Abstract:Objective:To explore the clinical efficacies of GengNianAnKang decoction combined with low-dose hormone and psychological intervention in treatment of perimenopausal syndrome.Methods:A total of 220 cases were diagnosed as perimenopausal syndrome in outpatient of the hospital from January 2006 to May 2010,then they were randomly divided into two groups: 109 cases in control group were treated with low-dose hormone replacement therapy(HRT),while 111 cases in observation group were treated with GengNianAnKang decoction combined with low-dose HRT.The cases in the two groups were treated with psychological intervention for three courses of treatment,then evaluation of therapeutic efficiency was performed.Results:After three courses of treatment,the curative rates in observation group and control group were 51.82% and 33.33%,respectively,there was significant difference between the two groups(P0.05).The total effective rates in observation group and control group were 98.18% and 87.96%,respectively,there was significant difference between the two groups(P0.05).After treatment,there was significant difference in serum estradiol level and the change of thickness of endometrium in intergroup contrast(P0.01),but there was no significant difference in intragroup contrast(P0.05).Conclusion:GengNianAnKang decoction combined with low-dose HRT and psychological intervention are effective in treatment of perimenopausal syndrome,the safety is high,and the method is worthy to be popularized in clinic.

Acupuncture for Perimenopausal Symptoms in Women Who Underwent Oophorectomy - A Comparative Study

Abstract:Objective: To explore the effects of acupuncture on perimenopausal symptoms in women who underwent oophorectomy. Methods: 67 women who had undergone oophorectomy were divided into an acupuncture group (n = 36) and a comparison group ( n = 31) according to their wishes. The first group was treated by acupuncture and the latter group by Livial. Clinical symptoms were assessed by the modified Kupperman index. The levels of venous blood serum beta-endorphin (beta-EP), follicle stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E-2) and the maturation index (MI) of vaginal epithelial cells were assessed. Results: There were no significant group differences for the MI of vaginal exfoliative cells nor for the levels of FSH, LH and E-2 after treatment (p > 0.05), but Kupperman scoring and the levels of beta-EP differed significantly between the acupuncture and the Livial group (p < 0.05). No side-effects were reported in either group. Conclusion: Acupuncture results in a significant improvement in perimenopausal symptoms in women who have had an oophorectomy. Acupuncture performs as well or better than Livial. Yet, this result may have been influenced by a potential bias and the small sample size.

Abstract:Background An increasing number of women suffer from perimenopausal syndrome (PMS) and the global burden of this disease has been steadily rising. Acupoint application therapy and Chinese herbal medicine (CHM) are widely used as effective methods for treating PMS, but the efficacy was inconsistent and the evidence should be summarized by quantitively analysis. Objective The purpose of this systematic review and meta-analysis was to evaluate the clinical efficacy and safety of the acupoint application combined with the CHM for the treatment of PMS. Methods We searched eight databases from their inception to August 2022 to identify relevant studies. Only randomized controlled trials (RCTs) focusing on acupoint application combined with CHM for the treatment of PMS were included in this study. To assess the clinical efficacy and safety, meta-analysis was used to quantitively synthesize the effect estimates. Subgroup analysis, publication bias assessment and sensitivity analysis were also performed. We further assessed whether the included studies had reported on the purity and potency of the CHM used in their trials. Results A total of 8 RCTs with 560 participants were included in the systematic review and meta-analysis, of which none of them included a description of an independent testing of purity or potency of the CHM product used. There were significant differences between the acupoint application combined with CHM and CHM alone in terms of Kupperman Menopausal Index (KMI) score (MD = –2.91, 95%CI: –3.91 to –1.91), total effective rate (RR = 1.22, 95% CI: 1.11 to 1.34), Pittsburgh Sleep Quality Interview (PSQI) score (MD = –2.86, 95% CI: –3.61 to –2.10) and reduction in the serum level of luteinizing hormone (LH) (MD = –2.52, 95% CI: –4.70 to –0.34), whereas there were no differences between the two groups regarding lowering serum level of follicle-stimulating hormone (FSH) (MD = –1.66, 95% CI: –3.98 to 0.67) and elevating serum level of oestradiol (E 2 ) (MD = 2.41, 95% CI: –0.70 to 5.52). For the comparation between the acupoint application combined with CHM and western medicine (WM), the KMI score (MD = –6.80, 95%CI: –7.95 to –5.65) was substantially different, while the PSQI score (MD = –0.60, 95% CI: –1.88 to 0.68) was not substantially different. The total effective rate in the combined group (91.7%) was higher than the western medicine group (83.49%). Conclusion Acupoint application combined with CHM may enhance the efficacy and safety of patients with PMS. However, due to the lack of description of an independent testing of purity or potency of the CHM product used in the trials, as well as blinding of participants and investigators, these results should be interpreted with caution.

