Abstract:Objective To set up an animal model of mini tissue flaps and explore the mechanism of retrograde vein drainage.Methods A mini tissue flap based on the saphenous artery(flap size:1.0 cm×1. 0 cm)was created on both hind limbs of 30 Sprague Dawley rats.The left and right hind limbs of each rat were randomly assigned into group 1 and 2.In group 1,the flaps were elevated and the saphenous artery and its accompanying veins were kept in situ.In group 2,the flaps were elevated and only the saphenons artery were kept in situ,while its accompanying veins and other superficial veins were ligated.Survival of the flaps,their appearance and consistency were observed.Reconfiguration of venous drainage of the flaps was evaluated by angiography.Results Flaps of group 1 had normal skin color at postoperative day 1.Only 3 flaps in this group showed mild swelling which subsided at pestoperative day 2.All flaps had normal consistency.All the flaps of group 2 showed mild cyanosis and obvious swelling at postoperative day 1.There was tension of the flaps. Swelling of the flaps decreased and color of the flaps turned normal at postoperative day 2.Tension of the flaps also reduced.For swelling of the flaps,the difference between the two groups were statistically significant in the first and second days postoperatively(P0.01),but insignificant in the 3,7,9 and the 14 days postoperatively. For flap survival,in group 1 all flaps survived with normal skin color,good appearance and elasticity except one flap presented about 0.1 cm marginal skin necrosis.In group 2,there were 2 flaps with about 0.1 em marginal skin necrosis and 3 flaps with complete necrosis.The survived flaps had normal skin color and no swelling,but their appearance and elasticity were slightly worse than that of the flaps in group 1.Conclusion The survival mechanism of mini tissue flaps is different from that of conventional flaps.Restoration of venous drainage relies on collateral anastomosis between the flap and its tissue bed and surrounding margins in the first 3 postoperative days.When arterial flow is patent,mini tissue flaps still have a high survival rate even if the venous flow is obstructed due to a failed vein anastomosis.However a good venous anastomosis is required to ensure good flap appearance and consistency.
Lower limb soft tissue reconstruction using free ALT flaps: multimodal parameter analysis to predict the level of spontaneous reinnervation

Abstract:Introduction: This work aims to assess lower limb free flaps spontaneous sensory recovery by comparing and analyzing a single standardized reconstructive procedure, namely the free noninnervated anterolateral thigh (ALT) flap in order to evaluate which flap or patient-related factors may predict flap reinnervation. Methods: Between January 2010 and March 2018 all nonreinnervated ALT flaps for lower limb coverage performed at our institution were screened. We excluded from the study flaps with less than 18 months of follow-up time, neurotized flaps, and those from patients who missed the last follow up. Sensory modalities that were evaluated included the two-point discrimination (2PD) test, measured in mm; and the Semmes-Weinstein monofilament (SWM) test, measured in gram. The sensory parameter results were compared and analyzed according to flap size (two groups; <160 cm2 vs. > 160 cm2), and post-op time of testing (two groups; <18-28 months vs. > 28 months). Results: Twenty-one ALT free flaps were finally retained by this study. Our findings showed that flaps of smaller surface area showed a significantly better return in sensory discrimination 2PD and in sensory cutaneous pressure perception SWM testing. Conclusion: This work establishes for the first time some key quantitative data that can help predict free flap spontaneous reinnervation outcomes when using the same ALT flap. In our series, flaps surface remains the main discriminant value for a better sensory recovery.

Abstract:The usefulness of the Ouchterlony double-diffusion test was enhanced by modification of the technique to include a 24-hr period during which gels were pretreated with antiserum. The modified and conventional techniques were compared for effectiveness in estimating concentration of hepatitis-associated (Australia) antigen (HAA) and for sensitivity. Only with the modified test was a linear relationship found to exist between the distance of a precipitin line from the antigen well (PA) and the logarithm of antigen concentration. Therefore, it was possible to estimate the relative concentration of HAA in serum by referring its PA in the modified test to the curve of PA versus potency for a standard preparation of antigen. In addition, the modified test was found to be eightfold more sensitive than the conventional technique, with the result that 1.6 times as many positive sera were detected in routine screening.
Survival, Regeneration And Functional Recovery Of Motoneurons After Delayed Reimplantation Of Avulsed Spinal Root In Adult Rat

