Abstract:Background and aim: Melasma (ML), naevus fusco-caeruleus zygomaticus (NZ), freckles (FC), cafe-au-lait spots (CS), nevus of ota (NO), and lentigo simplex (LS), are common skin diseases causing hyperpigmentation. Deep learning algorithms learn the inherent laws and representation levels of sample data and can analyze the internal details of the image and classify it objectively to be used for image diagnosis. However, deep learning algorithms that can assist clinicians in diagnosing skin hyperpigmentation conditions are lacking. Methods: The optimal deep-learning image recognition algorithm was explored for the auxiliary diagnosis of hyperpigmented skin disease. Pretrained models, such as VGG-19, GoogLeNet, InceptionV3, ResNet50V2, ResNet101V2, ResNet152V2, InceptionResNetV2, DesseNet201, MobileNet, and NASNetMobile were used to classify images of six common hyperpigmented skin diseases. The best deep learning algorithm for developing an online clinical diagnosis system was selected by using accuracy and area under curve (AUC) as evaluation indicators. Results: In this research, the parameters of the above-mentioned ten deep learning algorithms were 18333510, 5979702, 21815078, 23577094, 42638854, 58343942, 54345958, 18333510, 3235014, and 4276058, respectively, and their training time was 380, 162, 199, 188, 315, 511, 471, 697, 101, and 144 min respectively. The respective accuracies of the training set were 85.94%, 99.72%, 99.61%, 99.52%, 99.52%, 98.84%, 99.61%, 99.13%, 99.52%, and 99.61%. The accuracy rates of the test set data were 73.28%, 57.40%, 70.04%, 71.48%, 68.23%, 71.11%, 71.84%, 73.28%, 70.39%, and 43.68%, respectively. Finally, the areas of AUC curves were 0.93, 0.86, 0.93, 0.91, 0.91, 0.92, 0.93, 0.92, 0.93, and 0.82, respectively. Conclusions: The experimental parameters, training time, accuracy, and AUC of the above models suggest that MobileNet provides a good clinical application prospect in the auxiliary diagnosis of hyperpigmented skin.
Automatic identification of benign pigmented skin lesions from clinical images using deep convolutional neural network

Abstract:Objective: We aimed to develop a computer-aided detection (CAD) system for accurate identification of benign pigmented skin lesions (PSLs) from images captured using a digital camera or a smart phone. Methods: We collected a total of 12,836 clinical images which had been classified and location-labeled for training and validating. Four models were developed and validated; you only look once, v4 (YOLOv4), you only look once, v5 (YOLOv5), single shot multibox detector (SSD) and faster region-based convolutional neural networks (Faster R-CNN). The performance of the models was compared with three trained dermatologists, respectively. The accuracy of the best model was further tested and validated using smartphone-captured images. Results: The accuracies of YOLOv4, YOLOv5, SSD and Faster R-CNN were 0.891, 0.929, 0.852 and 0.874, respectively. The precision, sensitivity and specificity of YOLOv5 (the best model) were 0.956, 0.962 and 0.952, respectively. The accuracy of YOLOv5 model for images captured using a smart-phone was 0.905. The CAD based YOLOv5 system can potentially be used in clinical identification of PSLs. Conclusion: We developed and validated a CAD system for automatic identification of benign PSLs using digital images. This approach may be used by non-dermatologists to easily diagnose by taking a photo of skin lesion and guide on management of PSLs.
A deep learning-based hybrid artificial intelligence model for the detection and severity assessment of vitiligo lesions

