Abstract:Purpose: The objective of this study was to compare and assess the accuracy of computed tomography (CT)-based target delineation with that of magnetic resonance imaging (MRI)-based on high-dose-rate brachytherapy (HDR-BT) for patients with cervical cancer. Material and methods: Data of 20 patients with locally advanced cervical cancer were collected and evaluated. Dimensions, conformity, and dose parameters of high-risk clinical target volume (CTVHR) as well as D-0.(1cc), D-1(cc), and D-2cc of organs at risk (OARs) based on MRI were compared with those based on CT. Results: Average age of 20 patients included was 57.8 years. Width, thickness, and volumes of CT-based CTVHR (CTVHR-CT) were significantly overestimated compared with those of MRI-based CTVHR (CTVHR-MR). Mean values of dice similarity coefficient (DSC), Hausdorff distance (HD), and centroid distance (AV) of CTVHR were 0.82 cm, 0.96 cm, and 0.35 cm, respectively. Dose values of CTVHR-CT were significantly lower compared with those of CTVHR-MR. Concerning OARs, geometrical and dosimetric values on CT were comparable to those on MRI. Conclusions: The delineated ranges of CTVHR were significantly over-estimated on CT compared with MRI. D-98 and D-90 of CTVHR-CT were lower than CTVHR-MR. DSC and AV of CTVHR and CTVIR were similar to each other; however, there was a difference in terms of HD. CT images regarding pre-BT MR images for delineating were not enough and MRI fusion is still required.

Abstract:Purpose: To propose a dosimetry algorithm for three-dimensional (3D) intensity modulated brachytherapy (IMBT) and to develop an inverse treatment planning method applying the dosimetry algorithm. Method and Materials: A Matlab based prototype 3D IMBT treatment planning system was developed. The system consisted of three main components: (1) a comprehensive source commissioning method for intensity modulated sources based on Monte Carlo (EGSnrc) simulations; (2) a “modified TG43” (mTG43) dose calculation algorithm for IMBT dosimetry; (3) an inverse IMBT treatment planning method based on Dose Volume Histogram (DVH) or Dose Surface Histogram (DSH) constraints and simulated annealing optimization algorithm. The system was applied for planning of an intracavitary accelerated partial breast irradiation (APBI) case treated with Xoft Axxent electronic brachytherapy. Plan quality, planning and delivery time of the IMBT plan were compared with the original plan used for the patient's treatment. Results: For the patient studied, IMBT plan showed better plan quality compared with the original plan. With similar coverage to the target, high dose region V200 was decrease by 16.1%. Maximum doses to skin and ribs were reduced by 56 cGy and 104 cGy in one fraction respectively. Mean dose to ipsilateral and contralateral breasts and lungs were also slightly reduced by IMBT. Conclusion: Application of three-dimensional intensity modulation in brachytherapy treatment planning is both feasible and promising. 3D IMBT improves the quality of APBI brachytherapy treatment plan, increasing dose uniformity in target and reducing the dose to critical structures.

Abstract:Purpose: The feasibility of intensity modulated brachytherapy (IMBT) to improve dose conformity for irregularly shaped targets has been previously investigated by researchers by means of using partially shielded sources. However, partial shielding does not fully explore the potential of IMBT. The goal of this study is to introduce the concept of three dimensional (3D) intensity modulated brachytherapy and solve two fundamental issues regarding the application of 3D IMBT treatment planning: The dose calculation algorithm and the inverse treatment planning method.Methods: A 3D IMBT treatment planning system prototype was developed using the MATLAB platform. This system consists of three major components: (1) A comprehensive IMBT source calibration method with dosimetric inputs from Monte Carlo (EGSnrc) simulations; (2) a "modified TG-43" (mTG-43) dose calculation formalism for IMBT dosimetry; and (3D) a physical constraint based inverse IMBT treatment planning platform utilizing a simulated annealing optimization algorithm. The model S700 Axxent electronic brachytherapy source developed by Xoft, Inc. (Fremont, CA), was simulated in this application. Ten intracavitary accelerated partial breast irradiation (APBI) cases were studied. For each case, an "isotropic plan" with only optimized source dwell time and a fully optimized IMBT plan were generated and compared to the original plan in various dosimetric aspects, such as the plan quality, planning, and delivery time. The issue of the mechanical complexity of the IMBT applicator is not addressed in this study.Results: IMBT approaches showed superior plan quality compared to the original plans and the isotropic plans to different extents in all studied cases. An extremely difficult case with a small breast and a small distance to the ribs and skin, the IMBT plan minimized the high dose volume V(200) by 16.1% and 4.8%, respectively, compared to the original and the isotropic plans. The conformity index for the target was increased by 0.13 and 0.04, respectively. The maximum dose to the skin was reduced by 56 and 28 cGy, respectively, per fraction. Also, the maximum dose to the ribs was reduced by 104 and 96 cGy, respectively, per fraction. The mean dose to the ipsilateral and contralateral breasts and lungs were also slightly reduced by the IMBT plan. The limitations of IMBT are the longer planning and delivery time. The IMBT plan took around 2 h to optimize, while the isotropic plan optimization could reach the global minimum within 5 min. The delivery time for the IMBT plan is typically four to six times longer than the corresponding isotropic plan.Conclusions: In this study, a dosimetry method for IMBT sources was proposed and an inverse treatment planning system prototype for IMBT was developed. The improvement of plan quality by 3D IMBT was demonstrated using ten APBI case studies. An Inverse Dose Optimization Algorithm for Three-Dimensional Brachytherapy

