Abstract:In China, acute stigma accompanying an HIV diagnosis can lead to self-isolation. In a cultural setting where family relationships are highly valued and contribute critically to well-being, such self-isolation can thwart HIV self-management and engagement in medical care, and so heighten risk for health disparities. To understand this phenomenon, we conducted individual in-depth interviews with 34 persons living with HIV/AIDS (PLwHA) in Shanghai and Beijing. Inductive content analysis revealed a range of forms of self-isolation motivation, beliefs, and behaviors influenced by: 1) internalized stigma and desire to avoid discrimination; 2) HIV-related factors such as HIV knowledge and disease progression; and 3) familial factors such as a sense of responsibility and family members' reactions. Based on a proposed framework centering on dialectical family influences (whereby PLwHA are pushed away from, yet pulled toward the family fold), implications for provision of multidisciplinary care in medical settings are considered, including culturally appropriate strategies to decrease health disparities.
Collectivism culture, HIV stigma and social network support in Anhui, China: a path analytic model.

Abstract:HIV stigma is rooted in culture and, therefore, it is essential to investigate it within the context of culture. The objective of this study was to examine the interrelationships among individualism-collectivism, HIV stigma, and social network support. A social network study was conducted among 118 people living with HIVAIDS in China, who were infected by commercial plasma donation, a nonstigmatized behavior. The Individualism-Collectivism Interpersonal Assessment Inventory (ICIAI) was used to measure cultural norms and values in the context of three social groups, family members, friends, and neighbors. Path analyses revealed (1) a higher level of family ICIAI was significantly associated with a higher level of HIV self-stigma (β=0.32); (2) a higher level of friend ICIAI was associated with a lower level of self-stigma (β=-035); (3) neighbor ICIAI was associated with public stigma (β=-0.61); (4) self-stigman was associated with social support from neighbors (β=-0.27); and (5) public stigma was associated with social support from neighbors (β=-0.24). This study documents that HIV stigma may mediate the relationship between collectivist culture and social network support, providing an empirical basis for interventions to include aspects of culture into HIV intervention strategies.

Abstract:Natural partial structures of lipopolysaccharide (LPS) as well as synthetic analogues and derivatives of lipid A were compared with respect to inhibit the binding of 125I-labelled Re-chemotype LPS to mouse macrophage-like J774.1 cells and to induce cytokine-release in J774.1 cells. LPS, synthetic Escherichia coli-type lipid A (compound 506) and tetraacyl precursor Ia (compound 406) inhibited the binding of 125I-LPS to macrophage-like J774.1 cells and induced the release of tumor necrosis factor alpha (TNF alpha) and interleukin 6 (IL-6). Deacylated R-chemotype LPS preparations were completely inactive in inhibiting binding and in inducing cytokine-release. Among tetraacyl compounds, the inhibition-capacity of LPS-binding was in decreasing order: PE-4 (alpha-phosphonooxyethyl analogue of 406) > 406 >> 404 (4'-monophosphoryl partial structure of 406) > 405 (1-monophosphoryl partial structure of 406). In the case of hexaacyl preparations, compounds 506, PE-1 (alpha-phosphonooxyethyl analogue of 506) and PE-2 (differing from PE-1 in having 14:0 at positions 2 and 3 of the reducing GlcN) inhibited LPS-binding and induced cytokine release equally well, whereas preparation PE-3 (differing from PE-2 in containing a beta-phosphonooxyethyl group) showed a substantially lower capacity in binding-inhibition and cytokine-induction. The conclusion is that chemical changes in the hydrophilic lipid A backbone reduce the capacity of lipid A to bind to cells, whereas the number of fatty acids determines the capacity of lipid A to activate cells. These results indicate that the bisphosphorylated hexosamine backbone of lipid A is essential for specific binding of LPS to macrophages and that the acylation pattern plays a critical role for LPS-promoted cell activation, i.e. cytokine induction.

Abstract:Underpinned by the notion that communication equality is crucial in developing communication infrastructures for health and well-being, this study explores experiences of COVID-19 information retrieving in a low-income suburban area in Aotearoa from a culture-centered lens. Drawing from in-depth interviews with ethnic minority residents, we reveal two polarizing experiences: at one end, residents were confident in the government's representation of COVID-19 but also fearful and anxious, while at the other end, residents were skeptical of formal institutions and expressed alternative views about COVID-19. The findings illuminate how community distrust is intertwined with communication inequality, which can further entrench and magnify health inequality, informing key recommendations for culturally centering pandemic communications and creating infrastructures to challenge disinformation.

