Abstract:Objective:To evaluate the efficacy and safety of oxycodone combined with dexmedetomidine in the postoperative analgesia in the patients underwent laparoscopic radical surgery of colon cancer.Methods:Sixty colon cancer patients underwent laparoscopic radical surgery and recevied patient-controlled intravenous analgesia after operation were randomly divided into control group and experiment group (n=30)using The "Envelope Method". The patients in control group were given 0.5 mg·kg-1 oxycodone,and the patients in experiment group were given 0.5 mg·kg-1 oxycodone and 2 μg·kg-1 dexmedetomidine.The parameters of analgesic pump in two groups were set as follows:the continuous perfusion rate was 1 mL·h-1 ,the single bolus was 1.5 mL,the lock-out time was 10 min,and the total volume was 100 mL.The VAS scores of pain at rest and activity pain of the patients in two groups were evaluated at exiting the post anesthesia care unit (T1 ),and at 6 h (T2 ),12 h (T3 ),24 h (T4 )and 48 h (T5 )after operation.The dose of analgesic drugs used,times of emergent rescue analgesia and anus exhaust time after operation of the patients in two groups were recorded;the postoperative adverse responses,vital signs and Ramsay scores of the patients in two groups were observed.Results:The VAS scores of activity pain at T2 -T5 and pain at rest at T2 -T4 of the patients in experiment group (P T2-T5 =0.000;P T2 =0.018,P T3 =0.001,P T4 =0.010)were lower than those in control group;the doses of analgesic drugs used and times of emergent rescue analgesia of the patients in experiment group were significantly lower than those in control group (P =0.000,P =0.029);the anus exhaust time after operation was significantly earlier than that in control group (P =0.030);the cases of nausea of the patients in experiment group were significantly lower than that in control group (P =0.023), and there were no significantly differences in the other various adverse effects of the patients between two groups (P >0.05).Conclusion: Oxycodone combined with dexmedetomidine is effective and safe for the postoperative analgesia after laparoscopic radical surgery of colon cancer.
Preemptive analgesia effects of flurbiprofen axetil in patients divesting tonsils

Abstract:Objective To observe the postoperative analgesia effects of pre-operation IV flurbiprofen axetil and its adverse reaction in patients after divesting their tonsils. Methods Thirty two patients were randomly divided into two groups ( n=16 each): flurbiprofen group(group F) and fentanyl control group(group C).In group F , 1 mg/kg flurbiprofen was used intravenously after anesthesia induction and the operation respectively. In group C, fentanyl 15～20 μg/kg was intravenously used for patient-controlled analgesia until 48 hrs after divesting tonsils. Analgesia score and the incidence of adverse reactions were mensurated by the method of visual analogue scale (VAS) at 1, 2, 4, 6, 9, 12 and 24 hrs after divesting tonsils in the two groups. Results Compared with group C, the VAS scores in group F at 1,2,4,6 and 24 hrs after the operation had no significant difference ( P>0.05), but higher at 9 and 12 hrs than that of group C ( t=3.31,9.19 , P <0.05);the incidences of postoperative vomiting and drowsiness significantly reduced( χ 2=6.00,5.93, P<0.05). Conclusion Flurbiprofen has good effects of postoperative analgesia in the patients after divesting tonsils and reduces the incidence of adverse reaction.
Effects of parecoxib sodium preemptive analgesia on perioperative cytokine responses and stress responses in patients undergoing ophthalmology surgery

