Abstract:Introduction: Cetuximab, an anti-EGFR monoclonal antibody, often cause skin toxicity, most commonly acneiform rash. We present a rare case of glomerulonephritis associated with cetuximab therapy. Case report: A 58-year-old male patient recently completed cetuximab-based chemotherapy for metastatic colorectal adenocarcinoma. He presented with acute renal failure, anasarca edema and nephrotic proteinuria. The amount of protein in the 24-h urine test was over 15.6 grams. Management & outcome: The patient showed a dramatic improvement in renal function shortly after terminated of cetuximab therapy without immunosuppressive therapy. Discussion: Therefore, drugs targeting epidermal growth factor receptor (EGFR) monoclonal antibody were thought to trigger nephrotic syndrome by causing glomerular damage. As a result, physicians using EGFR monoclonal inhibitors should be very careful about renal functions and proteinuria in patients.
A Randomized, Double-blind, Placebo-controlled Phase 2 Trial of Epigallocatechin-gallate in the Prevention of Radiation-induced Dermatitis

Abstract:Radiation-induced dermatitis (RID) is a class effect associated with radiotherapy for breast cancer. Anecdotal use of topical EGCG solution has been reported in patients with breast cancer. We aimed to assess topical EGCG solution for prevention of RID in patients with breast cancer. This study enrolled women aged 18 years or older with postmastectomy radiotherapy who had histologically confirmed breast cancer and completed adjuvant chemotherapy. Participants were randomly assigned (2:1) to receive either topical EGCG solution (660umol/L) or placebo (0.9% NaCl Normal Saline) until two weeks after radiation completion. The primary endpoint was incidence of grade 2 or worse RID between EGCG group and placebo group. The second endpoints included: the rate of grade 2 or worse radiation-induced discomforts; RID and symptom severity; RID index (RIDI, designed to account for not only the maximum grade, but also for the duration of RID) and symptoms scores (pain, burning-feeling, itching, pulling-feeling and tenderness) index throughout the treatment. RID was evaluated by investigator according to the Radiation Therapy Oncology Group (RTOG) scale weekly. Symptoms related to RID were measured by patients using the Skin Toxicity Assessment Tool. Safety assessments were done according to Common Terminology Criteria for Adverse Events, version 4.0. The objective was to demonstrate a 22.3% decrease in that incidence of grade ≥2 toxicity. Assuming a 10% attrition rate, to detect this difference with a power of 0.80 using a two-sided test at significance level 0.05, it was necessary to recruit 180 patients. This trial is registered at ClinicalTrials.gov, number NCT02580279. Between November 2014 and June 2019, 165 were evaluable for efficacy finally. Median radiation dose of both groups was 50Gy. The occurrence of grade≥2 RID was significantly lower (50.5% v 72.2%; P = 0.008) with the use of EGCG than with placebo. The maximum scores of RID was also significantly lower (P = 0.014). The maximum and grade≥2 discomforts in radiotherapy volumes of patients were significantly lower for the EGCG treatment versus placebo except pulling-feeling score (All P<0.05). The mean RIDI of patients in EGCG was 5.22 ± 1.60, compared with 6.21± 1.56 (T = -3,79, P<0.001) in placebo group. The differences were statistically apparent in all symptom index comparisons (All P<0.05). Overall, 118 (71.5%) of 165 patients had at least one adverse event. The most frequently reported grade 3 and 4 adverse events in the safety set were leukopenia (deemed prior chemotherapy or radiation related). 4 patients had adverse events definitely or suspected to be related to EGCG treatment, with pricking skin sensation (three [2.7%]) and application site pruritus (one [0.9%]). Prophylactic use of EGCG solution substantially reduced the incidence and severity of RID in patients receiving adjuvant radiotherapy and could be a new standard of skin care for patients receiving adjuvant radiotherapy.
Cetuximab Enhanced the Cytotoxic Activity of Immune Cells During Treatment of Colorectal Cancer