Abstract:PURPOSE: To investigate an implementation method and the results of an inverse dose optimization algorithm, Gradient Based Planning Optimization (GBPO), for three-dimensional brachytherapy.METHODS: The GBPO used a quadratic objective function, and a dwell time modulation item was added to the objective function to restrict the dwell time variance. We retrospectively studied 4 cervical cancer patients using different applicators and 15 cervical cancer patients using the Fletcher applicator. We assessed the plan quality of GBPO by isodose lines for the patients using different applicators. For the 15 patients using the Fletcher applicator, we utilized dose-volume histogram (DVH) parameters of HR-CTV (D100%, V150%) and organs at risk (OARs) (D0.1cc, D1cc, D2cc) to evaluate the difference between the GBPO plans and the IPSA (Inverse Planning Simulated Annealing) plans, as well as the GBPO plans and the Graphic plans.RESULTS: For the 4 patients using different applicators, the dose distributions are conformable. For the 15 patients using the Fletcher applicator, when the dwell time modulation factor (DTMF) is less than 20, the dwell time deviation reduces quickly; however, after the DTMF increased to 100, the dwell time deviation has no remarkable change. The difference in dosimetric parameters between the GBPO plans and the IPSA plans is not statistically significant (P>0.05). The GBPO plans have a higher D100% (3.57 ± 0.36, 3.38 ± 0.34; P<0.01) and a lower V150% (55.73 ± 4.06, 57.75 ± 3.79; P<0.01) than those of the Graphic plans. The differences in other DVH parameters are negligible between the GBPO plans and the Graphic plans.CONCLUSIONS: The GBPO plans have a comparable quality as the IPSA plans and the Graphic plans for the studied cervical cancer cases. The GBPO algorithm could be integrated into a three-dimensional brachytherapy treatment planning system after studying more sites.

Abstract:Purpose:Currently, when external beam radiotherapy (EBRT) is combined with brachytherapy, they are planned independently of one another. Standard EBRT strives to deliver homogeneous physical dose to target, ignoring the non‐homogeneous nature of brachytherapy. Here, we take advantage of the difference in dose distribution and the resulting biologic effects from both modalities, and examine an alternative approach to deliver homogeneous biological effect. Specifically, we implement the new methodology to the combination of permanent seed brachytherapy and IMRT for the treatment of prostate cancer.Methods:Prostate cancer treatments consisting of 125I (or 103Pd) implants (110 Gy or 100 Gy, respectively) followed by 45 Gy in 25 IMRT fractions are considered. A biological iso‐effective dose (IED) is calculated from clinically‐derived biologic parameters to determine the desired dose for each voxel within the target and dose constraints for normal tissue. Steep dose gradients are avoided by replacing the voxel doses required by IED with iso‐dosimetric optimization structures for IMRT planning. For sub‐volumes receiving large brachytherapy doses less than 45 Gy physical dose constraints are imposed; similarly, for cool sub‐volumes, more than 45 Gy physical dose constraints are imposed.Results:Retrospective evaluation of patient cases shows that improvement in therapeutic ratio is obtained with the new IED‐based treatment plan optimization. Dose distribution, IED‐volume histograms, and IED‐equivalent DVH for a 45‐fraction IMRT treatment are presented.Conclusion:We describe a new biological optimization‐based treatment planning implementation. IED is calculated (rather than physical dose) while respecting practical aspects of the EBRT plan. The resulting plans predict reduced toxicity to urethra, bladder and rectum. The results require further clinical verification in a larger number of cases. Abstract:Purpose: To develop a Monte Carlo based dose calculation and evaluation toolkit for electronic brachytherapy sources, capable of simulating Intensity Modulated Brachytherapy (IMBT). Material and methods: This toolkit used Monte Carlo code EGS4 to calculate the dose distribution for a treatment in a realistic virtual human phantom converted from the patient's CTimages and contours. An in‐house Matlab program was developed to analyze the dose distribution and generate DVHs and isodoses. Results: The system was benchmarked by comparing the calculated radial dose functions (in water) with experimental data published. Difference was within 3%. A typical intracavitary accelerated partial breast irradiation (APBI) treatment plan using Xoft Axxent electronic brachytherapy source was simulated using this toolkit. DVHs and isodoses revealed that the dose to the ribs were high in this plan due to proximity of balloon to the chest wall and the high absorption coefficient of bone to low energy X‐rays. A simple IMBT plan using partial block could conform the isodose distribution to the target, reduce the dose to ribs and chest wall without compromising the dose homogeneity to the target or increasing the dose to other critical structures. Conclusion: A Monte Carlo based dose calculation and evaluation system was developed for electronic brachytherapy sources. Benchmarked through published data, this system is capable of producing reliable and detailed dose distributions for both isotropic and intensity modulated sources. The feasibility of intensity modulation in improving the plan quality was proved.
A Clinical Monte Carlo Dosimetric Evaluation in Accelerated Partial Breast Electronic Brachytherapy