Abstract:We have established that extensive reinnervation and functional recovery follow immediate reimplantation of avulsed ventral roots in adult rats. In the present study, we examined the consequences of reimplantation delayed for 2 weeks after avulsion of the C6 spinal root. Twelve and 20 weeks after delayed reimplantation, 57% and 53% of the motoneurons in the injured spinal segment survived. More than 80% of surviving motoneurons regenerated axons into the reimplanted spinal root. Cholinesterase-silver staining revealed axon terminals on endplates in the denervated muscles. The biceps muscles in reimplanted animals had atrophied less than those in animals with avulsion only, as indicated by muscle wet weight and histological appearance. After electrical stimulation of the motor cortex or the C6 spinal root, typical EMG signals were recorded in biceps of reimplanted animals. The latency of the muscle potential at 20 weeks was similar to that of sham-operated controls. Behavioral recovery was demonstrated by a grooming test and ipsilateral forepaw movements were well coordinated in both voluntary and automatic activities. These results demonstrate that ventral root reimplantation can protect severed motoneurons, enable the severed motoneurons to regenerate axons, and enhance the recovery of forelimb function even when it is delayed for 2 weeks after avulsion. Endogenous automatic nerve discharge promotes nerve repair: an optimized animal model

Abstract:Exogenous electrical nerve stimulation has been reported to promote nerve regeneration. Our previous study has suggested that endogenous automatic nerve discharge of the phrenic nerve and intercostal nerve has a positive effect on nerve regeneration at 1 month postoperatively, but a negative effect at 2 months postoperatively, which may be caused by scar compression. In this study, we designed four different rat models to avoid the negative effect from scar compression. The control group received musculocutaneous nerve cut and repair. The other three groups were subjected to side-to-side transfer of either the phrenic (phrenic nerve group), intercostal (intercostal nerve group) or thoracodorsal nerves (thoracic dorsal nerve group), with sural nerve autograft distal to the anastomosis site. Musculocutaneous nerve regeneration was assessed by electrophysiology of the musculocutaneous nerve, muscle tension, muscle wet weight, maximum cross-sectional area of biceps, and myelinated fiber numbers of the proximal and distal ends of the anastomosis site of the musculocutaneous nerve and the middle of the nerve graft. At 1 month postoperatively, compound muscle action potential amplitude of the biceps in the phrenic nerve group and the intercostal nerve group was statistically higher than that in the control group. The myelinated nerve fiber numbers in the distal end of the musculocutaneous nerve and nerve graft anastomosis in the phrenic nerve and the intercostal nerve groups were statistically higher than those in the control and thoracic dorsal nerve groups. The neural degeneration rate in the middle of the nerve graft in the thoracic dorsal nerve group was statistically higher than that in the phrenic nerve and the intercostal nerve groups. At 2 and 3 months postoperatively, no significant difference was detected between the groups in all the assessments. These findings confirm that the phrenic nerve and intercostal nerve have a positive effect on nerve regeneration at the early stage of recovery. This study established an optimized animal model in which suturing the nerve graft to the distal site of the musculocutaneous nerve anastomosis prevented the inhibition of recovery from scar compression.
Specificity of Motor Axon Regeneration: A Comparison of Recovery Following Biodegradable Conduit Small Gap Tubulization and Epineurial Neurorrhaphy

Abstract:Functional recovery is often unsatisfactory after lesions in the peripheral nervous system despite the strong potential for regeneration and advances in microsurgical techniques. Axonal regeneration in mixed nerve into inappropriate pathways is a major contributing factor to this failure. In this study, the rat femoral nerve model of transection and surgical repair was used to evaluate the specificity of motor axon regeneration as well as functional and morphological recovery using biodegradable conduit small gap tubulization compared to epineurial neurorrhaphy. 12 weeks after nerve repair, the specificity was assessed using the retrograde neurotracers TB and DiI to backlabel motor neurons that regenerate axons into muscle and cutaneous pathways. To evaluate the functional recovery of the quadriceps muscle, the quadriceps muscle forces were examined. The quadriceps muscle and myelinated axons were assessed using electrophysiology and histology. The results showed that the specificity of motor axon regeneration (preferential reinnervation) was significantly higher when the nerve transection was treated by biodegradable conduit small gap tubulization and there was no significant difference between the two suture methods with respect to the functional and morphological recovery. This study demonstrated that the quicker and easier biodegradable conduit small gap tubulization may get more accurate reinnervation than traditional epineurial neurorrhaphy and produced functional and morphological recovery equal to traditional epineurial neurorrhaphy.
Altered Neural Pathways and Related Brain Remodeling: A Rat Study Using Different Nerve Reconstructions

