Abstract:After reviewing the emergence of online newspapers, we offer observations based on historical and design analyses of major US sites, supplemented top-down by innovators in the Americas and Europe and bottom-up by sites serving one locality in Massachusetts. Despite losing typical print elements, the late modern designs emphasize text, with minimal multimedia content, especially on local sites. Instead of giving outlet to news handicraft, corporate and promotional models abound. The web flattens hierarchies, exposes content sources, and deforms journalistic authority by disarticulating the audience. Historical parallels include 19th-century flows of design innovation from advertising into news and of informational tasks from reporting into photojournalism. Newspapers can coexist with the internet while surrendering some tasks, such as archiving factual background, becoming instead more analytical advocates.

Effectiveness of interventions to reduce flour dust exposures in supermarket bakeries in South Africa

Abstract:Rationale: A recent study of supermarket bakery workers in South Africa demonstrated that 25% of workers were sensitised to flour allergens and 13% had baker's asthma. Evidence on exposure reduction strategies using specifically designed interventions aimed at reducing the risk of baker's asthma is scarce. Objectives: The aim of this study was to evaluate the effectiveness of different control measures to reduce airborne flour dust exposure using a randomised design. Methods: A group-randomised study design was used to assign 30 bakeries of a large supermarket chain store to two intervention groups and a control group, of which 15 bakeries were studied. Full-shift environmental personal samples were used to characterise exposure to flour dust and wheat and rye allergens levels pre-intervention (n=176) and post-intervention (n=208). Results: The overall intervention effect revealed a 50% decrease in mean flour dust, wheat and rye allergen exposure. The reduction in exposure was highest for managers (67%) and bakers (47%), and lowest for counterhands (23%). For bakers, the greatest reduction in flour dust was associated with control measures such as the use of the mixer lid (67%), divider oil (63%) or focused training (54%). However, the greatest reduction (80%) was observed when using a combination of all control measures. Conclusions: A specially designed intervention strategy reduced both flour dust and allergen levels. Best results were observed when combining both engineering controls and training. Further studies will investigate the long-term health impact of these interventions on reducing the disease burden among this group of bakers.

Abstract:Scientific discussions are focussed on establishing safety limits for the presence of per and polyfluoroalkyl substances (PFAS) in food, in response to growing evidence of health risks associated with their exposure. To the best of the authors' knowledge, most studies have so far focused on animal-derived food, which generally represents the highest source of ingested PFAS. Only recently research has shifted to the monitoring of these contaminants also in vegetables and baked processed foods. Flours have not been extensively investigated and data on their presence is difficult to find in literature. The absence of data has resulted in the European Regulation currently not specifying any regulatory limits for PFAS in flours. This study aims to collect informative data on the presence of these contaminants in flours from different origin and to infer information regarding the total exposure deriving from this staple food. This study assessed the amount and type of PFAS present in traditional, organic, protein flours and feed. The most detected PFAS at quantifiable concentrations were PFOA and PFDA. Flours for human consumption contained PFAS in the range 0.015–0.176 ng/g, while flours for feed and from crickets contained PFAS in the range 0.033–1.963 ng/g. Differences have been observed between the three groups of samples in the average PFAS content; the difference between protein and organic resulted statistically significative. Our data suggest that consuming 200 g of flour per day could significantly contribute to the total weekly intake of PFAS.

Abstract:Wheat-based products are largely consumed by children worldwide. Deoxynivalenol (DON) is known for its acute and chronic toxicity and is the most common contaminant of cereal grains. Since no legal limits are set for DON in wheat-based products and specific foods intended for children over 3 years on the market, a high risk of overexposure to this contaminant may emerge. The main objective of the study, conducted in 2018-2019, was to produce a wheat flour intended for children over three years, characterized by a high level of safety in terms of DON content, to be used to produce wheat-derived products. The dedicated flour was produced by adopting tailored procedures like the selection of wheat suppliers, the predetermination of the safe contamination of DON in the final products, and the evaluation of the transfer rate from the wheat flour to derived products (bread, breadsticks, biscuits, plumcake, and focaccia). The results showed that the daily exposure of children was considered to be safe, in a range between 7% (biscuits) and 67% (bread) of DON tolerable daily intake (TDI) and that only by producing a flour characterized by DON levels much lower than those in force, can "safe" products be marketed.

