Abstract:Background Epidemiology is considered to be the fundamental science of public health and plays an important role in clinical competence and professional development. The objective of this study is to evaluate the effectiveness of a short-term course for the teaching of epidemiology, which was designed as a community-based class for medical students. Method This course was designed according to Kern’s six-step approach to curriculum development. A total of 75 undergraduates were recruited. Forty-one students were assigned to an experimental group engaged in theoretical teaching and practical courses, while 34 students were assigned to the control group only taking theoretical courses. All participants were asked to complete a pre- and post-course survey and to take a test after completing the course. The scores between the experimental and control groups were compared using the Wilcoxon test. Result The experimental group showed significantly higher self-assessment scores in course understanding ( p = 0.0126) and clinical practice skills ( p = 0.0005) after completing the course, while no significant difference was observed in the control group. In addition, students in the experimental group reported significantly higher interest ( p = 0.0015), stronger learning motivation ( p = 0.0113) and a better mastery of epidemiology ( p = 0.0167) after completing the course than those in the control group. However, test scores ( p = 0.0859) and pass rates ( p = 0.1755) demonstrated no statistical significance between the two groups. Conclusion The short-term practical course in epidemiology exerted significantly positive effects on the improvement of student learning enthusiasm, course understanding and clinical practice skills. These findings provide new ideas and statistical evidence for the development of epidemiological instruction. Future studies should explore how to more widely and optimally apply community-based courses to the teaching of epidemiology.

Abstract:Background: The journal club is a ubiquitous and time-honored tradition within medical education. However, in recent years, open educational resources (OERs) have become increasingly influential in how physicians interact with the medical literature across multiple specialties. The authors sought to explore how emergency medicine (EM) resident physicians reconcile different perspectives across OERs into their educational experience at the journal club. Methods: From January 2018 to September 2019, the authors enrolled 25 EM residents from four teaching sites associated with the University of British Columbia, Canada, to participate in either a focus group (seven residents) or individual interviews (18 residents). The authors used a snowball sampling technique. Using a constructivist grounded theory analysis, two investigators independently reviewed transcripts, meeting regularly to discuss themes until sufficiency was achieved. Results: The study data expand the theoretical understanding of the resident journal club experience. Residents used multiple sources including OERs to learn about new evidence in the specialty. The rise of OERs helped residents to focus on developing critical appraisal skills and social bonds during the journal club. The local journal club gained a new relevancy in acting as a quality control mechanism against the premature adoption of research findings discussed in OERs. Discussion: To date, most educators assume that residents prepare for a journal club by reading the selected articles and applying knowledge from their previous education. Instead, our findings suggest a more dynamic experience that integrates OERs. OERs enhance the journal club experience by allowing junior residents to more easily participate in discussions and to broaden the discussion to multiple clinical settings. Understanding these processes could inform future educational strategies around the journal club.
A Scoping Review for Debate-Style Journal Clubs in Health Professional Education

Abstract:Objectives: The objectives of this scoping review are to (1) describe the available literature regarding the utility of the debate-style journal club for health professional learners on literature evaluation skills and (2) report the themes found within debate-style journal club research and assessments in the context of professional education. Findings: A total of 27 articles written in the English language were included in this scoping review. Published evaluations of debate-style journal clubs have been predominantly based in the pharmacy profession (48%, n = 13), but are reported in other health professions, such as medicine (22%, n = 6), dentistry (15%, n = 4), nursing (7%, n = 2), occupational therapy (4%, n = 1), and physical therapy (4%, n = 1), as well. The skills assessed in these studies often included critical literature evaluation, application of literature to patient care, critical thinking, knowledge retention, use of supporting literature, and debate-specific skills. Learners typically reported better understanding and application of the literature, and enjoying the experience more than traditional journal clubs, but note the increased assessor and learner time requirement for debating. Pharmacy learner-specific articles more often utilized a traditional, team-based debate format, incorporated grading rubrics for skill assessment and debate performance, and included a grading component for the debate in the course. Summary: Debate-style journal clubs are well-received by learners but require an additional time commitment. Debate platforms, format, rubric use and validation, and outcome assessment vary across published reports.

