Anti-proliferation Effects of Sirolimus Sustained Delivery Film in Rabbit Glaucoma Filtration Surgery.

Abstract:Purpose: To investigate the efficacy, safety, and mechanisms of Sirolimus sustained delivery film on prevention of scar formation in a rabbit model of glaucoma filtration surgery.Methods: Sixty-four New Zealand white rabbits who underwent trabeculectomy in the right eye were randomly allocated to one of the four treatment regimens: Sirolimus sustained delivery film treatment group (Group A), or drug-free film treatment group (Group B), or 30 ng/ml Sirolimus-soaked sponge treatment group (Group C), or no adjunctive treatment group (Group D), and each group consists of 16 rabbits. Intraocular pressure (IOP), morphologic changes of bleb, anterior chamber flare, and corneal endothelial cell count and complications were evaluated over a 28-day period follow-up time. Aqueous humor samples were gathered from Group A, and the concentration of Sirolimus was measured regularly post-operation. Rabbits were sacrificed on the 7th, 14th, and 28th day post-operation separately, and the fibroblast hypertrophy, infiltration of inflammatory, and proliferation of new collagen fiber formation in each group were evaluated with HE and Masson staining. Proliferative cell nuclear antigen (PCNA) and fibroblast apoptosis were evaluated by immunohistochemistry and terminal deoxynucleotidyl transferasemediated dUTP nick end labeling (TUNEL) assay at the 28th day post-operation.Results: Both Sirolimus sustained delivery film (Group A) and Sirolimus alone (Group C) were well tolerated in this model, and significantly prolonged bleb survival compared with no drug treatment group (Group B and D; p<0.001). Group A had the longest bleb survival time in comparison with other groups (p<0.001). There were significant differences in IOP readings between Group A and other groups at the last follow-up (p<0.05). The concentration of Group A maintained stable for over 2 weeks, drops from (10.56 +/- 0.05) ng/ml at day 3 to (7.74 +/- 0.05) ng/ml at day 14. The number of corneal endothelial cells of Group A was not statistically significant between pre and post-operation. Histologic examination demonstrated that eyes treated with Sirolimus, especially the Sirolimus sustained delivery film, showed an obvious reduction in subconjunctival fibroblast scar tissue formation compared with no drug treatment groups, and had minimal evidence of inflammatory cell infiltration and new collagen deposition in the subconjunctiva. Immunohistochemistry assay showed that PCNA-expression was lower in the Group A (16.25 +/- 3.24%) compared to other groups (p<0.01). TUNEL assay showed a significant increase in the number of apoptotic fibroblasts around the surgical area in Group A and Group C (9.75 +/- 1.71% and 8.50 +/- 1.92%) compared to the Group B and D (p<0.01).Conclusions: Sirolimus drug sustained delivery film can inhibit inflammatory cell activity, impede fibroblast proliferation activity, and induce fibroblast apoptosis in the filtration surgery sites in rabbit. The results indicate a safe and effective treatment strategy in anti-scaring treatment in glaucoma surgery.

Abstract:Purpose: The objective of the present study was to investigate the efficacy of rapamycin on rabbit lens epithelial cell proliferation, migration, and secrcetion of extracellular matrix fibronectin (Fn). Methods: Rabbit lens epithelium cells (rLECs) were isolated from 1 month old rabbit. rLECs were either cultured for 24, 48, or 72 h with different doses of rapamycin (0.1, 1, and 10 ng/ml). The proliferation kinetics, proliferating cell nuclear antigen (PCNA) expression, and basic fibroblast growth factor (bFGF)-induced migration of rLEC was determined by methyl thiazol tetrazolium (MTT) assay, western blotting and transwell chamber assay, respectively. The effect of rapamycin on the synthesis of Fn was examined via immunofluorescence. Results: Rapamycin significantly inhibited rLEC proliferation and PCNA protein expression when administered doses and time periods except for 0.1 ng/ml for 24 h. bFGF-induced migration rLECs was inhibited by pretreatment with rapamycin for 48 h. Extracellular matrix Fn formation of rLECs was also reduced by rapamycin. Conclusions: In our study, rapamycin strongly inhibited rLEC proliferation, bFGF-induced migration, and extracellular matrix Fn formation. Thus, rapamycin may have a potential inhibition of posterior capsule opacificatin (PCO) and needs further study.
SB-431542 Inhibition of Scar Formation after Filtration Surgery and Its Potential Mechanism.

