Abstract:Few studies have examined the clozapine in cohort studies of Chinese patients with schizophrenia in rural primary care. The objective of this two-year cohort study was to describe the usage of clozapine and investigate and identify the demographic, clinical correlations and risk variables which affect the use of clozapine in patients with schizophrenia. A random cluster sampling technique was used, and participants were collected from China National Psychiatric Management System (CNPMS). The variables for clozapine use in individuals with schizophrenia who had undergone a two-year follow-up were determined using the generalized estimating equation (GEE). In this study, 742 patients with schizophrenia were invited, and 491 completed the two-year follow-up study. Being married, more years of education, more waist circumference, using mood stabilizer, using anticholinergic, higher ITAQ (Insight and Treatment Attitude Questionnaire) scores were more significantly related to the use of clozapine. Older age of onset, using second-generation antipsychotics (SGAs) except clozapine predicted a lower prevalence of using clozapine. The usage of clozapine was very common in patients with schizophrenia treated by primary care physicians, and was influenced by a variety of factors, including price of drugs, clinical factors, health regulations, and the characteristics of treatment environment. Further examination of the rationale and appropriateness of clozapine in primary care in China is necessary.
Anxiety and depression levels and their influencing factors in community-dwelling patients with schizophrenia in China during the COVID-19 pandemic

Abstract:Abstract Purpose To (1) assess the prevalence of anxiety and depression symptoms in community-dwelling patients with schizophrenia in China during the epidemic; (2) explore possible influencing factors. Methods Using a cross-sectional survey, we collected 15165 questionnaires. Assessments included demographic information, concern about COVID-19-related information, sleep status, anxiety and depressive symptoms, and accompanying illnesses. The 7-item Generalized Anxiety Disorder (GAD-7) and the 9-item Patient Health Questionnaire (PHQ-9) were used to evaluate depression and anxiety levels. Group comparison was conducted by t-test, ANOVA, or chi-square test wherever suitable with Bonferroni pairwise correction. Multivariate logistic regression were performed to identify predictors for anxiety and depression. Results 16.9% patients had at least moderate anxiety, and 34.9% had at least moderate depression. T-test showed that females scored higher on GAD-7 and PHQ-9 than males, and patients without accompanying long-standing diseases, who were not concerned about the COVID-19 had lower GAD-7 and PHQ-9 scores. ANOVA showed that participants aged from 30 to 39, with higher education scored higher on GAD-7, and patients with better sleep, having less concern about the COVID-19 had lower GAD-7 and PHQ-9 scores. Regression analysis indicated that participants aged 30–39 and 40–49 positively predicted anxiety, whereas patients aged 30–39 years positively predicted depression. Poor sleep, accompanying diseases, and concerning the COVID-19 pandemic were more likely to experience anxiety and depression. Conclusion Chinese community-dwelling schizophrenia patients had high anxiety and depression rates during the pandemic. These patients warrant clinical attention and psychological intervention, especially those with risk factors.