Association of a Gamified Journal Club on Internal Medicine Residents' Engagement and Critical Appraisal Skills

Abstract:Background: Prior literature demonstrates internal medicine residents have suboptimal competence in critical appraisal. Journal clubs are a common intervention to address this skill, but engagement and critical appraisal skill improvement are variable. Objective: We evaluated journal club engagement and critical appraisal skills after implementation of a gamified format. Methods: This was a single-arm study, conducted from July 1, 2020 to June 30, 2021, involving internal medicine residents at 2 US programs. Residents participated in a 12-month gamified journal club that sorted residents into 2 teams. Residents attended an orientation followed by 6 to 10 monthly, hour-long competitions. In each competition, a subset of the resident teams competed to answer a clinical prompt by critically appraising an original article of their choice. A chief medical resident or faculty member moderated each session and chose the winning team, which received a nominal prize of candy. The primary outcome was engagement, measured by a 7-question survey developed de novo by the authors with Likert scale responses at baseline and 12 months. The secondary outcome was critical appraisal skills assessed by the Berlin Questionnaire. Results: Sixty-one of 72 eligible residents (84.7%) completed both engagement surveys. Residents reported statistically significant improvements in most dimensions of engagement, including a higher likelihood of reading articles before sessions (posttest minus pretest score -1.08; 95% CI -1.34 to -0.82; P<.001) and valuing time spent (posttest minus pretest score -0.33; 95% CI -0.55 to -0.11; P=.004). Critical appraisal skills marginally improved at 12 months (posttest minus pretest score -0.84; 95% CI -1.54 to -0.14; P=.02). Conclusions: Our study demonstrates a gamified journal club was associated with improvements in engagement and minimal change in critical appraisal skills.

Abstract:INTRODUCTION APC and TP53 are tumor suppressor genes known to be altered frequently in human esophageal adenocarcinoma (EAC), which arises as a complication of reflux disease. To determine the functional role of these genes in the development of EAC, we have created reflux in mice gene-targeted for either Trp53 or Apc. METHODS Wild-type (WT), p53-knockout (Trp53-/-), or Apc-mutated (ApcMin/+) mice were generated in our breeding colony. Total gastrectomy with esophagojejunostomy was performed at 6 weeks of age, creating jejunoesophageal reflux. Unoperated control mice were maintained under identical conditions. Mice were sacrificed at 30 weeks of age. Histology of the esophagus and jejunal anastamosis or gastroesophageal junction was reviewed by a single pathologist blinded to the genotype of the animal. RESULTS The esophagus was normal in all of the unoperated mice (6 ApcMin/+, 6 WT, and 6 Trp53-/-). All operated mice (6 ApcMin/+, 12 WT, and 4 Trp53-/-) had esophagitis, with squamous hyperplasia and early focal ulceration. Barrett's metaplasia was identified in 33% of the operated ApcMin/+ (2/6) and 25% of the Trp53-/- (1/4) mice, but not in the WT mice. Of 4 operated Trp53-/- mice, all developed severe dysplasia of the squamous epithelium and 2 (50%) had EAC on histology, although no gross tumors were seen. No severe dysplasia or carcinoma was identified in any of the ApcMin/+ or WT mice. CONCLUSIONS Loss of either Trp53 or Apc leads to the development of columnar metaplasia, whereas loss of Trp53, but not Apc, leads to development of cancer in mice with jejunoesophageal reflux.