Abstract:The Italian Interministerial Decree of February 11, 2021, introduces the diesel engine exhaust (DDE) among the carcinogenic occupational compounds, also establishing an occupational exposure limit. Elemental carbon (EC), improperly called black carbon, has been proposed as a tracer of DDE exposure; EC is the carbon that is quantified in the ambient matrixes after all the organic carbon has been removed; traditionally, EC is measured with a thermo-optical analytical technique. EC determination and relative interpretation are challenging for the following reasons: (i) the scarce availability of equipped laboratories hampers EC analysis, (ii) EC interpretation is not easy due to the lack of reference values. Finally, (iii) the limit value of 0.050 mg/m3 of EC in the workplace appears too high compared to recently published exposure data. All these aspects stimulate a reflection on the significance of EC data in the context of both occupational hygiene and occupational medicine.
Occupational exposure to crystalline silica in artificial stone processing

Abstract:Respirable Crystalline Silica (RCS) is a hazardous substance with known effects that can be well correlated with exposure levels that still persist in many traditional sectors, such as construction or stone processing. In the past decade, exposure scenarios for RCS have been found in the sector of artificial stone processing. The aim of this study is to evaluate the levels of RCS in facilities specialized in the production of artificial stone countertops and other accessories for the furnishing of kitchens, bathrooms, and offices after the introduction of some preventive technical measures such as wet processing or local exhaust ventilation systems. The study involved 51 subjects in four facilities. Personal silica exposure assessment was carried out using GS3 cyclones positioned in the breathing zone during the work shift. Quantitative determination of silica was carried out by X-ray diffraction analysis. Respirable dust levels were in the range 0.046-1.154 mg/m3 with RCS levels within the range <0.003-0.098 mg/m3. The highest exposure was found in dry finishing operations. Although there was a remarkable reduction in RCS exposure levels compared to what was observed in the past before the introduction of preventive measures, the data still showed hazardous exposure levels for some of the monitored activities.

Abstract:The occurrence of halogenated organic pollutants in indoor dust can be high due to the presence of textile, electronic devices, furniture, and building materials treated with these chemicals. In this explorative study, we focused on emerging organic pollutants, such as novel brominated flame retardants (nBFRs) and some perfluoroalkyl substances, together with legacy polychlorinated biphenyls (PCBs) and polybrominated diphenyl ethers (BDEs) in settled dust collected in houses and workplaces such as one office and two electrotechnical and mechanical workshops. The total contribution of the investigated pollutants was lower in house and in office dusts except for few nBFRs (such as bis (2-ethylhexyl)-3,4,5,6-tetrabromo-phthalate at a concentration of 464.5 ng/g in a house and hexachlorocyclopentadienyldibromocyclooctane at 40.4 ng/g in the office), whereas in electrotechnical and mechanical workshops a high incidence of PCBs, BDEs, and nBFRs occurred (for example, BDE 209 at a concentration of 2368.0 ng/g and tetrabromobisphenol A at 32,320.1 ng/g in electrotechnical and mechanical workshops). Estimated daily intakes were also calculated, showing that domestic and occupational environments can lead to a similar contribution in terms of human exposure. The higher exposure contribution was associated to nBFRs, whose EDIs were in the range of 3968.2–555,694.2 pg/kg bw/day. To provide a complete view about the indoor contamination, in this investigation, we also included polycyclic aromatic hydrocarbons (PAHs) and their oxygenated and nitrated derivatives. Definitely, dust collection represents a simple, fast, and cost-effective sampling and dust contamination level can be a useful indicator of environment healthiness. Besides, the presented method can be a smart tool to provide a time and money saving technique to characterize 99 pollutants thanks to a single sample treatment.

