Abstract:For many years, successful noninvasive blood glucose monitoring assays have been announced, among which near-infrared (NIR) spectroscopy of skin is a promising analytical method. Owing to the tiny absorption bands of the glucose buried among a dominating variable spectral background, multivariate calibration is required to achieve applicability for blood glucose self-monitoring. The most useful spectral range with important analyte fingerprint signatures is the NIR spectral interval containing combination and overtone vibration band regions. A strategy called science-based calibration (SBC) has been developed that relies on a priori information of the glucose signal (“response spectrum”) and the spectral noise, i.e., estimates of the variance of a sample population with negligible glucose dynamics. For the SBC method using transcutaneous reflection skin spectra, the response spectrum requires scaling due to the wavelength-dependent photon penetration depth, as obtained by Monte Carlo simulations of photon migration based on estimates of optical tissue constants. Results for tissue glucose concentrations are presented using lip NIR-spectra of a type-1 diabetic subject recorded under modified oral glucose tolerance test (OGTT) conditions. The results from the SBC method are extremely promising, as statistical calibrations show limitations under the conditions of ill-posed equation systems as experienced for tissue measurements. The temporal profile differences between the glucose concentration in blood and skin tissue were discussed in detail but needed to be further evaluated.

Abstract:Noninvasive glucose detection is highly required for more convenient and less pain glycaemic monitoring. Most of currently used methods are invasive. In this paper, a near-infrared reflectance spectroscopy (NIRS) is proposed to detect blood glucose to protect patient absent of pain. NIRS is a safe, simple and efficient technology applied in many fields. Experiments, based on Oral Glucose Tolerance Test (OGTT), were conducted to collect data modeling with partial least squares (PLS) regression. 42 samples of fingertip blood and palm were measured by commercially available blood glucose meter and NIRS separately at the same time. The glucose concentration range is between 5 and 12 mmol·L-1. With leave-one-out cross-validation, we obtained a result of root mean square error of cross-validation (RMSECV) of 1.16 mmol·L-1 for all the data. With the pre-processing methods of normalization and un-informative variables elimination reducing noise and eliminating some additional effects, we get a better result of 0.79 mmol·L-1. A RMSECV of 0.41 mmol·L-1 for individual modeling is much less than the total modeling. It has a broad application prospect in individual customization.

Abstract:<h3 class="a-plus-plus">Abstract</h3> <p class="a-plus-plus">Diabetes mellitus is a widespread disease with greatly rising patient numbers expected in the future, not only for industrialized countries but also for regions in the developing world. There is a need for efficient therapy, which can be via self-monitoring of blood glucose levels to provide tight glycemic control for reducing the risks of severe health complications. Advancements in diabetes technology can nowadays offer different sensor approaches, even for continuous blood glucose monitoring. Non-invasive blood glucose assays have been promised for many years and various vibrational spectroscopy-based methods of the skin are candidates for achieving this goal. Due to the small spectral signatures of the glucose hidden among a largely variable background, the largest signal-to-noise ratios and multivariate calibration are essential to provide the method applicability for self-monitoring of blood glucose. Abstract:Noninvasive and minimally invasive techniques for monitoring glucose via the skin are reviewed. These approaches rely either on the interaction of electromagnetic radiation with the tissue or on the extraction of fluid across the barrier. The structure and physiology of the skin make the technical realization of transdermal glucose monitoring a difficult challenge. The techniques involving transdermal fluid extraction circumvent and/or compromise the barrier function of skin's outermost and least permeable layer, the stratum corneum, by the application of physical energy. While sonophoresis and microporation methods, for example, are in relatively early-stage development, a device using reverse iontophoresis [the GlucoWatch Biographer (Cygnus, Inc., Redwood City, CA)] is already commercially available. Optical techniques to monitor glucose are truly noninvasive. The tissue is irradiated, the absorbed or scattered radiation is analyzed, and the information is processed, to provide a measure proportional to the concentration of glucose in the dermal tissue. These techniques include near-infrared and Raman spectroscopy, polarimetry, light scattering, and photoacoustic spectroscopy. By contrast, impedance spectroscopy measures changes in the dielectric properties of the tissue induced by blood glucose variation. Large-scale studies in support of efficacy of these methodologies are as yet unavailable. At present, therefore, transdermal fluid extraction technologies are offering greater promise in terms of practical and realizable devices for patient use. The truly noninvasive allure of the optical approach assures continued and intense research activity--for the moment, however, an affordable, efficient, and portable system is not on the immediate horizon.