Abstract:Introduction The American Association for Geriatric Psychiatry (AAGP) Journal Club is a new initiative sponsored by the Teaching and Training Committee to promote awareness, disseminate knowledge, and refine clinical practice by exchange of ideas and information in the context of the latest research in geriatric psychiatry. This quarterly virtual forum discusses recently published peer-reviewed articles in the American Journal of Geriatric Psychiatry (AJGP) on the diagnosis, treatment/management, policy, and future directions of psychiatric disorders in older adults. The journal club is open to all members of the AAGP, scholars, and trainees. The purpose of this study is to evaluate the efficacy of the completed inaugural year of this initiative by assessing the knowledge, efficacy, and attitudes of attendees of the journal club. Methods A total of 4 journal club sessions were given during the 2022-2023 academic year (one session per quarter). Topics of the journal club were focused on themes of Diversity, Equity, and Inclusion, basic and translational research, clinical review articles, and health policy. Each session was led by an expert guest speaker (typically the first or last author of the publication being presented), followed by a moderated interactive discussion with the attendees. Quantitative and qualitative data from journal club attendees were collected anonymously between 10/21/22 - 6/5/23. Demographic data such as affiliated institution, level of training, degree status, and field of study were obtained. Knowledge level and familiarity on the journal club topics were compared before and after the intervention utilizing pre- and post-surveys. Participants were also assessed post intervention via Likert scale on the organization, understanding, efficacy, and clinical application/relevance of the journal club topic, as well as the ability to disseminate learned knowledge. Qualitative data in the form of free response was also collected on attendee's perception of potential areas of improvement. These responses were internally reviewed and implemented into the subsequent academic year of the journal club. Results Over four sessions, the journal club series had 125 attendees. 49 attendees completed the pre-lecture surveys, and 33 attendees completed the post-lecture surveys. Of the survey participants, 46% identified as clinicians, 5% as researchers, 25% as educators, 16% as fellows/residents, 2% as students, and 5% as other. Of those who completed the post-lecture surveys, 100% of participants either strongly agreed (55%) or agreed (45%) the journal club lecture increased their knowledge in the subject matter. Compared to before the journal club session, participants after the session were four times more comfortable having discussions on the topic with others and felt fairly knowledgeable in the topic. 12% of participants had no knowledge at all of the topic before the journal club lecture, whereas only 3% rated so after the lecture. Overall, 97% of participants strongly agreed (61%) or agreed (36%) that the lecture was organized in a manner that helped them understand the underlying concepts of the topic, and 94% of participants strongly agreed (61%) or agreed (33%) that they would recommend the journal club lectures to their peers. 90% of participants strongly agreed (48%) or agreed (42%) that they can apply the information learned from the journal club series to their practice and career. Conclusions The results of this study provided valuable information surrounding the efficacy of the AAGP journal club. There was an increase in reported knowledge of the topics, ability to discuss the topic with others, and application to individual practices after all journal club sessions. Findings of the study may be used to improve the design and implementation of the journal club in future years. The educational model utilized to develop and deliver the journal can be adapted and disseminated to other educational initiatives.

