This study primarily aimed to assess the feasibility of an exercise intervention with follow-up by analysing recruitment numbers, screening procedures, acceptability, preferences, and safety of the exercise intervention as well as retention in follow-up. Abstract:The purpose of the study was to assess the effectiveness of the impact of physical therapy on physical status and activities of daily living in elderly people with Parkinson's disease and senile asthenia. Materials and methods. The study involved 57 elderly people diagnosed with Parkinson's disease in combination with frailty. They were divided into two groups: group 1 – they were engaged according to the principles of polyclinic rehabilitation, and group 2 – they were engaged according to the developed program of physical therapy for 6 months, rehabilitation training was carried out three times a week. The aim of the program was to increase physical status and activities of daily living of patients with the implementation of active methods of physical therapy (kinesitherapy) and, thus, to maintain general physical activity; to increase muscle strength and flexibility; to facilitate the initiation of movements; to improve joint mobility; to maintain correct posture; to improve walking and maintain balance; to prevent pain syndromes and contractures; to prevent respiratory disorders; to adapt to the changed conditions of daily activity. The physical therapy program was created taking into account the specifics of each comorbid condition using functional training on the Prosedos platform, therapeutic exercises, massage, elements of occupational therapy, nutrition correction, patient education. We consistently achieved individual short- and long-term goals of rehabilitation in the SMART-format within the patient-centered rehabilitation model. This helped to select such elements of functional training, which made it possible to individually adjust the most affected movements in each patient, and interfere with normal functioning. The control group consisted of 24 people without signs of frailty and Parkinson's disease. The effectiveness of the program was assessed by the dynamics of the 6-minute walk test (distance traveled, level of fatigue according to the Borg scale), Mini-Balance Evaluation Systems Test, Barthel Activities of daily living Index, Lawton Instrumental activities of daily living Scale. Patients were examined twice – before and after rehabilitation measures (with an interval of 6 months). Results and discussion. In elderly patients with Parkinson's disease and sarcopenia, deterioration of physical status (expressed fatigue and a short distance according to the results of the 6-minute test), violation of indicators of static and dynamic balance and, as a result, violation of performance of instrumental and basic activities of daily life were found (p <0.05 of the parameters of the control group). The developed program of physical therapy caused a statistically significant better effect on the studied indicators of functioning and activities of daily life compared to the general polyclinic program for all studied indicators (р <0.05). Conclusion. Elderly patients with the comorbidity of Parkinson's disease and frailty need the development of physical therapy programs taking into account and correcting the specifics of each disease, which will increase the overall efficiency of recovery processes and improve the performance of activities of daily life

Abstract:Objective: Aim: The aim of the study is todetermine the feasibility of using a home pulmonary rehabilitation program and evaluate its impact on patients with COPD in the GOLD B group. Patients and methods: Materials and Methods: The study was conducted on the basis of the «Healthy Movements» studio (Poltava). Patients were involved in the study after receiving secondary (specialized) medical care, the basis of which was physical therapy treatment and exercise therapy in accordance with the clinical protocol approved by the internal order of the health care institution. A total of 30 people (aged 59 to 68.4 years) with II degree chronic obstructive pulmonary disease (50 % ≤ FEV1 < 80 % of normal) in remission took part in the study. Research methods: pedagogical, medical and biological , methods of mathematical statistics. Results: Results: Each patient confirmed the achievement of the general goal, namely, increasing the number of therapeutic exercises from 3 to 5 times a week, improving the quality of life, more active participation in improving their health and awareness of the disease. The patients considered self-management training to be the most valuable. Conclusion: Conclusions: Pulmonary rehabilitation is indicated for all patients, regardless of the degree of the disease. The most ef f ective are 6-12 week programs that include breathing exercises, self-management training and training of the patient's environment, strengthening exercises, psychological support, diet therapy.