Abstract:In non-invasive blood glucose measurement, the sensitivity of glucose-induced optical signals within human tissue is a crucial reference point. This study evaluates the sensitivity of glucose-induced diffuse reflectance in the 1000–1700 nm range. A key factor in understanding this sensitivity is the rate at which the scattering coefficient changes due to glucose, as it is significantly higher than in non-living media and predominantly influences the diffuse light signal level when blood glucose levels change. The study measured and calculated the changes in the scattering coefficient at 1314 nm, a wavelength chosen for its minimal interference from glucose absorption and other bodily constituents. Based on the Mie scattering theory and the results at 1314 nm, the changes in the scattering coefficient within the 1000–1700 nm range were estimated. Subsequently, the sensitivity of the glucose signal across this range was determined through Monte Carlo (MC) simulations. The findings from 25 human trials indicate that the measured sensitivities at five other typical wavelengths within this band generally align with the sensitivities calculated using the aforementioned method. This research can guide the identification of blood glucose signals and the selection of wavelengths for non-invasive blood glucose measurements.

Abstract:The prevalence of diabetes is rapidly increasing worldwide and can lead to a range of severe health complications that have the potential to be life-threatening. Patients need to monitor and control blood glucose levels as it has no cure. The development of non-invasive techniques for the measurement of blood glucose based on photoacoustic spectroscopy (PAS) has advanced tremendously in the last couple of years. Among them, PAS in the mid-infrared (MIR) region shows great promise as it shows the distinct fingerprint region for glucose. However, two problems are generally encountered when it is applied to monitor real samples for in vivo measurements in this MIR spectral range: (i) low penetration depth of MIR light into the human skin, and (ii) the effect of other interfering components in blood, which affects the selectivity of the detection system. This review paper systematically describes the basics of PAS in the MIR region, along with recent developments, technical challenges, and data analysis strategies, and proposes improvements for the detection sensitivity of glucose concentration in human bodies. It also highlights the recent trends of incorporating machine learning (ML) to enhance the detection sensitivity of the overall system. With further optimization of the experimental setup and incorporation of ML, this PAS in the MIR spectral region could be a viable solution for the non-invasive measurement of blood glucose in the near future.

Abstract:Near-infrared spectroscopy continues to be a method with great potential for developing noninvasive devices for monitoring blood glucose in patients with diabetes mellitus. This article discusses the main problems hindering creation of accurate noninvasive sensors, which are linked mainly with difficulties in mathematical modeling of the distribution of irradiation in biological tissues and the need for improvements to the component basis for designing portable devices. Current approaches to improving measurement systems and mathematical systems for computing glucose concentrations are reviewed.
Preliminary Clinical Validation of a Differential Correction Method for Improving Measurement Accuracy in Noninvasive Measurement of Blood Glucose Using Near-Infrared Spectroscopy

Abstract:One of the main challenges in the noninvasive sensing of blood glucose by near-infrared (NIR) spectroscopy is the background variations from light source drift, sweating, and temperature change at the human–machine interface. In this paper, a differential correction method based on the spectra from the floating-reference position and measuring position is proposed to eliminate these spectral variations from background interferences. Its effectiveness was validated by in vitro and in vivo experiments in which the diffuse reflectance of intralipid solutions and human skin was collected at the source distances of 0.6 mm and 2 mm by the custom-built system with six super-luminescent emitting diodes (SLEDs) light source. The results showed that, for the in vitro experiments of intralipid solutions, the coefficients of variations of diffuse reflectance decreased by 20.5% under all the six wavelengths after differential correction. For the in vivo experiments of oral glucose tolerance tests (OGTTs), partial least squares (PLS) regression models between glucose concentrations and the diffuse reflectance from palm skin were built, and the root mean square error of cross validation (RMSECV) decreased by 38.0% on average after the differential correction. Further, the spectra of the oral water tolerance tests (OWTTs) were collected for correlation with glucose concentration in OGTTs, and their correlation coefficients (R) decreased by 35.0% on average after the differential correction. Therefore, this differential correction method based on the spectra from the floating-reference position and measuring position can weaken the influence of background variations on the NIR spectroscopy and has promising potential in in vivo detection, especially for noninvasive blood glucose measurement.

