Abstract:Background Thumb polydactyly is a common congenital abnormality of the hand. Surgical treatment includes reduction and reconstruction. A new type of thumb polydactyly and the achieved treatment are reported. Methods A 4-year-old boy with thumb polydactyly is reported. The duplicated two thumbs had one complete metacarpal and a separated triphalangeal phalanx. The ulna thumb was removed via periosteal incision, and a whole complex tissue sleeve was used to repair the ulna part of the joint capsule. Results The postoperative aesthetic and functional results were satisfactory. Conclusions A new type of thumb polydactyly that cannot be matched with the classification of either Wassel or Wood is described. The thumb polydactyly was treated by ablation via periosteal incision and ulna metacarpophalangeal joint capsule reconstruction. Three-dimensional computed tomography could be a good way to examine the area preoperatively. Level of Evidence V This journal requires that authors assign a level of evidence to each article. A systematic review of the claw toe deformity: What is known and what is needed apart from surgical procedures

Abstract:The claw toe deformity is a painful condition that mainly affects the adult population. Although there are many different treatments to solve the deformity, there is no optimal procedure to restore the normal foot mechanics. The objective of this review was to identify the technical features of the kinematic evaluation methods used in patients with claw toes. Furthermore, the aim of this review was to clarify what is known and what is needed apart from the surgical procedures to correct the claw toe deformity, with the purpose of reducing risk factors of falling in elderly people. A search in electronic databases, such as Scopus (n = 78), Google Scholar (n = 705) and ScienceDirect (n = 290) was conducted. There were seven articles (43.75%) related to the fixation (arthrodesis) of proximal and distal interphalangeal joints, one article (6.25%) describes the correction of the claw toe through plantar plate tenodesis and release of collateral ligaments, four articles (25%) describe the procedure of tendon transfer, one article (6.25%) describes flexor digitorum brevis tenotomy and a proximal interphalangeal joint arthrolysis, and another article (6.25%) presented the impact of partial phalanx osteotomy to treat the claw toe deformity. In conclusion, the review indicates that there are several studies related to the treatment or correction of claw toe deformity. However, there is a lack of knowledge of the postsurgical effects of treating claw toe condition, especially in the structural mechanics (plantar pressure distribution, stability, gait, foot mechanics and so on) of the foot after the correction of the claw toe deformity. The analysis of the foot mechanics after the correction of the claw toes has to be paramount to determine the benefits of the correction.

Classical Surgical Alternatives for the Treatment of Lesser Toe Deformities

Abstract:Lesser toe deformities are the second most common deformities around the foot and ankle. Early reports of procedures for lesser toe correction date back to the 1880s. Lesser toe deformities have a high variability of appearance. Thorough assessment of the joints involved in the deformity is necessary. Numerous techniques of soft tissue and osseous interventions are available and need to be included for individual correction. Osseous corrections are usually accompanied by soft tissue procedures. Kirschner-wire fixation is still an effective method of fixation, but internal fixation is probably the way to go for the future if cost-effectiveness can be improved.
First and Second Metatarsals Endobutton Fixation Combined with First and Fifth Bunionectomies for Correction of Splay Foot

Abstract:Background:The effective treatment for splay foot is still controversial because of large injury, many complications and un-sure curative effect in common osteotomy methods. Objective:To assess the clinical and radiographic outcomes of 1st and 2nd metatarsal Endobutton fixation with 1st and 5th bun-ionectomies in patients with symptomatic splay foot. Methods:From January 2012 to June 2012, 6 adult patients (8 feet) with symptomatic splay foot underwent 1st and 2nd meta-tarsal Endobutton fixation with 1st and 5th bunionectomies. The radiological measurement based on PACS 5.0 system and the American Orthopedic Foot and Ankle Society (AOFAS) score were used for evaluation pre-and post-operatively. Results:The average duration of follow-up was (17.7±4.8) months (range, 12-20 months) in all patients. The postoperative hallux valgus angle, 1st and 2nd intermetatarsal angle, and maximum distance between 1st and 5th metatarsal heads decreased significantly (27.0°±5.1° vs 15.9°±4.2°, 16.3°±3.2° vs 8.3°±1.9°, [9.0±1.0]cm vs [8.0±1.0]cm, P<0.05). The AOFAS score was increased by 55.5%(preoperative:50.1±8.7, postoperative:77.9±7.3, P<0.05). Conclusion:For symptomatic splay foot, surgical correction by 1st and 2nd metatarsal Endobutton fixation with 1st and 5th bun-ionectomies can effectively restore the normal foot angular relationship, provide satisfactory results with minimal trauma, early postoperative weight-bearing and obvious improvement of foot function. But long-term curative effect still needs fur-ther observation.

