Abstract:Objective: To observe the therapeutic effects of bracketless and invisible orthodontic treatment on periodontitis, as well as on gingival crevicular fluid and serum interleukin-6 (IL-6), matrix metalloproteinase-8 (MMP-8) and tumors. The impact of necrosis factor-alpha (TNF-α) levels fills the current knowledge gap regarding the impact of different orthodontic treatment modalities on biomarker levels in periodontitis patients. Methods: 100 patients with malocclusion secondary to periodontitis were selected as subjects.They were divided into a control group (n=50) and a study group (n=50) according to the random number method. The control group was treated with a straight wire appliances, and the study group was given bracketless and invisible orthodontic treatment. Clinical effects, Periodontal indicators [plaque index (PLI), gingival crevicular bleeding index (SBI), gingival index (GI), periodontal pocket probe depth (PD), clinical attachment loss (CAL)], gingival crevicular fluid and serum IL-6, MMP-8 and TNF-α levels and the incidence of adverse reactions were compared between the two groups. The uniqueness of this method is that it compares the impact of traditional straight-wire orthodontic treatment and invisible orthodontic treatment without brackets on biomarker levels and clinical effects in patients with periodontitis. In order to understand the role of orthodontic treatment methods in Provides useful information for use in periodontitis treatment. Results: The main findings of this study highlight the significant impact of bracketless clear braces in improving periodontal indicators and cytokine levels. Patients treated with bracketless clear braces demonstrate better clinical outcomes in periodontitis treatment compared with traditional straight-wire orthodontic treatment. The response rate of the study group was higher than that of the control group (94.00% vs. 72.00%) (P < .05). After 2 years of treatment, PLT, SBI, GI, PD and CAL were decreased in both groups and the observation group was significantly lower than the control group (P < .05). After 6 months of treatment, the levels of IL-6, MMP-8 and TNF-α in gingival crevicular fluid and serum were decreased in both groups, and the observation group was significantly lower than the control group (P < .05). There was no significant difference in the incidence of adverse reactions between the two groups (P > .05). Conclusion: The treatment of periodontitis without brackets has a significant effect, which can improve the periodontal condition and reduce the levels of IL-6, MMP-8 and TNF-α in gingival crevicular fluid and serum. Bracketless invisible braces have shown potential clinical significance in improving periodontal indicators and cytokine levels in patients with periodontitis, providing support for providing more comfortable and effective orthodontic treatment options, which may help promote patients' Oral health. These findings suggest the positive role of bracketless invisible braces in comprehensive periodontal treatment, which is expected to influence the practice of orthodontics and periodontal treatment and improve patient treatment experience and effects.
[Effect of invisible appliance without bracket and self-locking bracket appliance on periodontal and subgingival flora in patients with periodontitis]

Abstract:Purpose: To observe the effect of non-bracket invisible appliance and self-locking bracket appliance on periodontal health and subgingival flora in patients with periodontitis. Methods: One hundred and ten patients with periodontitis who received orthodontic treatment were divided into invisible group and self-locking bracket group. The periodontal index including probing depth (PD), gingival index (GI), sulcular bleeding index (SBI) and plaque index (PLI) ，and the levels of inflammatory factors in the gingival crevicular fluid were examined. Visual analogue scale (VAS) was used to evaluate periodontal pain before and after correction, and the curative effect was evaluated. The status of common pathogenic bacteria in the subgingival plaque of patients after correction was detected by PCR. The data were analyzed with SPSS19.0 software package. Results: Twelve months after treatment and at the end of treatment, PLI of the self-locking bracket group was significantly higher than the invisible group(P＜0.05), but PD, GI and SBI between the two groups had no significant difference. After correction, the inflammatory factors in the invisible group were significantly lower than those in the self-locking bracket group(P＜0.05). At 1d after treatment, pain index in the invisible group was significantly lower than the self-locking group(P＜0.05). The clinical total effective rate of the invisible group was significantly higher than the self-locking group (89.10% vs 76.36%). After treatment, the detection rate and the content of subgingival pathogenic bacteria in the self-locking bracket group were significantly higher than the invisible group(P＜0.05). Conclusions: During orthodontic treatment of periodontitis, wearing non-bracket invisible appliance can effectively inhibit inflammatory response and proliferation of subgingival pathogens, which is more conducive to the maintenance of periodontal health and oral hygiene.
[Effect of different orthodontic techniques on alveolar bone changes in the upper incisor area of patients with periodontitis]

