analyze the diagnostic accuracy of deep learning of MRI images on AD and MCI models. Methods: A meta-analysis was performed across PubMed, Embase, and Cochrane library databases following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, focusing on the diagnostic accuracy of deep learning. Subsequently, methodological quality was assessed using the QUADAS-2 checklist. Diagnostic measures, including sensitivity, specificity, likelihood ratios, diagnostic odds ratio, and area under the receiver operating characteristic curve (AUROC) were analyzed, alongside subgroup analyses for T1-weighted and non-T1-weighted MRI. Results: A total of 18 eligible studies were identified. The Spearman correlation coefficient was -0.6506. Meta-analysis showed that the combined sensitivity and specificity, positive likelihood ratio, negative likelihood ratio, and diagnostic odds ratio were 0.84, 0.86, 6.0, 0.19, and 32, respectively. The AUROC was 0.92. The quiescent point of hierarchical summary of receiver operating characteristic (HSROC) was 3.463. Notably, the images of 12 studies were acquired by T1-weighted MRI alone, and those of the other 6 were gathered by non-T1-weighted MRI alone. Conclusion: Overall, deep learning of MRI for the diagnosis of AD and MCI showed good sensitivity and specificity and contributed to improving diagnostic accuracy.
