Abstract:In the context of rapid development, Beijing, the capital of China, is facing huge challenges in providing fair healthcare resources to residents. Although Beijing has the best healthcare resources nationwide, a highly concentrated population and uneven distribution of hospitals make the supply of medical resources tight and unbalanced. The objective of this study is to explore the healthcare resource inequality in Beijing based on spatial accessibility. The two-step floating catchment area method was improved to measure healthcare accessibility by defining a novel distance attenuation function that conforms to the specific travel behavior of taxies to hospitals. We explored the inequality among different places and different populations. It was found that the spatial inequality of healthcare resources was evident and typical, with the dominant resources concentrated in the city center. Some regions are always in an advantageous position regardless of traffic conditions. The impact of some social-economic factors on healthcare accessibility was analyzed, which exhibited significant spatial heterogeneity. Hospital deserts for different vulnerable populations were identified. Besides children with massive hospital deserts at the city fringe, other vulnerable populations have no distinct disadvantage. These results offer profound comprehension of healthcare inequality to assist in healthcare resources management and policy-making in Beijing.

Abstract:Due to the close links between quality of life standards and level of regional development, it is important to gain an improved understanding of the factors that contribute to unequal spatial accessibility of medical and health services in China. This study analyzed 2859 counties using the average shortest distance traveled from settlements to medical facilities to calculate spatial accessibility. The Theil index at three levels (three partitions, eight economic zones and provinces) was used to assess the inequality between regions. Then, a method of spatial autocorrelation was used to assess the spatial agglomeration characteristics of spatial accessibility. To explore the underlying reasons for the imbalance in spatial accessibility, a total of eight indicators in three aspects (nature, society, and economy) were collected and geographically weighted regression (GWR) was employed to investigate spatial heterogeneity. We observed marked disparities in spatial accessibility to medical facilities at the national level. In particular, there appears to be improved spatial accessibility and lesser regional inequality in eastern and central regions as opposed to western Chinese regions; in coastal regions instead of inland regions; and in municipalities such as Beijing, Tianjin, and Shanghai, but not in other provinces and autonomous regions. Besides, significant global autocorrelation and obvious clusters were revealed in a spatial pattern analysis. A wide range of hot-spot areas (clusters of poor spatial accessibility) mainly concentrated in Tibet and other areas with complex terrain and lagging social and economic development. A number of cold-spot areas (clusters of good spatial accessibility) mostly scattered in built-up areas especially municipalities and well-developed urban agglomeration such as the Yangtze River Delta, the Pearl River Delta, and the Wuhan 1 + 8 city circle. Furthermore, the relationships between influencing factors and spatial accessibility were also investigated. The influence of altitude on spatial accessibility gradually decreased from the West to the East, while the influence of slope decreased from the South to the North with prominent provinces of Qinghai and Gansu. Population density exerted a higher impact on the distance in the southwest region whereas the urbanization rate influenced the northeast and southeast coastal region more intensively. In contrast, the pattern of educational level was relatively discrete. The influencing pattern of economic factors in both per area GDP and tertiary industry output share of accessibility showed apparent regional characteristics in the southeast coastal areas, as well as in the northwestern and northeastern parts. Outcomes from this study can be used to provide important information to aid policy making, while also facilitating future research aimed at improving the understanding of equality and sustainable development of medical facilities.

Assessing Spatial Accessibility to Primary Health Care Services in Beijing, China.

Abstract:Primary health care has been emphasized as a pillar of China's current round of health reforms throughout the previous decade. The purpose of this study is to analyze the accessibility of primary health care services in Beijing and to identify locations with a relative scarcity of health personnel. Seven ecological conservation districts, which are relatively underdeveloped, were selected in the study. The Gini coefficient and Lorenz curve, as well as the shortest trip time and modified two-step floating catchment area (M2SFCA) approach, are used to quantify inequalities in primary health care resources and spatial accessibility. The Gini coefficient of primary medical services was calculated as high as 0.705, showing a significant disparity in primary care services. A total of 81.22% of communities reached the nearest primary care institution within 15 min. The average accessibility of primary healthcare services, as measured by the number of health professionals per 1000 population, was 2.34 in the 1715 communities of seven ecological conservation districts. Three hundred and ninety-one communities (22.80%) were identified with relatively low accessibility. More primary health professionals should be allocated to Miyun, Mentougou, and Changping Districts. Overall, the primary healthcare resources were distributed unevenly in most districts. According to our study, expanding primary healthcare institutions, increasing the number of competent health professionals, and enhancing road networks will all be effective ways to increase spatial accessibility and reduce primary healthcare service disparity in Beijing.
Examining the Spatial and Social Heterogeneities of Accessing Hierarchical Healthcare Services Through the Lens of Equity: A Case Study of the Central Urban Area in Shanghai, China