Developing an innovative cross-cultural strategy to promote HIV/AIDS prevention in different ethnic cultural groups of China

Abstract:The HIV and STIs epidemic in China has had a significant impact among China's ethnic minorities. However, the official traditional approach, which has used an anti-epidemic social campaign, has not paid any attention to the diversity of cultural backgrounds of the many ethnic minority groups. This study carried out in Sichuan Province is the first to explore how to use cultural resources for developing an effective strategy for promoting HIV prevention in different cultural groups in China. One hundred and fifty male volunteers drawn from the Yi (50), Tibetan (50) and majority Han (50) cultural groups were assigned to a direct training programme. After training, these participants spread safe sex messages to other contacts who became an indirect peer diffusion group. A third group of 150 male volunteers from the same three cultural groups but from another relatively comparable community acted as controls. Each participant was interviewed before and after the intervention to assess knowledge, attitudes and behavioural intentions regarding HIV/AIDS prevention. The study examined the cultural appropriateness and effectiveness of peer-led health message diffusion in promoting condom use through a traditional oral communication approach from the direct training groups to the indirect intervention groups and broad peer networks within the Yi, Tibetan and Han cultural communities. Key findings showed that the peer-based oral communication strategy was effective for encouraging condom use with casual sexual partners in both the direct training group and the indirect peer diffusion group in all three cultural groups. There was no significant change in any of the comparison groups. Although change in the majority Han cultural group was generally greater than in the ethnic minority groups, the results clearly suggest that the methods can be successfully adopted to promote safe sexual behaviour in different cultural groups of China.

Abstract:Bisexual behaviours are relatively common among men who have sex with men in China. This pilot study aims to reveal the complex processes through which such men manage their sexuality, family responsibilities and sexual behaviours in a rural Chinese setting. A total of 15 men who have sex with men were recruited by purposive sampling. Face-to-face in-depth interviews were conducted to explore participants’ views about their sexual experiences and practices. The Chinese traditional moral code, family values and gender roles that form the crucial components of Confucianism were reflected in the participants’ efforts to maintain familial and social harmony through a compromised form of sexual partnership. Most study participants demonstrated a mixed experience of social stigma, sexual naiveté and ignorance of HIV and sexually transmitted infections (STIs). Under cultural and family pressure, men who have sex with men entered heterosexual marriages with the intention of maintaining a balance between their collectivist (familial) obligations and their individualistic (same-sex sexual) desires. However, the opaque nature of their concurrent sexual relationships may endanger their personal health and accelerate HIV and STI transmission. Reducing the stigma and social prejudice associated with male same-sex sexual relations is essential for any culturally sensitive HIV-prevention programme to succeed in rural China.
Culture and Perceptions on Cancer Risk and Prevention, Information Access, and Source Credibility: a Qualitative Interview Study in Chinese Adults

Abstract:We aimed to explore cancer risk and prevention perceptions, information access, and source credibility evaluation about cancer risk and prevention in adults in an Eastern cultural context (China). We conducted 32 in-depth interviews in two Chinese cities: Shanghai and Wuhan, from September 2016 to January 2017. Though not fully supported by scientific evidence, our findings showed that stress was among the top five concerns in the participants’ perceptions of cancer risk and prevention. Obesity and infectious agents, significant cancer risks for Chinese populations, did not receive so much attention among the participants. Principles of Eastern culture, such as change, contradiction, and holism, were reflected in the participants’ interpretations and understandings of cancer risk and prevention. Beliefs in fate or destiny, as associated with Eastern culture, were also apparent in the strongly fatalistic views towards cancer prevention of some participants. Individual responsibility for cancer risk, rather than collective responsibility (such as government or society), was emphasised by participants. Except for personal social networks, the information channels where the participants frequently accessed cancer risk and prevention information (the media) were completely different from the information channels they trusted (healthcare system). Our findings provide useful references for cancer risk and prevention communication and education in China.