Abstract:This report discusses the case of a young female patient diagnosed with macrodactyly of the toes, a condition that significantly affected her daily life. From the age of three to 11, she underwent treatment due to the severe impact of her deformity, particularly on her ability to move comfortably and wear suitable footwear. The patient's macrodactyly presented a complex clinical challenge, necessitating multiple surgical procedures to manage it effectively. These surgeries included soft tissue reduction to decrease the bulk of the enlarged digits, epiphysiodesis to halt the growth of the affected bones, and amputations to address the disproportionate enlargement of the toes. Each surgical intervention was aimed at improving both the function and appearance of the affected foot, with a focus on enhancing the patient's mobility and comfort. Despite the difficulties associated with recovery, the patient showed significant improvements in her ability to walk and in the aesthetic appearance of her foot. This case underscores the importance of developing individualized treatment plans that consider the unique needs of each patient and setting realistic expectations for outcomes. It also highlights that, while surgical interventions can lead to functional and cosmetic benefits, the extent of these improvements may be limited due to the inherent complexities of macrodactyly. The case calls attention to the need for ongoing research and the accumulation of clinical experience to refine treatment approaches for macrodactyly. Such advancements are crucial for optimizing therapeutic outcomes and improving the quality of life for patients affected by this rare condition.

Abstract:Flavodoxin can function as an alternative electron acceptor for photosystem I (PSI) in place of ferredoxin under iron-limiting conditions. The isiB gene, encoding the flavodoxin in Synechococcus sp. PCC 7002, was overexpressed in Escherichia coli. Under the conditions employed, most recombinant flavodoxin (rFlvd) was in soluble form with cofactor correctly inserted. The absorption spectrum of rFlvd was identical to that of the native flavodoxin of the cyanobacteria. Photoreduction of rFlvd by PSI particles and thylakoid membranes was determined directly by monitoring the absorption change at 467 nm. The optimal conditions for rFlvd photoreduction were determined. Compared to other methods currently employed to measure PSI activity such as oxygen uptake in the presence of methyl viologen and NADP+ photoreduction in the presence of ferredoxin and ferredoxin:NADP+ oxidoreductase, measurement of PSI activity with flavodoxin as an electron acceptor has several advantages. It measures the full-chain electron transfer chain of PSI since flavodoxin accepts electrons from FA/FB and it is much simpler than the method with NADP+ photoreduction. With this method, we found that the affinity of wild-type PSI for rFlvd was 35% higher than that of the PsaE-less PSI, showing that this method is sensitive to structural changes of PSI. Our results demonstrate that rFlvd photoreduction is an effective and simple method for PSI activity measurement.