Abstract:Electronic brachytherapy has been recently utilized in accelerated partial breast irradiation. Deficient tissue inhomogeneity correction in the dose calculation draws physicians' attentions. In this study, a clinical Monte Carlo based dose evaluation was assessed to demonstrate these dosimetric impacts. The dosimetric characteristics of Xoft S700 Axxent X-ray source (Xoft, Fremont, CA) were first evaluated using EGS4 Monte Carlo code to verify the calculation model and source parameter accuracy. A whole body female photographic phantom with defined structures derived from National Library of Medicine's Visible Human Project utilizing all clinical parameters was then used in the dose calculation. A 5cm diameter spherical balloon was inserted in the right breast of the phantom. The volume expanded 1cm from the balloon surface was designed to define as planning target volume (PTV). The prescribed dose per fraction was 3.4 Gy to 5 user-selected points (anterior, posterior, right lateral, superior and inferior) of the PTV. Dose, dose volume histograms of the PTV and normal tissues, isodose distributions in axial, sagittal and coronal planes crossing the center of the balloon were evaluated. Using measured 50 kVp source spectrum provided by Xoft Inc, the simulated radial dose function agrees well with the published data within 3%. After corrected for vender addressed 6% balloon attenuation, the PTV dose reported from the commercial treatment planning system is within 1% compared to our calculation result. The simulation shows 100% PTV covered by 90.6% of the prescription dose with excellent dose uniformity. When accounted for the tissue heterogeneity, changes in the isodose distribution were clearly observed. The two dimensional dose distributions show high skin and rib doses due to photoelectric effect with reasonably acceptable doses to all other surrounding normal tissues. Our preliminary EGS4 Monte Carlo simulation study demonstrates the dosimetric impact in the electronic brachytherapy, especially on the surrounding normal tissues, when tissue inhomogeneity consideration is taken into account.
Assessing cumulative dose distributions in combined external beam radiotherapy and intracavitary brachytherapy for cervical cancer by treatment planning based on deformable image registration

Abstract:BACKGROUND: This study aimed to validate the feasibility of deformable image registration (DIR) in assessing the cumulative dose distributions in combined external beam radiotherapy (EBRT) and intracavitary brachytherapy (ICBT) for cervical cancer.METHODS: This retrospective study included 23 patients with stage IIB disease treated with combined EBRT to the whole pelvis (50.4 Gy in 28 fractions) using an intensity-modulated radiotherapy technique with 6-MV X-ray, followed by three-dimensional (3D) ICBT (28 Gy in 4 fractions). Tumor gross target volume at diagnosis (GTV-Tinit), tumor gross target volume before brachytherapy, high-risk clinical target volume (HR-CTV), intermediate-risk clinical target volume (IR-CTV), and parametrium and organs at risk were recontoured on computed tomography images of EBRT and ICBT, respectively. The dose-volume parameters were also determined. The DIR results were reviewed using MIM Maestro (Reg Review) and modified by function (Reg Refine). To evaluate the accuracy of DIR, DIR-based cumulative dose-volume histogram (DVH) parameters and simple DVH parameter addition were compared using Wilcoxon rank-sum tests.RESULTS: The cumulative dose distributions of EBRT and four ICBT sessions were successfully illustrated using DIR. The mean tumor diameters were 68.35 cm3 at diagnosis and 29.63 cm3 at ICBT initiation. The mean tumor regression was 56.6%. The median minimum dose covering 90% (D90) of HR-CTV, GTV-Tinit, IR-CTV, and parametrium were 69.58±4.94, 68.81±7.98, 59.28±3.78, and 60.97±1.1 Gyα/β=10, respectively, for DIR and 69.11±5.68, 68.49±8.62, 58.89±3.59, and 61±1.49 Gyα/β=10, respectively, with conventional simple DVH parameter addition.No statistically significant differences in dosimetric parameters were observed between the two methods.CONCLUSIONS: Although there were limitations in the DIR accuracy, DIR-based dose accumulation was significantly beneficial in visually showing the cumulative dose distribution in the target area to clinicians in combined radiotherapy for cervical cancer in routine clinical practice.

