Abstract:Accurate and efficient dose calculation is an important prerequisite to ensure the success of radiation therapy. However, all the dose calculation algorithms commonly used in current clinical practice have to compromise between calculation accuracy and efficiency, which may result in unsatisfactory dose accuracy or highly intensive computation time in many clinical situations. The purpose of this work is to develop a novel dose calculation algorithm based on the deep learning method for radiation therapy. In this study we performed a feasibility investigation on implementing a fast and accurate dose calculation based on a deep learning technique. A two-dimensional (2D) fluence map was first converted into a three-dimensional (3D) volume using ray traversal algorithm. 3D U-Net like deep residual network was then established to learn a mapping between this converted 3D volume, CT and 3D dose distribution. Therefore an indirect relationship was built between a fluence map and its corresponding 3D dose distribution without using significantly complex neural networks. Two hundred patients, including nasopharyngeal, lung, rectum and breast cancer cases, were collected and applied to train the proposed network. Additional 47 patients were randomly selected to evaluate the accuracy of the proposed method through comparing dose distributions, dose volume histograms and clinical indices with the results from a treatment planning system (TPS), which was used as the ground truth in this study. The proposed deep learning based dose calculation algorithm achieved good predictive performance. For 47 tested patients, the average per-voxel bias of the deep learning calculated value and standard deviation (normalized to the prescription), relative to the TPS calculation, is 0.17%±2.28%. The average deep learning calculated values and standard deviations for relevant clinical indices were compared with the TPS calculated results and the t-test p-values demonstrated the consistency between them. In this study we developed a new deep learning based dose calculation method. This approach was evaluated by the clinical cases with different sites. Our results demonstrated its feasibility and reliability and indicated its great potential to improve the efficiency and accuracy of radiation dose calculation for different treatment modalities.

Dose Prediction Using a Three-Dimensional Convolutional Neural Network for Nasopharyngeal Carcinoma With Tomotherapy

Abstract:Purpose This study focused on predicting 3D dose distribution at high precision and generated the prediction methods for nasopharyngeal carcinoma patients (NPC) treated with Tomotherapy based on the patient-specific gap between organs at risk (OARs) and planning target volumes (PTVs). Methods A convolutional neural network (CNN) is trained using the CT and contour masks as the input and dose distributions as output. The CNN is based on the “3D Dense-U-Net”, which combines the U-Net and the Dense-Net. To evaluate the model, we retrospectively used 124 NPC patients treated with Tomotherapy, in which 96 and 28 patients were randomly split and used for model training and test, respectively. We performed comparison studies using different training matrix shapes and dimensions for the CNN models, i.e., 128 ×128 ×48 (for Model I), 128 ×128 ×16 (for Model II), and 2D Dense U-Net (for Model III). The performance of these models was quantitatively evaluated using clinically relevant metrics and statistical analysis. Results We found a more considerable height of the training patch size yields a better model outcome. The study calculated the corresponding errors by comparing the predicted dose with the ground truth. The mean deviations from the mean and maximum doses of PTVs and OARs were 2.42 and 2.93%. Error for the maximum dose of right optic nerves in Model I was 4.87 ± 6.88%, compared with 7.9 ± 6.8% in Model II ( p =0.08) and 13.85 ± 10.97% in Model III ( p <0.01); the Model I performed the best. The gamma passing rates of PTV 60 for 3%/3 mm criteria was 83.6 ± 5.2% in Model I, compared with 75.9 ± 5.5% in Model II ( p <0.001) and 77.2 ± 7.3% in Model III ( p <0.01); the Model I also gave the best outcome. The prediction error of D 95 for PTV 60 was 0.64 ± 0.68% in Model I, compared with 2.04 ± 1.38% in Model II ( p <0.01) and 1.05 ± 0.96% in Model III ( p =0.01); the Model I was also the best one. Conclusions It is significant to train the dose prediction model by exploiting deep-learning techniques with various clinical logic concepts. Increasing the height (Y direction) of training patch size can improve the dose prediction accuracy of tiny OARs and the whole body. Our dose prediction network model provides a clinically acceptable result and a training strategy for a dose prediction model. It should be helpful to build automatic Tomotherapy planning.