Abstract:Objective To investigate the effects of parecoxib sodium preemptive analgesia on perioperative cytokine responses and stress respses and postoperative analgesia in patients undergoing ophthalmology surgery. Methods One hundred ASA Ⅰ- Ⅱ patients undergoing ophthalmology surgery were randomly divided into 2 groups of fifty: the parecoxib group received parecoxib 40 mg by muscle injection, the control group received an equal volume of 0. 9% normal saline two ml. Venous blood samples were obtained at 10 minutes before parecoxib or 0. 9% normal saline was injected (T1) , 30 minutes ( T2) and 60 minutes (T3) after surgery was initiated, the moment when surgery was finished ( T4), 6 hours (T5 ), 12 hours(T6)and 24 hours (T7 ) after surgery was finished for determination of the plasma levels of IL-6, IL-8,IL-1RA,TNF and IL-1β. At the same time, the plasma levels of epinephrine, norepinephrine, cortisol and adrenocorticotropic hormone at Tl - T6 were measured. VAS scores with patients were recorded at 1,2, 6,12,24 h after surgery. Result The change of the plasma levels of IL-6, IL-8, IL-1RA, TNF, IL-1β in the two groups:in the parecoxib group the plasma levels of TNF and IL-1β did not decreased significantly as compared with the control group ( P > 0. 05 ) . The plasma levels of IL-6, IL-8 at T4 - T6 decreased significantly as compared with the control group (P < 0. 01) . The plasma levels of IL-1RA at T3 - T6 decreased significantly as compared with the control group ( P < 0. 01 ). The change of the plasma levels of epinephrine, norepinephrine, cortisol and adrenocorticotropic hormone in the two groups: in the parecoxib group the plasma levels of epinephrine, norepinephrine at T2 ,T3 decreased significantly as compared with the control group ( P < 0. 01 ). In the parecoxib group the plasma levels of cortisol at T2 - T4 decreased significantly as compared with the control group(P <0.01 ). In the parecoxib group the plasma levels of adrenocorticotropie hormone at T2 - T5 decreased significantly as compared with the control group ( P <0. 01 ). Postoperation VAS scores:VAS scores of the parecoxib group were less than that of the control group (P < 0. 05). Conclusion Preoperatively-administered pareeoxib during ophthalmology surgery can produce better analgesia effect, reduce the production of cytokines, decrease central nervous system sensitization so as to improve the quality of postoperative pain relief. At the same time, it can reduce perioperative stress hormone release so as to have a positive effect on post operative recovery.
Prospective investigation of intravenous patient-controlled analgesia with hydromorphone or sufentanil: impact on mood, opioid adverse effects, and recovery

Abstract:Background Radical surgery for colorectal cancer, associated with moderate to severe postoperative pain, needs multimodal analgesia with opioid for analgesia. Despite considerable advancements, the psychological implications and other side effects with opioid remain substantially unresolved. This study aimed to investigate the impact on mood, side effects relative to opioid, and recovery of the patients with hydromorphone or sufentanil intravenous patient-controlled analgesia (IV-PCA) in a multimodal perioperative analgesia regimen undergoing radical surgery for colorectal cancer. Methods Eighty patients undergoing elective laparoscopic or open radical surgery for colorectal cancer under general anesthesia were randomized to receive postoperative IV-PCA with either sufentanil (group S) or hydromorphone (group H). All patients received additionally flurbiprofen axetil 50 mg 30 min before the end of surgery and wound infiltration with 10 ml of 0.75% ropivacaine at the end of surgery. The primary endpoint was mood changes at 48 and 96 h after surgery. The secondary endpoints were the incidence of opioid-related adverse effects, recovery results and patient satisfaction after surgery. Results Seventy-two patients completed the study finally. There were no significant differences between the two groups with respect to preoperative parameters, surgical and anesthetic characteristics ( P > 0.05). No obvious significant differences were observed in VAS score (at rest and during mobilization) and rescue analgesics use ( P > 0.05). Compared with group S, the anger scores in the group H at 48 h and 96 h after surgery were significantly lower ( P = 0.012 and 0.005; respectively), but the incidences of pruritus and nausea were higher ( P = 0.028 and 0.008; respectively). There were no significant differences in the incidences of vomiting, respiratory depression, dizziness, Ramsay score, and hemodynamic changes between the two groups ( P > 0.05). Moreover, there were no significant differences in the time to gastrointestinal recovery, time to drainage tube removal, time to walk, hospital stay after surgery and patient satisfaction between the two groups ( P > 0.05). Conclusions Under the similar analgesia effect with different opoiods postoperatively, hydromorphone IV-PCA resulted in an improved mood, however, a higher occurrence of pruritus and nausea while compared to sufentanil IV-PCA in a multimodal perioperative analgesia regimen. Both regimens of opioid with IV-PCA may serve as promising candidates for good postoperative pain management, and provide with similar postoperative recovery for the patients undergoing radical surgery for colorectal cancer. Effects of Different Analgesia Regimens on Early Post-Operative Cognitive Dysfunction in Elderly Patients Undergoing Radical Resection of Cervical Carcinoma

