Abstract:Alzheimer’s disease (AD) is an irreversible, age-related progressive neurological disorder, and the most common type of dementia in aged people. Neuropathological lesions of AD are neurofibrillary tangles (NFTs), and senile plaques comprise the accumulated amyloid-beta (Aβ), loaded with metal ions including Cu, Fe, or Zn. Some reports have identified metal dyshomeostasis as a neurotoxic factor of AD, among which Cu ions seem to be a central cationic metal in the formation of plaque and soluble oligomers, and have an essential role in the AD pathology. Cu-Aβ complex catalyzes the generation of reactive oxygen species (ROS) and results in oxidative damage. Several studies have indicated that oxidative stress plays a crucial role in the pathogenesis of AD. The connection of copper levels in AD is still ambiguous, as some researches indicate a Cu deficiency, while others show its higher content in AD, and therefore there is a need to increase and decrease its levels in animal models, respectively, to study which one is the cause. For more than twenty years, many in vitro studies have been devoted to identifying metals’ roles in Aβ accumulation, oxidative damage, and neurotoxicity. Towards the end, a short review of the modern therapeutic approach in chelation therapy, with the main focus on Cu ions, is discussed. Despite the lack of strong proofs of clinical advantage so far, the conjecture that using a therapeutic metal chelator is an effective strategy for AD remains popular. However, some recent reports of genetic-regulating copper transporters in AD models have shed light on treating this refractory disease. This review aims to succinctly present a better understanding of Cu ions’ current status in several AD features, and some conflicting reports are present herein.

Abstract:We investigated oxidative stress markers and metal ions in patients with Alzheimer’s disease (AD). The serum levels of ceruloplasmin (CER), C-reactive protein (CRP), uric acid (UA), homocysteine (Hcy), copper, iron, and zinc were determined in 125 patients with AD (mild, n = 2 8; moderate, n = 42; and severe, n = 55) and 40 healthy control (HC) participants. Compared to HC, CER and UA levels were significantly lower in moderate and severe AD groups, whereas CRP and Hcy levels were significantly higher in the severe AD group. Copper level was significantly higher in moderate and severe AD groups than the other groups. Compared to HC, iron level was significantly higher in patients with AD, whereas zinc level was significantly lower in patients with AD. In patients with AD, the severity of cognitive impairment was positively correlated with CER, UA, and zinc levels, whereas it was negatively correlated with copper level. Taken together, our findings provide a novel approach to assess AD progression.

Abstract:Background: Alzheimer's disease (AD) is the most common cause of dementia and cognitive decline in the elderly. Although the etiology of AD is unknow, an increase in amyloid precursor protein (APP) leads to the toxic aggregation of Aβ plaques. Several factors, such as hypertension, diabetes, dyslipidemia, smoking, hormonal changes, and metal exposure, could increase the risk of developing AD. In this review, we will examine the role of copper (Cu) in the pathophysiology of AD, as well as the mechanisms involved in neurotoxicity and cognitive decline. Methods: This review was conducted in accordance with PRISMA guidelines. We performed a comprehensive literature analysis over the last ten years on AD and Cu. Only late-onset Alzheimer's disease was considered; only studies on elderly people of both sexes were included. Results: A total of seven articles were picked for this review, three studies focused on non-ceruloplasmin-bound Copper (non-Cp-Cu) and four on ceruloplasmin-bound Copper (Cp-Cu). The results showed higher Cu concentrations in patients compared to healthy controls. Conclusions: Elevated concentrations of Cu may contribute to the progression of AD, potentially interacting with ATP7B mutations, oxidative stress (OS), and amyloid-β plaques. Future research is needed to provide more robust evidence and better characterize the relationship between AD and Cu.

