Abstract:AbstractBackgroundAlzheimer’s disease (AD) is a progressive and debilitating neurological disorder that impacts millions of individuals globally. Prompt and accurate detection and diagnosis of Alzheimer’s disease are pivotal for providing effective care and management to patients. However, early identification of the disease remains challenging due to the complexity of diagnostic procedures and the similarity of symptoms with age‐related cognitive decline. With the world transitioning towards a more digital‐focused environment, there is an increasing potential to utilize human‐computer interaction technologies to aid in the early detection of Alzheimer’s disease.MethodWe recruited 40 healthy individuals, 40 patients with mild cognitive impairment (MCI), and 40 patients with early AD. By incorporating fusion perception‐enhancing technologies into intelligent identification devices, such as eye‐tracking and body motion detection, peripheral modal biometric information can be gathered from subjects.ResultThe study found that in human‐computer interaction experiments using eye‐tracking, the patients with early AD showed significantly longer completion times and free rates of eye motion and movement trajectory than the normal group and those with MCI. The correlation between eye motion trajectory and movement trajectory was notably lower as compared to the normal group and the MCI group. During the human‐computer interaction physical movement experiment, the AD group recorded lower target accuracy rate and screen click rate as compared to the normal group and patients with MCI. Similarly, in the human‐computer interaction clock drawing experiment, the AD group demonstrated longer completion time and pen‐up thinking time, as well as lower clock drawing scores. Furthermore, the total completion time of human‐computer interaction tasks was notably shorter as compared to conventional cognitive screening tests. In conclusion, peripheral biomarkers obtained from the aforementioned human‐computer interaction task were analyzed using machine learning techniques, resulting in the identification of early‐stage Alzheimer’s disease with a classification accuracy of 91.1%.ConclusionHence, this non‐invasive and accurate technique may prove instrumental in identifying potential cognitive deficits associated with this disease while also being widely applicable for screening early AD.
The Changes of Central Auditory Processing Function and Its Electroencephalogram in Patients with Mild Cognitive Impairment and Early Alzheimer’s Disease

Abstract:Background: The aim of this study is to assess the central auditory processing (CAP) function and its electroencephalogram (EEG) in patients with mild cognitive impairment (MCI) and the early stage of Alzheimer's disease (AD).Methods: In this study, 25 patients with early AD, 22 patients with MCI, and 22 matched healthy controls (HC) were included. After cognitive assessment, binaural processing function was assessed using the staggered spondaic word (SSW) test, and auditory working memory was assessed by auditory n-back paradigm, while EEG was recorded. Patients' behavioral indicators, event-related potentials (ERPs) components, and function connection (FC) were compared be-tween groups and the related factors were analyzed. Results: The difference of the accuracy of behavioral tests for the three groups of subjects was significant and all the behavioral indicators were positively correlated with cognitive function scores. Intergroup differences in amplitude (p < 0.05) and latency (p < 0.01) were significant for P3 in the 1-back paradigm. In the SSW test, AD and MCI patients showed reduced connectivity between the left frontal lobe and the whole brain in the 8-band, while in the n-back paradigm, patients with MCI and early AD showed reduced association of frontal leads with central and parietal leads in the 8-band.Conclusions: Patients with MCI and early AD have reduced CAP functions including binaural processing function and auditory working memory functions. This reduction is significantly associated with reduced cognitive function, and is reflected in different patterns of changes in ERP as well as functional connectivity in the brain.