Abstract:Parkinson’s disease is a chronic and progressive degenerative disease of the central nervous system. The disorder is classified as extra pyramidal system diseases. In order to improve the patient’s condition and increase the level of quality of life, pharmacological treatment and rehabilitation are used to eliminate progressive disability. The help of the surrounding environment affects not only the course of the disease, but above all the emotional well-being of the patient. Family support and its participation in the whole therapeutic process greatly optimize its results. The aim of this work is to present the most important issues regarding kinesiotherapy in Parkinson’s disease and to demonstrate the impact of the involvement of the whole family system on the course and results of therapy. Kinesiotherapy as an element of treatment plays a very important role in the process of improving and adapting the patient to perform daily nursing activities. Kinesiotherapeutic treatments have not only a significant impact on the symptoms occurring in the course of the disease, but also on the overall health. The individual selection of therapy and the involvement of the entire team of therapists is extremely important in eliminating both movement and extrinsic symptoms. The correct implementation of the physiotherapist’s recommendations, the patient’s involvement, as well as the active participation of the patient’s environment, is a condition for the effectiveness of the entire process of maintaining health and fitness. Kinesiotherapy prevents permanent disability and helps maintain the longest possible independence and good quality of life. The involvement of the entire family support system has an impact on the results of the therapy.
Recommended Physiotherapy Modalities for Oncology Patients with Palliative Needs and Its Influence on Patient-Reported Outcome Measures: A Systematic Review

Abstract:Background: This review aims to explore the role of physiotherapy in early and traditional palliative care (PC) for oncology patients, focusing on its impact on six patient-reported outcomes (PROMs), namely fatigue, pain, cachexia, quality of life (QoL), physical functioning (PHF), and psychosocial functioning (PSF). The purpose is to assess the effectiveness of various physiotherapy interventions and identify gaps in the current research to understand their potential benefits in PC better. Methods: A systematic literature search was conducted across PubMed, Embase, and Web of Science, concluding on 21 December 2023. Two independent reviewers screened the articles for inclusion. The Cochrane Risk of Bias Tool 2 was employed to assess the risk of bias, while the GRADE approach was used to evaluate the certainty of the evidence. Results: Nine randomized controlled trials (RCTs) were included, with most showing a high risk of bias, particularly in outcome measurement and missing data. Cognitive behavioral therapy (CBT) was the only intervention that significantly reduced fatigue, enhanced PHF, and improved QoL and emotional functioning. Graded exercise therapy (GET) did not yield significant results. Combined interventions, such as education with problem-solving or nutritional counseling with physical activity, showed no significant effects. Massage significantly improved QoL and reduced pain, while physical application therapies were effective in pain reduction. Mindful breathing exercises (MBE) improved QoL but had a non-significant impact on appetite. The overall certainty of the evidence was low. Conclusions: Physiotherapy can positively influence PROMs in oncology PC; however, the low quality and high risk of bias in existing studies highlight the need for more rigorous research to confirm these findings and guide clinical practice.

Abstract:Background: Chronic Obstructive Pulmonary Disease (COPD) is a leading cause of morbidity and mortality worldwide, characterized by persistent respiratory symptoms and airflow limitation. Despite the availability of various treatments, there is a growing interest in exploring more effective strategies to manage symptoms, enhance exercise capacity, and improve the quality of life for COPD patients. Pulmonary rehabilitation, including both supervised and home-based exercises, has emerged as a promising approach. Objective: The aim of this study was to evaluate the effectiveness of a combined approach of Active Cycle of Breathing Technique (ACBT) with a home-based exercise regimen in improving functional exercise capacity and quality of life in patients with COPD, compared to ACBT alone. Methods: This was a randomized controlled trial involving 44 COPD patients, divided into two groups: Group A received ACBT alone, while Group B participated in ACBT plus a home-based exercise program. The intervention lasted for four weeks. Outcome measures included the St. George’s Respiratory Questionnaire (SGRQ) for quality of life, the 30-second arm curl test for upper extremity function, and the 6-minute walk test (6MWT) for lower extremity function. Data were analyzed using SPSS version 25, with the Mann-Whitney and Wilcoxon tests for between-group and pre-post intervention comparisons, respectively. Results: Group B showed more significant improvements in exercise capacity and respiratory symptoms compared to Group A. Specifically, the 6MWT distances increased from an average of 365.60±57.81 meters to 370.40±54.29 meters in Group B, indicating enhanced lower extremity function. Similarly, the SGRQ scores, reflecting quality of life, demonstrated more substantial improvement in Group B, though specific numerical results for this and the 30-second arm curl test were not significantly different between groups. Conclusion: The study concludes that combining ACBT with a home-based exercise regimen significantly benefits COPD patients by improving functional exercise capacity and potentially enhancing the quality of life. This combined approach could serve as a viable addition to the COPD management strategy, emphasizing the importance of comprehensive, patient-centered care.
Assessment of the functional status of patients with rheumatoid arthritis as a result of the physical therapy programme