Abstract:This multicenter, randomized, controlled clinical study was designed to address the effectiveness of combined traditional-Chinese-medicine- (TCM-) based psychotherapy and Chinese herbal medicine (CHM) in the treatment of menopausal syndrome. Altogether 424 eligible women diagnosed as menopausal syndrome and categorized as Kidney-Yin/Kidney-Yang deficiency pattern in TCM were randomly assigned into 4 groups and accepted TCM-based psychotherapy (PSY), CHM, PSY + CHM, or placebo therapies, respectively, for 12 weeks, and another 12 weeks were taken as the followup. Kupperman Index (KI) and the Menopause-Specific Quality of Life (MENQOL) with its four subscales (vasomotor, physical, psychosocial, and sexual) were employed for efficacy assessment. Results showed that 400 participants completed 12-week treatment, of which 380 finished the record of KI and MENQOF at week 24. The average adjusted number of KI score decreased between baseline and 12 weeks in all groups. Statistically significant differences were detected in the average adjusted change between the PSY + CHM group and placebo at overall time points ( P < 0.05 ). No severe adverse events occurred in each group and no significant differences were indicated between any of the three groups and placebo in adverse event proportion. We concluded that TCM psychotherapy combined with CHM has a favorable outcome in treating menopausal syndrome.

Acupuncture and Auricular Acupressure in Relieving Menopausal Hot Flashes of Bilaterally Ovariectomized Chinese Women: A Randomized Controlled Trial

Abstract:The objective of this study is to explore the effects of acupuncture and auricular acupressure in relieving menopausal hot flashes of bilaterally ovariectomized Chinese women. Between May 2006 and March 2008, 46 bilaterally ovariectomized Chinese women were randomized into an acupuncture and auricular acupressure group ( n = 21) and a hormone replacement therapy (HRT) group (Tibolone, n = 25). Each patient was given a standard daily log and was required to record the frequency and severity of hot flashes and side effects of the treatment felt daily, from 1 week before the treatment started to the fourth week after the treatment ended. The serum levels of follicle stimulating hormone (FSH), LH and E 2 were detected before and after the treatment. After the treatment and the follow-up, both the severity and frequency of hot flashes in the two groups were relieved significantly when compared with pre-treatment ( P < .05). There was no significant difference in the severity of hot flashes between them after treatment ( P > .05), while after the follow-up, the severity of hot flashes in the HRT group was alleviated more. After the treatment and the follow-up, the frequency of menopausal hot flashes in the HRT group was reduced more ( P < .05). After treatment, the levels of FSH decreased significantly and the levels of E 2 increased significantly in both groups ( P < .05), and they changed more in the HRT group ( P < .05). Acupuncture and auricular acupressure can be used as alternative treatments to relieve menopausal hot flashes for those bilaterally ovariectomized women who are unable or unwilling to receive HRT.

Abstract:Ethylene is one of the important signature gases in spontaneous combustion forecasting of coal goaf area. It is very important to detect the concentration of ethylene accurately. However, the absorption spectrum of the other interfering gases is always in the ethylene spectrum during the ethylene detecting, which has a great impact on the ethylene's accuracy. In this paper, a spectral correction method is presented to extract pure ethylene spectra. The experiments show that the detection accuracy is improved by nearly 10 times by using this method, so realized the high accuracy detection of ethylene.

