Abstract:Neutrophils have key roles in ischemic brain injury, thrombosis, and atherosclerosis. As such, neutrophils are of great interest as targets to treat and prevent ischemic stroke. After stroke, neutrophils respond rapidly promoting blood–brain barrier disruption, cerebral edema, and brain injury. A surge of neutrophil-derived reactive oxygen species, proteases, and cytokines are released as neutrophils interact with cerebral endothelium. Neutrophils also are linked to the major processes that cause ischemic stroke, thrombosis, and atherosclerosis. Thrombosis is promoted through interactions with platelets, clotting factors, and release of prothrombotic molecules. In atherosclerosis, neutrophils promote plaque formation and rupture by generating oxidized-low density lipoprotein, enhancing monocyte infiltration, and degrading the fibrous cap. In experimental studies targeting neutrophils can improve stroke. However, early human studies have been met with challenges, and suggest that selective targeting of neutrophils may be required. Several properties of neutrophil are beneficial and thus may important to preserve in patients with stroke including antimicrobial, antiinflammatory, and neuroprotective functions.

G-CSF and neuroprotection: a therapeutic perspective in cerebral ischaemia.

Abstract:In several experimental studies of cerebral ischaemia, G-CSF (granulocyte colony-stimulating factor) exerted neuroprotective effects through different mechanisms, including mobilization of haemopoietic stem cells, anti-apoptosis, neuronal differentiation, angiogenesis and anti-inflammation. Hence, G-CSF not only inhibits neuron death, but also generates 'new' neural tissue formation. A small pilot trial reports on the safety and feasibility of G-CSF therapy in stroke patients. According to this evidence, we can speculate that G-CSF, being used either alone or in combination with another agent, should have a dual activity beneficial both to acute neuronal protection and long-term plasticity after cerebral ischaemia, thus proposing that G-CSF is an ideal new drug for stroke and neurodegenerative diseases.
Fibronectin and neuroprotective effect of granulocyte colony-stimulating factor in focal cerebral ischemia.

Abstract:Stroke is one of the leading causes of unnatural death and disability. No effective therapy is available. Recombinant human granulocyte colony-stimulating factor (rhG-CSF), as a mobilizing agent for bone marrow stem cells, can promote stem cell mobilization, homing to brain after cerebral ischemia. In the present study, the administration of G-CSF significantly increased number of CD34+ cells in the marginal zone of the infarction. Rats receiving G-CSF had higher survival rate and lower infarction volume. Neurological behavior was improved, and the expression of fibronectin in the ischemic brain was increased, as compared to rats treated with vehicle. To mimic the ischemia–reperfusion injury in experimental animals, we employed hippocampal slice cultures that were first treated with oxygen and glucose deprivation (OGD) and then with oxygen–glucose resupply, finding that fibronectin significantly increased the neurite outgrowth of OGD hippocampal slices, upregulated the expression of Bcl-2 protein, and ameliorated the ultrastructure damage of OGD hippocampal slices.

Abstract:Granulocyte-colony stimulating factor (G-CSF), a member of the cytokine family of hematopoietic growth factors, is 19.6 kDa glycoprotein which is responsible for the proliferation, maturation, differentiation, and survival of neutrophilic granulocyte lineage. Apart from its proven clinical application to treat chemotherapy-associated neutropenia, recent pre-clinical studies have highlighted the neuroprotective roles of G-CSF i.e., mobilization of haemopoietic stem cells, anti-apoptotic, neuronal differentiation, angiogenesis and anti-inflammatory in animal models of neurological disorders. G-CSF is expressed by numerous cell types including neuronal, immune and endothelial cells. G-CSF is released in autocrine manner and binds to its receptor G-CSF-R which further activates numerous signaling transduction pathways including PI3K/AKT, JAK/STAT and MAP kinase, and thereby promote neuronal survival, proliferation, differentiation, mobilization of hematopoietic stem and progenitor cells. The expression of G-CSF receptors (G-CSF-R) in the different brain regions and their upregulation in response to neuronal insult indicates the autocrine protective signaling mechanism of G-CSF by inhibition of apoptosis, inflammation, and stimulation of neurogenesis. These observed neuroprotective effects of G-CSF makes it an attractive target to mitigate neurodegeneration associated with neurological disorders. The objective of the review is to highlight and summarize recent updates on G-CSF as a therapeutically versatile neuroprotective agent along with mechanisms of action as well as possible clinical applications in neurodegenerative disorders including AD, PD and HD.