Abstract:BACKGROUND: Function recovery is related to cortical plasticity. The brain remodeling patterns induced by alterations in peripheral nerve pathways with different nerve reconstructions are unknown.OBJECTIVE: To explore brain remodeling patterns related to alterations in peripheral neural pathways after different nerve reconstruction surgeries.METHODS: Twenty-four female Sprague-Dawley rats underwent complete left brachial plexus nerve transection, together with the following interventions: no nerve repair (n = 8), grafted nerve repair (n = 8), and phrenic nerve transfer (n = 8). Resting-state functional MR images of brain were acquired at the end of seventh month postsurgery. Amplitude of low-frequency fluctuation (ALFF), regional homogeneity (ReHo), and functional connectivity (FC) were compared among 3 groups. Behavioral observation and electromyography assessed nerve regeneration.RESULTS: Compared with brachial plexus injury group, ALFF and ReHo of left entorhinal cortex decreased in nerve repair and nerve transfer groups. The nerve transfer group showed increased ALFF and ReHo than nerve repair group in left caudate putamen, right accumbens nucleus shell (AcbSh), and right somatosensory cortex. The FC between right somatosensory cortex and bilateral piriform cortices and bilateral somatosensory cortices increased in nerve repair group than brachial plexus injury and nerve transfer groups. The nerve transfer group showed increased FC between right somatosensory cortex and areas including left corpus callosum, left retrosplenial cortex, right parietal association cortex, and right dorsolateral thalamus than nerve repair group.CONCLUSION: Entorhinal cortex is a key brain area in recovery of limb function after nerve reconstruction. Nerve transfer related brain remodeling mainly involved contralateral sensorimotor areas, facilitating directional "shifting" of motor representation.

Abstract:INTRODUCTION:Thumb and fingertip resurfacing continues to be a challenge for hand surgeons, as the treatment varies widely. Being donor nerves in the flap for sensory restoration, the dorsal cutaneous nerves in hand have been widely described in the literature. However, their applications in providing anatomical information to sufficiently harvest the longitudinal axis of a robust flap are usually underestimated. This study reports reconstruction of thumb and fingertip defects with the distally based dorsal fasciocutaneous (DDF) flap and explores the effect of intraoperative nerve identification technique on clinical outcomes.HYPOTHESIS:The nerve identification technique can contribute considerably to the improvement of flap circulation, with less risk of poor venous return.MATERIALS AND METHODS:A retrospective study was conducted in 89 patients who had thumb or fingertip defects treated with the DDF flap from February 2014 to June 2020. Patients were divided into two groups based on whether intraoperative nerve identification technique was used. Flap survival was evaluated. The follow-up was conducted by using smartphone or by face-to-face visit. Time for complete wound healing, time to return to work, hand performance, the total active motion of the joints and static two-point discrimination of flaps were analyzed.RESULTS:The 46 flaps (group A) that underwent intraoperative nerve identification technique had an 8.7% venous congestion rate versus a 27.9% venous congestion rate in the 43 flaps (group B) without using nerve identification technique (p=0.018). The mean follow-up was 11.08 (range 6-26) months. Time for complete wound healing and time to return to work were significantly shorter in group A (p=0.026 and p=0.012). Michigan Hand Outcomes Questionnaire (MHQ) scores were similar between groups. In the face-to-face follow-up, there was no significant difference in the total active motion of the injured fingers and static two-point discrimination between the two groups (p=0.657 and p=0.182).DISCUSSION:The use of nerve identification technique could decrease the odds of postoperative venous congestion in DDF flap for thumb and fingertip reconstruction, and result in improved outcomes in terms of time for complete wound healing and time to return to work.LEVEL OF EVIDENCE:III; retrospective comparative study.
Experiment of the Alteration of Vascular and Endogenous Vascular Endothelial Growth Factor in the Survival Process of the Reverse-Flow Axial Skin Flap