Abstract:Abstract Background Urban birth and upbringing are consistently associated with schizophrenia and other psychoses but the key urban exposures remain unknown. China has previously found prevalence of psychosis higher in rural areas but has shown the largest displacement of population from rural into urban areas of any country in the world over the past 40 years. Studies of psychotic experiences (PE) show inconsistency but this may be due to confounding of PEs by depressive symptoms. This suggests the need to differentiate between PEs on a population continuum with non-affective psychosis and those secondary to common mental disorders when studying urbanicity. Our aims were to investigate effects of exposure to urban birth and upbringing on psychosis in a large Chinese undergraduate sample. Methods Cross-sectional surveys conducted annually during first year of university, 2014–2018, n=39,446. Self-reported categorical measures of psychosis included psychoticism, paranoid ideation, and schizotypal symptoms using SCL-90-R, and lifetime clinical diagnosis of schizophrenia; depressive symptoms using PHQ 9; putative etiological risk factors of family history and childhood maltreatment; urbanicity measured according to birth place in a major city (level 5 of a 5-level rural-urban scale), length of residence in urban location, and length of residence during three critical 5 year periods, birth - 15 years. We studied effects on our 4 psychosis phenotypes of (i) urban birth, (ii) urban living, (iii) critical times of exposure to urban environment, (iv) putative etiological risk factors, before and after adjusting for depression. We investigated associations between etiological risk factors and urban exposures. Results We identified 2,143 (5.4%) participants above a determined cut-off for psychoticism, 2,081 (5.3%) for paranoia, 760 (1.9%)with schizotypal symptoms, and 53 (0.1%) with schizophrenia. Effects of urban exposure on our psychosis phenotypes were only revealed following adjustments for depression in our models: Urban birth was associated with Paranoia (1.46, 1.24–1.70), schizotypal symptoms (1.90,1.48-2.42), and schizophrenia (2.30, 1.14–4.63), but not psychoticism. All four phenotypes were associated with 10–15 years of exposure to urban living, but not shorter periods. Only schizophrenia was associated with critical timings of total exposures of 1–3 years and 4–5 years during the first 5 years of life to an urban environment. There were no associations or negative associations between putative etiological factors and urban exposures. Discussion We confirmed that urban birth and living were associated with PEs and schizophrenia in this large sample of Chinese university students, but these findings only emerged after adjusting for depression. Depression is more prevalent in rural Chinese samples and previous studies may have been confounded by effects of PEs secondary to depression. There was a gradient of association between paranoia, schizotypal symptoms and schizophrenia, the latter showing strongest effects in association with urban exposures of birth and length of time in an urban environment. Only schizophrenia showed effects of critical timing of exposure to urban environment during infancy. Finally, we could not identify what exposures in the urban environment contributed to psychosis in our sample - although we could identify the etiological factors that did not. Among Sichuan students, there was no indication that urban effects were due to increased risk from demographic factors of male sex, lower family income, increased genetic risk, or child maltreatment, although these factors showed some effects on psychosis across the entire sample which included previous rural residents.
A Multilevel Analysis on the Association Between Socioeconomic Status and Psychiatric Disorders

Abstract:OBJECTIVE:To explore the association between psychiatric disorders and socioeconomic status such as education, income, employment and area of community settings in Zhejiang province.METHODS:Data were from the epidemiological survey of mental illnesses aged 15 and older in 2001 (n = 14 632). Psychiatric disorders were measured by GHQ-12 with a cut-point 4. Logistic multilevel analysis was the main statistical method being used.RESULTS:The overall prevalence of psychiatric disorders was 18.5%(95% confidence intervals, 17.9% to 19.1%), higher than the data from Hebei province (u = 4.39, P < 0.001). Annual household income and employment were strongly associated with psychiatric disorders especially for those with low income (odds ratio 3.45, 95%CI: 1.72 - 6.67) and unemployment (odds ratio 2.03, 95%CI: 1.73 - 2.40). Factors as regional effect and education level showed weak or inconsistent association with psychiatric disorders after controlling on other indicators.CONCLUSION:Inverse gradient relation between psychiatric disorders and SES seemed as the main result of low SES individual characteristics and some minor effects at the county level.
Temporal Trends in the Incidence and Disability Adjusted Life Years of Schizophrenia in China Over 30 Years

Abstract:Background Schizophrenia is an important public health problem in China. This study aims to assess the long-term trends in the incidence and disability-adjusted life years (DALYs) rate of schizophrenia in China between 1990 and 2019. Methods The incidence and DALYs data were drawn from the Global Burden of Disease Study 2019, and an age–period–cohort model was used in the analysis. Results The age-standardized incidence rate (ASIR) and age-standardized DALYs rate (ASDR) of schizophrenia increased by 0.3 and 3.7% for both sexes between 1990 and 2019. For males, the local drift for incidence was higher than 0 ( P < 0.05) in those aged 10 to 29 years (local drifts, 0.01 to 0.26%) and lower than 0 ( P < 0.05) in those aged 35 to 74 years (local drifts, −1.01 to −0.06%). For females, the local drift was higher than 0 ( P < 0.05) in those aged 10 to 34 years (local drifts, 0.05 to 0.26%) and lower than 0 ( P < 0.05) in those aged 40 to 74 years (local drifts, −0.86 to −0.11%). The local drift for DALYs rate was higher than 0 ( P < 0.05) in the age group from 10 to 69 years (local drifts, 0.06 to 0.26% for males and 0.06 to 0.28% for females). The estimated period and cohort relative risks (RR) for DALYs rate of schizophrenia were found in monotonic upward patterns, and the cohort RR for the incidence increased as the birth cohort moved forward starting with those born in 1972. Conclusion Although the crude incidence of schizophrenia has decreased in China, the ASIR, ASDR, and crude DALYs rate all showed a general increasing trend over the last three decades. The DALYs rate continue to increase as the birth cohort moved forward, and the increasing trend of incidence was also found in individuals born after 1972. More efforts are needed to promote mental health in China.

