Abstract:Background: Immune checkpoint inhibitors (ICIs) are now a therapeutic standard in advanced non-small cell lung cancer (NSCLC), but strong predictive markers for ICIs efficacy are still lacking. We evaluated machine learning models built on simple clinical and biological data to individually predict response to ICIs. Methods: Patients with metastatic NSCLC who received ICI in second line or later were included. We collected clinical and hematological data and studied the association of this data with disease control rate (DCR), progression free survival (PFS) and overall survival (OS). Multiple machine learning (ML) algorithms were assessed for their ability to predict response. Results: Overall, 298 patients were enrolled. The overall response rate and DCR were 15.3% and 53%, respectively. Median PFS and OS were 3.3 and 11.4 months, respectively. In multivariable analysis, DCR was significantly associated with performance status (PS) and hemoglobin level (OR 0.58, p < 0.0001; OR 1.8, p < 0.001). These variables were also associated with PFS and OS and ranked top in random forest-based feature importance. Neutrophil-to-lymphocyte ratio was also associated with DCR, PFS and OS. The best ML algorithm was a random forest. It could predict DCR with satisfactory efficacy based on these three variables. Ten-fold cross-validated performances were: accuracy 0.68 ± 0.04, sensitivity 0.58 ± 0.08; specificity 0.78 ± 0.06; positive predictive value 0.70 ± 0.08; negative predictive value 0.68 ± 0.06; AUC 0.74 ± 0.03. Conclusion: Combination of simple clinical and biological data could accurately predict disease control rate at the individual level.

Abstract:Abstract Accurate prediction of drug response using machine learning (ML) remains a significant challenge in drug development as well as in personalized cancer therapy. Furthermore, there is a scarcity of rigorous external validation studies for evaluating drug response prediction models. Even when available, such validations are often confined to in-vitro studies. Orthotopic patient-derived xenograft (O-PDX) mice serve as essential preclinical models that closely replicate the human tumor microenvironment and assess the therapeutic response to cancer treatments. We present CertisAITM, a novel ensemble of machine learning models that have been trained on a vast array of experimental high-throughput screenings of both monotherapies and combination therapies, incorporating over 4500 investigational and FDA-approved drugs across 10 major cancer indications. The results of our ML drug response prediction models show an average R2 of 0.75 and RMSE of 0.62 (across internal 5-fold cross-validation) for all 10 cancer prediction models. Our evaluation of CertisAI's drug response predictions within O-PDX pharmacology studies has shown a notable overall correlation between actual observed tumor growth inhibition (TGI) and predicted TGI, with an r value of 0.45 across seven indications. The most accurate prediction model achieved an r value of 0.7 for colorectal cancer, encompassing 37 treatments in 6 O-PDX studies. Altogether, these results highlight CertisAI’s potential in enhancing pre-clinical model selection as well as personalized treatment strategies in oncology. Abstract:Immune checkpoint inhibitors (ICIs) are standard-of-care as first-line (1L) therapy for advanced non-small cell lung cancer (aNSCLC) without actionable oncogenic driver mutations. While clinical trials demonstrated benefits of ICIs over chemotherapy, variation in outcomes across patients has been observed and trial populations may not be representative of clinical practice. Predictive models can help understand heterogeneity of treatment effects, identify predictors of meaningful clinical outcomes, and may inform treatment decisions. We applied machine learning (ML)-based survival models to a real-world cohort of patients with aNSCLC who received 1L ICI therapy extracted from a US-based electronic health record database. Model performance was evaluated using metrics including concordance index (c-index), and we used explainability techniques to identify significant predictors of overall survival (OS) and progression-free survival (PFS). The ML model achieved c-indices of 0.672 and 0.612 for OS and PFS, respectively, and Kaplan–Meier survival curves showed significant differences between low- and high-risk groups for OS and PFS (both log-rank test p < 0.0001). Identified predictors were mostly consistent with the published literature and/or clinical expectations and largely overlapped for OS and PFS; Eastern Cooperative Oncology Group performance status, programmed cell death-ligand 1 expression levels, and serum albumin were among the top 5 predictors for both outcomes. Prospective and independent data set evaluation is required to confirm these results.

