Abstract:Dermatomycosis of the hair, skin, or nails is one of the most common fungal infections worldwide. Beyond permanent damage to the affected area, the risk of severe dermatomycosis in immunocompromised people can be life-threatening. The potential risk of delayed or improper treatment highlights the need for a rapid and accurate diagnosis. However, with traditional methods of fungal diagnostics such as culture, a diagnosis can take several weeks. Alternative diagnostic technologies have been developed which allow for an appropriate and timely selection of an antifungal treatment, preventing nonspecific over-the-counter self-medication. Such techniques include molecular methods, such as polymerase chain reaction (PCR), real-time PCR, DNA microarray, next-generation sequencing, in addition to matrix-assisted laser desorption ionization-time of flight (MALDI-TOF) mass spectrometry. Molecular methods can help close the 'diagnostic gap' observed with traditional cultures and microscopy and allow for a rapid detection of dermatomycosis with increased sensitivity and specificity. In this review, advantages and disadvantages of traditional and molecular techniques are discussed, in addition to the importance of species-specific dermatophyte determination. Finally, we highlight the need for clinicians to adapt molecular techniques for the rapid and reliable detection of dermatomycosis infections and to reduce adverse events.
Comparison of an automated DNA extraction and 16S rDNA real time PCR/sequencing diagnostic method using optimized reagents with culture during a 15-month study using specimens from sterile body sites

Abstract:Background 16S rDNA-PCR for the identification of a bacterial species is an established method. However, the DNA extraction reagents as well as the PCR reagents may contain residual bacterial DNA, which consequently generates false-positive PCR results. Additionally, previously used methods are frequently time-consuming. Here, we describe the results obtained with a new technology that uses DNA-free reagents for automated DNA extraction and subsequent real time PCR using sterile clinical specimens. Results In total, we compared 803 clinical specimens using real time PCR and culturing. The clinical specimens were mainly of orthopedic origin received at our diagnostic laboratory. In 595 (74.1%) samples, the results were concordant negative, and in 102 (12.7%) the results were concordant positive. A total of 170 (21.2%) clinical specimens were PCR-positive, of which 62 (36.5% from PCR positive, 7.7% in total) gave an additional benefit to the patient since only the PCR result was positive. Many of these 62 positive specimens were strongly positive based on crossingpoint values (54% < Cp 30), and these 62 positive clinical specimens were diagnosed as medically relevant as well. Thirty-eight (4.2%) clinical specimens were culture-positive (25 of them were only enrichment culture positive) but PCR-negative, mainly for S. epidermidis , S. aureus and C. acnes . The turnaround times for negative specimens were 4 hours (automated DNA extraction and real time PCR) and 1 working day for positive specimens (including Sanger sequencing). Melting-curve analysis of SYBR Green-PCR enables the differentiation of specific and unspecific PCR products. Using Ripseq, even mixed infections of 2 bacterial species could be resolved. Conclusions For endocarditis cases, the added benefit of PCR is obvious. The crucial innovations of the technology enable timely reporting of explicit reliable results for adequate treatment of patients. Clinical specimens with truly PCR-positive but culture-negative results represent an additional benefit for patients. Very few results at the detection limit still have to be critically examined.

Evaluation of a Liquid Media MALDI-TOF MS Protocol for the Identification of Dermatophytes Isolated from Tinea capitis Infections

Abstract:One of the most common types of tinea is the superficial infection of the hair and scalp area known as tinea capitis. It is responsible for frequent outbreaks in nurseries and schools and represents a global health problem. Correct identification of the infection agent is essential in the determination of the infection source, epidemiological course, and treatment initiation. The conventional identification methods (direct exam, culture, DNA sequencing) are time-consuming, require experienced staff, are time-consuming, and the latter is expensive for routine identifications. Matrix-assisted laser desorption ionization time-of-flight mass spectrometry (MALDI-TOF MS) is gaining new ground for routine identification of filamentous fungi. The main advantages of MALDI-TOF MS are its rapid and accurate identification capability, relatively low cost, and easy integration into the laboratory routine. Its accuracy heavily depends on the quality of the reference spectra database. Identification of clinical isolates with MALDI-TOF MS protocol requires a sub-culturing step to ensure reliable identification. It can take days to weeks before fungal growth appears on solid medium. In this study, a unique MALDI-TOF MS protocol using liquid cultures of dermatophyte species was developed in order to shorten the turnaround time for the culture and identification of clinical isolates. Material and Method A standard MALDI-TOF MS protocol was adapted for liquid instead of solid cultures. Three different databases were tested. Results Using the liquid media MALDI-TOF MS protocol, a global rate of 62% correct identification (RCI) was obtained, compared with 87% for the protocol based on solid cultures. Trichophyton tonsurans was not correctly identified in all isolates using liquid cultures, with 88% of the isolates misidentified as Trichophyton interdigitale. The turnaround time for primary isolates for the solid and liquid protocols were respectively 11.7 and 11.6 days (no significant difference between both methods (p = 0.96)). Conclusions The newly designed liquid MALDI-TOF MS protocol did not lead to a significantly shorter turnaround time for the identification of dermatophytes isolated from tinea capitis infections. The turnaround time for the method with primary isolates was not significantly lower, and the rate of correct identification decreased remarkably, which emphasizes the need for a sub-culturing step. Using different database did not lead to improvement in turnaround time or rate of correct identification. This study highlights the importance of the medium and the reference database when performing MALDI-TOF MS.