Abstract:Annotation. A differentiated strategy for the rehabilitation treatment of patients with rheumatoid arthritis is the development and implementation of a comprehensive therapeutic programme that includes the use of non-drug measures in addition to conservative therapy. The purpose of the paper is to evaluate the functional status of patients with rheumatoid arthritis as a result of the proposed physical therapy programme. Materials and methods. A total of 188 patients with rheumatoid arthritis were examined, with an average age of 44.9±7.6 years. All patients were randomly assigned to the control (n=92) and intervention (n=96) groups. Patients in the control group underwent rehabilitation in accordance with the recommendations of the regulatory document of the Ministry of Health of Ukraine. The physical therapy programme for patients of the main group is developed in accordance with the domains of the ICF, taking into account the factors that affect the level of functional impairment, health status and quality of life. The effectiveness of the proposed physical therapy programme on the functional status of patients with rheumatoid arthritis was assessed by the results of the Stanford Health Assessment Questionnaire. Results. At the beginning of the study, the total score for patients in the control group was 6.3±0.7 points and for the intervention group - 6.5±9.5 points. The greatest difficulties arose in getting dressed, including tying laces and fastening buttons, opening and closing a tap, bringing a full cup to the mouth, bending over and picking up an object from the floor. After 3 months, significantly better changes occurred in patients of the main group compared to the control group. So, in the patients of the main group, the total score decreased to 3.9±11.1 points, in the control group this score was worse and amounted to 4.7±2.3 points. After the treatment and physical therapy in 6 months, the functional status of patients in both groups improved, but the total score in the main group was significantly better and closer to normal (1.8±12.2 points) than in patients in the control group (2.9±4.1 points). Significantly better results of functional status indicators in patients of the main group indicate the effectiveness of the proposed physical therapy programme. Conclusion. Increased motor activity in the joints, muscle strength in the affected upper extremities as a result of the proposed physical therapy programme contributed to the improvement of daily activities and functional status of patients in the main group, as evidenced by the results of the Stanford Health Interview. Thus, significantly better changes occurred in the patients of the main group compared to the control group, where conventional rehabilitation treatment was performed.

Abstract:Objective: This study evaluates the impact of physiotherapy interventions on health outcomes and explores the correlation between physiotherapy session characteristics and improvements in health among older individuals. Methods: In a cross-sectional design, 384 older adults with chronic conditions such as arthritis, osteoporosis, Chronic Obstructive Pulmonary Disease (COPD), diabetes, and hypertension were recruited. Results: The proportion of arthritis (39.1%) and hypertension (45.8%) was notably high. Participants receiving physiotherapy showed significant improvements in pain levels (mean reduction from 5.09 to 2.95), mobility scores (improvement from 3.0 to 3.96), and functional independence. A positive correlation was identified between the frequency of physiotherapy sessions and pain reduction (r = 0.26, p = 0.035), and a stronger correlation between session duration and both pain reduction (r = 0.38, p = 0.002) and mobility improvement (r = 0.43, p = 0.001). High satisfaction rates with physiotherapy were reported, and age was found to be a significant negative predictor of health outcomes (Coef. = -0.3402, p = 0.0009). Conclusion: Physiotherapy interventions significantly improve health outcomes in older adults with chronic diseases.

Abstract:Abstract Background Currently, on inpatient medical wards at University Hospitals of Leicester NHS Trust, first contact by physiotherapy and occupational therapy is made when patients become medically optimised for discharge. This is due to a number of reasons such as staffing and resource shortages. Aim Analyse the effects of early intervention by therapy on patients on a geriatric medicine inpatient ward. It is hypothesised that earlier intervention can improve patient and service outcomes. Method A data sheet was created to capture baseline information including mobility/care needs prior to admission, date of initial contact by therapy, mobility/care on discharge, length of stay and discharge destination. Data was collected over two phases; initial therapy contact at point of patient being medically optimised, and then with the planned intervention of proactive therapy input early in a patient’s admission. Results The data shows an improvement in all measured patient outcomes in the intervention group. The average time from admission to therapy first contact reduced from 7 days to 2 days. The average length of stay reduced from 14.5 days to 7 days in the intervention group. 70% of patients left the hospital with a reduction in their mobility status in the control group, whereas only 32% of patients left with worse mobility in the intervention group. 41% of patients in the control group left with new or increased care provision compared to 36% in the intervention group. The data also showed that a higher proportion of patients were mobilised by ward staff and less patients were discharged to 24-hour care settings in the intervention group. Conclusion Therapy (with the help of the multi-disciplinary team) should proactively identify patients in need of therapy input as soon as safely possible. This research shows that adopting this approach leads to improvements for our patients and our service.

