Abstract:Back pain is common and costly. Although lumbar disc degeneration has long been regarded as a major contributor to back pain, how disc degeneration leads to back pain remains unclear. Recent studies observed microglia activation in the spinal cord after disc degeneration, suggesting activated microglia may be involved in discogenic back pain. To determine whether microglia activation participates in disc degeneration-induced back pain, we used a modified disc puncture-induced degeneration-related back pain mouse model to examine the changes in spinal microglia and investigate the potential link between microglia activation and discogenic back pain. In this study, 46 CX3CR1(GFP/+) male mice were used in experimental and sham groups. A modified posterolateral retroperitoneal approach was used to expose the L3/L4 disc to induce the needle puncture in the experimental group. Behavioral tests, including grip force and physical function, were used to measure back pain at pre- and postsurgery. The L3 dorsal root ganglions and lumbar spinal cord were obtained at postoperative weeks 1 to 4 followed by immunofluorescence with different antibodies. Micrographs were obtained by confocal microscopy, and morphometric measurements of microglia were analyzed using Imaris. The punctured disc underwent progressive degeneration and mice with disc degeneration showed impaired grip force and physical function. Compared to the control mice, the number of microglia in the lumbar spinal cord was significantly increased in the disc-punctured animals. Moreover, accumulated microglia exhibited larger soma size and lesser ramification in the disc-injured mice. Immunofluorescence demonstrated colony-stimulating factor 1, a cytokine that promotes microglia repopulation, was significantly increased in L3 dorsal root ganglions, whereas its receptor colony-stimulating factor 1 receptor was upregulated on microglia in the disc-injured mice. In summary, lumbar disc puncture caused progressive disc degeneration which induced microglia activation and back pain in mice. Increased colony-stimulating factor 1/colony-stimulating factor 1 receptor signaling is involved in the disc degeneration-induced microglia activation and back pain.

Abstract:Study Design. An autopsy study.Objective. To investigate associations between various types of lumbar endplate lesions, disc degeneration (DD), and back pain history.Summary of Background Data. The well-innervated vertebral endplate has been suspected as a source of back pain. Previously, we observed 4 types of lumbar endplate lesions with distinct morphological characteristics. Their roles in DD and back pain remain unclear.Methods. From a lumbar spine archive of 136 men (mean age, 52 yr), back pain, back injury, and occupation history data for 69 subjects and discography data for 443 discs from 109 subjects were available for study. Back pain history was categorized as none, occasional, or frequent. DD was judged from discography. Endplate lesions were classified as Schmorl's nodes, fracture, erosion, or calcification, and lesion size was rated as none, small, moderate, or large. Associations between endplate lesions and DD, back pain history, back injury, and occupation history were examined.Results. Presence of endplate lesions was associated with frequent (odds ratio [OR] = 2.57) but not occasional back pain. However, large endplate lesions were associated with both occasional (OR = 8.68) and frequent (OR = 17.88) back pain. This association remained after further controlling for DD. Also, the presence of each type of endplate lesion was associated with adjacent DD (OR = 2.40-9.71), with larger lesions associated with more severe DD. Endplate erosion lesions were more strongly associated with adjacent DD than Schmorl's nodes. Although back injury history was associated with the presence of fracture and erosion lesions, heavy occupation was associated with the presence of Schmorl's nodes.Conclusion. Endplate lesions are associated with back pain as well as being closely associated with adjacent DD, with a clear dosage effect. Different types of endplate lesions seem to have different magnitudes of associations with DD. Lumbar endplate lesions may be an important key to better understand both DD and back pain.