Abstract:Purpose: To explore the effects of different orthodontic techniques on the changes of alveolar bone density, height and gingival crevicular fluid (GCF) bleeding in upper incisor in patients with periodontitis. Methods: Twenty-three patients with moderate periodontitis who underwent orthodontic treatment from January 2016 to December 2019 in the Department of Stomatology of Jiangxi Pingxiang People's Hospital were divided into experimentalexperimental group(n=12) and control group(n=11). Patients in the experimental group were treated with bracket-less invisible appliance, while patients in the control group were treated with conventional lip-side fixed appliance. Changes of gingival crevicular hemorrhage index, probing depth, alveolar bone height and bone density in the incisor area were compared between the two groups before and after treatment. Statistical analysis was performed on data using SPSS 22.0 software package. Results: After treatment, the bone mineral density of the upper incisor alveolar crest (L1) of the two groups decreased, and the rate of decrease in the experimental group was significantly less than that in the control group (P<0.05). The bone density at the apical point of the anatomical root of the control group at 1 mm (L3) was also significantly reduced, but there was no significant change in the experimental group. There was no significant change in the alveolar bone height of the upper incisor area between the two groups before and after treatment (P>0.05). Root resorption of the upper incisors in the experimental group was significantly lower than that in the control group(P<0.05). After treatment, the probing depth and GCF bleeding index of the two groups decreased, and the change in the experimental group was significantly greater than that in the control group(P<0.05). Conclusions: Bracket-free invisible appliance for periodontitis is more conducive to the restoration of alveolar bone mineral density in the upper incisor than the conventional lip-side fixed appliance, and both of which have the same effect on bone height and are safe and effective orthodontic treatment.

Abstract:AIM: Chronic periodontitis leads to gingival swelling, hyperplasia, and tooth mobility, which affects orthodontic treatment. The aim of this study was to investigate the application of cefaclor in orthodontics through micro-implant anchorage in patients with periodontitis. METHODS: A retrospective study was conducted on patients with periodontitis who received micro-implant anchorage treatment in the department of orthodontics at the First People's Hospital of Yongkang City from July 2019 to January 2022. According to different treatment regimens, these patients were divided into the test group (patients receiving cefaclor and micro-implant anchorage treatment) and the control group (patients receiving micro-implant anchorage treatment only). The plaque index (PLI), gingival index (GI), sulcus bleeding index (SBI), and serum inflammatory factor levels were compared between the two groups after treatment. RESULTS: One hundred and five patients were included in the study, (44 males and 61 females, median age 21 [15–25] years), 51 in the cefaclor group and 54 in the no cefaclor group. After treatment, the PLI, GI, and SBI scores in the two groups were higher than those before treatment, and the levels of serum inflammatory markers significantly increased (p < 0.05). After treatment, the PLI, GI, and SBI scores in the test group were significantly lower than those in the control group (p < 0.001). The levels of serum interleukin-1β, interleukin-6, interleukin-8, and tumor necrosis factor-α were significantly lower in the test group, and the interleukin-2 level was higher in the test group (p 0.05). CONCLUSIONS: Cefaclor and micro-implant anchorage have a good clinical effect on orthodontics in patients with periodontitis, improving periodontal health and reducing inflammatory response.