Abstract:An accurate diagnosis of tinea unguium is necessary for the selection of antimycotics and successful treatment. To rapidly and accurately identify the aetiological agents causing tinea unguium, we improved upon the conventional boiling method for DNA extraction and developed a novel real-time PCR detection system that includes two assays. The two assays, based on the amplification of ribosomal internal transcribed spacer regions and 28S rDNA, were designed to detect pan-dermatophyte and Trichophyton rubrum , respectively. The analytical sensitivities of both assays permitted the detection of ten copies of plasmid DNA template. The analytical specificity of the detection system was confirmed using 11 dermatophyte strains and 25 non-dermatophyte strains. In total, 165 nail specimens were examined by microscopy, culture, conventional PCR, and the novel real-time PCR method. Real-time PCR gave positive results in 47.3 % of the specimens (78), a rate exceeding those obtained using microscopy (72, 43.6 %), conventional PCR (69, 41.8 %), and culture (49, 29.7 %). All conventional PCR-positive specimens were detected by real-time PCR, and real-time PCR detected nine specimens that were missed by conventional PCR. The results from latent class analysis, and further calculations, showed that real-time PCR was the most sensitive method, but the diagnostic specificity of the four approaches was equivalent. In particular, molecular approaches may be more effective than microscopy and culture when the clinical symptoms of tinea unguium are not evident.
A retrospective analysis of the concordance of in-house fungal culture and a commercial quantitative PCR from 16 dermatology referral practices across the USA (2018-2019)

Abstract:Background: Commercial quantitative (q)PCR and fungal culture can be used concurrently or individually to test for dermatophytosis with a reported high sensitivity and specificity. Hypothesis/objectives: The aims of this retrospective study were: (i) to evaluate the concordance of a commercial qPCR with in-house fungal culture for the initial diagnosis of dermatophytosis and for monitoring for mycological cure during treatment in dermatology private practice; and (ii) determine the sensitivity and specificity of qPCR overall, (iii) for initial diagnosis and (iv) for treatment monitoring in dogs and cats. Animals: Four-hundred and twenty-seven client-owned dogs and 188 client-owned cats. Materials and methods: Retrospective evaluation of electronic medical records from 615 client-owned dogs and cats presented to 16 dermatology referral practices across the USA from 2018 to 2019. Concordance of qPCR with in-house fungal culture and sensitivity and specificity were determined from 667 paired samples. Results: qPCR agreed with in-house fungal culture in 63 of 85 positive tests and 571 of 582 negative tests, with an overall sensitivity and specificity of 74.1% [95% confidence interval (CI) 63.5-83.0] and 98.1% (95%CI 96.6-99.0), respectively. qPCR sensitivity and specificity for the initial diagnosis of dermatophytosis and for treatment monitoring were 72.4% (95%CI 59.1-83.3) and 98.7% (95%CI 97.3-99.5), and 77.8% (95%CI 57.7-91.4) and 92.0% (95%CI 80.1-97.8), respectively. Conclusions and clinical relevance: Compared to in-house fungal culture, qPCR was less sensitive and more specific than reported previously. These findings suggest that a diagnosis of dermatophytosis and determining mycological cure should continue to be based on a combination of complementary diagnostic tests.
Retrospective Comparison of Polymerase Chain Reaction and Culture-Based Identification for Diagnosis of Microsporum canis and Trichophyton Spp. in Shelter Cats and Dogs