Abstract:BACKGROUND/AIMS:Cetuximab is a chimeric IgG1 monoclonal antibody which targets the extracellular domain of epidermal growth factor receptor. This antibody is widely used for colorectal cancer (CRC) treatment but its influence on the immune system is incompletely understood.METHODS:The immune influence of cetuximab therapy in CRC patients was investigated by analyzing peripheral blood mononuclear cells using flow cytometry. We undertook in vitro cytotoxicity and cytokine-profile assays to ascertain the immunomodulatory effect of cetuximab treatment.RESULTS:The number of CD3+ T, CD8+ T, and natural killer (NK) cells was increased significantly and T-regulatory cells reduced gradually after cetuximab treatment. Percentage of CD4+ T, natural killer T (NKT)-like, invariant NKT, and dendritic cells was similar between baseline patients and cetuximab patients. Expression of CD137 on NK and CD8+ T cells was increased significantly after 4 weeks of cetuximab therapy. In vitro cetuximab treatment markedly increased expression of CD137 and CD107a on NK and CD8+ T cells. Cetuximab treatment promoted the cytotoxic activity of NK and CD8+ T cells against tumor cells.CONCLUSION:Cetuximab treatment promotes activation of the immune response but alleviates immunosuppression: this might be the underlying anti-CRC effect of cetuximab.
Clinical management of cutaneous toxicity of anti-EGFR agents

Abstract:Cutaneous toxicity is the most evident adverse effect of epidermal growth factor receptor (EGFR) inhibitors because of the specific role played by EGFR in skin biophysiology. Dermatological adverse reactions, mainly folliculocentric, have been widely reported in the literature. However, the mechanism of these reactions is not well defined and their management is still a matter of debate. In this paper keratinocyte differentiation, activation and pathways regulating gene expression are reviewed in order to improve the understanding of adverse skin reactions and obtain success in their management. The authors had the opportunity to treat skin reactions induced by cetuximab in a cohort of patients affected by metastatic colorectal carcinoma. The aims of this clinical approach were to control the signs and symptoms of skin toxicity in order to avoid delay in cancer therapy and to use nondrug agents for the treatment of drug-induced skin reactions.

Abstract:Objective: To evaluate the efficacy and safety of cetuximab combined with chemotherapy in colorectal cancer （CRC）. Methods: 35 cases of CRC were retrospectively analyzed. Efficacy and adverse events were observed. Results: 29 cases of CRC were evaluated by RECIST criteria, showing 7 PR （partial response, 24.1%） and 15 SD （stable disease, 51.8%）, disease control rate （DC） was 75.9%. Subgroup analysis showed response rate （RR） of 36.4% and DC of 91% in the 1st line therapy, RR of 20% and DC of 70% in the 2nd line therapy, RR of 12.5% and DC of 62.5% in heavily pretreated cases. Rash appeared in 74.3% of patients （grade 3 was 8.6%）, and the severity was relevant with disease control rate （DC）. Neutropenia of grade 3/4 was 14.3%, and infusion related reaction （IRR） of grade 3 happened in 1 case （2.9%）. Conclusion: Cetuximab combined with chemotherapy is safe and effective for patients with metastatic colorectal cancer. The combination therapy shows high DC, especially in 1st line therapy. Severity of rash may predict efficacy.

Abstract:OBJECTIVE:To evaluate the efficacy and safety of cetuximab in combination with chemotherapy for gastrointestinal cancers.METHODS:The data of 35 colorectal cancers (CRC), 11 advanced gastric cancers (AGC) and 3 other gastrointestinal cancers treated with cetuximab in combination with chemotherapy were retrospectively reviewed, and the efficacy and adverse events were analyzed.RESULTS:Of the 49 patients, 37 were evaluable. Twenty-nine CRC were evaluated by RECIST criteria with a PR rate of 24.1%, SD rate of 51.8% and disease control rate (DC) of 75.9%. Subgroup analysis showed that the response rate (RR) and DC rate were 36.4% and 90.9% for the first-line therapy, 20.0% and 70.0% for the second-line therapy, 12.5% and 62.5% in the heavily pre-treated cases, respectively. Six patients with AGC were assessed for efficacy, including 2 PR for first-line therapy, 1 CR and 1 SD for second-line therapy, and 2 with progressive disease in the heavily pre-treated cases. Rash was observed in 79.6% of the patients with a grade 3 rate of 8.2%, and the severity was relevant to the DC. Grade 3 or 4 neutropenia was observed in 16.3%, and grade 3 infusion related reaction (IRR) happened in 4.1% (2 cases).CONCLUSION:Cetuximab combined with chemotherapy is safe and effective for patients with gastrointestinal cancer. In colorectal cancers, the combination therapy shows a high disease control rate, especially in first-line therapy. The severity of rash may be parallel with efficacy. The combination therapy is also promising in the treatment of AGC.