Abstract:As life expectancy continues to increase, improving the quality of life (QoL) for older adults becomes an important issue. This study investigated the impact of a two-week intensive rehabilitation program at the Techirghiol Balneal and Rehabilitation Sanatorium on older adults' QoL, focusing on physical and cognitive function. The study employed a comprehensive geriatric assessment to evaluate the progress of 156 patients over 65 from admission to discharge. We used the Scale for Identifying Fall Risk Factors (STRATIFY) scale to assess the risk of falling, the Visual Analogue Scale (VAS) to assess pain levels, and the Functional Independence Measure (FIM) to assess motor and cognitive abilities. The program included multi-parametric evaluations and personalized treatment plans. Statistical analysis of these data led to the following results: The STRATIFY scale showed a significant improvement in patient functionality and a decrease in the risk of falling during hospitalization, with a mean difference in scores between admission and discharge ranging from 0.141 to 0.372, with a p-value of less than 0.001, confirming the clinical significance of this improvement. The VAS showed a significant reduction in pain or symptom intensity, reflected by a mean decrease of -3.141 between admission and discharge. The FIM recorded a mean increase of 1.436 in patients' motor capacity between admission and discharge, reflecting improved adaptation to daily activities, especially in the areas of self-care, sphincter control, transfer, and locomotion. Social participation and health status were positively influenced, demonstrating the benefits of short-term, intensive rehabilitation. The two-week rehabilitation program significantly improved the QoL of older adult patients. These outcomes suggested that active aging strategies could be effectively integrated into medical and institutional care frameworks, highlighting the necessity for policies that support older adults' involvement in economic and social contexts.
Effects of Different Exercise Interventions on Quality of Life in Breast Cancer Patients: A Randomized Controlled Trial

Abstract:Purpose: The aim of the present study was to evaluate the effects of different exercise interventions on quality of life parameters in breast cancer patients during 1 year of outpatient rehabilitation. Material and Methods: A total of 115 breast cancer patients met the eligibility criteria and completed the study. Participants were randomly allocated for the water exercise interventions (group A, n = 45), for the Pilates exercise interventions (group B, n = 40), and yoga exercise interventions (group C, n = 30). The 3 groups attended relevant programs for 1 year and received 144 rehabilitation sessions. Quality of life parameters were assessed using the Functional Assessment of Cancer Therapy questionnaire with a specific module for breast cancer patients (FACT-B). Quality of life data were recorded at baseline and after 6 and 12 months of exercise interventions. Results: A significant increase in quality of life indicators was observed in participants of all groups. Based on the results of the 12-month monitoring, patients of group A scored significantly more points for emotional well-being compared with group B and group C by 1.40 points ( P < .05) and 1.69 points ( P < .01), respectively, as well as by breast cancer subscale by 2.15 points ( P < .05) compared with group B. Patients in group C scored significantly better compared with group A in social/family well-being by 2.80 points ( P < .01). Conclusions: It was found that using water exercise intervention is more effective for improving emotional well-being and decreasing negative symptoms associated with breast cancer treatment compared with Pilates and yoga interventions, while yoga was more effective in improving social/family well-being. Further research on water interventions for different populations is warranted.