Abstract:e13556 Background: Immune checkpoint inhibitors (ICI) are used to manage patients with both small cell (SCLC) and non-small cell (NSCLC) lung cancer. However, ICI response rates are often low, and identifying patients that will benefit from ICIs can be challenging. The value of biomarkers used to predict ICI response, such as PD-L1, Combined Positive Score (CPS) or tumor mutational burden (TMB) have been debated. Furthermore, the resources needed to assess these biomarkers may not be available in many centres. Developing more accurate and accessible tools that predict ICI responses could enable a precision medicine approach that improves patient outcomes. This study aimed to use a novel machine-learning (ML) algorithm to predict response to different ICI therapies in patients with lung cancer based on clinically available data. Methods: 334 eligible records were cleaned and reprocessed from textual to categorical data using one hot encoding. Complete datasets were available for 161 patients. Differences in the data distribution were handled using the Synthetic Minority Oversampling Technique. Six ML algorithms were trained, including Linear regression, Support Vector Classifier, XGBoost Classifier, Random Forest, Decision Tree, and Gaussian Naive Bayes Classifier. These algorithms used 80% of the training data, were tested on 20% of validation data and used the Grid Search Cross-Validation technique for hyperparameter optimization. Results: For the 161 patients included in the final analysis, the mean age was 68 years and 48% were female. 9% of patients had SCLC and 80% had NSCLC. Patients receiving Pembrolizumab, Nivolumab and Atezolizumab comprised 62%, 11% and 25%, respectively. The artificial intelligence (AI) algorithm predicted and stratified ICI response better than PD-L1 levels. Of the ML algorithms, XGBoost Classifier predicted response with the most accuracy, 64% (0.61 F1 score). This model found that good performance status (0-1), female gender and adenocarcinoma sub-type predicted response to ICI. On the other hand, M1, N2 staging, male gender, squamous cell carcinoma sub-type and receiving Atezolizumab were predictive of disease progression. Conclusions: This study developed multiple novel ML models to predict responses to ICIs in lung cancer. XGBoost Classifier used clinically available data to show that the type of ICI a patient receives, their histopathology sub-type and their TMN staging impact ICI response. Future work will aim to improve accuracy and predict ICI toxicity by including data from multiple centres, different cancer types and additional clinical variables. [Table: see text]

Abstract:e13603 Background: Oncology therapeutic development has high failure rates with substantial costs. Advances in technology, including molecular characterisations of cancer and computational power, provide the opportunity to better model therapeutic response for existing and emerging therapies. Methods: Concr adapted Bayesian statistical principles used by astrophysicists to effectively integrate multi-dimensional data (genomic & transcriptomic) for accurate modelling of cancer biology. Our proprietary computational framework was applied to predict clinical response and survival through construction of digital twins and in silico simulations of clinical trials. Predicted log odds ratios (LOR) were generated for drug response and compared to published data from 8 suitable historical clinical trials. Three distinct models underpin this framework: Drug Efficacy, Treatment Response & Overall Survival (OS). Three publicly available datasets were used for training: Cancer Therapeutic Response Portal ( in vitro dose-response data), Cancer Cell Line Encyclopaedia and TCGA. Results: For all 8 clinical studies, the digital twin model accurately simulated both trial arms, compared drug efficacy across arms and predicted which treatment was most active. Unblinded evaluation: Our model accurately predicted gemcitabine would have greater clinical benefit than 5-FU (LOR -0.10, P <0.0001) in metastatic pancreatic cancer. In advanced breast cancer, the model predicted docetaxel had a higher response rate than doxorubicin, and carboplatin response rates were similar to docetaxel in BRCA wild-type patients. In platinum-sensitive recurrent ovarian cancer, the model predicted cisplatin-based combination had a higher response rate than paclitaxel. Blinded evaluation: Using data for paclitaxel, the model correctly predicted that nab-paclitaxel+gemcitabine response rates were higher than gemcitabine in metastatic pancreatic cancer (predicted LOR -0.090, p = CMF, adjuvant TC > AC and adjuvant tamoxifen was superior to capecitabine. The underlying pan-tumour Overall Survival model used a Random Survival Forest approach, trained on 23 cancers with predictive accuracy of 0.78 AUC ROC and C-index 0.71. To enable cohort enrichment for drug response, the model segmented patient cohorts into treatment responder and non-responder. Using this approach, the log odds increased for drug response rate for 16 of the 19 different cancers and 14 of the different 17 drugs. Conclusions: Our digital twin model represents transformative technology for clinical trial simulation with strong potential for accelerating and de-risking clinical drug development, and utility for synthetic control arms.