Abstract:Objective To investigate the survival mechanisms of the reverse-flow flap. Methods Rectangle skin flaps based on the deep circumflex iliac vessel( DCIV), which comprised the adjacent territory based on the posterior intercostal arteries(PIA),were designed and elevated on the rat's right dorsum .The flaps were served as the reverse-flow flaps model. The survival area, the vessels redistribution, the change of pedicle vessel's caliber, and VEGF immunohistochemistry were evaluated immediately, and on the 1st, 2nd, 3rd, 5th, 7th day after operation respectively. Results All flaps survived. Blood supplied by DCIV flowed to the distal and refluxed to the proximal through the vascular anastomosis between the two territories. The direction of blood flow in the distal territory run in a reversed pattern after operation. VEGF immunohistochemistry was negative immediately after operation. The caliber of vascular anastomosis and DCIV became obviously wider and the number of vascular anastomosis became conspicuously more 1 day after operation than that before operation, meanwhile, VEGF immunohistochemistry became active. All data reached to the peak 3 days after operation. On the 7th day, the vasculature of the flap became similar to that of the conventional axial flap and the caliber of DCIV decreased, VEGF immunohistochemistry became negative. Conclusion The survival mechanism of the reverse-flow axial skin flap was the increased number and dilated caliber of vessels. In the end,the new vasculature of the flaps were similar to the conventional axial flaps. Endogenous VEGF play an important role in this process.
Survival, Regeneration and Functional Recovery of Motoneurons in Adult Rats by Reimplantation of Ventral Root Following Spinal Root Avulsion

Abstract:We investigated the functional recovery of motoneurons after reimplanting an avulsed ventral root in a rat model of traction injury The eighth cervical root (C8) was avulsed by controlled traction and immediately reimplanted to the spinal cord. Spinal nerves from neighbouring segments (C5, C6, C7 and T1) were ligated and cut. After 12 or 20 weeks, the survival, regeneration and functional recovery of spinal motoneurons were evaluated by Nissl staining, retrograde labelling of motoneurons, NOS histochemistry, histological examination of muscle and nerve-muscle junction, electromyography and behavioural observation. In the control animals, about 14% or 11% of spinal motoneurons survived 12 or 20 weeks postinjury, respectively. By contrast, in animals with ventral root reimplantation, 62% and 55% of motoneurons survived at 12 or 20 weeks postinjury, respectively. Retrograde labelling and histological examination indicated that about 90% of the surviving motoneurons in the C8 segment regenerated axons into the reimplanted ventral root. Staining the muscles with silver and cholinesterase revealed new motor endplates in the reinnervated muscle. Functionally significant electromyographic responses in flexor digitorum superficialis and flexor carpi radialis were observed in experimental animals; however, the average latency of the motor action potentials was greater than normal control. The grasping test showed functional recovery of finger flexors and median nerve. In conclusion, our results indicate that spinal motoneurons can regenerate axons through reimplanted roots and reinnervate muscles to recover partial function.
Myelinated and unmyelinated nerve fibers reinnervate tissue-engineered dermo-epidermal human skin analogs in an in vivo model

Abstract:Purpose: The clinical application of autologous tissue-engineered skin analogs is an important strategy to cover large skin defects. Investigating biological dynamics, such as reinnervation after transplantation, is essential to improve the quality of such skin analogs. Previously, we have examined that our skin substitutes are reinnervated by host peripheral nerve fibers as early as 8 weeks after transplantation. Here, we wanted to investigate the presence and possible differences regarding myelinated and unmyelinated host nerve fibers 15 weeks after the transplantation of light and dark human tissue-engineered skin analogs. Methods: Human epidermal keratinocytes, melanocytes, and dermal fibroblasts were isolated from human light and dark skin biopsies. Keratinocytes and melanocytes were seeded on fibroblast-containing collagen type I hydrogels after expansion in culture. After additional culturing, the tissue-engineered dermo-epidermal skin analogs were transplanted onto full-thickness skin wounds created on the back of immuno-incompetent rats. Skin substitutes were excised and analyzed 15 weeks after transplantation. Histological sections were examined with regard to the ingrowth pattern of myelinated and unmyelinated nerve fibers into the skin analogs using markers, such as Substance P, NF200, and S100-Beta. Results: We found myelinated and unmyelinated peripheral host nerve fibers 15 weeks after transplantation in the dermal part of our human skin substitutes. In particular, we identified large-diameter-myelinated Aβ- and Aδ-fibers, and small-diameter C-fibers. Furthermore, we observed myelinated nerves in close proximity to CD31-positive blood capillaries. In the long run, both types of ingrown host fibers showed an identical pattern in both light and dark skin analogs. Conclusion: Our data suggest that myelinated and unmyelinated peripheral nerves reinnervate human skin substitutes in a long-term in vivo transplantation assay. Our tissue-engineered skin analogs attract A- and C-fibers to supply both light and dark skin analogs. Potentially, this process restores skin sensitivity and has, therefore, a significant relevance with regard to future application of autologous pigmented dermo-epidermal skin substitutes onto patients.
Effect of vascularized jejunal conduit flap on peripheral nerve regeneration in rats