The Efficacy and Safety of Granulocyte Colony-Stimulating Factor for Patients with Stroke

Abstract:Background Granulocyte colony-stimulating factor (G-CSF) has been shown to reduce lesion volume and improve functional outcome in experimental stroke models. However, whether G-CSF plays a role currently in patients with stroke remains uncertain. Our study aimed at examining the efficacy and safety of G-CSF in patients with acute ischemic stroke. Methods A comprehensive search was conducted in 5 online databases up to April 2014, and 10 studies with 711 patients met the criteria. Results The results showed that G-CSF was beneficial in improving the National Institutes of Health Stroke Scale (standardized mean difference [SMD], .43; 95% confidence interval [CI], .03-.82; P = .04) and modified Rankin Scale (mRS) scores (SMD, .72; 95% CI, .51-.93; P = .01), and elevating CD34 + count ( P P = .59), serious adverse events (relative ratio [RR], 1.12; 95% CI, .91-1.38; P = .28), or the death of serious adverse events (RR, 1.25; 95% CI, .82-1.91; P = .30) between groups at day 90. Adverse effect on vascular complications was not detected to be increased although G-CSF produced a marked elevation in the total leukocyte count (SMD, 3.52; 95% CI, 2.54-4.49; P Conclusions In conclusion, G-CSF is effective at mobilizing bone marrow–derived CD34 + stem cells to the peripheral blood. It also seems to improve the National Institutes of Health Stroke Scale and mRS scores. The administration of G-CSF appears to be safe and well tolerated. Further studies need to be done on a large sample to verify or fully characterize the results.
Colony-Stimulating Factors on Mobilizing CD34+ Cells and Improving Neurological Functions in Patients with Stroke: A Meta-Analysis and a Systematic Review

Abstract:Background and Purpose: CSF therapy is considered a promising therapeutic approach for stroke. We performed a meta-analysis to explore the safety and efficacy of CSF in published clinical stroke studies. Methods: We searched articles online and manually. Two reviewers selected studies independently, selecting data based on study quality, characteristics of intervention (administration time, observation time, type, dose, and injection approach of CSF), and the baseline characteristics of patients (age, sex, hypertension, diabetes, smoker, and lipids) were extracted. Main prognosis outcomes were measured as all-cause death in severe adverse events (SAE) and recurrent stroke in SAE. Secondary outcomes were measured as CD34(+) cell counts in periphery blood at day 5, National Institutes of Health Stroke Scale (NIHSS), and Barthel index (BI), Side effects of CSF were taken as the indicator of safety. STATA13 software was used to perform the meta-analysis. Keywords: Stroke, Colony-stimulating factor, Meta-analysis, therapy, Neurological Diseases Results: This meta-analysis involved 485 patients from eight studies. Among them, 475 patients from seven studies were gauged SAE (all-cause death), 393 patients from six studies were checked SAE (recurrent stroke); 137 patients from three studies underwent CD34(+) measurement, 389 patients from six studies were tested NIHSS and 307 patients from five studies accessed BI. Compared with the control group, both all-causes death (RR=1.73, 95%CI=(0.61, 4.92), P=0.735, I-2=0.0%) and recurrent stroke (RR=0.43, 95% CI=(0.14, 1.32), P=0.214, I-2=33.1%) present no statistical differences, indicating that the application of CSF does not statistically alter the prognosis of patients with stroke. The application of CSF effectively enhanced CD34(+) cell counts in periphery blood at day 5 (SMD=1.23, 95%CI=(0.54, 1.92), P=0.04, I-2=69.0%) but did not statistically impact NIHSS (SMD=-0.40, 95%CI=(-0.93, 0.13), P = 0.001, I-2=79.7%) or BI (SMD=0.04, 95% CI=(-0.38, 0.46), P=0.068, I-2=54.3%). Conclusion: Our study consolidates the security of CSF administration for its exerting no effect on detrimental outcomes. It has proven to be effective in elevating CD34(+) cell counts in periphery blood at day 5, indicating CSF may participate in stroke recovery, but its efficacy in stroke recovery remains detected.
The Role of Stem Cell Factor and Granulocyte-Colony Stimulating Factor in Brain Repair during Chronic Stroke