Abstract:Background and purpose:Coventional radiation has been used for decades,but the 3D dose distribution has not been studied as yet.In this study,different treatment plans of conventional irradiation technique for esophageal carcinoma have been evaluated by 3-D TPS.Methods:Five patients with esophageal carcinoma at upper-thoracic and five patients at middle-thoracic were enrolled in this study.Three conventional treatment plans were created for each patient,with a prescribed dose of 70 Gy.For tumor at upper-thoracic,plan 1 consisted of one anterior field and bilateral posterior oblique fields with wedges,plan 2 consisted of anterior bilateral oblique fields with wedges ,plan 3 consisted of a pair of AP-PA portals,then followed by right anterior oblique portals and left posterior oblique portals to spare spine cord.For tumor at middle thoracic,plan 1 consisted of one anterior field and bilateral posterior oblique fields.Plan 2 consisted of a pair of AP-PA portals and followed by another pair of parallel-opposed lateral off- cord fields as boost.Plan 3 used the same plan as for the lesion at upper-thoracic.The evaluation of each treatment plan was carried out by dose-volume histogram(DVH).Results:For tumor at upper-thoracic,on average,the maximum dose to spinal-cord in plan 3 was (57.1±4.9)Gy,and was higher than that in plan 1 and plan 2.Plan 1 increased mean lung dose from (12.8±2.1)Gy to (18.2±4.1)Gy(P=0.045)compared with plan 2,but it improved the homogenous dose of PTV2,especially in the patient with long tumor.For tumor at middle-thoracic,on average,plan 2 increased mean lung dose from (11.9±1.1)Gy to (13.0±0.6)Gy(P=0.045) compared with plan 3.Plan 2 increased V_(20) and V_(30) from (23.6±2.3)% to (29.2±1.9)%(P=0.004) and (13.9±2.3)% to (20.9±1.3)%(P=0.006) compared with plan 1.The trachea volume of(?)70 Gy(V_(70))in plan 3 was larger than that in plan 1[(20.3±15.9)% VS (10.5±9.8)%,P=0.058].Conclusion:For tumor at upper-thoracic,plan 1 and 2 were superior to plan 3.Irradiation to lung in plan 2 was lower than that in plan 1,but plan 1 improved the homogenous dose of PTV2 compared with plan 2,especially for patient with longer lesion.For tumor at upper-thoracic,plan 2 increased irradiation dose to lung compared with plan 1 and plan 3.Plan 1 was comparable with plan 3,but the dose of trachea in plan 1 may lower than that in plan 3.
Interactive Treatment Planning in High Dose-Rate Brachytherapy for Gynecological Cancer

Abstract:High dose-rate brachytherapy (HDRBT) is widely used for gynecological cancer treatment. Although commercial treatment planning systems (TPSs) have inverse optimization modules, it takes several iterations to adjust planning objectives to achieve a satisfactory plan. Interactive plan-modification modules enable modifying the plan and visualizing results in real time, but they update plans based on simple geometrical or heuristic algorithms, which cannot ensure resulting plan optimality. This project develops an interactive plan optimization module for HDRBT of gynecological cancer. By efficiently solving an optimization problem in real time, it allows a user to visualize a plan and interactively modify it to improve quality. We formulated an optimization problem with an objective function containing a weighted sum of doses to normal organs subject to user-specified target coverage. A user interface was developed that allows a user to adjust organ weights using scroll bars. With a simple mouse click, the optimization problem is solved in seconds with a highly efficient alternating-direction method of multipliers and a warm start optimization strategy. Resulting clinically relevant D2cc of organs are displayed immediately. This allows a user to intuitively adjust plans with satisfactory quality. We tested the effectiveness of our development in cervix cancer cases treated with a tandem-and-ovoid applicator. It took a maximum of 3 seconds to solve the optimization problem in each instance. With interactive optimization capability, a satisfactory plan can be obtained in <1 min. In our clinic, although the time for plan adjustment was typically <10min with simple interactive plan modification tools in TPS, the resulting plans do not ensure optimality. Our plans achieved on average 5% lower D2cc than clinical plans, while maintaining target coverage.