Abstract:Introduction: Canine and feline dermatophytosis is challenging to manage in shelter settings because of its transmissibility, potential for zoonosis and labour-intensive treatment. An efficient and accurate initial diagnostic plan is essential to confirm infection, prevent outbreaks and ensure public safety. While quantitative polymerase chain reaction (PCR) has gained attention for its rapid turnaround time, studies have shown variable sensitivity. This study aimed to assess the performance of PCR in detecting Microsporum canis and Trichophyton spp. in shelter cats and dogs when compared to culture-based identification. Methods: Between 1 June 2015 and 10 January 2023, 125,939 cats and dogs entered 34 sheltering locations in coastal Western Canada. Of these, 246 animals (48 dogs, 198 cats) had both culture and PCR results, and sufficient records for analysis. Toothbrush samples were collected for dermatophyte test medium (DTM)/enhanced sporulation agar (ESA) bi-plates at a central shelter location and submitted for PCR at a referral laboratory before treatment was initiated. The diagnostic accuracy of PCR was evaluated using the reference standard of a 14-day culture result combined with P-scoring, a semi-quantitative method to assess DTM culture colonies. Results: Culture readings identified 72/246 (29.27%) lesional animals as positive (63 cats [31.81%] and 9 dogs [18.75%]). PCR demonstrated an overall sensitivity of 86.1% and specificity of 94.8% for both animal species combined. PCR for M. canis showed a sensitivity of 86.4% and a specificity of 97.8%, while PCR for Trichophyton spp. showed a sensitivity of 84.6% and specificity of 97.9%. DTM/ESA culture was highly efficient, with positive results available within 7 days for most cases (59/63 cats [93.6%] and 9/9 dogs [100%]). Conclusion: The variability in dermatophyte PCR sensitivity across recent studies highlights the risks of reliance on PCR as a sole diagnostic tool for suspect animals, especially when considering the demonstrated efficacy of in-house culture.
Molecular detection and species identification of dermatophytes by SYBR-Green real-time PCR in-house methodology using hair samples obtained from dogs and cats

Abstract:The classical dermatophytes diagnosis is based on mycological culture and microscopy observation both human and animal hair, skin, and nail samples. The aim of this work was to develop the new in-house real-time PCR with pan-dematophyte reaction for detection and identification of the main dermatophytes directly from hair samples, providing a simple and rapid diagnosis of dermatophytosis in dogs and cats. An in-house SYBR-Green real-time PCR was designed and used for detecting a DNA fragment encoding chitin synthase 1 (CHS1). A total of 287 samples were processed by culture, microscopic examination with KOH 10%, and real-time PCR (qPCR) analysis. Melting curve analysis of the CHS1 fragment revealed to be reproducible, showing a single distinct peak for each species of dermatophyte, namely Trichophyton mentagrophytes, T. verrucosum, Microsporum canis, and Nannizzia gypsea (formerly M. gypseum). Then, out of the 287 clinically suspected cases of dermatophytosis, 50% were positive for dermatophytes by qPCR, 44% by mycological culture, and 25% by microscopic examination. Microsporum canis was identified in 117 samples tested by culture and 134 samples tested by qPCR, followed by N. gypsea in 5 samples (either tested by culture or qPCR) and T. mentagrophytes detected in 4 and 5 samples when tested by culture or qPCR, respectively. Overall, qPCR allowed the diagnosis of dermatophytosis in clinical samples. The results suggest this newly proposed in-house real-time PCR assay can be used as alternative diagnosis and rapid identification of dermatophytes frequently associated to clinical hair samples of dogs and cats.

Abstract:In recent years, the fast and accurate identification of the Candida species is of great importance as the response to antifungal treatment differs among species. Following the treatment of several immunosuppressive diseases, fungal infections can emerge. The aim of this study was to compare the accuracy, costs and time of result periods of the methods used in the identification of the most common human fungal infectious agent, Candida strains. From various clinical samples sent to the Microbiology Laboratory of Karabuk University Training and Research Hospital between July 2016-December 2017, a total of 91 yeast isolates cultivated in blood agar (Becton Dickinson, USA) and/or Sabouraud dextrose agar (SDA-Oxoid, UK), confirmed with colony morphology and microscopic appearance, identified as Candida species with a fully automated identification system (Phoenix™ Yeast ID Panel, Becton Dickinson Diagnostics, USA) were included in the study. All the samples were examined with sequence analysis using ITS1 forward 5'-TCC GTA GGT GAA CCT GCG G-3' and ITS4 reverse 5'-TCC TCC GCT TAT TGA TAT GC-3' primers (Iontek, Turkey) and the matrix-assisted laser desorption-ionisation time of flight mass spectrometry (MALDI TOF-MS) systems. Molecular sequence analysis was accepted as the gold standard method and the results were compared with those of the other methods MALDI TOF-MS and Phoenix™ Yeast ID Panel in respect of the accuracy of the identification of Candida strains. According to the results of the DNA sequence analysis of the 91 Candida isolates included in the study, 24 were identified as Candida albicans, 20 Candida tropicalis, 16 Candida parapsilosis, 13 Candida glabrata, seven Candida kefyr, six Candida krusei, two of each Candida dubliniensis, Candida guilliermondi and one Candida lusitaniae. Compared to the results of the DNA sequence analysis, the accurate identification of the fully automated Phoenix™ system and the MALDI TOF-MS system was found as 92.3% and 97.8%, respectively. In addition to accuracy, costs and time of result periods of the three methods were also compared. Disregarding the cost of the device in the 3 methods, when the comparison was made of the cost per test and the time to results after pure production in SDA agar, the MALDI TOF-MS system was determined to have the lowest costs and provided results in the shortest time. As some of the Candida strains have antifungal resistance, identification of the strains must be a priority in respect of starting early treatment. The MALDI TOF-MS system has high performance in accurate identification, low costs and the system provides the results within minutes, thereby allowing immediate decision to be made for the antifungal treatment to be started. Thus, the morbidity, mortality and cost rates will be reduced. In conclusion, as the MALDI TOF-MS is a rapid, reliable and low cost per test system, it can be considered suitable for routine use in laboratories.