Abstract:Background The early identification of Alzheimer's disease (AD) benefits patients, so creating a simple and convenient method is crucial for diagnosing early symptoms. Objective To offer a potential approach for the early detection of both AD and mild cognitive impairment (MCI). Methods Eye movement data from 66 patients were divided into three groups, including healthy control group (HC), MCI group, and AD group. We searched for parameters that can detect MCI at an early stage and drew receiver operating characteristic (ROC) curves. The correlation between eye movement parameters and cognitive scores was analyzed. Results The MCI group differed from the HC group in error correction rate of antisaccade ( p = 0.008) and total offset degrees (>4°) ( p = 0.011) of lateral fixation. The AD group had different overlap prosaccade accuracy ( p = 0.025), latency ( p = 0.009) and average completion time ( p = 0.015), gap prosaccade latency ( p = 0.005) and average completion time ( p = 0.005), antisaccade accuracy ( p = 0.006), error correction rate ( p 4°) ( p = 0.041) compared to the HC group. The AD group differed significantly from the MCI group in accuracy ( p = 0.001) and error correction rate ( p = 0.044) of antisaccades, the latency ( p = 0.009) and average completion time ( p = 0.025) of overlap prosaccade and the latency ( p = 0.038) of gap prosaccade, these parameters can serve as indicators to monitor the progress of the disease. Lateral fixation combined with antisaccade was more conducive to identifying MCI patients with the area under the ROC curve of 0.837. Most eye movement parameters had a light to moderate correlation with cognitive scores. Conclusions Eye movements can be used for early identification of MCI/AD patients and to monitor disease progression.

Abstract:Background: Patients with Alzheimer's disease (AD) are known to exhibit visuospatial processing impairment, as reflected in eye movements from the early stages of the disease. We investigated whether the pattern of gaze exploration during visual tasks could be useful for detecting cognitive decline at the earliest stage. Methods: Sixteen AD patients (age: 79.1 ± 7.9 years, Mini Mental State Examination [MMSE] score: 17.7 ± 5.3, mean ± standard deviation) and 16 control subjects (age: 79.4 ± 4.6, MMSE score: 26.9 ± 2.4) participated. In the visual memory task, subjects memorized presented line drawings for later recall. In the visual search tasks, they searched for a target Landolt ring of specific orientation (serial search task) or color (pop-out task) embedded among arrays of distractors. Using video-oculography, saccade parameters, patterns of gaze exploration, and pupil size change during task performance were recorded and compared between AD and control subjects. Results: In the visual memory task, the number of informative regions of interest (ROIs) fixated was significantly reduced in AD patients compared to control subjects. In the visual search task, AD patients took a significantly longer time and more saccades to detect the target in the serial but not in pop-out search. In both tasks, there was no significant difference in the saccade frequency and amplitude between groups. On-task pupil modulation during the serial search task was decreased in AD. The number of ROIs fixated in the visual memory task and search time and saccade numbers in the serial search task differentiated both groups of subjects with high sensitivity, whereas saccade parameters of pupil size modulation were effective in confirming normal cognition from cognitive decline with high specificity. Discussion: Reduced fixation on informative ROIs reflected impaired attentional allocation. Increased search time and saccade numbers in the visual search task indicated inefficient visual processing. Decreased on-task pupil size during visual search suggested decreased pupil modulation with cognitive load in AD patients, reflecting impaired function of the locus coeruleus. When patients perform the combination of these tasks to visualize multiple aspects of visuospatial processing, cognitive decline can be detected at an early stage with high sensitivity and specificity and its progression be evaluated.

Abstract:Introduction: Visual search impairment is a potential cognitive marker for Alzheimer's disease (AD) and amnestic mild cognitive impairment (aMCI). The aim of this study is to compare eye movements during visual tracking in AD and aMCI patients versus healthy controls (HCs). Methods: A prospective cohort study included 32 AD and 37 aMCI patients, and 33 HCs. Each participant was asked to look at the target object in a visual stimulus containing one target and eight distractors, and eye movements were recorded with EyeLink 1000 Plus. Results: AD patients had fewer fixations and shorter target fixation duration than aMCI patients and HCs. Fixation durations were also shorter in aMCI patients compared to HCs. Also, AD patients were more fixated on distractors than HCs. Discussion: Our findings revealed that visual search is impaired in the early stages of AD and even aMCI, highlighting the importance of addressing visual processes in the Alzheimer's continuum. Highlights: AD patients looked to distractors more and longer than the target compared to aMCI patients and older healthy individuals. aMCI patients had an impaired visual search pattern compared to healthy controls, just like patients with AD. The visual search task differentiated AD and aMCI patients from healthy individuals without dementia.