Abstract:Achieving equal access to healthcare is an essential goal of the Sustainable Development Goals and a key criterion for building healthy communities. In this study, we examine the spatial and social heterogeneities of accessing hierarchical healthcare services through the lens of equity in the central urban area of Shanghai. The BFCA method is applied to analyze the accessibility to healthcare with various levels of services and via travel modes from communities and identify communities with a spatial mismatch. The relationship between factors, including socio-economic factors, health status, medical choice, and accessing healthcare services are examined using a multi-nominal logistic model. The results show that 1) Accessing healthcare performs a spatial mismatch and core-periphery disparity in the central urban area. 2) The lacking the accessibility to high-tier healthcare and a walkable environment are two main spatial equity issues, especially the latter one. 3) The inequities of access to healthcare facilities are systematic differences in factors including age, family income, family expenditure, medical choice, and health status. The results highlight the geographic and demographic disparities in healthcare services through the lens of equity and provide suggestions to improve the healthcare coverage and develop a sustainable and healthy-oriented city.
Unmasking unexpected health care inequalities in China using urban big data: Service-rich and service-poor communities

Abstract:Geographic accessibility plays a key role in health care inequality but remains insufficiently investigated in China, primarily due to the lack of accurate, broad-coverage data on supply and demand. In this paper, we employ an innovative approach to local supply-and-demand conditions to (1) reveal the status quo of the distribution of health care provision and (2) examine whether individual households from communities with different housing prices can acquire equal and adequate quality health care services within and across 361 cities in China. Our findings support previous conclusions that quality hospitals are concentrated in cities with high administrative rankings and developmental levels. However, after accounting for the population size an “accessible” hospital serves, we discern “pro-poor” inequality in accessibility to care (denoted as GAPSD) and that GAPSD decreases along with increases in administrative rankings of cities and in community ratings. This paper is significant for both research and policy-making. Our approach successfully reveals an “unexpected” pattern of health care inequality that has not been reported before, and our findings provide a nationwide, detailed benchmark that facilitates the assessment of health and urban policies, as well as associated policy-making.

Abstract:Urbanization is intensifying in cities, accompanied by burgeoning inequalities, notably in the healthcare sector. Limited research offers a granular examination of healthcare resource accessibility across diverse population groups. In this study, we employ census data from all 6,164 communities in Shanghai to investigate the relationship between multi-scalar spatial dimensions and the socio-economic (dis)advantage of populations concerning healthcare resource accessibility. Distinct from previous studies, this research quantifies social inequality by considering the demographics of community residents and their interactions with healthcare facilities. We elucidate the stratified spatial distribution of healthcare accessibility across various spatial scales in Shanghai, leveraging Web Application Programming Interface (API) technology and examine the pervasive social inequalities across sub-district, district, and the central urban-suburban divide, focusing on specific population subgroups including gender, age, education level, wealth, household registration, ethnicity, occupation, and housing living facilities. Findings reveal that disparities in healthcare accessibility correlate with the quality of healthcare facilities, the socio-spatial attributes of localities, and the socio-economic and demographic profiles of the populations. The insights from this research will aid policymakers in enhancing the distribution and provision of healthcare resources, thereby promoting spatial and social equity in public healthcare system planning and implementation.

