Abstract:Abstract Cigarette smoking is posited as a potential factor in disrupting the balance of the human gut microbiota. However, existing studies with limited sample size have yielded inconclusive results. Here, we assessed the association between cigarette smoking and gut microbial profile among Chinese males from four independent studies (N total = 3,308). Both 16S rRNA and shotgun metagenomic sequencing methods were employed, covering 206 genera and 237 species. Microbial diversity was compared among non-smokers, current smokers, and former smokers. Actinomyces[g], Atopobium[g], Haemophilus[g], Turicibacter[g], and Lachnospira[g] were found to be associated with smoking status (current smokers v.s. non-smokers). Metagenomic data provided a higher resolution at the species level, particularly for the Actinomyces[g] branch. Additionally, serum Trans-3-Hydroxycotinine was found to have a potential role in connecting smoking and Actinomyces[g]. Furthermore, we revealed putative mediation roles of gut microbiome in the associations between smoking and common diseases including cholecystitis and type 2 diabetes. In conclusion, we characterized the gut microbiota profile in male smokers and further revealed their potential involvement in mediating the impact of smoking on health outcomes. These findings advance our understanding of the intricate associations between cigarette smoking and the gut microbiome.
Association of Cigarette Smoking with Oral Bacterial Microbiota and Cardiometabolic Health in Chinese Adults

Abstract:The interplay among cigarette smoking status, oral microbiota, and cardiometabolic health is poorly understood. We aimed to examine the association of cigarette smoking status with oral microbiota and to assess the association of the identified microbial features with cardiometabolic risk factors in a Chinese population. This study included 587 participants within the Central China Cohort, including 111 smokers and 476 non-smokers, and their oral microbiota was profiled by 16S rRNA sequencing. Both oral microbial alpha- and beta-diversity were distinct between smokers and non-smokers ( p < 0.05). With adjustment for sociodemographics, alcohol and tea drinking, tooth brushing frequency, and body mass index, the relative abundance of nine genera and 26 pathways, including the genus Megasphaera and two pathways involved in inositol degradation which have potentially adverse effects on cardiometabolic health, was significantly different between two groups (FDR q < 0.20). Multiple microbial features related to cigarette smoking were found to partly mediate the associations of cigarette smoking with serum triglycerides and C-reactive protein levels ( p- mediation < 0.05). In conclusion, cigarette smoking status may have impacts on the oral microbial features, which may partially mediate the associations of cigarette smoking and cardiometabolic health.