Abstract:A cross-sectional autopsy study.This study investigated the prevalence of endplate lesions, classified them on the basis of morphological features, and determined their distribution patterns through direct measurements of cadaveric spines as well as examining their associations with age.Endplate lesions may play a role in disc degeneration and back pain; however, related research has been rare. While Schmorl's nodes have received some attention, other endplate pathologies have been largely ignored. A systematic study of endplate lesions is needed to reveal the types and prevalence of lesions present in the adult lumbar spine.We studied 1148 vertebral endplates (L1-S1) from the cadaveric spines of 136 men (mean age, 52 yr). On the basis of morphological characteristics, 4 types of endplate lesions were identified, including Schmorl's nodes, fracture, erosion, and calcification. The lesion location, size, and involved endplate components were evaluated to depict their distribution patterns. The associations between endplate lesion findings and age were also examined.Endplate lesions were found in 45.6% of lumbar vertebral endplates. Schmorl's nodes were the most common and usually were small, located centrally, and most common in the upper lumbar region. Erosion and calcification lesions were relatively large and most common in the lower lumbar region. The presence of lesions on 1 endplate of the disc was associated with presence of lesions on the opposing endplate (odds ratio = 8.0, P < 0.001). Greater age was associated with the presence of each type of endplate lesion.Endplate lesions are common and tend to affect both adjacent endplates of a disc simultaneously. The distribution patterns of the various types of endplate lesions differ, suggesting that they may have different pathogenic origins. Age or associated factors may play an important role in the pathogenesis of endplate lesions.

Abstract:Background Modic changes are the MRI signal changes of degenerative lumbar vertebral endplate and which lead to or accelerate intervertebral disc degeneration. NLRP3, caspase-1, and interleukin-1β (IL-1β) play a pivotal role in the pathogenesis of many inflammatory diseases, such as osteoarthritis. However, the roles of IL-1β and its activators caspase-1 and NLRP3 are unclear in the degenerative endplate. Questions/purposes We asked: (1) What are the degenerative changes of the histologic features and chondrogenic markers’ gene expressions between the cartilaginous endplates of patients with Modic changes and trauma (control)? (2) How does the NLRP3/caspase-1/IL-1β axis in the cartilaginous endplates of patients with Modic changes compare with control (trauma) specimens? Methods Surgical specimens of cartilaginous endplates were divided into Modic changes (n = 56) and the trauma control (n = 16) groups. Hematoxylin and eosin and safranin O staining of cartilaginous endplate tissues were done to evaluate the extracellular matrix. Reverse transcription-polymerase chain reaction was performed on these tissues to investigate mRNA expression of type II collagen (Col II), SOX-9, matrix metalloproteinase-3, and a disintegrin like and metalloproteinase thrombospondin type I motifs-5. NLRP3, caspase-1, and IL-1β were evaluated by reverse transcription-polymerase chain reaction and immunohistochemistry. Results Hematoxylin and eosin and safranin O staining showed the extracellular matrix degraded in the cartilaginous endplates of patients with Modic changes but not in the control cartilaginous endplates. Chondrogenic Col II (p = 0.024) and SOX9 (p = 0.053) were downregulated in the Modic changes group compared with the control group. In contrast to the control group, the transcriptional levels of NLRP3 (p < 0.001), caspase-1 (p = 0.054), and IL-1β (p = 0.001) were all upregulated in the Modic changes group. Conclusions The expression of NLRP3, caspase-1, and IL-1β was upregulated in the patients with low back pain and Modic changes on MRI compared with patients with vertebral burst fracture without degenerative changes on MRI. The data suggest the NLRP3/caspase-1/IL-1β axis may be implicated in lumbar cartilaginous endplate degeneration. Clinical Relevance The NLRP3/caspase-1/IL-1β axis is active in cartilaginous endplates of patients with Modic changes and inflammatory cascades can exacerbate the cartilaginous endplate degeneration which may act as a trigger for intervertebral disc degeneration and low back pain. If these findings can be confirmed by others, we hope that new and effective therapy could be developed directed against this target.