Abstract:Introduction: Head and neck cancer patients often suffer from physical and cognitive impairments after cancer treatment. During rehabilitation, exercise therapy can improve physical function and quality of life (QoL). Surveys demonstrated patients’ preference for home training with low- to moderate-intensity. This study was conducted in order to develope a suitable home-based training program. Therefore, the feasibility and effects of a low- to moderate-intensity exercise intervention on physical functions and QoL were evaluated. Methods: Training was conducted as supervised group training and consisted of mobilization, coordination, resistance, stretching, and relaxation exercises. The intervention lasted 12 weeks with 2 training sessions per week. Feasibility, attendance rate, physical function (eg, range of motion, 6-minute walk test [6MWT]), and QoL (eg, EORTC QLQ-30) were analyzed. Results: Ten out of 12 participants completed the intervention (83%) with an average attendance rate of 83%. Participants showed significant improvements in selected physical functions. For example, head rotation increased by 11.2° ( P = .042), walking distance in the 6MWT increased by an average of 43.3 m ( P = .010), and the global QoL scale improved by 8.2 points ( P = .059). Additionally, there were positive changes in the physical function scale ( P = .008), cognitive function scale ( P = .015), and social function scale ( P = .031) of the EORTC QLQ-30. Conclusion: Data indicate that the exercise program was feasible and had positive effects on physical function and QoL. Future research will analyze the effects of a home-based exercise program on physical function and QoL in a large-scale study.

Abstract:Background: Evidence suggests that management of people with Chronic Obstructive Pulmonary Disease (COPD) in primary care has been suboptimal, in particular, with low referral rates to pulmonary rehabilitation (PR). The aim of this study was to evaluate the effectiveness of a GP-physiotherapist partnership in optimising management of COPD in primary care. Methods: A pragmatic, pilot, before and after study was conducted in four general practices in Australia. A senior cardiorespiratory physiotherapist was partnered with each general practice. Adults with a history of smoking and/or COPD, aged ≥ 40 years with ≥ 2 practice visits in the previous year were recruited following spirometric confirmation of COPD. Intervention was provided by the physiotherapist at the general practice and included PR referral, physical activity and smoking cessation advice, provision of a pedometer and review of inhaler technique. Intervention occurred at baseline, one month and three months. Main outcomes included PR referral and attendance. Secondary clinical outcomes included changes in COPD Assessment Test (CAT) score, dyspnoea, health activation and pedometer step count. Process outcomes included count of initiation of smoking cessation interventions and review of inhaler technique. Results: A total of 148 participants attended a baseline appointment where pre/post bronchodilator spirometry was performed. 31 participants with airflow obstruction on post-bronchodilator spirometry (mean age 75yrs (SD 9.3), mean FEV1% pred = 75% (SD 18.6), 61% female) received the intervention. At three months, 78% (21/27) were referred to PR and 38% (8/21) had attended PR. No significant improvements were seen in CAT scores, dyspnoea or health activation. There was no significant change in average daily step count at three months compared to baseline (mean difference (95% CI) -266 steps (-956 to 423), p = 0.43). Where indicated, all participants had smoking cessation interventions initiated and inhaler technique reviewed. Conclusion: The results of this study suggest that this model was able to increase referrals to PR from primary care and was successful in implementing some aspects of COPD management, however, was insufficient to improve symptom scores and physical activity levels in people with COPD. Abstract:Objectives: This study presents the application of post-occupancy evaluations (POEs) methodologies to facilities for palliative-hospice care, in order to directly assess the physical-spatial qualities better suited for the specific psychological needs of "fragile" users and to ensure better architectural quality in new projects and in renovation measures. Background: In international literature, there are few studies concerning the application of POEs to hospice, assessing the impact of individual environmental factors on users' psychoemotional reactions. The set of elements that patients and their families perceive as important, defining the architectural quality of a hospice, has not been sufficiently investigated yet. Method: Therefore, a POE was performed on a sample of hospice in Italy, by preparing a self-filled questionnaire addressing patients and relatives. Indicators of perceived physical-spatial humanization, which were validated for hospital facilities (perceived hospital environment quality indicators), were used for the assessment then implemented and harmonized in relation to hospice facilities features. Results: The study revealed the indicators of architectural quality most valued by users, whose reliability was then verified, along with their average quality, by highlighting recurring shortcomings. The global quality was found to be generally high. Below-average scores were found for indicators on usability, acoustic comfort of outdoors, visual and acoustic comfort of shared indoor spaces, and artificial lighting in the recovery room. Conclusion: The study generated new knowledge on hospice and confirmed the efficacy of POEs as a tool both for purchasers and designers. Indeed, the first results of this study allow to recalibrate building programs, revisiting layouts, and elaborating appropriate technical solutions.
Improving the wellbeing of caregivers of patients with COPD using a home-based pulmonary rehabilitation programme