Abstract:Introduction: Cetuximab, used to treat head and neck squamous cell carcinoma and metastatic colorectal cancer, can cause severe infusion reactions. Case presentation: We report an 87-year-old East Asian woman with stage IV ileocecal signet ring cell carcinoma who experienced severe allergic reactions to cetuximab despite pre-treatment. A dose escalation method, involving weekly incremental doses with comprehensive pre-treatment and close monitoring, was employed, successfully reducing allergic reactions and allowing safe administration. Conclusion: This approach demonstrates a viable alternative for patients with hypersensitivity to cetuximab, warranting further research for personalized treatment optimization.
Homologous platelet gel on radiation-induced dermatitis in a patient receiving head and neck radiotherapy plus cetuximab: A case report

Abstract:Introduction: Acute radiodermatitis is a significant complication of cancer radiotherapy, and platelet-based therapies are emerging as potential new treatments. Main symptoms and important clinical findings: In this report, we present the case of a patient with head and neck cancer undergoing radiotherapy combined with the monoclonal antibody cetuximab. After 4 weeks of this treatment, the patient developed cutaneous radiation dermatitis. Despite receiving standard treatment with corticosteroids and emollient cream, the lesion did not improve. Main diagnosis: cutaneous radiation dermatitis on head and neck cancer patient. Therapeutic interventions: Topical application of platelet gel was initiated on the wound. From the second week of radiotherapy to the 4th week, homologous platelet-rich plasma was applied on the dermatitis using a bandage, 4 times a day. Outcomes: The topical treatment with homologous platelet gel resulted in complete healing of the radiodermatitis, including restoration of the epidermis, reepithelialization, and reduction in associated pain. Conclusion: homologous platelet gel might be an alternative to standard treatment of radiation dermatitis.
Evaluation of the additional prophylactic effect of topical steroid ointment to systemic minocycline against anti-epidermal growth factor antibody-induced skin toxicities in metastatic colorectal cancer treatment

Abstract:Background: Anti-epidermal growth factor receptor (EGFR) antibodies often cause skin toxicities. Preemptive skin treatments using systemic antibiotics with or without topical steroid are reportedly effective although the most suitable method remains unclear. This study aimed to determine whether combination prophylaxis using systemic minocycline and topical steroid is superior to minocycline alone in a real-world metastatic colorectal cancer (mCRC) treatment. Methods: Patients with mCRC (n = 87) who received anti-EGFR monoclonal antibodies were retrospectively assessed. The primary objective was to compare the incidence of grade ≥ 2 overall skin toxicities during all treatment periods between the control group receiving prophylactic minocycline 100 mg/day, and the combination prophylaxis group receiving minocycline 100 mg/day + topical steroid. The incidence of each skin symptom was also evaluated. Results: The incidence of grade ≥ 2 overall skin toxicities was 63.6% in the control and 56.9% in the combination groups, with no significant difference (P = 0.63). Similarly, the incidence of grade ≥ 2 dry skin, fissures, paronychia, and pruritus did not significantly differ. In addition, incidence of all-grade skin toxicities was not different. However, the incidence of grade ≥ 2 papulopustular rashes was significantly lower in the combination group (23.1% vs. 50.0%, P = 0.03). Propensity score-matched analysis supported these results. Multivariate logistic regression analysis showed no significant association between combination prophylaxis and grade ≥ 2 overall skin toxicities, but it did show a reduction in grade ≥ 2 papulopustular rashes. Conclusion: Adding topical steroids to systemic minocycline did not mitigate grade ≥ 2 overall skin toxicities induced by anti-EGFR antibodies; however, it significantly improved papulopustular rashes.
Visual and subjective outcomes after diffractive trifocal lens implantation in clear lens exchange