Abstract:Objective: While cartilaginous endplate (CEP) avulsion is a common finding in discectomy due to lumbar disc herniation, its roles in residual back and leg pain, associations with Modic changes (MCs) and endplate defects (EPD) remain unknown.Design: Patients with a single-level lumbar disc herniation who underwent endoscopic discectomy were studied. On MR images, the adjacent endplates of the herniated disc were assessed for MCs and EPD. The presence of CEP avulsion was examined under endoscopic and visualized inspection. Back and leg pain were evaluated by a numeric rating scale (NRS) and the Oswestry Disability Index. Associations of CEP avulsion with adjacent MCs, EPD, and residual back and leg pain were examined. In addition, histological features of avulsed CEP were determined using gross staining and immunohistochemical methods.Results: A total of 386 patients were included. CEP avulsion was found in 166 (43%) patients, and adjacent MCs and EPD were observed in 117 (30.3%) and 139 (36%) patients. The presence of CEP avulsion was associated with greater age, adjacent MCs (OR = 2.60, 95%CI [1.61-4.19]) and EPD (OR = 1.63, 95%CI [1.03-2.57]). Among the 187 patients with >= 2 years follow-up, CEP avulsion was associated with residual back pain (OR = 2.49, 95%CI [1.29-4.82]) and leg pain (OR = 2.25, 95%CI [1.04-4.84]). Histologically, the avulsed CEP was characterized by multiple defects, apparent inflammation, and nucleus invasion, as well as the upregulation of IL-1 beta, caspase-1, and NLRP3 inflammasome.Conclusion: CEP avulsion was associated with MCs, EPD, and residual back and leg pain after discectomy, which may be attributed to NLRP3 inflammasome related inflammations. Abstract:Abstract Purpose The composition of the subchondral bone marrow and cartilage endplate (CEP) could affect intervertebral disc health by influencing vertebral perfusion and nutrient diffusion. However, the relative contributions of these factors to disc degeneration in patients with chronic low back pain (cLBP) have not been quantified. The goal of this study was to use compositional biomarkers derived from quantitative MRI to establish how CEP composition (surrogate for permeability) and vertebral bone marrow fat fraction (BMFF, surrogate for perfusion) relate to disc degeneration. Methods MRI data from 60 patients with cLBP were included in this prospective observational study (28 female, 32 male; age = 40.0 ± 11.9 years, 19–65 [mean ± SD, min–max]). Ultra-short echo-time MRI was used to calculate CEP T2* relaxation times (reflecting biochemical composition), water-fat MRI was used to calculate vertebral BMFF, and T1ρ MRI was used to calculate T1ρ relaxation times in the nucleus pulposus (NP T1ρ, reflecting proteoglycan content and degenerative grade). Univariate linear regression was used to assess the independent effects of CEP T2* and vertebral BMFF on NP T1ρ. Mixed effects multivariable linear regression accounting for age, sex, and BMI was used to assess the combined relationship between variables . Results CEP T2* and vertebral BMFF were independently associated with NP T1ρ ( p = 0.003 and 0.0001, respectively). After adjusting for age, sex, and BMI, NP T1ρ remained significantly associated with CEP T2* ( p = 0.0001) but not vertebral BMFF ( p = 0.43). Conclusion Poor CEP composition plays a significant role in disc degeneration severity and can affect disc health both with and without deficits in vertebral perfusion.

Abstract:To describe the microarchitecture of the annulus-endplate region with reference to disc herniation.Twenty-five motion segments (L1-2 to L5-S1) were harvested from five cadaveric spines (four males, one female). Superior and inferior annulus-endplate junctions were divided into anterior, lateral and posterior regions. Tissue blocks were fixed, decalcified, and embedded in paraffin. Thin sections (4 µm) were stained with H&E and Masson's trichrome for light microscopy.Annulus fibrosus presents a laminated structure, with lamellae becoming wider and more disordered close to the nucleus. Each lamella comprises parallel bundles of collagen fibers. On entering the hyaline cartilage endplates, fiber bundles from the outer annulus often change direction, branch into sub-bundles separated by hyaline cartilage matrix, and no longer exhibit the undulating "crimp" structure of the annulus. Their three-dimensional multileaf morphology appears to provide direct mechanical anchorage in vertebral bone. Calcification is present at the annulus-cartilage and cartilage-bone interfaces, especially posteriorly. In the inner annulus and nucleus, annulus fibers branch and merge obliquely with the hyaline cartilage matrix. The diameter of collagen fiber bundles and sub-bundles increased significantly in the outer annulus compared with middle annulus, and increased at lower spinal levels, but no significant differences were observed between superior and inferior endplates.The branching of annulus collagen fiber bundles within the endplates increases the interface area with surrounding matrix, reducing shear stresses and (probably) increasing strength. Calcification may also increase strength. These observations can explain why disc herniations often contain cartilage and bone fragments from the endplate.