Abstract:Objectives The aim of this study was to evaluate the effects of a home-based pulmonary rehabilitation (PR) programme on anxiety and depressive symptoms, general fatigue and burden in informal caregivers of patients with COPD. We also evaluated the baseline characteristics of both patients and caregivers that contributed to the change in caregiver's outcomes after PR. Methods In this retrospective study, patients with COPD were referred to an 8-week home-based PR programme consisting of a weekly supervised 90-min session. Informal caregivers were invited to participate in PR according to the patient's preference and its availability. Caregivers received educational support, behavioural therapies and self-management strategies using the same methods as for patients. Burden, anxiety and depressive symptoms, and general fatigue of caregivers were assessed at baseline and at the end of PR. Results 241 patients with COPD and 138 (57.3%) caregivers were included. The majority of the caregivers were women (70.5%) and spouses (90.3%) and had at least three comorbidities (57.3%). A large proportion of caregivers showed baseline high burden, anxiety symptoms and abnormal fatigue (40%, 40% and 45%, respectively). Burden, anxiety and depressive symptoms, and general fatigue of informal caregivers were all improved after PR (p<0.05). Long-term oxygen therapy and/or noninvasive ventilation, coronaropathy and/or peripheral arterial disease and a higher baseline modified Medical Research Council Dyspnoea scale score in patients with COPD were associated with a decrease in caregiver's burden after PR. Conclusion A large proportion of caregivers of patients with COPD showed anxiety symptoms, fatigue and a high burden. These outcomes were improved by integrating the caregiver into a home-based PR programme.

Abstract:Introduction: Palliative home care is essential for terminally ill patients. This integrated approach is not limited to physical care, but also embraces the psychological, social and spiritual aspects of the patient. This model of care, focused on the patient and their family, aims to ensure quality health care during the advanced stage of a disease. In addition, by reducing the need for hospital admissions, palliative home care reduces costs, providing a sustainable alternative for the health system. Objective: To describe the knowledge related to the clinical, emotional and economic impact of palliative home care in cancer patients Materials and Methods: A narrative review was conducted using databases such as PubMed, Cinahl and Cochrane Library between December 2023 and March 2024, using the Population, Intervention, Outcome (PIO) methodology. The survey generated 551 articles, of which only 6 were relevant to the study. The selection of studies was guided by inclusion and exclusion criteria, with a quality assessment using the Dixon Woods instrument. Results: The studies included in the review have demonstrated a positive and significant impact of palliative home care on the well-being and quality of life of terminal cancer patients. Some of these studies have examined the clinical efficacy of such treatments in mitigating the patient's symptoms, with conflicting results: while some have shown positive efficacy, others have not found the same result. Regarding the cost-effectiveness, the analysis highlighted a lack of definitive evidence on the possible economic advantage of palliative home care compared to hospital care. Conclusions: Palliative home care emerges as a crucial element in the nursing care of the terminally ill cancer patient: it offers essential psychological support, enabling patients to feel understood and listened to with regard to their needs and requirements. However, there are some discrepancies, particularly with regard to economic effects and symptom control.

Abstract:Introduction and objective: Ageing is a continuous, progressive process of functional reserve loss. Physical fitness and mental state have a significant impact on the functionality level of the elderly population. Assessing the degree elderly people's of independence regarding self-care is an important aspect of the Comprehensive Geriatric Assessment (CGA). The aim of the study was to assess the functional capacity of people over 65 years of age. Material and methods: The study involved 312 patients in hospital wards across Lower Silesia, south-west Poland. The criteria for participation were as follows: providing informed consent to participate in the study, intellectual capacity to be interviewed, and age over 65. The study applied the diagnostic survey method, together with the use of the VAS, Barthel, IADL and GDS scales. Results: Among the respondents, 59.94% were in a moderately severe condition according to the Barthel scale, average IADL score - 20.56 points; according to the GDS scale, 58.97% had no symptoms of depression. Respondents suffered from multiple chronic diseases, the most common of which were hypertension (71.47%); they also reported some medical issues, mainly back pain (47.44). Assessment of the correlation of the Barthel and GDS scales, as well as the IADL and GDS, revealed a significant and negative correlation at -0.49 and -0.50. Assessment of the correlation between the number of diseases and the Barthel scale was -0.49, the number of symptoms and the Barthel scale -0.4; pain severity and the Barthel scale -0.41, number of diseases and IADL -0.58, and number of symptoms and IADL -0.52. Conclusions: The greater the seniors' independence regarding instrumental activities of daily living, the weaker the symptoms of depression. Multimorbidity and experiencing pain impaired independence among the elderly.
Effects of Physiotherapy-Based Training and Involvement of Informal Caregivers on Selected Rehabilitative Outcomes of Stroke Survivors