Abstract:Background: Machine learning (ML) in translational medicine has led to prediction of clinical outcomes and identification of new biomarkers. We employ ML in prediction of pathologic complete response (pCR) in high-risk breast cancer patients in the neoadjuvant I-SPY2 TRIAL where not all novel agents have strong predictive biomarkers. Leveraging a ML approach using progressively expanded candidate genes, we explore the limitations of using only known mechanisms of action in predicting pCR, and the extent to which biology outside known drug action improves response prediction in the first 10 arms of the trial. Methods: ML random forest models were developed in I-SPY2 patients (n=982) with pre-treatment gene expression and pCR data across 10 treatment arms (PMID: 35623341), including inhibitors of HER2: neratinib (N), pertuzumab (P), TDM1/P; AKT (MK-2206); IGF1R (ganitumab); HSP90 (ganetespib); PARP/DNA repair (veliparib/carboplatin, VC); ANG1/2 (trebananib, T); immune checkpoints (PD1-inh); and Control (Ctr). Each HR/HER2 receptor/treatment arm subset (m=27) was evaluated independently. We employed a three-pronged feature-selection approach using (1) genes restricted to known mechanism of action of individual I-SPY2 agents (k=10 to 88 genes); (2) genes expanded to include targeted pathways for all 10 agents/combinations (k=282); and (3) an unbiased whole genome approach (k=17,990). Samples were partitioned with 75% used for training and cross-validation, and 25% held out as test sets. Predictive ML models were defined as those with performance ≥ 0.90 based on different performance metrics (e.g., AUC, sensitivity, specificity). Results: For each of the 27 subtype-treatment subsets, at least one high performing model was identified. In 6 subtype-treatment subsets, mechanism of action genes were sufficient to predict pCR: AKT/PI3K/HER genes in HR+HER2- N and HR-HER2+ P; DNA repair genes in HR+HER2- VC; angiogenesis-associated genes in HR+HER2+ T; and immune-associated genes in both HR+HER2- and HR-HER2- PD1-inh subsets. Expanded targeted pathway models were required to identify predictive models in 8 additional subtype-treatment pairs from the N, T-DM1/P, MK-2206, VC, T, and HER2+ Ctr arms, with significant contribution of DNA repair, immune, and HSP90 genes for multiple arms. A genome-wide approach was required for the remaining 13 subtype-treatment pairs with no previous models from the N, P, MK-2206, ganitumab, ganetespib, T, and HER2- Ctr arms. Even for subtype-treatment pairs where mechanism of action gene sets was sufficient for reasonable models, expanded gene sets resulted in improved performance. For instance, metabolism genes improved model performance for HR-HER2+ in N and Ctr, and for HR+HER2- in the PD1-inh arm; and mitochondrial and protein folding dysfunction genes improved response prediction in HR-HER2- in the ganetespib arm. Conclusion: Our study identifies mechanism of action biomarkers associated with response to each drug and elucidates possible off-target effects contributing to observed drug sensitivity and resistance. Abstract:Existing survival prediction models rely only on baseline or tumor kinetics data and lack machine learning integration. We introduce a novel kinetics-machine learning (kML) model that integrates baseline markers, tumor kinetics and four on-treatment simple blood markers (albumin, CRP, lactate dehydrogenase and neutrophils). Developed for immune-checkpoint inhibition (ICI) in non-small cell lung cancer on three phase 2 trials (533 patients), kML was validated on the two arms of a phase 3 trial (ICI and chemotherapy, 377 and 354 patients). It outperformed the current state-of-the-art for individual predictions with a test set c-index of 0.790, a 12-months survival accuracy of 78.7% and a hazard ratio of 25.2 (95% CI: 10.4 – 61.3, < 0.0001) to identify long-term survivors. Critically, kML predicted the success of the phase 3 trial using only 25 weeks of on-study data (predicted HR = 0.814 (0.64 – 0.994) versus final study HR = 0.778 (0.65 – 0.931)). Our model constitutes a valuable approach to support personalized medicine and drug development.