Abstract:Ethnic churches attended by first generation Chinese immigrants are uniquely positioned to address emerging HIV prevention and care needs within the Chinese community at-large. Efforts to develop faith-based HIV programs necessitate identifying how HIV intersects with the sinicization of Christianity within Chinese churches. This paper will review the process of contextualizing HIV within theological and cultural frameworks that are meaningful for ethnic Chinese church leaders and members. The authors specifically propose two points of integration between public health and ecclesial functions: (1) HIV stigma-mitigation initiatives as informed by Christo-centric teachings of compassion and justice, and (2) HIV prevention and care reframed as social responsibility and informed by the Christian tradition of evangelism. Systems and practices that hinder and promote the involvement of Chinese churches in HIV prevention, care, and stigma-reduction will be discussed.

Abstract:In China, there are currently an estimated 180,000 women between 16 and 45 years of age living with HIV. However, we know very little about their lived experiences. Given the spread of the AIDS epidemic in China and the burden it exerts on quality of life, there is an urgent need to understand how HIV affects Chinese women, particularly in the context of their marriages. How do they negotiate the extreme stigma of their illness in making decisions about disclosure and social support, especially in the context of their family life? We recruited 26 Chinese women with HIV in Beijing and Shanghai for in-depth interviews employing a phenomenological approach. We examined the process and outcomes of disclosure within the course of the women's search for social support. Women in HIV-discordant relationships often experienced a termination of their marriage after disclosure, yet others exhibited remarkable resilience, finding new strength through the challenge of their illness. Findings underscore the need for accessible and culturally acceptable interventions for Chinese women with HIV who face considerable stigma in their search for support.
Application of mental illness stigma theory to Chinese societies: synthesis and new directions

Abstract:The rapidly-evolving literature concerning stigma towards psychiatric illnesses among Chinese groups has demonstrated pervasive negative attitudes and discriminatory treatment towards people with mental illness. However, a systematic integration of current stigma theories and empirical findings to examine how stigma processes may occur among Chinese ethnic groups has yet to be undertaken. This paper first introduces several major stigma models, and specifies how these models provide a theoretical basis as to how stigma broadly acts on individuals with schizophrenia through three main mechanisms: direct individual discrimination, internalisation of negative stereotypes, and structural discrimination. In Chinese societies, the particular manifestations of stigma associated with schizophrenia are shaped by cultural meanings embedded within Confucianism, the centrality of "face", and pejorative aetiological beliefs of mental illnesses. These cultural meanings are reflected in severe and culturally-specific expressions of stigma in Chinese societies. Implications and directions to advance stigma research within Chinese cultural settings are provided.

Abstract:As the early COVID-19 outbreak sparked xenophobia against people of Asian and Chinese background, we collected data from Chinese migrants worldwide to test how discrimination at a macro-level was perceived by the Chinese during COVID-19 globally. Specifically, we examined (1) whether/how the Chinese migrants were aware of discrimination against their co-nationals during COVID; (2) if so, whether anger was a predominant reaction of these Chinese towards certain exposure to relevant information; (3) how responses of anger transcend across the group of Chinese migrants. Integrating the ecological approach to media and cultural psychology, as well as the intergroup perspective of social psychology, we conducted a study that explored the impact of traditional media exposure to discrimination on collective anger—a process mediated by national identity among the Chinese migrants. Findings provide some evidence that geographically dispersed mono-cultural groups may share or identify with collective emotions when facing xenophobic threats in a macro context. Further examination of cultural distance (between China and the host country) among the Chinese migrants also revealed a particular interaction between host newspaper coverage and cultural distance on national identity. These findings suggest further research to examine the emotional norms of similar cultures bonded via strong collective identities in times of intergroup threat and the theoretical possibility for diasporic identity processes.