Abstract:To examine associations between plasma metabolites and the gut microbiota with dietary patterns in a general Chinese population. The study group included 214 Chinese adults (105 men and 109 women, 50.93 ± 13.6 y) from the Wellness Living Laboratory (WELL) - China. Dietary data were collected through a validated food frequency questionnaire, and a k-means cluster analysis was performed to identify dietary patterns. An ultrahigh-performance liquid chromatography tandem mass spectroscopy was used to assess plasma metabolic profiles (759 metabolites). The gut microbiota data were analyzed using a 16 s rRNA sequencing. Based on the energy contribution of each food group, 3 dietary patterns were identified as the Chinese traditional diet, the high alcohol diet, and the modern diet. Overall, 7 metabolites were found significantly higher (ANCOVA < 0.05 adjusting for age, sex, use of medication, and body mass index, FDR < 0.05) in the alcohol diet than in the traditional diet. Of these,1,3,7-trimethylurate, androstenediol, nicotinamide riboside, N2, N5-diacetylornithine had a significant impact on the gut microbiota. Participants with high alcohol intake showed a significant depletion in Clostridum_XIVa (Log2FodChange = −26.3, P < 0.0001) and 4 unknown genera (Log2FodChange > −18, P < 0.0001) and enrichment in Gemmiger (Log2FodChange = 11.1, P < 0.0001) and Clostridium_XIVb (Log2FodChange = 14.0, P < 0.0001). Besides, Gemmiger (P = 0.003) and Clostridium_XIVb (P < 0.0001) were significantly overrepresented in participants who had high androstenediol, whereas Clostridum_XIVa was underrepresented in those who had high 1,3,7-trimethylurate (P = 0.0004), androstenediol (P < 0.0001), and nicotinamide riboside (P < 0.0001). No significant difference was found in metabolites between the traditional diet and the modern diet. The pattern of alcohol diet were shown to be associated with both plasma metabolomics and the gut microbiota data. More data is needed to identify the unknown genera and to explore the pathways. Abstract:The dysbiosis of oral microbiota is linked to numerous diseases and is associated with personal lifestyles, such as alcohol drinking. However, there is inadequate data to study the effect of alcohol drinking on oral microbiota from the Chinese population. Here, we profiled the oral microbiota of 150 healthy subjects in the Chinese population by 16S rRNA gene sequencing. The results showed that drinkers had significantly higher alpha diversity than non-drinkers. A significant difference in overall microbiota composition was observed between non-drinkers and drinkers. Additionally, using DESeq analysis, we found genus Prevotella and Moryella, and species Prevotella melaninogenica and Prevotella tannerae were significantly enriched in drinkers; meanwhile, the genus Lautropia, Haemophilus and Porphyromonas, and species Haemophilus parainfluenzae were significantly depleted in drinkers. PICRUSt analysis showed that significantly different genera were mainly related to metabolism pathways. The oxygen-independent pathways, including galactose, fructose and mannose metabolism pathways, were enriched in drinkers and positively associated with genera enriched in drinkers; while the pyruvate metabolism pathway, an aerobic metabolism pathway, was decreased in drinkers and negatively associated with genera enriched in drinkers. Our results suggested that alcohol drinking may affect health by altering oral microbial composition and potentially affecting microbial functional pathways. These findings may have implications for better understanding the potential role those oral bacteria play in alcohol-related diseases.

Characteristics of the Intestinal Microorganisms in Middle-Aged and Elderly Patients: Effects of Smoking

Abstract:Introduction: Smoking affects the occurrence and development of many diseases. We attempt to study the structure of intestinal flora in the middle-aged and elderly population as well as how smoking affects the intestinal flora. Methods: We collected population information, biochemical indicators, and patient feces from 188 middle-aged and elderly male patients, and their feces were tested for the 16S rRNA gene of intestinal flora. Results: We performed a cluster analysis on the intestinal structure of the included population and found that there was a significant difference in the number of smokers between each group (p = 0.011). Subsequently, the microbiological diversity analysis of current smokers and nonsmokers was carried out. The results indicated that there was a significant difference in species composition between the two groups (p = 0.029). Through the analysis on LEfSe differential bacteria, it was found that in current smoking patients, the abundances of the genus Bifidobacterium and the genus Coprobacillus were less, while the abundances of the genera Shigella, Paraprevotella, Burkholderia, Sutterella, Megamonas, and p-75-a5 under the family level of Erysipelotrichaceae were slightly high. We analyzed the correlation between the abundances of these eight different bacteria and clinical indicators. The results revealed the following: the abundance of the genus Bifidobacterium was negatively correlated with fasting blood glucose (r = -0.198, p = 0.006) and positively correlated with uric acid (r = 0.207, p = 0.004) and total bilirubin (r = 0.175, p = 0.017); Shigella bacteria were positively correlated with fasting blood glucose (r = 0.160, p = 0.028) and uric acid (r = 0.153, p = 0.036) levels; the genus Paraprevotella and BMI (r = -0.172, p = 0.018) are negatively correlated; the abundance of the genus Burkholderia was positively correlated with γ-glutamyltransferase (r = 0.146, p = 0.045) levels; Sutterella was correlated with fasting blood glucose (r = 0.143, p = 0.05) and creatinine level (r = -0.16, p = 0.027), which was positively correlated with fasting blood glucose and negatively correlated with creatinine. Conclusions: In middle-aged and elderly patients with cardiovascular disease, smoking can reduce the abundance of Bifidobacterium, while the abundances of some negative bacteria such as Burkholderia, Sutterella, and Megamonas increase.