Abstract:This study was designed to compare the effects of epidural and intravenous analgesia on early post-operative cognitive dysfunction (POCD) in elderly patients undergoing radical resection of cervical cancer. For this purpose, 74 patients aged 60-78 years [body mass index (BMI), 18-25 kg/m2; American Society of Anesthesiologists (ASA) classification score of I-III) undergoing radical resection of cervical cancer were divided into the epidural group (group E) and parenteral group (group P) groups (37 patients in each group). All patients underwent their surgical procedures under epidural anesthesia and intravenously-delivered general anesthesia. Patient-controlled analgesia (PCA) was supplied for 72 h after the surgery. Epidural analgesia was provided for the patients in group E and intravenous analgesia was provide for those in group P. General patient information was recorded and peripheral blood neutrophil counts, C-reactive protein (CRP) levels and interleukin (IL)-6 concentrations were measured immediately prior to the surgery (T0), and at 24, 48 and 72 h after the procedure (T1, T2 and T3, respectively). Visual analog scale (VAS) scores were also recorded at T1, T2 and T3, and the mini-mental state evaluation (MMSE) scores at T0, T1, T2, and T3 were calculated. Patients were diagnosed as having POCD according to their MMSE score differences between the peri-operative and post-operative values. The results revealed that the levels of CRP and IL-6 significantly increased in both groups after the surgery (T1-3). However, the CRP and IL-6 levels in group E were significantly lower than those in group P at all time points examined (P<0.05). The VAS scores in group E at T1, T2 and T3 were significantly lower than those in group P (P<0.05). Finally, the incidence of POCD in group E was significantly lower than that in group P (P<0.05). On the whole, the post-operative epidural analgesia reduced the systemic inflammatory response, the perceived pain, and the incidence of POCD in patients undergoing radical resection of cervical cancer, when compared with the effects of intravenous analgesia.

Abstract:Objective To investigate the effects of flurbiprofen axetil analgesia on inflammatory factors and postoperative analgesia in patients undergoing laparoscopic radical rectectomy for rectal cancer.Methods Eighty patients undergoing laparoscopic radical rectectomy for rectal cancer were randomly divided into 2 groups (n =40 in each),flurbiprofen axetil group received intravenous flurbiprofen axetil 50mg/5 ml 30 minutes before anesthesia induction,and control group received normal saline 5 ml instead.Venous blood samples were obtained at 10 minutes before flurbiprofen axetil or normal saline was injected,mimediately when surgery was finished,12 hours,and 24 hour safter the surgery w as finished,to determine the plasma levels of substance P and IL-6.At the same time,visual analogue scale(VAS) scores were recorded at 2,6,12 and 24 hours after the surgery.Results Compared to the control group,the plasma levels of substance P decreased in the flurbiprofen axetil group at immediately when surgery was finished and 12 hours [mimediately when surgery was finished:(105 ± 13)ng/L vs (155 ±24) ng/L,12 hours:(111 ± 12) ng/L vs(155 ± 31) ng/L,10 minutes:(92 ± 10) ng/L vs(90 ± 8) ng/L,P ＜0.05].The plasma levels of IL-6 decreased in the flurbiprofen axetil group at immediately when surgery was finished to 24 hours [mimediately when surgery was finished:(32 ± 8) ng/L vs (73 ± 15) ng/L,12 hours:(26 ±8) ng/L vs (85 ± 17) ng/L,24 hours:(20±8) ng/L vs (73 ±8)ng/L,10 minutes:(15 ±6)ng/L vs (15 ±5)ng/L,P ＜0.05].VAS status had significant difference at 2,6 and 12 hours postoperatively in the flurbiprofen axetil group compared to the control group [(2.1 ± 0.6) scores vs (5.6 ± 0.5) scores,(2.4 ± 0.5) scores vs (2.2 ±0.4) scores,(1.9 ± 0.9) scores vs (4.7 ± 0.5) scores,P ＜ 0.05].Conclusion flurbiprofen axetil can reduce the production of in flammatory factors and produce better analgesia effect.
[Effects of Preemptive Analgesia with Flurbiprofen on the Blood Sugar and Interleukin-6 of Patients after Radical Excision of Breast Cancer].

Abstract:OBJECTIVE To investigate the effects of preemptive analgesia with flurbiprofen on the blood sugar and Interleukin-6 of patients after radical excision of breast cancer. METHODS A total of 60 ASA I-II patients scheduled for radical excision of breast cancer were randomly assigned to three groups: group A, B and C (n = 20 each), patients of group A and C received intravenous flurbiprofen 100 mg before or at the end of surgery respectively. Blood samples were collected from the patients before anaesthesia induction and 1, 6, 24 h after surgery for the determination of blood sugar and serum interleukin-6 concentration. Analgesic efficacy was assessed after surgery based on visual analog scales (VAS). RESULTS The blood sugar and serum interleukin-6 concentration of the patients in three groups at different time points after surgery were significantly higher than those before surgery, and increased gradually in group B, and there were very significant difference between the time point of 1 h and 24 h after surgery (P < 0.01), but there were no increasing trend for those of group A and C. The blood sugar and serum interleukin-6 concentration of the patient of group A were significantly lower than those of group B and C (P < 0.01 or 0.05). The highest VAS of group A and C at different time points after surgery were significantly lower than that of group B(P < 0.05). CONCLUSION Preemptive analgesia with flurbiprofen 100 mg can effectively suppress the elevation of blood sugar and serum interleukin-6 concentration after radical excision of breast cancer, and is better than postoperation analgesia.
Administration of flurbiprofen axetil and dezocine for the postoperative analgesia in patients with non‐small cell lung cancer: A randomized, controlled study