Abstract:Asthma and allergies have become increasingly prevalent over the last few decades throughout the WHO European Region, with an average of more than 10% of children suffering from asthmatic symptoms. Asthma is one of the most common chronic diseases and it is a complex and heterogeneous disorder where genetics and environment play an interacting role. Childhood asthma is a major global health problem, which exerts a substantial burden on family, health care, and society as a whole. In decision-making on interventions to reduce the burden of childhood asthma an overview of the cost-of-illness of childhood asthma is an essential first step. In this report the cost-of-illness of childhood asthma in the countries of the European Union is assessed by order of the European Commission. Also the countries that will enter the European Union at the 1st of May 2004 are included. Occupational exposure to styrene in the fibreglass reinforced plastic industry: comparison between two different manufacturing processes

Abstract:Background: Styrene is used in manufacturing fiberglass reinforced plastics: and occupational exposure was related to neurotoxicology and genotoxicity. The sum of the metabolites mandelic and phenylglyoxylic acids is the ACGIH biomarker for occupational exposure with a BEI of 400 mg/g of creatinine in end shift urine corresponding to a airborne styrene concentration of 85 mg/m3. There are two main molding processes, open and closed, the last more effective at controlling worker's styrene exposure. Objectives: To compare the open molding process to the compression of fiber reinforced resin foils, a kind of closed molding, monitoring the styrene exposure of workers in two production sites (A and B). Methods: Environmental Monitoring was carried out by Radiello samplers and Biological Monitoring by means of the determination of MA and PGA with HPLC/MS/MS in 10 workers at Site A and 14 at Site B. Results: The median values for styrene exposure resulted 31.1 mg/m3 for Site A and 24.4 mg/m for Site B, while the medians for the sum of the two metabolites in the end shift urine were 86.7 e 33.8 mg/g creatinine respectively. There is a significant linear correlation between personal styrene exposure and the excretion of styrene metabolites (R = 0.74). Conclusions: As expected the exposure markers of the workers of the two production sites resulted higher in the open process. The analytical results of both environmental and biological monitoring were all below the occupational exposure limits, confirming the efficacy of the protective devices.

Abstract:Asthma in bakers and bakery workers is generally referred to as “baker's asthma.” Although a useful shorthand, this terminology belies the observation that asthma caused by the same agents occurs in workers in related occupations such as flour milling, other food production, and in industries that produce and package enzymes and other ingredients for the baking sector. Asthma among bakers and millers is arguably the most familiar and, notoriously, the oldest form of occupational asthma (OA). Most studies indicate that wheat (Triticum aestivum) flour proteins are allergens for 60%–70% of symptomatic bakers, although other cereals like rye (Secale cereale), barley (Hordeum vulgare), oats (Avena sativa), and corn, and noncereal sources, enzymes, and insects may be involved because bakeries are complex environments. Using a model similar for all chapters on specific agents causing OA, the following topics are covered in this chapter: Allergens, nature and source: In this area, considerable progress has been reached using recombinant allergens. A vast number of cereals are now used, increasing the likelihood of sensitization. An equally vast number of enzymes is also an important source of allergenicity. The soybean allergens involved in baker's asthma are predominantly high-molecular-weight (HMW) proteins present both in soybean hull and flour. The bakery environment is complex and there are several case reports of baker's asthma caused by molds and insects, although these are of marginal importance to the burden of disease in bakers; nonetheless they should be kept in mind in clinical setting if no sensitization to common bakery allergens is found. Using simulation techniques based on cross-sectional data to examine effectiveness of several strategies to reduce burden of baker's asthma, the most successful approach was one of rigorous health surveillance identifying sensitized or symptomatic workers.

Abstract:A pressure-measuring Moore-type endoprosthesis was implanted in a seventy-three-year-old patient who had sustained a displaced fracture of the femoral neck. The measurement and telemetry of contact pressures in the hip began in the operating room, and data were acquired periodically for more than thirty-six months. Unexpectedly high localized contact pressures between the acetabular cartilage and the prosthesis were recorded. Early in the period of recovery, activities such as using a bedpan or performing isometric exercise produced pressures that were close to those recorded during normal walking. The highest pressure, eighteen megapascals, was recorded one year postoperatively, while the patient was rising from a chair. High pressures occurred in the superior and posterior aspects of the acetabulum.