A Single Wavelength Mid-Infrared Photoacoustic Spectroscopy for Noninvasive Glucose Detection Using Machine Learning

Abstract:According to the International Diabetes Federation, 530 million people worldwide have diabetes, with more than 6.7 million reported deaths in 2021. Monitoring blood glucose levels is essential for individuals with diabetes, and developing noninvasive monitors has been a long-standing aspiration in diabetes management. The ideal method for monitoring diabetes is to obtain the glucose concentration level with a fast, accurate, and pain-free measurement that does not require blood drawing or a surgical operation. Multiple noninvasive glucose detection techniques have been developed, including bio-impedance spectroscopy, electromagnetic sensing, and metabolic heat conformation. Nevertheless, reliability and consistency challenges were reported for these methods due to ambient temperature and environmental condition sensitivity. Among all the noninvasive glucose detection techniques, optical spectroscopy has rapidly advanced. A photoacoustic system has been developed using a single wavelength quantum cascade laser, lasing at a glucose fingerprint of 1080 cm−1 for noninvasive glucose monitoring. The system has been examined using artificial skin phantoms, covering the normal and hyperglycemia blood glucose ranges. The detection sensitivity of the system has been improved to ±25 mg/dL using a single wavelength for the entire range of blood glucose. Machine learning has been employed to detect glucose levels using photoacoustic spectroscopy in skin samples. Ensemble machine learning models have been developed to measure glucose concentration using classification techniques. The model has achieved a 90.4% prediction accuracy with 100% of the predicted data located in zones A and B of Clarke’s error grid analysis. This finding fulfills the US Food and Drug Administration requirements for glucose monitors.

Abstract:The blood glucose concentrations of volunteers from diabetes patients and healthy adults (all patients and volunteers who joined this experiment gave their consents) were measured by using a modified WQF-200 FTIR spectrometer with a newly designed ATR accessory from the Beijing Rayleigh Analytical Instrument Corp. The determination basis for this technique from the physiological point of view is also discussed based on the experimental results, which indicated that the glucose measured by the FT-Mid-IR-ATR instrument is from the secretions on the skin and glucose components within the body. The secreted glucose components will increase with the time increasing. The authors' previous study([3-8]) demonstrated 1 120 cm(-1) band as an index to characterize the blood glucose. During the experiments, the authors used the band of 1 455 cm(-1) as internal standard because of its stability, and because the relative intensity of I-1 (120)/I-1 (455) band possesses the higher sensitivity. Meanwhile, from the spectra, the relative intensity of I-1 120/I-1 455 band of the glucose in both sources exhibits a linear relationship with blood glucose concentration within the body. The dried blood has the similar spectra as fingers'. The fingers' spectra will exhibit higher sensitivity if the time of measurement is longer after washing hand, and the results showed that when measured 10 minutes after washing hand, the sensitivity will be higher than that when measured 4 minutes after washing hand. All the results can be used in promoting a convenient, rapid and noninvasive way to monitor the continuous variation of blood glucose concentration of diabetes patients in real time.

Near Infrared Spectroscopy is one of the most popular methods used in studies; however, despite more than 20 years of research, a practical and reliable noninvasive NIR glucose sensor is yet to be developed. In this study, we investigated the feasibility of NIRS towards the detection of glucose concentration. Although we can obtain adequate sensitivity, our measurements suffer from poor selectivity due to the fact that we can only detect the impurity level of water by NIRS due to strong water absorbance.

Abstract:Diabetes is a serious health condition that requires patients to regularly monitor their blood glucose level, making the development of practical, compact, and non-invasive techniques essential. Optical glucose sensors-and, specifically, NIR sensors-have the advantages of being non-invasive, compact, inexpensive, and user-friendly devices. However, these sensors have low accuracy and are yet to be adopted by healthcare providers. In our previous work, we introduced a non-invasive dual-channel technique for NIR sensors, in which a long channel is utilized to measure the glucose level in the inner skin (dermis) layer, while a short channel is used to measure the noise signal of the superficial skin (epidermis) layer. In this work, we investigated the use of dual-NIR channels for patients with different skin colors (i.e., having different melanin concentrations). We also adopted a Monte Carlo simulation model that takes into consideration the differences between different skin layers, in terms of blood content, water content, melanin concentration in the epidermis layer, and skin optical proprieties. On the basis of the signal-to-noise ratio, as well as the sensitivities of both the epidermis and dermis layers, we suggest the selection of wavelengths and source-to-detector separation for optimal NIR channels under different skin melanin concentrations. This work facilitates the improved design of a compact and non-invasive NIR glucose sensor that can be utilized by patients with different skin colors.