Abstract:Flurbiprofen axetil or dezocine monotherapy has been applied for analgesia of postoperative non‐small cell lung cancer (NSCLC); however, their combination is rarely investigated. Consequently, the present study aimed to explore the effect of flurbiprofen axetil plus dezocine on postoperative pain, surgical outcomes and its safety profile in patients with NSCLC. A total of 150 patients with resectable NSCLC were enrolled and randomized into three groups: i) The flurbiprofen axetil plus dezocine group (n=50), ii) the flurbiprofen axetil group (n=51) and iii) the dezocine group (n=49). A total of 50 mg flurbiprofen axetil, 5 mg of dezocine or their combination were administered intravenously 3 h prior to surgery and subsequently every 12 h until day 3 (D3) following surgery. The postoperative pain was lower in the flurbiprofen axetil plus dezocine group compared with that of the flurbiprofen axetil group at 6 h (P=0.008), 12 h (P=0.003), day 1 (D1) (P=0.013), day 2 (D2) (P=0.036) and D3 (P=0.010); in addition, it was lower in the flurbiprofen axetil plus dezocine group compared with that of the dezocine group at 6 h (P=0.010), 12 h (P=0.012) and D1 (P=0.020). Patient‐controlled analgesia consumption was also lower in the flurbiprofen axetil plus dezocine group compared with that of the flurbiprofen axetil (P=0.010) and dezocine (P=0.002) groups. Furthermore, the length of hospital stay was lower in the flurbiprofen axetil plus dezocine group compared with that of the flurbiprofen axetil (P=0.008) and dezocine (P=0.048) groups, while other surgical outcomes and adverse events were similar among these three groups. Moreover, the expression of tumor necrosis factor‐α was lower in the flurbiprofen axetil plus dezocine group compared with that of the dezocine group at 12 h (P<0.001), D1 (P<0.001) and D3 (P=0.033). The data indicated that flurbiprofen axetil and dezocine combination was superior to monotherapy for postoperative analgesia in patients with resectable NSCLC.

Efficacy of oxycodone versus sufentanil for intravenous analgesia after radical resection of pulmonary carcinoma performed via video-assisted thoracoscope

Abstract:Objective To compare the efficacy of oxycodone versus sufentanil for intravenous analgesia after radical resection of pulmonary carcinoma performed via video-assisted thoracoscope.Methods One hundred fifty-four patients of both sexes, aged 18-64 yr, with body mass index of 18-25 kg/m2, of American Society of Anesthesiologists physical status Ⅰ or Ⅱ , scheduled for elective radical resection of pulmonary carcinoma performed via video-assisted thoracoscope, were randomly divided into either sufentanil group (group S, n=76) or oxycodone group (group O, n =78) using a random number table.The 2 groups received combined intravenous-inhalational anesthesia.When postoperative visual analogue scale (VAS) score ≥ 4, sufentanil 5 μg or oxycodone 2 mg was injected intravenously, and the administration was repeated when necessary until VAS score ≤ 3.Patient-controlled intravenous analgesia (PCIA) was then used for postoperative analgesia (lasting for 48 h).PCIA solution contained tropisetron 20 mg and sufentanil 200 μg in 100 ml of normal saline in group S.PCIA solution contained tropisetron 20 mg and oxycodone 50 mg in 100 ml of normal saline in group O.The PCIA pump was set up to deliver a 2 ml bolus dose with a 10 min lockout interval and background infusion at a rate of 1 ml/h.VAS score was maintained ≤3.When VAS scores ≥4, morphine 10 mg injected intramuscularly was used as rescue analgesic.The requirement for rescue analgesic, level of patient's satisfaction with analgesia,and analgesia-related adverse events were recorded.Results The incidence of nausea and vomiting was significantly lower in group O than in group S (P＜0.05).There was no significant difference in the requirement for rescue analgesic, level of patient's satisfaction, and incidence of dizziness and over-sedation between the two groups (P＞0.05).No patients developed respiratory depression and pruritus in the two groups.Conclusion Compared with sufentanil, oxycodone can produce similar analgesic efficacy when used for PCIA after radical resection of pulmonary carcinoma performed via video-assisted thoracoscope, with lower incidence of nausea and vomiting.