Abstract:PURPOSE To explore the inhibitive effect of SB-431542 (an ALK5 inhibitor) on scar formation after glaucoma surgery and to identify the potential pharmacologic target(s). METHODS Twenty-four New Zealand rabbits underwent filtration surgery on the right eye and were divided into a control group and three experimental groups (n=6). Human Tenon's fibroblast monolayer was scraped to generate a single gap, and then the control medium with SB-431542 only or containing 10 microg/L TGF-beta1 and SB-431542 (1-20 microM) was added. The cells were pretreated with SB-431542 or in control medium for 30 minutes before induction with 10 microg/L TGF-beta1 or 1 microg/L TGF-beta2. The expression of alpha-SM-actin, CTGF, and Col I, as well as changes in the Smad, ERK, P38, and AKT signaling pathways were detected. RESULTS In comparison with the control rabbits, the IOPs in the experimental groups remained at lower levels until day 25 (P<0.05) after the surgery. Histologic profiles showed that there was only a mild deposition of collagen in the subconjunctival space in the experimental groups. The cell growth and migration were inhibited effectively by SB-431542, regardless of whether TGF-beta was present in the culture system. SB-431542 abrogated TGF-beta-induced upregulation of alpha-SM-actin, CTGF, and Col I. It effectively inhibited the phosphorylation of Smad2 stimulated by TGF-beta but not that of the components of the MAPK pathways. CONCLUSIONS SB-431542 inhibits scar formation after glaucoma filtration surgery. The mechanism may be that SB-431542 interferes in the phosphorylation of Smad2, thus abrogating TGF-beta-induced fibroblast transdifferentiation and then decreasing Col I synthesis.
CSM enhances the filtration bleb survival in rabbit model of experimental glaucoma surgery.

Abstract:Purpose: The aim of this study is to investigate whether CS-g-MMCs conjugate (CSM) could be a new agent to prevent the post-operative fibrosis in a rabbit model of experimental glaucoma filtration surgery. Materials and methods: In a randomized, controlled, masked-observer study, 40 New Zealand White rabbits underwent trabeculectomy in the right eyes and randomly received subconjunctival injection of phosphate buffered saline, chitosan (CS), CSM (100 mu g/ml), CSM (200 mu g/ml) or Mitomycin C (100 mu g/ml). Bleb characteristics and anterior chamber depth were evaluated by slit-lamp examination. The animals were killed on day 14 and 28. Histopathology and immunohistochemistry were performed to determine the amount of the scarring and fibrosis. Ocular toxicity was assessed by histopathology and electron microscope. Results: We found that the five groups were similar with respect to intraocular pressure and anterior chamber depth. The medians for survival days were: 5.5, 8, 17.5, 28 and 16 in the PBS, CS, CSM100, CSM200 and MMC groups, respectively. Both the CSM200 and the MMC group showed a significantly larger bleb area than the CSM100, CS and the PBS group. Less scarring was seen on day 14 and 28 in CSM200 and MMC group than in the PBS, CS and CSM100 group by histology and immunohistochemistry assessment. No damages were found in the rabbit eyes in each group. Conclusions: Subconjunctival injection of CSM postoperatively can improve the filtration bleb survival in the rabbit model. It can be a safe and effective antimetabolite in glaucoma surgery.
Experimental studies of rapamycin eye drops on the prevention of penetrating keratoplasty rejection in a rat model

Abstract:· AIM: To study the effects of rapamycin eye drops on the allogeneic corneal transplantation in a rat model. · METHODS: SD rats were used as recipients and Wistar rats were used as donors. Animals were randomly assigned to 5 groups. A: control(ophthalmic solution solvent); B:(0.5g/L rapamycin eye drops); C:(1g/L rapamycin eye drops); D:(2g/L rapamycin eye drops); E:(10g/L CsA eye drops). 4 times a day and 100μL each times. Animals received the treatment from the second day after the operation until the rejection. The grafts were observed by slit-lamp microscope and the corneal survival time was recorded. The pathologic changes were measured by light microscope. · RESULTS: The mean survival time of group A was merely 11.00±1.85d, But the mean survival time of group B, group C, group D and group E was 19.71±3.86d, 22.17±4.07d , 31.71±8.44d and 25.00±8.85d respectively, which was statistically prolonged compared with the control group (P<0.01). The histopathological findings showed that the inflammation cells, neovascularity in each treated group were significantly fewer than that in control group at 14d after the operation. · CONCLUSIONS: It is indicated that rapamycin eye drops could markedly prolong the corneal allografts survival in a rat model. The suppressive effect increased along with higher concentration. ·
Effects of Genistein on the Formation of Posterior Capsule Opacification