Abstract:Aim To examine the immune responses in patients diagnosed as grade C periodontitis during orthodontic treatment. Materials and Methods Our study included seven orthodontic patients with grade C periodontitis and measured their levels of inflammatory cytokines in gingival crevicular fluid and plasma before orthodontic treatment, during the alignment and levelling phase, and during the detailing and finishing phase. The key signal pathways in the orthodontic process of patients with periodontitis were detected by KEGG analysis. Results Studies have shown that orthodontic treatment brings great improvement to patients with grade C periodontitis, and most of the local/systemic inflammatory cytokines can be reduced after orthodontic treatment. Simultaneously, orthodontic treatment can reduce the percentage of IFN‐γ+ Th1 cells in patients with grade C periodontitis. Abstract:Introduction: Patients with severe periodontitis typically present with pathologic tooth migration. To improve esthetics and masticatory function, orthodontic treatment is required. Research on periodontal orthodontic treatment has been sparse, particularly from the microbial perspective. Hence, we analyzed the microbial and clinical changes in patients with well-controlled periodontitis in the early stage of orthodontic treatment. Methods: Ten patients with well-controlled periodontitis were asked to collect saliva before and 1 and 3 months after appliance placement (T0, T1, and T2, respectively) and underwent clinical examinations before and 1, 3, and 6 months after appliance placement (T0, T1, T2, and T3, respectively). The microbial community of saliva was analyzed by 16S rRNA gene sequencing. Gingival index, the plaque index, and the probing pocket depth were clinically assessed. Results: The plaque index significantly increased from T0 to T1 and decreased at T2 and T3. The probing pocket depth and gingival index increased slightly at T2, but not significantly, in both the high-risk site and low-risk site. The alpha and beta diversity increased at T1. The microbial community structure was similar at T0 and T2. The relative abundance of core genera and periodontal pathogens was stable during the initial 3 months of orthodontic treatment. Conclusions: The orthodontic appliance promoted plaque accumulation and altered the microbial community of patients with well-controlled periodontitis during the first month of orthodontic treatment. The microbial community returned to the basal composition at 3 months after appliance placement, and the periodontal inflammation during the 6-months orthodontic treatment was under control.
The Microbial Changes in Subgingival Plaques of Orthodontic Patients: a Systematic Review and Meta-Analysis of Clinical Trials

Abstract:Abstract Background Orthodontic treatment was found to have an impact on the quantity and constitution of subgingival microbiota. However, contradictory findings regarding the effects of fixed appliances on microbial changes were reported. The aim of this systematic review was to investigate the microbial changes in subgingival plaques of orthodontic patients. Methods The PubMed, Cochrane Library, and EMBASE databases were searched up to November 20, 2016. Longitudinal studies observing microbial changes in subgingival plaques at different time points of orthodontic treatment are included. The methodological quality of the included studies was assessed by Methodological index for non-randomized studies (MINORS). The studies that reported the frequency of subgingival periodontopathogens were used for quantitative analysis. Other studies were analysed qualitatively to describe the microbial changes during orthodontic treatment. Results Thirteen studies were selected, including two controlled clinical trials, three cohort studies and eight self-controlled studies. Four periodontopathogens, including Aggregatibacter actinomycetemcomitans (Aa), Porphyromonas gingivalis (Pg), Prevotella intermedia (Pi) and Tannerella forsythia (Tf), were analysed. Following orthodontic appliance placement, the frequencies of Pg and Aa showed no significant change (P = 0.97 and P = 0.77), whereas the frequency of Tf significantly increased (P < 0.01) during short-term observation (0–3 months). The frequency of Pi showed a tooth-specific difference, as it presented no significant difference (P = 0.25) at the site of the first molar but was significantly increased (P = 0.01) at the incisor. During long-term observation (> = 6 months), two studies reported that the levels of subgingival periodontopathogens exhibited a transient increase but decreased to the pretreatment levels afterwards. After removal of the orthodontic appliance, the four periodontopathogens showed no significant difference compared with before removal. Conclusion The levels of subgingival pathogens presented temporary increases after orthodontic appliance placement, and appeared to return to pretreatment levels several months later. This indicates that orthodontic treatment might not permanently induce periodontal disease by affecting the level of subgingival periodontal pathogen levels. Further studies of high methodological quality are required to provide more reliable evidence regarding this issue.