Abstract:Background/aim: In the literature, almost all of the nerve conduits proposed for obtaining better nerve recovery were applied as graft materials. In this study, we aimed to propose a new nerve conduit model with a flap pattern and evaluate the effect of a pedicled vascularized jejunal flap on nerve regeneration after wrapping it around a sciatic nerve. Materials and methods: A total of 90 Wistar albino rats were randomly divided into nine groups with 10 rats in each. The first three groups constituted the control groups, whereas Groups 4-6 were the jejunum conduit (JC)-applied groups. A mucosa-resected JC (MRJC) was applied in Groups 7 and 8. Epineurial neurorrhaphy was performed in Groups 1, 4, and 7; repair with a nerve graft was applied in Groups 2, 5, and 8; and a 1-cm-long nerve defect was created in Groups 3, 6, and 9. After 2 months of follow-up, nerve regeneration was assessed by statistical analyses of the Sciatic Functional Index (SFI) and histopathological evaluation. Results: The MRJC groups had significantly better results in terms of SFI (p = 0.005). Statistical differences in axonal degeneration, axonal density, myelination, and disorganization were found between all control groups and MRJC groups (p = 0.022, p = 0.001, p = 0.001, and p = 0.039, respectively). Conclusion: In this study, the feasibility of wrapping around the nerve repair zones of pedicled autologous flaps designed in a tubular fashion was observed in a small rat model. The findings must be further validated with larger animals before clinical testing.
Resting-state brain network remodeling after different nerve reconstruction surgeries: a functional magnetic resonance imaging study in brachial plexus injury rats.

However, differences among reconstruction strategies at the brain network level have not been elucidated. This study aimed to explore intra-network changes related to altered peripheral neural pathways after different nerve reconstruction surgeries, including nerve repair, end-to-end nerve transfer, and end-to-side nerve transfer. Sprague-Dawley rats underwent complete left brachial plexus transection and were divided into four equal groups of eight: no nerve repair, grafted nerve repair, phrenic nerve end-to-end transfer, and end-to-side transfer with a graft sutured to the anterior upper trunk. Resting-state brain functional magnetic resonance imaging was obtained 7 months after surgery. The independent component analysis algorithm was utilized to identify group-level network components of interest and extract resting-state functional connectivity values of each voxel within the component. Alterations in intra-network resting-state functional connectivity were compared among the groups. Target muscle reinnervation was assessed by behavioral observation (elbow flexion) and electromyography. The results showed that alterations in the sensorimotor and interoception networks were mostly related to changes in the peripheral neural pathway. Nerve repair was related to enhanced connectivity within the sensorimotor network, while end-to-side nerve transfer might be more beneficial for restoring control over the affected limb by the original motor representation. The thalamic-cortical pathway was enhanced within the interoception network after nerve repair and end-to-end nerve transfer. Brain areas related to cognition and emotion were enhanced after end-to-side nerve transfer. Our study revealed important brain networks related to different nerve reconstructions. These networks may be potential targets for enhancing motor recovery.

Abstract:Objective: To investigate the effect of 810nm near-infrared (NIR) laser on the revascularization of ischemic flaps. Background: It has long been proved that photobiomodulation therapy (PBMT) improves the blood supply of flaps. NIR laser improves the treatment of hypodermis-located lesions and of flap survival, but basic research on the use of 810nm NIR laser for ischemic flap revascularization is still lacking. Materials and methods: We prepared two symmetrical long random-pattern flaps on the backs of 60 rats. Each flap was 6cm long, 1cm wide, and 1cm to the middle line. The flaps were divided into an irradiated flap group and an internal control group. The irradiated flaps underwent postoperative 810nm laser therapy with the energy density of 11.30J/cm(2) daily. The control flaps were covered by stainless steel to avoid laser irradiation. We observed the viability of the flaps. The flaps underwent Hematoxylin and Eosin (H&E) staining for the observation of histomorphology, immunohistochemical staining of factor VIII for the capillary count, -smooth muscle actin for the small arterial count, and vascular endothelial growth factor for the integrated optical density (OD) of the positive stained color. Results: The irradiated flaps showed significantly better flap survival than the control flaps. H&E staining showed that the irradiated flaps had clear tissue structure and little inflammatory cell infiltration. The control flaps demonstrated comparatively worse results. Vascular endothelial growth factor staining showed that the difference in integrated OD between the irradiated flaps and the control flaps was not statistically significant. -smooth muscle actin and factor VIII staining showed significantly greater numbers of arterioles and capillaries in the irradiated flaps than the control flaps after 4 days of irradiation. Conclusions: PBMT with 810nm NIR laser could enhance ischemic flap revascularization and increase flap viability.
Experimental Study on Treating Root Avulsion Injury of Brachial Plexus Via Combining Nerve Root Re-Implantation