Abstract:Background: We aimed to establish and validate a deep learning-based hybrid artificial intelligence (AI) model for the objective morphometric and colorimetric assessment of vitiligo lesions.Methods: Two main datasets containing curated images of vitiligo lesions from Chinese patients (Fitzpatrick skin types III or IV) were established, including one with 2,720 images for lesion localization study and the other with 1,262 images for lesion segmentation study. Besides, an additional test set containing 145 images of vitiligo lesions from other Fitzpatrick skin types (I, II, or V) was also generated. A 3-stage hybrid model was constructed. YOLO v3 (You Only Look Once, v3) architecture was trained and validated to classify and localize vitiligo lesions, with sensitivity and error rate as primary performance outcomes. Then a segmentation study comparing 3 deep convolutional neural networks (DCNNs), Pyramid Scene Parsing Network (PSPNet), UNet, and UNet++, was carried out based on the Jaccard index (JI). The architecture with the best performance was integrated into the model. Three add-on metrics, namely VAreaA, VAreaR, and VColor were finally developed to measure absolute, relative size changes and pigmentation, respectively. Agreement between the AI model and dermatologist evaluators were assessed.Results: The sensitivity of the YOLO v3 architecture to detect vitiligo lesions was 92.91% with an error rate of 14.98%. The UNet++ architecture outperformed the others in the segmentation study (JI, 0.79) and was integrated into the model. On the additional test set, however, the model achieved a lower detection sensitivity (72.41%) and a lower segmentation score (JI, 0.69). With respect to size changes, no difference was observed between the AI model, trained dermatologists (W=0.812, P<0.05), and Photoshop analysis (P=0.075, P=0.212 respectively), which all displayed good concordance.Conclusions: We developed a novel, convenient, objective, and quantitative deep learning-based hybrid model which simultaneously evaluated both morphometric and colorimetric vitiligo lesions from patients with Fitzpatrick skin types III or IV, rendering it suitable for the assessment of severity of vitiligo lesions in Asians in both clinic and research scenarios. More work is also warranted for its use in other ethnic skin groups.

Abstract:Cancerous skin lesions are one of the most common malignancies detected in humans, and if not detected at an early stage, they can lead to death. Therefore, it is crucial to have access to accurate results early on to optimize the chances of survival. Unfortunately, accurate results are typically obtained by highly trained dermatologists, who may not be accessible to many people, particularly in low-income and middle-income countries. Artificial Intelligence (AI) appears to be a potential solution to this problem, as it has proven to provide equal or even better diagnoses than healthcare professionals. This project aims to address the issue by collecting state-of-the-art techniques for image classification from various fields and implementing them. Some of these techniques include mixup, presizing, and test-time augmentation, among others. Three architectures were used for the implementation: DenseNet121, VGG16 with batch normalization, and ResNet50. The models were designed with two main purposes. First, to classify images into seven categories, including melanocytic nevus, melanoma, benign keratosis-like lesions, basal cell carcinoma, actinic keratoses and intraepithelial carcinoma, vascular lesions, and dermatofibroma. Second, to classify images into benign or malignant. The models were trained using a dataset of 8012 images, and their performance was evaluated using 2003 images. It's worth noting that this model is trained end-to-end, directly from the image to the labels, without the need for handcrafted feature extraction.

Abstract:Pigmented skin lesions are common spots or growths on the skin that originate from melanocyte cells. Skin cancer occurs as a result of the uncontrolled division of melanocyte cells. Skin cancer is common worldwide and its incidence has been increasing. Timely and accurate diagnosis of skin cancer is important in reducing deaths. Skin cancer cases are diagnosed by expert dermatologists, but the number of experts is insufficient for the population. Dermoscopy is one of the most commonly used methods for imaging skin lesions. Sometimes misdiagnosis can occur in the interpretation of dermoscopic images by human experts. Computer-aided systems help to make accurate and objective decisions in the diagnosis of skin lesions. Convolutional neural network(CNN) is a deep learning technique commonly used in the field of computer vision and is also widely used in medical image analysis In this study, a CNN model was proposed to classify seven different skin lesions in the HAM10000 dataset. The model provided a classification accuracy of 91.51%. The performance of the model was compared with similar studies in the literature and it was found that it showed higher success than most studies. The model was connected to a web application and evaluated in two phases by seven expert dermatologists. In the first phase, it was concluded that the model could diagnose skin lesions with 90.28% accuracy in practice. In the second phase, the model corrected the experts' misdiagnoses by 11.14%.