Abstract:Abstract : We consider a class of dynamic resource allocation problems a specific example of which is the Weapon-Target Assignment problem. This problem is concerned with the optimal assignment of resources in a military engagement. These problems are, in general, NP-Complete, so our aim is to provide insight into the problem and its solution. We will provide analytical results for simple cases of the problem. We will also provide sub-optimal algorithms, together with bounds on the objective function, under certain assumptions. The battle scenario of the military engagement being modeled is as follows. The offense launches a number of weapons ( the targets) which are aimed at valuable assets of the defense. The defense has a number of defensive weapons each of which can engage at most one target. The outcome of such an engagement is stochastic. In the static scenario all weapons are fired simultaneously. In the dynamic scenario some weapons are assigned and fired and the outcomes of these engagements are observed before further assignments are made.

Abstract:Objective To detect IL-23 expression in periodontal ligament induced by orthodontic force under different periodontal conditions and to guide clinic orthodontic treatment of patients with periodontitis.Methods Active periodontitis and non-active periodontitis models were built.Forty two male SD rats were randomly divided into three groups:healthyperiodontium movement,active periodontitis movement and non-active periodontitis movement.50g of mesial force was applied to move unilateral maxillary first molars of all three groups.At day 3 and 7,rats weresacrificed.By means of immunohistochemistry and RT-qPCR,IL-23 mRNA expressionin periodontal ligament were analyzed.Results The difference of IL-23mRNA expression between normal negative control group and healthyperiodontium movement group was not significant (P＞0.05).Compared with healthyperiodontium group,the expression of IL-23 mRNA in non-active periodontitis group had significantly increased (P＜0.05).However,it was less than that of positive periodontitis control (P＜0.01).Active periodontitis movement group presented a remarkable increase in IL-23mRNA expression than normal negative control,positive periodontitis control,healthyperiodontium and non-active periodontitis groups (P＜0.01).Conclusions IL-23 plays a role in regulating orthodontictooth movement with periodontitis.Orthodontic force causes limited increase of IL-23 mRNA expression in non-active periodontal tissues.During orthodontic treatment,careful monitoring of the periodontal predisposing factors is necessary.
The role of amyloidogenic proteins as a meeting point of type 2 diabetes and Parkinson's disease pathways

Abstract:The impairment of amyloid formation is associated with Parkinson disease (PD) and type 2 diabetes (T2D) in which the primary pathological characteristics are assembly of asynuclein into amyloid fiber and islet amyloid polypeptide (IAPP) formation in pancreatic b-cells, respectively. Horvath and Wittung-Stafshede tried to shed light on the crossreactivity between amyloid formation in PD and T2D by in vitro measurement of a-synuclein, IAPP, and pro-IAPP. The authors harbored 3 steps of biophysical in vitro methods to determine the fact that various peptides are in contact with each other in PD triggered by T2D pathogenesis. The results of individual protein aggregation reactions showed that larger a-synuclein amyloids may be assemblies of more than 1 a-synuclein proto-fibril. In all 3 cases (a-synuclein, IAPP, and pro-IAPP), preformed amyloid fiber seeds sped up the aggregation of monomers of the same protein. The cross-seeding of amyloid formation was studied and showed that preformed IAPP and a-synuclein amyloids seeds sped up a-synuclein amyloid and pro-IAPP formation, respectively, and preformed a-synuclein and pro-IAPP amyloid seeds inhibited the normal aggregation reaction of IAPP amyloid and a-synuclein amyloid, respectively. Atomic force microscopy images showed that the mixtures made by coaggregation of a-synuclein and pro-IAPP describe thicker fibrils in comparison with pro-IAPP alone. The pro-IAPP monomers inhibited a-synuclein aggregation in a concentration-dependent manner. The results contrasted when a-synuclein monomers were added to high concentrations of pro-IAPP; in such cases, a-synuclein monomers became incorporated into the pro-IAPP amyloids without impacting formation speed but allowing for increased thioflavin T emission. The differences in thickness and fragility together with the changed reaction kinetics imply that coassembled amyloids were formed between a-synuclein and IAPP. The majority of studies so far considered the individual disease-specific peptides. The authors in the present study shed light on how the interactions of peptides associated with different amyloid diseases may intermediate amyloid formation pathogenicity. In conclusion, the putative cross-reaction between a-synuclein and processed or unprocessed IAPP has been addressed in this survey for the first time, notably, that a-synuclein aggregation could be elevated by IAPP protein (both as monomer and amyloid seed). This result presents a likely explanation for why patients with T2D are more predisposed to PD, whereas PD patients are not prone to T2D by IAPP amyloid formation inhibition caused by a-synuclein amyloid aggregation.
Metagenomic Analysis of Oral and Intestinal Microbiome of Patients During the Initial Stage of Orthodontic Treatment.