Abstract:Cigarette smoke is a representative source of toxic chemical exposures to humans, and the adverse consequences of cigarette smoking are mediated by its effect on both neuronal and immune–inflammatory systems. Cigarette smoking also is a major risk factor for intestinal disorders, such as Crohn’s disease and peptic ulcer. On the other hand, cigarette smoking is protective against developing ulcerative colitis. The effects of cigarette smoking on intestinal disorders include changes in intestinal irrigation and microbiome, increases in permeability of the mucosa, and impaired mucosal immune responses. However, the underlying mechanism linking cigarette smoking with intestinal microbiota dysbiosis is largely unknown. In this communication, we first review the current knowledge about the mechanistic interaction between cigarette smoke and intestinal microbiota dysbiosis, which include the likely actions of nicotine, aldehydes, polycyclic aromatic hydrocarbons, heavy metals, volatile organic compounds and toxic gases, and then reveal the potential mechanisms of the lung–gut cross talk and skin-gut cross talk in regulating the balance of intestinal microbiota and the interrelation of intestinal microbiota dysbiosis and systemic disorders.

Abstract:BACKGROUND: To explore the effects of smoking and drinking on the microbiota in the saliva and three segments of the esophagus (upper, middle, and lower) in healthy individuals.METHODS: Paired saliva and brush specimens were obtained from 76 participants who underwent upper gastrointestinal (UGI) endoscopic examination for UGI cancer screening. The esophageal microbiota was investigated by 16S rRNA gene profiling via next-generation sequencing.RESULTS: The saliva samples from non-smoking and non-drinking participants had a greater abundance of Neisseria, Prevotella, Porphyromonas, and Rothia, and lower levels of Streptococcus, Actinobacillus, and Haemophilus compared to the esophagus. There were no significant differences in the abundance of most bacterial genera in the upper, middle, and lower oesophagus. Similarly, in the saliva of patients who smoke and drink, there was a higher prevalence of Neisseria, Prevotella, Porphyromonas, Fusobacterium, and Rothia, and a lower prevalence of Streptococcus, Actinobacillus, and Haemophilus compared to the esophagus. There were no significant differences in the abundance of most genera in the upper, middle, and lower esophagus of patients with a history of drinking and smoking. There were slight differences in the microbiota between smoking and drinking individuals and non-smoking and non-drinking individuals.CONCLUSIONS: This pilot study demonstrated microbial diversity at different taxonomic levels in the oral cavity and esophagus of non-drinking and non-smoking individuals, as well as healthy people who drink and smoke . There was a slight difference in the microbiota between non-drinking and non-smoking people and individuals with a history of drinking and smoking. These results suggested that oral or esophageal cancer caused by smoking and drinking may not be mediated by mechanisms that affect surface microorganisms.

Abstract:Objectives Alcohol consumption is not uncommon among people with HIV (PWH) and may exacerbate HIV-induced intestinal damage, and further lead to dysbiosis and increased intestinal permeability. This study aimed to determine the changes in the faecal microbiota and its association with alcohol consumption in HIV-infected patients. Methods A cross-sectional survey was conducted between November 2021 and May 2022, and 93 participants were recruited. To investigate the alterations of alcohol misuse on fecal microbiology in HIV-infected individuals, we performed 16s rDNA gene sequencing on fecal samples from the low to moderate drinking (n=21) and non-drinking (n=72) groups. Results Comparison between groups using alpha and beta diversity showed that the diversity of stool microbiota in the low to moderate drinkinge group did not differ from that of the non-drinking group (all P >0.05). The Linear discriminant Analysis effect size (LEfSe) algorithm was to determine the bacterial taxa associated with alcohol consumption, and the results showed altered fecal bacterial composition in HIV-infected patients who consumed alcohol, with Coprobacillus , Pseudobutyrivibrio and Peptostreptococcaceae enriched, and Pasteurellaceae and Xanthomonadaceae were depleted. In addition, by using the Kyoto Encyclopedia of Genes and Genomes (KEGG) functional microbiome features were also found to be altered in the low to moderate drinking group, showing a reduction in metabolic pathways ( P =0.036) and cardiovascular disease pathway ( P =0.006). Conclusion Low to moderate drinking will change the composition, metabolism and cardiovascular disease pathway of the gut microbiota of HIV-infected patients.