Abstract:Objective To investigate the analgesic effect and impact on the inflammatory cytokines response of the lipid microspheres flurbiprofen axetil injection in patients undergoing gastroenteral cancer transection surgery.Methods Sixty patients scheduled to take gastroenteral cancer transection surgery were randomly allocated into three groups, morphine group (M,n=20), postoperative flurbiprofen axetil group(F,n=20)and preemptive flurbiprofen axetil group(P,n=20).In group M the patients received patient controlled intravenous analgesia (PCIA) with morphine (loading dose 0.03 mg/kg, bolus 1 mg,lockout time 15 min,background dose 0 mg) after surgery. As for group F,50 mg flurbiprofen axetil were administered at the end of the surgery, then the same morphine PCIA scheme as group M was used in combination with the intermitted intravenous injection of flurbiprofen axetil per 50 mg at 12 h, 24 h, 36 h after surgery. The analgesic paradigm of group P was quite similar with group F, whereas the first injection of flurbiprofen axetil was administered 30 min before operation instead of the end of surgery. The analgesic effect assessed by visual analog scale (VAS) at rest at 6, 12, 24, 36, and 48 h after operation and the total consumed dosage of morphine were recorded. Furthermore, the venous blood was drawn before the induction of anesthesia, 6, 12, and 24 h after the surgery to determine the level of interleukin 6 (IL-6) and interleukin 10 (IL-10). Results At 6 h after surgery, the VAS of group P was significantly lower than group M. At 12 h post operation, the VAS of the group P and group F was markedly lower than group M. Moreover, the total consumed dosage of morphine was significantly less in the group P and group F compared with the group M. The basic level of IL-6 and IL-10 was too low to be measured. The concentration of IL-10, IL-6 reached peak levels at 6 h, 12 h after surgery respectively, and the level of IL-10 of the group M was significantly lower than the group F and group P at 6 h. In contrast, IL-6 of the group M was significantly higher than the group F and group P at 12 h, and still higher than the group P at 24 h.Conclusion Preemptive administration of the lipid microspheres flurbiprofen axetil might better control fierce pain after gastroenteral cancer transection surgery. Furthermore it could attenuate the inflammatory cytokines response through the inhibition of the expression of IL-6 and enhancement the production of IL-10.
EFFICACY OF OXYCODONE INJECTION COMBINED WITH PARECOXIB SODIUM THERAPY FOR POSTOPERATIVE PAIN AFTER REMIFENTANIL COMBINED WITH ANESTHESIA

Abstract:Objective:To observe the curative effect and adverse reactions of oxycodone hydrochloride injection combined with parecoxib sodium in the treatment of hyperalgesia after remifentanil anesthesia. Methods:Sixty cases of patients with lower abdominal surgery were randomly divided into three groups (20 cases each group): low dose of oxycodone group (Q1), high dose of oxycodone group (Q2) and oxycodone combined with parecoxib sodium group (Q+P). Thirty minutes before the end of surgery, patients in three group received intravenous oxycodone 0.10 mg/kg, oxycodone 0.15 mg/kg and oxycodone 0.10 mg/kg+parecoxib sodium 40 mg, respectively. The awakening time,extubation time,postoperative pain visual analogue scale (VAS),Alertness/sedation score (OAA/S) , agitation emergence (EA), and adverse drug reactions (respiratory depression, nausea and vomiting) in all patients were observed and recorded.Results:There was no statistical difference of the general information among the three groups. Awakening time and extubation time in group Q1 and group Q+P were shorter than those in group Q2, and the difference was statistically significant (P< 0.05). The OAA/S scores of group Q1 and group Q+P were higher than that of group Q2, while their incidence of drowsiness were lower than that of group Q2 (P< 0.05). Group Q2 and group Q+P were superior to group Q1 in terms of pain and agitation score (P< 0.05).Conclusion:The oxycodone injection combined with parecoxib sodium at 30 min before the end of surgery could effectively prevent remifentanil induced hyperalgesia and induce less adverse reactions.