Abstract:Skin cancer is a serious public health problem, with countless deaths due to skin cancer each year. Early detection, aggressive and effective primary focus is the best treatment for skin cancer, which is important to improve patients' prognosis and reduce the death rate of the disease. However, judging skin tumors by the naked eye alone is a highly subjective factor, and the diagnosis can vary greatly even among professionally trained physicians. Clinically, skin endoscopy is a commonly used method for early diagnosis. However, the manual examination is time-consuming, laborious, and highly dependent on the clinical practice of dermatologists. In today's society, with the rapid development of information technology, the amount of information is increasing at a geometric rate, and new technologies such as cloud computing, distributed, data mining, and meta-inspiration are emerging. In this paper, we design and build a computer-aided diagnosis system for dermatoscopic images and apply meta-heuristic algorithms to image enhancement and image cutting to improve the quality of images, thus increasing the speed of diagnosis, early detection, and early treatment.

Abstract:BackgroundDue to the lower reliability of laboratory tests, skin diseases are more suitable for diagnosis with AI models. There are limited AI dermatology diagnostic models combining images and text; few of these are for Asian populations, and few cover the most common types of diseases.MethodsLeveraging a dataset sourced from Asia comprising over 200,000 images and 220,000 medical records, we explored a deep learning-based system for Dual-channel images and extracted text for the diagnosis of skin diseases model DIET-AI to diagnose 31 skin diseases, which covers the majority of common skin diseases. From 1 September to 1 December 2021, we prospectively collected images from 6,043 cases and medical records from 15 hospitals in seven provinces in China. Then the performance of DIET-AI was compared with that of six doctors of different seniorities in the clinical dataset.ResultsThe average performance of DIET-AI in 31 diseases was not less than that of all the doctors of different seniorities. By comparing the area under the curve, sensitivity, and specificity, we demonstrate that the DIET-AI model is effective in clinical scenarios. In addition, medical records affect the performance of DIET-AI and physicians to varying degrees.ConclusionThis is the largest dermatological dataset for the Chinese demographic. For the first time, we built a Dual-channel image classification model on a non-cancer dermatitis dataset with both images and medical records and achieved comparable diagnostic performance to senior doctors about common skin diseases. It provides references for exploring the feasibility and performance evaluation of DIET-AI in clinical use afterward.

Abstract:<p>Skin cancer is among the 10 most common cancers. Recent research revealed the superiority of artificial intelligence (AI) over dermatologists to diagnose skin cancer from predesignated and cropped images. However, there remain several uncertainties for AI in diagnosing skin cancers, including lack of testing for consistency, lack of pathological proof or ambiguous comparisons. Hence, to develop a reliable, feasible and user‐friendly platform to facilitate the automatic diagnostic algorithm is important. The aim of this study was to build a light‐weight skin cancer classification model based on deep learning methods for aiding first‐line medical care. The developed model can be deployed on cloud platforms as well as mobile devices for remote diagnostic applications. We reviewed the medical records and clinical images of patients who received a histological diagnosis of basal cell carcinoma, squamous cell carcinoma, melanoma, seborrheic keratosis and melanocytic nevus in 2006–2017 in the Department of Dermatology in Kaohsiung Chang Gung Memorial Hospital (KCGMH). We used the deep learning models to identify skin cancers and benign skin tumors in the manner of binary classification and multi‐class classification in the KCGMH and HAM10000 datasets to construct a skin cancer classification model. The accuracy reached 89.5% for the binary classifications (benign vs malignant) in the KCGMH dataset; the accuracy was 85.8% in the HAM10000 dataset in seven‐class classification and 72.1% in the KCGMH dataset in five‐class classification. Our results demonstrate that our skin cancer classification model based on deep learning methods is a highly promising aid for the clinical diagnosis and early identification of skin cancers and benign tumors.</p>
A Deep Learning Based Framework for Diagnosing Multiple Skin Diseases in a Clinical Environment