Abstract:Objective To observe the therapeutic effect of nerve root re-implantation in treating brachial plexus avulsion injury.Methods Eighty Sprague-Dawley rats,weighting 200-250 g and unlimited to sex were randomly divided into three groups,with six rats each.Under operation microscopy,the C5、C6、C7 roots were avulsed and C6 anterior root re-implanted to spine cord after cutting the dorsal root and ganglion pleurale via a posterior approach.Then we cut branches erupt from C6 and affiliate to musculocutaneous nerve via an anterior approach,and the reimplanted C6 anterior root could be connected singly with musculocutaneous nerve.Left side was experimental side and right side was control side.We observe the restoration of elbow flexion,biceps brachii motion evoked potential,biceps brachii wet weight,muscle fibrin cross section area,and compare the difference of restoration rate in three groups one,two and three months later.Results One month after surgery,the muscle wet weight,muscle fibrin cross section area and muscle motion evoked potential began restore.Two months after surgery,the restoration rate of muscle wet weight,muscle fibrin cross section area and muscle motion evoked potential increased significantly compared to group one months.Three months after surgery,the restoration rate of muscle wet weight,muscle fibrin cross section area and muscle motion evoked potential increased significantly compared to group two months.Conclusion Muscle wet weight,muscle fibrin cross section area and muscle motion evoked potential can restore after nerve root reimplantation in brachial plexus avulsion,so nerve root reimplantation is a method to treat brachial plexus avulsion.
Resting-state Brain Network Remodeling after Different Nerve Reconstruction Surgeries:A Functional Magnetic Resonance Imaging Study in Brachial Plexus Injury Rats

Abstract:Distinct brain remodeling has been found after different nerve reconstruction strategies, including motor representation of the affected limb. However, differences among reconstruction strategies at the brain network level have not been elucidated. This study aimed to explore intranetwork changes related to altered peripheral neural pathways after different nerve reconstruction surgeries, including nerve repair, endto-end nerve transfer, and end-to-side nerve transfer. Sprague–Dawley rats underwent complete left brachial plexus transection and were divided into four equal groups of eight: no nerve repair, grafted nerve repair, phrenic nerve end-to-end transfer, and end-to-side transfer with a graft sutured to the anterior upper trunk. Resting-state brain functional magnetic resonance imaging was obtained 7 months after surgery. The independent component analysis algorithm was utilized to identify group-level network components of interest and extract resting-state functional connectivity values of each voxel within the component. Alterations in intra-network resting-state functional connectivity were compared among the groups. Target muscle reinnervation was assessed by behavioral observation(elbow flexion) and electromyography. The results showed that alterations in the sensorimotor and interoception networks were mostly related to changes in the peripheral neural pathway. Nerve repair was related to enhanced connectivity within the sensorimotor network, while end-to-side nerve transfer might be more beneficial for restoring control over the affected limb by the original motor representation. The thalamic-cortical pathway was enhanced within the interoception network after nerve repair and end-to-end nerve transfer. Brain areas related to cognition and emotion were enhanced after end-to-side nerve transfer. Our study revealed important brain networks related to different nerve reconstructions. These networks may be potential targets for enhancing motor recovery.