Abstract:BackgroundDysbiosis of the oral microbiome can lead to local oral disease and potentially to cancers of the head, neck, and digestive tract. However, little is known regarding exogenous factors contributing to such microbial imbalance.ResultsWe examined the impact of alcohol consumption on the oral microbiome in a cross-sectional study of 1044 US adults. Bacterial 16S rRNA genes from oral wash samples were amplified, sequenced, and assigned to bacterial taxa. We tested the association of alcohol drinking level (non-drinker, moderate drinker, or heavy drinker) and type (liquor, beer, or wine) with overall microbial composition and individual taxon abundance. The diversity of oral microbiota and overall bacterial profiles differed between heavy drinkers and non-drinkers (α-diversity richness p = 0.0059 and β-diversity unweighted UniFrac p = 0.0036), and abundance of commensal order Lactobacillales tends to be decreased with higher alcohol consumption (fold changes = 0.89 and 0.94 for heavy and moderate drinkers, p trend = 0.005 [q = 0.064]). Additionally, certain genera were enriched in subjects with higher alcohol consumption, including Actinomyces, Leptotrichia, Cardiobacterium, and Neisseria; some of these genera contain oral pathogens, while Neisseria can synthesize the human carcinogen acetaldehyde from ethanol. Wine drinkers may differ from non-drinkers in microbial diversity and profiles (α-diversity richness p = 0.048 and β-diversity unweighted UniFrac p = 0.059) after controlling for drinking amount, while liquor and beer drinkers did not. All significant differences between drinkers and non-drinkers remained after exclusion of current smokers.ConclusionsOur results, from a large human study of alcohol consumption and the oral microbiome, indicate that alcohol consumption, and heavy drinking in particular, may influence the oral microbiome composition. These findings may have implications for better understanding the potential role that oral bacteria play in alcohol-related diseases.

Abstract:Short-term intervention studies support a link between animal-based diet and the outgrowth of microorganisms capable of triggering inflammatory bowel disease. However, whether habitual animal fat intake is associated with gut-related health remains unclear. Thus, we collected dietary information, clinical data and fecal samples from 297 healthy young subjects and characterized gut microbiota by 16S rRNA gene sequencing and microbial metabolites, including short-chain fatty acids (SCFAs) and bile acids (BAs) using a gas chromatography coupled to time-of-flight mass spectrometry (GC-TOF/MS) system and ultra performance liquid chromatography coupled with tandem mass spectrometry (UPLC-MS/MS) platform, respectively. We found that the microbial diversity of butyric acid (rho = -0.17, p = 0.004 for the Shannon index) and the concentrations of butyric acid (rho = -0.33, p < 0.001) and valeric acid (rho = -0.28, p = 0.002) were negatively associated with animal fat consumption. In line with this, the abundance of SCFAs-producing bacteria such as Blautia, Eubacterium hallii, and Megamonas were significantly lower in the high animal fat group compared with the low animal fat group (all p < 0.05). Additionally, the high animal fat group had higher concentrations of total (p = 0.06) and unconjugated (p = 0.09) BAs relative to the lower animal fat groups. The findings of our study indicate that a diet with higher animal-based fat consumption is likely to be associated with moderately unfavorable impacts on gut microbial diversity, community, and regulation of fecal SCFAs, which may influence the host cardiometabolic health in the long term among healthy Chinese adults whose diet is in a nutrition transition.
Alcohol-induced gut microbiome dysbiosis enhances the colonization of Klebsiella pneumoniae on the mouse intestinal tract