Abstract:Background: Numerous studies have attempted to apply artificial intelligence (AI) in the dermatological field, mainly on the classification and segmentation of various dermatoses. However, researches under real clinical settings are scarce. Objectives: This study was aimed to construct a novel framework based on deep learning trained by a dataset that represented the real clinical environment in a tertiary class hospital in China, for better adaptation of the AI application in clinical practice among Asian patients. Methods: Our dataset was composed of 13,603 dermatologist-labeled dermoscopic images, containing 14 categories of diseases, namely lichen planus (LP), rosacea (Rosa), viral warts (VW), acne vulgaris (AV), keloid and hypertrophic scar (KAHS), eczema and dermatitis (EAD), dermatofibroma (DF), seborrheic dermatitis (SD), seborrheic keratosis (SK), melanocytic nevus (MN), hemangioma (Hem), psoriasis (Pso), port wine stain (PWS), and basal cell carcinoma (BCC). In this study, we applied Google's EfficientNet-b4 with pre-trained weights on ImageNet as the backbone of our CNN architecture. The final fully-connected classification layer was replaced with 14 output neurons. We added seven auxiliary classifiers to each of the intermediate layer groups. The modified model was retrained with our dataset and implemented using Pytorch. We constructed saliency maps to visualize our network's attention area of input images for its prediction. To explore the visual characteristics of different clinical classes, we also examined the internal image features learned by the proposed framework using t-SNE (t-distributed Stochastic Neighbor Embedding). Results: Test results showed that the proposed framework achieved a high level of classification performance with an overall accuracy of 0.948, a sensitivity of 0.934 and a specificity of 0.950. We also compared the performance of our algorithm with three most widely used CNN models which showed our model outperformed existing models with the highest area under curve (AUC) of 0.985. We further compared this model with 280 board-certificated dermatologists, and results showed a comparable performance level in an 8-class diagnostic task. Conclusions: The proposed framework retrained by the dataset that represented the real clinical environment in our department could accurately classify most common dermatoses that we encountered during outpatient practice including infectious and inflammatory dermatoses, benign and malignant cutaneous tumors.

Abstract:The surge in developing deep learning models for diagnosing skin lesions through image analysis is notable, yet their clinical black faces challenges. Current dermatology AI models have limitations: limited number of possible diagnostic outputs, lack of real-world testing on uncommon skin lesions, inability to detect out-of-distribution images, and over-reliance on dermoscopic images. To address these, we present an All-In-One \textbf{H}ierarchical-\textbf{O}ut of Distribution-\textbf{C}linical Triage (HOT) model. For a clinical image, our model generates three outputs: a hierarchical prediction, an alert for out-of-distribution images, and a recommendation for dermoscopy if clinical image alone is insufficient for diagnosis. When the recommendation is pursued, it integrates both clinical and dermoscopic images to deliver final diagnosis. Extensive experiments on a representative cutaneous lesion dataset demonstrate the effectiveness and synergy of each component within our framework. Our versatile model provides valuable decision support for lesion diagnosis and sets a promising precedent for medical AI applications.

Abstract:Pigmented skin disease is caused by abnormal melanocyte and melanin production, which can be induced by genetic and environmental factors. It is also common among the various types of skin diseases. The timely and accurate diagnosis of pigmented skin disease is important for reducing mortality. Patients with pigmented dermatosis are generally diagnosed by a dermatologist through dermatoscopy. However, due to the current shortage of experts, this approach cannot meet the needs of the population, so a computer-aided system would help to diagnose skin lesions in remote areas containing insufficient experts. This paper proposes an algorithm based on a fusion network for the detection of pigmented skin disease. First, we preprocess the images in the acquired dataset, and then we perform image flipping and image style transfer to augment the images to alleviate the imbalance between the various categories in the dataset. Finally, two feature-level fusion optimization schemes based on deep features are compared with a classifier-level fusion scheme based on a classification layer to effectively determine the best fusion strategy for satisfying the pigmented skin disease detection requirements. Gradient-weighted Class Activation Mapping (Grad_CAM) and Grad_CAM++ are used for visualization purposes to verify the effectiveness of the proposed fusion network. The results show that compared with those of the traditional detection algorithm for pigmented skin disease, the accuracy and Area Under Curve (AUC) of the method in this paper reach 92.1 and 95.3%, respectively. The evaluation indices are greatly improved, proving the adaptability and accuracy of the proposed method. The proposed method can assist clinicians in screening and diagnosing pigmented skin disease and is suitable for real-world applications.