Abstract:Objective: To generate synthetic CT (sCT) images with high quality from CBCT and planning CT (pCT) for dose calculation by using deep learning methods. Methods: 169 NPC patients with a total of 20926 slices of CBCT and pCT images were included. In this study the CycleGAN, Pix2pix and U-Net models were used to generate the sCT images. The Mean Absolute Error (MAE), Root Mean Squared Error (RMSE), Peak Signal to Noise Ratio (PSNR), and Structural Similarity Index (SSIM) were used to quantify the accuracy of the proposed models in a testing cohort of 34 patients. Radiation dose were calculated on pCT and sCT following the same protocol. Dose distributions were evaluated for 4 patients by comparing the dose-volume-histogram (DVH) and 2D gamma index analysis. Results: The average MAE and RMSE values between sCT by three models and pCT reduced by 15.4 HU and 26.8 HU at least, while the mean PSNR and SSIM metrics between sCT by different models and pCT added by 10.6 and 0.05 at most, respectively. There were only slight differences for DVH of selected contours between different plans. The passing rates of 2D gamma index analysis under 3 mm/3% 3 mm/2%, 2 mm/3%and 2 mm/2% criteria were all higher than 95%. Conclusions: All the sCT had achieved better evaluation metrics than those of original CBCT, while the performance of CycleGAN model was proved to be best among three methods. The dosimetric agreement confirmed the HU accuracy and consistent anatomical structures of sCT by deep learning methods.

Abstract:Background In boron neutron capture therapy (BNCT)—a form of binary radiotherapy—the primary challenge in treatment planning systems for dose calculations arises from the time‐consuming nature of the Monte Carlo (MC) method. Recent progress, including the use of neural networks (NN), has been made to accelerate BNCT dose calculations. However, this approach may result in significant dose errors in both the tumor and the skin, with the latter being a critical organ in BNCT. Furthermore, owing to the lack of physical processes in purely NN‐based approaches, their reliability for clinical dose calculations in BNCT is questionable. Purpose In this study, a physically constrained MC–NN (PCMC–NN) coupling algorithm is proposed to achieve fast and accurate computation of the BNCT three‐dimensional (3D) therapeutic dose distribution. This approach synergizes the high precision of the MC method with the speed of the NN and utilizes physical conservation laws to constrain the coupling process. It addresses the time‐consuming issue of the traditional MC method while reducing dose errors. Methods Clinical data were collected from 113 glioblastoma patients. For each patient, the 3D dose distributions for both the coarse and detailed dose grids were calculated using the MC code PHITS. Among these patients, the data from 14 patients were allocated to the test set, 9 to the validation set, and the remaining to the training set. A neural network, 3D‐Unet, was built based on the coarse grid dose and patient CT information to enable fast and accurate computation of the 3D detailed grid dose distribution of BNCT. Results Statistical evaluations, including relative deviation, dose deviation, mean absolute error (MAE), and mean absolute percentage error (MAPE) were conducted. Our findings suggested that the PCMC–NN algorithm substantially outperformed the traditional NN and interpolation methods. Furthermore, the proposed algorithm significantly reduced errors, particularly in the skin and GTV, and improved computational accuracy (hereinafter referred to simply as 'accuracy') with a MAPE range of 1.6%–4.0% and a maximum MAE of 0.3 Gy (IsoE) for different organs. The dose–volume histograms generated by the PCMC–NN aligned well with those obtained from the MC method, further validating its accuracy. Conclusions The PCMC–NN algorithm enhanced the speed and accuracy of BNCT dose calculations by combining the MC method with the NN algorithm. This indicates the significant potential of the proposed algorithm for clinical applications in optimizing treatment planning.

Abstract:A new method of dose calculation algorithm, called GPU-accelerated Monte Carlo and collapsed cone Convolution (GMCC) was developed to improve the calculation speed of BNCT treatment planning system. The GPU-accelerated Monte Carlo routine in GMCC is used to simulate the neutron transport over whole energy range and the Collapsed Cone Convolution method is to calculate the gamma dose. Other dose components due to alpha particles and protons, are calculated using the calculated neutron flux and reaction data. The mathematical principle and the algorithm architecture are introduced. The accuracy and performance of the GMCC were verified by comparing with the FLUKA results. A water phantom and a head CT voxel model were simulated. The neutron flux and the absorbed dose obtained by the GMCC were consistent well with the FLUKA results. In the case of head CT voxel model, the mean absolute percentage error for the neutron flux and the absorbed dose were 3.98% and 3.91%, respectively. The calculation speed of the absorbed dose by the GMCC was 56 times faster than the FLUKA code. It was verified that the GMCC could be a good candidate tool instead of the Monte Carlo method in the BNCT dose calculations.