Abstract:Hepatic cholesterogenesis was studied in pair-fed and pyridoxine-deficient rats as well as in rat liver homogenate systems. Crossover of various subcellular components from pair-fed homogenates into pyridoxine-deficient homogenate systems and vice versa was also done.On 8 weeks of pyridoxine deficiency, acetate-14C incorporation rates into liver cholesterol increased by a factor of approximately 10. The same phenomenon was observed with the total liver homogenate systems.Pyridoxine deficiency does not appear to affect HMG-CoA reductase activity of pyridoxine-deficient liver microsomes sufficiently to explain the rapid acetate-1-14C incorporation rates in this same tissue. The activating system(s) responsible for the 10-fold increase in acetate-14C incorporation rates into pyridoxine-deficient rat liver cholesterol appears to be located in the high-speed supernatant fraction. Abstract:Reading, presenting, and discussing peer-reviewed scientific reports, case studies, and reviews are essential to modern biology education. These exercises model crucial aspects of students' future professional activities and introduce the students to the current scientific concepts and methodology, data analysis, and presentation. A common format for working with primary literature is a journal club: presenting and discussing research literature in front of peers, which has many merits. However, in large modern classrooms, this format is very time-consuming and stressful, especially since presenting is not a commonly taught skill. We argue that student groups for whom the current educational and professional paradigms present a challenge due to a historical lack of representation or wellness issues are deprived of a key educational opportunity. To solve this problem, we formulated an approach called Peer-Reviewed Presentation Exchange (PRPE), which focuses on collaborative analysis, presentation, and review of research literature that includes (i) voice-narrated research presentations by students, (ii) checklists generated by the instructor to establish expectations for an informative presentation or review, and (iii) presentation assignment and peer review process. We tested this approach in an undergraduate cell biology class over 3 years. Pre- and post-assessments show significant gains in self-efficacy and knowledge not only by students who presented but also by the students who reviewed the presentations; therefore, peer-reviewed presentations are an effective tool for learning. Exit surveys show that the approach is seen as beneficial by most students. Our approach allows every student to speak and ask questions in a low-stress creative environment. It is an excellent customizable, trackable, and scalable low-stakes assessment tool.
Impact of a Journal Club Elective Course on Student Learning Measures

Abstract:Objective. To assess the impact of a journal club elective course on measures of student’s longitudinal performance throughout an accelerated three-year Doctor of Pharmacy (PharmD) curriculum. Methods. Students were separated into two groups (those who did and did not complete a journal club elective). The following primary and secondary student outcomes were assessed using hierarchical linear regression analysis: score on the Pharmacy Curriculum Outcomes Assessment (PCOA), overall course grade in the Pharmacotherapeutics IV course, overall course grade in the Research Design and Literature Evaluation II course, and average grade on acute/ambulatory care advanced pharmacy practice experiences (APPEs). Results. One hundred ninety-seven students were included in the study (73 students who completed the journal club elective and 124 students who did not). After controlling for baseline confounders, enrollment in the journal club elective was associated with students scoring 24.5 points higher on their overall scaled score on the PCOA. Enrollment in the journal club elective also appeared to add 2% to a student's overall grade in each of the courses and APPEs. All results were statistically significant. Conclusion. An elective journal club course can significantly improve multiple objective measures of pharmacy student learning. Components of this course, such as reading primary literature, presenting a journal club, learning from peers, and scaffolding of pharmacotherapeutic concepts are important elements to consider when designing a journal club curriculum.

Enhancing Students’ Skills in Applying Theory Through Collaborative and Active Learning: A Pre-Post Evaluation of an Innovative Theory Course for Graduate Public Health Students

Abstract:Modern public health practice includes planning, implementation, and evaluation of programs targeting the structural and social determinants of health. To prepare graduate students to engage with theory in complex, practical, and innovative ways, a new model of course delivery was created to focus on developing and applying theorizing skills to the social ecological model. Throughout the semester, students engage in collaborative learning to apply theory to real-world public health interventions focused on high-profile public health issues. Data were gleaned from three course sections ( N = 80) using a pre-post study design to evaluate students’ ability to apply theory at various levels of the social ecological model and confidence to meet course objectives. The post-course survey also included questions about the helpfulness of specific course elements and asked for recommendations for improvement. Descriptive statistics were calculated and differences between pre- and post-ratings were tested with paired samples t-tests and Wilcoxon signed rank tests. Students improved in their ability to apply theory by the end of the semester ( p < .001) and in their confidence in meeting all learning objectives ( p < .001). Most students found that this new format of engaging with theory in complex, applied, and innovative ways by working in teams, completing in-class activities, and open poster presentations beneficial. Engaging students in a new course format led to critical reflection about various levels of influence on health and enhanced students’ ability to apply theory at multiple levels of influence. Further refinement and course development will continue to facilitate developing theorizing skills.
Continuous In Vivo Measurement of Hepatic Lipoperoxidation Using Chemiluminescence: Halothane and Chloroform Compared