Abstract:Background: Schizophrenia remains a major public health challenge, and designing efforts to manage it requires understanding its prevalence over time at different geographic scales and population groups. Methods: Drawing on data from the Global Burden of Disease study 2019, annual percentage change of schizophrenia was assessed across different age, period and cohort groups at different geographic scales from 1990 to 2019. We examined associations of prevalence with the sociodemographic index. Results: Global prevalence of schizophrenia in 2019 was 23.60 million (95 % uncertainty interval: 20.23-27.15), with China, India, the USA and Indonesia accounting for 50.72 % of it. Global prevalence increased slightly from 1990 to 2019, with an annual percentage change of 0.03 % (95 % confidence interval 0.01-0.05). Regions with intermediate sociodemographic index accounted for greater proportion of prevalence increasing than regions with high index. Prevalence decreased among those born after 1979 in regions with intermediate sociodemographic index, whereas it consistently improved among all birth cohorts in regions with low index. Regardless of sociodemographic index, prevalence was highest among individuals 30-59 years old than younger or older groups. Conclusions: Prevalence of schizophrenia has shown small increases globally over the last three decades. The burden of disease is heavier in relatively less affluent regions, and it disproportionately affects individuals 30-59 years in all regions. Meanwhile, for regions with lower sociodemographic indices, the recent increasing burden among birth cohorts is more pronounced. These findings may help guide futural design of measures to manage or prevent schizophrenia in communities at higher risk.

Abstract:Objective: To assess the characteristics and factors affecting course of schizophrenia in a Chinese rural area. Method: An epidemiological investigation was conducted to identify all the patients with schizophrenia among 149 231 people in Xinjin County, Chengdu. Results: The total prevalence of schizophrenia was 4.13 per 1000 population. Males had an earlier mean age of onset (29.6 years) than females (32.3 years). Duration of illness before treatment and the total duration of illness were found to be significantly associated with level of remission. The status of treatment, family economy, housing, and families' care of patients had a significant effect on the clinical course of the illness. Conclusions: Duration of illness before treatment may be an important predictor of course in schizophrenia. Early treatment for the patients may produce higher level of improvement in prognosis. Education intervention and community-based service are urgent priorities for these patients.

Abstract:Objective Schizophrenia is a public concern in metropolises. Increases in city size may strengthen the correlation between prevalence of schizophrenia and indices of socioeconomic status, such as education. This study used population-based data of adults to investigate the association between education and schizophrenia in Chinese metropolises and its differences between inner city areas and outer suburbs. Methods Data was obtained from the Second China National Sample Survey on Disability in 2006, and analysis was restricted to 189,143 participants aged 18 years or older in all counties (districts) of Beijing, Shanghai, and Tianjin. Schizophrenia diagnoses were ascertained according to the International Statistical Classification of Diseases, 10th Revision. Logistic regression models were fitted to examine the association between education and schizophrenia. Results An inverse U-shaped pattern between education and schizophrenia was found in inner city areas of Chinese metropolises. Compared with the primary school or below group, the odds ratios of junior high school group and senior high school or above group was 2.79 (95% CI 1.96, 3.96) and 1.45(95% CI 0.99, 2.13), respectively. In outer suburbs, junior high school (OR = 0.87, 95% CI 0.63, 1.19) and senior high school or above groups (0.58, 95% CI 0.38, 0.87) were less likely to develop schizophrenia than the primary school or below group. Conclusions This study showed an association between education and schizophrenia in Chinese metropolises. In inner city areas, the association was an inverse U-shaped pattern between education and schizophrenia, whereas in suburban areas, the association was a negative linear pattern. Our findings can help identify high-risk populations of schizophrenia in Chinese metropolises. Programs for prevention and early intervention of schizophrenia will need to consider the socioeconomic disparities between inner city and outer suburban areas. Public policies can help improve mental health by developing social security for migrants in inner city areas and promoting regional equality.
A Multilevel Cross-Sectional Study of Mental Disorder in Community Settings in Zhejiang Province