Abstract:Objective.Patient dose estimation in x-ray-guided interventions is essential to prevent radiation-induced biological side effects. Current dose monitoring systems estimate the skin dose based in dose metrics such as the reference air kerma. However, these approximations do not take into account the exact patient morphology and organs composition. Furthermore, accurate organ dose estimation has not been proposed for these procedures. Monte Carlo simulation can accurately estimate the dose by recreating the irradiation process generated during the x-ray imaging, but at a high computation time, limiting an intra-operative application. This work presents a fast deep convolutional neural network trained with MC simulations for patient dose estimation during x-ray-guided interventions.Approach.We introduced a modified 3D U-Net that utilizes a patient's CT scan and the numerical values of imaging settings as input to produce a Monte Carlo dose map. To create a dataset of dose maps, we simulated the x-ray irradiation process for the abdominal region using a publicly available dataset of 82 patient CT scans. The simulation involved varying the angulation, position, and tube voltage of the x-ray source for each scan. We additionally conducted a clinical study during endovascular abdominal aortic repairs to validate the reliability of our Monte Carlo simulation dose maps. Dose measurements were taken at four specific anatomical points on the skin and compared to the corresponding simulated doses. The proposed network was trained using a 4-fold cross-validation approach with 65 patients, and evaluating the performance on the remaining 17 patients during testing.Main results.The clinical validation demonstrated a average error within the anatomical points of 5.1%. The network yielded test errors of 11.5 ± 4.6% and 6.2 ± 1.5% for peak and average skin doses, respectively. Furthermore, the mean errors for the abdominal region and pancreas doses were 5.0 ± 1.4% and 13.1 ± 2.7%, respectively.Significance.Our network can accurately predict a personalized 3D dose map considering the current imaging settings. A short computation time was achieved, making our approach a potential solution for dose monitoring and reporting commercial systems.
Boosting radiotherapy dose calculation accuracy with deep learning

Abstract:In radiotherapy, a trade-off exists between computational workload/speed and dose calculation accuracy. Calculation methods like pencil-beam convolution can be much faster than Monte-Carlo methods, but less accurate. The dose difference, mostly caused by inhomogeneities and electronic disequilibrium, is highly correlated with the dose distribution and the underlying anatomical tissue density. We hypothesize that a conversion scheme can be established to boost low-accuracy doses to high-accuracy, using intensity information obtained from computed tomography (CT) images. A deep learning-driven framework was developed to test the hypothesis by converting between two commercially-available dose calculation methods: AAA (anisotropic-analytic-algorithm) and AXB (Acuros XB).A hierarchically-dense U-Net model was developed to boost the accuracy of AAA dose towards the AXB level. The network contained multiple layers of varying feature sizes to learn their dose differences, in relationship to CT, both locally and globally. AAA and AXB doses were calculated in pairs for 120 lung radiotherapy plans covering various treatment techniques, beam energies, tumor locations, and dose levels.

Abstract:Cancer is a primary cause of morbidity and mortality worldwide. The radiotherapy plays a more and more important role in cancer treatment. In the radiotherapy, the dose distribution maps in patient need to be calculated and evaluated for the purpose of killing tumor and protecting healthy tissue. Monte Carlo (MC) radiation transport calculation is able to account for all aspects of radiological physics within 3D heterogeneous media such as the human body and generate the dose distribution maps accurately. However, an MC calculation for doses in radiotherapy usually takes a great mass of time to achieve acceptable statistical uncertainty, impeding the MC methods from wider clinic applications. Here we introduce a convolutional neural network (CNN), termed as Monte Carlo Denoising Net (MCDNet), to achieve the acceleration of the MC dose calculations in radiotherapy, which is trained to directly predict the high-photon (noise-free) dose maps from the low-photon (noise-much) dose maps. Thirty patients with postoperative rectal cancer who accepted intensity-modulated radiation therapy (IMRT) were enrolled in this study. 3D Gamma Index Passing Rate (GIPR) is used to evaluate the performance of predicted dose maps. The experimental results demonstrate that the MCDNet can improve the GIPR of dose maps of 1x107 photons over that of 1x108 photons, yielding over 10x speed-up in terms of photon numbers used in the MC simulations of IMRT. It is of great potential to investigate the performance of this method on the other tumor sites and treatment modalities.
MCDNet - A Denoising Convolutional Neural Network to Accelerate Monte Carlo Radiation Transport Simulations: A Proof of Principle With Patient Dose From X-Ray CT Imaging.