[Acupoint catgut embedding for perimenopausal syndrome: a clinical controlled trial]

Abstract:Objective: To compare the clinical efficacy differences between acupoint catgut embedding and Kuntai capsule for perimenopausal syndrome, so as to provide an effective treatment method for perimenopausal syndrome. Methods: Thirty-three cases in the embedding group were treated with acupoint catgut embedding at back-shu points and front-mu points of liver, spleen and kidney combined with syndrome differentiation and disease differentiation, ten days per times; the Kuntai group was treated with oral administration of Kuntai capsule, 4 capsules each time, three times per day. The Kupperman index (KI) was observed in the two groups before treatment after 10 days, 30 days and 60 days of treatment, respectively; the efficacy was evaluated according to the ratio of KI. Results: After the treatment, as treatment proceeded, the score of KI and ratio of KI were gradually reduced in two groups; the score of KI and ratio of KI in the embedding group after 10 days of treatment was lower than those in the Kuntai group (both P<0.05); after 10 days of treatment, the total effective rate was 36.4% (12/33) in the embedding group, which was superior to 3.0% (1/33) in the Kuntai group (P<0.05); however, after 30 days and 60 days of treatment, the differences of each index between two groups were not statistically significant (all P>0.05). Conclusion: Both the acupoint catgut embedding and Kuntai capsule could reduce the score of KI and improve clinical symptoms, and the acupoint catgut embedding has certain advantage on the early stage of treatment.

Abstract:ObjectiveTo study the clinical effect and the endocrine mechanism of acupuncture therapy on menopause syndrome.MethodThere were 47 cases with menopause syndrome, which were divided into two groups, 32 cases in the acupuncture group, treated by acupuncture with main acupoins of Shenshu (BL 23), Zusanli (ST 36) and Sanyinjiao (SP 6), and 15 cases in the control group, treated by orally taking the Oryzanol. The Kupperman index of the two groups was observed before and after treatment for evaluating the therapeutic effect. The changes of blood-serum E2, T, FSH, LH of the 25 cases in the acupuncture group were tested by radioimmunoassay before and after the treatment.ResultsThe acupuncture therapy can relieve the clinical symptoms of menopause syndrome, and its therapeutic effect was better than Oryzanol. After being treated by acupuncture therapy, the content of blood-serum E2 obviously increased, while the contents of LH and T declined, and the FSH also had a tendency to lower, but it was not statistically significant.ConclusionThe therapeutic effect of acupuncture therapy for menopause is confirmed. The acupuncture therapy can improve the function of hypophysial-gonadal axis of the patient with menopause syndrome, which may be the main mechanism of the acupuncture therapy for treating menopause syndrome.

Abstract:The aim of this study was to explore the effect of a defined formula of Chinese medicinal herbs, GengNianAn (GNA, also called menopausal symptom-relieving formula) formula in relieving menopausal symptoms in ovariectomized women.A double-blind randomized placebo-controlled trial.Between May 2003 and June 2006, 69 ovariectomized Chinese women were recruited to complete 12 weeks of intervention with either a defined formula of GNA (n = 36) or placebo (n = 33) taken twice daily as a beverage. Clinical symptoms were assessed by the modified Kupperman scale. The levels of venous blood serum follicle stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E(2)), and the maturation index (MI) of vaginal epithelial cells were respectively measured.There existed significant differences between the two groups in the total Kupperman scoring, MI of vaginal exfoliative cells, and the levels of FSH, LH, and E(2) after treatment (P < .05).Chinese herbs may be a useful alternative treatment for ovariectomized women suffering from menopausal symptoms, who are unable or do not want to receive hormone replacement therapy (HRT).
Effect of Acupressure on Menopausal Symptoms and Quality of Life in Postmenopausal Women: A Randomized Controlled Trial

Abstract:This study was conducted as a randomized controlled experimental trial with the aim of determining the effect of acupressure on menopausal symptoms and quality of life in postmenopausal women, as well as the correlation between the two. This randomized controlled experimental study was conducted between May and November 2021 with women coming to a Menopause School in Turkey. A Descriptive Characteristics Form, the Menopause Rating Scale, and the World Health Organization Quality of Life Scale Short Form were used to collect data. The study was performed with 61 patients in an acupressure group (n = 30) and a control group (n = 31). Acupressure was performed on the acupressure group in 30-minute sessions 3 times a week for 4 weeks, a total of 12 sessions. No intervention was performed on the control group. When the groups were compared, it was seen that the menopause symptom scores of the acupressure group were lower than those of the control group, and that their quality of life scores in the bodily and psychological areas were higher (P < .05). When within group comparisons were considered, it was seen that menopause symptoms declined over time in the acupressure group, and the scores of all dimensions of quality of life increased (P < .05). It was found that acupressure applied to women during menopause may reduce menopause symptoms and may improve their quality of life.
[Comprehensive traditional Chinese medicine intervention for perimenopausal syndrome in women: a community study]