Abstract:Abstract Background: Predicting the probability of tumor progression and tolerability with sufficient accuracy remains a significant challenge in advanced breast cancer. The objective of AI4ANNA study was to assess the predictive potential of machine learning (ML) methods with respect to tumor control and safety outcomes using German study data (RIBECCA, RIBANNA) and to identify the most relevant baseline factors for prediction. Methods: Anonymized study data from two studies of ribociclib and endocrine therapy in patients with advanced HR+, HER2– breast cancer were used for predictive analysis. RIBECCA (N=487) was a multicenter, open-label, single-arm, phase 3b trial, and RIBANNA (N=1904) is an ongoing non-interventional study evaluating the real-world efficacy and safety of first-line ribociclib in combination with aromatase inhibitor/fulvestrant, endocrine monotherapy or chemotherapy. Study baseline features were used to develop prediction models for a variety of tumor control (including progression-free survival (PFS), overall response rate (ORR) at week 24, death) and safety outcomes (including general number of adverse events (AEs) as well as selected AEs belonging to blood system, cardiac, hepatobiliary, and gastrointestinal disorders). LASSO (Least Absolute Shrinkage and Selection Operator) and XGBoost (eXtreme Gradient Boosting) ML algorithms were employed to train prediction models. LASSO was selected as a representative of fully interpretable, linear models and XGBoost as a representative of highly flexible, nonlinear models. Predictive performance of these two algorithms was compared and predictive value of single baseline features was assessed using feature permutation importance method. Results were validated internally within the training study (10 times repeated 5-fold stratified cross-validation) as well as externally, ie, implementation and training of the prediction models on one study and validation on the other and vice versa. Results: Moderate predictive signal (at baseline) could be identified for the following two outcomes: ORR (area under the curve [AUC] mean 0.628 [RIBANNA] and 0.626 [RIBECCA]) and PFS (AUC mean 0.626 [RIBANNA], 0.604 [RIBECCA]). Model performance could be validated with very similar AUCs by cross-study evaluation. Patients could be assigned to one of three risk groups. The most important features for ORR prediction included the presence of locally advanced cancer and metastases presenting as bone only disease and for PFS the presence of liver metastases, histological grade and prior (neo)adjuvant treatment. For three safety endpoints, a predictive signal (AUC >0.6) was identified only in one study but not in the other (AEs “QT prolongation”, “leukopenia grade 3/4” and serious AE “vomiting”). However, insufficient predictive signals were found for all other outcomes. Conclusion: Prediction models for tumor control and safety outcomes were trained on a broad number of clinical baseline features. Moderate predictive signals could be identified for ORR and PFS. Even though the predictive performance (AUC) seems to be limited, patients could be assigned to one of three differently behaving risk groups at the baseline. Abstract:A major challenge of precision oncology is the identification and prioritization of suitable treatment options based on molecular biomarkers of the considered tumor. In pursuit of this goal, large cancer cell line panels have successfully been studied to elucidate the relationship between cellular features and treatment response. Due to the high dimensionality of these datasets, machine learning (ML) is commonly used for their analysis. However, choosing a suitable algorithm and set of input features can be challenging. We performed a comprehensive benchmarking of ML methods and dimension reduction (DR) techniques for predicting drug response metrics. Using the Genomics of Drug Sensitivity in Cancer cell line panel, we trained random forests, neural networks, boosting trees and elastic nets for 179 anti-cancer compounds with feature sets derived from nine DR approaches. We compare the results regarding statistical performance, runtime and interpretability. Additionally, we provide strategies for assessing model performance compared with a simple baseline model and measuring the trade-off between models of different complexity. Lastly, we show that complex ML models benefit from using an optimized DR strategy, and that standard models—even when using considerably fewer features—can still be superior in performance.