Abstract:Accessibility is one of the crucial criteria for measuring the ease of access to public services in a region. Given China's historical rural–urban dualism and rapid urbanization process, there exist significant disparities in public services within metropolises. This study selects Nanjing as a representative metropolis and employs the Gaussian two-step floating catchment area method and an improved potential model to calculate the accessibility of comprehensive hospitals. Spatial autocorrelation and urban–rural disparities are analyzed, confirming spatial variations in healthcare service equity. The results show that: 1 The spatial variability of accessibility to comprehensive hospitals is significant. The Gaussian two-step floating catchment method overestimates overall accessibility, and for Nanjing, the improved potential model with β = 1.5 proves more suitable. 2 Accessibility exhibits pronounced clustering characteristics. Healthcare conditions for residents in the northern part of Liuhe District, eastern part of Qixia District, western part of Pukou District, peripheral areas of Jiangning District, eastern part of Gaochun District, and residents in Lishui District need improvement. 3 Comprehensive healthcare services are relatively lacking in nearly 60% of rural areas. Our research findings provide valuable insights for improving spatial justice in public infrastructure in metropolises of developing countries.

Inequalities of spatial primary healthcare accessibility in China

Abstract:A key step to the establishment of a tiered healthcare system is equitable access to basic primary healthcare services for all. However, no quantitative research on the national status quo of primary healthcare accessibility in China exists. We filled this gap by estimating spatial accessibility to primary healthcare centers (PHCs) and mapping its inequality across the mainland China. Four national datasets during 2015–2018, including administrative boundaries, residential communities, points-of-interest (including PHCs), and road networks, were collected to calculate the distance to the nearest PHC for each community. Five other national datasets including census, elevation, land use, vegetation, and nightlight, were collected to model 100m × 100 m population grids, based on which geographical modeling was used to calculate PHC accessibility of each community. Inequalities in PHC accessibility across China were described with concentration indices. About 44% of communities across China representing approximately 30% of the overall population had no access to PHCs within their 6-km catchment areas; about 78% of communities across China representing approximately 68.4% of the overall population had no access to PHCs within their 1.5-km catchment areas. Some municipalities/provinces like Shanghai, Beijing, Tianjin, Jiangsu, Shandong, and Zhejiang generally had higher proximity to the nearest PHCs, while others like Tibet, Guizhou, and Guangxi had lower proximity to the nearest PHCs. However, assuming similar basic service capacity across all PHCs, Shanghai, Tianjin, and Chongqing showed the lowest PHC accessibility due to high population density. Variations in PHC accessibility existed, with more inequalities observed in the north and northeastern provinces and less inequalities in southwestern and south-central provinces. This study demonstrates primary healthcare accessibility and inequality at province and city levels, and identifies communities with lower proximity and accessibility to PHCs in China. It would serve as a starting point to facilitate precise healthcare planning and preparedness for health emergencies in China.

Abstract:Introduction: Population aging raises many fundamental questions for public health policymakers, one of which is how to address the inequity of medical services for older adults. Geographic information systems (GIS)-based accessibility has been widely employed to measure the inequity of access to healthcare facilities. However, the focus in literature lies generally on spatial patterns; few accessibility studies have focused on temporal inequity. This study focuses on the disparity of the elderly's healthcare inequity (EHI) in both spatial and temporal dimensions. Methods: EHI is measured by the elderly's medical services accessibility (EMSA) scores. Based on a road network analysis with real-time traffic data acquired by a Python-based web crawler, a Gaussian two-step floating catchment area (2SFCA) method is employed to calculate EMSA scores. Sensitivity analysis is presented for three threshold times (i.e., 5, 15, and 30 min) and different intervals of a time of day (TOD) period. Results: In the temporal patterns, the average EMSA (EMSA-avg) scores of daily variations vary from 0.0262 to 0.0556 at t(0) = 5 min, from 0.0704 to 0.1073 at t(0) = 15 min, and from 0.1734 to 0.2223 at t(0) = 30 min. Meanwhile, three "low value intervals (LVIs)" exist in TOD period. In the spatial patterns, the areas with frequent traffic congestions or no nearby hospitals are characterized by low-level accessibility for the 5 min catchment, while the same level areas for the 30 min are far from the city centre and equipped with inconvenient public transportation. Conclusions: The findings may help the policymakers in planning the medical service resources for the growing aging population and providing caregivers information for a timely and effective medical treatment.