Abstract:Low back pain significantly impacts individuals' quality of life, with intervertebral disc degeneration (IDD) being a primary contributor to this condition. Currently, IDD treatment primarily focuses on symptom management and does not achieve a definitive cure. The cartilage endplate (CEP), a crucial nutrient‐supplying tissue of the intervertebral disc, plays a pivotal role in disc degeneration. This review examines the mechanisms underlying CEP degeneration, summarizing recent advancements in understanding the structure and function of CEP, the involvement of various signaling pathways, and the roles of cartilage endplate stem cells (CESCs) and exosomes (Exos) in this process. The aim of this review is to provide a comprehensive reference for future research on CEP. Despite progress in understanding the role of CEP in IDD, the mechanisms underlying CEP degeneration remain incompletely elucidated. Future research poses significant challenges, necessitating further investigations to elucidate the complexities of CEP.

Morphometrics and Lesions of Vertebral End Plates Are Associated with Lumbar Disc Degeneration: Evidence from Cadaveric Spines.

Abstract:BACKGROUND:Prior studies have suggested that vertebral end plate morphometrics and lesions may play a role in disc degeneration. However, most prior end plate studies have been based on radiographic images, and findings of associations between end plate morphometrics (measurements of size and shape) and disc degeneration remain controversial. The present study investigated the prevalence of vertebral end plate lesions and determined end plate morphometrics through direct measurements of cadaveric spines, and it examined the association of these factors with disc degeneration.METHODS:We studied 600 vertebral end plates and 313 intervertebral discs from the cadaveric lumbosacral spines of seventy-six men (mean age, fifty-one years). Discography was performed to evaluate disc degeneration as indicated by disruption of the anulus fibrosus. The shape of the vertebral end plate and the presence of any lesions were visually evaluated. Lesions were rated as absent, small to moderate, or large. In addition, each end plate was digitized to quantify its area, circularity, and concavity. The association of end plate morphometrics and lesions with disc degeneration was examined.RESULTS:Vertebral end plate lesions were found in 72% (fifty-five) of the seventy-six lumbar spines and in 32.8% (197) of the 600 end plates. The presence of end plate lesions was associated with disc degeneration, with larger lesions being associated with more severe disc degeneration (odds ratio, 2.31 for small to moderate lesions [p < 0.01] and 3.54 for large lesions [p < 0.001]). Greater end plate area was also associated with more severe disc degeneration (odds ratio, 1.2 per cm2 [p < 0.05]).CONCLUSIONS:Vertebral end plate lesions were common and were associated with adjacent disc degeneration, with greater lesion size being associated with more severe disc degeneration. End plate morphometrics, particularly greater end plate size, may also play a role in the pathogenesis of disc degeneration.
Degeneration alters structure‐function relationships at multiple length‐scales and across interfaces in human intervertebral discs

Abstract:<p>Intervertebral disc (IVD) degeneration and associated back pain place a significant burden on the population. IVD degeneration is a progressive cascade of cellular, compositional, and structural changes, which results in a loss of disc height, disorganization of extracellular matrix architecture, tears in the annulus fibrosus which may involve herniation of the nucleus pulposus, and remodeling of the bony and cartilaginous endplates (CEP). These changes to the IVD often occur concomitantly, across the entire motion segment from the disc subcomponents to the CEP and vertebral bone, making it difficult to determine the causal initiating factor of degeneration. Furthermore, assessments of the subcomponents of the IVD have been largely qualitative, with most studies focusing on a single attribute, rather than multiple adjacent IVD substructures. The objective of this study was to perform a multiscale and multimodal analysis of human lumbar motion segments across various length scales and degrees of degeneration. We performed multiple assays on every sample and identified several correlations between structural and functional measurements of disc subcomponents. Our results demonstrate that with increasing Pfirrmann grade there is a reduction in disc height and nucleus pulposus T2 relaxation time, in addition to alterations in motion segment macromechanical function, disc matrix composition and cellular morphology. At the cartilage endplate‐vertebral bone interface, substantial remodeling was observed coinciding with alterations in micromechanical properties. Finally, we report significant relationships between vertebral bone and nucleus pulposus metrics, as well as between micromechanical properties of the endplate and whole motion segment biomechanical parameters, indicating the importance of studying IVD degeneration as a whole organ.</p>