Abstract:Abstract Background The homeostasis of metal ions, such as iron, copper, zinc and calcium, in the brain is crucial for maintaining normal physiological functions. Studies have shown that imbalance of these metal ions in the brain is closely related to the onset and progression of Alzheimer’s disease (AD), the most common neurodegenerative disorder in the elderly. Main body Erroneous deposition/distribution of the metal ions in different brain regions induces oxidative stress. The metal ions imbalance and oxidative stress together or independently promote amyloid-β (Aβ) overproduction by activating β- or γ-secretases and inhibiting α-secretase, it also causes tau hyperphosphorylation by activating protein kinases, such as glycogen synthase kinase-3β (GSK-3β), cyclin-dependent protein kinase-5 (CDK5), mitogen-activated protein kinases (MAPKs), etc., and inhibiting protein phosphatase 2A (PP2A). The metal ions imbalances can also directly or indirectly disrupt organelles, causing endoplasmic reticulum (ER) stress; mitochondrial and autophagic dysfunctions, which can cause or aggravate Aβ and tau aggregation/accumulation, and impair synaptic functions. Even worse, the metal ions imbalance-induced alterations can reversely exacerbate metal ions misdistribution and deposition. The vicious cycles between metal ions imbalances and Aβ/tau abnormalities will eventually lead to a chronic neurodegeneration and cognitive deficits, such as seen in AD patients. Conclusion The metal ions imbalance induces Aβ and tau pathologies by directly or indirectly affecting multiple cellular/subcellular pathways, and the disrupted homeostasis can reversely aggravate the abnormalities of metal ions transportation/deposition. Therefore, adjusting metal balance by supplementing or chelating the metal ions may be potential in ameliorating AD pathologies, which provides new research directions for AD treatment.

The dysregulation of metal homeostasis may cause neurotoxicity and directly addressing these dysregulated metals through metal chelation or mitigating the downstream neurotoxicity stands as a pivotal strategy for AD therapy. This review aims to provide an up-to-date comprehensive overview of the application of metal chelators and drugs targeting metal-related neurotoxicity, such as antioxidants (ferroptotic inhibitors), in the context of AD treatment. It encompasses an exploration of their pharmacological effects, clinical research progress, and potential underlying mechanisms.

[The use of sublingual captopril in hypertensive crises].

Abstract:The purpose of this study was to evaluate the antihypertensive efficacy of captopril (C) and its correlation with body mass index (BMI), age and initial blood pressure (BP) in hypertensive crises. The time of the beginning of the antihypertensive effect of C and its side effects were evaluated. Twenty patients, aged between 22 and 59 years, were included in this investigation. BP was measured after 15 min of rest; age, weight and height were recorded and BMI was calculated using the standard formula (Kg/m2). Patients were not taking antihypertensive medications. After the placebo was given to the patients, BP was measured at 30 and 60 min. Sublingual captopril 25 mg was administered and BP was again measured at 5, 10, 15, 30, 60, 120 y 240 min. There was not statistically significant difference between the values of BP before and after placebo. A significant decrease was observed post C, from 182.6/123.6 to 174.6/117.3 (p:SN/p < 0.02) in 10 min., to 169.9/114.9 (p < 0.04/p < 0.001) in 15 min., to 167.7/112.2 (p < 0.02/p < 0.0001) in 30 min., to 164.3/108.8 (p < 0.002/p < 0.0001) in 60 min., to 156.9/106.5 (p < 0.0001/p < 0.0001) in 120 min., to 161.3/107.0 (p < 0.0002/p < 0.0001) in 240 min. In 15 patients there was a decrease in diastolic BP > or = 5 mmHg, 10 min. post C. No correlation was found between decrease in BP and age, nor with BMI. Pearson r correlation index between the decrease in systolic BP and initial systolic BP was 0.63 (p < 0.003) and between the decrease in diastolic BP and initial diastolic BP, 0.59 (p < 0.005). Secondary effects were mild and well tolerated. In conclusion, C effectively reduced BP in hypertensive crises. Because the efficacy of C, its rapid onset of action and minimal side effects, sublingual Captopril should be considered a first line drug for hypertensive crises.