Abstract:The water vapour transmission properties of selected hydrophilic, lipophilic, and mixed hydrophilic‐lipophilic polymer systems, cast as free films, have been examined as a function of time, film thickness, plasticiser concentration, and film formulation. An inverse linear relationship was established between the logarithm of the water vapour transmission rate (Rwvt) and the logarithm of film thickness in the presence of the various plasticiser concentrations for the three systems studied. The lipophilic n‐butyl methacrylate films were found to be less permeable to moisture than either the hydrophilic hydroxypropyl cellulose or mixed methyl hydroxypropyl cellulose‐ethyl cellulose films. The butyl methacrylate films closely follow Fickian diffusion, whereas the permeability constants of the other two films increased linearly with film thickness over the range of thicknesses studied. These phenomena are discussed with respect to the relative attractions these films have for water vapour.

Abstract:In the modern metropolis, the coverage of community hospitals is getting larger and larger. But high-level hospitals treating serious and sudden illness are still kind of scarce social resources. Shenzhen is the forefront of China's reform and opening up, its solution to the problem of medical allocation can be used as a reference for other cities. In this paper, we assume that primary, secondary and tertiary hospitals are main places for treating major diseases in Shenzhen, and analyze each sub-districts' spatial accessibility to them based on travel time and spatial difference within the city. A kernel density two-step floating catchment area method (KD2SFCA) is used to calculate how many medical resources each sub-district could share, in which the travel time from residential district to hospitals is used as an important parameter in evaluating the accessibility between suppliers and demanders. According to statistics of travel time by both driving and public transportation, the impedance function in KD2SFCA is modified and actual data of Shenzhen is used to fit its parameters, which could better simulate the attenuation trend of hospital service capabilities over the travel time. The spatial accessibilities are calculated under different travel modes and multiple time thresholds, and then spatial autocorrelation analysis method is used to analyze the spatial correlation of residents' accessibility to these medical resources. From both analysis of spatial accessibility and spatial autocorrelation, distinct variations in spatial distribution of high-level hospitals could be observed: of which the south and central parts of Shenzhen have significantly higher accessibilities, while the eastern and western regions are relatively lower. In particular, 12 sub-districts gain quite lower scores than others, showing that constructions on high-level hospitals should be strengthened further. In conclusion, the spatial configuration of high level hospitals in Shenzhen is not well balanced, Further optimization is urgently needed.

Abstract:The unbalanced allocation of healthcare resources is a major challenge that hinders access to healthcare. Taking Shenzhen as an example, this study aimed to enhance equity in obtaining healthcare services, through measuring and visualizing the spatial accessibility of community healthcare centers (CHC), and optimizing CHC geospatial allocation. We used the number of health technicians per 10,000 to represent the CHC's service capacity, combined with resident points and census data to calculate the population the CHC needs to carry, and then analyzed the accessibility based on the Gaussian two‐step floating catchment area method. In 2020, five regions in Shenzhen had better spatial accessibility scores: Nanshan (0.250), Luohu (0.246), Futian (0.244), Dapeng (0.226), and Yantian (0.196). The spatial accessibility of CHCs shows a gradual decrease from the city center to the edge, which is affected by economic and topographic factors. With the support of the maximal covering location problem model, we selected up to 567 candidate locations for the new CHC, which could improve Shenzhen's accessibility score from 0.189 to 0.361 and increase the coverage population by 63.46% within a 15‐min impedance. By introducing spatial techniques and maps, this study provides (a) new evidence for promoting equitable access to primary healthcare services in Shenzhen and (b) a foundation for improving the accessibility of public service facilities in other areas.

Abstract:Accessibility equity is a critical factor in developing equitable and sustainable communities. Although numerous studies have analyzed accessibility inequity, few of them examined inter-group and intra-group inequity and made comparisons between private cars and public transit. In this paper, a Gaussian-based Two-step Floating Catchment Area method was applied to evaluate the spatial accessibility of multitier healthcare services among different income groups in Shanghai, China. The results show that high-income households have better access to healthcare services than low-income households. The gap between low- and high-income groups increases as the level of healthcare services rises. The Theil index was used to assess inequity between groups and within groups, as well as overall inequity. The inter-group inequity for public transit is greater than that for private cars, suggesting that accessibility by public transit is more unequally distributed across groups of people compared with private cars. Inter-group inequity increases with the tier of healthcare services. The bivariate local Moran index shows that underserved areas are found primarily in low-income neighborhoods. Overall, the research findings indicate the quality of public transit services might have a more significant impact on accessibility inequity than the spatial distribution of health services. This study provides valuable information for designing targeted policy provisions to improve equity and efficiency in healthcare access.