Abstract:Abstract Introduction Sleep disturbances have been shown to relate to the development of Alzheimer’s disease (AD) and its associated cognitive deficits, including visual attention function. Here, we investigated the potential interactive and additive effects of sleep disruption and AD on visual attention performance and its underlying neural mechanisms. Methods Preliminary analysis included 33 participants, 10 patients on the AD spectrum (mild cognitive impairment (MCI) and AD; 7 females; Mage: 70.24) and 23 cognitively-healthy older adults (17 females; Mage: 67.47). Participants completed a visual search task during magnetoencephalography (MEG). Participants also wore an Actigraph device on their non-dominant wrist for a two-week period. Actigraph data were cleaned using sleep diaries and analyzed using the Cole-Kripke algorithm. Total sleep time (TST) was calculated for each night of sleep and averaged across all nights of sleep collected. The MEG data were transformed into the time-frequency domain and significant task-related oscillatory responses were source-imaged using a beamformer. Results Behavioral analysis of the visual search task revealed a significant condition effect, such that reaction times (RT) in the conjunctive trials were significantly longer than those in the feature condition (p < .001). There was a trending group-by-condition interaction on RT (p = .080), such that patients on the AD spectrum showed larger differences in reaction times between conditions than healthy controls. Additionally, there was a significant group by TST interaction on RT (p < .001) showing that patients on the AD spectrum with less sleep had longer RT, while there was no such relationship in the controls. Regarding the MEG data, we observed robust neural responses in theta, alpha, and beta frequency bands. Analysis of source-imaged neural responses in the alpha range revealed significant conditional differences in the right posterior temporal lobe (p < .005), a significant group-by-condition interaction in the right temporoparietal junction (TPJ; p < .005), and interestingly, a group-by-TST interaction on conditional differences in alpha power (p < .005) in right occipital and cerebellar regions. Conclusion Our preliminary results show key relationships between total sleep time, visual attention behavioral performance, and neural recruitment, and how these relationships may differ in AD spectrum conditions. Abstract:Mild cognitive impairment (MCI) is a cognitive disorder characterized by memory impairment, wherein patients have an increased likelihood of developing Alzheimer’s disease (AD). The classification of MCI and different AD stages is therefore fundamental for understanding and treating the disease. This study aimed to comprehensively investigate the hemodynamic response patterns among various subject groups. Functional near-infrared spectroscopy (fNIRS) was employed to measure signals from the frontal and bilateral parietal cortices of healthy controls (n = 8), patients with MCI (n = 9), mild (n = 6), and moderate/severe AD (n = 7) during a digit verbal span task (DVST). The concentration changes of oxygenated hemoglobin (HbO) in various subject groups were thoroughly explored and tested. Result revealed that abnormal patterns of hemodynamic response were observed across all subject groups. Greater and steeper reductions in HbO concentration were consistently observed across all regions of interest (ROIs) as disease severity developed from MCI to moderate/severe AD. Furthermore, all the fNIRS-derived indexes were found to be significantly and positively correlated to the clinical scores in all ROIs (R ≥ 0.4, P < 0.05). These findings demonstrate the feasibility of utilizing fNIRS for the early detection of AD, suggesting that fNIRS-based approaches hold great promise for exploring the mechanisms underlying the progression of AD.