Abstract:Introduction Machine learning (ML) techniques have gained increasing attention in the field of healthcare, including predicting outcomes in patients with lung cancer. ML has the potential to enhance prognostication in lung cancer patients and improve clinical decision-making. In this systematic review and meta-analysis, we aimed to evaluate the performance of ML models compared to logistic regression (LR) models in predicting overall survival in patients with lung cancer. Methods We followed the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) statement. A comprehensive search was conducted in Medline, Embase, and Cochrane databases using a predefined search query. Two independent reviewers screened abstracts and conflicts were resolved by a third reviewer. Inclusion and exclusion criteria were applied to select eligible studies. Risk of bias assessment was performed using predefined criteria. Data extraction was conducted using the Critical Appraisal and Data Extraction for Systematic Reviews of Prediction Modeling Studies (CHARMS) checklist. Meta-analytic analysis was performed to compare the discriminative ability of ML and LR models. Results The literature search resulted in 3,635 studies, and 12 studies with a total of 211,068 patients were included in the analysis. Six studies reported confidence intervals and were included in the meta-analysis. The performance of ML models varied across studies, with C-statistics ranging from 0.60 to 0.85. The pooled analysis showed that ML models had higher discriminative ability compared to LR models, with a weighted average C-statistic of 0.78 for ML models compared to 0.70 for LR models. Conclusion Machine learning models show promise in predicting overall survival in patients with lung cancer, with superior discriminative ability compared to logistic regression models. However, further validation and standardization of ML models are needed before their widespread implementation in clinical practice. Future research should focus on addressing the limitations of the current literature, such as potential bias and heterogeneity among studies, to improve the accuracy and generalizability of ML models for predicting outcomes in patients with lung cancer. Further research and development of ML models in this field may lead to improved patient outcomes and personalized treatment strategies.
Multimodal machine learning model prediction of “individual” response to immunotherapy in 1L stage IV NSCLC.

Abstract:e21151 Background: Immunotherapy (IO) is the standard of care in 1L stage IV non-small cell lung cancer (NSCLC) cases that are not eligible for targeted therapies. Today, a level of PD-L1 expression of 50% or above is the only standard predictive biomarker for IO efficacy as monotherapy. However, a majority of patients fail to respond to the treatment and are exposed to potentially severe immune-related adverse events. There is thus an urgent need to discover new predictive signatures of response to IO in such setting. Methods: A retrospective 1-year cohort of 63 patients with advanced NSCLC, PD-L1 expression > 50%, and treated with 1L pembrolizumab monotherapy was analyzed to develop a machine learning-based algorithm predictive of response to immunotherapy. Multimodal baseline data were collected including clinical, biological, pathology, molecular, baseline CT scan data and clinical outcomes status (objective response at first evaluation, PFS, OS). For each patient, thoracic tumors were segmented in 3D by both an experimented pneumologist and a radiologist using the SOPHiA DDM for Radiomics platform. Radiomics features were then extracted following the IBSI standards and combined with the other data modalities. A filter-based variable selection method was applied before testing several machine learning algorithms to obtain an individual prediction of the response at first evaluation. The optimization criterion was the Area Under the ROC Curve (AUC). Due to the small size of the cohort, a nested leave-pair-out cross-validation was used to properly estimate the model performance. Results: A logistic regression algorithm reached an AUC of 0.85, a precision of 83%, a sensitivity of 77% and a specificity of 74% for predicting response at first evaluation. The algorithm was able to correctly predict 21 progressions among 28 observed and 27 disease controls among 35 observed. Features with highest weight were representative of the full scope of data modalities included in the model, highlighting the importance of a truly multimodal analysis. Indeed, withdrawing any specific data modality (e.g., radiomics features), led to a decrease of at least ̃10% of the AUC. Patients were then stratified into two groups, progression versus disease control, based upon their predicted response status at first evaluation. These two groups displayed a statistically significant difference in PFS (p < 0.001), suggesting that baseline multimodal data analysis could help predict long-term outcomes. Conclusions: This proof-of-concept study suggests that machine learning applied to baseline multi-modal data can help predict response to IO at the individual patient level, as well as stratify patients to inform long-term outcomes. This algorithm is being improved and validated through a large real-world multicentric international observational study including more than 4000 patients (DEEP-Lung-IV study, NCT04994795).