Abstract:SH3 domains are small protein modules that mediate the assembly of specific protein complexes, typically via binding to proline-rich sequences in their respective binding partners. Most of the α-spectrin SH3-domain (Spc-SH3) structures determined to date using X-ray diffraction have been solved from crystals belonging to the orthorhombic space group P2(1)2(1)2(1) with a needle-like morphology. All of these orthorhombic crystals exhibited a rapid growth rate. In addition to this crystal form, the R21D mutant of Spc-SH3 crystallizes in a new crystal form in the presence of sodium formate at pH values higher than 6. This new crystal form grows at a slower rate and belongs to the hexagonal space group P6(5)22, with unit-cell parameters a = b = 42.9, c = 127.5 Å. When both polymorphs of the R21D mutant of Spc-SH3 are simultaneously present into the same solution, it has been observed that the hexagonal crystals grow at the expense of the orthorhombic crystals. The availability of 1.1 Å resolution structures for both crystal forms allows the identification of key features that could account for the observed polymorphic behaviour.

Abstract:Abstract Background In dental laboratories, exposure to crystalline silica can occur during procedures that generate suspended mineral dusts, e.g. dispersion of mixing powders, removal of castings from molds grinding, polishing of castings and porcelain, and use of silica sand for blasting. There is also a large list of toxic agents (acrylic resins, polymeric materials, etc.) used to produce removable and fixed prostheses, but also impression materials and more. Using personal protective equipment and other aids reduces the exposure to these potentially harmful agents. Case presentation We report the case of a 42-year-old male dental technician who began to suffer from a dry cough and exertional dyspnea after approximately 15 years of work. The operations he conducted for his job resulted in the generation of crystalline silica, aluminum, chromium and titanium dust. The worker did not regularly wear personal protective equipment and some of the above operations were not carried out in closed circuit systems. The Chest X-ray showed diffused micronodules in the pulmonary interstitium of the upper-middle lobes, bilaterally, and a modest left basal pleural effusion. Simple spirometry showed small airway obstruction in its initial stage. High Resolution Computerized Tomography of the chest showed bilateral micronodulation of a miliariform type, with greater profusion to the upper lobes, also present in the visceral pleura, bilaterally. Histological examination showed aggregates of pigment-laden macrophages forming perivascular macules or arranged in a radial pattern around a core of sclerohyalinosis. Scanning Electron Microscopy and Energy Dispersive Spectrometry revealed several mineral particles, typically characterized by the presence of crystalline silica and metal aggregates. The environmental concentrations of total dust and its respirable fraction were all lower than the relative TLV-TWA—ACGIH, yet not negligible. Conclusions The above findings and a multidisciplinary assessment led to the diagnosis of mixed dust pneumoconiosis s/q with 2/2 profusion of occupational origin. This diagnosis in a dental technician was supported for the first time in literature by environmental exposure analysis.

Abstract:Objectives: occupational exposure to vapours, gases, dusts and fumes (VGDF) plays an important role in the development and exacerbation of respiratory diseases. The aim of this study is to evaluate the possible association of occupational exposure to airborne pollutants and chronic respiratory diseases. Design: multicase-control study. Setting and participants: cases of chronic respiratory diseases and controls from the Italian multicentric study Gene Environment Interaction in Respiratory Diseases (GEIRD). Main outcome measures: the occurrence of rhinitis, asthma, chronic bronchitis/chronic obstructive pulmonary disease (COPD), asthma severity, spirometry data, exhaled nitric oxide (FeNO) were examined in relation to chronic and acute occupational exposures to airborne pollutants using multiple regression models. Results: 2,943 subjects were enrolled in the study. Regularm exposure to VGDF was associated with a higher prevalence of chronic bronchitis/COPD (OR 1.40, 95%CI 0.98-1.99), especially in those also having asthma (OR 1.80, 95%CI 1.14-2.85), a lower prevalence of remittent asthma (OR 0.53, 95%CI 0.29-0.96) and, in those with asthma, an increased activity of the disease (severity score) (OR 1.77, 95%CI 1.20-2.60). No associations were observed between occupational exposure and prevalence of rhinitis, spirometry and FeNO data. Finally, an association was found between acute exposure to airborne pollutants (occupational and non-occupational) and the respiratory diseases investigated, in particular active asthma and asthma associated chronic bronchitis/BPCO. Conclusions: these data confirm a significant role of occupational exposure to airborne pollutants on respiratory health, underlying the importance of workplace exposure prevention, in particular for more susceptible subjects, as those with respiratory diseases.
Upper airway and eye involvement in polyurethane shoe sole production workers