Abstract:Highly effective targeted therapies are available to treat noncommunicable chronic inflammatory skin diseases. In contrast, the exact diagnosis of noncommunicable chronic inflammatory skin diseases is complicated by its complex pathogenesis and clinical and histological overlap. Particularly, the differential diagnosis of psoriasis and eczema can be challenging in some cases, and molecular diagnostic tools need to be developed to support a gold standard diagnosis. The aim of this work was to develop a real-time PCR-based molecular classifier to distinguish psoriasis from eczema in formalin-fixed and paraffin-embedded-fixed skin samples and to evaluate the use of minimally invasive microbiopsies and tape strips for molecular diagnosis. In this study, we present a formalin-fixed and paraffin-embedded-based molecular classifier that determines the probability for psoriasis with a sensitivity/specificity of 92%/100%, respectively, and an area under the curve of 0.97, delivering comparable results to our previous published RNAprotect-based molecular classifier. The psoriasis probability, as well as levels of NOS2 expression, positively correlated with the disease hallmarks of psoriasis and negatively with eczema hallmarks. Furthermore, minimally invasive tape strips and microbiopsies were effectively used to differentiate psoriasis from eczema. In summary, the molecular classifier offers broad usage in pathology laboratories as well as outpatient settings and can support the differential diagnosis of noncommunicable chronic inflammatory skin diseases on a molecular level using formalin-fixed and paraffin-embedded tissue, microbiopsies, and tape strips.
Species identification of dermatophytes isolated in China by matrix-assisted laser desorption ionisation-time-of-flight mass spectrometry

Abstract:Background and objectives: Matrix-assisted laser desorption ionisation-time-of-flight mass spectrometry (MALDI-TOF MS) is a novel technique for identifying dermatophytes. This study aimed to detect the limitation of MALDI-TOF MS applied to dermatophytes. Methods: A total of 113 DNA-sequenced dermatophyte isolates preserved at the Research Center for Medical Mycology of Peking University were selected for this study. Forty-two isolates were selected as reference strains used to create a supplementary database. Seventy-one isolates (Trichophyton rubrumseries,T benhamiaeseries,T mentagrophytes series species and T schoenleinii) were used to evaluate the suitability of the MALDI-TOF MS Biotyper system. MALDI Biotyper 4.0 software was employed to construct the main spectrum profile (MSP) dendrograms. Results: Correct identification rates at the species and genus levels were 90.1% and 91.5%, respectively, using Bruker Filamentous Fungi Library 1.0 combined with the novel database. The MSP dendrogram of the T rubrumseries showed unambiguous separation of T rubrum and T violaceum and that of the T benhamiaeseries distinguished T verrucosum, T benhamiae and T erinacei. Conversely, the MSP dendrogram of the T mentagrophytes series did not successfully distinguish T mentagrophytes,T interdigitale and T tonsurans. Conclusion: MALDI-TOF MS showed good performance in the identification and delineation of the T rubrumseries and T benhamiaeseries, but showed poor performance in T mentagrophytes series.
Precision diagnostics in paediatric dermatology: Advancing management of tinea capitis through dermatophyte PCR