Abstract:The clinical observation and assessment of extra-ocular movements is common practice in assessing neurodegenerative disorders but remains observer-dependent. In the present study, we propose an algorithm that can automatically identify saccades, fixation, smooth pursuit, and blinks using a non-invasive eye tracker. Subsequently, response-to-stimuli-derived interpretable features were elicited that objectively and quantitatively assess patient behaviors. The cohort analysis encompasses persons with mild cognitive impairment (MCI), Alzheimer's disease (AD), Parkinson's disease (PD), Parkinson's disease mimics (PDM), and controls (CTRL). Overall, results suggested that the AD/MCI and PD groups had significantly different saccade and pursuit characteristics compared to CTRL when the target moved faster or covered a larger visual angle during smooth pursuit. These two groups also displayed more omitted antisaccades and longer average antisaccade latency than CTRL. When reading a text passage silently, people with AD/MCI had more fixations. During visual exploration, people with PD demonstrated a more variable saccade duration than other groups. In the prosaccade task, the PD group showed a significantly smaller average hypometria gain and accuracy, with the most statistical significance and highest AUC scores of features studied. The minimum saccade gain was a PD-specific feature different from CTRL and PDM. These features, as oculographic biomarkers, can be potentially leveraged in distinguishing different types of NDs, yielding more objective and precise protocols to diagnose and monitor disease progression.

Abstract:Abstract: Alzheimer's disease (AD) has been considered one of the most challenging forms of dementia. The earlier the people are diagnosed with AD, the easier it is for doctors to find a treatment. Based on the previous literature summarizing the research results on the relationship between eye movement and AD before 2013, this paper reviewed 34 original eye movements research papers only closely related to AD published in the past ten years and pointed out that the prosaccade (4 papers) and antisaccade (5 papers) tasks, reading tasks (3 papers), visual search tasks (3 papers) are still the research objects of many researchers, Some researchers have looked at King-Devick tasks (2 papers), reading tasks (3 papers) and special tasks (8 papers), and began to use combinations of different saccade tasks to detect the relationship between eye movement and AD, which had not been done before. These reflect the diversity of eye movement tasks and the complexity and difficulty of the relationship between eye movement and AD. On this basis, the current processing and analysis methods of eye movement datasets are analyzed and discussed in detail, and we note that certain key data that may be especially important for the early diagnosis of AD by using eye movement studies cannot be miss-classified as noise and removed. Finally, we note that the development of methods that can accurately denoise and classify and quickly process massive eye movement data is quite significant for detecting eye movements in early diagnosis of AD.

Abstract:The authors consider the aspect of unconstrained algebraic restoration of a gray-level image, degraded due to defocusing, additive noise, etc., with additional nonnegative constraint on the individual pixel of gray-level image. The negative values in the gray level introduce absurdity in the entire representation. It is demonstrated that under certain conditions the constrained and unconstrained solutions for the restored image are the same. It is shown that using P. Saccadic eye movement variables as biomarkers for cognitive decline in elderly individuals

Abstract:Alzheimer's disease (AD) is the leading cause of Dementia, and mild cognitive impairment (MCI) is often considered a precursor to the development of AD dementia and other types of Dementia. Biomarkers such as amyloid beta are specific and sensitive in identifying AD and can identify individuals who have biological evidence of the disease but have no symptoms, but clinicians and researchers may not easily use them on a large scale. Ocular biomarkers, such as those obtained through eye tracking (ET) technology, have the potential as a diagnostic tool due to their accuracy, affordability, and ease of use. In this study, we show that eye movement (EM) metrics from an interleaved Pro/Anti-saccade (PS/AS) ET task can differentiate between cognitively normal (CN) and MCI subjects and that the presence of Aβ brain deposits, a biomarker of AD, significantly affects performance on these tasks. Individuals with Aβ deposits (Aβ+) performed worse than those without (Aβ-). Our findings suggest that eye-tracking measurements may be a valuable tool for detecting amyloid brain pathology and monitoring changes in cognitive function in CN and MCI individuals over time.Clinical Relevance- The PS/AS paradigm, which measures saccadic eye movements, can accurately detect subtle cognitive impairments and changes in the brain associated with Alzheimer's disease in CN and MCI individuals. This makes it a valuable tool for identifying individuals at risk for cognitive decline and tracking changes in cognitive function over time.