A new radiobiology-based HDR brachytherapy treatment planning algorithm used to investigate the potential for hypofractionation in cervical cancer

Abstract:Most commercially available treatment planning systems for brachytherapy operate based on physical dose and do not incorporate fractionation or tissue-specific response. The purpose of this study is to investigate the potential for hypofractionation in HDR brachytherapy, thereby reducing the number of implants required. A new treatment planning algorithm was built in order to optimize based on tissue and fractionation specific parameters. Different fractionation schemes were considered for 6 patients, and plans were created using the new algorithm. A baseline fractionation scheme consisting of 5 fractions was compared to hypofractionated plans of 1 to 4 fractions. The effectiveness of each plan was evaluated using radiobiological criteria taken from GEC-ESTRO guidelines. The results of this study indicate that an optimization algorithm based on biological parameters has similar functionality to traditional planning methods with the additional ability to account for fractionation effects. Using this algorithm, it was shown that plans consisting of 3 and 4 fractions have comparable target coverage with equivalent normal tissue exposure. In some specific cases, further fractionation may present acceptable target coverage as well.

Improvement of accumulated dose distribution in combined cervical cancer radiotherapy with deep learning-based dose prediction

Abstract:Purpose: Difficulties remain in dose optimization and evaluation of cervical cancer radiotherapy that combines external beam radiotherapy (EBRT) and brachytherapy (BT). This study estimates and improves the accumulated dose distribution of EBRT and BT with deep learning-based dose prediction. Materials and methods: A total of 30 patients treated with combined cervical cancer radiotherapy were enrolled in this study. The dose distributions of EBRT and BT plans were accumulated using commercial deformable image registration. A ResNet-101-based deep learning model was trained to predict pixel-wise dose distributions. To test the role of the predicted accumulated dose in clinic, each EBRT plan was designed using conventional method and then redesigned referencing the predicted accumulated dose distribution. Bladder and rectum dosimetric parameters and normal tissue complication probability (NTCP) values were calculated and compared between the conventional and redesigned accumulated doses. Results: The redesigned accumulated doses showed a decrease in mean values of V50, V60, and D2cc for the bladder (-3.02%, -1.71%, and -1.19 Gy, respectively) and rectum (-4.82%, -1.97%, and -4.13 Gy, respectively). The mean NTCP values for the bladder and rectum were also decreased by 0.02‰ and 0.98%, respectively. All values had statistically significant differences (p < 0.01), except for the bladder D2cc (p = 0.112). Conclusion: This study realized accumulated dose prediction for combined cervical cancer radiotherapy without knowing the BT dose. The predicted dose served as a reference for EBRT treatment planning, leading to a superior accumulated dose distribution and lower NTCP values.

Abstract:Objective To compare the gross tumor volume (GTV) and dose distributions for organs at risk (OAR) between external beam radiation therapy (EBRT) and low-dose rate (LDR) brachytherapy in the treatment of cervical vertebral metastases,in order to provide the basis for clinical treatment.Methods A total of 10 patients were randomly selected.Three-dimensional conformal radiotherapy (3D-CRT) planning was a single forth one with two oblique radiation fields.Intensity modulated radiotherapy (IMRT) planning included five equal radiation fields,and 2-arc volumetric-modulated arc radiotherapy (VMAT) was adopted.The images from Oncentra treatment planning system (TPS) were delivered to Prowess TPS to formulate LDR brachytherapy pre-operative simulation plan.Results For the same dose of spinal cord,the biologically effective dose (BED) of D90 and D95 of LDR were (184.15 ±25.84) Gy and (161.45 ±26.35) Gy.When the fraction scheme was 2 Gy/f,the D95 BED of VMAT,IMRT and 3D-CRT were (120.52 ±10.66) Gy,(119.19 ± 10.91) Gy and (96.24 ±28.65) Gy.When the fraction scheme was 3 Gy/f,the D95 BED 3 Gy/f of VMAT,IMRT and 3D-CRT were (122.19 ± 7.54) Gy,(122.37 ± 8.19) Gy and (95.94 ± 24.70) Gy.All parameters complied with the normal distribution,P =0.00 after paired t test.There was no statistical difference in V5 of the mucosa among VMAT,IMRT and LDR (P =0.16,0.34),and V5 of the mucosa of 3D-CRT was higher than that in LDR (P =0.00).The V30 and D of LDR were less than those of 3D-CRT,VMAT and IMRT,statistically (P =0.00).Other parameters of the thyroid gland,parotid gland and mandible were significantly different between LDR and 3D-CRT,VMAT and IMRT (P =0.00).Conclusion When the fraction schemes were 2 Gy/f and 3 Gy/f,3D-CRT,IMRT and VMAT had lower GTV doses than LDR,which could protect the OARs at the same time.Therefore,as to the cervical vertebral metastases after EBRT,LDR is a good choice for salvage treatment.