Abstract:Objective To analyze the clinical outcomes of a standard protocol of open reduction and internal fixation using a variable angle foot plate for a consecutive series of patients with acute Lisfranc joint injury.Methods This study retrospectively evaluated the clinical outcomes of 11 Chinese patients(13 feet) with acute Lisfranc joint injury who had been treated by open reduction and internal fixation with a variable angle foot plate from December 2016 to June 2017 at Department of Orthopaedics,West China Hospital.They were 8 men(10 feet) and 3 women(3 feet),aged from 19 to 57 years(average,36.4 years).Of them,10 were complicated with fracture of metatarsus,6 with cuneiform fracture and/or dislocation,4 with cuboid fracture,and one with cuboid fracture.According to the Myerson classification for Lisfranc fracture-dislocations,one case(one foot) belonged to type A,2(2 feet) to type Bl,5(6 feet) to type B2,2(3 feet) to type Cl and one(one foot) to type C2.The outcomes were evaluated using the visual analogue scale(VAS) and the midfoot scores of American Orthopedic Foot and Ankle Society(AOFAS).Results This cohort was followed up for 18 to 24 months(average,22 months).Their VAS scores were decreased significantly from preoperative 7.3±1.2(from 6 to 9) to postoperative 1.2±0.2(from 0 to 2)(P<0.05);their AOFAS midfoot scores were increased significantly from preoperative 0 to postoperative 84.6±4.6(P<0.05) Anatomic reduction was obtained in all the patients and all the fractures united successfully without any delayed union or nonunion.Superficial necrosis of the wound edge occurred in 2 cases without deep infection;skin anesthesia occurred in 2 and skin hypesthesia in 4,indicating a lesion of the superficial peroneal nerve.Conclusion Fixation of acute Lisfranc joint injury with a variable angle foot plate can lead to rigid stability,precise reduction and satisfactory short-term clinical outcomes.

Abstract:OBJECTIVES The social context within which women engaged in sex work at a popular truck stop in South Africa are placed at risk of human immunodeficiency virus (HIV) infection and the factors that influence their ability to reduce their risk were assessed. METHODS Using qualitative and quantitative techniques, an elected sex worker from within the group collected all data. RESULTS Given the various pressing needs for basic survival, the risk of HIV infection is viewed as one more burden imposed on these women by their lack of social, legal, and economic power. Violence, or the threat thereof, plays an important role in their disempowerment. In the few instances in which sex workers were able to insist on condom use, it resulted in a decrease in earnings, loss of clients, and physical abuse. CONCLUSIONS Recommendations to reduce the sex workers' risk for HIV infection include negotiation and communication skills to enable them to persuade their clients to use condoms; development of strategies through which they can maximally use their group strength to facilitate unified action; and accessibility of protective methods they can use and control, such as intravaginal microbicides.

Abstract:Background: Hallux valgus is a common forefoot disease in elder people. More than 130 procedures were used to correct the deformity. But there has been much debate on the importance of correcting the position of the sesamoid bones. Objective: The aim of the study is to explore the efficiency and essentiality of modified Lapidus arthrodesis to the first meta- tarsal-sesamoid system in hallux valgus. Methods: Twenty-one female patients with 29 hallux valgus feet were enrolled in this study. The age of the patients ranged from 46 to 71 years(60.3±9.6 years). Hallux valgus angle(HVA), intermetatarsal angle(IMA)and tibial sesamoid position (TSP)were measured before and after the modified Lapidus arthrodesis. The results were statistically analyzed to explore the relationship between the hallux valgus and TSP and the outcome of modified Lapidus arthrodesis in the correction of TSP. Results: TSP was significantly correlated with HVA and IMA. TSP was redressed by operation. There was no significant dif- ference in the absolute position of sesamoid bones before and after the operation. Conclusions: Modified Lapidus arthrodesis can effectively correct the first metatarsal-sesamoid system. TSP should be used for routine evaluation before surgical procedures of hallux valgus.
Advances in the Causes and Treatment of Floating Toes after the Weil Osteotomy: A Scoping Review

Abstract:The floating toe deformity is classified as a forefoot deformity wherein the distal portion of the toe does not establish touch with the ground, resulting in a suspended or elevated position while the finger is in a relaxed state. At first, it garnered considerable interest as a complication It is worth noting that this condition is particularly common in children under the age of 8, which usually disappears as the individual reaches maturity. Studies have shown that with the aggravation of floating toe deformity, its adverse effects on patients' gait and overall quality of life also increase. Despite the prevalence of floating toe deformity in clinical settings, there is a lack of comprehensive literature investigating its underlying causes and potential preventive strategies. This scope review follows the preferred reporting items for systematic reviews and meta-analyses extension for scoping reviews (PRISMA-ScR) statement guidelines for scope reviews. The literature was obtained from various full-text databases, including China National Knowledge Infrastructure Database (CNKI), Wanfang Database, PubMed, and Web of Science Database. Our search focused on published literature related to floating toes, Weil osteotomy, and distal metatarsal osteotomy, up until March 1, 2023. The literature search and data analysis are conducted by two independent reviewers. If there are any disagreements, a third researcher will participate in the discussion and negotiate a decision. Furthermore, two experienced foot and ankle surgeons conducted a thorough literature analysis for this review. Sixty-two articles were included. Through the clinical analysis of the structural changes of the forefoot before and after operation, the classification of floating toe was described, the causes of pathological floating toe were summarized, and the possible intervention measures for the disease were put forward under the advice of foot and ankle surgery experts. We comprehensively summarize the current knowledge system about the etiology of floating toe and put forward the corresponding intervention strategy. We recommend that future studies will focus on the improvement of surgical procedures, such as the combination of Weil osteotomy, proximal interphalangeal (PIP) arthrodesis and flexor tendon arthrodesis.