Abstract:Background We aimed to compare the anesthesia induction effects of oxycodone and sufentanil on postoperative pain in patients undergoing laparoscopic gallbladder-preserving cholecystolithotomy, as well as changes in serum levels of inflammatory factors (TNF-α, IL-6, and IL-10) in the perioperative period. Methods Sixty patients who underwent laparoscopic gallbladder-preserving cholecystolithotomy were evenly divided into oxycodone (O) and sufentanil (S) groups. In groups O and S, oxycodone (0.3 mg/kg) and sufentanil (0.3 ug/kg) were administered, respectively, followed by propofol (2 mg/kg) and rocuronium (0.6 mg/kg). In both groups, the intraoperative electroencephalography double-frequency index was used to guide the use of sedative and analgesic drugs, assessing the follow-up analgesic effect (VAS), degree of sedation (Ramsey), and postoperative complications at seven different time points (0, 0.5, 2, 4, 6, 8, and 24 h postoperatively). Results Compared with the S group, patients in the O group exhibited lower VAS scores within 24 h postoperatively ( P < 0.001), but there was no statistical difference between wound and shoulder pain scores ( P > 0.05). Regarding postoperative awakening and extubation duration, O group patients experienced shorter times and better remedial analgesia ( P < 0.05). In terms of the degree of sedation, the Ramsay score decreased at 0 h postoperatively compared with the S group ( P < 0.001). Conclusion Compared with sufentanil, oxycodone anesthesia induced better postoperative analgesia and less inflammatory responses in patients undergoing laparoscopic gallbladder-preserving cholecystolithotomy. Abstract:BACKGROUND According to clinical data, a significant percentage of patients experience pain after surgery, highlighting the importance of alleviating postoperative pain. The current approach involves intravenous self-control analgesia, often utilizing opioid analgesics such as morphine, sufentanil, and fentanyl. Surgery for colorectal cancer typically involves general anesthesia. Therefore, optimizing anesthetic management and postoperative analgesic programs can effectively reduce perioperative stress and enhance postoperative recovery. The study aims to analyze the impact of different anesthesia modalities with multimodal analgesia on patients' postoperative pain. AIM To explore the effects of different anesthesia methods coupled with multi-mode analgesia on postoperative pain in patients with colorectal cancer. METHODS Following the inclusion criteria and exclusion criteria, a total of 126 patients with colorectal cancer admitted to our hospital from January 2020 to December 2022 were included, of which 63 received general anesthesia coupled with multi-mode labor pain and were set as the control group, and 63 received general anesthesia associated with epidural anesthesia coupled with multi-mode labor pain and were set as the research group. After data collection, the effects of postoperative analgesia, sedation, and recovery were compared. RESULTS Compared to the control group, the research group had shorter recovery times for orientation, extubation, eye-opening, and spontaneous respiration (P < 0.05). The research group also showed lower Visual analog scale scores at 24 h and 48 h, higher Ramany scores at 6 h and 12 h, and improved cognitive function at 24 h, 48 h, and 72 h (P < 0.05). Additionally, interleukin-6 and interleukin-10 levels were significantly reduced at various time points in the research group compared to the control group (P < 0.05). Levels of CD3+, CD4+, and CD4+/CD8+ were also lower in the research group at multiple time points (P < 0.05). CONCLUSION For patients with colorectal cancer, general anesthesia coupled with epidural anesthesia and multi-mode analgesia can achieve better postoperative analgesia and sedation effects, promote postoperative rehabilitation of patients, improve inflammatory stress and immune status, and have higher safety.