Abstract:STUDY DESIGN:This study investigated the expression of 2 types of ADAMTS in human intervertebral cartilage endplate (CEP) and related mechanisms concerning tumor necrosis factor α (TNF-α)-induced nuclear factor κB signaling pathway. OBJECTIVE:To determine which type of ADAMTS is more strongly expressed and the role of TNF-α in CEP. SUMMARY OF BACKGROUND DATA:ADAMTS-4 and ADAMTS-5 were proven to be essential in the degeneration of articular cartilage and intervertebral disc. CEP is an important structure adjacent to the disc. However, the activities of ADAMTS in CEP are unclear. METHODS:CEPs were obtained from subjects after spinal surgery and categorized as members of either the Modic change group or the control group. Sections of these tissues were stained with hematoxylin-eosin, safranin O, and immunohistochemistry procedures for ADAMTS-4, ADAMTS-5, and TNF-α. Transcriptional levels of aggrecan, type I collagen, type II collagen, type X collagen, ADAMTS-4, ADAMTS-5, and TNF-α were investigated by quantitative real-time polymerase chain reaction. In addition, the effect of TNF-α on ADAMTS-5 and its potential mechanisms are investigated in cultured bovine endplate chondrocytes in vitro. RESULTS:Our data demonstrated that the degenerative changes associated with the expression of extracellular matrix proteins were correlated with increased levels of ADAMTS-5, but not ADAMTS-4, in the CEP of patients with Modic changes. The expression levels of TNF-α in the Modic change group were significantly increased, which was correlated with the enhanced expression of ADAMTS-5. Additional in vitro investigation confirmed that TNF-α could upregulate the expression of ADAMTS-5 by activating nuclear factor κB pathway in cultured bovine endplate chondrocytes. CONCLUSION:We conclude that the upregulation of TNF-α and ADAMTS-5, but not ADAMTS-4, may play an important role in degenerative CEP-induced low back pain. LEVEL OF EVIDENCE:N/A.

Abstract:Abstract The cartilaginous endplates (CEP) are key components of the intervertebral disc (IVD) necessary for sustaining the nutrition of the disc while distributing mechanical loads and preventing the disc from bulging into the adjacent vertebral body. The size, shape, and composition of the CEP are essential in maintaining its function, and degeneration of the CEP is considered a contributor to early IVD degeneration. In addition, the CEP is implicated in Modic changes, which are often associated with low back pain. This review aims to tackle the current knowledge of the CEP regarding its structure, composition, permeability, and mechanical role in a healthy disc, how they change with degeneration, and how they connect to IVD degeneration and low back pain. Additionally, the authors suggest a standardized naming convention regarding the CEP and bony endplate and suggest avoiding the term vertebral endplate. Currently, there is limited data on the CEP itself as reported data is often a combination of CEP and bony endplate, or the CEP is considered as articular cartilage. However, it is clear the CEP is a unique tissue type that differs from articular cartilage, bony endplate, and other IVD tissues. Thus, future research should investigate the CEP separately to fully understand its role in healthy and degenerated IVDs. Further, most IVD regeneration therapies in development failed to address, or even considered the CEP, despite its key role in nutrition and mechanical stability within the IVD. Thus, the CEP should be considered and potentially targeted for future sustainable treatments.
Expression of Matrix Metalloproteinases, Tissue Inhibitors of Metalloproteinases, and Interleukins in Vertebral Cartilage Endplate

Abstract:Objective: Turnover of cartilage endplate extracellular matrix (ECM) may play an important role in disc degeneration and low back pain (LBP). However, the expression pattern of pro-inflammatory factors, matrix metalloproteinases (MMP), and tissue inhibitors of metalloproteinases (TIMP) in the cartilage endplates (CEP) of intervertebral discs (IVD) is not understood. We aimed to examine the transcriptional levels of MMP, TIMP, and interleukins (IL), and the correlations between them. Methods: Thirty degenerated cartilage endplate samples from patients with LBP who underwent lumbar fusion surgery were included in the degenerated group. Ten patients without LBP history who underwent lumbar surgery because of vertebral burst fractures were included in the control group. The degenerative severity of the samples was evaluated by MRI, and hematoxylin-eosin and safranin O-fast green (SO-FG) staining. Real-time polymerase chain reaction (RT-PCR) was used to detect the mRNA levels of MMP-1, MMP-3, MMP-9, MMP-13, TIMP-1, TIMP-2, TIMP-3, IL-1α, IL-1β, and IL-6. The correlations between the levels of these genes were tested using Spearman's rho test. Results: Hematoxylin-eosin and SO-FG staining confirmed a decrease in cell number and proteoglycans in the degenerated cartilage endplate. MRI showed significant signal changes in degenerated cartilage endplates. Patients in the degenerated group showed a higher rate of endplate Modic changes when compared with the control group. MMP-3, MMP-9, TIMP-3, IL-1α, and IL-1β were elevated with statistical significance, while MMP-1, MMP-13, TIMP-1, TIMP-2, and IL-6 were changed without statistical significance or remained unchanged. Expression of MMP-3 was positively correlated with IL-1α (Spearman coefficient, 0.486; P < 0.05); expression of TIMP-3 was positively correlated with MMP-9, IL-1α, and IL-1β (Spearman coefficient, 0.577, 0.407, and 0.571, respectively; P < 0.05). Conclusion: MMP-3, MMP-9, TIMP-3, IL-1α, and IL-1β may play a role in the process of cartilage endplate degeneration. MMP-3 may be regulated by IL-1α, and TIMP-3 might be associated with MMP-9 and regulated by IL-1α and IL-1β.