Abstract:Non-invasive blood glucose measurement using near infrared (NIR) spectroscopy relies on wavebands that provide reliable information about spectral absorption. In this study, we investigated wavebands which are informative for blood glucose in the NIR shortwave band (900∼1450 nm) and the first overtone band (1450∼1700 nm) through a specially designed NIR Fourier transform spectrometer (FTS), which featured a test fixture (where a sample or subject’s finger could be placed) and all-reflective optics, except for a Michelson structure. Different concentrations of glucose solution and seven volunteers who had undergone oral glucose tolerance tests (OGTT) were studied to acquire transmission spectra in the shortwave band and the first overtone band. Characteristic peaks of glucose absorption were identified from the spectra of glucose aqueous solution by second-order derivative processing. The wavebands linked to blood glucose were successfully estimated through spectra of the middle fingertip of OGTT participants by a simple linear regression and correlation coefficient. The light intensity difference showed that glucose absorption in the first overtone band was much more prominent than it was in the shortwave band. The results of the SLR model established from seven OGTTs in total on seven participants enabled a positive estimation of the glucose-linked wavelength. It is suggested that wavebands with prominent characteristic peaks, a high correlation coefficient between blood glucose and light intensity difference and a relatively low standard deviation of predicted values will be the most informative wavebands for transmission non-invasive blood glucose measurement methods. This work provides a guidance for waveband selection for the development of non-invasive NIR blood glucose measurement.

Abstract:Diabetes mellitus is one of the most relevant noncommunicable diseases; the WHO figures in its latest update that 422 million people suffer from it; additionally, it has remained for more than 20 years within the 10 main causes of death worldwide; this disease affects the population at any age; glucose measurement is used to assist the treatment of this disease by different methods that are classified as invasive, minimally invasive, and noninvasive, the latter being an area of recent development due that it is not traumatic for patients. This work consists of the experimental characterization of an optical system for plasma glucometry using near infrared by spectrophotometry. This glucometry system is based on the employ of an infrared LED with a wavelength of 1650 nm, a beam angle of 16°, and an output power of 1.6 mW that passes through the analyte (glucose in blood plasma) that is contained in cuvettes of different materials (acrylic and quartz) to subsequently affect a photodiode with different active areas ranging from 0.06 mm to 1.5 mm in order to evaluate the efficiency by comparing the sensitivity in the presence of glucose making additions ranging 100 mg/dl–1000 mg/dl within a dark chamber. The experiments showed that the use of photodiodes with a larger active area and the use of quartz cuvettes show a higher sensitivity compared to photodiodes with small active areas and the use of acrylic cuvettes. This configuration presented an R2 of 0.99 and a sensitivity of 0.225 mV/1 mg/dl of glucose; despite the fact that the initial voltage in each of the experimental repetitions varies, the downward voltage pattern is maintained; based on this, it is concluded that this method using this setup is feasible for plasma glucose measurement.

Abstract:Frequent measurement of blood glucose concentration in diabetic patients is an important means for diabetes control. Blood glucose monitoring with noninvasive detection technology can not only avoid the pain of patients and eliminate the harm of some biological materials for measuring glucose in vivo but also improve the frequency of detection, so as to control blood glucose concentration more closely. Traditional blood glucose detection methods are invasive and have some limitations. In this study, the significance of noninvasive blood glucose testing was analyzed and was pointed out that noninvasive blood glucose testing can monitor the blood glucose concentration of patients and relieve the pain of patients. Then, this study analyzed the spectral detection methods of noninvasive blood glucose, including conservation of energy metabolism, near infrared spectroscopy, and other spectral detection methods. Finally, this study made a comprehensive analysis of the domestic and international clinical application of noninvasive glucose spectrum monitoring and summarized the clinical application status of noninvasive glucose spectrum monitoring.