Abstract:Objective To investigate the inhibitive effects of genistein on the formation of posterior capsule opacification after phaco surgery in rabbit eyes.Methods Phacoemulsification surgeries were performed in 24 rabbit eyes,then divided them into the control group(12 eyes)and the treated group(12 eyes),genistein was irrigated into the anterior chamber in the treated group during the surgery.Slitlamp,ophthalmoscopy,and corneal endothelial cell counts were monitored for 2 months following surgery and histopathological examination was performed at 2 months after surgery.Results There were no significant differences in cornea and iris tissues under slit lamp examination between the control and the treated rabbit eyes.Two months after surgery,6 eyes of the control group had posterior capsule opacification in the periphery area,3 eyes had opacity in the central area,two of treated rabbit eyes had posterior capsule opacification in the periphery area,two had slight opacity in the central area.Immunostaining results showed that PCNA positive cells could be seen on each capsule,but positive rates in control group were significant higher than those in treated group(t=3.5,P0.01).Conclusion Genistein may be effective for the prevention of posterior cataract.
Zotarolimus alleviates post-trabeculectomy fibrosis via dual functions of anti-inflammation and regulating AMPK/mTOR axis

Abstract:Objective Postoperative scar formation is the primary cause of uncontrolled intraocular pressure following trabeculectomy failure. This study aimed to evaluate the efficacy of zotarolimus as an adjuvant anti-scarring agent in the experimental trabeculectomy. Methods We performed differential gene and Gene Ontology enrichment analysis on rabbit follicular transcriptome sequencing data (GSE156781). New Zealand white Rabbits were randomly assigned into three groups: Surgery only, Surgery with mitomycin-C treatment, Surgery with zotarolimus treatment. Rabbits were euthanized 3 days or 28 days post-trabeculectomy. Pathological sections were analyzed using immunohistochemistry, immunofluorescence, and Masson staining. In vitro, primary human tenon's capsule fibroblasts (HTFs) were stimulated by transforming growth factor-β1 (TGF-β1) and treated with either mitomycin-C or zotarolimus. Cell proliferation and migration were evaluated using cell counting kit-8, cell cycle, and scratch assays. Mitochondrial membrane potential was detected with the JC-1 probe, and reactive oxygen species were detected using the DCFH-DA probe. RNA and protein expressions were quantified using RT-qPCR and immunofluorescence. Results Transcriptome sequencing analysis revealed the involvement of complex immune factors and metabolic disorders in trabeculectomy outcomes. Zotarolimus effectively inhibited fibrosis, reduced proinflammatory factor release and immune cell infiltration, and improved the surgical outcomes of trabeculectomy. In TGF-β1–induced HTFs, zotarolimus reduced fibrosis, proliferation, and migration without cytotoxicity via the dual regulation of the TGF-β1/Smad2/3 and AMPK/AKT/mTOR pathways. Conclusion Our study demonstrates that zotarolimus mitigates fibrosis by reducing immune infiltration and correcting metabolic imbalances, offering a potential treatment for improving trabeculectomy surgical outcomes.

Abstract:Subconjunctival fibrosis is the major cause of failure in both conventional and modern minimally invasive glaucoma surgeries (MIGSs) with subconjunctival filtration. The search for safe and effective anti-fibrotic agents is critical for improving long-term surgical outcomes. In this study, we investigated the effect of inhibiting the rapamycin-insensitive mTORC1/4E-BP1 axis on the transforming growth factor-beta 1(TGF-(31)-induced fibrotic responses in human Tenon's fibroblasts (HTFs), as well as in a rat model of glaucoma filtration surgery (GFS). Primary cultured HTFs were treated with 3 ng/mL TGF-(31 for 24 h, followed by treatment with 10 mu M CZ415 for additional 24 h. Rapamycin (10 mu M) was utilized as a control for mTORC1/4E-BP1 signaling insensitivity. The expression levels of fibrosis-associated molecules were measured using quantitative real-time PCR, Western blotting, and immunofluorescence analysis. Cell migration was assessed through the scratch wound assay. Additionally, a rat model of GFS was employed to evaluate the anti-fibrotic effect of CZ415 in vivo. Our findings indicated that both rapamycin and CZ415 treatment significantly reduced the TGF-(31-induced cell proliferation, migration, and the expression of pro-fibrotic factors in HTFs. CZ415 also more effectively inhibited TGF-(31mediated collagen synthesis in HTFs compared to rapamycin. Activation of mTORC1/4E-BP signaling following TGF-(31 exposure was highly suppressed by CZ415 but was only modestly inhibited by rapamycin. Furthermore, CZ415 was found to decrease subconjunctival collagen deposition in rats post GFS. Our results suggest that rapamycin-insensitive mTORC1/4E-BP1 signaling plays a critical role in TGF-(31-driven collagen synthesis in HTFs. This study demonstrated that inhibition of the mTORC1/4E-BP1 axis offers superior anti-fibrotic efficacy compared to rapamycin and represents a promising target for improving the success rate of both traditional and modern GFSs.
Qingguang'an-induced autophagy in TFs inhibits scar formation: A follow-up in vivo mechanistic investigation