Abstract:Medical facilities help to ensure a higher quality of life and improve social welfare. The spatial accessibility determines the allocation fairness and efficiency of medical facilities. It also provides information about medical services that residents can share. Although critical, scholars often overlooked the level of medical facilities, the composition of integrated transportation networks, and the size of service catchment in the literature on accessibility. This study aims to fill this research gap by considering the integrated transportation network, population scale, travel impedance between medical facilities and residential areas, and the impact of medical facilities’ levels on residents’ medical choices. An improved potential model was constructed to analyze the spatial accessibility of medical facilities in Changning District of Shanghai, China. Interpolation analysis was conducted to reveal the spatial accessibility pattern. Cluster and outlier analysis and Getis-Ord Gi* analysis were applied for the cluster analysis. Results show that the spatial accessibility of medical facilities is quite different in different residential areas of Changning District, Shanghai. Among them, the spatial accessibility of medical facilities is relatively high in Hongqiao subdistrict, Xinjing Town, and part of Xinhua Road subdistrict. In addition, residents have overall better access to secondary hospitals than to primary and tertiary hospitals in the study area. This study provides a spatial decision support system for urban planners and policymakers regarding improving the accessibility of healthcare facilities. It extends the literature on spatial planning of public facilities and could facilitate scientific decision making.

Abstract:Objective: Primary health care (PHC) is widely perceived to be the backbone of health care systems. Since the outbreak of COVID-19, PHC has not only provided primary medical services, but also served as a grassroots network for public health. Our research explored the accessibility, availability, and affordability of primary health care from a spatial perspective, to understand the social determinants affecting access to it in Hong Kong. Method: This constitutes a descriptive study from the perspective of spatial analysis. The nearest neighbor method was used to measure the geographic accessibility of PHC based on the road network. The 2SFCA method was used to measure spatial availability and affordability to primary health care, while the SARAR model, Spatial Error model, and Spatial Lag model were then constructed to explain potential factors influencing accessibility and availability of PHC. Results: In terms of accessibility, 95% of residents in Hong Kong can reach a PHC institution within 15 minutes; in terms of availability, 83% of residents can receive PHC service within a month; while in terms of affordability, only 32% of residents can afford PHC services with the support of medical insurance and medical voucher. In Hong Kong, education status and household income show a significant impact on accessibility and availability of PHC. Regions with higher concentrations of residents with post-secondary education receive more PHC resources, while regions with higher concentrations of high-income households show poorer accessibility and poorer availability to PHC. Conclusion: The good accessibility and availability of primary health care reflects that the network layout of existing PHC systems in Hong Kong is reasonable and can meet the needs of most residents. No serious gap between social groups further shows equality in resource allocation of PHC in Hong Kong. However, affordability of PHC is not ideal. Indeed, narrowing the gap between availability and affordability is key to fully utilizing the capacity of the PHC system in Hong Kong. The private sector plays an important role in this, but the low coverage of medical insurance in outpatient services exacerbates the crowding of public PHC and underutilization of private PHC. We suggest diverting patients from public to private institutions through medical insurance, medical vouchers, or other ways, to relieve the pressure on the public health system and make full use of existing primary health care in Hong Kong.
Identifying temporal variations in accessibility inequity to healthcare services across different travel strategies

Abstract:Introduction Although many studies have addressed accessibility inequity of healthcare services, little research has examined differences in accessibility and its spatiotemporal disparities across different travelling strategies at different times of a day. However, a static accessibility healthcare measure at one point in time may inadequately reflect the true equity of medical treatment over different time periods, often leading to over- or under-estimates and then possible bias in the results. Methods This study presents a spatial and temporal analysis of accessibility to high-level healthcare services accounting for driving, PT with- and without-metro strategies in Dalian, China, using the 3SFCA method. Then, several metrics such as the Gini index and Palma ratio are used to evaluate how accessibility inequity changes over time and varies across different transport modes. In addition, accessibility desert and spillover are proposed to capture extremely over- or under-served areas of healthcare services. Results The integration of metro can effectively help narrowing the accessibility gap between urban and suburban areas, regardless of time periods, while the accessibility inequity for driving during morning peak hours is the highest among all the nine examined scenarios. The accessibility disparities between the two travelling strategies within PT was statistically significant. Accessibility desert and spillover areas over time are also identified, which suggests the existence of different inequity patterns of extreme accessibility across spaces and time periods. Conclusions The variability of traffic conditions at different times of the day leads to variations in travel times, which in turn affects accessibility during different time periods across different transport modes. The results contribute to a better understanding of how spatial differences in healthcare accessibility are associated with the integration of travel strategies and temporal dynamics, and showing how to capture areas with extreme good/bad accessibility from an equity-focused time geography framework.