Abstract:Background: Eye-movement behavior has been used as a reliable tool to identify cognitive and behavioral patterns in individuals with different neuropsychiatric disorders including Alzheimer's disease (AD). Most studies in the field have been dedicated to evaluating eye-movement behavior during cognitive tasks in different protocols using multiple parameters. Objective: We aimed to evaluate the differences of eye-movement behavior in healthy subjects, subjects with mild cognitive impairment (MCI), and those with AD in a simple color task with and without cognitive demand. Methods: 91 subjects: 18 AD, 47 MCI, and 26 healthy controls had their oculomotor parameters assessed during baseline (no cognitive demand involved) and during a simple computational color memory task using an eye-tracker. Results: Baseline showed statistically different and heterogeneous results between normal cognition and MCI groups. Familiarization phase of the task could not discriminate between groups in any of the analyzed parameters. AD subjects made longer fixations and visits on distractors, and more frequent fixations and visits on the target areas than other groups during the response phase. Conclusion: Eye-tracking time-related parameters differentiate AD subjects from other groups under cognitive demand even in a simple color memory task.
Differential eye movement features between Alzheimer's disease patients with and without depressive symptoms

Abstract:Background: Accurately diagnosing depressive symptoms in Alzheimer's disease (AD) patients is often challenging. Eye movement parameters have been demonstrated as biomarkers for assessing cognition and psychological conditions. Aim: To investigate the differences in eye movement between AD patients with and without depressive symptoms. Methods: Eye movement data of 65 AD patients were compared between the depressed AD (D-AD) and non-depressed AD (nD-AD) groups. Logistic regression analysis was employed to identify diagnostic biomarkers and the ROC curve was plotted. The correlation between eye movement and HAMD-17 scores was assessed by partial correlation analysis. Results: The D-AD patients showed longer saccade latency and faster average/peak saccade velocities in the overlap prosaccade test, longer average reaction time and faster average saccade velocity in the gap prosaccade test, longer start-up durations, slower pursuit velocity, more offsets, and larger total offset degrees in the smooth pursuit test, and poorer fixation stability in both the central and lateral fixation tests compared to nD-AD patients. The start-up duration in the smooth pursuit test and the number of offsets in the central fixation test were identified as the diagnostic eye movement parameters for D-AD patients with the area under the ROC curves of 0.8011. Partial correlation analysis revealed that the start-up duration and pursuit velocity in the smooth pursuit test and the total offset degrees in the lateral fixation test were correlated with HAMD-17 scores in D-AD patients. Discussion and conclusions: Eye movement differences may help to differentiate D-AD patients from nD-AD patients in a non-invasive and cost-effective manner.

Subtle changes in specific aspects of executive function like inhibitory control have been found in MCI.AimsWe examined attentional and inhibitory control with the aim to distinguish between amnestic MCI patients and healthy controls.MethodUsing neuropsychological, behavioral, and oculomotor function experiments, we examined executive function in 59 normal control, 49, multiple domain amnestic MCI (a-MCI) subjects, and 21 early stage AD patients using eye tracking and Simon task as measures of attentional control, to determine which saccade and behavioral tasks were sensitive enough to identify a-MCI. Saccades were investigated in gap and overlap pro-saccade and anti-saccade tasks.ResultsScores on the Simon task were inversely correlated with general cognitive status and can distinguish a-MCI from controls with excellent specificity (AUC = 0.65 for reaction time and 0.59 for false responses). More importantly, our results showed that saccadic gains were affected in a-MCI and were the most sensitive measures to distinguish a-MCI from normal participants AST gap task AUC = 0.7, PST gap task AUC = 0.63, AST overlap task (AUC = 0.73). Moreover, these parameters were strongly correlated with neuropsychological measures. Using tests in parallel model, improved sensitivity up to 0.97.ConclusionThe present results enable us to suggest eye tracking along with behavioral data as a possible sensitive tools to detect a-MCI in preclinical stage.