Abstract:Purpose: To investigate the mechanism by which Qingguang'an inhibits scar formation in rabbits administered glaucoma filtering surgery (GFS). Methods: Combined trabeculectomy was performed in 100 rabbits diagnosed with glaucoma, which were assigned to five groups, including the no surgery, surgery only, mitomycin C (MMC; positive control), Qingguang'an (experimental) and PBS (negative control) groups. The animals were followed up at postoperative days 1-28. Ultrastructure was observed under a transmission electron microscope (TEM). Real-Time Polymerase Chain Reaction (RT-PCR), Western blot, Hematoxylin and Eosin (H&E) staining, Masson's trichrome staining and Immuno-histochemistry (IHC) were performed to assess the harvested blocks. Results: In the Qingguang'an group, intraocular pressure (IOP) on postoperative D28 was significantly lower than values in the no surgery, surgery only and PBS groups (P < 0.05). Its blebs kept better filtering function and less complications in follow-up, which be detected to have less fibroblasts and collagen deposition histologically. Compared with the PBS group, ATG5, Beclin1 and LC3-II mRNA levels were significantly increased while P62 was downregulated in the Qingguang'an group (P < 0.05). Correspondingly, ATG5 and Beclin1 protein amounts in the Qingguang'an group were increased while P62 was downregulated. The LC3-II/Ⅰ ratio tended to rise to the process of autophagy. Abundant autophagosomes were captured under TEM in this condition. Conclusions: Qingguang'an granules can inhibit scar formation in rabbits after GFS and restrain IOP increase by inducing autophagy in TFs.
Repair effect of human acellular dermal matrix on filtering blebs leakage in rabbit eye

Abstract:Background Package and tissue patch implantation are common methods for repair of filtering bleb leaking after anti-glaucoma surgery.But the scarring or re-leakage of filtering bleb probably occur again.Objective This study was to investigate the repair effect of acellular dermal matrix (ADM) on filtering bleb leaking in rabbit model and compare the effectiveness among ADM,amniotic membrane and conjunctival overlap.Methods Trabeculectomy was performed on 48 eyes of 24 New Zealand rabbits,and models of filtering bleb leaking were established.The models were randomized into ADM group,amniotic membrane group and conjunctival covering group based on randomized number table.ADM patches with 4 mm×4 mm were implanted across lamellar cornea and sclera at a bridge in the ADM group,and the same size of amniotic membranes were used in the amniotic membrane group,and conjunctiva was sutured to limbus in the conjunctiva overlap group.The intraocular pressure (IOP) was measured before surgery and 1 day,1 week,1 month,3 months and 6 months after surgery.The biocompatibility of materials above was assessed under the slit lamp microscope,and the status of filtering bleb was evaluated and compared with anterior segment optical coherent tomography (AS-OCT) 1 day,1 week,1 month,3 months and 6 months after surgery.Results Before surgery and 1 day,1 week,1 month,3 months and 6 months after surgery,the IOP was (26.9±4.3),(16.6±5.1),(22.1 ±6.2),(18.3±6.5),(22.7±2.5),(23.4±1.4) mmHg in the AMD group,(29.9±5.4),(14.9 ± 6.4),(21.6 ± 7.8),(26.3 ± 4.1),(26.0 ± 4.2) and (23.0 ± 5.3) mmHg in the amniotic membrane group,and (28.7 ±4.3),(15.7 ±7.0),(22.0±6.3),(28.2±4.1),(24.7 ±4.1),(23.0±2.7) mmHg in the conjunctival overlap group,showing significant differences among different groups and various time points (Fgroup =8.419,P=0.011 ;Ftme =15.543,P=0.000).The IOPs were significantly lower from 1 day through 3 months after operation than those before operation in the AMD group (P =0.000,0.000,0.006,0.045) ; while the IOPs were reduced only from 1 day through 1 week after operation in comparison with before operation in the amniotic membrane group and the conjunctival overlap group (P =0.000,0.001).One month after surgery,the IOPs were significantly declined in the ADM group compared with the amniotic membrane group and the conjunctival overlap group (P =0.001,0.000).The grafts were clear under the slit lamp microscope and exhibited the valid filtering bleb until 3 months after operation under the AS-OCT in the ADM group.However,the valid filtering bleb remained only 1 month after surgery in the amniotic membrane group and the conjunctival overlap group.Neovascularization on the filtering bleb was found 3 months in the AMD group but 1 month in the amniotic membrane group and the conjunctival overlap group.Conclusions Compared with amniotic membrane and conjunctival tissue,ADM patch for the repair of filtering bleb leakage can increase the survival duration of filtering bleb and remain lower IOP.
Experimental Study of Effect of Pars Plana Filtration Technique on Incision-Related Morphology of Glaucoma Rabbits