Abstract:Background: As perimenopausal syndrome is a particularly disturbing condition to the patient, it is practical and necessary to establish a program of comprehensive traditional Chinese medicine therapy for women with perimenopausal syndrome. Objective: To observe the therapeutic effects of a comprehensive traditional Chinese medicine therapy for women with perimenopausal syndrome in the community. Design, setting, participants and interventions: Women with perimenopausal syndrome who met the inclusion criteria in 3 communities of Shanghai were selected for this study. Comprehensive traditional Chinese medicine therapy, including administration of Chinese herbs, auricular point therapy, psychological counseling and the practice of Tai Chi, was applied for these women. Main outcome measures: The modified Kupperman index was measured once every 4 weeks for 12 weeks; the indexes of menopause-specific quality of life (MENQOL) questionnaire were measured before and after the treatment. Results: There was no significant difference in 4 groups of negative conversion ratio of the modified Kupperman index at the 4th, 8th and 12th week, respectively. The indexes of the MENQOL questionnaire showed significant differences in 4 groups before and after the treatment. The overall efficacy rate of the comprehensive traditional Chinese medicine therapy in women with perimenopausal syndrome achieved 97.84% at the 8th and the 12th week. Conclusion: The comprehensive traditional Chinese medicine therapy can achieve significant improvements in women with perimenopausal syndrome who exhibit both physical and psychological symptoms. With such significant clinical effects, the comprehensive traditional Chinese medicine therapy should be promoted in the community.
Effect of acupuncture on insomnia in menopausal women: a study protocol for a randomized controlled trial

Abstract:Background: The National Institutes of Health estimates the prevalence of insomnia in menopausal women at 40-50%. Some studies have shown that acupuncture might be effective in treating primary insomnia and insomnia related to depression and stroke. Although there are some programs supporting insomnia during the menopausal transition, there are few randomized controlled trials (RCT) to provide evidence regarding their effectiveness. We design a RCT of suitable sample size to verify the effectiveness of acupuncture in patients with insomnia during the menopausal transition and to form an optimized acupuncture treatment protocol. Method/design: In this randomized, single-site, single-blind, placebo-controlled trial, 84 eligible patients will be recruited and randomly assigned to either the acupuncture group (n = 42) or the sham control group (n = 42) in a 1:1 ratio. Participants will receive a total of 18 treatment sessions for eight consecutive weeks. Treatments will be given three times per week in the first four weeks, twice a week for the next two weeks, and finally once weekly for the final two weeks. Treatment will utilize eight main acupoints (GV20, GV24, GV29, RN6, RN4, SP6, HT7, EX-HN22) and extra two acupoints based on syndrome differentiation. The primary outcome will be assessed using the Pittsburgh Sleep Quality Index (PSQI). The secondary outcomes will be measured by sleep parameters recorded in the Actigraphy (SE, TST, SA), Insomnia Severity Index (ISI), Self-Rating Anxiety Scale (SAS), Self-Rating Depression Scale (SDS), and Menopause Quality of Life (Men-QOL). The primary outcomes will be assessed at baseline, week 4, week 8, and the first and third month after the end of treatment. Discussion: If the results confirm that acupuncture is effective and safe for the treatment on insomnia in menopausal women, this positive outcome could provide evidence for clinical application. Trial registration: Chinese Clinical Trial Registry, ChiCTR1800018645 . Registered on 10 January 2018.
Suppressing effects of caffeine on postextrasystolic potentiation in papillary muscles of guinea pigs.