Abstract:Objective: This study aimed to investigate the efficacy and safety of 3-dimensional printing noncoplanar template (3D-PNCT)-assisted computed tomography (CT)-guided high-dose-rate interstitial brachytherapy (HDR-ISBT) for reirradiation of pelvic recurrent cervical carcinoma after external beam radiotherapy. Methods: From January 2019 to August 2023, 45 eligible patients were enrolled in this prospective cohort. All patients underwent 3D-PNCT-assisted CT-guided HDR-ISBT with a prescribed dose of 4-7 Gy/fraction to the high-risk clinical target volume (HR-CTV) over 3-8 fractions, either for curative or palliative purposes. The primary endpoints were local progression-free survival (LPFS) and tumor response rate (TRR). The secondary outcome measures included overall survival (OS), toxicities, and symptom resolution. Results: Forty-five patients received 261 fractions of 3D-PNCT-assisted HDR-ISBT. Twenty-nine patients had isolated pelvic recurrence, and 16 patients had simultaneous extra-pelvic or distant recurrences. The TRR was 66.7%. The 2- and 5-year LPFS rates were 30.0% and 25.7%, respectively. The median OS was 23.2 months, and 2- and 5-year OS rates were 49.5% and 34.0%, respectively. The multivariate analysis indicated that squamous cell carcinoma, radical surgery, recurrence-free interval≥12 months, tumor diameter, pelvic recurrence type, and HR-CTV D90≥45 Gy were independent factors influencing LPFS (all p<0.05). D100≥21 Gy, V100≥83%, and V150≥45% were associated with better LPFS (all p<0.05). Tumor diameter and metastasis were independent predictive factors for OS (all p<0.05). The pain relief rate was 66.7% (10/15). Grade 3-4 toxicities occurred in 20.0% of patients. Conclusion: 3D-PNCT-assisted HDR-ISBT for reirradiation of recurrent cervical cancer proved to be an effective and safe alternative to radical surgery.

Abstract:Background and purpose: Ir192 vaginal brachytherapy (IBT) is commonly used for patients with postoperative endometrial cancer (EC). We devised a novel multichannel vaginal applicator that could be equipped with an electronic brachytherapy (EBT) device. We aimed to explore the differences in physical parameters between the EBT and IBT. Materials and methods: This retrospective study included 20 EC patients who received adjuvant IBT from March 1, 2023, to May 1, 2023. Multichannel vaginal cylinders were used, and three-dimensional plans were generated. We designed an electronic multichannel vaginal applicator model and simulated a three-dimensional EBT plan. In order to ensure comparability, D90 of the CTV for the EBT plan was normalized to be equivalent to that of the IBT plan for the same patient. Results: Twenty EBT plans were compared with 20 IBT plans. Results showed, the mean D90 value of clinical target volume (CTV) was 536.1 cGy for both treatment plans. For the mean dose of CTV, the EBT was significantly greater (738.3 vs. 684.3 cGy, p = 0.000). There was no significant difference in CTV coverage between the EBT and IBT plans. For high-dose areas (V200% and V150%), the EBTs were significantly greater. There were no significant differences in the maximum doses to the vaginal mucosa between the EBT and IBT, whether at the apex or in the middle segment. For the bladder and rectum, both the low-dose area and high-dose area were significantly lower in the EBT plans. For the conformity index, there was no significant difference between the EBT and IBT plans. For the dose homogeneity index, the EBT value was lower. Conclusion: In conclusion, under the premise of a three-dimensional brachytherapy plan, for patients receiving multichannel vaginal applicator brachytherapy, compared with IBT, EBT could reduce the dose to the surrounding organs at risk while maintaining the dose in the target area.
Image guided radiation therapy boost in combination with high-dose-rate intracavitary brachytherapy for the treatment of cervical cancer