[Deep learning-based fully automated intelligent and precise diagnosis for melanocytic lesions]

Abstract:Melanocytic lesions occur on the surface of the skin, in which the malignant type is melanoma with a high fatality rate, seriously endangering human health. The histopathological analysis is the gold standard for diagnosis of melanocytic lesions. In this study, a fully automated intelligent diagnosis method based on deep learning was proposed to classify the pathological whole slide images (WSI) of melanocytic lesions. Firstly, the color normalization based on CycleGAN neural network was performed on multi-center pathological WSI; Secondly, ResNet-152 neural network-based deep convolutional network prediction model was built using 745 WSI; Then, a decision fusion model was cascaded, which calculates the average prediction probability of each WSI; Finally, the diagnostic performance of the proposed method was verified by internal and external test sets containing 182 and 54 WSI, respectively. Experimental results showed that the overall diagnostic accuracy of the proposed method reached 94.12% in the internal test set and exceeded 90% in the external test set. Furthermore, the color normalization method adopted was superior to the traditional color statistics-based and staining separation-based methods in terms of structure preservation and artifact suppression. The results demonstrate that the proposed method can achieve high precision and strong robustness in pathological WSI classification of melanocytic lesions, which has the potential in promoting the clinical application of computer-aided pathological diagnosis.
Deep learning-based, computer-aided classifier developed with dermoscopic images shows comparable performance to 164 dermatologists in cutaneous disease diagnosis in the Chinese population

Abstract:<span>Background Diagnoses of Skin diseases are frequently delayed in China due to lack of dermatologists. A deep learning-based diagnosis supporting system can facilitate pre-screening patients to prioritize dermatologists' efforts. We aimed to evaluate the classification sensitivity and specificity of deep learning models to classify skin tumors and psoriasis for Chinese population with a modest number of dermoscopic images.Methods We developed a convolutional neural network (CNN) based on two datasets from a consecutive series of patients who underwent the dermoscopy in the clinic of the Department of Dermatology, Peking Union Medical College Hospital, between 2016 and 2018, prospectively. In order to evaluate the feasibility of the algorithm, we used two datasets. Dataset I consisted of 7192 dermoscopic images for a multi-class model to differentiate three most common skin tumors and other diseases. Dataset II consisted of 3115 dermoscopic images for a two-class model to classify psoriasis from other inflammatory diseases. We compared the performance of CNN with 164 dermatologists in a reader study with 130 dermoscopic images. The experts' consensus was used as the reference standard except for the cases of basal cell carcinoma (BCC), which were all confirmed by histopathology.Results The accuracies of multi-class and two-class models were 81.49% ± 0.88% and 77.02% ± 1.81%, respectively. In the reader study, for the multi-class tasks, the diagnosis sensitivity and specificity of 164 dermatologists were 0.770 and 0.962 for BCC, 0.807 and 0.897 for melanocytic nevus, 0.624 and 0.976 for seborrheic keratosis, 0.939 and 0.875 for the "others" group, respectively; the diagnosis sensitivity and specificity of multi-class CNN were 0.800 and 1.000 for BCC, 0.800 and 0.840 for melanocytic nevus, 0.850 and 0.940 for seborrheic keratosis, 0.750 and 0.940 for the "others" group, respectively. For the two-class tasks, the sensitivity and specificity of dermatologists and CNN for classifying psoriasis were 0.872 and 0.838, 1.000 and 0.605, respectively. Both the dermatologists and CNN achieved at least moderate consistency with the reference standard, and there was no significant difference in Kappa coefficients between them (P > 0.05).Conclusions The performance of CNN developed with relatively modest number of dermoscopic images of skin tumors and psoriasis for Chinese population is comparable with 164 dermatologists. These two models could be used for screening in patients suspected with skin tumors and psoriasis respectively in primary care hospital.</span>