Abstract:OBJECTIVE To investigate the effect of pars plana filtration technique on intraocular tissue and incision-related morphology in glaucoma rabbits.METHODS One group of eyes with high IOP as control group received no treatment,and other three groups of eyes as treatment groups received trabeculectomy,pars plana filtration surgery or pars plana filtration surgery combined silica gel pipe implantation respectively.Investigated the appearance of anterior segment,observed the intraocular tissue with ultrasonography,and then studied the ocular region histopathology under light microscope.RESULTS 1.Light microscope result: At postoperative1st week,there was nearly no significant difference on the number of inflammatory cells and proliferated fibroblasts infiltration around incision among the three treatment groups.At 4th week,the fibroblasts proliferated around incision and inflammatory reaction improved,filtered aisles blocked with scar formation in the 1st treatment group that received trabeculectomy.Filtered aisles turned narrow in the 2nd treatment group that received pars plana filtration surgery,but were still smooth in the 3rd treatment group that received pars plana filtration surgery combined silica gel pipe implantation.At 8th week,there was limited conglutination in scleral flap gap and incision scared over in the 1st treatment group.In the 2nd treatment group,conglutination limited in scleral flap gap and incision,then the scar formation made filtered aisles blocked incompletely.There was little scar formation round incision in the 3rd treatment group,but filtered aisles opened.2.UBM:Filtered aisles of eyes in the 2nd and 3rd treatment group kept longer time than in the 1st one.But in the 2nd treatment group,there was little vitreous in incision.3.Eyes in the three treatment groups found no abnormity such as flat chamber,retinal detachment or choroidal detachment.CONCLUSIONS Compared with trabeculectomy,pars plana filtration technique could make larger incision in order to inhibit the scar formation.The implantation of silica gel pipe could reduce the obstruction of fibrous proliferation membrane and the happening of vitreous wedge in pars plana incision.The security could be identified that the ocular tissues in glaucoma rabbits had no serious injury with this technique.
Losartan Attenuates Scar Formation in Filtering Bleb after Trabeculectomy.

Abstract:PURPOSE. To examine the effects of losartan on scar formation after trabeculectomy and on fibrotic changes of human Tenon's fibroblasts (HTFs).METHODS. Trabeculectomy was performed on New Zealand rabbits. They were randomized to receive one of the following treatments: 0.9% normal saline, mitomycin-C, or one of the three doses of losartan. Bleb morphology, IOP, and histopathology examination were performed. Primary cultured HTFs were treated with losartan or vehicle, with or without angiotensin II (Ang II). Cell proliferation was assessed by Cell Counting Kit-8 assay, and cell migration was detected by scratch wound and transwell assay. Transdifferentiation was evaluated through the expression of a-smooth muscle actin (alpha-SMA) by immunofluorescence, real-time PCR, and Western blot. The expression of fibronectin (FN) was evaluated by real-time PCR and Western blot.RESULTS. An amount of 5 mg/mL of losartan subconjunctival injection significantly decreased IOP postoperatively and attenuated wound healing of the filtering bleb in the rabbit model. Immunostaining results showed less myofibroblast and collagen deposition around the bleb area in the losartan-treated eyes. Losartan (10(-5) M) in vitro significantly attenuated Ang II's stimulatory effects on proliferation and migration of HTFs. Expressions of a-SMA and FN in these cells were also decreased by losartan pretreatment.CONCLUSIONS. Losartan attenuates scar formation of filtering bleb after trabeculectomy likely via decreasing proliferation, migration, transdifferentiation, and extracellular matrix deposition of Tenon's fibroblasts. These results indicate that losartan may be an effective therapeutic agent in preventing bleb scar formation and in improving surgical outcome after trabeculectomy.
Experimental study on inhibiting the keratoplasty immunologic rejection in rat by rapamycin