Abstract:Metal homeostasis in the central nervous system (CNS) is a crucial component of healthy brain function, because metals serve as enzymatic cofactors and are key components of intra- and inter-neuronal signaling. Metal dysregulation wreaks havoc on neural networks via induction and proliferation of pathological pathways that cause oxidative stress, synaptic impairment, and ultimately, cognitive deficits. Thus, exploration of metal biology in relation to neurodegenerative pathology is essential in pursuing novel therapies for Alzheimer’s Disease and other neurodegenerative disorders. This review covers mechanisms of action of aluminum, iron, copper, and zinc ions with respect to the progressive, toxic accumulation of extracellular β-amyloid plaques and intracellular hyperphosphorylated neurofibrillary tau tangles that characterizes Alzheimer’s Disease, with the goal of evaluating the therapeutic potential of metal ion interference in neurodegenerative disease prevention and treatment. As neuroscientific interest in the role of metals in neurodegeneration escalates—in large part due to emerging evidence substantiating the interplay between metal imbalances and neuropathology—it becomes clear that the use of metal chelating agents may be a viable method for ameliorating Alzheimer’s Disease pathology, as its etiology remains obscure. We conclude that, although metal therapies can potentially deter neurodegenerative processes, the most promising treatments will remain elusive until further understanding of neurodegenerative etiology is achieved. New research directions may best be guided by animal models of neurodegeneration, which reveal specific insights into biological mechanisms underlying dementia.

Abstract:Alzheimer' s disease (AD) is a kind of neurodegenerative diseases often found in old people. The characteristic pathological features of Alzheimer' s disease are senile plaques, neurofibrillary tangles, the loss of neurons, and granulovacuolar degeneration. Though the mechanism of Alzheimer' s disease is very complicated and not understood clearly, the metal homeostasis,which amyloid beta, amyloid precursor protein and metallothionein-3 participated in, is associated with the development of Alzheimer' s disease. The transitional metals, such as copper and zinc, play a key role in the physiologic function. Abnormally high levels of metals were found in the brain of Alzheimer' s disease patients compared with the healthy people. The aberrant of metal homeostasis may be one of the reasons that induce Alzheimer' s disease. The imbalance of metal homeostasis in the brain of Alzheimer' s disease leads to the aggregation of amyloid beta and the generation of reactive oxygen species. The copper can generate reactive oxygen species via Fenton type reaction, resulting in toxicity to the cells. Metallothionein-3, as a metal homeostasis regulator, can protect against the neuronal toxicity of A beta by preventing copper-mediated A beta aggregation, abolishing the production of reactive oxygen species(ROS). This review focuses on the AD research progress regarding the metal homeostasis regulation with some related proteins.

Abstract:Alzheimer's disease (AD) is a neurodegenerative disorder characterized by the accumulation of amyloid - β extracellular plaques and tau interfibrillar tangles, leading to memory loss, cognitive decline, and behavioral changes. With dementia posing a growing global health concern, there is an urgent need for comprehensive strategies to address its challenges. The economic burden of dementia is projected to rise significantly, emphasizing the necessity for collaborative efforts in research and healthcare. In the United States alone, millions are affected by AD, with prevalence increasing with age and even affecting younger individuals. The complexity of AD involves intricate biological processes, including the aggregation of amyloid beta, oxidative stress, and metal ion dysregulation. Metal ions, particularly those from copper, iron, and zinc, play pivotal roles in AD pathology, influencing Aβ deposition and tau protein accumulation. Current treatments offer symptomatic relief but do not address the underlying disease mechanisms. This paper explores the potential of various chelating compounds to target metal ions involved in AD pathology. N-acylhydrazones, morpholine, chrysin, quinoline, oxindole, cyclam, catechol-based, and quinazolinone-based derivatives show promising chelation activity and therapeutic effects. Metal chelation therapy offers a targeted approach to AD treatment by addressing the core pathology. By selectively binding to metal ions implicated in disease progression, chelators may minimize side effects associated with broad-spectrum treatments. Additionally, chelators may offer neuroprotective effects beyond metal binding, further enhancing their therapeutic potential. Overall, metal chelation therapy presents a promising strategy in combating AD, with the potential to significantly impact disease progression and improve patient outcomes.