Abstract:This paper examines how African immigrants living with HIV negotiate and reconstruct their productive (i.e., educational and career opportunities), sexual, and reproductive identities. We used data from a mixed-methods study to explore how stigma and social networks in which participants were embedded shaped how they understood and negotiated their role expectations and responsibilities. Participants revealed how HIV not only changed their identities and limited their sex life, partner choices, and fundamental decisions about fertility and reproduction, but also presented them with the opportunity to reinvent/reshape their lives. Our analysis revealed that the cultural discourses about illness and HIV in participant's countries of origin, the acculturative and migratory stressors, and the competing influences and expectations from family and friends in their home and host countries shape their illness experience, and how they adjust to life with HIV. This paper builds on sociological understanding of illness experience as a social construct that shapes the ill person's identity, role, and function in society. Specifically, the paper contributes to discourses on how (i) participants' social location and identity (as transnational migrants adjusting to acculturative stressors associated with resettlement into a new country), (ii) cultural discourses about illness and HIV in their countries of origin, and (iii) embeddedness in transnational social networks influence health outcomes, including lived experiences with chronic illnesses and stigmatized conditions such as HIV.

Abstract:Introduction Ethnic minorities are considered one of the most vulnerable groups during the COVID-19 pandemic. However, the explanatory pathway of how their disadvantaged experiences during epidemics are related to the embedded and longstanding stigmas against them and how these embedded stigmas can affect their resilience in disease outbreaks are not well understood. This study investigated the experiences of ethnic minorities in the COVID-19 pandemic, and how their experiences were related to the embedded stigma toward them. Methods This study adopted a qualitative approach, interviewed 25 individuals (13 women and 12 men) from ethnic minority groups residing in Hong Kong from August 2021 to February 2022 in a semi-structured format. Thematic analysis was conducted to analyze the data. Results The participants were isolated and stereotyped as infectious during the COVID-19 pandemic at community and institutional levels. Their experiences did not occur suddenly during the pandemic but were embedded in the longstanding segregation and negative stereotypes toward ethnic minorities in different aspects of life before the pandemic. These negative stereotypes affected their resilience in living and coping with the pandemic. Conclusion The participants’ experiences during the COVID-19 pandemic were mostly disadvantageous and predominantly initiated by the mainstream stigmatization toward them by the local Chinese residents and government. Their disadvantaged experiences in the pandemic should be traced to the embedded social systems, imposing structural disparities for ethnic minorities when accessing social and medical resources during a pandemic. Because of the preexisting stigmatization and social seclusion of ethnic minorities in Hong Kong, the participants experienced health inequality, which stemmed from social inequality and the power differential between them and the Chinese locals. The disadvantaged situation of the participants negatively affected their resilience to the pandemic. To enable ethnic minorities better cope with future epidemics, merely providing assistance to them during an epidemic is barely adequate, but a more supportive and inclusive social system should be established for them in the long run.

Abstract:BACKGROUND:Although knowledge is a crucial component in contact theory delineating how prejudice changes toward out-groups with stigmatized conditions, little is known about the mediating role of knowledge on contact, stigmatizing attitudes, and behaviors toward mental illness.AIM:This study aimed to examine the mechanism underlie contact and stigma change by knowledge.METHODS:A total of 366 participants including family members (FM), mental health providers (MHP), and community residents (CR) recruited across communities in Hong Kong and completed measures of contact level, contact quantity, contact quality, mental health related knowledge, prejudice, and discriminatory behaviors. Structural equation modeling was adopted to test the association among the key variables.RESULTS:Higher level of contact was significantly correlated with better knowledge, less prejudice, and less discriminatory behaviors. Knowledge was directly and negatively correlated with prejudicial attitudes but was not significantly related to discriminatory behaviors. Furthermore, lower levels of prejudice were associated with less discriminatory behaviors.CONCLUSION:Enhancement of contact may increase understanding toward people with mental illness (PMI) and diminish stigmatizing attitudes and behaviors. Although prejudicial attitudes may be reduced by broadening mental health knowledge, increasing knowledge only might not ameliorate discriminatory behaviors. Future research should test mediators on contact and stigma by using longitudinal data.