Spatial Difference and Equity Analysis for Accessibility to Three-Level Medical Services Based on Actual Medical Behavior in Shaanxi, China

Abstract:The contradiction between the supply and demand of public medical resources in China is serious. On the basis of the “graded diagnosis and treatment” model, the Chinese government divides the medical grade and adjusts the allocation of medical facilities so as to alleviate the adverse impact of these issues on residents’ health. Although the government tries to guide residents’ medical treatment according to the level of medical facilities, there are differences between residents’ medical treatment mode and policy rules in reality. Therefore, it is of great significance to explore spatial differences in accessibility to medical services for residents on the basis of the actual medical behavior. This article takes Shaanxi province as the research area, and uses the improved node cost network analysis method with the space-time distance model and the two-step floating catchment area method, respectively, to analyze the spatial differences of accessibility to three-level medical services and evaluate the equity of accessibility in different areas and groups in Shaanxi. Results showed that the overall level of accessibility to primary medical services in the province is good, and spatial distribution is balanced; the polarization of accessibility to secondary and tertiary medical services is a serious issue, and within the research area, a band-shaped multicore spatial structure was formed with the built-up areas of various cities as high-level centers of accessibility. Provincial residents have poor equity to access three-level medical services, and the equity of accessibility to primary medical services is better than that to highly specialized medical services. There is no obvious gap between accessibility to three-level medical services for the aging and the nonaging populations in Shaanxi, but the unfair phenomenon between agricultural and the nonagricultural populations is prominent. In addition, this article found that the improvement in traffic conditions can produce space-time convergence and effectively weaken spatial deprivation. Therefore, developing public transportation is an effective approach to improve the equity of accessibility to medical services.