Abstract:PURPOSE: The purpose of this study was to demonstrate the dosimetric and clinical feasibility of image guided radiation therapy (IGRT) combined with high-dose-rate (HDR) intracavitary brachytherapy (ICBT) to improve dose distribution in cervical cancer treatment.MATERIAL AND METHODS: For 42 cervical cancer patients, magnetic resonance imaging (MRI) scans were acquired after completion of whole pelvic irradiation 45-46 Gy and 5 fractions of B + I (ICBT + IGRT) treatment were subsequently received. The high risk clinical target volume (HRCTV), intermediate risk clinical target volume (IRCTV), bladder, rectum, and sigmoid were contoured on the computed tomography (CT) scans. The total planning aim doses for HRCTV was D90% > 85 Gy, whilst constraints for rectum and sigmoid were D2cc < 75 Gy and D2cc < 90 Gy for bladder in terms of an equivalent dose in 2 Gy (EQD2) for external beam radiotherapy (EBRT) and brachytherapy boost. The IGRT plan was optimized on top of the ICBT dose distribution. A dosimetric comparison was made between B + I and optimized ICBT (O-ICBT) only.RESULTS: The mean D90% of HRCTV was comparable for B + I and O-ICBT (p = 0.82). For B + I plan, HRCTV D100%, IRCTV D100%, and IRCTV D90% were significantly increased by a mean of 10.52 Gy, 5.61 Gy, and 2.70 Gy, respectively (p < 0.01). The D2cc for bladder, rectum, and sigmoid were lower by a mean of 21.36, 6.78, and 10.65 Gy, respectively (p < 0.01). The mean rectum V60 Gy value over 42 patients was almost the same for both techniques but for bladder and sigmoid B + I had higher V60 Gy mean values as compared with the O-ICBT.CONCLUSIONS: B + I can improve dose distribution in cervical cancer treatment; it could be useful for tumors extended beyond the reach of intracavitary/interstitial brachytherapy (IC/ISBT) or for centers that are inexperienced or ill-equipped with IC/ISBT techniques. Additional confirmatory prospective studies with larger numbers of patients and longer follow-up are required to validate the durability.

Abstract:OBJECTIVES: Some patients are medically unfit for or averse to undergoing a brachytherapy boost as part of cervical cancer radiotherapy. In order to be able to definitively treat these patients, we assessed whether we could achieve a boost plan that would mimic our brachytherapy plans using external beam radiotherapy.METHODS: High dose rate brachytherapy plans of 20 patients with stage IIB cervical cancer treated with definitive chemoradiotherapy were included in this study. Patients had undergone computer tomography (CT) simulations with tandem and ovoids in place. Point "A" dose was 600-700 cGy. We attempted to replicate the boost dose distribution from brachytherapy plans using intensity-modulated radiotherapy (Varian Medical Systems, Palo Alto, CA, USA), volumetric modulated arc therapy (Rapid Arc, Varian Medical Systems, Palo Alto, CA, USA), or TomoTherapy (Accuray, Inc., Sunnyvale, CA, USA) with the brachytherapy 100% isodose line as our target. Target coverage, normal tissue dose, and brachytherapy point doses were compared with ANOVA. Two-sided p-values ≤0.05 were considered significant.RESULTS: External beam plans had excellent planning target volume (PTV) coverage, with no difference in mean PTV V95% among planning techniques (range 98-100%). External beam plans had lower bladder Dmax, small intestine Dmax, and vaginal mucosal point dose than brachytherapy plans, with no difference in bladder point dose, mean bladder dose, mean small intestine dose, or rectal dose. Femoral head dose, parametria point dose, and pelvic sidewall point dose were higher with external beam techniques than brachytherapy.CONCLUSIONS: External beam plans had comparable target coverage and potential for improved sparing of most normal tissues compared to tandem and ovoid brachytherapy.

Applications of Tissue Heterogeneity Corrections and Biologically Effective Dose Volume Histograms in Assessing the Doses for Accelerated Partial Breast Irradiation Using an Electronic Brachytherapy Source

Abstract:A low-energy electronic brachytherapy source (EBS), the model S700 Axxent x-ray device developed by Xoft Inc., has been used in high dose rate (HDR) intracavitary accelerated partial breast irradiation (APBI) as an alternative to an Ir-192 source. The prescription dose and delivery schema of the electronic brachytherapy APBI plan are the same as the Ir-192 plan. However, due to its lower mean energy than the Ir-192 source, an EBS plan has dosimetric and biological features different from an Ir-192 source plan. Current brachytherapy treatment planning methods may have large errors in treatment outcome prediction for an EBS plan. Two main factors contribute to the errors: the dosimetric influence of tissue heterogeneities and the enhancement of relative biological effectiveness (RBE) of electronic brachytherapy. This study quantified the effects of these two factors and revisited the plan quality of electronic brachytherapy APBI. The influence of tissue heterogeneities is studied by a Monte Carlo method and heterogeneous 'virtual patient' phantoms created from CT images and structure contours; the effect of RBE enhancement in the treatment outcome was estimated by biologically effective dose (BED) distribution. Ten electronic brachytherapy APBI cases were studied. The results showed that, for electronic brachytherapy cases, tissue heterogeneities and patient boundary effect decreased dose to the target and skin but increased dose to the bones. On average, the target dose coverage PTV V(100) reduced from 95.0% in water phantoms (planned) to only 66.7% in virtual patient phantoms (actual). The actual maximum dose to the ribs is 3.3 times higher than the planned dose; the actual mean dose to the ipsilateral breast and maximum dose to the skin were reduced by 22% and 17%, respectively. Combining the effect of tissue heterogeneities and RBE enhancement, BED coverage of the target was 89.9% in virtual patient phantoms with RBE enhancement (actual BED) as compared to 95.2% in water phantoms without RBE enhancement (planned BED). About 10% increase in the source output is required to raise BED PTV V(100) to 95%. As a conclusion, the composite effect of dose reduction in the target due to heterogeneities and RBE enhancement results in a net effect of 5.3% target BED coverage loss for electronic brachytherapy. Therefore, it is suggested that about 10% increase in the source output may be necessary to achieve sufficient target coverage higher than 95%.