Abstract:Background Tinea capitis (TC) is the most frequent dermatophyte infection in children requiring systemic and topical treatment for several weeks. Traditionally, diagnosis and treatment monitoring were based on microscopic examination and fungal culture of scales and plucked hairs, which both have significant limitations. Objectives To investigate the role of dermatophyte polymerase chain reaction (PCR) in the treatment of TC. Methods Scales and plucked hairs of children with TC were investigated by dermatophyte PCR, microscopic examination and fungal culture at baseline and during antifungal treatment. Results Seventeen children with TC were included. At baseline, sensitivity of PCR was 100% as compared to 60% and 87% for direct microscopy and fungal culture, respectively. Species identification by PCR and fungal culture was consistent in all cases. During follow‐up, analysis of 38 samples under treatment showed a sensitivity of PCR, direct microscopy and fungal culture of 68%, 26% and 89% while specificity was 84%, 100% and 100%, respectively. PCR during therapy proved to be false‐negative in six and false‐positive in three instances. The latter turned negative after 4 weeks without further systemic treatment. Conclusions Dermatophyte PCR is an excellent tool for baseline diagnostics of TC providing rapid and accurate results. Our findings suggest that due to the fast and reliable results, it may replace direct microscopy and fungal culture to confirm or exclude TC in children. In the treatment course, diagnostic accuracy and performance of PCR seem reduced as compared to fungal culture, limiting its value for treatment monitoring. Mycological cure ascertained by fungal culture should currently remain the therapeutic goal.

Molecular detection of fungal pathogens in clinical specimens by 18S rDNA high-throughput screening in comparison to ITS PCR and culture

Abstract:The rising incidence of invasive fungal infections and the expanding spectrum of fungal pathogens makes early and accurate identification of the causative pathogen a daunting task. Diagnostics using molecular markers enable rapid identification of fungi, offer new insights into infectious disease dynamics, and open new possibilities for infectious disease control and prevention. We performed a retrospective study using clinical specimens (N = 233) from patients with suspected fungal infection previously subjected to culture and/or internal transcribed spacer (ITS) PCR. We used these specimens to evaluate a high-throughput screening method for fungal detection using automated DNA extraction (QIASymphony), fungal ribosomal small subunit (18S) rDNA RT-PCR and amplicon sequencing. Fungal sequences were compared with sequences from the curated, commercially available SmartGene IDNS database for pathogen identification. Concordance between 18S rDNA RT-PCR and culture results was 91%, and congruence between 18S rDNA RT-PCR and ITS PCR results was 94%. In addition, 18S rDNA RT-PCR and Sanger sequencing detected fungal pathogens in culture negative (N = 13) and ITS PCR negative specimens (N = 12) from patients with a clinically confirmed fungal infection. Our results support the use of the 18S rDNA RT-PCR diagnostic workflow for rapid and accurate identification of fungal pathogens in clinical specimens.

Abstract:<span class="a-plus-plus abstract-section id-a-sec1"><h3 class="a-plus-plus">Purpose of Review</h3><p class="a-plus-plus">This review has incorporated the knowledge and experience of the leads of each of the laboratory working parties of the fungal PCR initiative in order to provide up-to-date information on the performance and developments of PCR methods for the detection of fungi that commonly cause invasive fungal disease (IFD).</p></span><span class="a-plus-plus abstract-section id-a-sec2"><h3 class="a-plus-plus">Recent Findings</h3><p class="a-plus-plus">Molecular diagnosis of IFD enhances the current repertoire of mycological investigations. Providing superior sensitivity and turn-around-time over classical approaches, yet maintaining the benefits of classical tests (e.g. species level identification and identifying resistance). Standardization for <em class="a-plus-plus">Aspergillus</em> PCR is almost complete; the recent release of commercial PCR assays for a wide range fungi (<em class="a-plus-plus">Aspergillus</em>, <em class="a-plus-plus">Candida</em>, <em class="a-plus-plus">Pneumocystis</em>, Mucorales and Pan-fungal) and availability of external quality control schemes (e.g. Quality Control of Molecular Diagnostics for <em class="a-plus-plus">Aspergillus</em>, <em class="a-plus-plus">Candida</em>, <em class="a-plus-plus">Pneumocystis</em>) means that fungal PCR testing is robust and ready for use, globally.</p></span><span class="a-plus-plus abstract-section id-a-sec3"><h3 class="a-plus-plus">Summary</h3><p class="a-plus-plus">Further work is needed to ascertain the utility of PCR in routine practice and to determine whether combining it with other biomarkers is an optimal strategy. PCR for detecting Mucorales sp. and on tissue, together with direct antifungal resistance detection in body fluids, may increase its diagnostic value across the board. This and the ability to diagnose Pneumocystis pneumonia and invasive candidiasis would go a long way towards attaining the long-held ambition of medical mycology to provide a comprehensive range of tests that can be relied upon to diagnose, at least, the common IFD. In short, PCR has a clear future and is close to achieving its full potential in our laboratories.</p></span>
Reveals Importance of Platelet Activating Factor in Asthma and Possibly Other Inflammatory States Editorial