Abstract:OBJECTIVE: To evaluate visual and subjective outcomes after diffractive intraocular lens (IOL) implantation in clear lens exchange. SETTING: Institut Catala de Retina, Barcelona, Spain. METHODS: Prospective, non-interventional study of patients undergoing clear lens refractive surgery with implantation of a trifocal IOL. Subjective refraction and visual acuity were analyzed for uncorrected distant visual acuity (UDVA) and corrected distant visual acuity (CDVA) and for uncorrected near visual acuity (UNVA) and corrected near visual acuity (CNVA), 1, 3 and 6 months after surgery. Aberrometry was performed to determine the higher-order root mean-square (RMS). Bilateral contrast sensitivity function was obtained under mesopic conditions. The quality of visual function was evaluated using the VF-14 method. RESULTS: Mean monocular CDVA was 0.06 ± 0.07 logMAR and mean monocular UNVA was 0.02 ± 0.05 logMAR. Spherical equivalent (SE) 6 months after surgery was 0.03 ± 0.35 D. The safety index for distant vision was 1.0 and the efficacy index was 0.91. Mean higher-order RMS was 0.27 μm. Contrast sensitivity was lower in the postsurgery period, with values within normal ranges for adults. VF-14 value was 92.96. CONCLUSION: Diffractive trifocal IOL implantation after clear lens exchange is a safe and effective method that provides excellent outcomes and high quality visual function in distant and near vision.

Abstract:Dermatitis is a skin condition caused by multiple causes, including radiotherapy treatment. Pharmacological treatments can become chronic and are not exempt from side effects. The latest recommendations of the American Academy of Dermatology establish the use of natural, nourishing, and moisturizing cosmetic products as prevention and the first therapeutic step for dermatitis. Alantel® is a cream developed to reduce redness and irritation, promote the local immune system, combat immunosenescence, and promote the healing of epidermal lesions. The objective was to evaluate the effect of a cream (Alantel) based on natural products at high concentrations for the preventive and curative treatment (at early stages) of radiation-induced dermatitis in patients with breast cancer.

Phase I study of topical epigallocatechin-3-gallate (EGCG) in patients with breast cancer receiving adjuvant radiotherapy

Abstract:OBJECTIVE: The purpose of this study was to investigate the safety, tolerability and preliminary effectiveness of topical epigallocatechin-3-gallate (EGCG) for radiation dermatitis in patients with breast cancer receiving adjuvant radiotherapy.METHODS: Patients with breast cancer who received radiotherapy to the chest wall after mastectomy were enrolled. EGCG solution was sprayed to the radiation field from the initiation of Grade 1 radiation dermatitis until 2 weeks after completion of radiotherapy. EGCG concentration escalated from 40 to 660 μmol l(-1) in 7 levels with 3-6 patients in each level. EGCG toxicity was graded using the NCI (National Cancer Institute Common Terminology Criteria for Adverse Events) v. 3.0. Any adverse event >Grade 1 attributed to EGCG was considered dose-limiting toxicity. The maximum tolerated dose was defined as the dose level that induced dose-limiting toxicity in more than one-third of patients at a given cohort. Radiation dermatitis was recorded weekly by the Radiation Therapy Oncology Group scoring and patient-reported symptoms.RESULTS: From March 2012 to August 2013, 24 patients were enrolled. Acute skin redness was observed in 1 patient and considered to be associated with the EGCG treatment at 140 μmol l(-1) level. Three more patients were enrolled at this level and did not experience toxicity to EGCG. The dose escalation stopped at 660 μmol l(-1). No other reported acute toxicity was associated with EGCG. Grade 2 radiation dermatitis was observed in eight patients during or after radiotherapy, but all decreased to Grade 1 after EGCG treatments. Patient-reported symptom scores were significantly decreased at 2 weeks after the end of radiotherapy in pain, burning, itching and tenderness, p < 0.05.CONCLUSION: The topical administration of EGCG was well tolerated and the maximum tolerated dose was not found. EGCG may be effective in treating radiation dermatitis with preliminary investigation.ADVANCES IN KNOWLEDGE: EGCG solution seemed to be feasible for treating radiation dermatitis in patients with breast cancer after mastectomy. It should be tested as a way to reduce radiation-induced normal tissue toxicity and complications in future years.
Management of acute infusion reactions associated with cetuximab therapy in clinic