Abstract:Background: The failure of nonsurgical treatment of patients with midfoot and hindfoot deformity secondary to diabetic Charcot arthropathy may lead to a rocker-bottom foot deformity with recurrent or persistent plantar ulceration. We report our experience with realignment and extended fusion with primary use of a medial column screw for this midfoot deformity. Methods: From July 2001 through July 2005, we performed reconstructive surgery on fifteen adults with diabetes mellitus who had a severe neuropathic midfoot deformity consisting of a collapsed plantar arch with a rocker-bottom foot deformity. Thirteen had a nonhealing midfoot plantar ulcer. All underwent realignment and arthrodesis with a medial column screw; some required additional fixation techniques depending on the extent of the deformity. Outcome measures included ulcer and surgical wound-healing, radiographic results, complications, and the need for amputation. Results: The mean duration of clinical follow-up was forty-two months. Fourteen patients were able to walk, and there were no recurrent plantar ulcers. Thirteen patients were able to wear custom-made extra-depth, wide-toed shoes with molded inserts. One patient without prior ulceration had development of a deep infection that necessitated an amputation. Four feet had a nonunion, one of which was symptomatic requiring a revision to obtain union. Conclusions: Surgical reconstruction of a collapsed neuropathic foot deformity is technically demanding, but a successful outcome can result in a plantigrade foot that is free of ulceration and abnormal pressure points and a patient who is able to walk. The procedure described has an acceptable degree of complications although it has a high rate of nonunion.

Abstract:Hallux valgus cannot be cured by conservative means. Merely the symptoms it causes can be ameliorated through the use of insole supports, toe spreaders or hallux valgus night splints. Most patients therefore early express a desire for surgical correction. Currently, more than 130 surgical techniques for the correction of splay foot with hallux valgus are known. Recurrences can be minimized by the rigorous use of insoles with retrocapital pads, and the avoidance of high-heeled and/or narrow fitting shoes.

Abstract:We estimate the consumption of alcohol during Prohibition using mortality, mental health and crime statistics. We find that alcohol consumption fell sharply at the beginning of Prohibition, to approximately 30 percent of its pre-Prohibition level. During the next several years, however, alcohol consumption increased sharply, to about 60-70 percent of its pre-prohibition level. The level of consumption was virtually the same immediately after Prohibition as during the latter part of Prohibition, although consumption increased to approximately its pre-Prohibition level during the subsequent decade.

Abstract:Aim and objective: The aim of the study was to analyze weather the Lateral Tarsal Flap with a Reverse Dorsalis Pedis Artery Pedicle can be used for repair of Forefoot Skin and Soft Tissue Defects. Material and method: The total sample size was comprised of 20 subjects reported to the Dept of hand and foot Surgery. The sample size comprised of 14 males and 6 females with the age of 10 to 70 years. The injury on first metatarsophalangeal joint injury were comprised of 6 patients, the injury on toes injury were comprised of 8 patients and the injury on distal metatatarsal injury were comprised of 6 patients. The skin and soft tissue injuries were evaluated using the modified Oestern-Tscherne classification and all patients were followed up for at least 8 months. Results: Results concluded that all the 20 flaps were successfully implicated, in almost 18 patients the healing was done primary intension and only in 2 patients it was by secondary intention. There was no wound infections reported in any patient. In all the patients, donor site successfully accepted grafts and all wounds healed with minimal scarring. No patient reported any kind of problem or pain on standing or walking. Maximum no of patients reported with no activity limitation with the reconstructed foot, however in some patients they experienced occasional restrictions in recreational activities. Conclusion: We concluded that LT flap with a reversed DPA pedicle is a very effective and reliable surgical technique in reconstruction of forefoot defects.