Abstract:Acute myocardial infarction is a critical type of cardiovascular disease with rapid onset and high mortality rate. Without timely medical treatment, patients may suffer from complete necrosis of myocardium and eventually lead to death due to inadequate blood vessel clearance. Early Percutaneous Coronary Intervention surgery after acute myocardial infarction can greatly reduce patient mortality rate. However, according to monitoring data, 12.3% of heart disease patients in Beijing die on their way to hospitals. Therefore, accurately measuring the accessibility of medical treatment for acute myocardial infarction, assessing medical fairness, and optimizing the allocation of medical resources are of great significance to ensure the people’s health and life.This study takes the acute myocardial infarction patient treatment as a simulation scenario, and explores methods of accurately measuring the accessibility of medical facilities by combining new data and new methods based on map Web API. The medical accessibility of residential communities in Beijing at different times of the day is accurately measured by combining big data on population. Then, using the geographic detector model, the relations between Social Economic Status and acute myocardial infarction medical accessibility is analyzed, and the fairness of medical treatment from the perspective of spatial differentiation is assessed. It identifies the deprived groups and regions in the medical treatment space,and provides research references for optimizing the layout of medical facilities.This study has three main findings: 1) the overall medical accessibility of residential areas in Beijing is good. When patients suffer from acute myocardial infarction, they can reach the nearest hospital capable of performing Percutaneous Coronary Intervention surgery within 120 minutes of the golden treatment time.However, at different times, medical accessibility shows significant differences. Medical accessibility during the rush hour（7:00-9:00） is the worst, while medical accessibility at 0:00-7:00 is the best. In addition, the urban medical treatment spatial pattern has obvious spatial differentiation and aggregation characteristics.The accessibility in the peripheral areas is significantly different from that in the central urban area, where the accessibility in the area within the Fifth Ring Road is far better than that outside the Fifth Ring Road. Some areas in the southwest and north of Beijing have poor accessibility. Furthermore, the population within the Fifth Ring Road has more medical resources than that outside the Fifth Ring Road, while it is the opposite for the access to medical resources. There is a certain degree of mismatch between the population and medical resources inside and outside the Fifth Ring Road. 2) The layout of medical resources is somewhat unfair,and spatial deprivation of deprived groups appears. Specifically, the higher the average years of education and family income, the better the medical accessibility, while the longer it takes for people engaged in blue-collar work to reach medical facilities. Among these factors, the average years of education has the strongest explanatory power, and both the average years of education and family income have a double-factor enhancement effect on medical accessibility. This indicates that high-educated groups tend to value living quality and the supply of medical services more, and they often have higher income and are more able to choose high-quality residential areas. 3) Rush hour traffic congestion has similar spatial patterns, but affects different groups. Traffic congestion during rush hour significantly increases the medical treatment time of residential areas in the north of Haidian District, the south of Changping District, and the south of Chaoyang District. The two rush hours have similar spatial characteristics, with rush hour traffic congestion being more severe during the morning rush hour. However, the residential areas where medical accessibility is prolonged during the morning rush hour have higher average family income and education level and lower proportion of blue-collar workers than the overall average, while the residential areas affected during the evening rush hour have lower average family income and education level and similar proportion of blue-collar workers to the overall average, indicating that the affected population during different rush hours has characteristics different socioeconomic conditions.The problems found in this study can provide decision-making references for the spatial optimization of medical facilities and the alleviation of road congestion, and can also support the optimization of the process of sending acute myocardial infarction patients to hospitals, and provide reference for the treatment of similar critical illnesses. In addition, special attention should be paid to the issue of medical fairness for low-educated individuals, low-income individuals, and blue-collar workers. By accelerating urban planning to improve the supply level of medical facilities and other public services in residential areas of these groups and the external traffic environment, social equity can be promoted.
Measuring the healthcare spatial deprivation in multiple perspectives: a case study of Ningbo city

Abstract:Background: Equalization of medical services is fundamental to the development of people-centered urbanization in Chinese-style modernization. In the past few decades, the achievements of the Chinese economy have remarkably increased the quantity and quality of healthcare services. Under rapid urbanization, large-scale population floating has led to a mismatch between supply and demand for healthcare services and raised the tension between spatial demographic reorganization and the relative stability of public healthcare service facilities. The current studies of healthcare spatial deprivation (HSD) mainly are focus on either supply, or demand, or accessibility based on census data. Therefore, it is necessary to build a multiple index that could give a physical result by using ordinary indices and mixing them together by relative methods to overcome this problem. Measure: We chose Ningbo city, Zhejiang province, located in the eastern coastal region of China, as the study area. Moreover, from 2000 to 2020, the urbanization rate of Ningbo rapidly increased from 55.75 to 78.0%. In order to show the HSD in a rapidly urbanizing city, we first consider the subdistrict as the scale, innovatively absolve the medical accessibility to the IRD (Index of Relative Disadvantage), and construct the Index of Healthcare Relative Spatial Deprivation (IHRSD) framework. Based on the seventh national census data, we apply an IHRSD with Entropy Weight Method, 2SFCA (Two-step Floating Catchment Area Method) and GDM (Geographical Detector Model) to measure where and who are vulnerable to deprive in healthcare. Result: Measured by IHRSD, (1) There is stronger healthcare spatial deprivation in peri-urban and developed-town in Ningbo; (2) Young childhood and fertile-women are vulnerable to healthcare spatial deprivation, in fact, they are spatially deprived in healthcare; (3) The socio-demographic attribute is a significant factor in healthcare spatial deprivation, especially the level of aging and population clustering; and (4) The relationship between healthcare spatial deprivation and accessibility shows an inverted U -shaped structure. Conclusion: Our experiments show that the problems of HSD are mainly from the characteristics of the population, the layout of healthcare service institutions, the accessibility of the traffic system and the natural conditions. Although the constructions of equalization of primary medical and health services, hierarchical diagnosis, and treatment, and the "healthcare complex" are currently trying to reduce the phenomenon of HSD. Faced with groups and location, the acquisition of the accurately healthcare service supply is the key to realizing healthcare spatial equity.