Abstract:Objective: To describe the clinical consequences of acute infusion reactions associated with cetuximab treatment and analyze corresponding managing experience to explore the prevention and treatment approaches. Methods: Clinic staff identified 24 patients with advanced gastric cancer and colorectal cancer who had received chemotherapy combined with cetuximab treatment and experienced an acute infusion reaction. Their clinical manifestation and infusion reaction management procedures were retrospectively analyzed. Results: Acute infusion reactions at gradeⅠ-Ⅱ were observed in 4 of 24 cases (16.7%), occurring during the first 5-10 min after the first administration of cetuximab. Patients complained chills, chest tightness, and low fever (37-38.5℃) after 0.5 h infusion. The symptoms were self relieved after 1-2 h. When cetuximab was recharged at reduced infusion rate and with close monitoring, no acute infusion reactions were observed in the 4 patients. Conclusion: Discontinuation of cetuximab therapy is not necessary for the patients who experienced grade Ⅰ-Ⅱ infusion reaction mild-to-moderate reactions can be managed by reduction of the infusion rate and close observation of the symptoms. Patients who experienced grade Ⅲ or Ⅳ infusion reactions may require permanent discontinuation of cetuximab therapy.
Successful cetuximab rechallenge in metastatic colorectal cancer: A case report

Abstract:Background: Metastatic colorectal cancer (mCRC) treatment has been evolving and increasingly driven by tumor biology and gene expression analysis. Rechallenge with epidermal growth factor receptor (EGFR) inhibitors (anti-EGFR) represents a promising strategy for patients with RAS wild-type (RAS-wt) mCRC and circulating tumor DNA has emerged as a potential selection strategy. Herein, we report the case of a RAS-wt mCRC patient who had a successful response to cetuximab rechallenge. Case summary: Our patient was diagnosed with stage IV RAS-wt, microsatellite-stable rectosigmoid junction adenocarcinoma. He was started on first-line treatment with FOLFIRI and cetuximab and achieved partial response, allowing for a left hepatectomy (R0), followed by post-operative chemotherapy and an anterior resection; progression-free survival (PFS) of 16 months was obtained. Due to hepatic and nodal relapse, second-line treatment with FOLFOX and bevacizumab was started with partial response; metastasectomy was performed (R0), achieving a PFS of 11 months. After a 15 months anti-EGFR-free interval, FOLFIRI and cetuximab were reintroduced upon disease progression, again with partial response and a PFS of 16 months. Following extensive hepatic relapse, cetuximab was reintroduced and a marked clinical and analytical improvement was seen, after only one cycle. RAS-wt status was confirmed on circulating tumor DNA. The patient's overall survival exceeded 5 years. Conclusion: Our case provides real-world data to support cetuximab rechallenge in later lines of RAS-wt mCRC treatment.
Cetuximab in treatment of metastatic colorectal cancer: background and clinical observation

Abstract:Today, the researchers continue the search for the most optimal regimens of drug therapy for metastatic colorectal cancer (mCRC) which are supposed to increase progression-free survival (PFS) and overall survival (OS), improve patient quality of life. Due to significant progress in chemotherapy (CT) and surgical treatment of mCRC, and the multidisciplinary approach, the treatment algorithms have changed. The increase in life expectancy of patients is observed when all three of the most active chemotherapy drugs in this disease: oxaliplatin (Oxa), irinotecan (Iri), fluoropyrimidines are administered. The inclusion of the targeted drug cetuximab in modern mCRC treatment regimens led to a statistically significant increase in the objective response rate (ORR), median PFS and OS. The article presents the results of the most significant clinical studies of the eficacy of the antiEGFR drug cetuximab in combination with standard CT regimens for the first- and second-line treatment of mCRC, and describes a clinical case of the successful use of cetuximab in mCRC therapy.