Abstract:Introduction: This prospective study analyzed changes in the oral and intestinal microbiomes in patients before and after fixed orthodontic treatment, elucidating the impacts of fixed orthodontic treatment on patient health and metabolism. Methods: Metagenomic analysis was conducted on stool, dental plaque, and saliva samples from 10 fixed orthodontic patients. All the samples were sequenced with Illumina NovaSeq 6000 with a paired-end sequencing length of 150 bp. Identification of taxa in metagenomes and functional annotation of genes of the microbiota were performed using the data after quality control. Clinical periodontal parameters, including the gingiva index, plaque index, and pocket probing depth, were examined at each time point in triplicates. Patients also received a table to record their oral hygiene habits of brushing, flossing, and dessert consumption frequency over 1 month. Results: The brushing and flossing times per day of patients were significantly increased after treatment compared with baseline. The number of times a patient ate dessert daily was also fewer after treatment than at baseline. In addition, the plaque index decreased significantly, whereas the pH value of saliva, gingiva index, and pocket probing depth did not change. No significant differences were observed between the participants before and after orthodontic treatment regarding alpha-diversity analysis of the gut, dental plaque, or saliva microbiota. However, on closer analysis, periodontal disease-associated bacteria levels in the oral cavity remain elevated. Alterations in gut microbiota were also observed after orthodontic treatment. Conclusions: The richness and diversity of the microbiome did not change significantly during the initial stage of fixed orthodontic treatment. Effects Of Minocycline Combined With Tinidazole For Treatment Of Chronic Periodontitis

Abstract:Purpose: To investigate the therapeutic effects of minocycline combined with tinidazole in the treatment of chronic periodontitis (CP). Methods: Seventy-three CP patients treated May 2018–December 2019 at Yuyao People’s Hospital (Yuyao, China) were enrolled in this study: 34 were treated with minocycline alone (control group; CG) and 39 were treated with a combination of minocycline and tinidazole (observation group; OG). Both groups were treated continuously for four weeks and plaque index (PLI), bleeding index (BI), periodontal pocket depth (PD), periodontal attachment level (PAL) and alveolar bone height were compared before and after treatment. Pain was evaluated using the visual analogue scale (VAS). Levels of TNF-α and IL-6 before and after treatment were determined using an enzyme-linked immunosorbent assay. Adverse reactions were compared. Results: In each group, PLI, BI, PD, PAL and alveolar bone height were lower after treatment (P<0.05), and those in OG were lower than those in CG (P<0.05). TNF-α and IL-6 levels in both groups were lower after treatment (P<0.05), and the levels in serum of the OG were lower than those of the CG (P<0.05). After treatment, the VAS in OG was lower than that of CG (P<0.05). There was no significant difference in adverse reactions between groups (P>0.05). Conclusion: Minocycline combined with tinidazole was more effective in treating CP than minocycline alone. This drug combination improved the periodontal indexes and inflammatory reaction of CP and relieved their pain. No significant difference in adverse reactions was seen.