Abstract:Chronic stroke is a highly important but under-investigated scientific problem in neurologic research. We have reported earlier that stem cell factor (SCF) in combination with granulocyte-colony stimulating factor (G-CSF) treatment during chronic stroke improves functional outcomes. Here we have determined the contribution of bone marrow-derived cells in angiogenesis and neurogenesis, which are enhanced by SCF + G-CSF treatment during chronic stroke. Using bone marrow tracking, flow cytometry, 2-photon live brain imaging, and immunohistochemistry, we observed that the levels of circulating bone marrow stem cells (BMSCs) (CD34 + /c-kit + ) were significantly increased by SCF +G-CSF treatment. In addition, live brain imaging revealed that numerous bone marrow-derived cells migrate into the brain parenchyma in the treated mice. We also found that bone marrow-derived cells, bone marrow-derived endothelial cells, vascular density, and bone marrow-derived neurons were significantly augmented by SCF + G-CSF. It is interesting that, in addition to the increase in bone marrow-derived endothelial cells, the number of bone marrow-derived pericytes was reduced after SCF + G-CSF treatment during chronic stroke. These data suggest that SCF + G-CSF treatment can enhance repair of brain damage during chronic stroke by mobilizing BMSCs, and promoting the contribution of bone marrow-derived cells to angiogenesis and neurogenesis.

Abstract:OBJECTIVE Erythropoietin (EPO) exerts a neuroprotective effect in animal models of traumatic brain injury (TBI). However, its effectiveness in human patients with TBI is unclear. In this study, the authors conducted the first meta-analysis to assess the effectiveness and safety of EPO in patients with TBI. METHODS In December 2015, a systematic search was performed of PubMed, Web of Science, MEDLINE, Embase, the Cochrane Library databases, and Google Scholar. Only English-language publications of randomized controlled trials (RCTs) using EPO in patients with TBI were selected for analysis. The assessed outcomes included mortality, favorable neurological outcome, hospital stay, and associated adverse effects. Continuous variables were presented as mean difference (MD) with a 95% confidence interval (CI). Dichotomous variables were presented as risk ratio (RR) or risk difference (RD) with a 95% CI. Statistical heterogeneity was examined using both I2 and chi-square tests. RESULTS Of the 346 studies identified in the search, 5 RCTs involving 915 patients met the inclusion criteria. The overall results demonstrated that EPO significantly reduced mortality (RR 0.69, 95% CI 0.49-0.96, p = 0.03) and shortened the hospitalization time (MD -7.59, 95% CI -9.71 to -5.46, p < 0.0001) for patients with TBI. Pooled results of favorable outcome (RR 1.00, 95% CI 0.88-1.15, p = 0.97) and deep vein thrombosis (DVT; RD 0.00, 95% CI -0.05 to 0.05, p = 1.00) did not show a significant difference. CONCLUSIONS The authors suggested that EPO is beneficial for patients with TBI in terms of reducing mortality and shortening hospitalization time without increasing the risk of DVT. However, its effect on improving favorable neurological outcomes did not reach statistical significance. Therefore, more well-designed RCTs are necessary to ascertain the optimum dosage and time window of EPO treatment for patients with TBI.
Granulocyte-colony Stimulating Factor in Combination with Stem Cell Factor Confers Greater Neuroprotection after Hypoxic–Ischemic Brain Damage in the Neonatal Rats than a Solitary Treatment