Abstract:Cancer, being the second leading cause of death globally. So, the development of effective anticancer treatments is crucial in the field of medicine. Anticancer peptides (ACPs) have shown promising therapeutic potential in cancer treatment compared to traditional methods. However, the process of identifying ACPs through experimental means is often time-intensive and expensive. To overcome this issue, we employed a machine learning-based approach for the first time to develop an anticancer model using small molecules. Anticancer small molecules (ACSMs) are compounds that have been developed to target and inhibit cancer cells. In this study, we used 10,000 compounds to develop the machine learning models using five algorithms such as, Random Forest (RF), Light gradient boosting machine (LightGBM), K-nearest neighbors (KNN), Decision tree (DT) and Extreme Gradient Boosting (XGB). The developed models were evaluated using the test set and top three models were identified (RF, LightGBM and XGB). Furthermore, to validate the predictive performance of our models, we have performed external validation using an FDA approved anticancer compounds/drugs. Following this analysis, we found that our LightGBM model correctly predicted 9 compounds as active. However, RF and XGB exhibited some limitations by predicting 8 and 7 compounds as active out of 10, respectively. These results demonstrate that, when compared to RF and XGB, the LightGBM model showcase robust prediction capabilities, achieving a superior accuracy of 79% with an AUC of 0.88. These findings provide promising insights into the potential of our approach for predicting anticancer small molecules, highlighting the role of machine learning in advancing cancer treatment research.

Abstract:We report the case of an 84-year-old woman who had a nodal recurrence of melanoma 45 years after the primary diagnosis of an extremity cutaneous melanoma. It is believed to be the longest disease-free latency period reported between primary melanoma diagnosis and recurrence to date. Late recurrences of melanoma are rare and recurrence after four decades extremely rare. This article suggests melanoma is a disease with a potentially lifelong risk of recurrence and thus clinicians and patients must be vigilant and aware of this risk, particularly if late recurrences are to be recognised early and management optimised.

Abstract:Random forest model with 12 variables could be a useful tool to identify individuals who are most likely to have adverse outcomes (death, disease progression, discontinued study, or discontinued treatment) and clinical benefit from atezolizumab monotherapy. Laboratory tests, and tumor characteristics evaluation performed at baseline before starting immunotherapy may provide an important reference for weighing benefits and risks of therapy. Background Few models exist to predict mortality in cancer patients receiving immunotherapy. Our aim was to build a machine learning‐based risk stratification model for predicting mortality in atezolizumab‐treated cancer patients. Methods Data from 2538 patients in eight atezolizumab‐treated cancer clinical trials across three cancer types (non‐small‐cell lung cancer, bladder transitional cell carcinoma, and renal cell carcinoma) were included. The whole cohort was randomly split into development and validation cohorts in a 7:3 ratio. Machine‐learning algorithms (extreme gradient boosting, random forest, logistic regression with lasso regularization, support vector machine, and K‐nearest neighbor) were applied to develop prediction models. Model performance was mainly assessed by area under the receiver operating characteristic curve (AUC) value, calibration plot, and decision curve analysis. The probability of death risk was then stratified. Results One thousand and three hundred and seventy‐nine (54.33%) patients died. The random forest (RF) model was overall the best in terms of predictive performance, with the AUC of 0.844 (95% confidence interval [CI]: 0.826–0.862) in the development cohort and 0.786 (95% CI: 0.754–0.818) in the validation cohort for predicting mortality. Twelve baseline variables contributing to mortality prediction in the RF model were C‐reactive protein, PD‐L1 level, cancer type, prior liver metastasis, derived neutrophil‐to‐lymphocyte ratio, alkaline phosphatase, albumin, hemoglobin, white blood cell count, number of metastatic sites, pulse rate, and Eastern Cooperative Oncology Group (ECOG) performance status. A total of 1782 (70.2%) patients were separated into the high‐risk and 756 (29.8%) low‐risk groups. Patients in the high‐risk group were significantly more likely to die, experience disease progression, discontinue study, and discontinue treatment than patients in the low‐risk group (all p values 0.05). Conclusion RF model has good performance in mortality prediction and risk stratification for cancer patients receiving atezolizumab monotherapy.

Abstract:Personalized medicine enables precise tumor treatment for a patient's molecular genetic profile. To devise optimal targeted treatment plans for patients in a molecular tumor board, physicians must consider alterations on gene- and proteins levels but also cancer cell phenotypes. Machine learning can uncover buried patterns, extract pivotal information, and unveil corresponding insights from available data. Publicly available datasets provide the amounts of data necessary. This work outlines the efficacy of various machine learning algorithms which could eventually serve as clinical decision support in a precision oncology setting. Leveraging algorithms including Random Forest, Decision tree, XGBoost, Logistic regression, Gaussian Naive Bayes, k nearest neighbor, and AdaBoost, we conducted two experiments for the breast invasive carcinoma dataset. Incorporated data includes patient-, molecular- and treatment data. The aim of the investigation was to predict medication treatment or type of treatment based on genetic profile. After preprocessing and application of ML algorithms, the first results were promising. Multiple factors challenge application in clinical care settings without carefully considering the limitations.