Abstract:Abstract Background To assess the intra-fractional dosimetric variations of image-guided brachytherapy of cervical cancer. Methods A total of 38 fractions (9 patients) undergoing brachytherapy for cervical cancer underwent a CT scanning for treatment planning (planning CT) and a Cone-beam CT (CBCT) scanning immediately prior to delivery (pre-delivery CBCT). The variations of volumes as well as the dosimetric impact from treatment planning to delivery (intra-application) were evaluated. The dose volume histogram parameters including volume, D90 of high-risk clinical target volume (HRCTV) and D2cc of organs at risk (OARs) were recorded. Results The relative differences (mean ± 1SD) of the volume and D90 HRCTV across the two scans were − 2.0 ± 3.3% and − 1.2 ± 4.5%, respectively. The variations of D2cc for bladder, rectum, sigmoid and small intestine are − 0.6 ± 17.1%, 9.3 ± 14.6%, 7.2% ± 20.5% and 1.5 ± 12.6%, respectively. Most of them are statistically nonsignificant except the D2cc for rectum, which showed a significant increase ( P = 0.001). Using 5% and 10% uncertainty of physical dose for HRCTV at a 6 Gy × 5 high-dose-rate schedule, the possibility of total equivalent doses in 2 Gy fractions (EQD2) lower than 85 Gy is close to 0% and 3%, respectively. Performing similar simulation at 15% and 20% uncertainty of a 4 Gy physical dose for OARs, the possibility of total EQD2 dose exceeding 75 Gy is about 70%. Less than 1% of the total EQD2 of OARs would exceed 80 Gy. Conclusions Average intra-fractional dosimetric variation of HRCTV was small in an interval of less than 1 h, and the possibility of total EQD2 exceeding 85 Gy is higher than 97%. The intra-fractional dosimetric variations of OARs might result in an overdose for OARs in a single fraction or the whole treatment. It is necessary to detect unfavorable anatomical changes by re-imaging and take interventions to minimize applied doses and reduce the risk of complications.

Abstract:Purpose The ICRU/GEC-ESTRO released the ICRU Report No. 89, which introduced the concept of four-dimensional brachytherapy and ushered in a new era of brachytherapy for cervical cancer. The purpose of this study was to evaluate the local control and late toxicity of four-dimensional brachytherapy in cervical cancer through a systematic review and to reveal the dose-response relationship between the volumetric dose paraments and the local control rate via a probit model. Material and Methods We identified studies that reported the HR-CTV D90 and local control probabilities by searching the PubMed Database, the Web of Science Core Collection and the Cochrane Library Database through February 1st, 2022. Regression analyses were performed between the HR-CTV D90 and the local control probability using a probit model. Results Nineteen studies enrolling 3,616 patients were included. The probit model showed a significant relationship between the HR-CTV D90 value and IR-CTV D90 Vs. the local control probability, P < 0.001 and P = 0.003, respectively. The D90 for HR-CTV and IR-CTV corresponding to a probability of 90% local control was 79.1 GyEQD2,10 (95% CI:69.8 – 83.7 GyEQD2,10) and 66.5 GyEQD2,10 (95% CI: 62.8 - 67.9 GyEQD2,10), respectively. The limits for the prescribed dose of 85 GyEQD2,10 for HR-CTV D90 theoretically warranted a 92.1% (95% CI: 90.2% - 95.3%) local control rate, and 87.2% (95% CI: 82.4% - 91.8%) local control probability was expected for 65 GyEQD2,10 to IR-CTV D90. The probit model showed no significant relationship between the D2cc to organs at risk and the probability of grade 3 and above gastrointestinal or genitourinary toxicity. Conclusions Four-dimensional brachytherapy takes into account uncertain factors such as tumour regression, internal organ motion and organ filling, and provides a more accurate and more therapeutic ratio delivery through adaptive delineation and replanning, replacement of the applicator, and the addition of interstitial needles. The dose volume effect relationship of four-dimensional brachytherapy between the HR-CTV D90 and the local control rate provides an objective planning aim dose.