Abstract:Alcohol is one of the most commonly misused substances in our lives. However, long-term heavy drinking will increase the colonization of some opportunistic pathogens (e.g., Klebsiella pneumoniae ) in the body. Here, we revealed that binge-on-chronic alcohol consumption disrupted the balance between gut bacteria and fungi, induced the gut microbiome and metabolites dysbiosis, and promoted the colonization of K. pneumoniae in the intestine of mice. In particular, alcohol-taking disrupted intestinal bile acid metabolism and reduced the lithocholic acid concentration. However, a high concentration of lithocholic acid can protect against intestinal colonization of K. pneumoniae by inhabiting the bacterial growth and adhesion to the host cell. Hence, regulating the balance of gut microbiota and intestinal bile acid metabolism may be a potential strategy for reducing the risk of K. pneumoniae infection and spread.

Abstract:The oral microbiota has been observed to be influenced by cigarette smoking and linked to several human diseases. However, research on the effect of cigarette smoking on the oral microbiota has not been systematically conducted in the Chinese population. We profiled the oral microbiota of 316 healthy subjects in the Chinese population by 16S rRNA gene sequencing. The alpha diversity of oral microbiota was different between never smokers and smokers ( P = 0.002). Several bacterial taxa were first reported to be associated with cigarette smoking by LEfSe analysis, including Moryella ( q = 1.56E-04), Bulleidia ( q = 1.65E-06), and Moraxella ( q = 3.52E-02) at the genus level and Rothia dentocariosa ( q = 1.55E-02), Prevotella melaninogenica ( q = 8.48E-08), Prevotella pallens ( q = 4.13E-03), Bulleidia moorei ( q = 1.79E-06), Rothia aeria ( q = 3.83E-06), Actinobacillus parahaemolyticus ( q = 2.28E-04), and Haemophilus parainfluenzae ( q = 4.82E-02) at the species level. Two nitrite-producing bacteria that can increase the acidity of the oral cavity, Actinomyces and Veillonella , were also enriched in smokers with FDR-adjusted q -values of 3.62E-06 and 1.10E-06, respectively. Notably, we observed that two acid production-related pathways, amino acid-related enzymes ( q = 6.19E-05) and amino sugar and nucleotide sugar metabolism ( q = 2.63E-06), were increased in smokers by PICRUSt analysis. Finally, the co-occurrence analysis demonstrated that smoker-enriched bacteria were significantly positively associated with each other and were negatively correlated with the bacteria decreased in smokers. Our results suggested that cigarette smoking may affect oral health by creating a different environment by altering bacterial abundance, connections among oral microbiota, and the microbiota and their metabolic function.

Abstract:Introduction: The relationship between oral and gut microbiota in alcohol dependence (AD) is not well understood, particularly the effects of oral microbiota on the intestinal microbiota. The current study aimed to explore the association between oral and gut microbiota in AD to clarify whether oral microbiota could ectopically colonize into the gut. Methods: 16S rRNA sequence libraries were used to compare oral and gut microbial profiles in persons with AD and healthy controls (HC). Source Tracker and NetShift were used to identify bacteria responsible for ectopic colonization and indicate the driver function of ectopic colonization bacteria. Results: The α-diversity of oral microbiota and intestinal microbiota was significantly decreased in persons with AD (all p < 0.05). Principal coordinate analysis indicated greater similarity between oral and gut microbiota in persons with AD than that in HC, and oral-gut overlaps in microbiota were found for 9 genera in persons with AD relative to only 3 genera in HC. The contribution ratio of oral microbiota to intestinal microbiota composition in AD is 5.26% based on Source Tracker,and the AD with ectopic colonization showed the daily maximum standard drinks, red blood cell counts, hemoglobin content, and PACS scores decreasing (all p < 0.05). Discussion: Results highlight the connection between oral-gut microbiota in AD and suggest novel potential mechanistic possibilities.