Abstract:Objective: To evaluate the dynamic change of CD34 level during the survival process of narrow pedicle flaps. Methods: Twenty-five white pigs were randomly and equally divided into 5 experimental groups. Five different type of narrow pedicle with different length-to-width ratio were employed, and each type of narrow pedicle was covered with 5 different size random flaps and which was classified into A, B, C, D and E for 5 groups. Group A was control group. Each type narrow pedicle with 5 different skin flaps were implanted onto the back of the pigs along the midline of back with a reverse direction. A 0.3 cm×0.3 cm full thickness skin flap in the middle of distal segment was collected and on 3rd, 5th, 7th and 14th days of post-operation. The expression of CD34 was measured by immunohistochemistry and enzyme-linked immunosorbent (ELISA). Results: Histological examination showed that with the increasing of length-to-width ratio of the narrow pedicle skin flaps, the expression of CD34 increased in the skin flaps. Increased level of CD34 was found on 3rd day post-operation, and the peak expression was found on 7th day. Persistent high level of CD34 was found until 14th day. Conclusion: Increased CD34 level in the distal skin flap, there is the association between CD34 level and ischemia injury. Moreover, CD34 expression plays an important role during the repair processes of pedicle flaps.
Myocutaneous revascularization following graded ischemia in lean and obese mice

Abstract:Background: Murine models of diabetes and obesity have provided insight into the pathogenesis of impaired epithelialization of excisional skin wounds. However, knowledge of postischemic myocutaneous revascularization in these models is limited. Materials and methods: A myocutaneous flap was created on the dorsum of wild type (C57BL/6), genetically obese and diabetic (ob/ob, db/db), complementary heterozygous (ob+/ob-, db+/db-), and diet-induced obese (DIO) mice (n=48 total; five operative mice per strain and three unoperated mice per strain as controls). Flap perfusion was documented by laser speckle contrast imaging. Local gene expression in control and postoperative flap tissue specimens was determined by quantitative reverse transcription polymerase chain reaction (RT-PCR). Image analysis of immunochemically stained histologic sections confirmed microvascular density and macrophage presence. Results: Day 10 planimetric analysis revealed mean flap surface area necrosis values of 10.8%, 12.9%, 9.9%, 0.4%, 1.4%, and 23.0% for wild type, db+/db-, ob+/ob-, db/db, ob/ob, and DIO flaps, respectively. Over 10 days, laser speckle imaging documented increased perfusion at all time points with revascularization to supranormal perfusion in db/db and ob/ob flaps. In contrast, wild type, heterozygous, and DIO flaps displayed expected graded ischemia with failure of perfusion to return to baseline values. RT-PCR demonstrated statistically significant differences in angiogenic gene expression between lean and obese mice at baseline (unoperated) and at day 10. Conclusion: Unexpected increased baseline skin perfusion and augmented myocutaneous revascularization accompanied by a control proangiogenic transcriptional signature in genetically obese mice compared to DIO and lean mice are reported. In future research, laser speckle imaging has been planned to be utilized in order to correlate spatiotemporal wound reperfusion with changes in cell recruitment and gene expression to better understand the differences in wound microvascular biology in lean and obese states.
Supercharge End-to-Side Sensory Transfer to A Long Nerve Graft to Enhance Motor Regeneration in A Brachial Plexus Model—An Experimental Rat Study

Abstract:Background Long nerve grafts will affect muscle recovery. Aim of this study is to investigate if supercharged end-to-side (SETS) sensory nerve transfer to long nerve graft can enhance functional outcomes in brachial plexus animal model. Methods A reversed long nerve graft (20–23-mm) was interposed between C6 and musculocutaneous nerve (MCN) in 48 SD rats. The sensory nerves adjacent to the proximal and distal coaptation sites of the nerve graft were used for SETS. There were four groups with 12 rats in each: (A) nerve graft alone, (B) proximal SETS sensory transfer, (C) distal SETS sensory transfer, and (D) combined proximal and distal SETS sensory transfers. Grooming test at 4, 8, 12 and 16 weeks, and compound muscle action potentials (CMAP), biceps tetanic muscle contraction force, muscle weight and MCN axon histomorphologic analysis at 16 weeks were assessed. Results Grooming test was significantly better in group C and D at 8 weeks ( p = 0.02 and p = 0.04) and still superior at 16 weeks. There was no significant difference in CMAP, tetanic muscle contraction force, or muscle weight. The axon counts showed all experimental arms were significantly higher than the unoperated arms. Although the axon count was lowest in group C and highest in group D ( p = 0.02), the nerve morphology tended to be better in group C overall. Conclusion Distal sensory SETS transfer to a long nerve graft showed benefits of functional muscle recovery and better target nerve morphology. Proximal sensory inputs do not benefit the outcomes at all.