Abstract:Hepatic necrosis, rarely occurring after administration of modern halogenated general anesthetics, has been ascribed to free radical-induced lipoperoxidation. As evaluation of hepatic Chemiluminescence has been shown to provide a noninvasive assay for oxidative radical reactions, we have made continuous in vivo measurements of hepatic light output in the phenobarbital-induced halothane anesthetized rat and compared them with observations made during chloroform anesthesia. Chloroform produced a far more rapid and more profound increase in Chemiluminescence, a phenomenon probably related to depletion of hepatic gluta- thione. These findings are consistent with significant differences in hepatic pathology associated with the two agents.
A novel 6D-approach to radically transform undergraduate medical education: preliminary reflections from MBRU

Abstract:Background: Designers of undergraduate medical education (UME) need to address the exponentially expanding volume and variability of scientific knowledge, where by didactic teaching techniques need to be augmented by innovative student-centric pedagogical strategies and implementation of milieus, where information, communication and technology-enabled tools are seamlessly integrated, and lifelong information gathering, assimilation, integration and implementation is the ultimate goal. In UME, the basic sciences provide a solid scaffold allowing students to develop their personal critical decisional framework as well as define the understanding of normal human physiology, pivotal for the identification, categorization and management of pathophysiology. However, most medical schools confine themselves to "stagnant curricula", with the implementation of traditional "teacher centered" pedagogical techniques in the dissemination of the courses pertaining to basic sciences in UME. Method: To tackle the above paucity, we present a novel "6D-Approach" for the dissemination of concepts in basic sciences through mentored journal-clubs. The approach is informed by a teaching principle derived from Constructivism. The technique in which the 6D-approach can be implemented in UME, is shown using an example from a first-year course of Molecular Biology and Principles of Genetics at our medical school. A reflection on the impact of 6D-Approach for students as well as instructors is also presented. Result: The 6D-approach was positively received by the students and the formal feedback for the course: Molecular Biology and Principles of Genetics, where the approach was repeatedly employed, indicated that students expressed satisfaction with the teaching strategies employed in the course, with ~ 89% of the students in the cohort strongly agreeing with the highest grading score "extremely satisfied". Further, the approach through the use of mentored journal clubs encourages retention of knowledge, critical thinking, metacognition, collaboration and leadership skills in addition to self-evaluation and peer feedback. Conclusion: Hence, through the 6D-Approach, our attempt is to initiate, advance and facilitate critical thinking, problem-solving and self-learning in UME, demonstrated by graduating accomplished, competent and safe medical practitioners.

Detailed discussions and analyses of clinical research articles by the residents are used to teach the important concepts and principles of medical research in a manner useful to the practicing physician. Discussion of research articles is structured by a guide for presenting the research articles to the group and by stimulus questions that address specific methodological issues related to the study being examined. Residents have been enthusiastic in their evaluation of the journal club; the experience has stimulated some of them to conduct their own research studies.

Abstract:Introduction/background: Course evaluation in health education is a common practice yet few comprehensive evaluations of health education exist that measure the impact and outcomes these programs have on developing health graduate capabilities. Aim/objectives: To explore how curricula contribute to health graduate capabilities and what factors contribute to the development of these capabilities. Methods: Using contribution analysis evaluation, a six-step iterative process, key stakeholders in the six selected courses were engaged in an iterative theory-driven evaluation. The researchers collectively developed a postulated theory-of-change. Then evidence from existing relevant documents were extracted using documentary analysis. Collated findings were presented to academic staff, industry representatives and graduates, where additional data was sought through focus group discussions - one for each discipline. The focus group data were used to validate the theory-of-change. Data analysis was conducted iteratively, refining the theory of change from one course to the next. Results: The complexity in teaching and learning, contributed by human, organizational and curriculum factors was highlighted. Advances in knowledge, skills, attitudes and graduate capabilities are non-linear and integrated into curriculum. Work integrated learning significantly contributes to knowledge consolidation and forming professional identities for health professional courses. Workplace culture and educators' passion impact on the quality of teaching and learning yet are rarely considered as evidence of impact. Discussion: Capturing the episodic and contextual learning moments is important to describe success and for reflection for improvement. Evidence of impact of elements of courses on future graduate capabilities was limited with the focus of evaluation data on satisfaction. Conclusion: Contribution analysis has been a useful evaluation method to explore the complexity of the factors in learning and teaching that influence graduate capabilities in health-related courses.
Efficacy of a novel journal club series at an academic hematology/oncology fellowship program.