Abstract:ABSTRACT Alcohol dependence (AD) is a worldwide epidemic of psychiatric disorders. Gut microbiota dysbiosis may be involved in the development of AD. Recently, the number of studies on the relationship between gut microbiota and alcohol is increasing. However, most previous studies were focused on rodent models and gut microbiota, and the data about the alteration of the gut fungi in AD patients were scarce. Convincing evidence of the gut microbiota’s role in the development of AD is lacking. Here, we investigated the characteristics of gut microbiota (bacterial) and mycobiota (fungal) dysbioses in AD patients and assessed the role of the gut microbiome on the behaviors of rats by fecal microbiota transfer, trying to illuminate the possible microbiota mechanism in AD development. We found that AD patients developed gut microbiota dysbiosis, displayed by the fact that the bacterial genus of Ruminococcaceae significantly decreased and that of Lachnospiraceae increased and the fungal genus of Saccharomyces and Kurtzmaniella obviously increased and that of Candida decreased. Alcohol consumption disturbs the gut equilibrium between bacteria and fungi. The fungal-to-bacterial species ratio was significantly decreased, and the bacterial-fungal trans-kingdom interactions were reduced. By transplanting the gut microbiota from AD patients to rats, we confirmed that the microbiota from AD patients induced the behavioral alterations associated with alcohol dependence in rats, including increased anxiety- and depression-like behaviors, reduced exploratory and recognition memory, and higher alcohol preference. Meanwhile, the microbiota from the AD patient donor upregulated the expression levels of cholecystokinin receptors in the frontal lobe, hippocampus, and cortex in rats, which might be involved in the development of alcohol dependence. Our results demonstrate that AD patients developed gut bacterial and fungal microbiota dysbioses and the gut microbiota may be involved in the development of alcohol addiction by regulating the endogenous cholecystokinin and related receptors’ expression. Our data open new avenues for adjuvant treating AD patients via administrating the intestinal microbiota. IMPORTANCE Intestinal microbiome dysbiosis is associated with psychiatric disease through the “microbiota-gut-brain” axis. Here, we revealed that there was obvious intestinal microbiome (including bacterial and fungal) dysbiosis in alcohol-dependent patients. Alcohol consumption seriously disturbs the gut equilibrium between bacteria and fungi, reduces the interactions among bacterial-fungal trans-kingdom, and increases intestinal permeability. Gut microbiota should be considered as a whole to study the development of alcohol dependence. The gut microbiome of alcohol-dependent patients increased the anxiety- and depression-like behavior in rats. The gut microbiota dysbiosis may promote the development of alcohol dependence by regulating the endogenous cholecystokinin (CCK) and related receptors. Hence, regulating the balance of gut microbiota and the endogenous CCK may be a potential strategy for reducing the risk of relapse in alcohol addiction patients.

Abstract:ABSTRACT Alcohol intake causes many diseases including neuropsychiatric symptoms, nutritional deficiency, progressive pancreatitis, liver cirrhosis, and ischemic heart disease. The gut microbiota changes significantly after alcohol exposure. Alcohol consumption tends to increase in underage and young people, but the feature of the gut microbiota in puberty remains largely unexplored. In this study, we conducted alcohol-exposed pubertal and adult mice model to investigate the intestinal damage and gut microbiota change. Interestingly, the responses of pubertal mice and adult mice after alcohol exposure were different. We found that alcohol dehydrogenase decreased and aldehyde dehydrogenase increased in the liver of pubertal mice, thus reducing the accumulation of toxic acetaldehyde. Furthermore, alcohol exposure caused less intestinal injury in pubertal mice. Through the analysis of metagenome assembly genome, we obtained many unrecognized bacterial genomes. Limosillactobacillus reuteri (cluster_56) and Lactobacillus intestinalis (cluster_57) were assembled from the samples of pubertal mice, which were involved in the production of indole acetic acid and the transformation of bile acids in response to alcohol exposure. This study provided a new insight to investigate the gut microbiota change and explained the difference of the gut microbiota after alcohol exposure between pubertal mice and adult mice. IMPORTANCE This study elucidates the significant impact of alcohol exposure on the gut microbiota and metabolic pathways in mice, highlighting the differential responses between adolescent and adult stages. Alcohol exposure was found to damage the intestinal barrier, alter the microbial composition by decreasing beneficial bacteria like Lactobacillus , and increase harmful bacteria such as Alistipes . The study also discovered unique microbial changes and resilience in pubertal mice. Species-level metagenomic analysis revealed specific microbial taxa and metabolic functions affected by alcohol. Metagenome-assembled genomes (MAGs) found many species that could not be annotated by conventional methods including many members of Lachnospiraceae , greatly expanding our understanding of the gut microbiota composition. These findings underscore the need for further research on alcohol’s effects on various organs and the implications of microbial metabolites on disease progression.