Abstract:It is well known that the strength of postextrasystolic potentiation (PESP) is dependent on the prematurity of the ectopic beat, though the fundamental mechanism of the potentiation is still obscure. In this study, the effect of a resting interval on the strength of PESP was investigated in isolated papillary muscles of guinea pigs in the presence or absence of caffeine, which inhibits the functions of sarcoplasmic reticulum (SR). PESP of a fixed coupling interval increased and then decreased as the resting interval was prolonged. The maximum of PESP was obtained at a resting interval of 3 to 4 sec. The dependency of PESP on a coupling interval was decreased considerably by 5 x 10(-4) M caffeine and removed completely by 10(-2) M caffeine. Although 5 x 10(-4) M caffeine decreased the degree of contraction of postextrasystole, the maximum contraction of postextrasystole was still obtained at a resting interval of 3 to 4 sec. After the application of 10(-2) M caffeine, the postextrasystolic contraction gradually declined as the resting interval was prolonged. We conclude that SR Ca release contributes largely to a mechanism of PESP and increases in contribution as the coupling interval of an extrastimulation shortens, and that the optimal resting interval is determined by a balance between the activity of SR function and the activity of the sarcolemmal Ca extrusion mechanism.
Efficiency and safety of acupuncture for women with premature ovarian insufficiency: study protocol for a randomized controlled trial.

Abstract:Acupuncture has been widely used as an alternative and complementary therapy for premature ovarian insufficiency (POI) in China. However, research to date has not shown that acupuncture is effective for POI compared with hormone replacement therapy (HRT). We will conduct a randomized, controlled, and outcome assessor-blind trial to evaluate the efficacy and safety of acupuncture on POI. Seventy-six patients with POI will be randomly assigned to two groups. The treatment group will receive twenty-eight one-hour sessions of acupuncture treatments, and the control group will receive 12-week HRT. The whole study will consist of a 12-week treatment plan and a 12-week follow-up session. The primary outcome is measured by changes in serum anti-Müllerian hormone and follicle-stimulating hormone (FSH) levels at weeks 12 and 24. Secondary outcome measures include estradiol, luteinizing hormone (LH), LH/FSH ratio, Kupperman index, and menstrual condition. This trial is expected to clarify whether or not acupuncture is effective and safe for POI compared with HRT.

Quantitative Study on the Efficacy of Acupuncture in the Treatment of Menopausal Hot Flashes and Its Comparison with Non-Hormonal Drugs

Abstract:Background The efficacy of acupuncture in the treatment of menopausal hot flashes has always been controversial, which limited the use of acupuncture. This study aimed to compare the efficacy of acupuncture to that of sham acupuncture (SA), placebo pills, and non-hormonal drugs to provide the necessary quantitative information for establishing medication guidelines for menopausal hot flashes. Methods A comprehensive literature search was performed using public databases. Randomized clinical studies on acupuncture therapy for the treatment of hot flashes in menopausal women were identified. A time-course model was established to describe the efficacy characteristics of acupuncture and SA, which were compared with the efficacy of non-hormonal drugs and placebo pills reported in the literature. Results A total of 17 studies involving 1123 subjects were included. It was found that the baseline number of hot flashes was an important factor affecting the efficacy of acupuncture and SA. After correcting the baseline to eight hot flashes per day, the frequency of hot flashes decreased from baseline for traditional acupuncture (TA), electro-acupuncture (EA), TA&EA (merger analysis of TA and EA), and SA were 3.1 (95% CI: 2.8, 3.4), 3.6 (95% CI: 3.2, 4.0), 3.2 (95% CI: 2.9, 3.5), and 2.6 (95% CI: 2.2, 3.0) times/day at week 8, respectively. Compared with findings reported in the literature, we found the efficacy of EA was comparable to that of selective serotonin reuptake inhibitors/serotonin–norepinephrine reuptake inhibitors (SSRIs/SNRIs) and neuroleptic agents such as gabapentin and escitalopram. Furthermore, the efficacy of TA&EA was significantly higher than that of placebo pills (2.3, 95% CI: 1.8, 2.9). Conclusions The efficacy of TA&EA was higher than that of SA and significantly higher than that of placebo pills. The efficacy of EA was higher than that of TA, significantly higher than that of SA, and comparable to that of SSRIs/SNRIs and neuroleptic agents.
Acupuncture for comorbid depression and insomnia in perimenopause: A feasibility patient-assessor-blinded, randomized, and sham-controlled clinical trial