Abstract:X-ray computed tomography (CT) is a popular diagnostic imaging tool that has caused public concern over potential radiation risks to the patient. Monte Carlo (MC) simulations are the most accurate methods to calculate x-ray interactions with the patient's body and voxel-wise dose distributions, but the statistical methods suffer from extremely long computing time that is required to achieve necessary statistical precision. In this paper, we propose and demonstrate the Monte Carlo Denoising Net (MCDNet), a convolutional encoder-decoder neural network, for the purpose of accelerating the MC radiation transport simulations for patient CT dosimetry. A unique set of full-body anatomically realistic adult voxel phantoms of various sizes and a GPU-based parallel MC code were used to produce adequate training and testing data for supervised learning. Gamma index passing rate (GIPR) was used to evaluate the performance of predicted dose maps. For CT scan protocols considered in this study, MCDNet is found to have the ability of predicting dose maps of 9.9 x 10(7) photons from corresponding dose maps of 1.3 x 10(6) photons, yielding 76x speed-up in terms of photon numbers used in the MC simulations. MCDNet is the first CNN-based method to speed-up MC radiation transport simulations involving 3D and heterogeneous patient anatomies for x-ray CT. Future studies will test the feasibility of applying the deep-learning based denoising strategy to other MC radiation transport applications.

Abstract:AbstractMonte Carlo (MC) simulation, as the most accurate dose calculation algorithm, is available in the MultiPlan treatment planning system for Cyberknife. The main purpose of this work was to perform experiments to thoroughly investigate the accuracy of the MC dose calculation algorithm. Besides the basic MC beam commissioning, two test scenarios were designed. First, single beam tests were performed with a solid water phantom to verify the MC source model in simple geometry. Then, a lung treatment plan on a CIRS thorax phantom was created to mimic the clinical patient treatment. The plan was optimized and calculated using ray tracing (RT) algorithm and then recalculated using MC algorithm. Measurements were performed in both a homogeneous phantom and a heterogeneous phantom (CIRS). Ion‐chamber and radiochromic film were used to obtain absolute point dose and dose distributions. Ion‐chamber results showed that the differences between measured and MC calculated dose were within 3% for all tests. On the film measurements, MC calculation results showed good agreements with the measured dose for all single beam tests. As for the lung case, the gamma passing rate between measured and MC calculated dose was 98.31% and 97.28% for homogeneous and heterogeneous situation, respectively, using 3%/2 mm criteria. However, RT algorithm failed with the passing rate of 79.25% (3%/2 mm) for heterogeneous situation. These results demonstrated that MC dose calculation algorithm in the Multiplan system is accurate enough for patient dose calculation. It is strongly recommended to use MC algorithm in heterogeneous media.
On Distributed and Self-Organized Inter-Cell Interference Mitigation for OFDMA Downlink in IMT-Advanced Radio Systems

Abstract:This paper introduces a set of novel RRM philosophies: 1) the distributed Robin Hood philosophy, and 2) the self-organized proactive donation philosophy. By applying these philosophies with the soft-frequency reuse scheme, we proposed two novel inter-cell interference mitigation algorithms for the OFDMA based IMT advanced air interface: 1) distributed and dynamic power robbing (DDPR), and 2) self-organized and dynamic power shaping with proactive donation (SDPS/PD). Compared with the existing soft-frequency reuse schemes, our proposed algorithms are dynamic in the nature and more efficient and flexible. And for some scenarios, e.g. the user deployed wireless access points in a shopping center, the SDPS algorithm is the only possible interference mitigation solutions. Preliminary simulation results show that the DDPR algorithm can effectively reduce the average cell buffer size and average interference among the multi-cell environment.
Dose calculation on kilovoltage cone-beam CT imaging for head and neck radiotherapy