Abstract:OBJECTIVE:To confirm the effect of cigarette smoking on β-cell function, and further investigate the mediation effect of abdominal obesity.METHODS:Participants would include 1440 Chinese smokers who had participated in a community-based chronic disease screening project in Guangzhou and Zhuhai from 2006 to 2007. They were interviewed with structured questionnaire on their socio-demographic status and smoking behaviors. Waist-to-hip ratio (WHR) and fasting serum C-peptide concentration were also measured.RESULTS:After adjustment for the potential confounding factors, when compared with smokers with consumption 1 - 10 cigarettes/day, smokers with consumption of 11 - 20 cigarettes/day (adjusted OR = 1.53, 95%CI: 1.22 - 1.90) or > 20 cigarettes/day (adjusted OR = 1.92, 95%CI: 1.32 - 2.79) had significant higher risks to get C-peptide concentration larger than its median. Furthermore, 37.54% of the effect of cigarette smoking on C-peptide concentration was partially mediated by abdominal obesity.CONCLUSION:Cigarette smoking might be a risk factor for β-cell dysfunction and abdominal obesity.

Abstract:Alcoholic liver disease (ALD), a disorder caused by excessive alcohol intake represents a global health care burden. ALD encompasses a broad spectrum of hepatic injuries including asymptomatic steatosis, alcoholic steatohepatitis (ASH), fibrosis, cirrhosis, and hepatocellular carcinoma (HCC). The susceptibility of alcoholic patients to develop ALD is highly variable and its progression to more advanced stages is strongly influenced by several hits (i.e., amount and duration of alcohol abuse). Among them, the intestinal microbiota and its metabolites have been recently identified as paramount in ALD pathophysiology. Ethanol abuse triggers qualitative and quantitative modifications in intestinal flora taxonomic composition, mucosal inflammation, and intestinal barrier derangement. Intestinal hypermeability results in the translocation of viable pathogenic bacteria, Gram-negative microbial products, and pro-inflammatory luminal metabolites into the bloodstream, further corroborating the alcohol-induced liver damage. Thus, the premise of this review is to discuss the beneficial effect of gut microbiota modulation as a novel therapeutic approach in ALD management.