Abstract:OBJECTIVE:To investigate the prevalence of mental disorder through the 12-item general health questionnaire (GHQ-12) survey and its related factors in community settings in Zhejiang Province.METHODS:A muti-level model was analyzed using data of 14,632 respondents nested in counties (cities), townships (urban districts), villages (urban neighborhoods) from the epidemiological survey of mental illnesses above 15 years old.RESULTS:The mean score measured by GHQ-12 was (1.54 +/- 2.15), which was lower than that of samples from four cities of Guangzhou, Chongqing, Taiyuan and Hangzhou (U = 11.08, P < 0.01). The mental disorder clustered within county (city) level, which represented 4.3% (chi(2) = 3.94, P < 0.05) of the total variance including four levels. After controlling for the county level, the mental disorder was associated with age, marital status, residency patterns, education, occupation and household incoming. In particular, it was reported that higher household incoming was significantly associated with better mental disorder among population in community settings.CONCLUSION:The prevalence of mental disorder clustered at the county (city) level is relatively lower with vulnerable groups including divorced, widowed, unemployed, retired, lower-income ones in community settings in Zhejiang Province.

Abstract:Objective: There has been no large-scale survey of schizophrenia in China involving both rural and urban areas using standardized assessment tools and diagnostic criteria. This study aimed to determine the lifetime prevalence of schizophrenia and its socio-demographic correlates in Beijing, China.Methods: A total of 5926 subjects were randomly selected in Beijing and interviewed using the Composite International Diagnostic Interview (CIDI 1.0). Basic socio-demographic and clinical data were collected during the interviews.Results: The lifetime prevalence of schizophrenia was 0.49%, and 0.44% and 0.55% for men and women, respectively. Unmarried status, lower monthly income, urban abode and positive family history were associated with an increased risk of schizophrenia; 9.7% of the subjects with lifetime schizophrenia reported a history of suicide attempts. The percentage of subjects with schizophrenia who were receiving treatment and their preference to seek treatment from mental health professionals were 58% and 29%, respectively.Conclusions: National surveys are urgently needed to further explore the prevalence of schizophrenia in China. The low percentage of subjects treated for schizophrenia is a serious public health issue that should be addressed in the near future. (C) 2008 Elsevier B.V. All rights reserved.
A study on the changing trend and influencing factors of hospitalization costs of schizophrenia in economically underdeveloped areas of China

Abstract:The public health problems caused by schizophrenia are becoming increasingly prominent and can place a huge economic burden on society. This study takes Gansu Province as an example to analyze the level and changing trend of the economic burden of schizophrenia inpatients in economically underdeveloped areas of China. Using a multi-stage stratified cluster sampling method, 39,054 schizophrenics from 197 medical and health institutions in Gansu Province were selected as the research objects, and their medical expenses and related medical records were obtained from the medical information system. The rank sum test and Spearman rank correlation were used for univariate analysis. Quantile regression and random forest were used to analyze the influencing factors. The results show that the average length of stay of schizophrenics in Gansu Province of China was 52.01 days, and the average hospitalization cost was USD1653.96 from 2014 to 2019. During the six years, the average hospitalization costs per time decreased from USD2136.85 to USD1401.33. The average out-of-pocket costs per time decreased from USD1238.78 to USD267.68. And the average daily hospitalization costs increased from USD38.18 to USD41.25. The main factors influencing hospitalization costs are length of stay, proportion of medications, and schizophrenic subtype. The hospitalization costs per time of schizophrenics in Gansu Province have decreased but remain at a high level compared to some other chronic non-communicable diseases. In the future, attention should be paid to improving the efficiency of medical institutions, enhancing community management, and promoting the transformation of the management model of schizophrenia.