Abstract:It is presently believed that multiple mediators and interactions among mediators are involved in the pathogenesis of asthma. How then is an important mediator identified? In the case of platelet activating factor (PAF), the history goes back a long way. PAF is a family of structurally related ether-linked phospholipids that are formed as a result of the action of phospholipase A 2 and acetyltransferase on membrane alkylacyl phospholipids. Its profound effects as a potent cause of airway microvascular leakage, bronchoconstriction, sustained increase in bronchial smooth muscle responsiveness, and pulmonary vasoconstriction share many features with clinical asthma and led to the suggestion that PAF could play an important role in the pathogenesis of asthma. Various cells in the airways produce PAF. In addition to airway cells, eosinophils, the major leukocytes that are recruited into asthmatic airways, are capable of producing large amounts of PAF. Furthermore, PAF causes the release of potent inflammatory mediators from eosinophils, and eosinophils from asthmatics exhibit a markedly increased sensitivity in their chemotactic response toward PAF compared with eosinophils from normal subjects. Finally, a significant increase in PAF receptors are reported in the lungs of asthmatic patients (1). These cascades suggest that important “positive feedback” mechanisms involving PAF may be at play in the “eosinophilic inflammation” that occurs in asthmatic airways. Like many other inflammatory mediators, the proinflammatory actions of PAF are normally limited by the presence of PAF acetylhydrolase which catalyzes the degradation of PAF and related phospholipids, thus inactivating them and limiting their proinflammatory actions. A decrease in PAF acetylhydrolase would be predicted to exaggerate inflammatory and allergic responses involving PAF. Absent acetylhydrolase activity occurs in 4% of the Japanese population (2), and a report by Stafforini and associates in this issue of The Journal (3) shows that this inherited deficiency of PAF acetylhydrolase is the result of a point mutation in exon 9 and that this mutation abolishes enzymatic activity completely. This discovery has specific implications. First, it will allow rapid identification of a group of individuals predisposed to asthma. Second, it provides strong evidence that PAF plays a significant role in asthma. Early clinical studies with PAF receptor antagonists did not provide unequivocal evidence that PAF plays a pivotal role in asthma. Perhaps the clinical studies with PAF receptor inhibitors have not yet studied the appropriate group of patients. For example, when asthmatic patients die, pathological studies show thickening of the airways (from edema) and mucous plugs obstructing peripheral airways. PAF potently increases extravasation and causes hypersecretion, so it may be that this aspect of asthma is one on which to focus therapy with PAF antagonists. Expression and secretion of inflammatory mediators is not obvious in healthy persons. Perhaps, a good experimental system for studying the role of PAF is in an animal with a knockout for the gene for PAF acetylhydrolase. Under these conditions, the role of PAF will be exaggerated and thus easier to define when inflammation is induced. Another method for exaggerating the effects of PAF is the administration of inhibitors to abolish enzymatic activity (4). While a relatively small number of individuals of Japanese descent (4%) have a total absence of PAF acetylhydrolase activity, the heterozygous trait is present in 27% of the Japanese population (3). Even the smaller decrease in activity in heterozygotes could have significant physiologic effects in the presence of inflammatory bodily responses. The enzyme deficiency in the Japanese study reported here has been implicated in asthma, but it is possible that PAF acetylhydrolase deficiency also plays a role in other diseases. PAF has been implicated as a mediator in adult experimental respiratory distress syndrome (ARDS), especially due to bacteremia (5). In experimental ischemia-induced reperfusion injury, PAF receptor antagonists have been shown to reduce lung injury after cardiopulmonary bypass (6). In pulmonary hypertension (7) and interstitial fibrosis induced by bleomycin (8), PAF receptor antagonists attenuate lung responses. Thus, there are a series of inflammatory diseases that need to be reexamined in the light of the present knowledge that genetic and acquired modifications of PAF acetylhydrolase could be important in the pathogenesis of pulmonary hypertension, pulmonary fibrosis, ARDS, and other inflammatory lung diseases. Similarly, diseases in other organs where PAF may play a role need to be reexamined with drug or “knockout” blockade of this enzyme that causes the inactivation of PAF. One final comment: It is most interesting that the discovery of the importance of a molecule has been established definitively by identifying a limited population that genetically lacks the ability to break down this inflammatory mediator! It has opened a potentially profitable field of research of a molecule that has long been incriminated (but not previously definitively proven) to be important in various inflammatory states.
Monocentric evaluation of the Novaplex dermatophyte multiplex qPCR assay in the diagnosis of dermatophytoses