Abstract:Fungal infection is an uncommon cause of peritonitis in patients on continuous ambulatory peritoneal dialysis (CAPD). This report describes the clinical and microbiological features of 10 cases of fungal peritonitis. Although all patients survived, morbidity was high. Abscess and adhesion formation were particular problems. Only two patients were able to return to CAPD after microbiological cure. Currently available treatment strategies for fungal peritonitis are reviewed.

Abstract:Alzheimer's disease (AD) is the most prevalent illness among the neurodegenerative disorders that are progressive, irreversible and incurable so far. Although the etiological mechanism of AD is still not fully understood, considerable evidences support the idea that metals are critically involved as factors or cofactors in the etiopathogenesis of the disease. As a result, metal-involved hypotheses, such as the amyloid cascade hypothesis, the metal ion hypothesis and the oxidative stress hypothesis, are prevailing in the etiology of AD, leading to the development of metal-involved therapeutics. However, current approaches treat patients after first diagnosis using separate therapeutic and diagnostic agents, which is unfavorable for exploring the optimal therapeutic time window and efficacy of the drugs. Theranostics that combines therapy and diagnosis into a single package can efficiently exert the synergistic effect of individual drugs. Thus, as a cutting-edge tool, metal-involved theranostics has promising potential to overcome the weakness of traditional approaches to AD therapy. This review focuses on the reported metal-involved theranostic agents based on metal chelators, metal complexes, and metal nanoparticles. Their design principles, mechanism of action and potential applications are elaborated in detail. The limits and potential directions of this emerging strategy are also discussed with a view to future research.

Expert Opinion on Therapeutic Targets

Abstract:Importance of the field: Increased localization of Zn, Fe, Cu and Al within the senile plaques (SP) exacerbates amyloid beta (A beta)-mediated oxidative damage, and acts as catalyst for A beta aggregation in Alzheimer's disease (AD). Thus, disruption of aberrant metal-peptide interactions via chelation therapy holds considerable promise as a rational therapeutic strategy against Alzheimer's amyloid pathogenesis.Areas covered in this review: The complexities of metal-induced genesis of SP are reviewed. The recent advances in the molecular mechanism of action of metal chelating agents are discussed with critical assessment of their potential to become drugs.What the reader will gain: Taking into consideration the interaction of metals with the metal-responsive elements on the Alzheimer's amyloid precursor protein (APP), readers will gain understanding of several points to bear in mind when developing a screening campaign for AD-therapeutics.Take home message: A functional iron-responsive element (IRE) RNA stem loop in the 5' untranslated region (UTR) of the APP transcript regulates neural APP translation. Desferrioxamine, clioquinol, tetrathiolmolybdate, dimercaptopropanol, VK-28, and natural antioxidants, such as curcumin and ginko biloba need critical evaluation as AD therapeutics. There is a necessity for novel screens (related to metallobiology) to identify therapeutics effective in AD.