Abstract:Skin cancer is one of the most lethal kinds of human illness. In the present state of the health care system, skin cancer identification is a time-consuming procedure and if it is not diagnosed initially then it can be threatening to human life. To attain a high prospect of complete recovery, early detection of skin cancer is crucial. In the last several years, the application of deep learning (DL) algorithms for the detection of skin cancer has grown in popularity. Based on a DL model, this work intended to build a multi-classification technique for diagnosing skin cancers such as melanoma (MEL), basal cell carcinoma (BCC), squamous cell carcinoma (SCC), and melanocytic nevi (MN). In this paper, we have proposed a novel model, a deep learning-based skin cancer classification network (DSCC_Net) that is based on a convolutional neural network (CNN), and evaluated it on three publicly available benchmark datasets (i.e., ISIC 2020, HAM10000, and DermIS). For the skin cancer diagnosis, the classification performance of the proposed DSCC_Net model is compared with six baseline deep networks, including ResNet-152, Vgg-16, Vgg-19, Inception-V3, EfficientNet-B0, and MobileNet. In addition, we used SMOTE Tomek to handle the minority classes issue that exists in this dataset. The proposed DSCC_Net obtained a 99.43% AUC, along with a 94.17%, accuracy, a recall of 93.76%, a precision of 94.28%, and an F1-score of 93.93% in categorizing the four distinct types of skin cancer diseases. The rates of accuracy for ResNet-152, Vgg-19, MobileNet, Vgg-16, EfficientNet-B0, and Inception-V3 are 89.32%, 91.68%, 92.51%, 91.12%, 89.46% and 91.82%, respectively. The results showed that our proposed DSCC_Net model performs better as compared to baseline models, thus offering significant support to dermatologists and health experts to diagnose skin cancer.

Abstract:PURPOSE:There are currently few specific artificial intelligence (AI) studies for Breast Imaging Reporting and Data System (BI-RADS) category 4A lesions. This study aimed to establish an AI diagnostic model of breast lesions using two-dimensional grayscale ultrasound imaging and to compare its performance with that of radiologists.METHODS:The ultrasound images of 1311 lesions were evaluated by radiologists according to the BI-RADS categories, using pathology results as reference. Two classification standards (standards 1 and 2) for benign and malignant lesions were defined and used to calculate the diagnostic performance of radiologists, altogether and individually. The breast lesion images were also used to develop an AI diagnostic model.RESULTS:The diagnostic performance of AI and that of the radiologists were compared using the area under the receiver operating characteristic curve (AUC), sensitivity, specificity, accuracy, positive predictive value (PPV), and negative predictive value (NPV). All parameters of diagnostic performance, except for sensitivity and NPV, improved with standard 2. For the 202 lesions in the test set, the diagnostic performance of the AI model had 77.0% accuracy, 82.0% sensitivity, 71.7% specificity, 79.3% PPV, 75.1% NPV, and an AUC of 0.846. When the AI model was used to analyze category 4A lesions, the PPV was 9.3%, which was better than that of the radiologists, although not significantly.CONCLUSIONS:Deep learning technology shows a good performance in classifying benign and malignant breast lesions. It may be potentially used in practice to improve diagnostic accuracy and reduce unnecessary biopsies of breast lesions.

Abstract:BACKGROUND: Artificial intelligence is advancing at an accelerated pace into clinical applications, providing opportunities for increased efficiency, improved accuracy, and cost savings through computer-aided diagnostics. Dermatopathology, with emphasis on pattern recognition, offers a unique opportunity for testing deep learning algorithms.AIMS: This study aims to determine the accuracy of deep learning algorithms to diagnose three common dermatopathology diagnoses.METHODS: Whole slide images (WSI) of previously diagnosed nodular basal cell carcinomas (BCCs), dermal nevi, and seborrheic keratoses were annotated for areas of distinct morphology. Unannotated WSIs, consisting of five distractor diagnoses of common neoplastic and inflammatory diagnoses, were included in each training set. A proprietary fully convolutional neural network was developed to train algorithms to classify test images as positive or negative relative to ground truth diagnosis.RESULTS: Artificial intelligence system accurately classified 123/124 (99.45%) BCCs (nodular), 113/114 (99.4%) dermal nevi, and 123/123 (100%) seborrheic keratoses.CONCLUSIONS: Artificial intelligence using deep learning algorithms is a potential adjunct to diagnosis and may result in improved workflow efficiencies for dermatopathologists and laboratories.