Abstract:Objective To study the immunosuppressive effects of rapamycin(RPM) on keratoplasty immunologic rejection in a rat model.Methods SD rats were used as recipients,and Wistar rats were used as donors.Animals were randomly assigned to the following four groups:control(0.5%carboxymethyl cellulose,IP);RPM (2.5 mg·kg~ -1 ·d~ -1 ,IP);CsA(10 mg·kg~ -1 ·d~ -1 ,IP);RPM+CsA (1.5 mg·kg~ -1 ·d~ -1 +5 mg·kg~ -1 ·d~ -1 ,IP).Treatment was began on the day of operation and were administered for 12 days,the grafts were inspected by clinical evaluation.Results The mean survival time of the control group was merely 11.29 d±1.50 d,but the mean survival time of the treated groups with RPM and CsA were respectively 20.57 d±2.37 d and 20.86 d±2.12 d,and the mean survival time of the combined therapy group was 22.57 d±1.99 d which was statistically prolonged compared with the control group (P<0.01).Conclusion It is indicated that RPM could markedly prolong the survival time of corneal allografts by inhibiting graft rejection,and combined therapy with RPM and CsA produce synergistic effect.
Influence of rapamycin combined with cyclosporin A eye drops on rejection reaction after high-risk corneal transplantation in rats

Abstract:Aim: To observe the immunosuppressive effect of rapamycin eye drops on immunological rejection after high-risk corneal transplantation in rats, and observe the effects of eye drops combined with cyclosporin A. Methods: The experiment was performed at the Animal Laboratory of Liaoning Medical University from December 2004 to March 2005. Totally 50 healthy male SD rats were used as recipients, and 23 female Wistar rats were used as donors. SD rats with corneal vascularization were taken as recipients to establish animal model of high-risk penetrative corneal transplantation. 1 Totally 40 recipients were involved in the result analysis, and randomly assigned into 4 groups, 10 rats in each group. Control group (ophthalmic solution solvent), Rapamycin-treated group (2 g/L rapamycin eye drops), Cyclosporin A-treated group (10 g/L cyclosporin A eye drops), 100 μL every time; Rapamycin and cyclosporin A-treated group (2 g/L rapamycin eye drops 50 μL and 10 g/L cyclosporin A eye drops 50 μL). The animals were given the drugs 4 times every day from the 2nd day after operation, and the animals were administered until rejection. 2 The corneal grafts were inspected by slit-lamp microscope clinically. The sum of turbidness, hydroncus and new vessels was as rejection reaction index on that day. When the index ≥ 5 or turbidness of grafts reached 3, it was considered the occurrence of rejection reaction. The corneal survival time was recorded. 3 At day 14 after corneal transplantation, 4 animals in each group were randomly selected for corneal histological test. Results: Totally 40 rats were involved in the result analysis. 1 Comparison of corneal survival time in each group: The corneal survival time in the Control group, Rapamycin-treated group, Cyclosporin A-treated group and Rapamycin and cyclosporin A-treated group was (7.67±1.03, 16.67±1.63, 15.50±2.43, 21.33±2.94) days, respectively. The survival time of the corneal grafts in each treated group was significantly prolonged as compared with the Control group (P < 0.01). The combined medicine group was significantly superior to the simple medicine group (P < 0.01). There was no significant difference between the Rapamycin-treated group and Cyclosporin A-treated group (P > 0.05). 2 Corneal histomorphology change at day 14 after corneal transplantation in each group: The inflammation cells, neovascularity and edema of the corneal grafts in each treated group were significantly fewer than those in the Control group, and there were no inflammation cells and neovascularity in the center of the grafts in combined treated group. Conclusion: 2 g/L rapamycin eye drops can markedly prolong the survival time of corneal grafts in high-risk corneal transplantation. The combined therapy with 10 g/L cyclosporin A eye drops produces synergistic effect, and the therapeutic efficacy is superior to rapamycin or cyclosporin A alone.
Effect of SMILE-derived Decellularized Lenticules As an Adhesion Barrier in a Rabbit Model of Glaucoma Filtration Surgery.