Abstract:Objective To study the feasibility of dose calculation using kilovoltage X-ray cone-beam CT (KVCBCT) imaging for head-and-neck radiation therapy.Methods 11 patients with nasopharyngeal carcinoma were scanned with KVCBCT to adjust position before treatment, and rescanning images with KVCBCT after correction were input a treatment-planning system.The dose was recalculated by applying the patients′ treatment plans based on planning CT to the KVCBCT images.The dose distributions and dose volume histograms (DVH) of the tumor and critical structures were compared with the original treatment plan.Results The DVH and dose distribution of the plan based on the KVCBCT are compared with that of the planning CT, and they shows a good consistency for the 11 cases.The doses calculated from the planning CT and KVCBCT were compared on the isocenter planes.Using γ analysis with a criterion of 3%/3 mm, 98.0%±1.33% of the points on the isocenter planes in the planning CT and KVCBCT.The difference of the dose to target volume was<1% and to normal structure was<2%.Conclusions This study indicated that CBCT images can be used to make a treatment plan with its individual hounsfield unit-electron density calibration curve.
Transformer-Integrated Hybrid Convolutional Neural Network for Dose Prediction in Nasopharyngeal Carcinoma Radiotherapy

Abstract:Radiotherapy is recognized as the major treatment of nasopharyngeal carcinoma. Rapid and accurate dose prediction can improve the efficiency of the treatment planning process and the quality of radiotherapy plans. Currently, deep learning-based methods have been widely applied to dose prediction for radiotherapy treatment planning. However, it is important to note that existing models based on Convolutional Neural Networks (CNN) often overlook long-distance information. Although some studies try to use Transformer to solve the problem, it lacks the ability of CNN to process the spatial information inherent in images. Therefore, we propose a novel CNN and Transformer hybrid dose prediction model. To enhance the transmission ability of features between CNN and Transformer, we design a hierarchical dense recurrent encoder with a channel attention mechanism. Additionally, we propose a progressive decoder that preserves richer texture information through layer-wise reconstruction of high-dimensional feature maps. The proposed model also introduces object-driven skip connections, which facilitate the flow of information between the encoder and decoder. Experiments are conducted on in-house datasets, and the results show that the proposed model is superior to baseline methods in most dosimetric criteria. In addition, the image analysis metrics including PSNR, SSIM, and NRMSE demonstrate that the proposed model is consistent with ground truth and produces promising visual effects compared to other advanced methods. The proposed method could be taken as a powerful clinical guidance tool for physicists, significantly enhancing the efficiency of radiotherapy planning. Validation of a Deep Learning-Based Material Estimation Model for Monte Carlo Dose Calculation in Proton Therapy.

Abstract:Objective. Computed tomography (CT) to material property conversion dominates proton range uncertainty, impacting the quality of proton treatment planning. Physics-based and machine learning-based methods have been investigated to leverage dual-energy CT (DECT) to predict proton ranges. Recent development includes physics-informed deep learning (DL) for material property inference. This paper aims to develop a framework to validate Monte Carlo dose calculation (MCDC) using CT-based material characterization models.Approach.The proposed framework includes two experiments to validatein vivodose and water equivalent thickness (WET) distributions using anthropomorphic and porcine phantoms. Phantoms were irradiated using anteroposterior proton beams, and the exit doses and residual ranges were measured by MatriXX PT and a multi-layer strip ionization chamber. Two pre-trained conventional and physics-informed residual networks (RN/PRN) were used for mass density inference from DECT. Additional two heuristic material conversion models using single-energy CT (SECT) and DECT were implemented for comparisons. The gamma index was used for dose comparisons with criteria of 3%/3 mm (10% dose threshold).Main results. The phantom study showed that MCDC with PRN achieved mean gamma passing rates of 95.9% and 97.8% for the anthropomorphic and porcine phantoms. The rates were 86.0% and 79.7% for MCDC with the empirical DECT model. WET analyses indicated that the mean WET variations between measurement and simulation were -1.66 mm, -2.48 mm, and -0.06 mm for MCDC using a Hounsfield look-up table with SECT and empirical and PRN models with DECT. Validation experiments indicated that MCDC with PRN achieved consistent dose and WET distributions with measurement.Significance. The proposed framework can be used to identify the optimal CT-based material characterization model for MCDC to improve proton range uncertainty. The framework can systematically verify the accuracy of proton treatment planning, and it can potentially be implemented in the treatment room to be instrumental in online adaptive treatment planning.
Improving dose calculation accuracy in preclinical radiation experiments using multi-energy element resolved cone-beam CT