Abstract:Objectives: This cross-sectional study was aimed to investigate the prevalence of work-related upper and lower airway and eye symptoms in 118 workers in polyurethane shoe soles (PSS) production. Methods: Workplace monitoring of methylene diphenyl diisocyanate (MDI) and solvents was performed. Subjects completed a study-specific questionnaire and underwent anterior rhinoscopy, skin prick tests for common aeroallergens, spirometry, nasal peak inspiratory (NPIF) and expiratory flow (NPEF). Results: MDI and solvent levels were below threshold limit value-time-weighted average (TLW-TWA) except for two measures of dichloromethane and tetrachloroethylene, respectively, and in one measure of acetonitrile, which were higher then TLW-TWA. In exposed workers the prevalence of cough (p < 0.05) and nasal congestion at rhinoscopy (p = 0.05) was more frequent than in non-exposed workers. Occupational exposure (OR 4.5, 95% CI 1.2-16.5) and a low FEV1 (OR 2.6, 95% CI 1.1-6.3) were significant predictors of cough. Conclusions: In workers exposed to low levels of MDI and solvents in polyurethane shoe sole production there was a high prevalence of cough and nasal congestion. An improvement in the exhaust ventilation system and other preventive measures were needed.

Abstract:The paper contains the main lines of the modeling of a prototype for both medical and economical evaluation of radiotherapy centers using innovative therapies. We consider uncertainties according to patients and hospital complexes participating in the study. The corresponding part of the tool is called recruitment model. It evaluates a theoretical number of patients interested in treatments of one center, using spatialization into a context of competition. Moreover we developed a scheduling model, described briefly here and two shortly presented economical models in charge of the evaluation of the average price per treatment and the comparison between different radiotherapy strategies. We present preliminary results of the recruitment model.

Abstract:The aim of the present review was to identify all the biomarkers used for assessing the internal dose and the related early effects determined by the occupational exposure to formaldehyde. For this purpose, a systematic review was performed according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The protocol was registered in PROSPERO (ID: CRD42023416960). An electronic search of Pubmed, Scopus, and Web of Science was performed to collect all the papers concerning the focus of the review and published from the inception of each database until 18 September 2023. Articles were considered eligible if they reported data from observational studies, semi-experimental, and experimental studies on adult workers who were occupationally exposed to formaldehyde, regardless of gender or age. The quality assessment was performed using the adapted Newcastle–Ottawa Quality Assessment Scale. From 1524 articles, 52 were included. Few studies assessed the exposure to formaldehyde in occupational settings through biomarkers, especially by measuring formic acid in urine. The most common approach for evaluating the effects derived from occupational exposure to formaldehyde was the use of the cytogenetic biomarker micronucleus assay in peripheral blood lymphocytes and/or epithelial buccal cells.

Abstract:BACKGROUND Assessment of IgE-mediated sensitization to flour allergens is widely used to investigate flour-induced occupational asthma. The diagnostic efficiency of detecting specific IgE antibodies (sIgE) against wheat and rye flour, however, has not been thoroughly compared to other diagnostic procedures. OBJECTIVE We sought to evaluate the diagnostic accuracy of sIgE against wheat and rye compared to specific inhalation challenge (SIC) with flour as the reference standard. METHODS This retrospective multicenter study included 264 subjects who completed a SIC with flour in eight tertiary centers, of whom 205 subjects showed a positive SIC result. RESULTS Compared with SIC , sIgE levels ≥0.35 kU A /L against wheat and rye provided similar sensitivities (84%-85%, respectively), specificities (71%-78%), positive predictive values (91%-93%), and negative predictive value (56%-61%). Increasing the threshold sIgE value to 5.10 kU A /L for wheat and to 6.20 kU A /L for rye provided a specificity ≥95% and further enhanced the positive predictive value to 98%. Among subjects with a positive SIC, those who failed to demonstrate sIgE against wheat and rye (n=26) had significantly lower total serum IgE level and blood and sputum eosinophil counts, and a lesser increase in post-challenge fractional exhaled nitric oxide as compared to the subjects with detectable sIgE. CONCLUSION High levels of sIgE against wheat and/or rye flour strongly support a diagnosis of flour-induced occupational asthma without the need for performing a SIC. The absence of detectable sIgE against wheat and rye in subjects with a positive SIC seems to be associated with lower levels of T H 2 biomarkers.