Abstract:ObjectiveTo provide a rigorous comparison between patients with mild cognitive impairment due to Alzheimer’s disease (MCI-AD) and healthy elderly, as well as to assess the value of electroencephalography (EEG) in terms of early diagnosis, we conducted a neutral image recognition memory task involving individuals with positive biomarkers including β amyloid deposition, pathologic tau or neurodegeneration.MethodsThe task involving study and test blocks was designed to evaluate participants’ recognition memory. Electroencephalogram was recorded synchronously to elicit event-related potentials in patients with MCI-AD and healthy control subjects. We further analyzed differences between groups or conditions in terms of behavioral performance, time domain, and time-frequency domain.ResultsThe MCI-AD cohort showed a slower response time to old/new images and had low accuracy regarding behavioral performance. The amplitude of the late positive complex for the old/new effects was significantly suppressed in the MCI-AD cohort when compared with that in the HC cohort. The amplitude of the late old/new effects was correlated with the Auditory Verbal Learning Test recognition score in all participants. The time-frequency domain analysis revealed that correct recognition of old items elicited a decrease in beta power, mainly limited to the HC cohort. Moreover, the combination of behavioral (processing speed and accuracy) and electrophysiological (average amplitude and relative power of delta band) measures contributes to classifying patients with MCI-AD from healthy elderly people.ConclusionChanges of old/new effects, accuracy and response time are sensitive to the impairment of recognition memory in patients with MCI-AD and have moderate value in predicting the incipient stage of AD.

Abstract:Abstract The dynamics of the resting brain exhibit transitions between a small number of discrete networks, each remaining stable for tens to hundreds of milliseconds. These functional microstates are thought to be the building blocks of spontaneous consciousness. The electroencephalogram (EEG) is a useful tool for imaging microstates, and EEG microstate analysis can potentially give insight into altered brain dynamics underpinning cognitive impairment in disorders such as Alzheimer’s disease (AD). Since EEG is non-invasive and relatively inexpensive, EEG microstates have the potential to be useful clinical tools for aiding early diagnosis of AD. In this study, EEG was collected from two independent cohorts of probable AD and cognitively healthy control participants, and a cohort of mild cognitive impairment (MCI) patients with four-year clinical follow-up. The microstate associated with the frontoparietal working-memory/attention network was altered in AD due to parietal inactivation. Using a novel measure of complexity, we found microstate transitioning was slower and less complex in AD. When combined with a spectral EEG measure, microstate complexity could classify AD with sensitivity and specificity > 80%, which was tested on an independent cohort, and could predict progression from MCI to AD in a small preliminary test cohort of 11 participants. EEG microstates therefore have potential to be a non-invasive functional biomarker of AD.

The Antibiotic Susceptibility of Propionibacterium acnes: A 15‐year Bacteriological Study and Retrospective Evaluation

Abstract:Propionibacterium acnes strains were isolated from the comedones of 46 acne patients, and their susceptibilities to penicillin G, ampicillin, erythromycin, clindamycin, tetracycline, doxycycline, minocycline, cephalexin and gentamycin were studied by examining their minimum inhibitory concentrations. Two additional standard strains were included in the study. In the results, differing antibiotic susceptibilities were found, and P. acnes strains were the most sensitive to erythromycin and clindamycin, followed by ampicillin and minocycline. The highest levels of resistance were observed against clindamycin, erythromycin and tetracycline. Non‐resistant strains were detected only against minocycline. A comparison of these results with others obtained by similar studies in our department during the last 15 years showed a progressive tendency toward antibiotic resistance for Propionibacterium acnes, probably related to the systemic administration of antibiotics for the treatment of bacterial infections other than acne.