Abstract:Since the mid-1980s, there has been little progress in improving survival of patients diagnosed with osteosarcoma. Survival prediction models play a key role in clinical decision-making, guiding healthcare professionals in tailoring treatment strategies based on individual patient risks. The increasing interest of the medical community in using machine learning (ML) for predicting survival has sparked an ongoing debate on the value of ML techniques versus more traditional statistical modelling (SM) approaches. This study investigates the use of SM versus ML methods in predicting overall survival (OS) using osteosarcoma data from the EURAMOS-1 clinical trial (NCT00134030). The well-established Cox proportional hazard model is compared to the extended Cox model that includes time-varying effects, and to the ML methods random survival forests and survival neural networks. The impact of eight variables on OS predictions is explored. Results are compared on different model performance metrics, variable importance, and patient-specific predictions. The article provides comprehensive insights to aid healthcare researchers in evaluating diverse survival prediction models for low-dimensional clinical data.

Abstract:592 Background: The objective of the current analysis of the I-SPY 2 data from multiple breast cancer clinical trials for stages II and III cancers was to look at the possibility of developing a machine learning algorithm to predict pathological complete response using multimodal data. Methods: Imagining, clinical and biomarkers data form the I-SPY 2 trial was used in this analysis. Four different experiments were designed for to assess different data pre-processing and machine learning methods mainly deep neural networks (DNN) and random forest (RF) classifiers. Experiment 1 and 3 use DNNs while Experiment 2 and 4 used RFs; more over the first two experiments used data that was continuous in nature from the point of view of pre-performed imaging analysis while in later two experiments we had binarized all the variables using a median cutoff for high vs low values. The variables used were treatment recieved, HER2/neu receptor status, hormone receptor status, age, ethnicity, gynecological history, MRI feature data. All models were evaluated using robust testing methods that included sensitivity, specificity, positive predictive values (PPV), negative predictive values (NPV), accuracies (training and test sets), area-under the receiver-operator curve and F1-Score. Results: From a total of 985 patients with clinical data we used 384 that had multi-feature MRI data. Time series data for the initial scan (T0) and a scan 3 weeks later (T1). The final comparative analysis showed the best model among the 4 experiment the best performing algorithms for this analysis was random forests using median cutoffs with a best F1-score (0.43), specificity (86.4%), PPV (47.8%), accuracies (train sets, 100%; test sets, 75%), and AUROCs (0.78, 0.56-0.73). Conclusions: The use of machine learning to predict pathological complete response using multimodal data shows good potential as a digital biomarker. However, these results need further validation before a clinical tool is available for the clinicians. [Table: see text]

Abstract:Abstract Purpose Risk stratification in patients with lung adenocarcinoma (LUAD) is mandatory for treatment guiding and outcome prediction. Amongst clinical parameters including histological analyses, imaging procedures provide important information. The present study aimed to investigate the ability of machine learning models trained on clinical and 2-deoxy-2-[18F]fluoro-D-glucose ([ 18 F]FDG) positron emission tomography/computed tomography (PET/CT) derived radiomic data to predict overall survival (OS), tumor grade (TG), and histologic growth pattern risk (GPR) in treatment naïve LUAD patients. Methods 421 treatment naïve patients with histologically diagnosed lung adenocarcinoma and available [ 18 F]FDG PET/CT imaging were retrospectively analyzed. Four patient cohorts were generated based on the available data for predicting 4-year OS (n = 276), 3-year OS (n = 280), TG (n = 298) and GPR (n = 265). [ 18 F]FDG-positive lesions were delineated semiautomatically, from which 2082 radiomic features were extracted and combined with endpoint-specific clinical and demographic parameters. Machine learning models were built for the prediction of 4-year OS (M 4OS ), 3-year OS (M 3OS ), tumor grading (M TG ) and histologic growth pattern risk (M GPR ), respectively. Monte Carlo (MC) cross-validation with 100-folds and 80:20 training to validation split was employed as a performance evaluation for all models. Kaplan-Meier survival analysis was performed to assess the association between the M 4OS and M 3OS predictions with OS. Results Area under the receiver operator characteristics curve (AUC) was highest for M 4OS (AUC 0.88), followed by M 3OS (AUC 0.84), while M TG and M GPR performed equally well (AUC 0.76). Predictions of M 4OS (HR 0.128, p < 0.000001) and M 3OS (HR 0.0942, p < 0.000001) were independently associated with OS. Conclusion In our retrospective cohorts, machine learning models demonstrated the ability to prognosticate long-term survival outcomes in patients with lung adenocarcinoma. Furthermore, tumor lesions could be characterized according to their histologic grade and predominant growth pattern risk with high accuracy.