Abstract:Abstract Objective. Cone-beam CT (CBCT) in modern pre-clinical small-animal radiation research platforms provides volumetric images for image guidance and experiment planning purposes. In this work, we implemented multi-energy element-resolved (MEER) CBCT using three scans with different kVps on a SmART platform (Precision x-ray Inc.) to determine images of relative electron density (rED) and elemental composition (EC) that are needed for Monte Carlo-based radiation dose calculation. Approach. We performed comprehensive calibration tasks to achieve sufficient accuracy for this quantitative imaging purpose. For geometry calibration, we scanned a ball bearing phantom and used an analytical method together with an optimization approach to derive gantry angle specific geometry parameters. Intensity calibration and correction included the corrections for detector lag, glare, and beam hardening. The corrected CBCT projection images acquired at 30, 40, and 60 kVp in multiple scans were used to reconstruct CBCT images using the Feldkamp–Davis–Kress reconstruction algorithm. After that, an optimization problem was solved to determine images of rED and EC. We demonstrated the effectiveness of our CBCT calibration steps by showing improvements in image quality and successful material decomposition in cases with a small animal CT calibration phantom and a plastinated mouse phantom. Main results. It was found that artifacts induced by geometry inaccuracy, detector lag, glare, and beam hardening were visually reduced. CT number mean errors were reduced from 19% to 5%. In the CT calibration phantom case, median errors in H, O, and Ca fractions for all the inserts were below 1%, 2%, and 4% respectively, and median error in rED was less than 5%. Compared to the standard approach deriving material type and rED via CT number conversion, our approach improved Monte Carlo simulation-based dose calculation accuracy in bone regions. Mean dose error was reduced from 47.5% to 10.9%. Significance. The MEER-CBCT implemented on an existing CBCT system of a small animal irradiation platform achieved accurate material decomposition and significantly improved Monte Carlo dose calculation accuracy.

Abstract:Purpose One technical barrier to patient-specific computed tomography (CT) dosimetry has been the lack of computational tools for the automatic patient-specific multi-organ segmentation of CT images and rapid organ dose quantification. When previous CT images are available for the same body region of the patient, the ability to obtain patient-specific organ doses for CT - in a similar manner as radiation therapy treatment planning - will open the door to personalized and prospective CT scan protocols. This study aims to demonstrate the feasibility of combining deep-learning algorithms for automatic segmentation of multiple radiosensitive organs from CT images with the GPU-based Monte Carlo rapid organ dose calculation. Methods A deep convolutional neural network (CNN) based on the U-Net for organ segmentation is developed and trained to automatically delineate multiple radiosensitive organs from CT images. Two databases are used: The lung CT segmentation challenge 2017 (LCTSC) dataset that contains 60 thoracic CT scan patients, each consisting of five segmented organs, and the Pancreas-CT (PCT) dataset, which contains 43 abdominal CT scan patients each consisting of eight segmented organs. A fivefold cross-validation method is performed on both sets of data. Dice similarity coefficients (DSCs) are used to evaluate the segmentation performance against the ground truth. A GPU-based Monte Carlo dose code, ARCHER, is used to calculate patient-specific CT organ doses. The proposed method is evaluated in terms of relative dose errors (RDEs). To demonstrate the potential improvement of the new method, organ dose results are compared against those obtained for population-average patient phantoms used in an off-line dose reporting software, VirtualDose, at Massachusetts General Hospital. Results The median DSCs are found to be 0.97 (right lung), 0.96 (left lung), 0.92 (heart), 0.86 (spinal cord), 0.76 (esophagus) for the LCTSC dataset, along with 0.96 (spleen), 0.96 (liver), 0.95 (left kidney), 0.90 (stomach), 0.87 (gall bladder), 0.80 (pancreas), 0.75 (esophagus), and 0.61 (duodenum) for the PCT dataset. Comparing with organ dose results from population-averaged phantoms, the new patient-specific method achieved smaller absolute RDEs (mean +/- standard deviation) for all organs: 1.8% +/- 1.4% (vs 16.0% +/- 11.8%) for the lung, 0.8% +/- 0.7% (vs 34.0% +/- 31.1%) for the heart, 1.6% +/- 1.7% (vs 45.7% +/- 29.3%) for the esophagus, 0.6% +/- 1.2% (vs 15.8% +/- 12.7%) for the spleen, 1.2% +/- 1.0% (vs 18.1% +/- 15.7%) for the pancreas, 0.9% +/- 0.6% (vs 20.0% +/- 15.2%) for the left kidney, 1.7% +/- 3.1% (vs 19.1% +/- 9.8%) for the gallbladder, 0.3% +/- 0.3% (vs 24.2% +/- 18.7%) for the liver, and 1.6% +/- 1.7% (vs 19.3% +/- 13.6%) for the stomach. The trained automatic segmentation tool takes This work shows the feasibility to perform combined automatic patient-specific multi-organ segmentation of CT images and rapid GPU-based Monte Carlo dose quantification with clinically acceptable accuracy and efficiency.
A Feasibility Study on Deep Learning-Based Radiotherapy Dose Calculation