You can register here Intervertebral disc degeneration (IVDD) is the leading cause of low back pain, where degeneration and death of nucleus pulposus cells within the intervertebral disc (IVD) can be obviously revealed.This degeneration can result in an imbalance in the extracellular matrix due to the loss of proteoglycans and water content, which can further lead to catabolic and anabolic dysfunction of the IVD. Recently, the dysfunction of cartilage endplate (CEP) during aging has drawn large attention due to its essential functions in contributing nutrient exchange and maintaining IVD homeostasis. Furthermore, the inflammation and disturbed homeostasis of CEP not only accelerate the degradation of nucleus pulposus extracellular matrix, but also exacerbate IVDD by causing nucleus pulposus cell death through other pathological factors. Here in this review, we summarized the possible pathological factors and the underlying mechanisms of the CEP inflammation-induced IVDD, including exosomes degeneration, CEP calcification, ferroptosis, mechanical changes, and cell senescence. Besides, changes of miRNAs, pain-related neural reflex arc and pathways associated with CEP inflammation-induced IVDD are also reviewed. In addition, new strategies specifically designed for CEP inflammation-induced IVDD are also discussed in the last section. We hope this paper can not only offer some new insights for advancing novel strategies for treating IVDD, but also serve as a valuable reference for researchers in this field.

Lycorine Suppresses Endplate-Chondrocyte Degeneration and Prevents Intervertebral Disc Degeneration by Inhibiting NF-κB Signalling Pathway

Abstract:Background/Aims: Cartilaginous endplate (CEP) degeneration is an important cause for intervertebral disc (IVD) degeneration that leads to low-back pain. The identification of compounds that may prevent CEP degeneration is of interest for the prevention of IVD degeneration. Methods: Catabolic protease expression in the CEP of disc degeneration patients was first assessed. The toxicity, function and underlying mechanism of lycorine (LY) on CEP-derived chondrocytes degeneration were assessed in vitro by flow cytometry analysis and western blotting. The concentration and function of LY in rat-tail disc-degeneration models were also assessed by HPLC (High Performance Liquid Chromatography) quantification and histological analysis. Results: In CEP cells, Interleukin (IL)-1β upregulated the expression of matrix metalloproteinase (MMP)-3, MMP-13, a disintegrin and metalloproteinase with thrombospondin motifs (ADAMTS)-4 and ADAMTS-5 that is critical for the degradation of cartilage extracellular matrix. Interestingly, LY suppressed the expression of these enzymes via the inhibition of nuclear factor-κB (NFκB) signalling and thus prevented IL-1β-induced endplate cell degeneration in vitro. More importantly, LY also reduced the expression of MMP-3, MMP-13, ADAMTS-4 and ADAMTS-5 in CEP and exerted a protective effect on both CEP and nucleus pulposus (NP) degeneration. In addition to its inhibitory effect on matrix-degrading protease expression, LY treatment also reduced positive regulators of proinflammatory cytokines, such as MIF, which can be secreted by CEP cells and subsequently target NP cells. Conclusion: LY could serve as a potential drug for treating IVD disease.
Biomechanical properties of lumbar endplates and their correlation with MRI findings of lumbar degeneration