Abstract:Introduction Postoperative pain is a main component influencing the recovery of patients with lung cancer. The combination of patient-controlled intravenous analgesia (PCIA) and paravertebral nerve block for postoperative analgesia in patients undergoing thoracoscopic lobectomy for lung cancer can achieve a satisfactory analgesic effect and promote early rehabilitation of patients. The objective is to investigate the optimal dose of oxycodone for PCIA combined with paravertebral nerve block, to achieve effective multimodal analgesia management in patients undergoing thoracoscopic lung cancer lobectomy. Methods and analysis This prospective, double-blind, single-centre, parallel-group, superiority study from 7 April 2023 to 31 December 2024 will include 160 participants scheduled for thoracoscopic lobectomy for lung cancer. Participants will be randomly assigned to four groups in a 1:1:1:1 ratio: OCA group (oxycodone: 0.5 mg/kg), OCB group (oxycodone: 1.0 mg/kg), OCC group (oxycodone: 1.5 mg/kg) and one sufentanil group (sufentanil: 2 μg/kg). Flurbiprofen 50 mg and ondansetron 16 mg are added to each group. All the drugs are diluted with 0.9% saline in a 100 mL volume, with a background infusion rate of 2 mL/hour, a bolus dose of 0.5 mL and a lockout interval of 15 min. The primary outcome is pain scores at rest and dynamic at 24 hours after surgery using a Numeric Rating Scale (NRS). Dynamic NRS scores are defined as NRS when coughing. NRS scores will be assessed at 2, 4, 12, 24 and 48 hours postoperatively. The secondary outcomes include the following variables: (1) NRS score at rest and dynamic at 2, 4, 12 and 48 hours postoperatively; (2) total dose of sufentanil or oxycodone consumption in PCIA; (3) the times of patient-controlled analgesia; (4) Ramsay Sedation Score (RSS) at 2, 4, 12, 24 and 48 hours after the surgery; (5) extubation time; (6) serum C-reactive protein and interleukin six levels; (7) incidence of postoperative nausea and vomiting; (8) incidence of itching; (9) incidence of respiratory depression and (10) gastrointestinal recovery (exhaust time). Ethics and dissemination The First Affiliated Hospital of Shandong First Medical University's Ethics Committee granted consent for this study (approval number: YXLL-KY-2022(116)). To enable widespread use of the data gathered, we plan to publish the trial's findings in an appropriate scientific journal after it is complete. Trial registration number NCT05742256.

Abstract:To observe the efficacy of intravenously injected oxycodone intraoperative on postoperative urinary catheter-related bladder discomfort (CRBD).Patients with ASA I-III received trans-urethral resection prostate under general anesthesia were observed. Patients were randomized allocated to the group control (n = 45) received placebo and the group oxycodone (n =46 ) received 0.03mg/kg of oxycodone before the end of operative 10min. The incidence and severity (mild, moderate, severe) of CRBD were assessed at 0, 1/2 h, 2 h and 6 h postoperatively. VAS scores were used to assess pain intensity during the same period. Postoperative PCA analgesic sufentanil dose and the incidences of nausea, vomiting, dizziness, over sedation were recorded in these patients.Compared with the control group, the incidence of CRBD was significantly lower in the oxycodone group at 0 [22 (49 %) vs. 10 (22%); P = .007], 1/2h [18 (40%) vs. 9 (20%); P= .033], 2h [11 (24%) vs. 4 (9%); P = .001]. The severity of CRBD at 0 [mild, 9 (38%) ; moderate 9 (20%), severe 4 (9%)] was lower in the group Q than the controlled group [mild, 4 (38%) P ? .023; moderate 5 (11%), P ?.034, severe 1 (2%), P ? .012]. 1/2 h [mild, 11 (24%) Vs 5(11%), P ? .020]. Compared with the group C, VAS scores were lower in group Q at 0, 1/2h (P ? .001) and significantly decreased sufentanil dosage within 6h ( P= .001). There were no significant differences in the incidence of postoperative adverse effects between two groups.Oxycodone can effectively prevent patients with CRBD after TURP without incurring serious adverse effects.

Abstract:Objective To observe the post-laparoscopic analgesic effect of oxycodone combined with dexmedetomidine for gastric cancer patients.Methods A total of 40 gastric cancer patients who underwent laparoscopy during November 2016 and January 2017 and assessed with physical status Ⅰ or Ⅱ according to American Society of Anesthesiologists,were randomly divided into 2 groups:oxycodone group (group O,n =20) and oxycodone plus dexmedetomidine group (group OD,n =20).The anestheasic effects at the time points of hours 2,5,8,11 and 24 after the operation were assessed by using visual analog pain score,times for pressing the pump,Ramsay score and patient's satisfaction index of the two groups.At the same time,the adverse effects of two groups were recorded and observed.Results The analgesic effect after starting the pump at hours 2,5,8,11,24 and 25,the VAS scores and the pressure time in group OD were significantly lower than in group O (P ＜ 0.05),and the Ramsay scores were insignificantly different between the groups (P ＞ 0.05).In group OD,the patient's satisfaction level was significantly higher than in group O (P ＜ 0.05).Nausea,vomiting and dizziness occurred in both groups without significantly difference between the two groups.Conclusion Oxycodone combined with dexmedetomidine can provide satisfactory analgesia for gastric cancer patients receiving laparoscopic surgery.