Abstract:AIM: To review and assess the evidence related to cetuximab treatment in metastatic colorectal cancer (mCRC) with regard to KRAS status.METHODS: PubMed, EMBASE, Cochrane database and American Society of Clinical Oncology meeting abstracts were searched for randomized controlled trials (RCTs) reporting the effect of KRAS status on efficacy of chemotherapy regimen with or without cetuximab in mCRC. Baseline information such as sex and age was summarized from the included studies. Hazard ratios of progression-free survival (PFS) and overall survival (OS) as well as objective response based on KRAS status were extracted for analysis.RESULTS: A total of 8 RCTs with 6780 patients were included. The combined analysis showed that cetuximab failed to improve the OS and PFS in patients with mCRC. However, in subgroup analysis, the pooled data showed that addition of cetuximab to irinotecan containing chemotherapy regimen was sufficient to improve OS and PFS in wild-type KRAS mCRC patients, but not in patients with mutant-type KRAS. The addition of cetuximab increased the incidence of adverse events such as diarrhea, rash, skin toxicity/rash, and nausea and vomiting. There was no significant publication bias existing in the included studies.CONCLUSION: The clinical benefit of cetuximab was only confirmed in patients with wild-type KRAS. KRAS status could be considered a biomarker of efficacy of cetuximab.
Cetuximab Plus Irinotecan in Treatment of Metastatic Colorectal Cancer

Abstract:Objective To evaluate the clinical efficacy and safety of cetuximab plus irinotecan in the treatment of metastatic colorectal cancer,and to investigate the correlation of KRAS mutation and clinical efficacy.Methods Analyzed the 15 patients of metastatic colorectal cancer with histologically confirmed diagnosis treated with cetuximab in combination with irinotecan or FOLFIRI regimen.KRAS mutation was detected in 12 tumor tissues and investigated the correlation of KRAS mutation and clinical efficacy.Results(1)Efficacy analysis:The objective response rate was 26.7%,4 patients with PR(partial response),7 patients with SD(stable disease)and 4 patients with PD(progression disease).The disease control rate was 73.3% and the median time of tumor progression was 16 weeks.The major toxicities were acneform eruptions,diarrhea and bone marrow depression,their incidence rate was 80%(12/15).46.7%(7/15)and 60%(9/15)respectively.In 12 acneform eruptions patients,the objective response rate was 33.3% and the disease control rate was 83.3%.(2)Correlation of KRAS mutation and efficacy:KRAS gene was detected in 12 patients.7 patients of KRAS were wild type,and the objective response rate was 28.6%,the disease control rate was 85.7% and the median time of tumor progression was 18 weeks.5 patients of KRAS were mutation type,and the objective response rate was 20%,the disease control rate was 60% and the median time of tumor progression was 12 weeks.Because of less cases,the difference had no statistical significance.Conclusion Cetuximab in combination with irinotecan has clinically significant activity in patients with refractory metastatic colorectal cancer.The toxicity,except rash,is not more frequent among the patients receiving cetuximab in comb ination with irionotecan than irionotecan alone.Acneform eruptions maybe prognosticate clinical benefit.Therapeutic effect of the patients with wild type KRAS may superior to those patients with mutant type KRAS when receiving cetuximab therapy.
Risk Factor Analysis for Anti-epidermal Growth Factor Receptor Monoclonal Antibody-induced Problematic Skin Toxicities in Patients With Liver Metastatic Colorectal Cancer