Abstract:How stiffness and strength of the human lumbar endplate vary with location, spinal level, and its correlation with MRI findings of lumbar degeneration, has not been reported in detail. 27 lumbar spines (16 male, 11 female, 31–49yrs) were harvested from cadavers without history of lumbar lesion or trauma. Disc and endplate degeneration was evaluated from MRI. Micro-CT was used to evaluate endplate microstructure. Indentation tests were performed to quantify stiffness and strength at 23 sites on each endplate from load-displacement graphs. Results showed that stiffness and strength increased from the centre of the endplate towards its periphery. There was no general age-related reduction in endplate stiffness or strength, although strength decreased slightly with age opposite the inner annulus. Disc degeneration was associated with a 39–46% decrease in stiffness, and a 21–30% decrease in strength, with effects being greatest near the endplate periphery. The presence of Modic changes had a similar effect. Strength and stiffness consistently increased at lower spinal levels, and were consistently greater in the inferior endplate (relative to the vertebra). Gender had little influence, although stiffness in the peripheral endplate was greater in males. BV/TV, SMI, Tb.Th and BMD were positively correlated with strength. We conclude that endplate properties reflect compressive stresses within adjacent intervertebral discs. Weaker and softer endplates may indicate reduced mechanical loading in decompressed discs that are stress-shielded by the neural arch. Preoperative MRI evaluation of endplate integrity could reduce the risk of implant subsidence following inter-body fusion.
Finite element model reveals the involvement of cartilage endplate in quasi-static biomechanics of intervertebral disc degeneration

Abstract:Background and objective: The intrinsic link between the compositional and structural attributes and the biomechanical functionality is evident in intervertebral discs. However, it remains unclear from a biomechanical perspective whether cartilage endplate (CEP) degeneration exacerbates intervertebral disc degeneration. Methods: This study developed and quantitatively validated four biphasic swelling-based finite element models. We then applied four quasi-static tests and simulated daily loading scenarios to examine the effects of CEP degradation. Results: Under free-swelling conditions, short-term responses were prevalent, with CEP performance changes not significantly impacting response proportionality. The creep test results showed the more than 50 % of the strain was attributed to long-term responses. Stress-relaxation testing indicated that all responses increased with disc degeneration, yet CEP degeneration's impact was minimal. Daily load analyses revealed that disc degeneration significantly reduces nucleus pulposus pressure and disc height, whereas CEP degeneration marginally increases nucleus pressure and slightly decreases disc height. Conclusions: Glycosaminoglycan content and CEP permeability are critical to the fluid-dependent viscoelastic response of intervertebral discs. Our findings suggest that CEP contributes to disc degeneration under daily loading conditions.
Effect of Different Injury Morphology of the Endplate on Intervertebral Disc Degeneration: Retrospective Cohort Study

Abstract:We propose a simplified classification for the morphology characteristics of endplate injury based on 3D CT: mild, moderate, and severe. Our results indicate that moderate and severe endplate injury cause severe disc degeneration and long‐term loss of intervertebral height and Cobb angle. This may allow surgeons to predict patient prognosis and guide treatment for thoracolumbar fractures. Objectives To describe a simplified classification scheme for endplate injury morphology based on 3D CT and to examine possible associations between endplate injury morphology and vertebral space and other variables such as type of fracture and disc degeneration in a group of patients with thoracolumbar fractures. Methods This study was a retrospective cohort study. We collected patients with thoracolumbar fractures admitted from January 2015 to August 2020 and divided them into three groups based on the morphology of endplate injury (45 cases of mild endplate injury, 54 cases of moderate endplate injury, and 42 cases of severe endplate injury, SEI). Data of vertebral body and intervertebral space height and angle, the Pfirrmann grade, endplate healing morphology were collected during preoperative, postoperative, and long‐term follow‐up of patients in each group. One‐way analysis of variance (ANOVA), chi‐squared test, and repeated measurement ANOVA were used to compare and analyze the influence of endplate injury morphology on patient prognosis. Results Most moderate injuries to the endplate (fissure‐type injury) and severe injuries (irregular depression‐type injury, Schmorl's node‐type injury) resulted in significant disc degeneration in the long‐term transition. This study also showed significant differences in the height of the anterior margin of the injured spine and the intervertebral space height index during this process. Conclusions The current study suggests that although the region of injury in endplate fissure‐type injury is small preoperatively, it may be a major factor in leading to severe disc degeneration, loss of intervertebral height, and Cobb angle in the long term. The results of our study therefore may allow surgeons to predict the prognosis of patients with thoracolumbar fractures and guide their treatment.