Abstract:Neonatal hypoxia–ischemia (HI) is a devastating condition resulting in neuronal cell death and often culminates in neurological deficits. Granulocyte-colony stimulating factor (G-CSF) has been shown to have neuroprotective activity via inhibition of apoptosis and inflammation in various stroke models. Stem cell factor (SCF) regulates hematopoietic stem cells in the bone marrow and has been reported to have neuroprotective properties in an experimental ischemic stroke model. In this study, we aim to determine the protective effects of G-CSF in combination with SCF treatment after experimental HI. Seven-day-old Sprague–Dawley rats were subjected to unilateral carotid artery ligation followed by 2.5 h of hypoxia. Animals were randomly assigned to five groups: Sham (n = 8), Vehicle (n = 8), HI with G-CSF treatment (n = 9), HI with SCF treatment (n = 9), and HI with G-CSF + SCF treatment (coadministration group; n = 10). G-CSF (50 μg/kg), SCF (50 μg/kg), and G-CSF + SCF (50 μg/kg) were administered intraperitoneally 1 h post HI followed by daily injection for 4 consecutive days (five total injections). Animals were euthanized 14 days after HI for neurological testing. Additionally, assessment of brain, heart, liver, spleen, and kidney atrophy was performed. Both G-CSF and G-CSF + SCF treatments improved body growth and decreased brain atrophy at 14 days post HI. No significant differences were found in the peripheral organ weights between groups. Finally, the G-CSF + SCF coadministration group showed significant improvement in neurological function. Our data suggest that administration of G-CSF in combination with SCF not only prevented brain atrophy but also significantly improved neurological function.

Abstract:INTRODUCTION: THE SYNDROME AND ITS PROGNOSIS Early death after acute ischaemic stroke is most frequently caused by space occupying ischaemic brain oedema. With complete middle cerebral artery (MCA) territory infarction, including the basal ganglia, and occasionally additional infarction of the anterior cerebral artery or the posterior cerebral artery territories, the large post ischaemic oedematous infarct may cause mass effect, raised intracranial pressure (ICP), and so herniation and brain death (Hacke et al. 1996; Frank 1995; Ropper 1984; Berrouschot et al. 1998). Clinically, the patients present with dense hemiplegia, head- and eye-deviation, progressive deterioration of consciousness over the first 24–48 h and reduced ventilatory drive. Herniation occurs between day 2 and day 5 after onset. Because older patients tend to have more brain atrophy, this condition is more frequently seen among patients under 60 years of age. Abstract:Recombinant human granulocyte-colony stimulating factor (hG-CSF) has been shown to protect the nervous system after brain ischemia. However, the neuroprotective mechanism of hG-CSF remains unclear. The present study established a rat model of cerebral ischemia/reperfusion and subcutaneously injected recombinant hG-CSF after reperfusion for 2 hours. Cerebral cortical protein was extracted following 14 days of reperfusion and subjected to two-dimensional electrophoresis. In brain ischemic rats, 56 different protein spots were screened, including 17 that were upregulated and 17 that were downregulated, compared with the sham-surgery group. Matrix assisted laser desorption ionization/time of flight mass spectrometry was used to determine peptide mass fingerprinting. Following a National Center for Biotechnology Information database search and confirmation with the Swiss-Prot database, 19 spots were identified as known proteins. Following hG-CSF treatment, 35 different protein spots were found, including 16 that were downregulated and 19 that were upregulated. Six were known proteins, including dihydropyrimidinase-associated protein 2, glial fibrillary acidic protein, endomucin, Rho GDP dissociation inhibitor, Rab GDP dissociation inhibitor and guanine-nucleotide-binding protein. Results indicate that hG-CSF is involved in neuroprotection after brain ischemia, possibly by regulating the expression of various neural regeneration-associated proteins at the subacute stage.
Cerebral ischemia induces astrocytes to express granulocyte colony stimulating factor and glial cell fine-derived neurotrophic factor