Abstract:Purpose Machine learning (ML) is a strong candidate for making accurate predictions, as we can use large amount of data with powerful computational algorithms. We developed a ML based model to predict survival of patients with colorectal cancer (CRC) using data from two independent datasets.Materials and Methods A total of 364,316 and 1,572 CRC patients were included from the Surveillance, Epidemiology, and End Results (SEER) and a Korean dataset, respectively. As SEER combines data from 18 cancer registries, internal validation was done using 18-Fold-Cross-Validation then external validation was performed by testing the trained model on the Korean dataset. Performance was evaluated using area under the receiver operating characteristic curve (AUROC), sensitivity and positive predictive values.Results Clinicopathological characteristics were significantly different between the two datasets and the SEER showed a significant lower 5-year survival rate compared to the Korean dataset (60.1% vs. 75.3%, p < 0.001). The ML-based model using the Light gradient boosting algorithm achieved a better performance in predicting 5-year-survival compared to American Joint Committee on Cancer stage (AUROC, 0.804 vs. 0.736; p < 0.001). The most important features which influenced model performance were age, number of examined lymph nodes, and tumor size. Sensitivity and positive predictive values of predicting 5-year-survival for classes including dead or alive were reported as 68.14%, 77.51% and 49.88%, 88.1% respectively in the validation set. Survival probability can be checked using the web-based survival predictor (http://colorectalcancer.pythonanywhere.com).Conclusion ML-based model achieved a much better performance compared to staging in individualized estimation of survival of patients with CRC.

Abstract:Background and objective: The standard of care in Acute Myeloid Leukemia patients has remained essentially unchanged for nearly 40 years. Due to the complicated mutational patterns within and between individual patients and a lack of targeted agents for most mutational events, implementing individualized treatment for AML has proven difficult. We reanalysed the BeatAML dataset employing Machine Learning algorithms. The BeatAML project entails patients extensively characterized at the molecular and clinical levels and linked to drug sensitivity outputs. Our approach capitalizes on the molecular and clinical data provided by the BeatAML dataset to predict the ex vivo drug sensitivity for the 122 drugs evaluated by the project. Methods: We utilized ElasticNet, which produces fully interpretable models, in combination with a two-step training protocol that allowed us to narrow down computations. We automated the genes' filtering step by employing two metrics, and we evaluated all possible data combinations to identify the best training configuration settings per drug. Results: We report a Pearson correlation across all drugs of 0.36 when clinical and RNA sequencing data were combined, with the best-performing models reaching a Pearson correlation of 0.67. When we trained using the datasets in isolation, we noted that RNA Sequencing data (Pearson: 0.36) attained three times the predictive power of whole exome sequencing data (Pearson: 0.11), with clinical data falling somewhere in between (Pearson 0.26). Lastly, we present a paradigm of clinical significance. We used our models' prediction as a drug sensitivity score to rank an individual's expected response to treatment. We identified 78 patients out of 89 (88 %) that the proposed drug was more potent than the administered one based on their ex vivo drug sensitivity data. Conclusions: In conclusion, our reanalysis of the BeatAML dataset using Machine Learning algorithms demonstrates the potential for individualized treatment prediction in Acute Myeloid Leukemia patients, addressing the longstanding challenge of treatment personalization in this disease. By leveraging molecular and clinical data, our approach yields promising correlations between predicted drug sensitivity and actual responses, highlighting a significant step forward in improving therapeutic outcomes for AML patients.