Abstract:Superficial fungal infections caused by dermatophytes are a prevalent global health concern. Rapid and accurate diagnosis of these pathogens through molecular tools would offer a substantial advantage for early detection and effective treatment. The conventional fungal culture presents inherent limitations, including extended result delivery delay and variable sensitivity. This study aimed to evaluate the performance of the multiplex real-time PCR Novaplex dermatophyte assay (Seegene) in comparison to traditional mycological methods including direct examination and culture. A total of 312 nail, skin, and scalp samples collected from patients with suspected superficial fungal infections for mycological diagnosis were retrospectively subjected to the Novaplex dermatophyte assay. Overall, 170 (54.6%) and 186 (59.6%) samples tested positive for dermatophyte culture and dermatophyte PCR, respectively. The concordance between PCR and culture for dermatophyte detection was 87.2%. There were 158 culture-positive/PCR-positive samples, 12 culture-positive/PCR-negative samples, and 28 culture-negative/PCR-positive samples. The sensitivity of PCR against culture varied according to the dermatophyte target, ranging from 90.5% (Trichophyton mentagrophytes/interdigitale/benhamiae), 91.2% (Trichophyton rubrum), to 100% (Microsporum spp. and Trichophyton tonsurans). When considering the final diagnosis using composite criteria, the sensitivity and specificity for the diagnosis of dermatophytosis were 92.9% and 96.6% for PCR, 86.7% and 100% for culture, and 95.4% and 92.2% for direct examination and culture combined, respectively. The Seegene Novaplex dermatophyte assay is an easy-to-use automated one-step extraction-PCR system that offers satisfactory performance for routine diagnosis of dermatophytoses in clinical laboratories, particularly in non-specialized centers. However, it cannot fully replace conventional mycology due to its inability to detect mold infections and to identify dermatophytes at the species level.
Immunoregulation in Idiopathic Inflammatory Myopathies: From Dendritic Cells to Immature Regenerating Muscle Cells

Abstract:Idiopathic inflammatory myopathies (IIM) are a group of chronic muscle disorders of unknown origin that lead to muscle destruction. The original classification of IIM proposed by Bohan and Peter in 1975 including polymyositis (PM) and dermatomyositis (DM) was recently revised with the development of histopathological approaches and the discovery of myositis-specific and myositis-associated antibodies that led to the characterization of new entities. In addition to PM and adult and juvenile DM, these novel classifications distinguish inclusion-body myositis (IBM), immune-mediated necrotizing myopathy, overlap myositis, and cancer-associated myositis1,2. While distinct immunopathogenic mechanisms may occur in each subset of myositis, all IIM share a common inflammatory background associated with clinical, histological, and serological overlap. Several molecular pathways, such as adaptive and innate immune responses, autoimmunity, and nonimmune mechanisms, could influence the pathogenesis of IIM. However, the exact contribution of each one in the development of distinct phenotypes remains unclear. In this issue of The Journal , Gendek-Kubiak and Gendek highlight the contribution of dendritic cells (DC)3. We summarize here recent insights in myositis immunoregulation from DC that are central in the development of adaptive and innate response to nonimmune mechanisms. ### Dendritic cells DC are crucial for the development of adaptive and innate immune responses. DC, subdivided into myeloid DC (mDC) and plasmacytoid DC (pDC), are present in lymphatic and blood systems and peripheral organs. They are equipped with a range of pattern-recognition receptors (PRR) and serve as sentinels of the immune system. Their activation after stimulation of PRR, such as Toll-like receptors (TLR) or C-type lectin receptors (CLR), by microbial components and/or endogenous ligands leads to their maturation and the development of an effector T cell immune response. Abstract:BackgroundMALDI-TOF MS is an analytical method that has recently become integral in the identification of microorganisms in clinical laboratories. It relies on databases that majorly employ pattern recognition or fingerprinting. Biomarker based databases have also been developed and there is optimism that these may be superior to pattern recognition based databases. This study compared the performance of ribosomal biomarker based MALDI-TOF MS and conventional methods in the identification of selected bacteria and yeast.MethodsThe study was a cross sectional study identifying clinically relevant bacteria and yeast isolated from varied clinical specimens submitted to a clinical laboratory. The identification of bacteria using conventional Vitek 2™ automated system, serotyping and MALDI-TOF MS was performed as per standard operating procedures. Comparison of sensitivities were then carried out using Pearson Chi-Square test and p-value of <0.05 was considered statistically significant. Secondary outcomes analyzed included the major and minor error rates.ResultsOf the 383 isolates MALDI-TOF MS and conventional methods identified 97.6 and 95.7% (p = 0.231) to the genus level and 97.4 and 88.0% (p = 0.000) to the species level respectively. Biomarker based MALDI-TOF MS was significantly superior to Vitek 2™ in the identification of Gram negative bacteria and Gram positive bacteria to the species level. For the Gram positive bacteria, significant difference was observed in the identification of Coagulase negative Staphylococci (p = 0.000) and Enterococcus (p = 0.008). Significant difference was also observed between serotyping and MALDI-TOF MS (p = 0.005) and this was attributed to the lack of identification of Shigella species by MALDI-TOF MS. There was no significant difference observed in the identification of yeast however some species of Candida were unidentified by MALDI-TOF MS.ConclusionBiomarker based MALDI-TOF MS had good performance in a clinical laboratory setting with high sensitivities in the identification of clinically relevant microorganisms.