Abstract:Purpose: Various dose calculation algorithms are available for radiation therapy for cancer patients. However, these algorithms are faced with the tradeoff between efficiency and accuracy. The fast algorithms are generally less accurate, while the accurate dose engines are often time consuming. In this work, we try to resolve this dilemma by exploring deep learning (DL) for dose calculation. Methods: We developed a new radiotherapy dose calculation engine based on a modified Hierarchically Densely Connected U-net (HD U-net) model and tested its feasibility with prostate intensity-modulated radiation therapy (IMRT) cases. Mapping from an IMRT fluence map domain to a 3D dose domain requires a deep neural network of complicated architecture and a huge training dataset. To solve this problem, we first project the fluence maps to the dose domain using a modified ray-tracing algorithm, and then we use the HD U-net to map the ray-tracing dose distribution into an accurate dose distribution calculated using a collapsed cone convolution/superposition (CS) algorithm. Results: It takes about one second to compute a 3D dose distribution for a typical 7-field prostate IMRT plan, which can be further reduced to achieve real-time dose calculation by optimizing the network. For all eight testing patients, evaluation with Gamma Index and various clinical goals for IMRT optimization shows that the DL dose distributions are clinically identical to the CS dose distributions. Conclusions: We have shown the feasibility of using DL for calculating radiotherapy dose distribution with high accuracy and efficiency.

Abstract:Purpose: This study aimed to compare the dose evaluation methods by constructing simulation models using the Monte Carlo calculation code and propose an evaluation method for cone beam CT (CBCT) that ensures accuracy and practicality. Methods: The Particle and Heavy Ion Transport code System (PHITS) ver. 3.26 was used as the Monte Carlo calculation code. CBCT doses were measured by CB dose index (CBDI) and American Association of Physicists in Medicine task group 111 (TG111) methods. The CBDI was compared with the equilibrium doses obtained by the TG111 method. Results: Although CBDI was lower than equilibrium doses obtained by the TG111 method, its practicality was ensured because it can be measured using the dosimeter and phantom that are commonly used. In contrast, the TG111 method guarantees accuracy, but it is difficult to prepare a long phantom to obtain the equilibrium dose. The TG111 method with a phantom length of 15 cm underestimated the equilibrium dose by 20% compared to that with a phantom length of 45 cm that satisfies the dose equilibrium. Therefore, the equilibrium dose obtained by the TG111 method with a phantom length of 15 cm is multiplied by 1.20 to obtain the equilibrium dose equivalent to that with a phantom length of 45 cm. Conclusion: This study has proposed the dose evaluation method that combines guarantees accuracy and practicality in CBCT.