Abstract:Alzheimer's disease (AD) has become a leading public health issue due to our nation's burgeoning aging population. These issues are further complicated by the fact that neither preventive measure and effective treatment nor definitive in vivo diagnostic tool for this burdensome disease is currently available. Genetic, biochemical, and neuropathological data argue that Abeta amyloidosis, which originates from the Abeta amyloidogenic processing of a metalloprotein-amyloid precursor protein (APP), is the key event in AD pathology. However, neurochemical factors that impact upon this age-dependent protein disorder in brain are not well recognized. Considerable evidence is mounting that dyshomeostasis of the redox-active biometals, Cu and Fe, and oxidative stress contribute to the neuropathology of Alzheimer's disease (AD). Present data suggest that metals can interact directly with Abeta peptide, the principle component of Abeta amyloid that is one of the primary lesions in AD. The binding of metals to Abeta modulates several physiochemical properties of Abeta that are thought to be central to the pathogenicity of the peptide. First, we and others have shown that metals can promote the in vitro aggregation into tinctorial and disordered protein-Abeta amyloid. Studies have confirmed that insoluble amyloid plaques in post-mortem AD brain are abnormally enriched in Cu, Fe, and Zn. Conversely, metal chelators dissolve these proteinaceous deposits from postmortem AD brain tissue and attenuate cerebral Abeta amyloid burden in APP transgenic mouse models of AD. Second, we have demonstrated that redox-active Cu(II), and to a lesser extent, Fe(III), are reduced in the presence of Abeta with concomitant production of reactive oxygen species (ROS)-hydrogen peroxide (H2O2) and hydroxyl radical (OH.). These Abeta/metal redox reactions, which are silenced by redox-inert Zn(II), but exacerbated by biological reducing agents, may lead directly to the widespread oxidation damages observed in AD brains. Moreover, studies have also shown that H2O2 mediates Abeta cellular toxicity and increases the production of both Abeta and amyloid precursor protein (APP). Third, the 5'untranslated region (5'UTR) of APP mRNA has a functional iron-response element (IRE) which is consistent with biochemical evidence that APP is redox-active metalloprotein. Hence, the redox interactions between Abeta, APP, and metals may be at the heart of a pathological positive feedback system wherein Abeta amyloidosis and oxidative stress promote each other. The emergence of redox-active metals as key players in AD pathogenesis strongly argues that amyloid-specific metal-complexing agents and antioxidants be investigated as possible disease-modifying agents for treating this horrible disease. In addition, the AD diagnosis currently relies on behavior-based tests-e.g., Mini-Mental State Examination (MMSE). However, it is not specific for AD as many factors may lead to memory impairment and cognitive failure. The only definitive diagnosis for AD is the post-mortem examination of brain tissue for cerebral Abeta amyloidosis, the neuropathological hallmark of AD (2,3). Thus, development of in vivo amyloid imaging agents can not only provide definitive AD diagnostic tools but also speed up the preclinical and clinical assessment of novel AD therapeutic agents targeting Abeta amyloid. In summary, study of AD metallobiochemistry cements a solid foundation for discovering novel theranostic agents of AD.

Abstract:Alzheimer's disease is a rapidly worsening public health problem. The current lack of effective treatments for Alzheimer's disease makes it imperative to find new pharmacotherapies. At present, the treatment of symptoms includes use of acetylcholinesterase inhibitors, which enhance acetylcholine levels and improve cognitive functioning. Current reports provide evidence that the pathogenesis of Alzheimer's disease is linked to the characteristic neocortical amyloid-beta deposition, which may be mediated by abnormal metal interaction with A beta as well as metal-mediated oxidative stress. In light of these observations, we have considered the development of drugs that target abnormal metal accumulation and its adverse consequences, as well as prevention or reversal of amyloid-beta plaque formation. This paper reviews recent observations on the possible etiologic role of A beta deposition, its redox activity, and its interaction with transition metals that are enriched in the neocortex. We discuss the effects of metal chelators on these processes, list existing drugs with chelating properties, and explore the promise of this approach as a basis for medicinal chemistry in the development of novel Alzheimer's disease therapeutics.