Abstract:Fine-needle aspiration was reported as showing cells Case report highly suspicious of malignancy and an initial diagnosis of chest wall sarcoma was made. She was referred to A 50-year-old woman presented with a 2-day history of left loin pain. Plain X-rays and IVU conﬁrmed a left the thoracic surgical unit, where subsequent needle biopsy of the tumour was inconclusive and an open mid-ureteric stone 7 mm in diameter. The following day she developed rigors and was treated with antibiotics procedure was planned. At operation, frozen-section analysis showed inﬂammatory changes with no obvious (amoxycillin/clavulanic acid). The symptoms persisted and the stone appeared static on further X-ray, so she malignancy. The mass arose from the costochondral junctions of the third and fourth ribs, with some involve- underwent ureteroscopy and insertion of a 6 F ureteric stent (Cook Urological). She improved considerably over ment of the overlying pectoralis muscle and the under-lying pleura. The appearances suggested a chest wall the next few days and was allowed home. She was re-admitted 3 weeks later to remove the stent. On this abscess with destruction of the rib and the adjacent cartilage. These were excised, together with the under- admission she still had symptoms and signs of lower urinary tract infection, and urine culture revealed heavy lying pleura. Subsequent histology of para B n sections conﬁrmed the presence of a benign chest wall mass, growth of Escherichia coli , for which she was prescribed antibiotics (cephradine 250 mg 8-hourly for 7 days).
Application of molecular biology techniques in the identification of pathogenic fungi and the diagnosis of fungal infection

Abstract:With the increasing incidence and mortality of fungal infection, the requirements for strict diagnostic approaches became a very urgent issue. Because of the traditional detective techniques, such as culture, gave poor diagnostic outcomes, the molecular biological techniques are expected to develop the potential diagnostic approaches. During the past decades, we have carried out serial studies on the molecular properties of pathogenic fungi, and we would like to review as following. Firstly, we applied several molecular tools in classification and identification of pathogenic fungi. We performed random amplification of polymorphic DNA (RAPD), restriction fragment length polymorphism (RFLP) and other techniques in studying the typing, to classify and identify the properties of Dermatophytes, Candida spp., Cryptococcus neoformans, Dematiaceous fungi, and Aspergillus spp. Interestingly, we found the same T. rubrum strain might infect different sites of the host, while a site-specificity displayed in T. mentagrophytes. This finding indicated the genetic discrepancies among the fungi. Beside, we also found that the E. dermatitis strains with different virulences possessed some discrepancies at gene level. We then developed a PCR-based molecular procedure to identify the novel species in Exophiala spp. As the applicable strategy, we also investigated the rDNA sequence properties in several fungi. And as a result, we submitted for the first time to GenBank the complete sequence of Aspergillus fumigatus rDNA/ITSI/ITSII, which provided the basis for designing the species-specific probes and for its further clinical applications. Secondly, we have tried to develop the molecular diagnostic approaches based on our DNA sequence data which were used for identification studies previously. By analyzing the DNA sequence of Aspergillus fumigatus rDNA/ITSI/ITSII, we developed a nested PCR method to detect Aspergillus fumigatus genes. Our preliminary results indicated that this PCR-based molecular approach has great importance in the diagnosis of invasive aspergillosis. We also designed the species-specific probes and then established several in situ hybridization procedures. We found these hybridization methods could get the positive rate up to 81% (13/16), which suggests that these methods have potential diagnostic value for invasive candidiasis and aspergillosis. Based on our experiences, we would conclude that the molecular biological techniques possess great value to investigate the biological properties of pathogenic fungi, and we are looking forward to see more and more molecular tools will be used in the pathogenic mechanisms of fungal infections and antifungal activity studies.