Abstract:Although there have been reports of the successful diagnosis of skin disorders using deep learning, unrealistically large clinical image datasets are required for artificial intelligence (AI) training. We created datasets of standardized nail images using a region-based convolutional neural network (R-CNN) trained to distinguish the nail from the background. We used R-CNN to generate training datasets of 49,567 images, which we then used to fine-tune the ResNet-152 and VGG-19 models. The validation datasets comprised 100 and 194 images from Inje University (B1 and B2 datasets, respectively), 125 images from Hallym University (C dataset), and 939 images from Seoul National University (D dataset). The AI (ensemble model; ResNet-152 + VGG-19 + feedforward neural networks) results showed test sensitivity/specificity/ area under the curve values of (96.0 / 94.7 / 0.98), (82.7 / 96.7 / 0.95), (92.3 / 79.3 / 0.93), (87.7 / 69.3 / 0.82) for the B1, B2, C, and D datasets. With a combination of the B1 and C datasets, the AI Youden index was significantly (p = 0.01) higher than that of 42 dermatologists doing the same assessment manually. For B1+C and B2+ D dataset combinations, almost none of the dermatologists performed as well as the AI. By training with a dataset comprising 49,567 images, we achieved a diagnostic accuracy for onychomycosis using deep learning that was superior to that of most of the dermatologists who participated in this study.

Abstract:Skin diseases pose significant challenges in China. Internet health forums offer a platform for millions of users to discuss skin diseases and share images for early intervention, leaving large amount of valuable dermatology images. However, data quality and annotation challenges limit the potential of these resources for developing diagnostic models. In this study, we proposed a deep-learning model that utilized unannotated dermatology images from diverse online sources. We adopted a contrastive learning approach to learn general representations from unlabeled images and fine-tuned the model on coarsely annotated images from Internet forums. Our model classified 22 common skin diseases. To improve annotation quality, we used a clustering method with a small set of standardized validation images. We tested the model on images collected by 33 experienced dermatologists from 15 tertiary hospitals and achieved a 45.05% top-1 accuracy, outperforming the published baseline model by 3%. Accuracy increased with additional validation images, reaching 49.64% with 50 images per category. Our model also demonstrated transferability to new tasks, such as detecting monkeypox, with a 61.76% top-1 accuracy using only 50 additional images in the training process. We also tested our model on benchmark datasets to show the generalization ability. Our findings highlight the potential of unannotated images from online forums for future dermatology applications and demonstrate the effectiveness of our model for early diagnosis and potential outbreak mitigation.

Abstract:For diagnosing melanoma, hematoxylin and eosin (H&E) stained tissue slides remains the gold standard. These images contain quantitative information in different magnifications. In the present study, we investigated whether deep convolutional neural networks can extract structural features of complex tissues directly from these massive size images in a patched way. In order to face the challenge arise from morphological diversity in histopathological slides, we built a multicenter database of 2241 digital whole-slide images from 1321 patients from 2008 to 2018. We trained both ResNet50 and Vgg19 using over 9.95 million patches by transferring learning, and test performance with two kinds of critical classifications: malignant melanomas versus benign nevi in separate and mixed magnification; and distinguish among nevi in maximum magnification. The CNNs achieves superior performance across both tasks, demonstrating an AI capable of classifying skin cancer in the analysis from histopathological images. For making the classifications reasonable, the visualization of CNN representations is furthermore used to identify cells between melanoma and nevi. Regions of interest (ROI) are also located which are significantly helpful, giving pathologists more support of correctly diagnosis.

Evaluating Machine Learning-based Skin Cancer Diagnosis

Abstract:This study evaluates the reliability of two deep learning models for skin cancer detection, focusing on their explainability and fairness. Using the HAM10000 dataset of dermatoscopic images, the research assesses two convolutional neural network architectures: a MobileNet-based model and a custom CNN model. Both models are evaluated for their ability to classify skin lesions into seven categories and to distinguish between dangerous and benign lesions. Explainability is assessed using Saliency Maps and Integrated Gradients, with results interpreted by a dermatologist. The study finds that both models generally highlight relevant features for most lesion types, although they struggle with certain classes like seborrheic keratoses and vascular lesions. Fairness is evaluated using the Equalized Odds metric across sex and skin tone groups. While both models demonstrate fairness across sex groups, they show significant disparities in false positive and false negative rates between light and dark skin tones. A Calibrated Equalized Odds postprocessing strategy is applied to mitigate these disparities, resulting in improved fairness, particularly in reducing false negative rate differences. The study concludes that while the models show promise in explainability, further development is needed to ensure fairness across different skin tones. These findings underscore the importance of rigorous evaluation of AI models in medical applications, particularly in diverse population groups.