Abstract:More than 30% of patients who undergo surgery will experience postoperative nausea and vomiting (PONV) if no prophylactic treatments are used. Although numerous studies have been performed to investigate the factors related to PONV, the effect of perioperative intravenous oxycodone on the incidence of PONV has not been well investigated. In this study, gynecological patients (grade I-II, aged 18-65 years, scheduled to undergo elective gynecological laparoscopic surgery under general anesthesia) were randomized to the oxycodone group or the sufentanil group. In the oxycodone group, patients received intravenous oxycodone for the induction and maintenance of anesthesia, as well as postoperative analgesia, while sufentanil was used in the sufentanil group. The primary outcome variable was the incidence of PONV. As secondary outcomes, time to first PONV, the rating of the most severe PONV, postoperative pain scores at different time points, hemodynamics, and side effects were evaluated. We found that, compared with sufentanil, oxycodone decreased the incidence of PONV by 13.5% (P = .041). The time to first vomiting was longer in the oxycodone group than in the sufentanil group. Postoperative pain scores at different time points and hemodynamics were comparable between the oxycodone and sufentanil groups. We concluded that the incidence of PONV in gynecological patients who underwent laparoscopic surgery was lower when using intravenous oxycodone for anesthesia induction, anesthesia maintenance, and postoperative analgesia than when using intravenous sufentanil. However, oxycodone and sufentanil provided the same stable hemodynamics during surgery and satisfactory postoperative analgesia.

STUDY OF FLURBIPROFEN AXETIL ON PAIN RELIEF AND INTESTINAL SAFETY AFTER COLON SURGERY IN RATS

Abstract:Objectives:To investigate the influence of flurbiprofen axetil on bowel function, pain behaviorand anastomotic healing after colon surgery. Methods: Forty-eight male SD rats (6-8 weeks) were randomlydivided into 4 groups (n =12), and all rats underwent colon resection and anastomosis. They received intralipid (Zgroup, 0.4 ml/d), low dose of flurbiprofen axetil (F1 group, 12.5 mg/kg/d), moderate dose of flurbiprofen axetil(F2 group, 25 mg/kg/d), and high dose of flurbiprofen axetil (F3 group, 37.5 mg/kg/d), respectively, after surgeryuntil postoperative day 3 (POD 3). Feces was observed at 12, 24, 36, 48 and 60 h after surgery. Pain behavior wasmeasured from postoperative day 0 to day 3. Six rats in each group were sacrificed on postoperative day 3 and day7, respectively. Bursting pressure, COX-2 expression (immunohistochemistry) and hydroxyproline levels (alkalinehydrolysis) of the perianastomotic tissues were measured. Results: No significant difference in postoperativerecovery of defecation were observed among groups. F1, F2, F3 groups showed a lower response frequencyto stimulation of von Frey filament (10 g) (P < 0.01). Compared with Z group, the bursting pressure, COX-2expression and hydroxyproline levels were lower in F3 group both on POD 3 and POD 7 (P < 0.05); the burstingpressure on POD 7 and COX-2 expression on POD 3 were lower in F2 (P < 0.05). Conclusion: Flurbiprofenaxetil relieves postoperative pain after colon surgery with no remarkable influence on bowel function. High doseflurbiprofen axetil may increase the risk of anastomotic leakage after colon surgery.

Abstract:To investigate the efficacy and safety of flurbiprofen axetil in postoperative analgesia in upper abdominal surgery. This was a multicenter, randomized, positive drug parallel controlled double-blind clinical study. Patients undergoing upper abdominal surgery were randomly divided to receive flurbiprofen axetil or tramadol. The VAS pain scores at rest and on coughing (pulmonary function training) were assessed immediately before drug usage (T1) to evaluate the efficacy of postoperative analgesia. Repeat assessment of the VAS was performed after T1. The timing of the recovery of the gastrointestinal function and the preoperative and postoperative IL-6, cortisol, and blood glucose levels were recorded as secondary endpoints. Vital signs and the occurrence of adverse reactions were evaluated for the assessment of safety. A total of 240 patients were enrolled in the current study; 119 used flurbiprofen axetil for postoperative analgesia. The VAS scores at rest and on coughing did not differ between the two groups to a statistically significant extent (P > 0.05). However, the reduction of the VAS score at rest in the flurbiprofen axetil group was greater than that in the tramadol group at 4–24 h after T1. The reduction of the VAS score on coughing at 8 h after T1 was greater in the flurbiprofen axetil group. The incidence of adverse reactions was significantly lower in the flurbiprofen axetil group, with only one adverse reaction recorded. In contrast, 18 adverse reactions were reported in the tramadol group. Flurbiprofen axetil showed superior efficacy to tramadol in early postoperative analgesia after upper abdominal surgery. Flurbiprofen axetil was associated with a significantly lower incidence of adverse reactions in comparison to tramadol.