Abstract:Introduction: Community-based services for psychosis are an important part of mental health services in China. We analyzed community-based follow-up and treatment services for psychosis in China in 2019 to provide evidence for policymaking and services delivering. Methods: Data from the National Information System for Psychosis were used to analyze usage of the information system and the registration, management, and treatment situation of patients with psychosis in China in 2019 and compared the results of 2019 with that of 2018. Results: In 2019, 100% of cities and counties used the information system and 6,230,157 patients were registered. In 2019, there were 5,944,724 registered patients (94.05%) in China under community-based follow-up services and 5,375,252 patients (85.04%) had regular follow-ups; 5,295,657 patients (83.78%) were treated by taking antipsychotics and 3,354,251 (53.07%) patients took medication regularly; 4,974,314 patients at home (87.81%) were in stable condition; and there was a significant difference in patients' stable condition and medication-taking rates among eastern, central, and western regions (p<0.05). Compared with 2018, the nationwide regular management rate, medication-taking rate, regular medication-taking rate, and patients' stable rate increased by 2.35%, 2.48%, 11.29%, and 7.21% respectively. Conclusions and Implications for Public Health Practice: Compared with 2018, the level of management and treatment improved significantly but still needs to be further strengthened, especially in western China.
Impact of urban birth and upbringing on expression of psychosis in a Chinese undergraduate population

Abstract:Abstract Background Urban birth and upbringing show consistent associations with psychotic illness but the key urban exposures remain unknown. Associations with psychotic-like experiences (PEs) are inconsistent. These could be confounded by common mental disorders associated with PEs. Furthermore, associations between PEs and urban exposures may not extrapolate to psychotic disorders such as schizophrenia. Methods Annual cross-sectional surveys among first year Chinese undergraduates 2014–2019 ( n = 47,004). Self-reported, hierarchical categorisation of psychosis: from psychoticism, paranoid ideation, schizotypal symptoms, nuclear syndrome using SCL-90-R, to clinical diagnosis of schizophrenia. Depressive symptoms using PHQ 9. Dissociative symptoms and posttraumatic stress disorder (PTSD) measured using PCL-C. Etiological factors of family history and childhood disadvantage. We studied effects of urban birth, urban living and critical times of exposure in childhood on psychosis phenotypes. Results Associations with urbanicity were found only after adjustments for depression. Urban birth was associated with paranoia (AOR 1.34, 1.18–1.53), schizotypal symptoms (AOR 1.59, 1.29–1.96), and schizophrenia (AOR 2.07, 1.10–3.87). The same phenotypes showed associations with urban residence > 10 years. Only schizophrenia showed an association with urban exposure birth-3 years (AOR 7.01, 1.90–25.86). Child maltreatment was associated with both psychosis and depression. Urbanicity measured across the total sample did not show any associations with demography, family history of psychosis, or child maltreatment. Sensitivity analysis additionally adjusting for dissociative symptoms and PTSD showed the same pattern of findings. Conclusions Urban birth and urban living showed a hierarchical pattern of increasing associations from paranoid ideation to schizotypal disorder to schizophrenia, confirming that associations for psychotic experiences could be extrapolated to schizophrenia, but only after adjusting for confounding from depression, dissociative symptoms and PTSD. Several etiological factors were the same for psychosis and depression. Future studies of PEs should adjust for confounding from common mental disorders and dissociative symptoms. Effects of urbanicity on psychosis were not explained by demography, family history of mental disorder, or child maltreatment.

Abstract:There is little evidence on the association between area-level social factors and schizophrenia in China. This study aimed to investigate the relationship between county-level social factors and schizophrenia in Chinese adults aged 18 years old and above. We obtained data from the Second China National Sample Survey on Disability, and selected 1,909,205 adults for analysis. Schizophrenia was ascertained according to the International Statistical Classification of Diseases, Tenth Revision. Multilevel logistic regressions showed that areas with higher urbanization rate was associated with increased risk of schizophrenia (areas with moderate urbanization rate: OR = 1.28, 95% CI: 1.15, 1.44; areas with high urbanization rate: OR = 1.48, 95% CI: 1.26, 1.75). Stratified analyses found that, in female adults, urbanization rates, divorce rates and socioeconomic conditions were positively associated with increased risks for schizophrenia. However, in male adults, low socioeconomic conditions was related to decreased risk of schizophrenia, and there was no significant association between rates of divorce and risks for schizophrenia. In conclusion, county-level social factors, in the form of urbanization rate, divorce rate, and socioeconomic conditions, were associated with the risk of schizophrenia in Chinese adults. Gender differences were found in these associations.
Regional years of life lost, years lived with disability, and disability-adjusted life-years for severe mental disorders in Guangdong Province, China: a real-world longitudinal study