Abstract:BACKGROUND:To investigate the effects of small incision lenticule extraction (SMILE)-derived decellularized lenticules on intraocular pressure (IOP) and conjunctival scarring in a rabbit model of glaucoma filtration surgery.METHODS:Trabeculectomy was performed on both eyes of New Zealand rabbits. A decellularized lenticule was placed in the subconjunctival space in one eye of the rabbits (the decellularized lenticule group), and no adjunctive treatment was performed in the fellow eye (the control group). The filtering bleb features and IOP were evaluated 0, 3, 7, 14, 21, and 28 days after surgery, and histopathologic examination was performed 28 days after surgery.RESULTS:Decellularized lenticules significantly increased bleb survival and decreased IOP postoperatively in the rabbit model with no adverse side effects. The histopathologic results showed a larger subconjunctival space and less subconjunctival fibrosis in the decellularized lenticule group.CONCLUSIONS:Decellularized lenticules can prevent postoperative conjunctiva-sclera adhesion and fibrosis, and they may represent a novel antifibrotic agent for trabeculectomy.
An experimental study on homoharringtonine liposome and glaucoma filtration surgery.

Abstract:PURPOSE:To observe the inhibitory action of homoharringtonine liposome during the healing process of wounds in the filtering sites. METHODS:Posterior sclerectomies were performed in 14 rabbits. Postoperatively one eye of each rabbit received subconjunctival injections of HH liposome and fellow eye received saline injection in a randomized masked fashion. RESULTS:Fourteen days after operation the IOP of experimental eyes reduced significantly (P < 0.01) as compared with the controlled eyes, and the number of remaining filtering blebs increased noticeably (P < 0.05). Pathohistological examination revealed that the number of fibroblasts per square micron in the filtering sites and the thickness of the scars in the center of the filtering sites of the experimental eyes were less than those of the controlled eyes. No serious ocular toxic and side effects were found. CONCLUSION:This experiment suggest that homoharringtonine liposome can markedly inhibit the scar formation of filtering sites after glaucoma filtering surgery, promote the formation of filtering blebs, and provide an experimental evidence for its clinical use in future.
Anti-fibrosis potential of pirarubicin via inducing apoptotic and autophagic cell death in rabbit conjunctiva

Abstract:This study investigated the potential efficacy of pirarubicin (THP) in modulating rabbit conjunctival fibrosis both in vitro and in vivo and characterized the underlying mechanisms. Primary rabbit conjunctival fibroblasts (RCF) were cultured and treated with THP or mitomycin C (MMC) for 5 min, followed by assaying for cell viability, cell cycle distribution, apoptotic and autophagic pathways. The production of reactive oxygen species (ROS) and chemotaxis of macrophages by RCF were evaluated using 2′,7′-dichlorofluorescein diacetate (DCFH-DA) labeling and transwell migration assay, respectively. Limbal stem cell excision in combination with alkali burn was performed on the rabbits to establish a model of limbal deficiency and conjunctival fibro-vascular invasion. After three months, the modeled fibro-vascular tissue was excised combined with topical subconjunctival 5-min exposure to THP compared with MMC intraoperatively. The recurrence of postoperative fibrosis and the expression of apoptosis, autophagy, and inflammation markers were evaluated by immunohistochemistry. All modeled rabbits developed conjunctival fibro-vascular lesions, which were similar to human recurrent pterygium (HRP). Both THP and MMC inhibited RCF proliferation and arrested cell cycle at the G0/G1 phase. In particular, 7.5 μmol/L THP remarkably promoted RCF autophagy by upregulating the levels of Beclin 1, Atg 5/12 conjugate, and LC3B, whereas, 15 μmol/L THP significantly triggered a cascade of mitochondrial-associated RCF apoptosis. THP induced the production of ROS and enhanced the chemoattraction of macrophages by RCF. Similar to 600 μmol/L MMC, both 7.5 μmol/L and 15 μmol/L THP attenuated postoperative conjunctival fibrosis in the models; 7.5 μmol/L THP preferentially enhanced autophagy while causing fewer side effects. THP exerted its antifibrotic action by modulating autophagy in RCF, inducing cell cycle arrest, and mitochondrial-mediated apoptosis. THP at the dose of 7.5 μmol/L prevented postoperative conjunctival fibrosis in an animal model.

Abstract:Background: To investigate the inhibitory effect of a hyaluronic acid hydrogel loaded with hydroxycamptothecin (HCPT) on scar formation after filtration surgery in a rabbit model. Methods: Scleral fibroblasts were isolated and extracted from rabbits' eyes. After treatment with different concentrations of HCPT, cytotoxicity was detected using the 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide assay, and proliferation and extent of apoptosis were analysed using flow cytometry. Hydrogels loaded with different dosages of HCPT were prepared and placed under the scleral flap after the filtration surgery. One day, one week, and two weeks after surgery, follicular, conjunctival, corneal, and anterior chamber inflammation and iris and lens changes were observed. Results: In vitro, compared with cells not treated with HCPT, cells treated with HCPT had decreased survival rate and proliferation, and the apoptosis level increased with increasing HCPT concentrations (p < 0.05). In vivo, the flattening time of filtering blebs in the three groups treated with different dosages of HCPT hydrogel was delayed. The degrees of oedema, inflammation, and bleeding were similar to those observed in the control group. The HCPT hydrogel effectively downregulated the expression of collagen 1 and 3 and tissue inhibitor of metalloproteinase 2 and upregulated the expression of matrix metalloproteinase 2 in a dose-dependent manner. Conclusions: HCPT significantly inhibited the growth of rabbits' scleral fibroblasts and effectively inhibited scar formation after filtering surgery by accelerating the degradation of extracellular matrix deposition.
Topical administration of rapamycin promotes retinal ganglion cell survival and reduces intraocular pressure in a rat glaucoma model