Second, we have demonstrated that redox-active Cu(II) and, to a lesser extent, Fe(III) are reduced in the presence of Abeta with concomitant production of reactive oxygen species (ROS), hydrogen peroxide (H2O2) and hydroxyl radical (OH.). These Abeta/metal redox reactions, which are silenced by redox-inert Zn(II), but exacerbated by biological reducing agents, may lead directly to the widespread oxidation damages observed in AD brains. Moreover, studies have also shown that H2O2 mediates Abeta cellular toxicity and increases the production of both Abeta and amyloid precursor protein (APP). Third, the 5' untranslated region (5'UTR) of APP mRNA has a functional iron-response element (IRE), which is consistent with biochemical evidence that APP is a redox-active metalloprotein. Hence, the redox interactions between Abeta, APP, and metals may be at the heart of a pathological positive feedback system wherein Abeta amyloidosis and oxidative stress promote each other. The emergence of redox-active metals as key players in AD pathogenesis strongly argues that amyloid-specific metal-complexing agents and antioxidants be investigated as possible disease-modifying agents for treating this horrible disease.
Altered Copper Transport in Oxidative Stress-Dependent Brain Endothelial Barrier Dysfunction Associated with Alzheimer′s Disease

Abstract:Oxidative stress and blood-brain barrier (BBB) disruption due to brain endothelial barrier dysfunction contribute to Alzheimer′s Disease (AD), which is characterized by beta-amyloid (Aβ) accumulation in senile plaques. Copper (Cu) is implicated in AD pathology and its levels are tightly controlled by several Cu transport proteins. However, their expression and role in AD, particularly in relation to brain endothelial barrier function remains unclear. In this study, we examined the expression of Cu transport proteins in the brains of AD mouse models as well as their involvement in Aβ42-induced brain endothelial barrier dysfunction. We found that the Cu uptake transporter CTR1 was upregulated, while the Cu exporter ATP7A and/or ATP7B were downregulated in the hippocampus of AD mouse models, and in Aβ42-treated human brain microvascular endothelial cells (hBMECs). In the 5xFAD AD mouse model, Cu levels (assessed by ICP-MS) were elevated in the hippocampus. Moreover, Aβ42-induced reactive oxygen species (ROS) production, ROS-dependent loss in barrier function in hBMEC (measured by transendothelial electrical resistance), and tyrosine phosphorylation of VE-cadherin were all inhibited by either a membrane permeable Cu chelator or by knocking down CTR1 expression. These findings suggest that dysregulated expression of Cu transport proteins may lead to intracellular Cu accumulation in the AD brain, and that Aβ42 promotes ROS-dependent brain endothelial barrier dysfunction and VE-Cadherin phosphorylation in a CTR1-Cu-dependent manner. Our study uncovers the critical role of Cu transport proteins in oxidative stress-related loss of BBB integrity in AD.

Abstract:Alzheimers disease (AD), which accounts for over 80 percent of senile dementia cases, is a polygenic and multifactorial complex disease. However, an overwhelming body of data suggests that the key to AD pathology lies in the biology of a small protein called the Aβ peptide that forms the proteinaceous deposits known as Aβ amyloid that characterizes AD. Considerable progress has been made in understanding the metabolic pathways that generate and clear Aβ. Less well understood are the neurochemical factors that may be important in promoting age-dependent Aβ amyloidosis, the likely precursor to AD. Findings suggest that cerebral biometal dyshomeostasis, oxidative stress, and microglia immune inflammatory responses are intimately involved in Aβ amyloid deposition and AD pathogenesis. With these new findings has come an increasing awareness of the potential of antioxidant/metal chelation/anti-inflammatory therapies as AD treatments. The major impediment to the use of metal chelators and antioxidants are their unfavorable drug profiles that include poor target specificity and low therapeutic potency. However, future development of new bifunctional and multifunctional molecules that contain both amyloid-binding and metal-chelating/antioxidant moieties, is thought to overcome many of the limitations of previous chelating and antioxidant drugs. Herein we have reviewed the pathogenic roles of brain biometal dysregulation, oxidative stress, and neuroinflammation in AD. We have also discussed the potentials of amyloid-targeted metal chelation, anti-oxidative stress, and anti-inflammation as potential Alzheimers therapies.