Abstract:Background and Objectives: The purpose of this study was to determine the effects of physiotherapy-based training and involvement of informal caregivers (ICGs) on selected rehabilitative outcomes of stroke survivors (SSs). Methods: Seventy-one (71) pairs of ICGs and SSs, which is 142 participants, comprising 39 pairs in the study group (STG) and 32 pairs in the control group (CTG) completed the study. The mean ages of ICGs in the STG and the CTG were 38.82 ± 15.37 years and 39.16±15.01 years respectively, with no significant difference (p > 0.05) between them. The mean ages of SSs in the STG and the CTG were56.21±10.91 years and 60.03±12.11 years respectively, with no significant difference (p > 0.05) between them. The ICGs in the STG had a physiotherapy-based training the ICGs in the CTG had no such training. All the SSs in both groups received conventional physiotherapy. Barthel Index (BI}, Stroke Impact Scale (SIS), and Stroke Specific Quality of Life (SSQoL) were used to evaluate the SSs at baseline and after the 12thweek of treatment. Statistical analyses involved both descriptive and inferential statistics. The level of significance was set at p<0.05. Results: There was a higher significant improvement (p= 0.001) in the BI score of the STG than that of the CTG (p= 0.012). SIS scores showed significant improvement in the SSs’ strength (p= 0.001); ability to use the hand (p=0.004); balance, and participation (p=0.000). SSQoL scores showed there were significant improvement in the SSs’ family roles (p= 0.002); language (p=0.005); mobility (p= 0.003); self-care, social roles and upper limbs (p=0.000). The male ICGs had a positive impact on the SSQoL outcome (p= 0.025), strength (p= 0.010), and balance (p= 0.013) of the SSs. The ICGs’ age had a positive relationship with the SSs’ ability to use the hand (p= 0.018), and participation (p= 0.020). ICGs as family members had a positive impact on the SSs quality of life and activity level. In the CTG, SIS scores showed significant improvement in strength (p= 0.005); activity (p=0.002); balance (p=0.019); ability to use the hand (p=0.003) and participation (p=0.001). SSQoL scores showed there was significant improvement in the SSs’ mobility (p= 0.019); selfcare, social roles and upper limbs (p= 0.000). Conclusion: The findings of this study reveal that physiotherapy-based training and involvement of Informal caregivers had significant effects on physical function such as walking, stair climbing, and bathing as well as the quality of life of stroke survivors.

Abstract:Objectives Exercise, feedback and advice are the key treatment strategies of musculoskeletal problems. The aims of this study were to determine patients', physiotherapists', and other stakeholders' perspectives about supported home physiotherapy for the management of musculoskeletal problems and to identify the barriers and facilitators to rolling out this model of physiotherapy service delivery. Methods This study was conducted as part of a process evaluation run alongside a large trial designed to determine whether supported home physiotherapy is as good or better than a course of in-person physiotherapy. Forty interviews were conducted with 20 trial participants, 15 physiotherapists, and 5 other stakeholders. The interviews were semi-structured and based on interview guides. Each interview was transcribed and a three-tiered coding tree was developed. Results Six key themes were identified. Supported home physiotherapy (i) is convenient for some patients, (ii) does not always align with patients' and therapists' expectations about treatment (iii) is suitable for some but not all, (iv) can reduce personal connection and accountability, (v) has implications for physiotherapists' workloads, and (vi) has barriers and facilitators to future implementation. Conclusions Findings suggest that patients are far more accepting of supported home physiotherapy than physiotherapists assume. This model of service delivery could be rolled out to improve access to physiotherapy and to provide a convenient and effective way of delivering physiotherapy to some patients with musculoskeletal conditions if our trial results indicate that supported home physiotherapy is as good or better than in-person physiotherapy.