Abstract:BACKGROUND: One of the most prevalent foot abnormalities is valgus deformity of the first toe. Many procedures have been proposed to correct this deformity, but one of the most Scarf osteotomy of the first metatarsal bone, which is complemented by the Akin osteotomy of the proximal phalanx of the first finger, is one of the most widely used. This publication analyzes the mid-term effect of repairing the deformity of the first toe using the methods described above. AIMS: To evaluate the mid-term outcomes of hallux valgus correction using Scarf osteotomy and the combination of Scarf and Akin osteotomies to the literature. MATERIALS AND METHODS: This is a cross-sectional, observational study. From January to December 2017, 129 patients (147 ft.) received forefoot deformity. Clinical and radiographic parameters were studied using the AOFAS, FFI, and VAS scales and questions related to the subjective assessment of patients by the treatment. RESULTS: The studys findings revealed an increase in AOFAS scores from 59.0 (1688, SD=18.6) before surgery to 85.0 (53100, SD=13.3; p 0.001) 5 yr later. The VAS scores fell from 5.7 (010, SD=2.2) to 2.4 (05, SD=1.4; p 0.001). After surgery, the FFI scale score decreased from 40.9 (1078, SD=18.1) to 11.3 (019, SD=5.0; p 0.001). According to the survey, 96.7% of patients would agree to a second operation, 96.9% of those who had surgery would recommend it to their relatives, and 94.6% were satisfied with the aesthetic result of the operation. Most patients (95.4%) were satisfied with the treatment in terms of the disappearance of pain and discomfort. The functional results of the surgical surgery were satisfactory to 98.4% of the patients who were consulted. Radiographic HVA, IMA angle, and Hardy and Clapham sesamoid position scores all indicated favorable mid-term outcomes with no deformity recurrence. In 46.3% of patients (68 patients), there was limited mobility in the first metatarsophalangeal joint. A complication in superficial soft tissue irritation occurred in four patients (2.7%). Iatrogenic varus deviation of one toe was observed in one patient. Metal fixators were removed from patients in 34 patients (23.1%). The removal was completed within a year in 17 cases (11.6%). CONCLUSIONS: Scarf osteotomy, alone or in combination with Akin osteotomy, is a successful treatment for hallux valgus.

Abstract:Background: The failure of nonsurgical treatment of patients with midfoot and hindfoot deformity secondary to diabetic Charcot arthropathy may lead to a rocker-bottom foot deformity with recurrent or persistent plantar ulceration. We report our experience with realignment and extended fusion with primary use of a medial column screw for this midfoot deformity. Methods: From July 2001 through July 2005, we performed reconstructive surgery on fifteen adults with diabetes mellitus who had a severe neuropathic midfoot deformity consisting of a collapsed plantar arch with a rocker-bottom foot deformity. Thirteen had a nonhealing midfoot plantar ulcer. All underwent realignment and arthrodesis with a medial column screw; some required additional fixation techniques depending on the extent of the deformity. Outcome measures included ulcer and surgical wound-healing, radiographic results, complications, and the need for amputation. Results: The mean duration of clinical follow-up was forty-two months. Fourteen patients were able to walk, and there were no recurrent plantar ulcers. Thirteen patients were able to wear custom-made extra-depth, wide-toed shoes with molded inserts. One patient without prior ulceration had development of a deep infection that necessitated an amputation. Four feet had a nonunion, one of which was symptomatic requiring a revision to obtain union. Conclusions: Surgical reconstruction of a collapsed neuropathic foot deformity is technically demanding, but a successful outcome can result in a plantigrade foot that is free of ulceration and abnormal pressure points and a patient who is able to walk. The procedure described has an acceptable degree of complications although it has a high rate of nonunion.