Abstract:Objective To explore the neuroprotective mechanisms of granulocyte colony stimulating factor (G-CSF) in ischemic brain injury. Methods Brain tissues were taken out from MCAO and sham operated Sprague-Dawley rats 7 days after operation. The expression of G-CSFR, GDNF, MAP2 and GFAP was measured by using immunofluorescence co-staining. Results The expression of G-CSFR and GDNF were widely distributed in the neurons in normal brain tissues, not in the astrocytes. However, in ischemic peripheral zone, part of G-CSFR and GDNF positive cells merged with GFAP positive cells, in normal brain tissues, most G-CSFR positive cells were co-expressed with GDNF. Conclusion Cerebral ischemia induces astrocytes to express G-CSFR and GDNF, suggesting that endogenous neuroprotection by cerebral ischemia may be related with the expression and production of G-CSFR and GDNF in astrocytes in ischemic peripheral zone.

Abstract:<p>Stroke is the leading cause of disability and the second leading cause of death worldwide. With the global population aged 65 and over growing faster than all other age groups, the incidence of stroke is also increasing. In addition, there is a shift in the overall stroke burden towards younger age groups, particularly in low and middle-income countries. Stroke in most cases is caused due to an abrupt blockage of an artery (ischemic stroke), but in some instances stroke may be caused due to bleeding into brain tissue when a blood vessel ruptures (hemorrhagic stroke). Although treatment options for stroke are still limited, with the advancement in recanalization therapy using both pharmacological and mechanical thrombolysis some progress has been made in helping patients recover from ischemic stroke. However, there is still a substantial need for the development of therapeutic agents for neuroprotection in acute ischemic stroke to protect the brain from damage prior to and during recanalization, extend the therapeutic time window for intervention and further improve functional outcome. The current review has assessed the past challenges in developing neuroprotective strategies, evaluated the recent advances in clinical trials, discussed the recent initiative by the National Institute of Neurological Disorders and Stroke in USA for the search of novel neuroprotectants (Stroke Preclinical Assessment Network, SPAN) and identified emerging neuroprotectants being currently evaluated in preclinical studies. The underlying molecular mechanism of each of the neuroprotective strategies have also been summarized, which could assist in the development of future strategies for combinational therapy in stroke treatment.</p>

Abstract:Objective To investigate the effects of recombinant human granulocyte colony-stimulating factor (rhG-CSF) on the prognosis and the expression of macrotubule-associated protein 2 (MAP-2) mRNA in rats after focal cerebral ischemia. Methods Focal brain ischemia in rats was performed with the intraluminal filament occlusion. The effects of rhG-CSF were evaluated by mortality rate and neurological severity score (NSS). Reverse transcriptional polymerase chain reaction ( RT-PCR ) method was used to measure MAP-2 mRNA expression in ischemic brain tissue of rhG-CSF group and non-rhG-CSF group. Results Compared to that of non-rhG-CSF group the mortality of rhG-CSF group was reduced significantly and NSS was improved markedly. The expression of MAP-2 mRNA in rhG-CSF group recovered earlier than that of non-rhG-CSF. Conclusion The rhG-CSF may alleviate the brain damage following ischemia-reperfusion and improve the functional prognosis.
Neurotherapeutic potential of erythropoietin after ischemic injury of the central nervous system