The results from different selected spectrum regions and reprocessing methods show as follows: 1) In the spectrum region of 9,000-12,000 cm-1 using MIN-MAX normalization reprocessing method to set up a model and predict, the errors of prediction were more than 36 mg.dL-1; the model errors were under 16 mg.dL-1 and predictive errors were less than 25 mg.dL-1 by using smoothing and second derivatives in the same region; in the another region of 4,000-5,000 cm-1 with smoothing and first derivatives both error of model and prediction were more than 25 mg.dL-1. 2) In the region of 4,000-9,000 cm-1 with smoothing and second derivatives, the errors from model were less than 15 mg.dL-1, the errors of prediction were less than 31 mg.dL-1; selecting the spectrum regions of 6,100-7,500 cm-1 and 4,200-4,700 cm-1 by a quantitative software OPU VERSION 3.01 with smoothing, first derivatives and vector normalization, the errors of model were no more than 11 mg.dL-1, the errors of predication were no more than 22 mg.dL-1. 3) Choosing the sample from the subject itself to establish the model in 9,000-12,000 cm-1 with smoothing and second derivatives, the errors of model and predication were less than 15 mg.dL-1 and 11 mg.dL-1 respectively. The results showed that the method was feasible to monitoring the blood glucose concentration by selecting an appropriate spectrum region and reprocessing methods with partial least square algorithm which set up a mathematical model. Excepting the number of sample, selected spectrum region and reprocessing methods, the other influencing factors such as the variant from the subject were discussed also.

Abstract:Non-invasive glucose monitoring (NIGM) represents an attractive alternative to finger pricking for blood glucose assessment and management of diabetes. Nevertheless, current NIGM techniques do not measure glucose concentrations in blood but rely on indirect bulk measurement of glucose in interstitial fluid, where glucose is diluted and glucose dynamics are different from those in the blood, which impairs NIGM accuracy. Here we introduce a new biosensor, termed depth-gated mid-infrared optoacoustic sensor (DIROS), which allows, for the first time, non-invasive glucose detection in blood-rich volumes in the skin. DIROS minimizes interference caused by the stratum corneum and other superficial skin layers by time-gating mid-infrared optoacoustic signals to enable depth-selective localization of glucose readings in skin. In measurements on the ears of (female) mice, DIROS displays improved accuracy over bulk-tissue glucose measurements. Our work demonstrates how signal localization can improve NIGM accuracy and positions DIROS as a holistic approach, with high translational potential, that addresses a key limitation of current NIGM methods.

Abstract:Noninvasive measurement of blood glucose is a focal topic in the world.The mathematic relations regarding instrument precision,regression method and prediction accuracy are put forward.They are tested and analyzed through the determination of glucose concentration in an aqueous solution with nearinfrared(NIR) spectroscopy and chemometrics.The results show that the propagation functions provide a prediction accuracy estimate method for spectroscopic measurement.In addition,they can determine the necessary instrumental requirement for the expected prediction accuracy when certain regression method is used.This enables theoretic guides to the measuring methods and references for prediction accuracy analysis of the noninvasive glucose measurement and even the nearinfrared spectroscopy. 
Research on the Best Measurement Situation Between Optical Probe and Tissue Surfaces in Non-Invasive Detection.

Abstract:Near-infrared spectroscopy is often used for the non-invasive detection of composition in the human body, such as that of blood glucose and haemoglobin, due to its high penetration depth into tissues. Although it is feasible to position the optical probe precisely, contact situation between probe and human tissues is a difficult problem to determine because of physiological tremor and mechanical performance of bio-soft tissue. Here, we proposed a novel estimation method for the situation between the optical probe and tissue surfaces based on the dynamic auto-correlation matrix of two-dimensional correlation spectroscopy (2DCOS) and radar chart. The diffuse reflectance spectra from the left palm of 4 healthy volunteers were collected while the optical probe gradually approached and pressed bio-tissues with a custom-design controlling device. 2DCOS in the wavelength with lower absorption (1000-1400 nm) was calculated under the perturbation of relative-distance and contact pressure between the optical probe and tissue surface. The synchronous 2DCOS showed that the surface reflection and diffuse reflectance were greatly affected by the contact conditions in 1100 nm, 1220 nm, and 1300 nm. Then the dynamic auto-correlation matrix of 2DCOS was established for the adjacent spectra, and the significant difference wavelengths were used to build radar charts to determine the critical contact situation visually. Results showed that the maximum variations of dynamic auto-correlation matrix appeared at near 1300 nm, and the relative distance between the probe and tissue corresponding to the critical contact state can be easily observed with radar charts with 0.25 mm uncertainty, which was consistent with the self-feeling of each volunteer. So this method can be applied to exactly determine the optimal measurement status for the non-invasive body composition detection in vivo. It is important for the design of human-machine interface and the accuracy improvement of body composition measurements.