Abstract:11027 Background: Journal club (JC) is an essential tool in hematology/oncology fellowship to develop critical appraisal skills. The traditional JC format of reviewing an article followed by group discussion may not provide optimal education in appraising literature. We applied a novel JC curriculum and measured impact on fellow comfort with trial design, statistics, and endpoint interpretation. Methods: A novel JC curriculum was implemented for hematology/oncology fellows at Vanderbilt in the 2021-2022 academic year. JC for the prior year utilized the traditional format. We developed five focused learning objectives (LO’s) for each JC and emphasized the goal was to not place the paper in clinical context but to focus on developing the foundational tools for critical appraisal skills. The sessions involved (1) 15 minute introductory lecture from a biostatistician to cover foundations of the LO’s; (2) group discussion led by an upper year fellow to describe the paper in the context of the LO’s; (3) question and answer session from the fellows to both the statistician and a clinical trial expert. We developed anonymous pre and post surveys to understand the impact of the novel JC curriculum on a 5-point Likert scale. Two sample unpaired t-test was used to compare mean differences. Results: A total of 15/22 fellows (68%) completed the pre-survey and 12/22 fellows (55%) completed the post survey. There was an even distribution of fellows by year in the pre survey and 33% first years, 41% second years, and 25% third years in the post survey. There was a statistically significant improvement in pre and post mean fellow comfort in understanding differences in trial types (2.9 vs. 4, p < 0.001), randomization strategy (2.6 vs. 4.25, p < 0.001), endpoints (3 vs. 4.1, p < 0.001), and interpretation of the statistical methodology of represented data (3 vs. 4.3, p < 0.001) on a 1-5 Likert scale (5 being entirely comfortable). In a subset analysis, we did a direct comparison of fellows who had previously participated in both JC formats (n = 9); the new format was rated as a more useful tool for learning data interpretation than prior with a mean of 4.8/5. When comparing fellow perception in the utility of traditional compared to the novel JC, there was a statistically significant mean improvement in utility of JC to prepare fellows to understand differences in trial types (1.9 vs. 4.1, p < 0.001), randomization strategy (1.4 vs. 4.5, p < 0.001), endpoints (2.7 vs. 4.5, p < 0.001), and interpretation of the statistical methodology of represented data (2.33 vs. 4.6, p < 0.001) favoring the novel JC. Conclusions: Fellows’ comfort with trial design and content significantly improved with our novel JC. To our knowledge, this is the first time such a curriculum has been used in hematology/oncology training. Our successfully piloted novel JC model can be adapted to other fellowships to improve foundational skills in the critical appraisal of clinical trials.

Abstract:Clozapine is increasingly being used for clinical indications in addition to treatment-resistant schizophrenia; this article reviews the relevant literature. The first section reassesses the risks associated with clozapine treatment, particularly agranulocytosis. The next section discusses its use for schizophrenia in patients who are treatment resistant, not treatment resistant, and intolerant of traditional drug treatments. Subsequent sections address its use in mood disorders, neurologic conditions, comorbid substance abuse, aggressive behavior, and childhood schizophrenia. Each includes the initial rationale for the use of clozapine in the disorder, a critical evaluation of the relevant literature, and theories as to why clozapine's unique pharmacodynamic profile may be efficacious for the specific condition. This body of literature suggests clozapine may be an effective treatment for a wide range of disorders.