Association Between Vertebral Endplate Defects and Patient-Reported Symptoms: an Immunohistochemical Study Investigating the COX-2/PGE-2/EP-4 Axis

Abstract:Background ContextVertebral endplate defects are often implicated in degenerative disc disorders, yet their connection to patient-reported symptoms remains unclear. COX-2 and PGE-2 are known for their roles in inflammation and pain, with EP-4 receptor involvement in pain signaling. Examining their expression in vertebral endplate tissues may provide insights into pathomechanism of low back pain.PurposeTo investigate the association between endplate defects and patient-reported symptoms and to further clarify the role of the COX-2/PGE-2/EP-4 axis in the pathogenesis of chronic low back pain.Study Design/SettingRetrospective studyPatient SampleA total of 71 patients who had undergone single-level L4/5 or L5/S1 modified laminectomy decompression preserving proximal upper laminae and transforaminal lumbar interbody fusion surgery were included in this study, including 18 patients diagnosed with lumbar disc herniation, 19 with lumbar disc herniation accompanied by degenerative lumbar spinal stenosis, and 34 with degenerative spondylolisthesis.Outcome MeasuresDemographic data, Pfirrmann grade, Modic changes, endplate defect score, visual analog scale (VAS) for back and leg pain, and Oswestry Disability Index (ODI) before surgery, 3-month and 6-month follow-up, and the percentage of immune-positive cells (COX-2, PGE-2, and EP-4) in endplate tissue sections.MethodsPatients were divided into Defect and Non-defect groups according to endplate morphology on lumbar MR. All intraoperative endplate specimens were immediately fixed in 10% formaldehyde, and then embedded in paraffin 3 days later for tissue sections. The outcome measures were compared between the Defect group and Non-defect group. Data were analyzed using independent t-tests and χ² tests. Pearson's rank correlation test was used to assess correlations between patient-reported symptoms and the percentage of immune-positive cells in the groups. Multivariable logistic regression models using the forward stepwise likelihood ratio method were used to identify the factors that were independently associated with endplate defects.ResultsThe age of Defect group was significantly higher than that of Non-defect group (52.5±7.7 vs. 57.2±9.1. P=0.024). There were no significant differences in gender, diagnosis, BMI, comorbidities, or surgical level between the two groups. Modic changes (Type Ⅱ/Type Ⅲ) were more common in patients of Defect group than Non-defect group (38.5% vs. 11.1%, P<0.001), and so was disc degeneration (Pfirrmann grade Ⅳ/Ⅴ) (69.2% vs. 33.3%, P<0.001). Defect group had significantly higher VAS-Back (6.5±2.0 vs. 4.9±1.6, P<0.001) and ODI scores (62.9±10.7 vs. 45.2±14.8, P<0.001) than Non-defect group, while there was no significant differences between the two groups during the 3 and 6-month follow-up after surgery. Histologically, Defect group was characterized by upregulation of COX-2, PGE-2, and EP-4 in endplate tissue sections. Both in Defect and Non-defect groups, VAS-Back showed moderate positive correlations with the expressions of COX-2 (r=0.643; r=0.558, p both<0.001), PGE-2 (r=0.611; r=0.640, p both<0.001), and EP-4 (r=0.643; r=0.563, p both<0.001). Multivariate regression analyses reveled that percentage of COX-2-positive cells was associated with endplate defects (OR=1.509, 95%CI [1.048∼2.171], P=0.027), as well as percentage of PGE-2-positive (OR=1.291, 95%CI [1.106∼1.508], P=0.001) and EP-4-positive cells (OR=1.284, 95%CI [1.048∼2.171], P=0.003).ConclusionsPatients with endplate defects had worse quality of life, more severe disc degeneration and Modic changes, and up-regulated COX-2/PGE-2/EP-4 axis expression in cartilage endplates in patients with defected endplates. Inflammatory factors may significantly contribute to the onset and progression of chronic low back pain in patients with endplate defects, consequently impacting patient-reported symptoms.