Abstract:Aim: Temporomandibular joint disorder (TMD), which affects the masticatory muscles, temporomandibular joint, and surrounding tissues, can manifest as inflammation. This study aims to explore the expression levels of the inflammatory biomarkers, interleukin (IL)-1β and C-reactive protein (CRP), in TMD patients who have undergone orthodontic treatment. Materials and methods: Buccal swabs from 105 postorthodontic treatment patients were analyzed using real-time polymerase chain reaction to assess the expression levels of IL-1β and CRP in each group after messenger ribonucleic acid extraction. Patients were also examined using the Diagnostic Criteria for TMD (DC/TMD) to determine if they met the criteria for a TMD diagnosis. The TMD group was subdivided into three categories based on the DC/TMD. Results: The study included 37 patients who did not develop TMD (group 0) and 68 participants who developed TMD after orthodontic treatment, including 17 with pain-related TMDs (group 1), 29 with intra-articular TMDs (Group 2), and 22 with combined pain-related and intra-articular TMDs (group 3). CRP expression was higher than IL-1β in groups 1 and 2, and IL-1β expression was higher than CRP in group 3. The Kruskal-Wallis test showed that IL-1β and CRP expression levels in groups 1, 2, and 3 were not statistically different. Sex and adult age had considerable effects on the occurrence of TMD in patients after orthodontic treatment. Conclusions: Higher IL-1β expression was found in postorthodontic treatment patients with more complex TMD. This study strengthens the evidence of inflammation through IL-1β and CRP expression in individuals with TMD, especially after orthodontic treatment.

Abstract:OBJECTIVE:To evaluate the effect of scaling and root planing combined with local application of 2% minocycline hydrochoride ointment(Periocline) on the level of collagenase activity releasing from PMNs in GCF.METHODS:This was a randomized, single-blinded, split mouth design study. The target sites of 31 patients with moderate to severe periodontitis (PD > 5 mm) was randomized by left-side and right-side into one of the two groups (the test group and the control group). Subsequently each patient was scheduled for two appointments for the full-month supra-gingival scaling and oral hygiene instruction. At baseline all patients received subgingival scaling and root planing and were followed by 4-time applications of Periocline with one-week interval for totally four weeks in the test group. All patients were taken GCF for measuring collagenase by the method of Nakashima et al (1996) and were examined based on four parameters, including plaque index, pocket depth, attachment loss and bleeding on probing.RESULTS:Four weeks after root planing with 4-time applications of Periocline, all clinical parameters and active collagenase in GCF decreased significantly in the test groups[(593 +/- 112) mU/sample vs (311 +/- 98) mU/sample, P < 0.001]. On the other hand, simple root-planing therapy had limited effect on the active collagenase[(611 +/- 123) mU/sample vs (523 +/- 127) mU/sample], although all clinical parameters were also reduce significantly. Interestingly, seven weeks later, gingival inflammation and the active collagenase were reduced more significantly than those after four-week root planing in both groups[Test: (311 +/- 98) mU/sample vs (207 +/- 57) mU/sample;CONTROL:(523 +/- 127) mU/sample vs (345 +/- 117) mU/sample, P < 0.001]. At eleven week, gingival inflammation and the level of collagenase activity were rebounded in the control group[(467 +/- 108) mU/sample]. However, in the test group they were kept in lower level[(213 +/- 121) mU/sample]. The level of collagenase activity showed significantly positive correlation with GCF volumes and AL in two groups.CONCLUSION:These results indicate that local application of 2% minocycline hydrochoride ointment may effectively inhibit the level of active collagenase from PMNs and may inhibit connective tissue breakdown by inhibiting neutrophil collagenase.
Subgingival Microbial Changes During the First 3 Months of Fixed Appliance Treatment in Female Adult Patients.

Abstract:Although periodontal diseases during fixed appliance treatment are a common issue, few studies have focused on the clinical and microbial factors associated with orthodontic appliances. Hence, we investigated changes in the subgingival microbial community and their association with periodontal changes at the early stage of fixed appliance treatment. Subgingival plaques from ten female patients with fixed appliances were obtained at three time points: before, 1 month and 3 months after the placement of the brackets (T0, T1 and T2). The 16S rRNA gene sequencing was used to analyze the microbial community of the subgingival plaque. The Plaque Index (PI) and Gingival Bleeding Index (GBI) were also recorded. The GBI significantly increased at T2, and the PI showed a temporary increase without a significant difference. The alpha diversity indices were stable. However, the beta diversity was significantly higher at T2 compared to T0 and T1. The relative abundance of core microbiomes at the genus level was relatively stable. Four periodontal pathogens at the species level, including Prevotella intermedia (Pi), Campylobacer rectus (Cr), Fusobacterium nucleatum (Fn), and Treponema denticola (Td), increased without significant differences. The subgingival microbial community affected by fixed appliance treatment might cause transient mild gingival inflammation.