Abstract:As a highly stigmatized social illness, AIDS has caused tremendous social fears and consequent practices of social exclusion and social discrimination all over the world, simply because it's three widely recognized qualities, that is, high mortality, multi-channel contagious, and very limited chance of being cured. Based on empirical survey data gathered from two specific groupsthe AIDS patients, and the ordinary villagersin two Chinese rural communities, this article gives out a quantitative analysis of the relationship between local AIDS epidemiology and level of social discrimination against AIDS patients and their family members within local communities. The author not only presents descriptive data of the forms and levels of locally embedded social discrimination against AIDS victims, but also brings out two regression models to explain levels of social discrimination that reported by AIDS patients and the ordinary villagers respectively. In the first two parts of this article, the author has traced relevant literatures regarding two topics: first, the rise of AIDS as a social illness and the responses of global social sciences towards it; and then, the current status of Chinese academic efforts on AIDS.
Culture, Contraception, and Colorblindess: Youth Sexual Health Promotion as a Gendered Racial Project

Abstract:Feminist scholars have identified how race and gender discourses influence the creation and implementation of school-based sexual health education and the provision of health care, yet there are few studies that examine how race and gender work in sexual health promotion as it occurs through community-based public health efforts. Drawing on three years of ethnographic research in a low-income Puerto Rican community, this article demonstrates how a gendered racial project of essentializing Latinx culture surrounding young women’s sexuality and reproduction works to both obscure and reinforce race and racism in sexual health promotion. Professional stakeholders mobilize culture as an explanation for high birth rates among young Latinas in the city and reproduce a “Latino culture narrative” in which Latina gender and sexuality is understood as deterministic and homogenous. Simultaneously, an ideology of colorblindness enables the uncritical promotion of long-acting reversible contraception and obscures the history of reproductive oppression experienced by women of color. I consider how colorblindness and culture narratives allow stakeholders to abdicate responsibility for gendered racial inequality and conclude by advocating for the incorporation of racial and reproductive justice frameworks in sexual health promotion.
Male and female Asians living with HIV: a text-mining analysis comparing their concerns

Abstract:Introduction: Asians are "a forgotten population" whose HIV prevention and treatment needs have long been ignored. Studies on people living with HIV/AIDS (PLWHA) have primarily reported on physical and psychological conditions among men and gender minorities. Methods: We used data-mining to select words and word patterns from pooled in-depth interviews conducted with 33 women and 12 men ( n = 45) who were HIV positive and residing in San Francisco, United States, Shanghai, Beijing, or Taipei, China. We analyzed gender-based data (male vs. female) within the participant responses. Results: Disclosure of HIV serostatus was discussed by both genders of PLWHA. Participants were concerned whether they should disclose their diagnosis and how to disclose it to their family members. Specifically, for women, family relationships and financial burdens were discussed most often. In terms of men, HIV disclosure was the key concern followed by disclosure of their sexual preference and concerns over what people were saying about them in the community. Conclusion: This project compared and contrasted concerns of Asian HIV-positive men and women. As healthcare providers promoting self-management by HIV-positive individuals of both genders, it is important to consider that there may be significant differences between them. Abstract:BACKGROUND The finding that most people with alcoholism are also heavy smokers prompted several research groups to evaluate the effects of ethanol on neuronal nicotinic acetylcholine receptor (nAChR) function. Data collected in vitro indicate that physiologically relevant concentrations of ethanol inhibit the functional activation of homomeric alpha7 nAChRs, which are one of the most abundant nAChR subtypes expressed in the mammalian brain. The studies outlined here used alpha7 gene knockout (null mutant) mice to evaluate the potential role of alpha7 nAChRs in modulating selected behavioral and physiological effects produced by ethanol. METHODS Current evidence indicates that many responses to ethanol are not genetically correlated. Therefore, the authors measured the effects of acute administration of ethanol on several behaviors that are altered by both ethanol and nicotine: two tests of locomotor activity, acoustic startle, prepulse inhibition of acoustic startle, and body temperature. Ethanol-induced durations of loss of righting reflex and ethanol elimination rates were also determined. These studies used null mutant (alpha7(-/-)) and wild-type (alpha7(-/-)) mice. RESULTS Relative to alpha7(+/+) mice, alpha7(-/-) mice were more sensitive to the activating effects of ethanol on open-field activity, ethanol-induced hypothermia, and duration of loss of the righting response. Deletion of the alpha7 gene did not influence the effects of ethanol on Y-maze crossing or rearing activities, acoustic startle, or prepulse inhibition of startle. Gene deletion did not alter ethanol metabolism. CONCLUSIONS These results indicate that some but not all of the behavioral effects of ethanol are mediated in part by effects on nAChRs that include the alpha7 subunit and may help to explain the robust association between alcohol consumption and the use of tobacco.