Abstract:Erythropoietin (EPO) is one of the most successful biopharmaceuticals in history and is used for treating anemia of different origins. However, it became clear that EPO could also work in a neuroprotective, antiapoptotic, antioxidative, angiogenetic and neurotropic way. It causes stimulation of cells to delay cell apoptosis, especially in the central nervous system. In rodent models of focal cerebral ischemia, EPO showed an impressive reduction of infarct size by 30% and improvement of neurobehavioral outcome by nearly 40%. A large animal model dealing with ischemia and reperfusion of the spinal cord showed that EPO could reduce the risk of spinal cord injury significantly. In addition, some clinical studies tested whether EPO works in real live clinical settings. One of the most promising studies showed the innocuousness and improvements in follow-up, outcome scales and in infarct size, of EPO-use in humans suffering from ischemic stroke. Another study ended unfortunately in a negative outcome and an increased overall death rate in the EPO group. The most possible reason was the involvement of patients undergoing simultaneously systemic thrombolysis with recombinant tissue plasminogen activator. An experimental study on rats demonstrated that administration of EPO might exacerbate tissue plasminogen activator-induced brain hemorrhage without reducing the ischemic brain damage. This case shows clearly how useful animal models can be to check negative side effects of a treatment before going into clinical trials. Other groups looked in human trials at the effects of EPO on the outcome after ischemic stroke, relation to circulating endothelial progenitor cells, aneurysmal subarachnoid hemorrhage, traumatic brain injury, hemoglobin transfusion thresholds and elective first-time coronary artery bypass surgery. Most of the results were positive, but are based mostly on small group sizes. However, some of the most neglected facts when focusing on experimental setups of ischemia of the central nervous system are issues like age and comorbidities. It might be extremely worthy to consider these points for future projects, because EPO might influence all these factors.

Abstract:Numerous studies explored the therapeutic effects of erythropoietin (EPO) on neurodegenerative diseases. Few studies provided comprehensive and latest knowledge of EPO treatment for ischemic stroke. In the present review, we introduced the structure, expression, function of EPO, and its receptors in the central nervous system. Furthermore, we comprehensively discussed EPO treatment in pre-clinical studies, clinical trials, and its therapeutic mechanisms including suppressing inflammation. Finally, advanced studies of the therapy of EPO derivatives in ischemic stroke were also discussed. We wish to provide valuable information on EPO and EPO derivatives’ treatment for ischemic stroke for basic researchers and clinicians to accelerate the process of their clinical applications.

Protection of Granulocyte-Colony Stimulating Factor to Hemorrhagic Brain Injuries and Its Involved Mechanisms: Effects of Vascular Endothelial Growth Factor and Aquaporin-4

Abstract:Granulocyte-colony stimulating factor (G-CSF) has protective effects on many neurological diseases. Here, we aimed to test G-CSF's effects on perihematomal tissue injuries following intracerebral hemorrhage (ICH) and examine whether the effects were functionally dependent on vascular endothelial growth factor (VEGF) and aquaporin-4 (AQP4). We detected the expression of perihematomal VEGF, VEGF receptors (VEGFRs) and AQP4 at 1, 3 and 7 days after ICH. Also, we examined the effects of G-CSF on tissue injuries by ICH in wild type mice, and tested whether such effects were VEGF and AQP4 dependent by using VEGFR inhibitor -SU5416 and AQP4 knock-out (AQP4(-/-)) mice. Furthermore, we assessed the related signal transduction pathways via astrocyte cultures. We found G-CSF highly increased perihematomal VEGF, VEGFR-2 and AQP4. Importantly, G-CSF led to neurological functional improvement in both types of mice by associating with reduction of brain edema, blood-brain barrier (BBB) permeability and neuronal death and apoptosis and statistical analysis suggested AQP4 was required for these effects. Besides, except BBB leakage alleviation, the above effects were attenuated but not counteracted by SU5416, suggesting involvement of VEGF. G-CSF up-regulated phosphoryla-tion of extracellular signal-regulated kinase (ERK) and signal transducer and activator of transcription 3 (STAT3) as well as VEGF and AQP4 proteins in cultured astrocytes. The latter was inhibited by ERK and STAT3 inhibitors respectively. Our data suggest the protective effects of G-CSF on perihematomal tissue injuries after ICH are highly associated with the increased levels of VEGF and AQP4, possibly act through C-Jun amino-terminal kinase and ERK pathways respectively.