Types of Studies Reviewed. The authors systematically searched the PubMed, Web of Science, Cochrane Library, and Embase databases to collect related studies. After extracting data and assessing quality, the authors performed a meta-analysis and trial sequential analysis. The authors used the Grading of Recommendations Assessment, Development and Evaluation system to assess the quality of the evidence. Results. The authors included 9 studies in the quantitative synthesis analysis. Clear aligners were better for periodontal health, including plaque index (mean difference [MD], -0.53; 95% confidence interval [CI], -0.85 to -0.20; P = .001), gingival index (MD, -0.27; 95% CI, -0.37 to -0.17; P < .001), and probing depth (MD, -0.35; 95% CI, -0.67 to -0.03; P = .03), than were fixed appliances. However, the trial sequential analysis outcome indicated a false-positive meta-analysis result for probing depth. The authors downgraded the level of the evidence because of the risk of bias and inconsistency. Conclusions and Practical Implications. Clear aligners were better for periodontal health than fixed appliances and might be recommended for patients at high risk of developing gingivitis. However, high-quality studies still are required.

An Observational Study on Changes in the Oral and Gut Microbiota through Professional Mechanical Tooth Cleaning, including Tooth-Brushing Instructions in Patients with Multi-Bracket Appliances

Abstract:Multi-bracket appliances have long been established as tools for treating malocclusions. However, the complex construction and difficulty in cleaning due to their long-term intraoral retention have hindered the prevention of caries and periodontitis. In this study, professional mechanical tooth cleaning (PMTC), including tooth-brushing instructions, was continuously performed for 3 months in 24 patients who had worn multi-bracket appliances for more than 6 months, and changes in the oral and gut microbiota were examined using one-way repeated-measures analysis of variance. Additionally, changes in bacterial flora associated with different treatment durations were verified using the Pearson correlation coefficient. The results showed that continuous PMTC significantly reduced the amount of plaque in the oral cavity. No significant changes were observed in the oral or gut microbiota and no significant increase in pathogenic bacteria was observed. Therefore, our results suggest that continuous PMTC during orthodontic treatment with multi-brackets may inhibit the growth of pathogenic bacteria by maintaining a clean oral environment and avoiding dysbiosis in both the oral and gut microbiota. Significant changes in the gut microbiota with different treatment durations suggested that differences in food intake and food choices at each treatment stage of orthodontic treatment may affect the gut microbiota.

Clinical study on minocycline combined with Iodine Glycerol in treatment of periodontitis

Abstract:Objective To investigate the clinical effect of minocycline combined with Iodine Glycerol in treatment of periodontitis. Methods Patients (90 cases and 107 teeth) with periodontitis in Shenzhen Yantian District People's Hospital from February 2014 to September 2015 were divided into the control group (45 cases, 55 teeth) and treatment group (45 cases, 52 teeth) according to serial number method. Patients in the control group were injection administered with iodine glycerol in the periodontal bag, no diet or mouthwash within 1 h, once weekly. Patients in the treatment group were injection administered with Minocycline Hydrochloride Ointment in the periodontal bag on the basis of the control group, no diet or mouthwash within 1 h, once weekly. Patients in two groups were treated for four weeks. After treatment, the clinical efficacies were evaluated, and inflammation indexes and periodontal indexes were compared.Results After treatment, the clinical efficacies in the control and treatment groups were 78.18% and 96.15%, respectively, and there was difference between two groups (P < 0.05). After treatment, the levels of hs-CRP, TNF-α, EA and PGE2 levels in two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the observational indexes in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P<0.05). After treatment, GI, MD, PD, AL and ABL in two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the observational indexes in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P<0.05).Conclusion Minocycline Hydrochloride Ointment combined with iodine glycerol has clinical curative effect in treatment of periodontitis, can decrease inflammatory response, improve periodontal status, and promote periodontal tissue recovery, which has a certain clinical application value.