Abstract:Abstract Background To understand the magnitude and spatial–temporal distribution of the regional burden attributable to severe mental disorders is of great essential and high policy relevance. The study aimed to address the burden of severe mental disorders by evaluating the years of life lost, years lived with disability, and disability-adjusted life-years (DALYs) in Guangdong, China. Methods We undertook a longitudinal study based on a multicenter database established by the Health Commission of Guangdong, involving a total of 21 prefectures and four economic regions in the Guangdong province. A total of 520,731 medical records from patients with severe mental disorders were collected for 2010–2020. Data were analyzed via an integrated evaluation framework by synthesizing prevalence estimates, epidemiological adjustment as well as comorbidity assessment to develop internally consistent estimates of DALY. DALY changes during 2010–2020 were decomposed by population growth and aging and further grouped by Socio-demographic Index (SDI). DALYs were projected to 2030 by the weighted median annualized rate of change in 2010–2020. Results In 2010–2020, the average DALYs for severe mental disorders reached 798,474 (95% uncertainty interval [UI]: 536,280–1,270,465) person-years (52.2% for males, and 47.8% for females). Severe mental disorders led to a great amount of disease burden, especially in Guangzhou, Shenzhen, and Foshan cities. Schizophrenia and mental retardation with mental disorders were the two leading sources of the burden ascribed to severe mental disorders. Population growth and aging could be accountable for the increasing burden of severe mental disorders. Economic regions with higher SDI carried a greater burden but had lower annualized rates of change in DALYs. The overall burden of severe mental disorders is projected to rise modestly over the next decade. Conclusions The findings urge prioritization of initiatives focused on public mental health, prevention strategies, health resources reallocation, and active involvement of authorities to effectively address the anticipated needs.

Abstract:Objective: Quality of life (QoL) has been always an important way to evaluate the outcomes of schizophrenia, but there have been few previous longitudinal studies and few in middle-income countries. This study aimed to explore the QoL in Chinese patients with schizophrenia treated in primary mental health care and the risk factors of QoL over time. Methods: Patients with schizophrenia treated in primary mental health care in rural/regional areas in Luoding, Guangdong, PR China, were evaluated with an extended questionnaire including the Chinese version of the World Health Organization Quality of Life (WHOQOL-BREF) at baseline and 2-year follow-up. Bivariate and multivariate analyses were conducted including Generalized Estimated Equation analyses (GEE). Results: Four hundred and ninety-one patients with schizophrenia in primary care completed the 2-year follow up evaluation. The QoL physical, environmental, and social relationships domains showed improvement after the 2-year period, but the psychological domain did not. GEE results showed that earlier age of onset, older age, being employed, being unmarried, the thicker waist circumference, less use of clozapine or other SGAs, fewer hospitalizations, more frequent insomnia, more severe depressive and negative symptoms as well as worse treatment insight were independently associated with poor QoL in patients with schizophrenia. Conclusion: According to our results, to improve the quality of life of patients with schizophrenia in primary care, we should pay more attention to the treatment of depression, negative and insomnia symptoms of schizophrenia, the choice and dosage of antipsychotic medication and improvement in the treatment compliance. The combined use of educational and behavioral strategies may improve treatment adherence.