Abstract:Glaucoma is a progressive optic neuropathy that has become the most common cause of irreversible blindness worldwide. Studies have shown that the protein mammalian target of rapamycin (mTOR) is a serine/threonine kinase that plays a central role in regulating numerous functions, such as growth, proliferation, cytoskeletal organization, metabolism, and autophagy. Clinical trials have shown that Rho-associated protein kinase (ROCK) inhibitors reduced IOP in patients with glaucoma and ocular hypertension (OHT). In this study, we explored whether rapamycin (RAPA) eye drops can reduce intraocular pressure (IOP) and protect retinal ganglion cells (RGCs). Our results indicated that in rats treated with RAPA, the drug was detected in the aqueous humor (AH), and the IOP was reduced. This may be related to the inhibition of RhoA protein activation by RAPA and regulation of the actin cytoskeleton in trabecular meshwork (TM) cells. In addition, the retinal thickness and the survival rate of RGCs were significantly reduced in the OHT group compared with the control group. These changes in the OHT group were significantly improved after treatment with RAPA. This may be because RAPA inhibited the activation of glial cells and the release of proinflammatory factors, thereby attenuating further damage to the retina and RGCs. Taken together, the results of this study demonstrated that RAPA not only reduced IOP but also protected RGCs, suggesting that RAPA is likely to be an effective strategy for the treatment of glaucoma.

Abstract:PURPOSE Despite 77 per cent antenatal care coverage and 90 per cent skilled attendant at delivery, adjusted maternal mortality in Iran is 76 per 100,000 births. Low quality of maternal health services is one cause of maternal morbidity and mortality. However, few and limited studies have been devoted to the quality of postpartum care in Iran. This study aims to assess quality of care in postpartum wards of Shaheed Beheshti Medical Science University hospitals to show weakness and gaps areas in the care procedure for future improvement intervention programs. It is a descriptive study to assess quality of care in postpartum wards of Shaheed Beheshti Medical Science University hospitals, in 2003. DESIGN/METHODOLOGY/APPROACH Using quota sampling, 60 healthy women were recruited for the study. Data were collected using three forms including a questionnaire with demographic and obstetrics questions, a check-list for the postpartum care and education quality assessment. Control of vital signs, uterus assessment, perineum assessment, leaving bed, urinary system assessment, digestive system assessment, breast examination, extremities assessment, psychological assessment, as well as education about perineum self-care, breast-feeding, infant care, education before discharge and educational method. Validity and reliability of the questionnaire and checklist were assessed prior to use. Data were analyzed using SPSS. FINDINGS Results showed compatibility of provided postpartum care with the standards as follows: method of patient's education (52.68 per cent); control of vital signs (43.21 per cent); education about breast-feeding (26.06 per cent); care in getting out of bed (25.83 per cent); psychological care (19.36 per cent); urinary system assessment (16.66 per cent); education about perineum care (13.12 per cent); uterus assessment (10.6 per cent); digestive system assessment (9.69 per cent); patient's education before discharge (7.99 per cent); education about infant's care (7.81 per cent); perineum assessment (6.72 per cent); breast examination (1.11 per cent); and assessment of extremities (0.81 per cent). The study demonstrated that weak postpartum care was provided in 82 per cent of cases but also that mothers were satisfied with provided care in all domains of care. Mothers were very satisfied with facilities and less satisfied with personnel interaction with their visitors in hospital. There was no significant correlation between quality of services and clients' satisfaction (Spearman test, p < 0.05). ORIGINALITY/VALUE For the first time in Iran, this study has evaluated quality of care in postpartum wards of hospitals based on the defined standards. The study provided a defined standard for postpartum care, which is necessary for regular monitoring and evaluation and so evidence-based intervention programs to improve the system of care. It was also postulated that mothers' satisfaction with care is not always a good indicator of services quality.