Abstract:Abstract Background: Alcohol intake can alter the gut microbiome, which may play a role in the development of cancer and other chronic diseases. However, there have been limited population-based studies investigating the associations of habitual and recent alcohol intake with the gut microbiome, particularly among Black Americans. Methods: We investigated the association of alcohol intake with the gut microbiome among 538 Black American participants of the Southern Community Cohort Study (150 men and 388 women). Habitual and recent alcohol intake was assessed at cohort baseline (2002-2009) and stool collection (2018-2021), respectively. DNA was extracted from fecal samples for gut microbiome profiling using shotgun metagenomic sequencing. Generalized linear models were employed to evaluate the associations between alcohol intake and gut microbiome diversity and composition, with adjustments for sociodemographic characteristics, other lifestyle factors, and comorbidities. False discovery rate (FDR)<0.1 was considered statistically significant. Results: The mean age at enrollment was 53.2 ± 7.7 years, with a mean interval of 13.8 years (range: 9.0-18.1 years) between baseline and stool collection. Neither habitual nor recent alcohol intake was associated with microbiome diversity indices, and recent alcohol intake was not associated with microbial taxa abundance either. However, habitual alcohol intake, both total amount and subtypes, showed significant associations with microbial taxa abundance primarily in men, including Actinobacteria, Proteobacteria, Firmicutes, and classes Bacilli and Clostridia within these phyla (all FDR<0.1). Specifically, total alcohol and red wine intakes were inversely associated with genus MGYG-HGUT-02719 in class Clostridia; beer consumption was inversely associated with genus Anaerostipes; white wine intake showed negative associations with order RF32 and genus Eubacterium and a positive association with genus Paenibacillus. Most of these associations remained significant after adjusting for BMI and history of cardiometabolic diseases or cancer. Conclusion: We identified gut microbiome taxa associated with habitual alcohol intake among Black American men, including several with anti- or pro-gastrointestinal-cancer properties. However, the magnitudes of these associations were generally small. Further research is needed to determine if these bacteria play a mediating or modifying role in alcohol-disease relationships. Abstract:Abstract Alcohol is part of the usual diet of millions of individuals worldwide. However, not all individuals who drink alcohol experience the same effects, nor will everyone develop an alcohol use disorder. Here we propose that the intestinal microbiota (IMB) helps explain the different consumption patterns of alcohol among individuals. 507 humans participated in this study and alcohol consumption and IMB composition were analyzed. On the other hand, in 80 adult male Wistar rats, behavioral tests, alcohol intoxication, fecal transplantation, administration of antibiotics and collection of fecal samples were performed. For identification and relative quantification of bacterial taxa was used the bacterial 16 S ribosomal RNA gene. In humans, we found that heavy episodic drinking is associated with a specific stool type phenotype (type 1, according to Bristol Stool Scale; p < 0.05) and with an increase in the abundance of Actinobacteria ( p < 0.05). Next, using rats, we demonstrate that the transfer of IMB from alcohol-intoxicated animals causes an increase in voluntary alcohol consumption in transplant-recipient animals ( p < 0.001). The relative quantification data indicate that the genus Porphyromonas could be associated with the effect on voluntary alcohol consumption. We also show that gut microbiota depletion by antibiotics administration causes a reduction in alcohol consumption ( p < 0.001) and altered the relative abundance of relevant phyla such as Firmicutes , Bacteroidetes or Cyanobacteria ( p < 0.05), among others. Benjamini–Hochberg false discovery rate (FDR) correction was performed for multiple comparisons. These studies reveal some of the consequences of alcohol on the IMB and provide evidence that manipulation of IMB may alter voluntary alcohol consumption.

Abstract:INTRODUCTION: Both cigarette smoking and gut microbiota play important roles in colorectal carcinogenesis. We explored whether the association between smoking and colorectal cancer (CRC) risk varies by gut microbial enterotypes and how smoking-related enterotypes promote colorectal carcinogenesis.METHODS: A case-control study was conducted. Fecal microbiota was determined by 16S rDNA sequencing. The cases with CRC or adenoma were subclassified by gut microbiota enterotypes. Multivariate analyses were used to test associations between smoking and the odds of colorectal neoplasm subtypes. Mann-Whitney U tests were used to find differential genera, genes, and pathways between the subtypes.RESULTS: Included in the study were 130 CRC patients (type I: n=77; type II: n=53), 120 adenoma patients (type I: n=66; type II: n=54), and 130 healthy participants. Smoking increased the odds for type II tumors significantly (all p for trend <0.05) but not for type I tumors. The associations of smoking with increased odds of colorectal neoplasm significantly differed by gut microbiota enterotypes (p<0.05 for heterogeneity). An increase in carcinogenic bacteria (genus Escherichia shigella) and a decrease in probiotics (family Lachnospiraceae and Ruminococcaceae) in type II tumors may drive disease progression by upregulating oncogenic signaling pathways and inflammatory/oxidative stress response pathways, as well as protein phospholipase D1/2, cytochrome C, and prostaglandin-endoperoxide synthase 2 expression.CONCLUSIONS: Smoking was associated with a higher odds of type II colorectal neoplasms but not type I tumors, supporting a potential role for the gut microbiota in mediating the association between smoking and colorectal neoplasms.