Abstract:ObjectivesHealth disparities in schizophrenia are well established. However, it is less understood whether area-level socioeconomic status (SES) is differentially associated with schizophrenia depending on individual-level SES. Therefore, using a nationally large representative data, this study investigated the association between individual-level SES, area-level SES and their interaction with schizophrenia in Chinese adults from a multilevel perspective.SettingHousehold interviews in 734 counties (districts), 2980 towns (streets) and 5964 communities (villages) from 31 provinces, People's Republic of China, as part of the cross-sectional survey of Second China National Sample Survey on Disability.Participants1 909 205 men and women aged 18 years old and above.Primary and secondary outcome measuresA screen followed by clinical diagnosis was used to identify schizophrenia, and schizophrenia was ascertained according to the International Statistical Classification of Diseases, 10th Revision (code F20).Results1-SD increase in individual SES was associated with decreased risk of schizophrenia (OR=0.45,95% CI0.43 to 0.46). 1-SD increase in area-level SES was associated with increased risk of schizophrenia (OR=1.30,95% CI1.24 to 1.37). The interaction of individual SES and area-level SES was statistically significant (OR=1.05,95% CI1.02 to 1.08); as the level of area SES increased, schizophrenia risk of lower SES people grew faster than the risk of higher SES people.ConclusionsArea-level SES is particularly important to mental health of low SES individuals, with low SES people in high SES counties having the highest risk of schizophrenia than other groups. Action to reduce SES disparities in schizophrenia will require attention to the area-level context of low SES adults.

Abstract:Introduction Drivers for remission, relapse and violence-related behaviour among patients with schizophrenia are the most complicated issue. Methods and analysis This study aims to recruit a longitudinal cohort of patients with schizophrenia. Two suburban districts and two urban districts were randomly selected according to health service facilities, population, geographical region and socioeconomic status. Individuals (>18 years old) who received a diagnosis of schizophrenia following the International Classification of Diseases (10th edition) criteria within the past 3 years will be invited as participants. Assessments will be carried out in local community health centres. Data will be used to (1) establish a community-based schizophrenia cohort and biobank, (2) prospectively determine the course of multidimensional functional outcomes of patients with schizophrenia who are receiving community-based mental health treatment, and (3) map the trajectories of patients with schizophrenia and prospectively determine the course of multidimensional outcomes based on the differential impact of potentially modifiable moderators. Ethics and dissemination The study has been reviewed and approved by the Human Research Ethics Committee of Shanghai Mental Health Center (2021-67). Results of the study will be disseminated through peer-reviewed journals. If effective, related educational materials will be released to the public.

Epidemiological survey on schizophrenia in 7 areas of China

Abstract:Objective: The purpose of this study was to investigate the possible changes in the prevalence of schizophrenia in seven areas in 1993, 11 years after the 1982 psychiatric epidemiological survey and to identify the possible factors that may influence these changes. Methods: The same as that used in the 1982 psychiatric epidemiological survey over 12 areas. Results: The 1993 survey uncovered a life-time prevalence of 6.55‰ and time-point prevalence of 5.31‰ for schizophrenia. Either the life-time rate or time- point rate were all of slightly higher than that of 1982 (P > 0.05). Additionally, there was a clear social drift process of schizophrenics to the negative social living conditions. Conclusion: The slight higher but no statistic's difference in the above comparison data is possibly due to either the sampling error or the cases accumulated and the extended life expectancy of schizophrenics. It is needed to urge more attention from the health administration to be paid to schizophrenics in the public health planning.

Abstract:Abstract China accounts for 17% of the global disease burden attributable to mental, neurological and substance use disorders. As a country undergoing profound societal change, China faces growing challenges to reduce the disease burden caused by psychiatric disorders. In this review, we aim to present an overview of progress in neuroscience research and clinical services for psychiatric disorders in China during the past three decades, analysing contributing factors and potential challenges to the field development. We first review studies in the epidemiological, genetic and neuroimaging fields as examples to illustrate a growing contribution of studies from China to the neuroscience research. Next, we introduce large-scale, open-access imaging genetic cohorts and recently initiated brain banks in China as platforms to study healthy brain functions and brain disorders. Then, we show progress in clinical services, including an integration of hospital and community-based healthcare systems and early intervention schemes. We finally discuss opportunities and existing challenges: achievements in research and clinical services are indispensable to the growing funding investment and continued engagement in international collaborations. The unique aspect of traditional Chinese medicine may provide insights to develop a novel treatment for psychiatric disorders. Yet obstacles still remain to promote research quality and to provide ubiquitous clinical services to vulnerable populations. Taken together, we expect to see a sustained advancement in psychiatric research and healthcare system in China. These achievements will contribute to the global efforts to realize good physical, mental and social well-being for all individuals.