Abstract:Background and objective: Whilst acupuncture is widely used for treating psychosomatic diseases, there is little high-quality evidence supporting its application in comorbid perimenopausal depression (PMD) and insomnia (PMI) which are common complaints during climacteric. This feasibility, patient-assessor-blinded, randomized, sham-controlled clinical trial addresses this gap by investigating the efficacy and safety of acupuncture on depressed mood and poor sleep in women with comorbid PMD and PMI. Methods: Seventy eligible participants were randomly assigned to either real-acupuncture (RA) or sham-acupuncture (SA) groups. Either RA or SA treatment were delivered in 17 sessions over 8 weeks. The primary outcomes for mood and sleep were changes on 17-items Hamilton Depression Rating Scale (HAM-D17) and Pittsburgh Sleep Quality Index (PSQI) scores, from baseline to 16-week follow-up. Secondary outcome measures involved anxiety symptoms, perimenopausal symptoms, quality of life, participants' experience of and satisfaction with the acupuncture treatment. Blood samples were taken to measure reproductive hormone levels. Intention-To-Treat and Per-Protocol analyses were conducted with linear mixed-effects models. The James' and Bang's blinding indices were used to assess the adequacy of blinding. Results: Sixty-five participants completed all treatment sessions, and 54 and 41 participants completed the eight- and 16-week follow-ups, respectively. At post-treatment and 8-week follow-up, the RA group showed a significantly greater reduction in PSQI scores than the SA group did; although the reduction of HAM-D17 scores in RA group was significant, the change was not statistically different from that of SA. There were no significant mean differences between baseline and 16-week follow-up in either HAM-D17 or PSQI in either group. There were no significant between-group differences in serum reproductive hormone levels. All treatments were tolerable and no serious adverse events were reported, and the blinding was successful. Conclusion: Acupuncture is safe and can contribute to clinically relevant improvements in comorbid PMD and PMI, with satisfactory short-and medium-term effects. Whether the anti-depressive benefit of acupuncture is specific or non-specific remains to be determined. No evidence was found for any longer-term benefit of acupuncture compared to sham at 16 weeks. Further research is required to elucidate mechanisms underlying the short to medium term effects of acupuncture.

Methods: All randomized controlled trials related to acupoint catgut embedding therapy on PD will be searched in the following electronic databases: Cochrane Central Registry of controlled trials, PubMed, Wed of Science, EMBASE, Science Net, China Biomedical Literature Database, China Science Journal Database, China National Knowledge Infrastructure and Wan-Fang Database, from inception to May, 2021 were searched without language restrictions. The primary outcomes contain visual analog score, The Cox Menstrual Symptom Scale, while the secondary outcomes consist of adverse events and the recurrence rate. Two reviewers will independently perform data selection, data synthesis, and quality assessment. Data meeting the inclusion criteria will be extracted and analyzed by Revman v.5.3 software. Two reviewers will evaluate the study using the Cochrane collaborative bias risk tool. We will use the scoring method to assess the overall quality of the evidence supporting the main results. We will also use Spass software (version 19.0) for complex network analysis to explore the potential core prescription of acupoint application of traditional Chinese medicine in the treatment of PD. Results: This study will analyze the clinical effective rate, functional outcomes, quality of life, improvement of clinical symptoms of PD, and effective prescriptions of acupoint application for patients with PD. Conclusion: Our findings will provide evidence for the effectiveness and potential treatment prescriptions of acupoint application for patients with PD.PROSPERO registration number: CRD 42021244357.

Effect of Click-Press Therapy on the Modified Kupperman Score of Perimenopausal Syndrome

Abstract:Objective To investigate the effect and mechanism of click-press therapy for the patients with perimenopausal syndrome.Methods Sixty cases of peri-menopausal syndrome were equally divided into click-press group and Tibolone group,both were treated for 3 months.The modified Kupperman score,serum estradiol(E2),follicle stimulating hormone(FSH),and luteinizing hormone(LH)were evaluated before and after treatment respectively.Results After treatment,the modified Kupperman score in both groups was improved significantly(P0.01),but there was no significant difference between(P0.05);serum E2 of both groups was significantly increased(P0.01),and FSH and LH were significantly decreased(P0.01),without significant difference between(P0.05).Conclusion Both click-press therapy and Tibolone can relieve the symptoms of perimenopausal syndrome,increase E2 and decrease FSH and LH.The click-press therapy has the same effect as Tibolone.