Abstract:Background/aim: We previously reported that patients with metastatic colorectal cancer (mCRC) and baseline liver metastasis are at a higher risk of developing grade ≥2 overall skin toxicities when treated with anti-epidermal growth factor receptor (EGFR) monoclonal antibody. This study aimed to identify additional factors associated with skin toxicities induced by anti-EGFR treatment in patients with liver metastatic CRC. Patients and methods: Patients with liver metastatic CRC who initially received anti-EGFR monoclonal antibody-containing treatment (n=77) were retrospectively assessed. The primary endpoint was to identify the factor(s) responsible for the development of grade ≥2 overall skin toxicities. Additionally, factors for grade ≥2 rash and paronychia were evaluated. Results: The incidence of grade ≥2 overall skin symptoms, rash, and paronychia was 62.3%, 31.2%, and 28.6%, respectively. Multivariate Cox proportional hazard regression analyses revealed that age <65 years and anemia were independent baseline risk factors for grade ≥2 overall skin toxicities (adjusted hazard ratio 2.09, 95% confidence interval=1.10-3.97, p=0.02 for age; 2.36, 1.20-4.61, p=0.01 for anemia). In contrast, combination prophylaxis using systemic minocycline and corticosteroid ointment was a preventive factor (0.47, 0.25-0.88, p=0.02). Males and age <65 years were baseline risk factors for grade ≥2 rash, and combination prophylaxis was identified as a preventive factor. No factors were identified for paronychia. Conclusion: Age <65 years and anemia were identified as independent baseline risk factors. Additionally, combination prophylaxis was found to be a preventive factor against anti-EGFR monoclonal antibody-induced grade ≥2 overall skin toxicities in patients with liver metastatic CRC.
Oral mucositis associated with anti-EGFR therapy in colorectal cancer: single institutional retrospective cohort study

Abstract:Background: Chemotherapy-induced oral mucositis impairs the quality of life. The difference in severity of oral mucositis between different anti-epidermal growth factor receptor (EGFR) antibodies combined with cytotoxic drugs in colorectal cancer is unclear. The aim of this study was to investigate the differences in oral mucositis between panitumumab (Pmab) and cetuximab (Cmab) combined with 5-fluorouracil (5-FU). Methods: We conducted a retrospective cohort study. A total of 75 colorectal cancer outpatients treated with an anti-EGFR antibody combined with FOLFOX, FOLFIRI, or 5-FU/leucovorin as the first- to third-line treatment were included. The primary endpoint was the incidence of grade 2-3 oral mucositis. The secondary endpoint was the time to onset of oral mucositis. We also compared the incidence of toxicities of interest, skin toxicity, hypomagnesaemia and neutropenia, and time to treatment failure (TTF) between the two groups. Results: Thirty-two patients treated with Pmab and 43 patients treated with Cmab were evaluated. Patient characteristics were similar between the two groups. The incidence of grade 2-3 oral mucositis was significantly higher with Pmab than with Cmab (31.3% vs 9.3%, P < 0.05). Moreover, the incidence of grade 3 oral mucositis was significantly higher in patients treated with Pmab (18.8% vs 0%, P < 0.01). The mean (SD) cycles to onset of the worst oral mucositis was 3.0 (2.9) in the Pmab group and 2.3 (1.7) in the Cmab group (P = 0.29). Oral mucositis was characterized by glossitis and cheilitis. The incidences of other toxicities were the following (Pmab vs Cmab): grade 2-3 skin toxicity: 68.8% vs 74.4% (P = 0.61), grade 2-3 hypomagnesaemia: 9.3% vs 7.0% (P = 1.00), grade 3-4 neutropenia: 28.1% vs 37.2% (P = 0.46). The median TTF was not significantly different, i.e., 223 days vs 200 days (P = 0.39) for Pmab vs Cmab. Conclusions: Pmab-based chemotherapy resulted in significantly higher grades of oral mucositis compared with Cmab-based chemotherapy. The oral condition should be monitored carefully and early supportive care should be provided for patients treated with Pmab-based chemotherapy.
